Conceptual background, development, and preliminary data from the unified protocol for transdiagnostic treatment of emotional disorders,

Abstract Anxiety and mood disorders are common, chronic, costly, and characterized by high comorbidity. The development of cognitive behavioral approaches to treating anxiety and mood disorders has left us with highly efficacious treatments that are increasingly widely accepted. The proliferation of treatment manuals targeting single disorders, sometimes with trivial differences among them, leaves the mental health professional with no clear way to choose one manual over another and little chance of ever becoming familiar with most of them, let alone trained to competence in their delivery. Deepening understanding of the nature of emotional disorders reveals that commonalities in etiology and latent structures among these disorders supersedes differences. Based on empirical evidence from the domains of learning, emotional development and regulation, and cognitive science, we have distilled a set of psychological procedures that comprise a unified intervention for emotional disorders. The Unified Protocol (UP) is a transdiagnostic, emotion-focused cognitive behavioral treatment, which emphasizes the adaptive, functional nature of emotions, and seeks to identify and correct maladaptive attempts to regulate emotional experiences, thereby facilitating appropriate processing and extinction of excessive emotional responding to both internal (somatic) and external cues. The treatment components of the UP are briefly outlined. Theory and rationale supporting this new approach are described along with some preliminary evidence supporting its efficacy. Red blood cell quantification microfluidic chip using polyelectrolytic gel electrodes

Abstract This paper reports on a novel microfluidic chip with polyelectrolytic gel electrodes (PGEs) used to rapidly count the number of red blood cells (RBCs) in diluted whole blood. The proposed microdevice is based on the principle that the impedance across a microchannel between two PGEs varies sensitively as RBCs pass through it. The number and amplitude of impedance peaks provide the information about the number and size of RBCs, respectively. This system features a low-voltage dc detection method and non-contact condition between cells and metal electrodes. Major advantages include stable detection under varying cellular flow rate and position in the microchannel, little chance of cell damage due to high electric field gradient and no surface fouling of the metal electrodes. The performance of this PGEs-based system was evaluated in three steps. First, in order to observe the size-only dependence of the impedance signal, three different sizes of fluorescent microbeads (7.2, 10.0, and 15.0,,m; Bangs laboratories, USA) were used in the experiment. Second, the cell counting performance was evaluated by using 7.2,,m fluorescent microbeads, similar in size to RBCs, in various concentrations and comparing the results with an animal hematoanalyzer (MS 9-5; Melet schloesing laboratories, France). Finally, in human blood sample tests, intravenously collected whole blood was just diluted in a PBS without centrifuge or other pretreatments. The PGE-based system produced almost identical number of RBCs in over 800-fold diluted samples to the results from a commercialized human hematoanalyzer (HST-N402XE; Sysmex, Japan). [source]


Twelve immunotherapy drugs that could cure cancers

Summary: Immune T cells can kill cancer cells. Cancer vaccines function by increasing the number of immune T cells. There are exceedingly strict biologic limits imposed on the immune system to prevent excessive T-cell activation and expansion. The same biological restrictions limit cancer vaccines. Immunotherapeutic agents that circumvent the biological restrictions have been invented and formulated, including (i) dendritic cell activators and growth factors, (ii) vaccine adjuvants, (iii) T-cell stimulators and growth factors, (iv) immune checkpoint inhibitors, and (v) agents to neutralize or inhibit suppressive cells, cytokines, and enzymes. Few of these agents are broadly available for the development of effective multiple component regimens. The major problem facing immunotherapy today is a lack of broad availability of agents already in existence. The National Cancer Institute has developed a well-vetted ranked list of agents with high potential to serve as immunotherapeutic drugs. This review focuses on 12 of the agents, all with proven ability to augment T-cell responses. Alone, each has little chance of making substantial inroads into cancer therapy. In combinations dictated by biology, the agents are overwhelmingly likely to have an impact. Future availability of these agents for development of innovative combination cancer therapy regimens will provide a benchmark for the resolve of the national cancer therapy translational research enterprise. One of the most significant results of 9/11 has been to provoke the most serious crisis in the transatlantic relationship,the subject of Robert Kagan's influential and provocative treatise. Lauded by some as one of the more important contributions to the study of world politics in recent years and attacked by others as possibly the most misguided analysis of European,American relations ever, Kagan sets forth in stark, realist terms why the rift is serious, long-term and unlikely to be overcome by neat diplomatic footwork. However, as this commentary seeks to show, if Kagan is right there is little chance of constructing anything like a ,new world order'. Moreover, if the clash continues, far from enhancing American power in the world, it is more likely to weaken it. [source]


