Factors associated with successful establishment of breastfeeding in very preterm infants

Abstract Aim:, To describe feeding practices at hospital discharge in relation to characteristics of the very preterm infants (VPI) and their mothers. Methods:,Design. Prospective hospital-based registration of very preterm infants born with a gestational age ,32 weeks in Denmark during 2004,2008. Subjects. Healthy mothers and VPI without diseases causing eating disabilities at discharge. Results:, A total of 478 VPI were registered. At discharge, 60% were exclusively breastfed, 35% were exclusively bottlefed, and 5% were both breast- and bottle-fed. Mothers of high social class (p = 0.000) and ,not smoking' (p = 0.003) were significantly more often breastfeeding their preterm infant(s) at discharge. Single births infants tended more often to be breastfed (p = 0.09). Infant age at discharge and duration of hospitalization did not influence breastfeeding at discharge. Increase in weight z-score from birth to discharge was largest in the bottlefeeding-group compared with the breastfeeding-group (p = 0.000) probably as a result of feeding practice the last week(s) of hospitalization. Conclusion:, Breastfeeding can successfully be established in very preterm infants. Mothers of low social classes, smokers, multiple birth and very preterm infants with low weight for age may need extra attention in breastfeeding establishing policies. [source]


Effect of Professional Postpartum Support on Infant Feeding Patterns Among Breastfeeding Participants in the WIC Program

The purpose of this study was to determine if professional breastfeeding support could postpone formula introduction and prolong breastfeeding among participants in the Special Supplemental Nutrition Program for Women, Infants and Children (WIC). Fifty women in the experimental group received home visits and phone calls from a lactation educator following hospital discharge; 50 control group women were encouraged to contact a lactation educator as needed. Infants' ages at formula introduction and breastfeeding continuation rates were not significantly different between the groups. Common reasons for breastfeeding cessation included returning to work/school, inadequate milk supply, and latch-on/suckling problems. Use of telephone contacts, delay of breastfeeding support until after hospital discharge, and early formula supplementation may have contributed to the ineffectiveness of the intervention. [source]


Introduction of solids and formula to breastfed infants: a longitudinal prospective study in Uppsala, Sweden

The introduction of solids and formula was studied among 506 breastfed infants in Uppsala, Sweden, based on daily recordings during the first year. The mothers had previously breastfed at least 1 infant for at least 4 mo. Thirty-four per cent of the infants were introduced to solids before the age of 4 mo (4,6 mo is recommended in Sweden). Accustoming the infants to solids was a lengthy process. Life-table analyses showed a median duration of 28 d from the first introduction of solids to consumption of >10 ml daily, and 46 d before the infants ate 100 ml of solids in 1 d for the first time. These durations were longer the younger the infant was at the introduction of solids. Thirty-two per cent of infants given formula consumed 100 ml the first time it was given, and 49% did so within 1 wk, regardless of infant age. Conclusion: Parents and healthcare personnel need to be aware that accustoming breastfed infants to solid food is a lengthy process, and that there is a strong age effect on this duration. It is also important to consider what consequences the (usually) more abrupt introduction of formula might have on breastfeeding. [source]


Individual Differences in the Contribution of Maternal Soothing to Infant Distress Reduction

This study investigates individual differences in the contribution of specific maternal regulatory behaviors to the mother-infant dyad's regulation of infant distress response. Additionally, we examined the stability of infants' stress responses and the stability of specific maternal soothing behaviors. The sample included 128 mother-infant dyads that were observed during an inoculation at 2 and 6 months. The average intensity of infant cry response showed modest stability across age only before controlling for the infant's general state of irritability, and the duration of crying was not stable. Of the 8 specific maternal regulatory behaviors studied, affection, touching, and vocalizing showed the strongest stability across infant age. Finally, an index of the contingency between maternal soothing and infant cry reduction at 2 months predicted shorter cry duration but not cry intensity at 6 months. The results of this study indicate that infants whose mothers showed a greater contribution to reducing their distress at 2 months showed a shorter duration of crying 4 months later. This suggests a possible longitudinal influence of maternal regulation on infants' distress responses. [source]


Prenatal cocaine exposure and infant sleep at 7 months of age: The influence of the caregiving environment

The primary goal of this study was to examine sleep problems in a sample of cocaine-exposed 7-month-old infants and to determine if maternal psychopathology mediated any existing association between substance exposure and sleep behaviors. We also examined the differences in sleep behaviors of cocaine-exposed infants in parental custody and cocaine-exposed infants in nonparental custody. Participants were 65 cocaine-exposed and 53 nonexposed infants and their primary caregivers who were recruited at delivery and assessed at 7 months of infant age. As expected, women who used cocaine during pregnancy had more psychiatric symptoms than nonusers. Prenatal exposure to heavier amounts of cocaine was significantly related to more severe sleep difficulties, and maternal anxiety mediated this association. Approximately 28% of cocaine mothers lost custody of their infants by 7 months of age. Nonmaternal caregivers had significantly fewer symptoms of psychopathology than the cocaine-using women who retained custody of their children. Infants who were in nonparental care at 7 months of age also had less severe sleep problems than did infants who remained in parental care. [source]


