Trade Liberalization and the Fiscal Squeeze: Implications for Public Investment

This article examines the impact of trade liberalization on the level and structure of government expenditures across countries, with particular emphasis on low income countries. It develops the argument that the policies employed during trade liberalization have resulted in a fiscal squeeze as a result of declining tax revenues and rising interest expenditures. To surmount this fiscal hurdle, expenditures on physical capital, which have negligible political ramifications, have been reduced. Other more politically sensitive expenditures, such as spending on social capital, have been financed by incurring additional debt. However, additional debt has exerted upward pressure on interest payments, further exacerbating the fiscal situation. The statistical analysis carried out to examine the evidence uses panel data for eighty developing and industrialized countries over the period 1970,98 and employs a fixed,effects regression framework to account for country,specific characteristics. The results indicate that trade liberalization has indeed resulted in declining revenues and higher interest expenditures and that these factors have contributed to the observed decline in infrastructure spending. Aid to Agriculture, Growth and Poverty Reduction

Agriculture and rural growth promotion show a recent ,comeback' in development cooperation, but action on the ground so far is not sufficient. After years of neglect, policy makers have recognized that poverty reduction in many low income countries can only be achieved if development efforts are clearly focused on the sector which employs most of the poor, and the space where most of the poor live. The importance of agricultural growth was amply demonstrated during the economic transformation of Asia. Forty years ago, Asia was a continent of widespread poverty. Today, most Asian countries are experiencing significant growth and poverty reduction. Rapid growth in productivity in the small-farm sector helped drive this process. Sub-Saharan Africa, however, failed to achieve rapid agricultural growth and remains mired in poverty and hunger. If Africa is to halve poverty by 2015 in accordance with the Millennium Development Goals (MDG), agriculture will need to maintain an annual growth rate of 6 per cent between 2000 and 2015. China's experience from 1978 to 1984 shows such growth is possible. Achieving the desired rapid rates of growth in Africa will require coherent policies by governments and donors, a substantial investment of public resources in rural infrastructure and access to agricultural technology, and significant improvement in national governance. Après s'en être désintéressés pendant des années, les décideurs politiques finissent par admettre que, dans beaucoup de pays a faible revenu, il ne sera pas possible de réduire la pauvreté sans focaliser les efforts de développement sur les secteurs qui emploient le plus de pauvres et les zones dans lesquelles ils vivent pour la plupart. Les transformations économiques de l'Asie montrent bien l'importance de la croissance agricole. Il y a quarante ans, l'Asie étaient le continent de la pauvreté généralisée. Aujourd'hui, la plupart des pays d'Asie connaissent une croissance très significative et la pauvreté s'y réduit. L'augmentation de la productivité dans le secteur des petites exploitations a contribuéà la mise en ,uvre de ce processus. L'Afrique sub-saharienne, au contraire, n'a pas réussi à développer une croissance agricole rapide, ce qui la fait s'embourber dans la faim et la pauvreté. Si l'Afrique doit réduire de moitié la pauvreté d'ici 2015 comme l'y invitent les objectifs millénaires du développement (MDG), il faudra y maintenir un taux de croissance annuel de 6% pour l'agriculture entre 2000 et 2015. L'expérience de la Chine entre 1978 et 1984 montre que c'est possible. Bei der wirtschaftlichen Transformation in Asien wurde sehr deutlich, wie wichtig das landwirtschaftliche Wachstum ist. Vor 40 Jahren war Armut in Asien weit verbreitet. Heute zeichnen die meisten asiatischen Länder durch signifikantes Wachstum und durch Armutsverringerung aus. Ein schneller Anstieg der Produktivität bei den kleineren landwirtschaftlichen Betrieben half dabei, diese Entwicklung voran zu treiben. In den afrikanischen Ländern unterhalb der Sahara (Sub-Sahara-Afrika) konnte schnelles landwirtschaftliches Wachstum jedoch nicht erreicht werden, dort dominieren weiterhin Armut und Hunger. Wenn Afrika gemäß der Millenniumsentwicklungsziele (Millennium Development Goals, MDG) die Armut bis zum Jahr 2015 halbieren soll, muss die Landwirtschaft eine jährliche Wachstumsrate von sechs Prozent zwischen den Jahren 2000 und 2015 aufrecht erhalten. Die Erfahrungen aus China aus den Jahren 1978 bis 1984 belegen, dass ein solches Wachstum möglich ist. Damit die gewünschten hohen Wachstumsraten in Afrika erzielt werden können, sind kohärente Politikmaßnahmen seitens der Regierungen und der Geldgeber, erhebliche Investitionen von öffentlichen Ressourcen in die ländliche Infrastruktur und in den Zugang zur Agrartechnologie sowie eine signifikante Verbesserung der nationalen Governance erforderlich. Global map of the prevalence of symptoms of rhinoconjunctivitis in children: The International Study of Asthma and Allergies in Childhood (ISAAC) Phase Three

