Cardiogenic Unilateral Pulmonary Edema: An Unreported Complication of a Digestive Endoscopic Procedure

Unilateral pulmonary edema is an uncommon clinical situation that may be difficult to distinguish from other conditions that cause lung infiltrates. Most cases occur in the right lung, and there are no reports about cardiogenic unilateral pulmonary edema as a complication of an endoscopic procedure of gastrointestinal tract. The authors describe a case of a 79-year-old woman with acute cardiac heart failure that developed soon after a diagnostic upper and lower digestive endoscopy. Continuous positive airway pressure, intravenous nitroglycerin, and furosemide treatment resulted in rapid improvement of symptoms and the progressive resolution of left-sided infiltrates on chest radiography. This case is of particular importance because of the rarity of cardiogenic unilateral edema in the left lung. This clinical finding was associated with the prolonged rest on the left side during the gastrointestinal endoscopic procedure. [source]


Lactic Acid Chemical Peels as a New Therapeutic Modality in Melasma in Comparison to Jessner's Solution Chemical Peels

BACKGROUND Many chemicals have been used in the skin peeling for melasma such as Jessner's solution and glycolic acid. Lactic acid is an ,-hydroxy acid that has not been used before in chemical peeling of melasma. OBJECTIVE The purpose of the present work was to evaluate the efficacy and safety of lactic acid in chemical peeling of melasma in comparison to Jessner's solution chemical peels. METHODS This study was conducted at the Department of Dermatology and Venereology, Baghdad Hospital, in the period between April 2001 and August 2002. Thirty patients with melasma were included in this study. They were mostly of skin type IV according to Fitzpatrick's classification, 26 (86.67%) were women, and 4 (13.33%) were men, with an age range from 18 and 50 years (mean±SD, 33.53±6.96 years). Full clinical examination was done to all patients including Wood's light. The severity of melasma was assessed by MASI (Melasma Area Severity Index). Pure lactic acid full strength (92%, pH 3.5) was used as a new peeling agent on the left side of the face while Jessner's solution was applied to the right side of the face. The chemical peeling sessions were done every 3 weeks until the desired response was achieved. Follow-up was carried out for 6 months after the last session. RESULTS Six patients were defaulted from the study after the first session for unknown reasons. Twenty-four patients completed the study. Twenty (83.33%) were women and four were men (16.67%). Wood's light examination showed increased contrast in all patients of mostly epidermal melasma. The number of sessions ranged from 2 to 5. All patients showed marked improvement as calculated by MASI score before and after treatment, and the response was highly statistically significant. No side effect was recorded in all treated patients. CONCLUSION Lactic acid was found to be an effective and safe peeling agent in the treatment of melasma, and it was as effective as Jessner's solution. [source]


Treatment of Inflammatory Facial Acne Vulgaris with Intense Pulsed Light and Short Contact of Topical 5-Aminolevulinic Acid: A Pilot Study

BACKGROUND Photodynamic therapy (PDT) with topical 5-aminolevulinic acid (ALA) and red light (550,700 nm) has been introduced for effective treatment of facial acne. Untoward side effects are common, however. OBJECTIVE To evaluate the efficacy and safety of the short contact of topical ALA and intense pulsed light (IPL) in treatment of inflammatory facial acne. METHODS Fourteen patients with inflammatory facial acne were treated with IPL on the left side and combination of IPL and topical ALA on the right side at 3- to 4-week intervals for three sessions. Clinical photographs and lesion counts were obtained for evaluation. RESULTS All patients revealed a reduction in number of acne lesions on both sides. On the ALA-pretreated side, lesion counts decreased 87.7% at 12 weeks after the last treatment (p<.01). Meanwhile, lesion counts on the nonpretreated side decreased 66.8% (p<.01). In addition, a number of lesion counts on the ALA-pretreated side decreased. Mild edema and minimal crust developed on the combined-treatment side. CONCLUSION Short contact of topical ALA and IPL or IPL alone showed some beneficial effect in treatment of inflammatory facial acne; however, degree of improvement was better and remained longer with the combined regimen. Morphogenesis of the node and notochord: The cellular basis for the establishment and maintenance of left,right asymmetry in the mouse

Abstract Establishment of left,right asymmetry in the mouse embryo depends on leftward laminar fluid flow in the node, which initiates a signaling cascade that is confined to the left side of the embryo. Leftward fluid flow depends on two cellular processes: motility of the cilia that generate the flow and morphogenesis of the node, the structure where the cilia reside. Here, we provide an overview of the current understanding and unresolved questions about the regulation of ciliary motility and node structure. Analysis of mouse mutants has shown that the motile cilia must have a specific structure and length, and that they must point posteriorly to generate the necessary leftward fluid flow. However, the precise structure of the motile cilia is not clear and the mechanisms that position cilia on node cells have not been defined. The mouse node is a teardrop-shaped pit at the distal tip of the early embryo, but the morphogenetic events that create the mature node from cells derived from the primitive streak are only beginning to be characterized. Recent live imaging experiments support earlier scanning electron microscopy (SEM) studies and show that node assembly is a multi-step process in which clusters of node precursors appear on the embryo surface as overlying endoderm cells are removed. We present additional SEM and confocal microscopy studies that help define the transition stages during node morphogenesis. After the initiation of left-sided signaling, the notochordal plate, which is contiguous with the node, generates a barrier at the embryonic midline that restricts the cascade of gene expression to the left side of the embryo. The field is now poised to dissect the genetic and cellular mechanisms that create and organize the specialized cells of the node and midline that are essential for left,right asymmetry. Mediolateral intercalation in planarians revealed by grafting experiments

Abstract We investigated how planarians organize their left,right axis by using ectopic grafting. Planarians have three body axes: anteroposterior (A-P), dorsoventral (D-V), and left,right (L-R). When a small piece is implanted into an ectopic region along the A-P and D-V axes, intercalary structures are always formed to compensate for positional gaps. There are two hypotheses regarding L-R axis formation in this organism: first, that the left and right sides of the animal may be recognized as different parts, and L-R intercalation can induce midline structures (asymmetry hypothesis); second, that both sides may have symmetrical positional values, and mediolateral (M-L) intercalation creates positional values along the L-R axis (symmetry hypothesis). We performed ectopic grafting experiments in the head region of the planarian, Dugesia japonica, to examine these hypotheses. A left lateral fragment containing a left auricle was implanted into the medial region of the host. Ectopic structures were always formed only on the left side of the graft, where lateral tissues abutted onto the medial tissues. However, no morphologic change was induced on the right side of the graft, where left-sided tissues faced onto right-sided tissues. Molecular marker analyses indicated that ectopic structures formed on the left side of the graft were induced by M-L intercalation, supporting the "symmetry hypothesis." When the midline tissues were implanted into a lateral region, they induced a complete ectopic head, demonstrating that M-L intercalation may be sufficient to establish the L-R axis in planarians. The emergence of the chordate body plan: some puzzles and problems