Maternal substance use and mother,infant feeding interactions

The purpose of this study was to examine the impact of maternal polydrug cocaine use during pregnancy and associated risk factors such as maternal psychopathology and negative infant temperament on the quality of mother,infant feeding interactions at 2 months of infant age. Participants were 45 mother,infant dyads (19 cocaine-exposed and 26 nonexposed) who were recruited at birth and assessed again 2 months of infant age. Mother,infant interactions during feeding were videotaped and coded with regard to dyadic reciprocity, maternal noncontingency, and dyadic conflict. Results indicated that maternal cocaine use was associated with higher dyadic conflict. Moreover, cocaine-using mothers were also more likely to use marijuana and alcohol, and use of such substances was associated with lower dyadic reciprocity and higher maternal noncontingency during interactions. Results also suggested that one pathway to higher dyadic conflict during interactions among cocaine-using mothers was through the impact of cocaine on infant risk conditions like lower gestational age and negative temperament (e.g., higher distress to novelty). Interventions focusing on promoting the quality of mother,infant interactions in combination with substance abuse treatment may be especially promising for this population. ©2001 Michigan Association for Infant Mental Health. Delayed decline of , -globin expression in infant age associated with the presence of G,,158 (C,T) polymorphism

Summary Persistent production of fetal hemoglobin (HbF) in adult has ameliorative effects on hemoglobinopathies and great efforts are currently made to achieve an exhaustive understanding of the molecular mechanisms of the switching in globin gene expression. One of the factors reported to be associated with the expression of fetal globin genes is the Xmn I G,,158 polymorphism, although it is still unclear if it is involved in this mechanism either by itself or in strong linkage disequilibrium with other loci. Here, we report a novel effect of the Xmn I G,,158 site that was found associated with a significant delayed decline of HbF production in infant age. The prolonged decay trend was enhanced when the G,,158 C,T substitution was co-inherited with a , -thalassemic trait. Our observations reinforce the hypothesis that this region plays an important role in the expression of the , -globin genes and give new insights on the intriguing and still poorly understood mechanisms of globin gene expression switching. Non-fatal injuries among Pacific infants in Auckland: Data from the Pacific Islands Families First Two Years of Life study

Aims: Child injury is the leading cause of mortality and morbidity in developed countries. While Pacific infant death rates are relatively high in New Zealand, little is known about non-fatal injury rates. We seek to describe maternally reported injury in Pacific infants aged between 0,24 months. Methods: A cohort of Pacific infants born during 2000 in Auckland, New Zealand, was followed. Maternal home interviews were conducted at 6 weeks, 12 months and 24 months postpartum and injury events were recalled. Marginal models using generalized estimating equations (GEEs) were used to analysis the longitudinal data. Results: The inception cohort included 1398 infants at 6 weeks, 1241 infants at 12 months and 1161 infants at 24 months. The age-specific injury incidence per 1000 person-years exposure was estimated at 48 (95% CI: 23, 88) injuries for infants aged 0,6 weeks, 106 (95% CI: 88, 127) injuries for infants aged 7 weeks,12 months and 174 (95% CI: 151, 199) injuries for infants aged 13,24 months. In the multivariable GEE model, older infants (P < 0.001), infants who were male (P = 0.01), born to Pacific Island fathers and non-Pacific Island mothers (P < 0.001), and in higher or unknown income groups (P = 0.01) were significantly more likely to suffer injury events. No significant two-factor interaction with infant age was identified. Conclusions: Among Pacific infants, non-fatal injury is common and injury incidence rates are considerably higher than national levels. Male infants and those born into ethnically mixed families, where the father was of Pacific Island ethnicity and the mother was non-Pacific, were at increased relative risk of injury and might benefit from specific injury prevention targeting. However, given the high injury incidence levels found, we advocate that investigation and targeting of culturally appropriate prevention strategies for all Pacific families with young children is required to reduce injury rates for Pacific infants in New Zealand. Metabolic alkalosis with hypoelectrolytemia in infants with cystic fibrosis

