Selected AbstractsLimited Opportunities for Paramedic Student Endotracheal Intubation Training in the Operating RoomACADEMIC EMERGENCY MEDICINE, Issue 10 2006Bradford D. Johnston MD Abstract Background Paramedics, who often are the first to provide emergency care to critically ill patients, must be proficient in endotracheal intubation (ETI). Training in the controlled operating room (OR) setting is a common method for learning basic ETI technique. Objectives To determine the quantity and nature of OR ETI training currently provided to paramedic students. Methods The authors surveyed directors of paramedic training programs accredited by the Commission on Accreditation of Allied Health Education Programs. An anonymous 12-question, structured, closed-response survey instrument was used that requested information regarding the duration and nature of OR training provided to paramedic students. The results were analyzed by using descriptive statistics. Results From 192 programs, 161 completed surveys were received (response rate, 85%). OR training was used at 156 programs (97%) but generally was limited (median, 17,32 hours per student). Half of the programs provided fewer than 16 OR hours per student. Students attempted a limited number of OR ETI (median, 6,10 ETI). Most respondents (61%) reported competition from other health care students for OR ETI. Other identified hindering factors included the increasing OR use of laryngeal mask airways and physicians' medicolegal concerns. Respondents from 52 (33%) programs reported a recent reduction in OR access, and 56 (36%) programs expected future OR opportunities to decrease. Conclusions Despite its key role in airway management education, the quantity and nature of OR ETI training that is available to paramedic students is limited in comparison to that available to other ETI providers. Torres Craft enterprises are a unique form of small and medium-sized enterprise (SME) and represent an amalgam between the manufacture of products and the fulfilment of artistic vision. Craft enterprises typically have insufficient resources, inadequate access to specialised expertise and limited opportunities to make an impact on the marketplace. One way to redress these limitations and to improve the competitive position of craft enterprises is by forming networks, or cooperative alliances. The case study of ,Homethrown', a successful marketing network for potters, provides insight into the advantages and limitations of this type of strategic alliance, as well as guidelines for fostering cooperation and moderating conflict. Copyright © 2002 Henry Stewart Publications [source] The Reserve Capacity Model as a Framework for Understanding Psychosocial Factors in Health DisparitiesAPPLIED PSYCHOLOGY: HEALTH AND WELL-BEING, Issue 1 2009Linda C. Gallo Substantial research has identified a robust, linear association between socioeconomic status (SES) and health, across many different populations and outcomes. Among myriad influences, psychosocial factors may contribute to SES-related gradients in health. In the current manuscript, we describe the Reserve Capacity Model (Gallo & Matthews, 2003) as a potential framework through which to examine psychosocial pathways in health disparities. The model posits that SES is connected to health, in part through associations with stress and concomitant negative emotions, and their subsequent links with bio-behavioral functioning. In addition, low SES may predict enhanced emotional and physiological reactivity to stress due to a deficiency in resilient psychosocial resources (i.e. "reserve capacity") and limited opportunities to replenish resource reserves. In addition, resources might represent a direct mediational pathway from SES to health. We describe several studies in which we have examined various tenets of the Reserve Capacity Model, and comment on future research directions to better understand the roles of psychosocial variables in health disparities. [source] A Novel Approach to Teaching PeriocardiocentesisACADEMIC EMERGENCY MEDICINE, Issue 2009Mara Aloi Pericardiocentesis can be a life-saving intervention but it is associated with significant complications if not performed appropriately. Periocardiocentesis should ideally be done under ultrasound guidance. However, in many community emergency departments (EDs) ultrasound is not routinely available, leaving the ED physician the anatomic approach. Emergency medicine residents need to achieve competence in this procedure using both techniques, but there are limited opportunities for residents to perform this procedure on live patients. Many residency programs teach this procedure using cadaver models. We have devised a model, made with readily available, inexpensive materials, that allows residents to practice pericardiocentesis using both the subxiphoid and parasternal approach in a setting that we feel is more life-like than with that using the dessicated tissue of routine cadaver models. Materials: Two racks of pork or beef ribs, wire, pericardiocentesis tray, Betadine, plastic baggies. The racks of ribs are wired together to make a thorax and are then covered with an opaque covering to simulate skin. One plastic bag is filled with Betadine and then placed within another plastic bag containing water, to simulate pericardial effusion. The bag-within-bag set-up is then secured on the under surface of the thorax. Residents can then practice either the parasternal or subxiphoid approach with the intent of aspirating clear fluid. Aspiration of dark-fluid represents cardiac penetration and is considered an unsuccessful procedure. Bags can be replaced as needed, but based on our experience, approximately 10 attempts were done before any significant leakage occurred. Total cost for the materials was less than $35. [source] Influence of canopy tree size on stand basal area may reflect uncoupling of crown expansion and trunk diameter growthAUSTRAL ECOLOGY, Issue 2 2003Christopher H. Lusk Abstract A recent article by Midgley and colleagues suggests that large