A practical large-strain solid finite element for sheet forming

Abstract An alternative approach for developing practical large-strain finite elements has been introduced and used to create a three-dimensional solid element that exhibits no locking or hourglassing, but which is more easily and reliably derived and implemented than typical reduced-integration schemes with hourglassing control. Typical large-strain elements for forming applications rely on reduced integration to remove locking modes that occur with the coarse meshes that are necessary for practical use. This procedure introduces spurious zero-energy deformation modes that lead to hourglassing, which in turn is controlled by complex implementations that involve lengthy derivations, knowledge of the material model, and/or undetermined parameters. Thus, for a new material or new computer program, implementation of such elements is a daunting task. Wang,Wagoner-3-dimensions (WW3D), a mixed, hexahedral, three-dimensional solid element, was derived from the standard linear brick element by ignoring the strain components corresponding to locking modes while maintaining full integration (8 Gauss points). Thus, WW3D is easily implemented for any material law, with little chance of programming error, starting from programming for a readily available linear brick element. Surprisingly, this approach and resulting element perform similarly or better than standard solid elements in a series of numerical tests appearing in the literature. The element was also tested successfully for an applied sheet-forming analysis problem. Many variations on the scheme are also possible for deriving special-purpose elements. Primetime Spin: Media Bias and Belief Confirming Information

This paper develops a model of media bias in which rational agents acquire all their news from the source that is most likely to confirm their prior beliefs. Despite only wishing to make the correct decision, agents act as if they enjoy receiving news that supports their preconceptions. By exclusively gathering information from a source biased towards his prior, there is little chance an agent will be persuaded to change his mind. Moreover, it is shown that even an unbiased agent prefers to receive biased news as it is unlikely to produce conflicting reports. The media caters to the informational demands of consumers and accordingly slants its reporting. It is shown that competition may not decrease bias, but may actually enhance it. Finally, even when it increases bias, competition may improve welfare by expanding the market for news. [source]


Chemotherapy with Cyclophosphamide, Vincristine, and Prednisolone (COP) in Cats with Malignant Lymphoma: New Results with an Old Protocol

This retrospective study in 61 cats with malignant lymphomas examined the efficacy of a well-established chemotherapy protocol (cyclophosphamide, vincristine, and prednisolone [COP]) in the Netherlands, a country with a low prevalence of feline leukemia virus (FeLV). Twenty-two cats (36.1%) had mediastinal lymphoma, 11 (18.0%) had alimentary lymphoma, 7 (11.5%) had peripheral lymphoma, 8 (13.1%) had nasal lymphoma, and 13 (21.3%) had miscellaneous lymphoma (including renal lymphoma in 2 [3.3%]). Of the 54 cats that were tested, only 4 (7.4%) were FeLV positive. Complete remission (CR) was achieved in 46 of the 61 cats (75.4%). The estimated 1- and 2-year disease-free periods (DFPs) in the 46 cats with CR were 51.4 and 37.8%, respectively, whereas the median duration of remission was 251 days. The overall estimated 1-year survival rate in all cats was 48.7%, and the 2-year survival rate was 39.9%, with a median survival of 266 days. The median survival time and the 1-year survival rate for mediastinal lymphoma were 262 days and 49.4%, respectively. Siamese cats had a more favorable prognosis for survival and remission than other breeds. Response to therapy in this study was shown to be a significant prognostic indicator. CR is necessary for long-term survival. Cats that did not achieve CR had little chance of survival for longer than 1 year. Young Siamese cats in this study had a greater tendency to develop mediastinal malignant lymphoma at a young age, and all were FeLV negative. In comparison with results reported in other studies with different combination chemotherapy protocols, these are among the highest percentages of remission and the longest survival rates for cats with malignant lymphoma. Patterns of helping effort in co-operatively breeding banded mongooses (Mungos mungo)

Abstract In most co-operative breeding species, some individuals contribute much more to helping behaviour than others. The most well-established explanation of such variation is based on kin selection and suggests that, in the absence of detectable differences in relatedness, individuals who suffer lower costs for a given level of help should contribute more. Differences in helping effort between dominance/sex categories were investigated in co-operatively breeding banded mongooses Mungos mungo in Uganda. The most conspicuous form of help in this species is provided by individuals who babysit offspring at the den while the rest of the pack goes off to forage. Across eight groups, the survival rate of pups increased with the average number of babysitters guarding them, consistent with the hypothesis that helpers benefit the brood that they guard. There was no difference between dominant males, subordinate males and breeding females in total contributions to babysitting. Subordinate males, however, contributed more to babysitting in the mornings, which were the longest and presumably the most energetically expensive sessions of the day. In six litters in one well-studied pack, dominant males and breeding females reduced their contribution to babysitting for the period that females were in oestrus. By contrast, subordinate males increased their contribution to become the main babysitters during this time. These results are consistent with the hypothesis that, where helping conflicts with breeding, individuals with little chance of direct reproduction can help at a lower fitness cost than those with a high probability of successful reproduction. The Korean Peninsula Peace Regime: How to Build It

