Spatial Analysis and Surname Analysis: Complementary Tools for Shedding Light on Human Longevity Patterns

Summary Starting from the observation that human longevity patterns show regional variations, we applied Spatial Analysis (using the Geographic Information System) and Surname Analysis to highlight the effect of the population genetic structure on such patterns. The study was carried out in Calabria, a southern Italian region which is characterized by a wide variability of geographic features (high mountains and deep valleys which created geographic isolates in the past). We identified three zones of high longevity: a male and a female longevity zone were located near the town of Cosenza (northern Calabria), while a male longevity zone was located in a mountainous and quite isolated part of the province of Reggio Calabria (southern Calabria). The latter zone was characterized by the lowest Female/Male ratio in nonagenarians observed to date. By applying surname analysis (Fisher's alpha) we found a significant negative correlation between surname abundance and index of longevity, showing that this isolated zone of male longevity presents a high level of inbreeding. On the whole, the results showed the effectiveness of spatial analysis in revealing geographical longevity patterns, and highlighted the importance of the population genetic structure in shaping such patterns. FS07.1 A survey of occupational hand eczema in Denmark

Background:, The need for prevention to reduce the number of occupational hand eczema is high. Occupational hand eczema is the most frequently recognised work-related disease in Denmark. Previous findings have shown that almost half of all cases develop a chronic condition with persistent dermatitis, and the annual cost to society is immense. Aims:, The aim of this study was to survey the trends and development of occupational hand eczema in Denmark and thereby help to ensure future successful prevention of chronic disabling occupational hand eczema. Methods:, 758 patients with recognised occupational hand eczema were included prospectively in the period October 2001- November 2002. Data on diagnoses, disease duration, severity, absence from work and occupation was obtained from The Danish National Board of Industrial Injuries and an additional questionnaire was administered by mail. Results:, 621 patients answered the questionnaire (response rate 82%). Irritant contact dermatitis was the most frequent diagnosis and the female/male ratio was 2:1. High prevalence was found in particularly wet occupations. 19 per cent had sick leave more than 5 weeks per year and the mean disease duration was 4.8 years (median 2.1 years). 68.2% had chronic changes. Conclusion:, The results showed a marked gender difference in the pattern of diagnosis and occupation. The impact of occupational hand eczema is still high with prolonged absence from work and a high percentage of chronic disease. The results of the study give important suggestions for future preventive strategies for health authorities. [source]


Dynamics of a protected black rhino (Diceros bicornis) population: Pilanesberg National Park, South Africa

Achieving maximum productivity in remnant populations of black rhinoceros is crucial to the persistence of this species. It was, therefore, investigated whether the black rhino population of Pilanesberg National Park had become regulated by resource limitation 22 years after introduction in 1979. Inter-calving intervals (which are not restricted to yearly time increments, due to asynchronous reproduction) decreased with an increase in rainfall, while the percentage of male calves born increased with increasing rainfall. The percentage of reproductive cows achieving maternal success increased with increasing density until 0.085 rhinos/km2, after which it decreased. This positive relationship at low densities is largely due to changes in the female age structure and the adult female/male ratio. The age at first calving tended to increase with increasing density, while mortality was not related to rainfall or density. It is concluded that the Pilanesberg black rhino population is showing the first signs of density dependence. It is proposed that black rhino conservators should monitor the percentage of cows achieving maternal success to detect early indications of density dependent resource limitation and use this as a criteria for decisions regarding metapopulation management. Vitreoretinal surgery in Behçet's disease with severe ocular complications