The emergence of the chordate body plan: some puzzles and problems. ,Acta Zoologica (Stockholm) 91: 4,10 Rather than being sessile filter feeders, ancestral chordates are now thought to have evolved from more active benthic animals, possibly hemichordate-like, that took to swimming, to generate something resembling modern amphioxus. This general picture conceals a number of specific problems that underline how little we understand the transition in detail. I will address three. First, and closest to resolution is the issue of dorsoventral inversion, which has implications for understanding how an internalized brain evolved. This is because the mouth, dorsal after inversion, has first to be moved out of the way. Its migration down the left side of the head during amphioxus development may be a recapitulation of this event. Two other puzzles, both further from resolution are: (1) the significance, if any, of the neurenteric canal, which may be telling us something important about the true nature of deuterostomy, specifically whether hemichordates and echinoderms are deuterostomes for a different reason than chordates, and (2) whether the functional digestive tract of chordates is a secondary replacement of an earlier structure whose fate remains unexplained. Resolving these latter two issues will require a better understanding of molecular level events during development in protochordates and their immediate invertebrate relatives. [source]


The magnocellular theory of developmental dyslexia

Abstract Low literacy is termed ,developmental dyslexia' when reading is significantly behind that expected from the intelligence quotient (IQ) in the presence of other symptoms,incoordination, left,right confusions, poor sequencing,that characterize it as a neurological syndrome. 5,10% of children, particularly boys, are found to be dyslexic. Reading requires the acquisition of good orthographic skills for recognising the visual form of words which allows one to access their meaning directly. It also requires the development of good phonological skills for sounding out unfamiliar words using knowledge of letter sound conversion rules. In the dyslexic brain, temporoparietal language areas on the two sides are symmetrical without the normal left-sided advantage. Also brain ,warts' (ectopias) are found, particularly clustered round the left temporoparietal language areas. The visual magnocellular system is responsible for timing visual events when reading. It therefore signals any visual motion that occurs if unintended movements lead to images moving off the fovea (,retinal slip'). These signals are then used to bring the eyes back on target. Thus, sensitivity to visual motion seems to help determine how well orthographic skill can develop in both good and bad readers. In dyslexics, the development of the visual magnocellular system is impaired: development of the magnocellular layers of the dyslexic lateral geniculate nucleus (LGN) is abnormal; their motion sensitivity is reduced; many dyslexics show unsteady binocular fixation; hence poor visual localization, particularly on the left side (left neglect). Dyslexics' binocular instability and visual perceptual instability, therefore, can cause the letters they are trying to read to appear to move around and cross over each other. Hence, blanking one eye (monocular occlusion) can improve reading. Thus, good magnocellular function is essential for high motion sensitivity and stable binocular fixation, hence proper development of orthographic skills. Many dyslexics also have auditory/phonological problems. Distinguishing letter sounds depends on picking up the changes in sound frequency and amplitude that characterize them. Thus, high frequency (FM) and amplitude modulation (AM) sensitivity helps the development of good phonological skill, and low sensitivity impedes the acquisition of these skills. Thus dyslexics' sensitivity to FM and AM is significantly lower than that of good readers and this explains their problems with phonology. The cerebellum is the head ganglion of magnocellular systems; it contributes to binocular fixation and to inner speech for sounding out words, and it is clearly defective in dyslexics. Thus, there is evidence that most reading problems have a fundamental sensorimotor cause. But why do magnocellular systems fail to develop properly? There is a clear genetic basis for impaired development of magnocells throughout the brain. The best understood linkage is to the region of the Major Histocompatibility Complex (MHC) Class 1 on the short arm of chromosome 6 which helps to control the production of antibodies. The development of magnocells may be impaired by autoantibodies affecting the developing brain. Magnocells also need high amounts of polyunsaturated fatty acids to preserve the membrane flexibility that permits the rapid conformational changes of channel proteins which underlie their transient sensitivity. But the genes that underlie magnocellular weakness would not be so common unless there were compensating advantages to dyslexia. In developmental dyslexics there may be heightened development of parvocellular systems that underlie their holistic, artistic, ,seeing the whole picture' and entrepreneurial talents. Minireview: On the homology of the protocoel in Cephalochordata and ,lower' Deuterostomia

Abstract Hypotheses regarding the homology of the protocoel in planktonic deuterostome larvae and mesodermal structures in ontogenetic stages of cephalochordates are evaluated. The prevalent ,classical' hypothesis describes the protocoel as being homologous with the diverticula of Hatschek, which, on the left side, develop into the preoral pit, subsequently into Hatschek's pit and groove (in part). This hypothesis is based mainly on the position of Hatschek's diverticula anterior to the rest of the mesoderm during their enterocoelic origin. It is shown here that during development the mesodermal segment that develops into Hatschek's nephridium is the most anterior one prior to formation of Hatschek's diverticula, and this segment assumes an anteriormost position after differentiation of Hatschek's diverticula. Additional similarities between this segment and protocoels are: (i) presence of endomesodermal cells with podocytic extensions, (ii) excretory function, (iii) relatively early ontogenetic origin, (iv) probable lack of association with nervous structures, (v) probable ectodermal origin of a portion of the canal, and (vi) position relative to the mouth opening. Therefore, homology between the protocoel and the segment that becomes Hatschek's nephridium is proposed. It is concluded that a glandular structure homologous to the diverticula of Hatschek and anterior to the protocoel/Hatschek's nephridium is a synapomorphy of notochordates or chordates. [source]


Evidence of Robust Coupling of Atrioventricular Mechanical Function of the Right Side of the Heart: Insights from M-Mode Analysis of Annular Motion

