Innovative Techniques for Placement of Implantable Cardioverter-Defibrillator Leads in Patients with Limited Venous Access to the Heart

Background: Because of venous occlusion, intracardiac shunting, previous surgery, or small size placement of implantable cardioverter-defibrillator (ICD) leads may not be possible using traditional methods. The purpose of this study was to evaluate and describe innovative methods of placing ICD leads. Methods: The records of all patients undergoing ICD implantation at our institution were reviewed to identify patients with nontraditional lead placement. Indications for ICD, method of lead and coil placement, defibrillation thresholds, complications, and follow-up results were reviewed retrospectively. Results: Eight patients (aged 11 months to 29 years) were identified. Six patients with limited venous access to the heart (four extracardiac Fontan, one bidirectional Glenn, one 8 kg 11-month-old) underwent surgical placement of an ICD coil directly into the pericardial sac. A second bipolar lead was placed on the ventricle for sensing and pacing. Two patients with difficult venous access had a standard transvenous ICD lead inserted directly into the right atrium (transatrial approach) and then positioned into the ventricle. All patients had a defibrillation threshold of <20 J, although one patient required placement of a second coil due to an elevated threshold. There have been no complications and two successful appropriate ICD discharges at follow-up (median 22 months, range 5,42 months). Conclusions: Many factors may prohibit transvenous ICD lead placement. Nontraditional surgical placement of subcutaneous ICD leads on the pericardium or the use of a transatrial approach can be effective techniques in these patients. These procedures can be performed at low risk to the patient with excellent defibrillation thresholds. [source]

Methadone in pregnancy: treatment retention and neonatal outcomes

ABSTRACT Aim To examine the association between retention in methadone treatment during pregnancy and key neonatal outcomes. Design Client data from the New South Wales Pharmaceutical Drugs of Addiction System was linked to birth information from the NSW Midwives Data Collection and the NSW Inpatient Statistics Collection from 1992 to 2002. Measurements Obstetric and perinatal characteristics of women who were retained continuously on methadone maintenance throughout their pregnancy were compared to those who entered late in their pregnancies (less than 6 months prior to birth) and those whose last treatment episode ended at least 1 year prior to birth. Findings There were 2993 births to women recorded as being on methadone at delivery, increasing from 62 in 1992 to 459 births in 2002. Compared to mothers who were maintained continuously on methadone throughout their pregnancy, those who entered treatment late also presented later to antenatal services, were more likely to arrive at hospital for delivery unbooked, were more often unmarried, indigenous and smoked more heavily. A higher proportion of neonates born to late entrants were born at less than 37 weeks gestation and were admitted to special care nursery more often. Conclusion Continuous methadone treatment during pregnancy is associated with earlier antenatal care and improved neonatal outcomes. Innovative techniques for early engagement in methadone treatment by pregnant heroin using women or those planning to become pregnant should be identified and implemented. [source]


Theory of pH-stat titration

Abstract Innovative techniques are being studied to assess the activity of bioreactors and to improve the performance and operational stability of biological processes. Among these techniques, the pH-stat titration is applicable to any bioreaction involving pH variations. Up to now, the main application of the pH-stat titration has been for nitrification monitoring. In this article, we present a theoretical model of pH-stat titration, which predicts the response to any reaction involving the production or consumption of protons, hydroxyl ions, or inorganic carbon chemical species (CO2, HCO3,, CO3=). This model is a useful tool to understand pH-stat titrations, to define their applicability and limits, and to select the best experimental conditions for specific applications. Tests have been performed to compare experimental pH-stat titration rates in the presence of carbon dioxide and HCO3, producing reactions to the values predicted by the model and a very satisfying correspondence was found. © 2003 Wiley Periodicals, Inc. Compound-specific stable-isotope (,13C) analysis in soil science

