METHODS All members of the Icelandic Nurses Association of working age were invited to participate in an Internet-based survey. Each nurse was assigned a unique password mailed to his/her home along with information on how to access the survey Web site. Each nurse could submit answers only once. On submission, data were automatically coded and saved in a database under encrypted numerical identifiers. FINDINGS A total of 463 nurses (18% response rate) participated by submitting answers. The sample was representative of the population in terms of demographic characteristics. Information resources most valued when planning nursing care included text-based progress notes (77%), nursing care plans (52%), doctor's orders (49%), verbal information (48%), and documented nursing diagnoses (37%). Of the participants, 58% said NANDA was used in their workplace; 28% said no standardized nursing documentation was used; 19% reported using NIC always or sometimes when documenting nursing interventions; and 20% never used NIC. NOC use was reported only by researchers. Of the sample, 86% reported that it is important or necessary for nurses to standardize documentation practices; 30% found NANDA useful in education; 56% found it useful for clinical work; 17% for research; and 7% found it not useful at all. Nine percent believed that NANDA diagnoses were not descriptive enough of patients' problems, and 23% found their wording problematic. No statistically significant differences were found between reported use of or attitudes toward NANDA and NIC when comparing nurses who use electronic patient record systems that support NANDA and NIC documentation and those who use paper documentation only. DISCUSSION The sample may have been somewhat biased toward computer use and classification system use for standardized and computerized documentation. However, results indicate that although Icelandic nurses give free-text progress notes and verbal information a higher priority than nursing diagnoses as an information resource for care planning, they have a positive attitude toward NANDA. NANDA and NIC are still used inconsistently in clinical practice, and 28% of participants claimed not to use any form of standardized documentation. CONCLUSIONS In an effort to standardize clinical documentation among nurses, Icelandic health authorities must follow their documentation mandates with educational and technologic support to facilitate the use of NANDA, NIC, and (after its translation) NOC in nursing documentation practices. Recent progress in information management focuses on building e-infrastructures for the optimization of the research workflow, through both policy-driven and user-pulled dynamics. For decades, High Energy Physics (HEP) has pioneered innovative solutions in the field of information management and dissemination. In light of a transforming information environment, it is important to assess the current usage of information resources by researchers and HEP provides a unique test bed for this assessment. A survey of about 10% of practitioners in the field reveals usage trends and information needs. Community-based services, such as the pioneering arXiv and SPIRES systems, largely answer the need of the scientists, with a limited but increasing fraction of younger users relying on Google. Commercial services offered by publishers or database vendors are essentially unused in the field. The survey offers an insight into the most important features that users require to optimize their research workflow. These results inform the future evolution of information management in HEP and, as these researchers are traditionally "early adopters" of innovation in scholarly communication, can inspire developments of disciplinary repositories serving other communities. [source] Developing a Mental Health Guide for Families and Carers of People with Intellectual DisabilitiesJOURNAL OF APPLIED RESEARCH IN INTELLECTUAL DISABILITIES, Issue 2 2007Anastasia Gratsa Background, Although there is already information available about caring for someone with a mental health problem, there is very little written for families and carers of people with intellectual disabilities. Materials and methods, This paper describes the development of a Guide as such a resource. The consultation stage involved interviews and focus groups with families, service users and paid carers (n = 20) to highlight their needs and preferred format. Additional carers (n = 12) completed structured questionnaires to evaluate the Guide. Results, The initial consultation showed that an easily accessible and user-friendly information resource was needed. The evaluation showed that family carers were satisfied with the Guide, finding it easy to read and covering most of the information required. However, they did highlight areas that needed further detail. Conclusions, The involvement of carers in the development of such resources is essential, emphasizing the benefit of carers' knowledge and experience to professionals. [source] Teaching Evidence-based Medicine to Medical StudentsACADEMIC EMERGENCY MEDICINE, Issue 12 2004Richard B. Ismach MD Evidence-based medicine (EBM) is the rubric for an approach to learning and practicing medicine that applies skills from clinical epidemiology, library science, and information management to clinical practice. Teaching EBM effectively requires a longitudinal approach throughout medical education. This presents many opportunities for academic emergency