Background: Extensive data exist regarding annular descent and ventricular function. We have already demonstrated significant differences in amplitude and timing of events between maximal mitral (MAPSE) and tricuspid (TAPSE) annular plane systolic excursion as well as described quantitative temporal differences in annular ascent (AA) between the right and left sides of the heart. However, whether any relationship exists between annular ascent and descent components remains uninvestigated. Methods: Left ventricular ejection fraction (LVEF), right ventricular fractional area change (RVFAC), MAPSE, TAPSE, MV, and TV AA as well as pulsed tissue Doppler of the lateral MV and TV annuli were recorded from 53 patients. Results: In this population (age 55 ± 17 years) mean LVEF was 55 ± 19%, mean RVFAC was 47 ± 20%, mean MAPSE was 2.11 ± 0.72 cm, mean TAPSE was 1.48 ± 0.44 cm, mean MV AA was 0.52 ± 0.17 cm, TV AA was 0.96 ± 0.47, MV A-wave 0.10 ± 0.04 cm/s, and TV A-wave was 0.13 ± 0.05 cm/s. A more robust correlation was seen between TV AA and RVFAC than between MV AA and LVEF and also between TV AA and pulsed TDI TV A-wave velocity than between MV AA and pulsed TDI MV A-wave. Conclusion: Our data reveal that mechanical systolic functions of the atria and the ventricles are more closely coupled on the right than on the left side of the heart. Whether this is a result of anatomic linking or chamber geometry will require further study. [source]


Systematic and morphological studies of the genus Chaetopleurophora Schmitz (Diptera: Phoridae) occurring in Japan

Abstract Japanese species of the genus Chaetopleurophora are reviewed. All belong to the C. erythronota group. The following three species from Japan are described: C. rhomboidea sp. nov., C. pygidialis Schmitz and C. dividua sp. nov. The male and female genitalia are studied and further examples of unique characters of the genus including asymmetric features are added. The male aedeagus of the genus is illustrated for the first time. The aedeagus of the species treated in this study consists of only two components, the inner core plate and the outer jacket plate. The jacket plate wraps sinistrally around the core plate. The combination of the core plate and the jacket plate forms complex, asymmetric features of the aedeagus in the Phoridae. The structure around the genital opening in the female genitalia protrudes posteriorly under the segment IX + X, and shows asymmetric features in C. rhomboidea sp. nov. with a bilaterally different degree of sclerotization, shifted genital opening to the right side and a membranous ribbon just on the left side. In addition, C. dividua sp. nov. is different from most of the species in the C. erythronota group, and very closely related to C. multiseriata (known in North America) in the male and female genitalia, wing venation and bristle formation on the scutellum. It is suggested that C. dividua sp. nov. forms a monophyletic group with C. multiseriata and the related species. [source]


Penetrating injury at the thoracic inlet in a Paint-Arab mare

Summary A 12-year-old Paint-Arab mare was admitted for evaluation of a penetrating chest laceration at the thoracic inlet. The left brachiocephalic muscle was transected and the recurrent laryngeal nerve was traumatised. Subsequent to the injury, the horse developed Horner's syndrome on the left side of the neck and face, Grade IV left laryngeal hemiplegia, dysphagia, cough and subcutaneous emphysema. The defect was closed in multiple layers. Antimicrobial and antiinflammatory therapy was instituted along with local wound care. The mare remained bright and responsive and the wound healed normally. The mare showed no signs of respiratory distress. Dysphagia and ptosis persisted at 30 days post trauma. [source]


Lesion of the anterior branch of axillary nerve in a patient with hereditary neuropathy with liability to pressure palsies

We report the case of a 30-year-old woman affected by hereditary neuropathy with liability to pressure palsies (HNPP), who developed a painless left axillary neuropathy after sleeping on her left side, on a firm orthopaedic mattress, in her eighth month of pregnancy. Electromyography (EMG) showing neurogenic signs in the left anterior and middle deltoid, and normal findings in the left teres minor, posterior deltoid and other proximal upper limb muscles, demonstrated that the lesion was at the level of the axillary anterior branch. A direct compression of this branch against the surgical neck of the humerus seems the most likely pathogenic mechanism. This is the first documented description of an axillary neuropathy in HNPP. Knowledge of its possible occurrence may be important for prevention purposes. [source]


Ulnar neuropathy at the elbow due to unusual sleep position

Abnormal strain of the ulnar nerve over the sulcus due to an unusual sleep position is a rare cause of ulnar neuropathy at the elbow. A 57-year-old patient with Mandelung's deformity developed progressive weakness in the flexion of fingers 4 and 5 and in finger straddling on the left side. Additionally, there was slight wasting of the left hypothenar and the left interossei muscles. Motor and sensory nerve conduction studies of the left ulnar nerve showed delayed conduction velocities over the left ulnar sulcus. He preferred to sleep in a left lateral position with his head lying on a headrest roll, his left forearm being flexed at 110° and his hand lying either under his cheek or placed on the roll. Only three weeks after the patient had been advised to change his sleep position and to sleep without the headrest roll, weakness markedly improved. This case shows that sleeping in a lateral position with the head on a headrest roll and the hand placed on the roll or under the cheek may cause ulnar neuropathy at the elbow. Change of such a habitual sleep position promptly resolves the symptoms. [source]


Axillary intertriginous granular parakeratosis responsive to topical calcipotriene and ammonium lactate

A 70-year-old white man presented to our clinic with a 6,8-month history of a pruritic, occasionally burning eruption in both axillae. He had been using the same deodorant for more than 1 year and denied any changes in laundry detergent, soaps, or shampoos. He also denied application of other topical products. On physical examination, there were slightly erythematous, lichenified plaques in both axillae, with more extensive involvement of the left side (Fig. 1). Histologic examination of a punch biopsy specimen from a left axillary plaque revealed hyperkeratosis with retention of nuclei and keratohyaline granules in the stratum corneum (Fig. 2). The stratum granulosum was slightly thickened, and the epidermis was mildly acanthotic. Patch tests of the patient's deodorant and shampoo were negative. The patient was advised to discontinue use of his deodorant. His right axilla was treated with topical calcipotriene, applied twice daily, and his left axilla was treated with topical 12% ammonium lactate, applied twice daily. One month later, the lesions in the right axilla had completely resolved. The left axilla was slightly improved, but still exhibited dusky erythematous plaques. After one additional month of treatment with ammonium lactate, the left axillary lesions completely resolved. S-100-negative atypical granular cell tumor: report of a case

