Probable trigeminal autonomic cephalgia in a 3-month-old male infant

To my knowledge trigeminal autonomic cephalgias (TACs) have not previously been reported in infancy. The diagnosis is dependent on an accurate history, including parents noting any physical signs at the time of the episode. Obtaining a clear history can be challenging when such symptoms occur in preverbal children. Similarly, physical signs, being transient, may have resolved by the time the parents take the child to a doctor. In addition, the investigations may also be normal. In such circumstances, taking a photograph during an episode can confirm the diagnosis. I describe a case of probable trigeminal autonomic cephalgia starting in a 3-month-old male infant who presented with screaming episodes associated with characteristic changes seen on his face. Investigations, including cranial magnetic resonance imaging, electroencephalography, and urinary catecholamines, were normal. The diagnosis was confirmed from a photograph taken by the parents at the time of the attack. As the condition is very rare in young children, there is little information available in the literature on using treatment for prophylaxis or for aborting acute episodes in this age group. Ultrastructural changes in skeletal muscle of the tail of the lizard Hemidactylus mabouia immediately following autotomy

Although autotomy and subsequent regeneration of lizard tails has been extensively studied, there is little information available on ultrastructural changes that occur to the muscle fibers at the site of severance. Thus, in the present study, we examine the ultrastructure of the musculature of the remaining tail stump of the lizard Hemidactylus mabouia immediately after autotomy. Our results show that exposed portions of the skeletal muscle fibers of the stump that are unprotected by connective tissue bulge to produce large mushroom-like protrusions. These exposed portions show abnormal structure but suffer no leakage of cytoplasmic contents. Many small and large vesicular structures appeared between myofibrils in the interface at this disarranged region (distal) and the other portion of the fibers that remain unchanged (proximal). These vesicles coalesce, creating a gap that leads to the release of the mushroom-like protrusion. So, our results showed that after the macroscopic act of autotomy the muscular fibers release part of the sarcoplasm as if a second and microscopic set of autotomic events takes place immediately following the macroscopic act of autotomy. Presumably these changes pave the way for the formation of a blastema and the beginning of regeneration. [source]


The International Adult Literacy Survey in Britain: Impact on policy and practice

Abstract There is increasing concern for the skills of the workforce in the UK and elsewhere, but despite this concern until recently there has been little information available which objectively measures basic skills in adults. In this paper, evidence derived from the prose scale of the International Adult Literacy Survey (IALS, 1996) is outlined, with emphasis on the performance of adults at the lowest levels, 1 and 2 in the United Kingdom. A new analysis based on the 183 adults who self-reported learning disabilities demonstrates that over 50% of this group perform at level 1 on the prose scale. Over 60% report that these disabilities persist into adult life, although this number falls to 50% in the youngest age group, reflecting changes in recognition of learning disabilities within the education system. The paper concludes with a case study of the redefinition of basic skill levels in Britain based on the IALS levels. The impact of the IALS findings on policy and practice, and in particular through the recommendations of the Moser report, are discussed. Copyright © 2002 John Wiley & Sons, Ltd. [source]


Longitudinal development of equine conformation from weanling to age 3 years in the Thoroughbred

Summary Reasons for performing study: There is little information available to define conformational changes with age using an objective but practical method of recording specific body measurements. Objective: To analyse conformation objectively in a population of racing Thoroughbreds and describe the changes from weanling to age 3 years. Methods: Annual photographs were taken over 4 years and conformation measurements made from photographs using specific reference points marked on the horses. Results: Correlation analysis revealed highly significant, moderate to strong relationships between long bone lengths and wither height for all ages. All long bone lengths showed moderate to strong relationships with each other for all ages. The front and rear pastern angles were significantly correlated with the angle of the dorsal surface of the front and rear hooves, respectively, for all. Wither height, croup height and length of neck topline, neck bottomline, scapula, humerus, radius and femur increased significantly from age 0,1 year and age 1,2 years. Hoof lengths (medial and lateral, right and left) grew significantly between the ages of 0 and 1 and 1 and 2 years, but decreased in length between age 2 and 3 years. Horses became more offset in the right limb between weanling and age 3 years, but the offset ratios did not change with age on the left limb. The angle of the scapula (I), shoulder and radiometacarpus significantly increased between all age groups (became more upright). The angle of the dorsal surface of the hooves (both front and hind) decreased significantly from ages 0 to 1 and 1 to 2 years, but showed no significant difference between ages 2 and 3 years. Conclusions: A strong relationship between long bone lengths and wither height for all ages supports the theory that horses are proportional. Longitudinal bone growth in the distal limb increased only 5,7% from weanling to age 3 years and is presumably completed prior to the yearling year. Several growth measures increased from ages 0 to 1 and 1 to 2 years, but did not increase from age 2,3 years; indicating that growth rate either showed or reached a plateau at this time. Potential relevance: This study provides objective information regarding conformation and skeletal growth in the Thoroughbred which can be utilised for selection and recognition of significant conformational abnormalities. Obesity and metabolic syndrome in histone demethylase JHDM2a-deficient mice

