[source] Diagnosing and treating diabetic foot infectionsDIABETES/METABOLISM: RESEARCH AND REVIEWS, Issue S1 2004Benjamin A. Lipsky Abstract Foot infections are a common, complex and costly complication of diabetes. We have made considerable progress in establishing consensus definitions for defining infection. Similarly, we have learned much about the appropriate ways to diagnose both soft tissue and bone infections. Accompanying these advances have been improvements in our knowledge of the proper approaches to antibiotic (and surgical) therapy for diabetic foot infections. Furthermore, investigators have explored the value of various adjunctive therapies, especially granulocyte colony stimulating factors and hyperbaric oxygen, for improving outcomes. This paper presents a summary of a minisymposium on infection of the diabetic foot that was held at the fourth International Symposium on the Diabetic Foot, in Noordwijkerhout, The Netherlands. Mavromoustakis Abstract Adaptive behaviour of swarm-based agents (BT Technol. J. 1994; 12:104,113; AAMAS Conference '02, Melbourne, Australia, Month 1,2, 2002; Softcomput. J. 2001; 5(4):313,317.) is being studied in this paper with respect to network throughput for a certain amount of data traffic. Algorithmically complex problems like routing data packets in a network need to be faced with a dynamically adaptive approach such as agent-based scheme. Particularly in interconnected networks where multiple networks are participating in order to figure a large-scale network with different QoS levels and heterogeneity in the service of delay sensitive packets, routing algorithm must adopt in frequent network changes to anticipate such situations. Split agent-based routing technique (SART) is a variant of swarm-based routing (Adapt. Behav. 1997; 5:169,207; Proceedings of 2003 International Symposium on Performance Evaluation of Computer and Telecommunication Systems,SPECTS, Montreal, Canada, July 20,24, 2003; 240,247.) where agents are split after their departure to the next node on a hop-by-hop basis. Packets that are delay sensitive are marked as prioritized which agents recognize-as being a part of a packet- and try to influence the two-way routing tables. Thorough examination is made, for the performance of the proposed algorithm in the network and the QoS offered, taking into account a number of metrics. It is shown that the split agent routing scheme applied to interconnected networks offers a decentralized control in the network and an efficient way to increase overall performance and packet control reducing at the same time the packet loss concept. Wang Summary We initially developed a new flow cytometric (FCM) reference method for the enumeration and staging of nucleated red blood cells (NRBC) in 1997 [Wang et al., 1998 (XIth International Symposium on Technological Innovations in Laboratory Haematology, Banff, Canada, 1998); Tsuji et al., 1999 (Cytometry, 1999)]. The method used CD45 antibody and propidium iodide staining to separate NRBCs from other cells. Accuracy and precision were enhanced because larger numbers of cells were counted than was possible with the manual method. We also developed a method for automated NRBC counting on a haematology analyser, the XE-2100 (Wang, 1988). NRBC were separated from other cells using a special lysing buffer and a fluorescent dye. The XE-2100 was found to detect peripheral and cord blood NRBC accurately and precisely when compared with cell morphology or FCM control methods. The FCM NRBC staging method was established through the identification of different NRBC populations following the novel staining and lysing method. To evaluate the method further, we sorted samples containing NRBCs using a FACSort and investigated NRBC staging on the Sysmex XE-2100TM based on the cell sorting results. Data were analysed using special software (ida). First, we used the data in various parameter combinations. We then established gates to classify the NRBC populations. Finally, we analysed blood specimens from patients with different types of diseases to explore possible clinical applications. Bruce msc, bschons Aim:, This paper describes a three-year project, the aims of which were to disseminate information regarding the assessment and management of children's pain, to reach consensus on the essential elements of pain assessment for children and to track nurses' perceived changes in pain assessment and management practices in hospital settings over time. Background:, Despite the availability of research, guidelines and standards, paediatric nurses inconsistently practice evidence-based pain management. Project methods:, Nurses attending the 5th International Symposium on Paediatric Pain were invited to a workshop to discuss best practices and consider ways to share information regarding the assessment and management of children's pain. A website was designed and participants provided hospital guidelines, exchanged information via the Internet and completed periodic surveys. Conclusion:, Web-based resources were positively received. Consensus regarding the essential elements of pain management was achieved, but nurses identified organizational and resource restraints as barriers to achieving best practices. Surveys identified perceived increases in some hospital and ward level resources and activities to improve children's pain care. Sharing of information related to children's pain management and benchmarking of practice should be encouraged. Consensus building and networking may facilitate change, but organizational commitment is also required for successful implementation of new practices.