A 38-year-old man presented with a solitary, round, 1.2 × 1.2 cm, bluish-colored, dome-shaped, hard nodule on the left side of the neck, which had grown over 2 months (Fig. 1). The nodule was nontender and nonmovable. Light microscopy revealed that the neoplasm was situated in the reticular dermis with extension into the papillary dermis. The tumor showed expansile growth with smooth and round borders, and was made up of sheets of cells arranged in nests or lobules separated by thin delicate connective tissue septa. The tumor cells were round, oval, or polygonal in shape with distinct cellular borders. The cells had abundant eosinophilic granular cytoplasm, and considerable variation of cellular and nuclear size was noted (Fig. 2a). The tumor cell nuclei were vesicular and some had pleomorphism (Fig. 2b). Sometimes multiple nucleoli were seen. Mitoses and necrosis were virtually absent. Immunohistochemical staining revealed that some of the cytoplasmic granules stained positively with periodic acid,Schiff (PAS) after diastase treatment. Tumor cells showed strong reactivity for CD68 and neuron-specific enolase, and negative results for S-100, factor XIIIa, cytokeratin, desmin, CD34, and smooth muscle actin. Electron microscopy revealed that the tumor was composed of polygonal cells with round to irregular nuclei, and the cytoplasm contained numerous secondary lysosomes. The tumor was completely excised. Limb bones asymmetry and stress in medieval and recent populations of Central Europe

Abstract Monitoring the degree of asymmetry in different parts of the human body can contribute to population studies, as it may be connected indirectly with the social structure, living conditions, and also with biomechanical stress affecting the person. Analysis of asymmetry may also assess preferential use of the right or left of the body during specific activities. This study is based on the measurements of bones of the upper and lower limbs of skeletons derived from the remarkable medieval cemeteries of Mikul,ice-Kostelisko (78 male, 132 female) and Pru,ánky (66 male, 69 female) (9th,12th centuries AD), and a series of skeletons representing a recent population from Bohemia (143 male, 157 female). The objective was to assess directional asymmetry (DA), fluctuating asymmetry (FA) and antisymmetry (AS) of the dimensions of the evaluated bones, and to use these data to compare the characteristics of the medieval and recent populations. DA was recorded in most dimensions. In the upper limb, the humerus exhibited the greatest expression of asymmetry, and, with the exception of the clavicle, DA was always more pronounced on the right side. Conversely, DA was less prevalent in the lower limb bones. It was more pronounced on the transverse, sagittal and circumferential dimensions of the diaphyses and epiphyses than on the length, and in most cases it was on the left side. The FA values were very low, and almost negligible in relation to the size. Nevertheless, FA was markedly more frequent on the lower than on the upper limb. In contrast to the medieval population, the recent population had higher FA and DA values. Craniofacial trauma in the Prehispanic Canary Islands

Abstract The current project is a study of craniofacial trauma in a large sample (n,=,896) of Prehispanic Canary Islanders (PCIs). The possible causes and social implications of the trauma found are considered, with reference to archaeological and historical data. Variables include the island, period and ecology, the sex and age of the individuals, the distribution of lesions across the skull (by side and by individual bone) and ante-mortem tooth loss. The results show a fairly high trauma rate (16%), a low prevalence of peri-mortem trauma (3.8% of all lesions), higher prevalence of trauma in males than in females (25% vs. 13% of all individuals), more cranial than facial lesions (8.9% vs. 3.5% of all elements) and more lesions on the left side of the skull (6.7% vs. 4.5% of all elements) which suggests that the lesions were sustained through intentional rather than accidental agency. There was no correspondence between trauma prevalence and ecology. The archaeological and historical data support the assertion that the lesions may be the result of skirmishing between groups, using weapons such as slingshots, stones and staves. A case study of possible differential diagnoses of a medieval skeleton from Denmark: leprosy, ergotism, treponematosis, sarcoidosis or smallpox?

Abstract This paper uses macroscopic and radiological examinations to provide differential diagnoses of pathological lesions in the skeleton of a young woman, 20,25 years of age, which triggered the Danish palaeopathologist Vilhelm Møller-Christensen's interest in leprosy. The skeleton was incomplete, but the majority of bones of the upper body, as well as the skull, were present. The pathological changes consisted of medullary and cortical lytic foci, periosteal reaction and enhanced cortical density. The lesions were most extensive on the left side, especially around the elbow, wrist and scapula. Treponematosis, leprosy, smallpox, ergotism, rheumatoid arthritis, tuberculosis and sarcoidosis are all reviewed with regard to bone and joint pathology and their likelihood of being the correct diagnosis. We concluded that the most plausible diagnosis is treponematosis, but neither sarcoidosis nor smallpox can be completely excluded. Copyright © 2007 John Wiley & Sons, Ltd. [source]


Lingual cortical mandibular defects (Stafne's defect): an anthropological approach based on prehistoric skeletons from the Canary Islands

Abstract While posterior lingual mandibular depressions (Stafne's defect) are often discussed in clinical reports, they are rarely the subject of anthropological research. This situation is paradoxical since osteologists and skeletal biologists are in a position to enhance understanding of the trait's aetiology by systematically recording the trait in recent and prehistoric skeletal collections. This report reviews anthropological studies of cortical defects of the mandible, recommends a protocol for recording observations in trait variation, and presents new data for the prevalence of Stafne's defect,lingual cortical defects of the mandibular corpus. Among the Guanches of Tenerife in the Canary Islands (Spain), the prevalence of lingual cortical defects is 3.32% (15/452), males are more frequently affected than females, and there is a tendency for individuals with antemortem tooth loss to display larger lesions than individuals without antemortem loss of teeth. Defects of the left side are somewhat more variable in position than defects located on the right. In comparative perspective, prevalence of lingual cortical defects among the Guanches is high, given the overall 1.07% prevalence reported for archaeological series (Finnegan & Marcsik,1980), but is similar to figures reported for the Avar period sample from Hungary 3.73%. Prospects for the use of lingual cortical defects as non-metric traits of value in population distance studies remain uncertain since variation in trait expression may have a high environmental component. However, if human osteologists routinely include observations of lingual mandibular cortical defects in their analysis of skeletal collections, the data required to elucidate factors responsible for the trait's cultural, ecological, temporal and geographical patterning will more rapidly become established. Hand-assisted laparoscopic and open living donor nephrectomy in Korea