Abstract This review provides current state of the art of compound-specific stable-isotope-ratio mass spectrometry (,13C) and gives an overview on innovative applications in soil science. After a short introduction on the background of stable C isotopes and their ecological significance, different techniques for compound-specific stable-isotope analysis are compared. Analogous to the ,13C analysis in bulk samples, by means of elemental analyzer,isotope-ratio mass spectrometry, physical fractions such as particle-size fractions, soil microbial biomass, and water-soluble organic C can be analyzed. The main focus of this review is, however, to discuss the isotope composition of chemical fractions (so-called molecular markers) indicating plant- (pentoses, long-chain n-alkanes, lignin phenols) and microbial-derived residues (phospholipid fatty acids, hexoses, amino sugars, and short-chain n-alkanes) as well as other interesting soil constituents such as "black carbon" and polycyclic aromatic hydrocarbons. For this purpose, innovative techniques such as pyrolysis,gas chromatography,combustion,isotope-ratio mass spectrometry, gas chromatography,combustion,isotope-ratio mass spectrometry, or liquid chromatography,combustion,isotope-ratio mass spectrometry were compared. These techniques can be used in general for two purposes, (1) to quantify sequestration and turnover of specific organic compounds in the environment and (2) to trace the origin of organic substances. Turnover times of physical (sand < silt < clay) and chemical fractions (lignin < phospholipid fatty acids < amino sugars , sugars) are generally shorter compared to bulk soil and increase in the order given in brackets. Tracing the origin of organic compounds such as polycyclic aromatic hydrocarbons is difficult when more than two sources are involved and isotope difference of different sources is small. Therefore, this application is preferentially used when natural (e.g., C3-to-C4 plant conversion) or artificial (positive or negative) 13C labeling is used. Substanzspezifische Stabilisotopenanalyse (,13C) in der Bodenforschung Dieser Artikel fasst den Stand der Forschung bezüglich der substanzspezifischen Stabilisotopenanalyse (,13C) zusammen. Innovative Anwendungen und ein Ausblick für künftige Forschungsaktivitäten werden anhand von Fallbeispielen gegeben. Zunächst wird die ökologische Bedeutung von stabilen C-Isotopen kurz erläutert. Daran schließt sich ein methodischer Teil an, in welchem die verschiedenen Techniken gegenüber gestellt werden. Analog zu ,13C-Messungen der Feinerde mittels Elementaranalysator-Isotopenverhältnis-Massenspektrometrie können physikalisch isolierte Fraktionen (z.,B. Korngrößenfraktionen, mikrobielle Biomasse, DOC) analysiert werden. Der Schwerpunkt dieses Übersichtsartikels liegt jedoch in der Diskussion der C-Isotopensignatur chemischer Fraktionen (sog. Biomarker), welche Rückschlüsse auf Herkunft und Dynamik pflanzlicher (Pentosen, langkettige n-Alkane, Ligninphenole) und mikrobieller Rückstände (Phospholipidfettsäuren, Hexosen, Aminozucker und kurzkettige n-Alkane) sowie anderer interessanter Substanzen im Boden erlaubt wie z.,B. ,Black Carbon" und polyzyklische aromatische Kohlenwasserstoffe. Zu diesem Zweck kommen innovative Techniken zum Einsatz wie z.,B. Pyrolyse-Gaschromatographie-Isotopenverhältnismassenspektrometrie, Gaschromatographie-Verbrennungs-Isotopenverhältnismassenspektrometrie und Flüssigkeitschromatographie-Oxidations-Isotopenverhältnismassenspektrometrie. Innovative ökologische Anwendungen werden erläutert, welche sich prinzipiell in zwei Kategorien einteilen lassen: (1) Quantifizierung der Sequestrierung und des Umsatzes dieser Verbindungen in der Umwelt; (2) Untersuchung der Herkunft spezifischer organischer Substanzen. Umsatzzeiten physikalischer (Sand < Schluff < Ton) und chemischer Fraktionen (Lignin < Phospholipidfettsäuren < Aminozucker , Zucker) sind generall kleiner als jene der gesamten organischen Substanz in der Feinerde und nehmen in der in Klammern angegebenen Reihenfolge zu. Die Untersuchung der Herkunft organischer Substanzen (z.,B. polyzyklischer aromatischer Kohlenwasserstoffe) ist problematisch, weil die Unterschiede der Isotopensignatur verschiedener Quellen gering sind und meist mehr als zwei Quellen zur Isotopensignatur des untersuchten Biomarkers beitragen. Deswegen sollte die Untersuchung der Herkunft organischer Substanzen auf Tracer-Experimente beschränkt werden, wie z.,B. nach natürlicher (C3-C4-Pflanzenwechsel) bzw. künstlicher (13C-An- oder -Abreicherung) Markierung. Efficient repair mechanism of real-time broadcast services in hybrid DVB-SH and cellular systems

In order to ensure good video quality and location-independent access to multimedia content, digital video broadcasting-satellite service to handhelds (DVB-SH) takes advantage of several innovative techniques. The major one is the forward error correction (FEC) scheme implemented at the link layer called multiprotocol encapsulation-inter-burst FEC (MPE-IFEC). MPE-IFEC supports reception in situations of long duration erasure spanning several consecutive time slice bursts (lasting several seconds) due to characteristics of the land mobile satellite channel which are easily hampered by obstacles such as trees, buildings, or overpasses. However, when deep signal fades last for larger durations, the MPE-IFEC correction capacity is insufficient and MPEIFEC fails, causing a service interruption. In this paper , we propose a repair mechanism and a suitable architecture for real-time streaming service error handling with reduced degrading effects such as picture freeze, video frame degradation, and video lag. Based on an analytical model of the performance of the MPE-IFEC, an iterative algorithm is proposed where the probability of recovery of lost bursts is computed and updated. The proposed algorithm controls the retransmission request on the cellular network. Simulation results show that by recovering only some specific lost bursts via the cellular path, the quality of experience here expressed in terms of burst error rate is improved. © 2009 Alcatel-Lucent.
