The Korean Society of Gastrointestinal Endoscopy (KSGE) has organized an ESD research group and made several plans to standardize pathologic and therapeutic points of view. [source] Clinical features and predictors of survival of AIDS-related non-Hodgkin's lymphoma in a population-based case series in Sydney, AustraliaHIV MEDICINE, Issue 5 2004MC Robotin Objectives To analyse clinical features and predictors of survival for AIDS-related non-Hodgkin's lymphoma (NHL) in the era of highly active antiretroviral therapy (HAART), compared to earlier in the HIV epidemic. Methods All AIDS-NHL cases diagnosed at three inner Sydney hospitals caring for people with AIDS during 1985,2001 were identified through medical record searches. Demographic, clinical, immunological and histopathological information was recorded. Year of NHL diagnosis was grouped into three periods, corresponding to whether monotherapy (1985,1991), dual therapy (1992,1995) or HAART (1996,2001) was the main treatment for HIV infection. Statistical comparisons were made between the pre-HAART and post-HAART eras. Results Three hundred cases of AIDS-NHL were identified. Divergent trends were identified for systemic and primary central nervous system (CNS) NHL. For systemic NHL, the CD4 count at NHL diagnosis increased markedly to 208 cells/,L in the post-HAART era (P=0.014) and there was a trend towards presentation as the first AIDS-defining illness (69%, P=0.053), and as earlier stage NHL disease (42%, P=0.048). Median survival time increased from 4.2 months in 1985,1991 to 19 months in the post-HAART era (P<0.001). In a multivariate model, predictors of poor survival from systemic NHL included: NHL diagnosis after another AIDS-defining illness (P<0.001), stage 4 NHL (P<0.001), presentation at extra lymphatic sites (P=0.001), and nonreceipt of chemotherapy (P=0.002). After adjusting for the factors, those diagnosed in the era of HAART had a significant 56% reduction in rate of death (P<0.001). In contrast, for CNS NHL, clinical features were little changed and survival did not improve in the era of HAART. Conclusions Systemic NHL is presenting earlier in the course of HIV disease, and at a less advanced NHL stage. There has been a marked improvement in survival in the era of HAART even after adjustment for other prognostic variables. In contrast, primary CNS NHL remains a disease which presents late in the course of HIV infection and is associated with a very poor prognosis. [source] Non surgical approach in diabetic macular edema : the future ?ACTA OPHTHALMOLOGICA, Issue 2009C CHIQUET Purpose To present the different non surgical therapeutical options of diabetic macular edema Methods The pathogenesis of diabetic macular edema is multifactorial. Hyperglycemia and poor systemic factor balance are major risk factors. Laser treatemnts and antiagiogenic treatments represent the main non surgical options to treat macular edema. Results Focal macular edema remains the best indication of laser treatment. Laser remains also the standard of care of diffuse macular edema but some edemas remain resistant. Several therapeutic options have been proposed : Steroid intravitreal injection and antiVEGF therapy (either PKC inhibitors, VEGF aptamers or VEGF antibodies) represent the future alternative treatments as well as their potential combination. Conclusion Laser remains the main treatment of diabetic macular edema. However, steroids and antiangiogenic agents either isolated or combined represent the main alternative treatment for non responding diffuse macular edema. [source] Saline Drainage Water, Irrigation Frequency and Crop Species Effects on Some Physical Properties of SoilsJOURNAL OF AGRONOMY AND CROP SCIENCE, Issue 1 2001Y. A. Al-Nabulsi This field study evaluated the effects of water quality, irrigation frequency and crop species on some physical properties of soils. The experiment had a split-split-plot design, with three irrigation water qualities (normal water, drainage water and a 1 : 1 mixture of freshwater and drainage water) as the main treatments, two irrigation frequencies (at 7- and 14-day intervals) as the subtreatments and two crops (barley and alfalfa) as the subsubtreatments. The soil infiltration rate was highest in the barley plot receiving freshwater irrigation at weekly intervals. The lowest soil infiltration rate was found in alfalfa plots receiving saline irrigation water at 14-day intervals. Bulk density and proportions of micropores [pore radius (r) < 1.4 µm] were higher and the proportion of macropores (r > 14.4 µm) was lower in barley than in alfalfa. Saline irrigation caused the greatest decrease in total porosity. The soil infiltration rate was higher with more frequent irrigation, and was highest in alfalfa plots receiving freshwater irrigation. The decrease in soil bulk density and infiltration rate was greater with saline drainage water, irrespective of the crop grown and the irrigation frequency. Die Infiltrationrate mit Frischwasser in wöchentlichen Abständen unter Gerste war hoch. Eine Behandlung mit Salzwasser in 14 tägigen Abständen unter Luzerne zeigte eine geringere Infiltrationsrate des Bodens. Bodendichte und der Anteil der Mikroporen (Poren mit einem Radius von r < 1,4 mm) waren größer und der Anteil der Makroporen (r > 14,4 mm) war unter Gerste geringer. Bewässerung mit Salzwasser verursachte die stärkste Abnahme in der Gesamtporosität. Die Infiltrationsrate des Bodens nahm mit der Häufigkeit der Bewässerung zu und zeigte den höchsten Wert bei Luzerne und einer Frischwasserbewässerung. Die Abnahme in der Bodendichte und der Infiltrationseigenschaften waren bei Salzwasserdrainage unabhängig von der Kulturpflanzenart und der Bewässerungshäufigkeit höher. [source] Primary cell cultures from anaplastic thyroid cancer obtained by fine-needle aspiration used for chemosensitivity testsCLINICAL ENDOCRINOLOGY, Issue 1 2008Alessandro Antonelli Summary Objective, Anaplastic thyroid cancer (ATC) is often inoperable and chemotherapy and radiotherapy are the main treatments. Until now, ,primary ATC cell cultures' (ANA) have been developed from surgical biopsies. We investigated the possibility of obtaining ANA from fine-needle aspiration (FNA-ANA) and testing their sensitivity to chemotherapeutic agents, which could enable treatments to be more effective and avoid unnecessary surgical procedures. Design and patients, The aim of this study was to obtain FNA-ANA from three ATC patients and to evaluate the chemosensitivity of FNA-ANA to chemotherapeutic agents. Measurements and results, FNA-ANA from ATC patients were cultured in RPMI 1640 and propagated in Dulbecco's modified Eagle's medium (DMEM). Chemosensitivity was evaluated by inhibiting the proliferation (analysing the number of viable cells by the cleavage of tetrazolium salts), by increasing the concentration of four different chemotherapeutic agents: bleomycin, cisplatin, gemcitabine and etoposide. The chemotherapeutic agents significantly inhibited (> 50%) FNA-ANA proliferation. Another ANA for each patient was obtained from a surgical biopsy specimen; the results for the chemosensitivity tests were similar to those obtained using FNA-ANA. Conclusions, Our study demonstrates the possibility of obtaining FNA-ANA, and opens the way to the use of FNA-ANA as a means of testing the chemosensitivity to different chemotherapeutic agents (and possibly the radiosensitivity) in each patient, avoiding unnecessary surgical procedures and the administration of inactive chemotherapeutics.