Abstract Background: We compared the results of hand-assisted laparoscopic living donor nephrectomy (LLDN) and conventional open living donor nephrectomy (OLDN). Methods: The clinical data on 49 hand-assisted LLDN and 21 OLDN on the left side performed at two institutions in Korea from January 2001 to February 2003 were reviewed. Demographic data of donors and recipients were similar in the two groups. Results: There was one conversion to an open procedure due to bleeding in the LLDN group. The median operation times (180 min in LLDN versus 170 min in OLDN) and warm ischemic times (2.5 min in LLDN versus 2.0 min in OLDN) in the two groups were similar. The estimated mean blood loss, duration of hospital stay and complication rate was also similar in the two groups. The LLDN group reported less pain (visual analog scale) postoperatively (4.1 versus 5.3), but this was not significant (P = 0.058). The time to oral intake in the LLDN group was significantly longer by an average of 1 day (P = 0.001). Return to work was sooner in the LLDN group (4.0 weeks versus 6.0 weeks; P = 0.026). The recipient graft function was equivalent between the two groups. Hand-assisted LLDN appears to be a safe and effective alternative to OLDN. Conclusion: Our findings suggest that this technique may give the ability provide grafts of similar quality to OLDN, while extending to the donors the advantages of a traditional LLDN procedure. Accurate motion-produced distance and direction under systematically distorted perception

Abstract:, We report the visually directed actions of soccer players. After perceiving the location of a target on their left side at the starting point and traveling toward the ball without seeing the target, the players could kick the ball accurately (Experiment 2). In contrast, if they were verbally asked the direction of the target in a similar situation, the perceived direction was systematically distorted (Experiment 3). Our major concern in explaining the distorted perception was whether the egocentric distance before locomotion was perceived accurately or not, and whether the updating of the target location during locomotion was accurate or not. Combining these two possibilities, there should be four hypotheses, each of which assumes either: (1) accurate egocentric distance and accurate updating, (2) inaccurate egocentric distance and accurate updating, (3) accurate egocentric distance and inaccurate updating, or (4) inaccurate egocentric distance and inaccurate updating. Based on these hypotheses, we conducted four simulations, which revealed that the combination of the perception of the accurate egocentric distance and the distorted updating that substituted the constant function for the sine function produced not only a good r2, but also three kinds of interactions obtained in Experiment 3. Why did the players, based on their distorted perception, perform accurately? We would like to suggest that through perceptual learning they might acquire a perceptual-motor relation as the inverse function of the physical-perceptual relation. [source]


Repigmentation of pretibial vitiligo with calcineurin inhibitors under occlusion

Summary Treatment of vitiligo is a challenge, especially in children. Recently, topical calcineurin inhibitors have been introduced in the management of vitiligo, but significant repigmentation is not achieved except on the face. Large pretibial lesions of a 15-year-old female with progressive vitiligo were treated twice daily over six months with 0.1% tacrolimus ointment on the right and 1% pimecrolimus cream on the left side without effect. Additional overnight occlusion with polyurethane and hydrocolloid foils during the following 18 months led to substantial repigmentation on both sides (tacrolimus-treated side, 88% repigmented area; pimecrolimus-treated side, 73%). Tacrolimus serum levels measured at four different time points did not exceed 1.8 ng/ml. This case report on a direct comparison of topical tacrolimus and pimecrolimus in vitiligo shows that on the shins considerable improvement could be induced with both agents only by additional long-term occlusion and that tacrolimus was somewhat more effective than pimecrolimus. Allometry, bilateral asymmetry and sexual differences in the vocal tract of common eiders Somateria mollissima and king eiders S. spectabilis

Intraspecific sexual differences, high variation, and positive allometry of sexually-selected external display structures are common. Many sexually-selected anatomical specializations occur in the avian vocal tract but intraspecific variation and allometry have been investigated little. The tracheal bulla bulla syringealis occurs in males of most duck species. We quantified variation and size-scaling of the bulla, plus sexual differences in size of trachea, bronchi, and vocal muscles, for 62 common eiders Somateria mollissima and 51 king eiders S. spectabilis. Trends were similar in both species. Bullar ossification and definitive size occurred early in life: bullar size did not differ between first-year and older males. Bullar size did not vary more than size of other body parts (CVs of 3.4,7.0% for bullar length and breadth). Bullar size scaled to body size with negative allometry or isometry. Vocal muscles were 10,50% thicker in males than females, a much greater sexual difference than in body size (CVs of 3,6% on linear body-size variables). Vocal muscles were larger on the left side in both sexes and bilateral asymmetry was slightly more pronounced in males. Low variation and a trend towards negative allometry suggest that bullar size is under stabilizing selection; if bullar size affects vocal attributes of voice, then the latter cannot be condition-dependent. We recommend comparative research on vocal communication, vocal individuality and vocal-tract anatomy and function in eiders and other ducks. [source]


Behavioral and morphological asymmetries in chukar Alectoris chukar copulation

Birds often exhibit greater reproductive tract development on the left side than right side. Behavioral evidence from the three species for which data has been published indicates that these species copulate more frequently on the left side of females than on the right side. Missing from the literature are studies that compare asymmetry in copulation behavior to asymmetry in reproductive tract morphology of the same individuals of both sexes within a single species. To better understand the potential for cryptic sexual selection to influence avian copulation, we measured asymmetries in chukar Alectoris chukar copulation using 24 male and 29 female chukar brought into captivity from the wild. Chukar copulated (n=37) more from the left side (n=30) of females than the right side (n=7). The left testis of males was consistently greater in size, mass and volume than the right testis. The left ovary and oviduct of females was consistently functional with no observable development of the right ovary or oviduct. Left-side bias in direction of copulation, larger left testes, and functional left vaginal openings may act in concert to deliver spermatozoa to the oviduct, promoting fertilization. [source]


Effect of enamel matrix proteins (Emdogain®) on healing after re-implantation of "periodontally compromised" roots

