Healthcare financing reform and the new single payer system in the Republic of Korea: Social solidarity or efficiency?

In July 2000, national health insurance in the Republic of Korea was transformed into a single insurer system. This major reform in healthcare financing resulted from the merger of more than 350 health insurance societies. Inequity in healthcare financing and the chronic financial situation of the health insurance societies for self,employed workers in rural areas have been the driving forces leading to the unified health insurance system. The unique institutional context together with political change opened the window of policy change, and various stakeholders such as politicians, rural self,employed workers, trade unions and civic groups were involved in the healthcare reform process. Fair income assessment of the self,employed and the role of the single insurer as a prudent purchaser of medical care will be vital for the new system to achieve its intended goal and improve social solidarity and efficiency of healthcare. GIS in archaeology,the interface between prospection and excavation

Abstract Archaeological prospection and excavation have the same research objective, namely, the study of the material culture of humans. They investigate the archaeological record but are based on different physical properties and work with different resolution and instrumentation. In addition to the study of literature concerning antique discoveries and the collection and evaluation of surface finds, it is aerial archaeology and geophysical prospection that are the most suitable methods of achieving the intended goal. Aerial photographs provide the archaeologist with a large-scale overview, and digital photogrammetric evaluation provides very detailed topographic maps and orthophotographs of the archaeological structures visible on the surface. These structures appear in various forms, through contrasts in the physical properties between the structures themselves and the material that surrounds them. In geophysical prospection, the contrasts between the physical properties of the archaeological structures and the surrounding material usually can be investigated only in the near-surface or with direct ground contact. These contrasts are not directly visible, however, and must instead be measured and converted into a comprehensible visualization. The prospection methods used in the interpretation process are not significantly different from one to another. Interpretation encompasses the localization and classification of archaeological structures, the analysis of their spatial relationships, as well as the creation of models showing the main stratification at a site. Unlike excavations, through archaeological interpretation of prospection data, various accurate archaeological models of the entire site and the surrounding landscape can be made available rapidly. These models can be used for targeted excavations, so as to further condense the information and to refine the models. If all the data are made available in a geographical information system (GIS), it can be combined and further analysed by the excavator as well as by the prospector. Skilled Support Within Intimate Relationships

The literature on social support within dyadic intimate relationships raises a seeming paradox: The availability of support tends to reduce distress, but its actual receipt is often unhelpful and at times engenders feelings of inadequacy, indebtedness, and inequity,unintended but potent side effects of the support transaction. Our review organizes this literature in order to solve the apparent paradox. Specifically, we theorize that, because support attempts are often unskilled and miscarried, they lead to greater costs than benefits. We identify four ways in which dyadic support can be unskillful, ways pertaining to its timing, content, process, or reciprocation. We suggest that when these are addressed, support can regain its intended goals of enhancing dyadic coping, reducing stress, and strengthening relationships. [source]


A comparison of learning outcomes and attitudes in student- versus faculty-led problem-based learning: an experimental study

Objectives To compare learning outcomes and perceptions of facilitator behaviours and small-group process in problem-based learning (PBL) groups led by students and those led by faculty. Design A prospective, Latin-square cross-over design was employed. Second-year medical students participated in 11 PBL cases over the course of the academic year. For each case, half the student groups were led by faculty and the other half by a student group member selected randomly to serve in the facilitator role. Learning outcomes were assessed by performance on objective examinations covering factual materials pertinent to the case. Perceptions of facilitator behaviours and of group functioning were assessed with a questionnaire completed at the end of each individual case. Focus-group discussions were held to gain more in-depth information about student perceptions and experience. Student-led sessions were observed at random by the investigators. Setting A state-supported, US medical school with a hybrid lecture-based and problem-based curriculum. Subjects One hundred and twenty-seven second-year medical students and 30 basic science and clinical faculty. Results No differences were detected in student performance on the objective evaluation based on whether the facilitator was a faculty member or peer group member, nor were there any differences in the perceptions of group process. Students gave peer facilitators slightly higher ratings in the second semester of the experiment. In the focus-group discussions, students voiced a general preference for student-led groups because they felt they were more efficient. Observation and focus-group reports suggest that groups led by students sometimes took short cuts in the PBL process. Conclusion In a hybrid lecture- and PBL-based curriculum, student performance on objective examinations covering PBL materials is unaffected by the status of the facilitator (student vs. faculty). However, in peer-facilitated groups, students sometimes took short cuts in the PBL process that may undermine some of the intended goals of PBL. [source]


An empirical analysis of green supply chain management in the German automotive industry

Abstract The purpose of this paper is the empirical investigation of the status quo of green supply chain management in the German automotive industry from a practitioner's point of view. Several aspects of green supply chain management, such as the point of time of implementation, the driving forces, the relevance of intended goals and their particular realization and the adoption of eco-programs with suppliers and customers as well as internal and external barriers are analyzed. Furthermore, the link to different performance criteria is tested by inductive statistics in order to show the potential of green supply chain management for competitiveness. For this study, managers from the automotive supply industry in Germany were asked about their estimations in this regard. The main conclusion is that managers express the need for green supply chain management on the one hand but also see corresponding problems in terms of required resources on the other hand.