physicians, especially in the setting of an emergency medicine clerkship. EBM is best taught at the bedside, although this depends on a skilled and interested faculty. Bedside teaching of EBM also requires ready access to modern information resources. Other venues for teaching EBM include morning report, teaching conferences, and journal clubs. Many tools can be used to aid the process, including Web-based sources such as UpToDate, textbooks, and Web-based tutorials, educational prescriptions, and critically appraised topics. (2010) European Journal of Cancer Care Mapping the information resources available to patients with colorectal cancer The objectives of this study were to identify gaps in information provision along the colorectal cancer (CRC) treatment pathway as provided by health services within the North Eastern Metropolitan Integrated Cancer Service in Victoria Australia; to evaluate the information and recommend consistent, high quality health information resources; and to recommend strategies to improve delivery of patient information. A random sample of health professionals (n= 47) from various disciplines at eight health service sites participated in semi-structured interviews regarding the types of information they provided to CRC patients. Information items were mapped against a published CRC patient management framework and evaluated. A total of 193 information items were collected with 24 items specific to CRC. Gaps in information provision were evident in the community, at diagnosis, in clinics, when treatment was determined and when completed. The quality of information delivery to CRC patients across the public health sites was variable. Resources were often unavailable, out of date and inaccessible in other languages. Results indicate a need to improve health information availability and resource delivery to all CRC patients across different health services particularly at diagnosis and after treatment. Further research is required to determine patient preferences for information about CRC. Despite its significant growth on a global scale, Internet access is limited in sub-Saharan Africa (SSA). Few studies have explored Internet access, use of electronic resources and ICT training among health information professionals in Africa. Objective:, The study assessed Internet access, use of electronic resources and ICT training among health information professionals in SSA. Methods:, A 26-item self-administered questionnaire in English and French was used for data collection. The questionnaire was completed by health information professionals from five Listservs and delegates at the 10th biannual Congress of the Association of Health Information and Libraries in Africa (AHILA). Results:, A total of 121 respondents participated in the study and, of those, 68% lived in their countries' capital. The majority (85.1%) had Internet access at work and 40.8% used cybercafes as alternative access points. Slightly less than two-thirds (61.2%) first learned to use ICT through self-teaching, whilst 70.2% had not received any formal training in the previous year. Eighty-eight per cent of respondents required further ICT training. Conclusions and recommendations:, In SSA, freely available digital information resources are underutilized by health information professionals. ICT training is recommended to optimize use of digital resources. To harness these resources, intergovernmental and non-governmental organizations must play a key role. CONCLUSIONS., Rigorous scientific studies have not identified links between autism and either thimerosal-containing vaccine or the measles, mumps, and rubella vaccine. PRACTICE IMPLICATIONS., Nurses are often in the position of providing advice regarding vaccines in their formal practice areas as well as in their daily lives. Families need current and credible evidence to make decisions for their children. Excellent vaccine information resources are available online. [source] Large-scale processes in ecology and hydrologyJOURNAL OF APPLIED ECOLOGY, Issue 2000R.W.G. Caldow 1. Several papers published in the 1980s stressed the importance of scaling issues, the inter-relatedness of patterns and processes at different scales of time and space, to our understanding of ecological systems. Scaling issues are of major theoretical interest and increasingly are of considerable applied importance. 2. In recognition of this, the Natural Environment Research Council, in partnership with the Scottish Executive Rural Affairs Department, funded a Thematic Programme entitled ,Large-scale Processes in Ecology and Hydrology'. The principal aim of this Programme was to integrate recent major developments in information resources and technologies with current theory in order to improve understanding of large-scale patterns and processes and their relationship to patterns and processes at smaller scales. 3. The Thematic Programme, which ran from 1995 until 1999, funded six research projects that have generated a large body of published papers. This volume, dedicated to the findings of the Programme, brings together outputs from all six projects with the aim of ensuring a rapid and widespread dissemination of the Programme's findings. A brief résumé of each of the papers is presented. 