Background:, Phase One of the International Study of Asthma and Allergies in Childhood (ISAAC) measured the global patterns of prevalence and severity of symptoms of rhinoconjunctivitis in children in 1993,1997. Methods:, International Study of Asthma and Allergies in Childhood Phase Three was a cross-sectional survey performed 5,10 years after Phase One using the same methodology. Phase Three covered all of the major regions of the world and involved 1 059 053 children of 2 age groups from 236 centres in 98 countries. Results:, The average overall prevalence of current rhinoconjunctivitis symptoms was 14.6% for the 13- to 14-year old children (range 1.0,45%). Variation in the prevalence of severe rhinoconjunctivitis symptoms was observed between centres (range 0.0,5.1%) and regions (range 0.4% in western Europe to 2.3% in Africa), with the highest prevalence being observed mainly in the centres from middle and low income countries, particularly in Africa and Latin America. Co-morbidity with asthma and eczema varied from 1.6% in the Indian sub-continent to 4.7% in North America. For 6- to 7-year old children, the average prevalence of rhinoconjunctivitis symptoms was 8.5%, and large variations in symptom prevalence were also observed between regions, countries and centres. Discussion:, Wide global variations exist in the prevalence of current rhinoconjunctivitis symptoms, being higher in high vs low income countries, but the prevalence of severe symptoms was greater in less affluent countries. Co-morbidity with asthma is high particularly in Africa, North America and Oceania. This global map of symptom prevalence is of clinical importance for health professionals. [source]

How are the benefits and costs of a customs union divided between member countries? Outcomes depend on the comparative advantage of members, relative to each other and relative to the rest of the world. Countries with a comparative advantage between that of their partners and the rest of the world do better than countries with an ,extreme' comparative advantage. Consequently, integration between low income countries tends to lead to divergence of member country incomes, while agreements between high income countries cause convergence. Results suggest that developing countries are likely to be better served by ,north-south' than by ,south-south' agreements. [source]


Child and adolescent mental disorders: the magnitude of the problem across the globe

Objective:, Describe objectively the global gaps in policy, data gathering capacity, and resources to develop and implement services to support child mental health. Methods:, Report on the World health Organization (WHO) child and adolescent mental health resources Atlas project. The Atlas project utilized key informants and was supplemented by studies that focused on policy. This report also draws on current epidemiological studies to provide a context for understanding the magnitude of the clinical problem. Results:, Current global epidemiological data consistently reports that up to 20% of children and adolescents suffer from a disabling mental illness; that suicide is the third leading cause of death among adolescents; and that up to 50% of all adult mental disorders have their onset in adolescence. While epidemiological data appears relatively uniform globally, the same is not true for policy and resources for care. The gaps in resources for child mental health can be categorized as follows: economic, manpower, training, services and policy. Key findings from the Atlas project include: lack of program development in low income countries; lack of any policy in low income countries and absent specific comprehensive policy in both low and high income countries; lack of data gathering capacity including that for country-level epidemiology and services outcomes; failure to provide social services in low income countries; lack of a continuum of care; and universal barriers to access. Further, the Atlas findings underscored the need for a critical analysis of the ,burden of disease' as it relates to the context of child and adolescent mental disorders, and the importance of defining the degree of ,impairment' of specific disorders in different cultures. Conclusions:, The recent finding of substantial gaps in resources for child mental health underscores the need for enhanced data gathering, refinement of the economic argument for care, and need for innovative training approaches. Targeted health insurance in a low income country and its impact on access and equity in access: Egypt's school health insurance

Abstract Governments are constantly faced with competing demands for public funds, thereby necessitating careful use of scarce resources. In Egypt, the School Health Insurance Programme (SHIP) is a government subsidized health insurance system that targets school children. The primary goals of the SHIP include improving access and equity in access to health care for children while, at the same time, ensuring programme sustainability. Using the Egyptian Household Health Utilization and Expenditure Survey (1995), this paper empirically assesses the extent to which the SHIP achieves its stated goals. Our findings show that the SHIP significantly improved access by increasing visit rates and reducing financial burden of use (out-of-pocket expenditures). With regard to the success of targeting the poor, conditional upon being covered, the SHIP reduced the differentials in visit rates between the highest and lowest income children. However, only the middle-income children benefitted from reduced financial burden (within group equity). Moreover, by targeting the children through school enrollment, the SHIP increased the differentials in the average level of access between school-going children and those not attending school (overall equity). Children not attending school tend to be poor and living in rural areas. Our results also indicate that original calculations may underestimate the SHIP financial outlays, thereby threatening the long run financial sustainability of the programme. POLICY IMPLEMENTATION IN AN AFRICAN STATE: AN EXTENSION OF KINGDON'S MULTIPLE-STREAMS APPROACH

Kingdon's multiple-streams framework, which emerged in the mid-1980s, today forms one of the indispensable analytical frameworks for understanding public policy agenda-setting. However, it is only in the context of wealthy countries that this approach has been validated for setting the agenda of national and international policies. This article reports the results of empirical research in an African state studying the transferability of a threefold theoretical innovation. The question under consideration is whether the multiple-streams framework is useful for examining public policy implementation at the local level and in the context of a low income country. The research findings confirm the premise that the multiple-streams framework can be extended and can lead to the formulation of several theoretical propositions. [source]