ABSTRACT. Purpose: To investigate effects of vitreoretinal surgery in Behçet's disease. Materials and Method: Vitreoretinal surgery was applied to 26 eyes of 21 patients with Behçet's disease. Preoperative and postoperative visual acuities, number and duration of attacks, anterior and posterior segment pathologies were evaluated. Results: The mean age of the patients was 33 years and female/male ratio was 6/15. The mean follow-up was 23 months. Visual acuity increased in 15 eyes (58%), did not change in 11 eyes (42%). In the postoperative period, there was a significant decrease in mean number of uveitis attacks compared to the preoperative period (p=0.001), as well as a significant decrease in the mean duration of uveitis attacks (p=0.001). In the postoperative follow-up, intravitreal haemorrhage in 2 eyes (8%), posterior subcapsular cataract in 5 eyes (19%) and corticonuclear cataract in 2 eyes (8%) were observed. Posterior capsular opacification (PCO) developed in 5 of 16 eyes (31%) having ECLE-IOL. CME continued in 3 eyes (12%). Conclusion: Vitreoretinal surgery has favourable effect on the visual and anatomic prognosis in Behçet's patients with severe ocular complications. [source]


Skin cancer trends in northern Jordan

Background, The Jordan Cancer Registry was established in 1996, since which time all cases of cancer have been reported and registered. We have used this registry to perform the first analysis of skin cancer in northern Jordan and to compare our findings with those of published reports from other regions. Methods, All histopathologically proven cases of skin cancer, reported during the years 1997 through 2001, were reviewed. Information regarding tumor type, age, gender, and anatomical location was collected. Results, A total of 272 cases of malignant skin tumors were diagnosed between the years 1997 and 2001. Basal cell carcinoma (BCC) was the commonest type, representing 52.9% of all skin cancers. Females were more frequently affected than males, with age-adjusted incidence rates of 23.3 and 19.7 per 100,000 of population, respectively. Squamous cell carcinoma (SCC) comprised 26.4% of the total, its age-adjusted incidence rate per 100,000 of population being 14.2 for males and 6.18 for females. the incidence rate increased in males and decreased in females during the study period. The incidence of both BCC and SCC increased with age. The head and neck region was the commonest site affected by both types of cancer. Malignant melanoma (MM) comprised 11.39% of all skin cancer cases, with a female to male ratio of 1.2 : 1. The median age at onset for female patients was 49 years while that for males was 70 years, and the commonest site affected was the lower limbs, followed by the trunk. Conclusions, In Jordan, sun-related skin cancers have relatively low incidences and a rather stable pattern, compared with other areas with similar climate and skin phenotypes. [source]


Age, growth and sexual development of solenette, Buglossidium luteum (Risso, 1810), in the central Aegean Sea

Summary Age, growth, spawning period and maturity of the solenette (Buglossidium luteum Risso, 1810) were studied in the central Aegean Sea to provide fisheries managers with essential data for science-based management. A total of 1220 samples were collected by trawl hauls from July 2004 to June 2007 in ,zmir Bay (Turkey). Sample sizes ranging from 5.3 to 11.6 cm total length were composed of 46% females, 32% males and 22% immature individuals, with a female to male ratio of 1 : 0.7. Age composition stages of the females were from I to IV, and males between I and III. The length,weight relationship was calculated as W = 0.0101L3.008 for all samples. Estimated von Bertalanffy growth parameters were L = 13.30 cm, to = ,0.440 year and k = 0.481 year,1, with a growth performance index of 1.93 (,'). The spawning period began in April and continued until July. Lengths at first maturity of females and males were 8.1 and 7.9 cm total length, respectively. Both sexes matured at the age of 2 years. Some biological characteristics of the garfish (Belone belone L., 1761) in Izmir Bay, Aegean Sea

Summary The present work was aimed at determining the growth parameters of the garfish, Belone belone, a commercially important pelagic fish species in Izmir Bay (Aegean Sea). A total of 347 garfish specimens was collected during the period January,December 1997 from purse-seine boats operating in Izmir Bay. Fork length and total weight of the specimens ranged from 26.0 to 54.5 cm and from 16.51 to 281.73 g, respectively. Maximum age group determined was V for females and IV for males. Female : male ratio was 1 : 0.45. Most specimens belonged to age group II. The von Bertalanffy growth equations for length and weight were for both sexes. [source]


Depressive symptoms in intellectual disability: does gender play a role?