4. The papers in this volume cover a wide variety of subjects ranging from ions to the flora and fauna of the United Kingdom. Nonetheless, each study has sought in various ways to quantify observed spatio-temporal patterns at a range of scales, to determine whether those patterns are consistent across scales and to identify the interactions between small-scale patterns and processes and those at larger scales. The importance of the spatial and temporal scales at which studies are conducted, the key role played by dispersal in spatial population dynamics, and the diversity of ways in which large-scale patterns and processes relate to those at smaller scales are highlighted in many of the papers. 4. All of the papers presented here have direct relevance to applied issues. These issues are diverse and include the control of invasive alien species, the conservation of declining, threatened or endangered species, the development of survey techniques, strategies for farmland, woodland and forestry management, and the assessment of pollution sensitivity. Thus, the Thematic Programme has addressed issues of considerable theoretical interest and has at the same time generated results and predictive models that are of considerable practical and policy relevance. Dinet Abstract The purpose of this exploratory study was to investigate the influence of two individual characteristics (Web experience and academic focus) of adolescents on the Web perception, using off-line questionnaires (a Lickert response scale) constituted on the basis of a series of interviews. Questions concerned: perceptions about the nature of information found in the Web; ,strategies' of access to the interesting Internet sites and the reliability of different information resources (libraries, television, Web, etc.). Results lead to the assumption that adolescents with high Web experience became more critical, less confident and less enthusiastic than adolescents with low Web experience and that, in some dimensions, perceptions of literature students are different to those of science students. Even if some interesting results were obtained, further research is needed to explore users' perceptions related to individuals' characteristics and to determine the generalisability of the influences identified in this exploratory study. [source] Sources of knowledge in clinical practice in postgraduate medical students and faculty members: a conceptual mapJOURNAL OF EVALUATION IN CLINICAL PRACTICE, Issue 4 2007Reza Yousefi-Nooraie MD Abstract Objectives, To determine the most important knowledge sources that can influence clinical practice and to cluster them in conceptual groups based on their relative importance. Methods, Faculty members, fellows and residents of a large teaching tertiary care hospital were asked to rate the importance of different resources in their daily clinical practice and their understanding of some common terms from evidence-based medicine. The knowledge sources were distributed in a two-dimensional map using multidimensional scaling and hierarchical cluster analysis. Results, A total of 250 of 320 recruited hospital staff returned the questionnaires. The most important resources in daily practice were English journals, text books and literature searching for faculty members, experience, text books and English journals for fellows and text books, experience and peers for residents. Regional journals were the least important resources for all study groups. About 62.7% of residents did not know the meaning of ,number needed to treat', 36.8%,confidence interval', 54.9%,confounding factor' and 44.6%,meta-analysis'. The percentages for faculty members were 41.3%, 37%, 42.2% and 39.1%. The knowledge sources were placed in four clusters in a point map derived from the multidimensional scaling process. Conclusion, The dominance of the traditional information resources and experience-based medicine debate which is the consequence of traditional approaches to medical education may be one of the considerable barriers to the dissemination of evidence-based medicine in developing countries. The evidence-based clinical practice guidelines could be used as a useful passive-predigested source for busy clinicians to make informed decisions. A considerable Western bias may undermine the local research in developing world. [source] Cognitive economy and satisficing in information seeking: A longitudinal study of undergraduate information behaviorJOURNAL OF THE AMERICAN SOCIETY FOR INFORMATION SCIENCE AND TECHNOLOGY, Issue 12 2009Claire Warwick This article reports on a longitudinal study of information seeking by undergraduate information management students. It describes how they found and used information, and explores their motivation and decision making. We employed a use-in-context approach where students were observed conducting, and were interviewed about, information-seeking tasks carried out during their academic work. We found that participants were reluctant to engage with a complex range of information sources, preferring to use the Internet. The main driver for progress in information seeking was the immediate demands of their work (e.g., assignments). Students used their growing expertise to justify a conservative information strategy, retaining established strategies as far as possible and completing tasks with minimum information-seeking effort. The time cost of using library material limited the uptake of such resources. New methods for discovering and selecting information were adopted only when immediately relevant to the task at hand, and tasks were generally chosen or interpreted in ways that minimized the need to develop new strategies. Students were driven by