Abstract Objective: The present experiment was performed to assess whether Emdogain® applied on the root surface of extracted teeth or teeth previously exposed to root planning can protect the tooth from ankylosis following re-implantation. Material and Methods: The experiment included two groups of dogs, including five animals each. The root canals of all mandibular third premolars (3 P 3) were reamed and filled with gutta-percha. A crestal incision was placed from the area of the second to the fourth premolar. Buccal and lingual full thickness flaps were elevated. With the use of a fissure bur, the crown and furcation area of 3 P 3 were severed in an apico-coronal cut. The distal and mesial tooth segments were luxated with an elevator and extracted with forceps. Group A: The mesial and distal segments of 3 P 3 were air dried on a glass surface for 60 min. The roots from the right side were conditioned and exposed to Emdogain® application. The roots from the left side received the same treatment with the exception of Emdogain® application. The mesial and distal tooth segments were re-implanted and the crown portions were severed with a horizontal cut and removed. The buccal and lingual flaps were mobilized and sutured to obtain complete coverage of the submerged roots. Group B: A notch was prepared in each root, 4,5 mm apical of the cemento-enamel junction. The area of the root that was located coronal to the notch was scaled and planned. The roots in the right side of the mandible were treated with Emdogain®, while the roots in the left side served as controls. After 6 months of healing, the dogs were killed and blocks containing one root with surrounding tissues were harvested, and prepared for histological examination, which also included morphometric assessments. Thus, the proportions of the roots that exhibited signs of (i) replacement (ii) inflammatory and (iii) surface resorption were calculated. Results and Conclusion: It was demonstrated that healing of a re-implanted root that had been extracted and deprived of vital cementoblasts was characterized by processes that included root resorption, ankylosis and new attachment formation. It was also demonstrated that Emdogain® treatment, i.e. conditioning with EDTA and placement of enamel matrix proteins on the detached root surface, failed to interfere with the healing process. Zusammenfassung Zielsetzung: Untersuchung, ob Emdogain®, wenn es auf die Wurzeloberfläche extrahierter Zähne oder von Zähnen, die zuvor eine Wurzelglättung bekommen haben, appliziert wird, die Zähne nach Reimplantation vor Ankylose schützen kann. Material und Methoden: Die Studie wurde bei 2 Gruppen von Hunden durchgeführt, die je 5 Tiere umfasste. Die Wurzelkanäle aller 3. Prämolaren des Unterkiefers (3 P 3) wurden aufbereitet und mit Guttapercha gefüllt. Ein Schnitt auf dem Limbus alveolaris wurde vom 2. zum 4 Prämolaren geführt. Bukkal und lingual wurde ein Vollschichtlappen mobilisiert. Mit einem Fissurenbohrer wurden die 3 P 3 mit einem Schnitt in koronoapikaler Richtung im Bereich der Krone und der Furkation geteilt. Die distalen und mesialen Zahnsegmente wurden mit einem Elevator luxiert und mit einer Zange extrahiert. Gruppe A: Die mesialen und distalen Segmente von 3 P 3 wurden auf einer Glasoberfläche 60 min lang luftgetrocknet. Die Wurzeln der rechten Seite wurden konditioniert und mit Emdogain® beschickt. Die Wurzeln der linken Seite erhielten die gleiche Behandlung mit der Ausnahme, dass keine Applikation von Emdogain® erfolgte. Die mesialen und distalen Wurzeln wurden reimplantiert und die Kronenanteile durch einen horizontalen Schnitt getrennt und entfernt. Die bukkalen und lingualen Lappen wurden mobilisiert und durch Naht ein vollständiger Verschluss der reimplantierten Wurzeln erreicht. Gruppe B: In jede Wurzel wurde 4,5 mm apikal der Schmelz-Zement-Grenze eine Kerbe präpariert. Der Bereich der Wurzel, der koronal dieser Kerbe lag, wurde gescalt und wurzelgeglättet. Die Wurzeln der rechten Unterkieferseite wurden mit Emdogain® behandelt, während die Wurzeln der linken Seite als Kontrolle dienten. Nach einer Heilung von 6 Monaten wurden die Hunde getötet und Blöcke, die eine Wurzel und das umgebende Gewebe enthielten, gewonnen und für die histologische Untersuchung präpariert, die auch morphometrische Befunde einschloss. Es wurden also die Anteile der Wurzeln berechnet, die Zeichen von (i) Ersatz- (ii) entzündlicher und (iii) Oberflächenresorption zeigten. Ergebnisse und Schlussfolgerungen: Es wurde gezeigt, dass die Heilung von reimplantierten Wurzeln, die extrahiert und von vitalen Zementoblasten befreit worden waren, durch Prozesse charakterisiert war, die Wurzelresorption, Ankylose und die Bildung neuen Attachments umfassten. Es wurde gezeigt, dass die Behandlung mit Emdogain®, d.h. Konditionierung mit EDTA und Applikation des Schmelz-Matrix-Proteins auf die freie Wurzeloberfläche diesen Heilungsprozess nicht beeinflussen konnte. Résumé Objectif: Cette expérimentation fut réalisée pour déterminer si Emdogain® appliqué sur la surface radiculaire de dents extraites ou de dents préalablement soumises à un surfaçage radiculaire pouvait protéger la dent de l'ankylose après réimplantation. Matériel et Méthodes: L'expérience comprenait 2 groupes de 5 chiens. Les canaux radiculaires de toutes les troisièmes premolaires mandibulaires (3 P 3) furent alésés et bouchés à la gutta-percha. Une incision crestale de la deuxième à la quatrième prémolaire permit de soulever un lambeau de pleine épaisseur vestibulaire et lingual. La couronne et la zone de furcation de 3 P 3 furent découpées à l'aide d'une fraise fissure d'apical en coronaire. Les segments distaux et mésiaux furent luxés avec un élévateur et extraits avec un davier. Groupe A: Les segments mésiaux et distaux de 3 P 3 furent séchés à l'air sur une plaque de verre pendant 60 min. Les racines du coté droit furent préparées et imprégnées d' Emdogain®. Les racines gauches reçurent le même traitement sans application d'Emdogain ®. Les segments mésiaux et distaux furent alors réimplantés et les couronnes découpées par un trait horizontal et éliminées. Les lambeaux vestibulaires et linguaux furent déplacés et suturés pour obtenir un recouvrement complet des racines enfouies. Groupe B: Une entaille a été préparée sur chaque racine, à 4,5 mm en apical de la jonction amélo-cémentaire. La surface de racine située coronairement à cette entaille fut alors détartrée et surfacée. Les racines du coté droit furent traitées par Emdogain® alors que les racines du coté gauche firent office de contrôle. Après 6 mois de cicatrisation, les chiens furent sacrifiés et des blocs contenant une racine et les tissus environnant furent prélevés pour un examen histologique et morphométrique. Ainsi, les proportions de racine présentant des signes de (i) remplacement (ii) d'inflammation et (iii) de résorption furent calculées. Résultats et conclusion: Nous avons démontré que la cicatrisation de racine réimplantées après extraction et élimination des cémentoblastes se caractérisait par un processus qui comprenait résorption radiculaire, ankylose et formation d'une nouvelle attache. Nous avons aussi démontré que le traitement par Emdogain®, c'est à dire conditionnement à l'EDTA et mise en place de protéines de la matrice améllaire sur la surface radiculaire, ne pouvait pas interférer avec le processus de cicatrisation. Lateral ridge augmentation by the use of grafts comprised of autologous bone or a biomaterial.