Abstract Background: Infants with cystic fibrosis (CF) can develop episodes of hyponatremic hypochloremic dehydration with metabolic alkalosis when they sweat excessively, which is not caused by sweating in normal infants. We investigated the incidence of the metabolic alkalosis with hypoelectrolytemia in CF infants, the possible risk factors for its occurrence and the importance of the manifestation in the diagnosis of CF. Methods: In order to evaluate the incidence and the risk factors for the development of this sweat-related metabolic disorder in CF, we reviewed the records of all children diagnosed as having CF before the age of 12 months in a 10-year period. Data analysis included medical history data, clinical features, biochemical parameters (blood pH, serum bicarbonate, sodium, chloride and potassium levels), sweat chloride test values, as well as genetic analysis data. Results: The prevalence of metabolic alkalosis in association with low serum electrolyte concentrations (hyponatremia, hypochloremia, and hypokalemia) in infant CF population in our region was 16.5%. We found no season predilection in its occurrence. Early infant age, breast-feeding, delayed CF diagnosis, heat exhaustion and the presence of severe CF transmembrane conductance regulator mutations are predisposed factors for the development of metabolic alkalosis with hypoelectrolytemia. Conclusions: The results from our study suggest that metabolic alkalosis with hypoelectrolytemia is a relatively common manifestation of CF in infancy. The possibility of CF should be seriously considered in any infant with this metabolic disorder. [source]


The effects of lactation and infant care on adult energy budgets in wild siamangs (Symphalangus syndactylus)

Abstract In mammals with biparental care of offspring, males and females may bear substantial energetic costs of reproduction. Adult strategies to reduce energetic stress include changes in activity patterns, reduced basal metabolic rates, and storage of energy prior to a reproductive attempt. I quantified patterns of behavior in five groups of wild siamangs (Symphalangus syndactylus) to detect periods of high energetic investment by adults and to examine the relationships between infant care and adult activity patterns. For females, the estimated costs of lactation peaked at around infant age 4,6 months and were low by infant age 1 year, whereas the estimated costs of infant-carrying peaked between ages 7 and 12 months, and approached zero by age 16 months. There was a transition from primarily female to male care in the second year of life in some groups. Females spent significantly less time feeding during lactation than during the later stages of infant care, suggesting that female siamangs do not use increased food intake to offset the costs of lactation. Female feeding time was highest between infant ages 16 and 21 months, a period of relatively low female investment in the current offspring that coincided with the period of highest male investment in infant care. This suggests that male care may reduce the costs of infant care for females in the later stages of a reproductive attempt. The female energy gain resulting from male care was likely invested in somatic maintenance and future reproduction, rather than the current offspring. Social dynamics and individual plasticity of infant care behavior in cooperatively breeding cotton-top tamarins

Abstract Individual variation in infant caretaking behavior is prevalent among marmoset and tamarin monkeys. Although most group members participate in infant care, the timing and amount provided differs greatly. In this study, we quantified general trends in infant carrying behavior by using a longitudinal database that included 11 years of instantaneous scan observations following 80 births of cotton-top tamarins. Using detailed focal observations on a subset of the same families (10 births) we identified influences that affected expression of infant care at the group and individual levels. Fathers were the primary carriers and paternal carry time gradually decreased with increasing infant age. Paternal carry time also decreased significantly with an increasing number of older sibling helpers. Most fathers began to carry on the first day postpartum. However, we report circumstances in which fathers delayed carrying until almost a month postpartum. Fathers retrieved infants the most, although adult brothers' rates of retrievals peaked and surpassed fathers' rates during week 4 postpartum. Fathers delayed rejection of infants until week 4, whereas mothers rejected infants immediately and throughout the eight weeks. Nonetheless, infants climbed onto their mothers more than onto any other family member. Mothers showed a high initial investment in carrying during the first two weeks, decreasing quickly thereafter. Maternal contributions to infant carrying remained low and relatively consistent regardless of group size. However, mothers dramatically increased their infant carrying behavior in families in which fathers were absent. Older siblings cared for infants more than did younger siblings, and brothers retrieved and carried infants more than did sisters. Milk composition varies in relation to the presence and abundance of Balantidium coli in the mother in captive rhesus macaques (Macaca mulatta)

Abstract Primate infants require extensive maternal investment, and lactation is the most expensive aspect of this investment. However, the relationship between maternal condition and milk composition has been largely uninvestigated in primates. To better understand this relationship, I collected mid-lactation milk samples from 46 captive multiparous rhesus macaques (Macaca mulatta) at the Caribbean Primate Research Center, Sabana Seca Field Station, Puerto Rico. The maternal variables assessed were age, weight, weight for crown,rump length (CRL), and presence of parasites. Additionally the analysis included infant age, weight, and sex. Protein concentration in milk showed little interindividual variation, whereas fat had a high variance. Mothers without the lower intestinal parasite Balantidium coli had a significantly higher fat concentration in milk than mothers with B. coli, but other parasite species (Trichuris trichiura and Strongyloides fulleborni) were not associated with milk fat concentration. Females with younger infants had a higher fat concentration in their milk than mothers with older infants; however, the association between B. coli and milk fat remained significant after controlling for infant age. Feeding behavior of lactating brown lemur females (Eulemur fulvus) in Mayotte: influence of infant age and plant phenology