A Korean peninsula peace regime has not yet been established, over a half century since the Korean armistice agreement. Two approaches to a Korean peninsula peace-regime-building process are required: inter-Korean and international approaches. The two Koreas should play leading roles at the inter-Korean and international levels in inter-Korean confidence-building measures by reducing tension through reconciliation and economic cooperation. A Korean peace forum involving the USA, China and the two Koreas under the 19 September joint agreement could conclude a Korean peninsula peace treaty to replace the armistice agreement and a peace regime could be institutionalized by implementing the inter-Korean basic agreement (1992) through inter-Korean cooperation. This article proposes a comprehensive, long-term roadmap for the Korean peninsula peace-regime-building process. The author makes three major arguments. First, the two Koreas and the four powers need to agree on a comprehensive roadmap for the Korean peninsula peace-regime-building process as suggested here. The inter-Korean and international approaches to the peace-regime-building process should be considered in parallel. Second, the North Korean nuclear issue should be resolved peacefully through the Six-Party Talks. Third, the two Koreas need to work together to find an alternative to their respective proposals. The author proposes a Korean peninsula peace treaty signed by the ROK, the DPRK, the USA and China as an alternative. Unless the two Koreas demonstrate their desire to cooperate through sincere deeds by implementing inter-Korean agreements and are willing to make concessions by working together for establishing a peace regime in Korea, there is little chance of achieving this goal. Interventions for treating traumatized permanent front teeth: avulsed (knocked out) and replanted

Background:, Dental trauma is common. One of the most severe injuries is when a permanent tooth is knocked completely out (avulsed) of the mouth. In most circumstances the tooth should be replanted as quickly as possible. There is uncertainty on how best to prepare teeth for replantation. Objectives:, To compare the effects of a range of interventions for managing traumatized permanent teeth with avulsion injuries. Search strategy:, The Cochrane Oral Health Group's Trials Register (to 28th October 2009); CENTRAL (The Cochrane Library 2009, Issue 4); MEDLINE (1950 to October 2009); EMBASE (1980 to October 2009); http://www.clinicaltrials.gov/;www.controlled-trials.com/ and reference lists of articles were searched. There were no language restrictions. Selection criteria:, Only randomized controlled trials (RCTs), that included a minimum follow-up period of 12 months, for interventions for avulsed and replanted permanent teeth were considered. Data collection and analysis:, Two review authors independently extracted data and assessed trial quality and the risk of bias in studies to be included. Main results:, Three studies, involving a total of 162 patients and 231 teeth were identified. Study one (with a high risk of bias) investigated the effect of extra-oral endodontics. This showed no significant difference in radiographic resorption compared with intra-oral endodontics provided at week 1 for teeth avulsed for longer than 60 minutes dry time. Study two (which had a moderate risk of bias) investigated a 10-minute soaking in thymosin alpha 1 prior to replantation and then its further use as a daily gingival injection for the first 7 days. They reported a strong benefit at 48 months (14% with periodontal healing in the control group versus 77% for the experimental group). Study three (with a high risk of bias) investigated a 20-minute soaking with gentamycin sulphate (4 × 107 U/L) for both groups prior to replantation and then the use of hyperbaric oxygen daily in the experimental group for 80 minutes for the first 10 days. They reported a strong benefit at 12 months (43% periodontal healing versus 88% for the experimental group). There was no formal reporting of adverse events. Authors' conclusions:, The available evidence suggests that extra-oral endodontics is not detrimental for teeth replanted after more than 60 minutes dry time. Studies with moderate/high risk of bias indicate that soaking in thymosin alpha 1 and gentamycin sulphate followed by hyperbaric oxygen may be advantageous. However, they have not previously been reported as interventions for avulsed teeth and need further validation. More evidence with low risk of bias is required and, with the low incidence of avulsed teeth, collaborative multicentre trials are indicated. Plain language summary:, Treatments for managing knocked out and replanted front teeth. Injuring your front teeth during childhood is common. One of the most severe injuries occurs when the tooth is knocked totally out of the mouth (avulsed). Often the best option is to replant the tooth as quickly as possible. This is true only for permanent teeth. Once replanted the tooth can heal in two ways if managed correctly. Ideally the ligament around the root reforms and the tooth can be expected to last as long as any other tooth; this is known as ,periodontal healing'. When there is too much damage to the ligament, healing occurs by bony replacement and the tooth is replaced by bone and lost over a few years. This is called ,bony healing'. Bony healing causes significant problems in the medium term for children and treatments for this are the subject of a different Cochrane review. A missing front upper tooth or teeth, as a result of not replanting an avulsed tooth or as a treatment for bony healing, can have a major effect on dental and facial ,good looks'. This can affect the individual's self-esteem and general social interaction, as well as how others think and see them. This Cochrane review investigated what treatments encourage the tooth to repair by periodontal healing. Three studies were found. The benefits of these treatments require further investigation before specific medicaments can be advised. This is because the studies had weakness in their design which may have influenced the benefits they found. The following general treatment principle can be concluded which reinforces current treatment guidelines: For teeth with little chance of periodontal healing, a root canal treatment can be carried out before the tooth is replanted without further detrimental effects.