Abstract Objective: The present investigation was performed to determine if a block of Bio-Oss® used as an onlay graft can be used as a scaffold for new bone formation. Material and methods: Five mongrel dogs were used. The mandibular premolars were extracted. On both sides of the mandible, the buccal bone plate was resected and defects, about 25 mm long, 8 mm high and 5 mm wide, were produced After 3 months of healing, a second surgical procedure was performed. In the left side, a block of Bio-Oss® was adjusted to the buccal bone wall. The graft had the shape of a cylinder and was retained with a miniscrew and covered with a collagen membrane. In the contra-lateral side of the mandible, a block biopsy was first obtained from the ascending ramus. This bone graft had the shape of a cylinder that was 8 mm in diameter and 3 mm thick. The graft was transferred to the experimental site, adjusted to the buccal wall, retained with a miniscrew, and covered with a membrane. The flaps were repositioned and closed with sutures to ensure a complete coverage of the experimental sites. After 6 months of healing, the dogs were sacrificed and the experimental sites dissected. The biopsies were processed for ground sectioning. The sections were stained in toluidine blue, examined in the microscope, and a number of histo- and morphometric assessments made. Results: The study demonstrated that cortical bone used as an onlay graft in the lateral aspect of the alveolar ridge, during a 6-month period of healing integrated with the host bone but underwent marked peripheral resorption. Thus, close to 30% of the height and 50% of the length of the graft was replaced with connective tissue. It was further observed that while the dimensions of a graft which contained a scaffold of cancellous bovine bone mineral remained unchanged, only moderate amounts of new bone formed at the base of this graft. Conclusion: Grafts of autologous cortical bone, placed on the surface of a one-wall defect, may undergo marked resorption during healing. A similar graft of Bio-Oss® may retain its dimension, and limited amounts of new bone will form within the biomaterial. [source]


The effect of a fibrin glue on the integration of Bio-Oss® with bone tissue

An experimental study in labrador dogs
Abstract Background: Bio-Oss® is a deproteinized bovine mineral used in bone augmentation procedures. The particles are often mixed with a protein product (Tisseel®) to form a mouldable graft material. Aim: The aim of the present experiment was to study the healing of self-contained bone defects after the placement of Bio-Oss® particles alone or mixed with Tisseel® in cylindrical defects in the edentulous mandibular ridge of dogs. Material and methods: In 4 labrador dogs, the 2nd, 3rd and 4th mandibular premolars were extracted bilaterally. 3 months later, 3 cylindrical bone defects, 4 mm in diameter and 8 mm in depth, were produced in the right side of the mandible. Following a crestal incision, full thickness flaps were raised and the bone defects were prepared with a trephine drill. The defects were filled with Bio-Oss® (Geistlich Biomaterials, Wolhuser, Switzerland) particles alone or mixed with Tisseel® (Immuno AG, Vienna, Austria), or left "untreated". A collagen membrane (Bio-Gide®, Geistlich Biomaterials, Wolhuser, Switzerland) was placed to cover all defects and the flaps were sutured. 2 months later, the defect preparation and grafting procedures were repeated in the left side of the mandible. After another month, the animals were sacrificed and biopsies obtained from the defect sites. Results: Bio-Oss® -treated defects revealed a higher percentage of contact between graft particles and bone tissue than defects treated with Bio-Oss®+ Tisseel® (15% and 30% at 1 and 3 months versus 0.4% and 8%, respectively). Further, the volume of connective tissue in the Bio-Oss® treated defects decreased from the 1 to the 3 month interval (from 44% to 30%). This soft tissue was replaced with newly formed bone. In the Bio-Oss®+ Tisseel® treated defects, however, the proportion of connective tissue remained unchanged between 1 and 3 months. Conclusion: The adjunct of Tisseel® may jeopardize the integration of Bio-Oss® particles with bone tissue. Zusammenfassung Hintergrund: Bio-Oss® ist ein entproteiniertes Mineral vom Schwein, was bei knöchernen Augmentationen verwendet wird. Die Partikel werden oft mit einem Proteinprodukt gemischt, um ein formbares Implantationsmaterial zu erhalten. Ziel: Das Ziel des vorliegenden Experimentes war das Studium der Heilung von selbst-erhaltenden Knochendefekten nach der Anwendung von Bio-Oss® Partikeln allein oder vermischt mit Tisseel® in zylindrischen Defekten im zahnlosen unteren Kieferkamm von Hunden. Materal und Methoden: Bei 4 Labradorhunden wurden die 2., 3. und 4. unteren Prämolaren beidseitig extrahiert. 3 Monate später wurden 3 zylindrische Knochendefekte, 4 mm im Durchmesser und 8 mm tief, auf der rechten Seite des Unterkiefers hergestellt. Nach einer krestalen Incision wurde ein voller Mukoperiostlappen mobilisiert und die knöchernen Defekte mit einem Trepanfräser präpariert. Die Defekte wurden mit Bio-Oss® Partikeln (Geistlich Biomaterial, Wolhuser, Schweiz) allen oder gemischt mit Tisseel® (Immuno AG, Wien, Österreich) gefüllt oder blieben "unbehandelt". Eine Kollagenmembran (Bio-Gide®, Geistlich Biomaterial, Wolhuser, Schweiz) wurde zur Abdeckung über alle Defekte gelegt und die Lappen reponiert und vernäht. 2 Monate später wurden die Defektpräparationen und die Implantationsmaßnahmen auf der linken Seite des Unterkiefers widerholt. Nach einem weiteren Monat wurden die Tiere getötet und Biopsien von den Defektseiten gewonnen. Ergebnisse: Mit Bio-Oss® behandelte Defekte zeigten einen höheren Prozentsatz von Kontakt zwischen Implantationsmaterial und Knochengewebe als die Defekte, die mit Bio-Oss® und Tisseel® behandelt worden waren (15% und 30% zum 1. Monat und 3. Monat versus 0.4% und 8%). Weiterhin verringerte sich das Volumen des Bindegewebes in den mit Bio-Oss® behandelten Defekten vom 1. zum 3. Monat (von 44% zu 30%). Dieses Weichgewebe wurde mit neu gebildetem Knochen ersetzt. In dem mit Bio-Oss® und Tisseel® behandelten Defekten blieb die Verteilung des Bindegewebes zwischen dem 1. und 3. Monat unverändert. Le but de l'étude présente a été d'étudier la guérison des lésions osseuses après le placement de particules de Bio-Oss® seules ou mélangées au Tisseel® dans des lésions cylindriques au niveau de la mandibule édentée de labradors. Matériaux et méthodes: Chez 4 labradors les 2ièmes, 3ièmes et 4ièmes prémolaires inférieures ont été avulsées bilatéralement. 3 mois après, 3 lésions osseuses et cylindriques de 4 mm de diamètre et de 8 mm de profondeur ont été produites du côté droit de la mandibule. A la suite d'une incision crestale, des lambeaux d'épaisseur complète ont été relevés et les lésions osseuses préparées avec un trépan. Les lésions ont été comblées par des particules de Bio-Oss® seul (Geistlich Biomaterials, Wolhuser, Suisse) ou mélangées au Tisseel® (Immuno AG, Vienne) ou laissées non-traitées. Une membrane collagène (Bio-Gide®, Geistlich Biomaterials, Wolhuser, Suisse) a été placée pour recouvrir toutes les lésions et les lambeaux ont ensuite été suturés. 2 mois après, les processus précités ont été répétés au niveau gauche de la mandibule. 1 mois plus tard, les animaux ont été tués et les biopsies prélevées. Résultats: Les lésions traitées par le Bio-Oss® ont révélé un % plus important de contact entre les particules du greffon et le tissu osseux que les lésions traitées avec le Bio-Oss®+Tisseel® (respectivement 15% à 30% à 1 et 3 mois versus 0.4% et 8%). De plus le volume de tissu conjonctif dans les lésions traitées par Bio-Oss® diminuait du mois 1 au mois 3, de 44 à 30%. Ce tissu mou a été remplacé par un os néoformé. Dans les lésions traitées par Bio-Oss®+Tisseel®, la proportion de tissu conjonctif demeurait inchangée entre les mois 1 et 3. Conclusions: L'addition de Tisseel® peut mettre en péril l'intégration des particules de Bio-Oss® au tissu osseux. [source]