trees give rise to inordinately high stand basal areas because they pack canopy space more efficiently than smaller trees. We argue that this phenomenon bears more relation to the fact that diameter increment is not necessarily accompanied by significant crown expansion during all stages of a tree's life. Using data from a canopy tree population in an old-growth temperate forest, we found that crown area scaled as roughly the 3/5 power of trunk basal area. Rather than reflecting fixed scaling laws, we suggest that this pattern arises because of limited opportunities for crown expansion in dense stands. Old canopy trees in dense stands can thus accumulate large basal areas without occupying a commensurately large canopy area. A dynamic programming model of savings is presented in the current paper which examines optimal savings strategies for farmers, using a case study of integrated rice,shrimp farms in Vietnam. It is shown that when savings are accounted for, the expected utility ranking of different risky farm choices may not differ that much between farmers with different levels of risk aversion. [source] DOES MATE LIMITATION IN SELF-INCOMPATIBLE SPECIES PROMOTE THE EVOLUTION OF SELFING?EVOLUTION, Issue 6 2010THE CASE OF LEAVENWORTHIA ALABAMICA Genetic diversity at the S-locus controlling self-incompatibility (SI) is often high because of negative frequency-dependent selection. In species with highly patchy spatial distributions, genetic drift can overwhelm balancing selection and cause stochastic loss of S-alleles. Natural selection may favor the breakdown of SI in populations with few S-alleles because low S-allele diversity constrains the seed production of self-incompatible plants. We estimated S-allele diversity, effective population sizes, and migration rates in Leavenworthia alabamica, a self-incompatible mustard species restricted to discrete habitat patches in rocky glades. Patterns of polymorphism were investigated at the S-locus and 15 neutral microsatellites in three large and three small populations with 100-fold variation in glade size. Populations on larger glades maintained more S-alleles, but all populations were estimated to harbor at least 20 S-alleles, and mate availabilities typically exceeded 0.80, which is consistent with little mate limitation in nature. Estimates of the effective size (Ne) in each population ranged from 600 to 1600, and estimated rates of migration (m) ranged from 3 × 10,4 to nearly 1 × 10,3. According to theoretical models, there is limited opportunity for genetic drift to reduce S-allele diversity in populations with these attributes. Although pollinators or resources limit seed production in small glades, limited S-allele diversity does not appear to be a factor promoting the incipient breakdown of SI in populations of this species that were studied. Smith The extensive grasslands of Africa with rainfall <500 mm (the tsetse boundary) offer limited opportunity for agriculture, so are optimal for pastoral activities. With the possible exception of donkeys,1 no ungulates were independently domesticated in sub-Saharan Africa. While wild cattle were found in Holocene North Africa, there is some debate on whether these were independently domesticated. This article will introduce some of the arguments on the problems of the transition from hunting to herding ways of life in Africa, and the beginnings of domestication. [source] The evolutionary species pool hypothesis and patterns of freshwater diatom diversity along a pH gradientJOURNAL OF BIOGEOGRAPHY, Issue 3 2005Jason Pither Abstract Aim, To interpret the unimodal relationship between diatom species richness and lake pH within the context of the evolutionary species pool hypothesis (SPH). We test the following primary prediction arising from the SPH: the size of the potential species pool (PSP) will increase along a gradient representing the historical commonness of different pH environments (pH commonness). To do this we assume that the present-day spatial dominance of near-neutral pH conditions compared with acidic and alkaline conditions reliably mimics the relative spatial availabilities of historical pH conditions among freshwater lakes. We also determine whether local richness represents a constant proportion of PSP size along the pH commonness gradient. Location, Two hundred and thirty-four lakes distributed over a 405,000 km2 region of the north-eastern United States of America. Methods, Sediment diatom morphospecies lists and pH data were acquired from the US Environmental Protection Agency's Environmental Monitoring and Assessment Program (EMAP) website. Using 248 morphospecies that occurred in at least 10 of the 234 lakes, four different measures of PSPs were calculated along the pH gradient. Local species richness was equated with the number of species occurring within the lake. Alpha diversity was equated with the average species richness of lakes with similar pH values. A combination of statistical methods were employed, including correlations, quadratic regression and piecewise regression. Results, PSP size increased significantly with pH commonness for all four measures of PSP size, thus supporting the primary prediction of the evolutionary SPH. Local richness comprised a larger proportion of the PSP within acidic lakes than within circumneutral lakes. Alpha diversity and lake species richness both increased significantly with pH commonness, but the former did so in a two-step fashion. We test and reject several alternative contemporary time-scale explanations for our findings. Main Conclusions, Our findings are consistent with the hypothesis that diatom taxonomic richness is presently lower within acidic and highly alkaline lakes than in circumneutral lakes owing to the limited opportunity in space and/or time for the evolution of suitably adapted species. Whereas ecological processes can explain why certain species are excluded from particular habitats, e.g. acidic lakes, they cannot account for why so few species are adapted to those habitats in the first place.