Abstract Altmann [Baboon Mothers and Infants, University of Chicago Press, 1980] and Dunbar and Dunbar [Animal Behavior 36:970,980, 1988] provided a model that predicts the amount of time spent feeding by lactating baboon females, as related to infant age. Dunbar's model further suggests that food quality affects the amount of time that females devote to feeding activity, and is predictable from rainfall and temperature data. In this study the model was tested with data recorded from births of the Mayotte brown lemur from four maternal dyads (Eulemur fulvus). This study also examines the correlation between female activity budget, quantities of fresh plant matter ingested, and suckling duration using data collected from focal animal samples on the mother,infant dyads. The relationships among rainfall, food supply, and food quality were also tested. It appears that female brown lemurs do not devote more time to feeding during the infant growth period. The data show that female brown lemurs increased their food intake during the early-lactating period when the frequency of suckling is the highest, and before infants begin to eat substantial amounts of solid foods. Thus, the frequency of feeding reflects the cost of lactation better than suckling duration. Furthermore, females did not appear to select foods according to their availability or rainfall levels. I hypothesize that the lack of convergence between lemur data and baboon observations is due to differences between their respective environments and their feeding ecology. Infant Affect During Parent,Infant Interaction at 3 and 6 Months: Differences Between Mothers and Fathers and Influence of Parent History of Depression

Fifty families participated in mother-infant and father-infant still-face interaction at infant ages 3 and 6 months as part of a study of affect in early parent-infant relationships. Infants' positive and negative affect and parents' positive affect and physical play were coded from videotapes. Consistent with previous research, during the normal condition, mothers displayed more positive affect than did fathers, and fathers were more likely than mothers to display physical play. Infants were more positive with mothers than with fathers. Parents' positive affect but not parent gender predicted infants' positive affect at 6 months. During the still-face condition, infants of parents with a lifetime history of depression were more likely to display negative affect and less likely to display positive affect than infants with no such parent history. Infants' affect was unrelated to parents' current level of depressive symptoms, which indicates the value of considering family history of psychopathology when examining individual differences in infants' affect. Negative emotionality, attachment quality, and behavior problems in early childhood

Negative emotionality, as well as attachment security and disorganization, are seen as major contributors to social adjustment and maladjustment in childhood. However, relatively little is known about whether infant negative emotionality and attachment quality operate together to affect developing behavior problems. The present study thus aims to contribute to this question. Participants were 64 healthy firstborn children and their primary caregivers. Negative emotionality was assessed at the infant ages of 4, 8, and 12 months using laboratory routines. At 18 months, the Strange Situation procedure was conducted to assess infant attachment security and disorganization, and at 30 months, the child's behavior problems were assessed within a structured clinical interview. Attachment security and attachment disorganization were significantly associated with subsequent behavior problems. There was no significant relation between infant negative emotionality and behavior problems. However, there were indications of a stronger association between attachment disorganization and behavior problems in infants high in negative emotionality. The results underpin the importance of attachment quality as well as negative emotionality in social adjustment. Disorganized attachment precedes poor adjustment, especially in infants high in negative emotionality. [source]

Disorganized infant attachment and preventive interventions: A review and meta-analysis

Infant disorganized attachment is a major risk factor for problematic stress management and later problem behavior. Can the emergence of attachment disorganization be prevented? The current narrative review and quantitative meta-analysis involves 15 preventive interventions (N = 842) that included infant disorganized attachment as an outcome measure. The effectiveness of the interventions ranged from negative to positive, with an overall effect size of d = 0.05 (ns). Effective interventions started after 6 months of the infant's age (d = 0.23). Interventions that focused on sensitivity only were significantly more effective in reducing attachment disorganization (d = 0.24) than interventions that (also) focused on support and parent's mental representations (d = ,0.04). Most sample characteristics were not associated with differences in effect sizes, but studies with children at risk were more successful (d = 0.29) than studies with at-risk parents (d = ,0.10), and studies on samples with higher percentages of disorganized attachment in the control groups were more effective (d = 0.31) than studies with lower percentages of disorganized children in the control group (d = ,0.18). The meta-analysis shows that disorganized attachments may change as a side effect of sensitivity-focused interventions, but it also illustrates the need for interventions specifically focusing on the prevention of disorganization.