Abstract Background Gender issues remain largely unaddressed in the dual diagnosis arena, even in the area of depression where there is a 2,:,1 female to male ratio in the general population. This paper argues that women with intellectual disability (ID) report higher levels of depressive symptoms than men with ID and that risk factors for depression identified for women in the general population are relevant to this group. Method Findings are based on structured interviews with 99 men and women with ID, with corroborative information provided from caregivers and casebook reviews. Results Overall, women reported higher levels of depression than men. Individuals with higher depression scores were more lonely and had higher stress levels than individuals with lower scores. Women with higher depression scores were more likely to report coming from abusive situations, to have poor social support from family and to be unemployed when compared to women with lower scores, but similar differences were not found when comparing men with higher and lower depression scores. Conclusion Men and women who report experiencing these psychosocial correlates of depression should be a target group for future prevention efforts, taking gender specific concerns into consideration. Spontaneous gallbladder pathology in baboons

Abstract Background, Gallbladder pathology (GBP) is a relatively uncommon, naturally occurring morbidity in both baboons and humans. Methods, A retrospective analysis was performed on 7776 necropsy reports over a 20 year period to determine the prevalence of baboon GBP. Results, Ninety-seven cases of GBP were identified, yielding a 20 year population prevalence of 1.25%. GBP is more common in adult female baboons, occurring with a female to male ratio of nearly 2:1. Among gallbladder pathologies, cholecystitis (35.1%) and cholelithiasis (29.9%) were the most prevalent abnormalities, followed by hyperplasia (16.5%), edema (15.5%), amyloidosis (5.2%), fibrosis (4.1%), necrosis (4.1%), and hemorrhage (1.0%). Conclusion, Many epidemiologic similarities exist between GBP in baboons and humans suggesting that the baboon may serve as a reliable animal model system for investigating GBP in humans. [source]


Review of left-to-right shunting patent ductus arteriosus and short term outcome in 98 dogs

The case records of 98 dogs with a left-to-right shunting patent ductus arteriosus (PDA) were reviewed. There were 35 breeds represented, with a female to male ratio of 3:1. Forty per cent of the dogs were older than one year at initial presentation and 31 per cent had clinical signs attributable to PDA. A left heart base continuous murmur of grade IV/VI or higher was noted in 90 per cent of the dogs. On electrocardiography, the most common abnormalities were tall R waves (63 per cent) and deep QII waves (62 per cent). The radiographic triad of dilation of the descending aorta with enlargement of the main pulmonary artery segment and left atrium, typical of PDA, was noted in only 26 per cent of cases. Two-dimensional (2D) and M-mode echocardiography detected left atrial enlargement (35 per cent) and an increased left ventricular diameter in diastole (82 per cent) and systole (84 per cent) as the most common abnormalities. Doppler echocardiography demonstrated increased aortic outflow velocities in 66 per cent of cases. The overall short-term successful outcome in this study was 95 per cent. There was no significant difference between surgical ductal ligation using a standard technique or the Jackson-Henderson technique in terms of survival, occurrence of haemorrhage or residual shunting. The number of interventional procedures used in this study was too low for statistical comparison, but there appeared to be a trend towards a higher rate of residual shunting and a lower fatality rate using a coil occlusion technique. The objective of this study was to examine the incidence, prevalence, natural history, and response to treatment of complex regional pain syndrome (CRPS). All Mayo Clinic and Olmsted Medical Group medical records with codes for reflex sympathetic dystrophy (RSD), CRPS, and compatible diagnoses in the period 1989,1999 were reviewed as part of the Rochester Epidemiology Project. The authors used IASP criteria for CRPS. The study population was in the Olmsted County, Minnesota (1990 population, 106,470). The main outcome measures were CRPS-I incidence, prevalence, and outcome. Seventy-four cases of CRPS-I were identified, resulting in an incidence rate of 5.46 per 100,000 person years at risk, and a period prevalence of 20.57 per 100,000. Female: male ratio was 4 : 1, with a median age of 46 years at onset. Upper limb was affected twice as commonly as lower limb. All cases reported an antecedent event and fracture was the most common trigger (46%). Excellent concordance was found between symptoms and signs; vasomotor symptoms were the most commonly present. The authors concluded that CRPS-I is of low prevalence, more commonly affects women than men, the upper more than the lower extremity, and three out of four cases undergo resolution. These results suggest that invasive treatment of CRPS may not be warranted in the majority of cases. [source]