Histone H3 lysine 9 (H3K9) methylation is a crucial epigenetic mark of heterochromatin formation and transcriptional silencing. Recent studies demonstrated that most covalent histone lysine modifications are reversible and the jumonji C (JmjC)-domain-containing proteins have been shown to possess such demethylase activities. However, there is little information available on the biological roles of histone lysine demethylation in intact animal model systems. JHDM2A (JmjC-domain-containing histone demethylase 2A, also known as JMJD1A) catalyses removal of H3K9 mono- and dimethylation through iron and ,-ketoglutarate dependent oxidative reactions. Here, we demonstrate that JHDM2a also regulates metabolic genes related to energy homeostasis including anti-adipogenesis, regulation of fat storage, glucose transport and type 2 diabetes. Mice deficient in JHDM2a (JHDM2a,/,) develop adult onset obesity, hypertriglyceridemia, hypercholesterolemia, hyperinsulinemia and hyperleptinemia, which are hallmarks of metabolic syndrome. JHDM2a,/, mice furthermore exhibit fasted induced hypothermia indicating reduced energy expenditure and also have a higher respiratory quotient indicating less fat utilization for energy production. These observations may explain the obesity phenotype in these mice. Thus, H3K9 demethylase JHDM2a is a crucial regulator of genes involved in energy expenditure and fat storage, which suggests it is a previously unrecognized key regulator of obesity and metabolic syndrome. Stress synergy between drought and a common environmental contaminant: studies with the collembolan Folsomia candida

Summary The term global change is used predominantly in connection with the global temperature increase and associated changes in weather patterns over the next century. In a broader sense it also covers other anthropogenic impacts on the environment such as habitat fragmentation and pollution. The individual effects of each of these stress types have been extensively studied in the biota. However, organisms will frequently encounter these stress types in combination rather than alone and there is little information available on the effects of stress combinations. Here an examination is made of the interaction between realistic levels of summer drought and a common contaminant of agricultural soil (4-nonylphenol, NP), on a widespread soil invertebrate, the collembolan Folsomia candida. These stress factors were tested individually and in combination using a full factorial design. This approach revealed the existence of highly significant Bliss type synergistic interaction between the two stress types. Thus, exposure to NP significantly reduced the drought tolerance of this organism and, reciprocally, the toxicity of NP (LC50) during realistic summer drought was more than doubled in comparison to the value obtained under optimal soil moisture conditions. Furthermore, it is shown that NP has a detrimental effect on the physiological mechanisms underlying this animal's drought tolerance, thus providing some explanation for the mechanisms involved in the synergy. It is argued that this type of synergy is unlikely to be confined to this particular combination of stresses and thus there is a need to study the interactions between dominant natural stresses and pollution. The most important implication of these results is that some of the effects of global climate changes can be predicted to be most severe in polluted areas. [source]


Haematological, hepatic and renal alterations after repeated oral or intraperitoneal administration of monoisoamyl DMSA.

Abstract Monoisoamyl 2,3-dimercaptosuccinic acid (MiADMSA), a vicinal thiol chelator, is gaining recognition recently as a better chelator than meso 2,3-dimercaptosuccinic acid (DMSA) in decreasing heavy metal burden in tissues because of its lipophilic character. There is, however, little information available on the toxicological properties of this chelator after repeated administration in animals. In the present study, we investigated the dose-dependent effect of MiADMSA on various biochemical parameters suggestive of alterations in haem biosynthesis and hepatic, renal and brain oxidative stress after 21 days of repeated intraperitoneal (i.p.) or oral (p.o.) administration to rats. The concentration of essential metals in blood and soft tissues was determined along with histopathological observations of hepatic and renal tissues. The results suggest that MiADMSA administration had no effect on blood ,-aminolevulinic acid dehydratase activity. However, an increase in zinc protoporphyrin and a decrease in haemoglobin levels were noted in animals given MiADMSA i.p. A moderate increase in serum alkaline phosphatase suggested mild hepatotoxicity at the highest dose (100 mg kg,1, i.p.). This was confirmed by histopathological examinations, which identified basophilic stippling, granulation of the cytoplasm, haemorrhage and congestion. At the highest dose, levels of hepatic thiobarbituric acid reactive substance and oxidized glutathione were increased above those of control values. Levels of hepatic reduced glutathione were decreased. Taken together, these observations point to oxidative stress. In animals administered MiADMSA i.p. there was an increase in the brain malondialdehyde levels at the two higher doses (50 and 100 mg kg,1). Essential metal status revealed a significant effect of MiADMSA (p.o.) in increasing blood zinc while significantly decreasing the kidney zinc level. The most significant adverse effect of MiADMSA was on copper concentration, which showed significant depletion from almost all major organs. Magnesium levels in blood decreased but increased in liver of MiADMSA-administered rats. Histopathological observations of liver and kidneys suggest few moderate lesions. It can be concluded that repeated administration of MiADMSA is compromised with some mild toxic effect, particularly the loss of copper. Efficacy and safety of over-the-counter analgesics in the treatment of common cold and flu