Color doppler sonographic analysis of uterine and ovarian artery blood flow in women with polycystic ovary syndrome

Abstract Purpose. To study the blood flow patterns of utero-ovarian circulation in polycystic ovary syndrome (PCOS) and to assess their relationship with clinical, metabolic, and hormonal data. Methods. Forty-three women with PCOS and 43 age-matched healthy controls underwent Doppler examination of the utero-ovarian circulation in the follicular phase. Demographic, hormonal, and metabolic parameters were determined. Student's t -test, ,2 -test, and Spearman correlation test were used for statistical analysis. Results. The ovarian artery pulsatility index (PI), resistance index (RI), and SD ratios were significantly lower in PCOS than in controls on the right side (p < 0.001, p = 0.02, p = 0.001, respectively) as well as on the left side (p < 0.001, p < 0.001, p < 0.001, respectively). The uterine artery systolic/diastolic (S/D) ratio was higher on both sides (p = 0.01) and the PI was higher on the left side (p = 0.02) in PCOS than in controls. The right uterine artery PI was positively correlated with luteinizing hormone and hemoglobin (r = 0.417, p = 0.043; r = 0.427, p = 0.033, respectively), the right uterine artery S/D was positively correlated with body mass index (r = 0.479, p = 0.015), and the left uterine artery PI was positively correlated with insulin (r = 0.458, p = 0.021). Conclusion. Doppler sonography of the utero-ovarian circulation may contribute to the evaluation of PCOS patients and a better understanding of the pathophysiology of this syndrome. © 2007 Wiley Periodicals, Inc. Sonography of the normal scapholunate ligament and scapholunate joint space

Abstract Purpose The aims of this study were to assess the visibility of the normal scapholunate ligament on sonography and to establish the normal scapholunate joint space width in the neutral position and radial and ulnar deviation. Methods Two hundred normal wrists in 100 subjects (55 men and 45 women; mean age, 40 years; range, 19,83 years) were examined with high-resolution sonography (5,12-MHz linear-array transducer). The visibility and thickness of the scapholunate ligament were recorded. The width of the scapholunate joint space, or interval, was measured in the neutral position and radial and ulnar deviation. The width of the distal radius was recorded as a comparative standard for the patients' body habitus. Results The dorsal scapholunate ligament was completely (100%) visible in 95 wrists (48%), partially (, 50%) visible in 60 (30%), barely (< 50%) visible in 15 (8%), and not visible in 30 (15%). The volar scapholunate ligament was completely visible in 13 wrists (7%), partially visible in 17 (9%), barely visible in 15 (8%), and not visible in 151 (76%). The proximal component of the ligament was not visible in any subject. Measurement of the scapholunate interval was limited by the lack of identifiable anatomic marks for reference. The mean width of the dorsal scapholunate interval was 4.2 mm (range, 2.3,6.3 mm) in the neutral position. The interval did not differ more than 2.5 mm between the left and right wrists. No predictable change in width on ulnar or radial deviation was evident. The mean scapholunate intervals and mean distal radial width were significantly wider in men than in women and on the right side than on the left side. Conclusions The dorsal scapholunate ligament is completely or partially visible in 78% of normal wrists. Its detection following injury may help to exclude the possibility of scapholunate dissociation. There is a quite wide variation in scapholunate interval widths on sonography and an unpredictable response with stress testing. Heart inflow tract of the African lungfish Protopterus dolloi

Abstract We report a morphologic study of the heart inflow tract of the African lungfish Protopterus dolloi. Attention was paid to the atrium, the sinus venosus, the pulmonary vein, and the atrioventricular (AV) plug, and to the relationships between all these structures. The atrium is divided caudally into two lobes, has a common part above the sinus venosus, and appears attached to the dorsal wall of the ventricle and outflow tract through connective tissue covered by the visceral pericardium. The pulmonary vein enters the sinus venosus and runs longitudinally toward the AV plug. Then it fuses with the pulmonalis fold and disappears as an anatomic entity. However, the oxygenated blood is directly conveyed into the left atrium by the formation of a pulmonary channel. This channel is formed cranially by the pulmonalis fold, ventrally by the AV plug, and caudally and dorsally by the atrial wall. The pulmonalis fold appears as a wide membranous fold which arises from the left side of the AV plug and extends dorsally to form the roof of the pulmonary channel. The pulmonalis fold also forms the right side of the pulmonary channel and sequesters the upper left corner of the sinus venosus from the main circulatory return. The AV plug is a large structure, firmly attached to the ventricular septum, which contains a hyaline cartilaginous core surrounded by connective tissue. The atrium is partially divided into two chambers by the presence of numerous pectinate muscles extended between the dorsal wall of the atrium and the roof of the pulmonary channel. Thus, partial atrial division is both internal and external, precluding the more complete division seen in amphibians. The present report, our own unpublished observations on other Protopterus, and a survey of the literature indicate that not only the Protopterus, but also other lungfish share many morphologic traits.