Pulmonary sequelae in long-term survivors of bronchopulmonary dysplasia

AbstractBackground: Bronchopulmonary dysplasia (BPD) is a common problem in premature babies. Long-term sequelae are the main concerns. Methods: A retrospective review of all BPD children born in Queen Mary Hospital, a teaching hospital of the University of Hong Kong, from January 1987 to December 1995 was conducted. Children with cerebral palsy, immunodeficiency, congenital heart disorders, renal or liver failure were excluded from analysis. Chest radiography (CXR), electrocardiogram (ECG) and pulse oximetry were routinely performed. Results: Fifty-five children completed the study. The female to male ratio was 1 : 1.1. The mean gestational age was 28 weeks. Twenty-five children were born with a birthweight of less than 1001 g. Mean age at assessment was 5.4 years. Twenty-four children (44%) demonstrated signs or symptoms of current asthma. Only seven children managed to perform the spirometry satisfactorily. One child had low forced vital capacity and one had hyperresponsive airway. The only risk factor found to be associated with current asthma was the birth month, with those children born early in the year at higher risk of developing current asthma. Seventeen of 48 children (35%) had a bodyweight below the third percentile at the corrected age of 1 year. Eleven of these seventeen children (65%) demonstrated catch-up growth at assessment. Abnormal CXR was found in 25 of 40 children (63%). All had normal pulse oximetry and ECG. Conclusions: Bronchopulmonary dysplasia children had a significantly higher risk than the general population of developing current asthma (odds ratio 4.7; 95% confidence interval 3.4,6.5; P<0.0001). The importance of birth month suggests that early life experience is important in the pathogenesis of asthma, even in BPD children. The long-term growth of BPD children was much better than previously reported. [source]


Adverse effects of neuromuscular blocking agents based on yellow card reporting in the U.K.: are there differences between males and females?,

Abstract Background Adverse drug reactions (ADRs) are known to occur during anaesthesia; in the U.K. such ADRs may be reported through the Yellow Card Scheme (YCS). Our aim was to determine the demographics of ADRs to neuromuscular blocking drugs without formal causality assessment. Methods A retrospective analysis of ADRs to seven neuromuscular blocking drugs reported via the YCS during a greater than 30-year period was performed. Sex and age were analysed in order to identify at risk groups. Results Of 998 reports, 969 included gender. Non-allergic suspected reactions occurred with almost the same frequency as those with an allergic component. The majority occurred in females 676 (70%), and significant sex differences were measured between drugs. Males were more likely to have suffered an ADR to atracurium (p,=,0.01) whilst females experienced more ADRs to suxamethonium (p,=,0.01). ADRs proved fatal in 81 (9%) of the 950 reports for single drugs. Mortality following suxamethonium was significantly higher in males at 22% compared with 9% females (p,<,0.001). More women than men were reported to have allergic reactions, 73% (362/499) compared with 27% (137/499) respectively. The female:male ratio for ADRs was reversed for subjects <,10 years compared with peak ADR reports during the decade from 31,40 years. Conclusions Sex differences in mortality exist in this analysis. The unexpected high frequency of non-allergic ADRs suggests that morbidity and mortality from reactions to established drugs is twice that expected from allergic reactions alone. Inhibitory Effect of Selenium Supplementation on the Reproductive Performance in Synchronized Merino Sheep at Range Conditions in a Selenium-deficient Area

