EFNS guidelines on the use of neuroimaging in the management of multiple sclerosis

Magnetic resonance (MR)-based techniques are widely used for the assessment of patients with suspected and definite multiple sclerosis (MS). However, despite the publication of several position papers, which attempted to define the utility of MR techniques in the management of MS, their application in everyday clinical practice is still suboptimal. This is probably related, not only, to the fact that the majority of published guidelines focused on the optimization of MR technology in clinical trials, but also to the continuing development of modern, quantitative MR-based techniques, that have not as yet entered the clinical arena. The present report summarizes the conclusions of the ,EFNS Expert Panel of Neuroimaging of MS' on the application of conventional and non-conventional MR techniques to the clinical management of patients with MS. These guidelines are intended to assist in the use of conventional MRI for the diagnosis and longitudinal monitoring of patients with MS. In addition, they should provide a foundation for the development of more widespread but rational clinical applications of non-conventional MR-based techniques in studies of MS patients. [source]


The stability of maternal ratings of medical student interviews

Introduction, Little information is available on the intrarater reliability of parent ratings of medical student interview skills. The aim of this study was to compare maternal ratings of a videotaped medical student interview, with ratings by the same mother of the same interview seen a month later. Method, Thirty mothers rated 1 of 2 ,medical student' interview videotapes (random allocation). An actor played the role of the student and demonstrated different student skill levels in each interview. Mothers rated the same interview again a month later (mean ± SD 34 ± 13 days). Maternal satisfaction was measured using the Medical Interview Satisfaction Scale (MISS, maximum score 203) and the Interpersonal Skills Rating Scale (IPS, maximum score 91) with higher scores indicating higher satisfaction. Results, No significant differences in mean maternal ratings were observed following the first and second viewings of each interview. The average percentage difference in first and second viewing scores was MISS 4%, IPS 8% with correlation coefficients MISS 0.96, IPS 0.87 (P < 0.05). Conclusions, The high levels of stability in maternal evaluations of simulated medical student interviews supports the use of maternal evaluations for longitudinal monitoring of medical student performance. Diffusion tensor MRI in rat models of invasive and well-demarcated brain tumors

Abstract Diffusion tensor imaging (DTI) and its metrics, such as mean diffusivity (MD) and fractional anisotropy (FA), have been used to detect the extent of brain tumors and understand tumor growth and its influence on the surrounding tissue. However, there are conflicting reports on how DTI metrics can be used for tumor diagnosis. The physiological interpretation of these metrics in terms of tumor growth is also not clear. The objective of this study was to investigate the DTI parameters in two rat brain tumor models (9L and F98) with different patterns of aggressiveness by longitudinal monitoring of tumor growth and comparing the DTI parameters of these two tumor models. In addition to the standard DTI metrics, MD and FA, we measured other metrics representing diffusion tensor shape, such as linear and planar anisotropy coefficients (CL and CP), and orientational coherence measured by lattice index (LI), to characterize the two tumor models. The 9L tumor had higher FA, CL, and LI than the F98 tumor. F98 had a larger difference in anisotropies between tumor and peritumor regions than 9L. From the eigenvalues, it was found that the increase in CL and trace of the 9L tumor was due to an increase in the primary eigenvalue, whereas the increase in CP in the peritumor region was due to an increase in both primary and secondary eigenvalues and a decrease in tertiary eigenvalue. Our results indicate that shape-oriented anisotropy measures, such as CL and CP, and orientational coherence measures, such as LI, can provide useful information in differentiating these two tumor models and also differentiating tumor from peritumoral regions. WT1 gene expression: an excellent tool for monitoring minimal residual disease in 70% of acute myeloid leukaemia patients , results from a single-centre study

Summary Following induction chemotherapy for acute myeloid leukaemia (AML), sensitive determination of minimal residual disease (MRD) in patients achieving complete remission (CR) should enable the detection of early relapse and allow intervention at a more favourable stage than at overt relapse. We have determined the expression levels of the Wilms' tumour gene (WT1) by real-time quantitative polymerase chain reaction (RQ-PCR) in peripheral blood and bone marrow in 133 newly diagnosed AML patients and compared them with those in healthy volunteers. At diagnosis, the WT1 level exceeded normal expression in 118 of 133 (89%) patients, and was high enough to allow for detection of a WT1 decrease of least 1000-fold in 98 of 133 (74%) patients following induction therapy. Concomitant monitoring of fusion transcripts (PML-RAR,, AML1-ETO, MLL-MLL, CBF, - MYH11, or DEK-CAN) in 38 patients identified different relationships between WT1 and fusion transcript levels, the AML1-ETO group showing remarkably low levels of WT1 compared with fusion transcript. In 32 patients analysed longitudinally there was close concordance between relapse and increased WT1 levels. Parallel longitudinal monitoring of WT1 and fusion transcript showed close correlation in 18 of 18 patients. We conclude that WT1 expression by RQ-PCR may be employed as a tool to detect MRD in the majority of fusion transcript-negative AML patients. [source]