Summary Rationale:, Common cold and flu are the most common human illnesses, and over-the-counter (OTC) analgesics are widely used to treat the pain and fever symptoms. Despite the every day use of these analgesic there is little information available in the literature on the efficacy and safety of these medicines in treating colds and flu symptoms. The aim of this review was to determine the safety and efficacy of the analgesics, aspirin, paracetamol and aspirin for the treatment of colds and flu. Methods:, Electronic databases and a personal database were searched and the information retrieved together with information from relevant textbooks has been integrated in the review. Results:, The literature search established that there is relatively little information on the use of analgesics in treating colds and flu and that much of the safety and efficacy data must be related to other pain and fever models. The review establishes that aspirin, paracetamol and ibuprofen are safe in OTC doses and that there is no evidence for any difference between the medicines as regards efficacy and safety for treatment of colds and flu (except in certain cases such as the use of aspirin in feverish children). There is also no evidence that these medicines prolong the course of colds and flu by any effect on the immune system or by reducing fever. Conclusion:, Despite the lack of clinical data on the safety and efficacy of analgesics for the treatment of colds and flu symptoms a case can be made that these medicines are safe and effective for treatment of these common illnesses. Characterization of microsatellite loci for the redbanded seabream, Pagrus auriga (Teleostei, Sparidae)

Abstract The redbanded seabream Pagrus auriga (Teleostei, Sparidae) is a species of a high commercial value in Spain. There is currently little information available about the genetic characteristics of both wild and cultured populations. In this survey, we have developed eight polymorphic microsatellites for the redbanded seabream using an enriched genome library protocol. All of them were polymorphic in the 64 individuals tested, 22 of which were wild specimens, and 42 were individuals from a captive reproductive broodstock. These markers can potentially be useful tools for use in population genetic studies. [source]


A survey of pediatric caudal extradural anesthesia practice

Summary Background:, Caudal extradural blockade is one of the most commonly performed procedures in pediatric anesthesia. However, there is little information available on variations in clinical practice. Objectives:, To perform a survey of members of the Association of Paediatric Anaesthetists of Great Britain and Ireland who undertake caudal anesthesia. Methods:, An ,online' World Wide Web questionnaire collected information on various aspects of clinical practice. The survey ran from April to June 2008. Results:, There were 366 questionnaires completed. The majority of respondents had >5 years of pediatric experience and performed up to ten caudal extradural procedures a month. The commonest device used was a cannula (69.7%) with 68.6% using a 22G device. There was a trend toward the use of a cannula in those anesthetists with <15 years experience, while those with >15 years experience tended to use a needle. Most anesthetists (91.5%) did not believe that there was a significant risk of implantation of dermoid tissue into the caudal extradural space. The majority used a combination of clinical methods to confirm correct placement. Only 27 respondents used ultrasound. The most popular local anesthetics were bupivacaine (43.4%) and levobupivacaine (41.7%). The most common additives were clonidine (42.3%) and ketamine (37.5%). The caudal catheter technique was used by 43.6%. Most anesthetists (74%) wear gloves for a single shot caudal injection. Conclusions:, This survey provides a snapshot of current practice and acts a useful reference for the development of enhanced techniques and new equipment in the future. [source]


A comparison of physical activity levels in two contrasting elderly populations in Thailand

There is little information available on physical activity patterns in the elderly in developing countries. This is particularly important at a time when developing countries are in transition and show signs of moving toward Western style patterns of physical activity and social structure. This study is unique in that it compares the physical activity levels in two contrasting communities: a residential home and a rural community of elderly subjects in Thailand. Ninety elderly people living in a residential home and 78 elderly people living with their families in a rural community were recruited. The men in the rural community were slightly lighter and leaner than the men in the residential community, but the differences were not significant. However, the women were significantly (P < 0.001) heavier and fatter in the residential home than in the rural community. This is consistent with the higher levels of physical activity in the rural community. There was considerable variation in physical activity in this population. The physical activity level (PAL) values ranged from 1.21 to 1.73 in the residential home and from 1.21 to 3.08 in the rural community. This is apparently the first study of physical activity patterns in elderly subjects in a developing country. It highlights the variation in activity within the rural community and between both communities. The opportunities for the elderly living in the residential home to have active lifestyles are limited. It is likely that this will result in increased morbidity in the final years of life. Am. J. Discovery, characterization and validation of single nucleotide polymorphisms within 206 bovine genes that may be considered as candidate genes for beef production and quality