Contents The effect of selenium (Se) supplementation on the reproductive performance of Merino ewes mated out of the normal breeding season was studied in a 2 (no Se supplementation vs Se supplementation) × 2 (ewes mated at natural oestrus vs synchronized oestrus) factorial design with 50 ewes per treatment combination. Synchronization of oestrus was achieved by intravaginal insertion of 40 mg cronolone sponges for 14 days and administration of 440 IU PMSG at sponge withdrawal. Se supplementation was by the addition to the concentrate with 0.5 mg/kg of Se in the form of selenomethionin, for the 3 months prior to the mating and during gestation. Ewes were kept under range conditions in south-west Spain, and exposed to Merino rams at a female: male ratio of 4 : 1 from 2 days following sponge withdrawal, or the equivalent time, for 21 days. Se supplementation alone did not improve significantly ewe fertility or lamb birth weight, but there was a positive effect of synchronizing ewes mated out of the breeding season as more of these ewes lambed and produced more lambs than those not synchronized. However, a strong interaction was found between synchronization and Se supplementation, causing a deleterious effect on the reproductive performance of ewes. This negative effect, presumably related to high embrionary mortality caused by Se toxicity, should be taken into account for oestrus synchronizing in Se-deficient areas. The supplementation with Se, for the prevention of nutritional myodystrophy degeneration in lambs, should be conducted in a fashion which does not generate high levels of Se in the ewes around the mating period. THE IMPACT OF SYNOPTIC CYTOLOGY REPORTING ON FINE-NEEDLE ASPIRATION CYTOLOGY OF THYROID NODULES

Background: Fine-needle aspiration cytology (FNAC) is integral to the diagnosis and management of patients with thyroid nodules. We introduced synoptic cytology reporting for thyroid nodules in 2004. The aim of this study was to examine the effect of synoptic cytology reporting in our institution. Methods: A comparative study of two 2-year periods (1 August 2002 to 1 August 2004 and 2 August 2004 to 2 August 2006) before and after the introduction of synoptic reporting was conducted from a prospectively collected database of patients presented with thyroid nodules. The only change during these periods was the format of FNAC reporting. We used the same radiological practice and cytopathology service throughout the study period. All patients are still being followed up. Results: There were a total of 660 patients. Of these, 376 were operated and 284 non-operated. The female to male ratio was 7:1. Comparing the two periods, the overall FNAC sensitivities were 60% versus 79.1%; specificities, 83.7% versus 79.4%; accuracy, 76% versus 79.3%; false-positive result, 16.3% versus 20.6% and false-negative result, 40% versus 20.9%. The non-diagnostic rates were 7.4% versus 3.15%. FNAC prompted surgery in 66.7% versus 100% in carcinoma and 56.4% versus 73.6% in adenoma. A benign FNAC prompted surgery in 15% versus 19.8% of cases. There was no thyroid cancer detected in the current follow up. Conclusions: Synoptic cytology reporting has resulted in an overall improvement in all measures of the tests. It is a simple and effective tool to use. Synoptic cytology reporting is therefore recommended for all endocrine surgical units. [source]


Increased frequency of extremely skewed X chromosome inactivation in juvenile idiopathic arthritis

Objective Juvenile idiopathic arthritis (JIA) is a childhood rheumatic disease of unknown etiology. Two subgroups of JIA, i.e., oligoarticular and polyarticular, are thought to have an autoimmune component, and show a higher female:male ratio. Skewed X chromosome inactivation (XCI) has previously been shown to be associated with scleroderma and autoimmune thyroiditis, 2 autoimmune disorders occurring predominantly in females. This study was undertaken to extend the analysis to the pediatric age group and to determine the XCI profiles of patients with JIA. Methods A polymorphic repeat in the androgen receptor gene was genotyped to determine XCI status in 81 female patients with JIA (21 with polyarticular disease and 60 with oligoarticular disease) and 211 healthy female controls. DNA obtained from venous blood samples was used for this analysis. Results Informative data were obtained on 62 JIA patients and 155 controls. Skewed XCI was observed in 14 patients (22.6%) and 11 controls (7.1%) (P = 0.0036), and extreme skewing was apparent in 8 patients (12.9%) and 2 controls (1.3%) (P = 0.0008). Conclusion Our findings in the present study indicate that skewed XCI may be a risk factor for the occurrence of autoimmune disorders, including JIA. [source]


Change in sex ratio, disease course and age at diagnosis in Oslo MS patients through seven decades