the demands of the task to use different types of information resources, but remained reluctant to move beyond keyword searches, even when they proved ineffective. They also lacked confidence in evaluating the relative usefulness of resources. Whereas existing literature on satisficing has focused on stopping conditions, this work has highlighted a richer repertoire of satisficing behaviors. [source] ICT-mediated diaspora studies: New directions in immigrant information behavior researchPROCEEDINGS OF THE AMERICAN SOCIETY FOR INFORMATION SCIENCE & TECHNOLOGY (ELECTRONIC), Issue 1 2008Ajit Pyati Organizer/Convener A growing and sizeable area of study within information behavior research focuses on the information needs and behaviors of immigrant populations (see Chu, 1999; Fisher, Durrance & Hinton, 2004; Caidi & Allard 2005; Srinivasan & Pyati, 2007). Some of the unique needs of these populations include information to aid with coping skills and social inclusion, as well as culturally specific information resources. Moreover, immigrant communities have information networks that span national boundaries, which affects their needs and uses of information and communication technologies (ICTs). This panel will focus on the role of ICTs in mediating the information environments of immigrant and diasporic communities. While focusing on how ICTs mediate immigrant information needs, this panel also contextualizes immigrant information behavior research within globalization and diaspora studies. For instance, the concept of "e-diaspora" is a term gaining in popularity, but rarely invoked in relation to immigrant information behavior research. How do new media technologies mediate and influence the information needs and behaviors of immigrant populations? Are localized immigrant information needs mediated by diasporic information sources? The panelists will focus on ICT-mediated services for immigrant populations within the context of both local and global information environments. Questions addressed include: In what ways do diasporic information environments shape local immigrant information needs and their social inclusion into the host society? How does the "digital divide" manifest itself in studies of ICT-mediated immigrant information behavior? The issues addressed by the panel are both timely and critical as evidenced by the ongoing debates in Europe, North America and elsewhere on immigration policy, on integration and identity, and the role of ICTs in a globalized world. This international perspective will be reflected in the composition of the panel. [source] Awareness and barriers to use of cancer support and information resources by HMO patients with breast, prostate, or colon cancer: patient and provider perspectivesPSYCHO-ONCOLOGY, Issue 2 2001Elizabeth G. Eakin This study assessed patient awareness and use,as well as obstacles to use,of HMO- and community-based psychosocial support services designed for cancer patients. Participants were a randomly selected group of patients from a large Northwest HMO, with breast (N=145), prostate (N=151), or colon cancer (N=72), and their oncology and urology providers (N=29). Patient awareness was highest for HMO-based services (68,90%) and lower for community- (33%) or Internet-based (10,14%) services, and use rates were low across all services (range 2,8%). Providers reported referring 70% of their patients to HMO cancer support services, but their estimates of actual patient use of these services (40%) were inflated. Providers reported few barriers to referring patients to support services. The most commonly reported patient barriers to using such services were already having adequate support, lack of awareness of the service, and lack of provider referral. Results of regression analyses suggest that education, physician referral, social support, and spirituality may be important influences on use of cancer support services. This study takes a first step toward understanding patient use of existing cancer support services and suggests ways to increase participation in these services. Green MD Abstract Introduction: Diplomates in the American Board of Internal Medicine (ABIM) Maintenance of Certification (MOC) program satisfy the self-evaluation of medical knowledge requirement by completing open-book multiple-choice exams. However, this method remains unlikely to affect practice change and often covers content areas not relevant to diplomates' practices. We developed and evaluated an Internet-based point of care (POC) learning portfolio to serve as an alternative. Methods: Participants enter information about their clinical questions, including characteristics, information pursuit, application, and practice change. After documenting 20 questions, they reflect upon a summary report and write commitment-to-change statements about their learning strategies. They can link to help screens and medical information resources. We report on the beta test evaluation of the module, completed by 23 internists and 4 internal medicine residents. Results: Participants found the instructions clear and navigated the module without difficulty. The majority preferred the POC portfolio to multiple-choice examinations, citing greater relevance to their practice, guidance in expanding their palette of information resources, opportunity to reflect on their learning needs, and "credit" for self-directed learning related to their patients. Participants entered a total of 543 clinical questions, of