Summary A large number of putative single nucleotide polymorphisms (SNPs) have been identified from the bovine genome-sequencing project. However, few of these have been validated and many will turn out to be sequencing artefacts or have low minor allele frequencies. In addition, there is little information available on SNPs within coding regions, which are likely to be responsible for phenotypic variation. Therefore, additional SNP discovery is necessary to identify and validate polymorphisms both in specific genes and genome-wide. Sequence-tagged sites within 286 genes were resequenced from a panel of animals representing a wide range of European cattle breeds. For 80 genes, no polymorphisms were identified, and 672 putative SNPs were identified within 206 genes. Fifteen European cattle breeds (436 individuals plus available parents) were genotyped with these putative SNPs, and 389 SNPs were confirmed to have minor allele frequencies above 10%. The genes containing SNPs were localized on chromosomes by radiation hybrid mapping and on the bovine genome sequence by Blast. Flanking microsatellite loci were identified, to facilitate the alignment of the genes containing the SNPs in relation to mapped quantitative trait loci. Of the 672 putative SNPs discovered in this work, only 11 were found among the validated SNPs and 100 were found among the approximately 2.3 million putative SNPs currently in dbSNP. The genes studied in this work could be considered as candidates for traits associated with beef production and the SNPs reported will help to assess the role of the genes in the genetic control of muscle development and meat quality. The allele frequency data presented allows the general utility of the SNPs to be assessed. [source]


Physiological changes in soybean (Glycine max) Wuyin9 in response to N and P nutrition

Summary Phosphorus deficiency is a very common problem in the acid soil of central China. Previous research has shown that starter N and N topdressing at the flowering stage (Rl) increased soybean (Glycine max) yield and N2 fixation (Gan et al, 1997, 2000). However, there is little information available concerning soybean response to P-fertiliser in soybean production in central China (Gan, 1999). A field experiment was conducted to investigate the response to P (0 kg P ha,1, 22 kg P ha,1, 44 kg P ha,1 before sowing) and N fertiliser application (N1: 0 kg N ha,1, N2: 25 kg N ha,1 before sowing, N3: N2 + 50 kg N ha,1 at the V2 stage and N4: N2 + 50 kg N ha,1 at the R1 stage) on growth, yield and N2 fixation of soybean. Both N and P fertiliser increased growth and seed yield of soybean (P < 0.01). Application of basal P fertiliser at 22 kg P ha,1 or 44 kg P ha,1 increased total N accumulation by 11% and 10% (P < 0.01) and seed yield by 12% and 13% (P < 0.01), respectively, compared to the zero P treatment. Although application of starter N at 25 kg N ha,1 had no positive effect on seed yield at any P level (P > 0.05), an application of a topdressing of 50 kg N ha,1 at the V2 or R1 stage increased total N accumulation by 11% and 14% (P < 0.01) and seed yield by 16% and 21% (P < 0.01), respectively, compared to the zero N treatment. Soybean plants were grown on sterilised Perlite in the greenhouse experiment to study the physiological response to different concentrations of phosphate (P1: 0 mM; P2: 0.05 mM; P3: 0.5 mM; P4:1.0 mN) and nitrate (N1: 0 mM with inoculation, N2: 20 mM with inoculation). The result confirmed that N and P nutrients both had positive effects on growth, nodulation and yield (P < 0.01). The relative importance of growth parameters that contributed to the larger biomass with N and P fertilisation was in decreasing order: (i) total leaf area, (ii) individual leaf area, (iii) shoot/root ratio, (iv) leaf area ratio and (v) specific leaf area. The yield increase at N and P supply was mainly associated with more seeds and a larger pod number per plant, which confirmed the result from the field experiment. [source]


Tropical spiny lobster (Panulirus ornatus) farming in Vietnam , bioeconomics and perceived constraints to development

Abstract This paper presents bioeconomic data on lobster farming in Vietnam, and perceived constraints to the development of the industry. The farms were found to be profitable, with an average benefit cost ratio of 1.44 and an average net revenue of 262 million VND year,1 (or just under US$15 000 year,1). Investment in the enterprise is high compared with other enterprises in the region. However, disease has the potential to devastate lobster crops and there is little information available to lobster farmers about disease prevention and management. Hence, the lobster enterprise is a high-risk high-return industry. The predominant perceived constraints to the development of lobster operations include water quality and temperature issues, insufficient access to credit, good-quality affordable feed and accurate information about technology improvements in lobster farming. It seems that improving the livelihood of lobster farmers in Vietnam is dependent on reducing their dependence on wild stocks for seed and feed, improving access to credit and improving technical and market information flows. Such improvements are likely to lead to higher profitability, given high export demand and hence sustained high prices for their lobster product. [source]