Objectives , To study changes in sex ratio, disease course, time from onset to diagnosis and age at diagnosis by year of birth in a well-defined population of multiple sclerosis (MS) patients. Materials and methods , Based on the Oslo MS Registry patients born from 1910 to 1980 with residence in Oslo at time of diagnosis were studied. Data were analyzed by 10-year intervals based on year of birth. Results , The female to male ratio increased significantly from 1.48 to 2.30 through seven decades. Also, the ratio of initial relapsing,remitting (RR-MS) to primary progressive (PP-MS) disease course increased significantly from 1.93 to 16.00. The time from onset to diagnosis and the mean age at diagnosis declined significantly during the same period. Conclusions , This study shows that there has been a change in MS sex ratio, disease course and age at diagnosis through a period of seven decades, suggesting an environmental factor mainly affecting women at a younger age and causing a RR-MS disease course. Background ,, In this article, current scientific knowledge on the chronic fatigue syndrome (CFS) is reviewed. The US case definition of CFS (the CDC-definition) is most widespread in research and clinical practice. Estimates of prevalence vary from 0.2% to above 2%. The female,male ratio is approximately 3:1. Clinical Features ,, Severe fatigue is the dominating complaint; it is worsened from exertions and not substantially relieved by rest. In addition, the patients might have a varying combination of accompanying symptoms. Clinical evaluation should be based upon standardized guidelines, including an assessment of functional impairments. Pathophysiology ,, The pathophysiology should be interpreted within a biopsychosocial framework. Present knowledge suggests that certain genetic polymorphisms and personality traits might be regarded as predisposing factors, some infections and severe psychosocial stress constitute precipitating factors, whereas disturbances of immunity, skeletal muscle, cognitive abilities, endocrine control and cardiovascular homeostasis are possible perpetuating factors. Treatment ,, Cognitive behavioural therapy and graded exercise therapy are of proven value in randomized controlled trials. Several pharmaceutical measures have been explored and found to have no beneficial effect. Most patients might expect long-term improvement, but full recovery is rare; however, the prognosis is better among adolescents. [source]


Hormone replacement therapy and lung cancer risk in Chinese

Abstract BACKGROUND. The association between hormone replacement therapy (HRT) and a reduced lung cancer risk has been reported in previous studies. There is a high female to male ratio in Chinese lung cancer patients, and female patients have different clinicopathological characteristics compared with Western patient populations. The authors investigated whether HRT may reduce lung cancer risk in Taiwan. METHODS. The authors used a case-control study design to investigate 826 women with lung cancer and 531 healthy controls. Personal interviews based on a structured questionnaire were performed to collect information on HRT use of at least 3 months, age, ethnicity, active and passive smoking, exposure to air pollution, cooking or incense fumes, body mass index (BMI), menopause, and family history of cancers. RESULTS. HRT use was associated with reduced lung cancer risk with a multivariate, adjusted odds ratio of 0.70 (95% CI, 0.53,0.94; P = .019). HRT use was associated with reduced odds ratio of lung cancer in all subset analyses stratified by histology, active and passive cigarette smoking, BMI, history of incense burning, cooking, and motorcycle riding, as well as family history of certain cancers. Mastitis in early infancy

Abstract Aim: To evaluate the clinical features and microbiological findings in young infants with mastitis. Methods: Retrospective review of medical records of 18 infants with breast inflammation during the first 3 mo of life seen in the paediatric emergency department between 1992 and 2002. Results: All were full-term infants with female,male ratio of 3.5,1. The age ranged from 12 to 45 d, with a peak in the 4th and 5th weeks of life. Only five patients had systemic manifestations, and five were pretreated with oral antibiotics (amoxicillin-clavulanic acid). The latter as well as seven additional cases required incision and drainage due to abscess formation. Bacterial cultures grew Staphylococcus aureus in 10 cases including all pretreated infants. In four of these cases, Gram stain showed the pathogen. After antimicrobial treatment, no recurrence was observed in any of the patients. Conclusions: These findings suggest that mastitis in early infancy should be treated with parenteral antibiotics guided by Gram stain when available and informative. Otherwise, ,-lactamase-resistant antibiotics are a reasonable empirical initial treatment pending culture results. Optimizing the management of infants with mastitis is important especially since abscess formation requiring incision may be detrimental for later breast development. [source]