which 250 (46%) resulted in a planned practice change. After completing the module, 14 of 27 (52%) participants committed to at least 1 change in their POC learning strategies. Discussion: Internists found the portfolio valuable, preferred it to multiple-choice examinations, often changed their practice after pursuing clinical questions, and productively reflected on their learning strategies. The ABIM will offer this portfolio as an elective option in MOC. [source] Learning Needs of Postpartum Women: Does Socioeconomic Status Matter?BIRTH, Issue 2 2005Wendy Sword PhD This study's aim was to examine women's concerns at the time of hospital discharge and unmet learning needs as self-identified at 4 weeks after discharge. Methods: Data were collected as part of a cross-sectional survey of postpartum health outcomes, service use, and costs of care in the first 4 weeks after postpartum hospital discharge. Recruitment of 250 women was conducted from each of 5 hospitals in Ontario, Canada (n = 1,250). Women who had given vaginal birth to a single live infant, and who were being discharged at the same time as their infant, assuming care of their infant, competent to give consent, and able to communicate in one of the study languages were eligible. Participants completed a self-report questionnaire in hospital; 890 (71.2%) took part in a structured telephone interview 4 weeks after hospital discharge. Results: Approximately 17 percent of participants were of low socioeconomic status. Breastfeeding and signs of infant illness were the most frequently identified concerns by women, regardless of their socioeconomic status. Signs of infant illness and infant care/behavior were the main unmet learning needs. Although few differences in identified concerns were evident, women of low socioeconomic status were significantly more likely to report unmet learning needs related to 9 of 10 topics compared with women of higher socioeconomic status. For most topics, significantly more women of both groups identified learning needs 4 weeks after discharge compared with the number who identified corresponding concerns while in hospital. Conclusions: It is important to ensure that new mothers are adequately informed about topics important to them while in hospital. The findings highlight the need for accessible and appropriate community-based information resources for women in the postpartum period, especially for those of low socioeconomic status. In order for this to happen students need to develop the capability to deal with information. This qualitative interview,based study examines lecturer perspectives on the roles of electronic information resources in undergraduate education. In line with the phenomenographic tradition, three functional categories of electronic information use are proposed. Firstly, the "electronic academic library" offers new opportunities for access to materials but lecturers indicated minimal change to their teaching approaches. Electronic resources were simply added to reading lists and the responsibility for developing students' information skills was seen to rest with librarians. Secondly, lecturers identified changes towards more constructivist approaches to learning, drawing upon new sources of primary data available electronically and described specific teaching approaches to assist students to develop the relevant information handling skills. Finally, lecturers were uncertain about the value of the wider information resources of the Internet/Web for students and were concerned about variable information quality and the possibilities of plagiarism. The research indicates that information use in student learning is a multi,faceted phenomenon. Much current discussion centres on the concept of information literacy which draws together information skills and subject-related skills and knowledge. Students do not merely require generic information skills but a knowledge of the discipline and the capability to handle complex information. Partnerships between academics and librarians are a way forward in helping students to develop as autonomous information users. Not only do the two professional groups offer different expertise, but they also bring different perspectives on the problematic balance between student autonomy and student support. Female sexual functioning is a complex process involving physiological, psychosocial and interpersonal factors. Sexual dysfunction (SD) is frequent (40,74%) among women with multiple sclerosis (MS), reflecting neurological dysfunction, psychological factors, depression, side effects of medications and physical manifestations of the disease, such as fatigue and muscle weakness. A conceptual model for sexual problems in MS characterizes three levels. Primary SD includes impaired libido, lubrication, and orgasm. Secondary SD is composed of limiting sexual expressions due to physical manifestations. Tertiary SD results from psychological, emotional, social, and cultural aspects. Sexual problems cause distress and may affect the family bond. Practical suggestions on initiation of discussion of sexual issues for MS patients are included in this review. Assessment and treatment of sexual problems should combine medical and psychosexual approaches and begin early after MS diagnosis. Intervention can be done by recognizing sexual needs, educating and providing information, by letting patients express their difficulties and referring them to specialists and other information resources.
