Selected AbstractsDie Detektionseffizienz von Vorausbohrungen im TunnelbauGEOMECHANICS AND TUNNELLING, Issue 5 2008Hans-Jakob Ziegler Dr. phil.nat. Vor allem im Zusammenhang mit den großen Tunnelprojekten werden Vorausbohrungen zur Erkundung des Gebirges systematisch durchgeführt. Wie die Erfahrungen vom Lötschberg- und Gotthard-Basistunnel zeigen, handelt es sich dabei um ein effizientes Hilfsmittel, um "unerwünschte Ereignisse" wie Schlamm- und Wassereinbrüche oder das Anfahren von unerwarteten Störungszonen zu vermeiden. Nur in drei Fällen traten solche "unerwünschten Ereignisse" bisher auf der betrachteten, ausgebrochenen Tunnelstrecke von insgesamt etwa 100 km auf. Dies zeigt klar den großen Nutzen von Vorausbohrungen für die Arbeits- und Planungssicherheit im modernen Tunnelbau. Die vorgestellten Beispiele belegen aber auch, dass auch mit den effizientesten Vorausbohrungen die Eintretenswahrscheinlichkeit von "unerwünschten Ereignissen" nie Null sein wird. The detection efficacy of reconnaissance drilling in tunnelling Mainly in connection with major tunnel projects, reconnaissance drilling is systematically used to survey the host rock. As shows the experience from the Lötschberg- and the Gotthard basetunnels, reconnaissance drilling is an efficient tool in avoiding "unwanted hazards" like mud or water intrusions, or the penetration of unexpected fracture or failure zones. On the observed excavated tunnel distance of about 100 km, up to now, such "unwanted hazards" occurred tree times only. This clearly demonstrates the important benefit of reconnaissance drilling for work and planning security in modern tunnelling. However, the examples presented also make obvious, that even with the most efficient reconnaissance drilling the probability of an "unwanted hazard" will never become zero. [source] Mixed-species bird flocks in dipterocarp forest of north-central Burma (Myanmar)IBIS, Issue 4 2001DAVID I. KING We studied the bird community in deciduous, dipterocarp forest of north-central Burma (Myanmar) during December 1994, March 1996, and January 1997 and 1999. Most members of this community participated in mixed-species flocks. Seventy-three flocks were encountered during our study, containing 52 species. Of these, 25 species occurred in more than 10% of flocks, and were included in our analyses. There were 26 significant correlations among species pairs, 25 of which were positive. Cluster analysis indicated that there were three principal types of flocks: one consisting mostly of small passerines and picids, commonly including Common Wood-Shrike, Small Minivet and White-browed Fantail, among others; a second type consisting mainly of sylviids, e.g. Arctic, Dusky and Radde's Warblers; and a third type which generally centred around Greater and Lesser Necklaced Laughingthrushes. Bird-eating hawks were numerous at these sites, and we witnessed several attacks on flocks during the study. Thus we infer that enhanced protection from predation is an important benefit conferred by flock membership. In contrast, there was little overlap in foraging behaviour among species, suggesting that foraging facilitation is a relatively minor benefit enjoyed by flock members, although we did observe White-browed Fantails and Greater Racket-tailed Drongos kleptoparasitizing other species on occasion. [source] Tracking of multiple target types with a single neural extended Kalman filterINTERNATIONAL JOURNAL OF INTELLIGENT SYSTEMS, Issue 5 2010Kathleen A. Kramer The neural extended Kalman filter is an adaptive state estimation routine that can be used in target-tracking systems to aid in the tracking through maneuvers without prior knowledge of the targets' dynamics. Within the neural extended Kalman filter, a neural network is trained using a Kalman filter training paradigm that is driven by the same residual as the state estimator. The difference between the a priori model used in the prediction steps of the estimator and the actual target dynamics is approximated. An important benefit of the technique is its versatility because little if any a priori knowledge of the target dynamics is needed. This allows the technique to be used in a generic tracking system that will encounter various classes of targets. In this paper, the neural extended Kalman filter is applied simultaneously to three separate classes of targets, each with different maneuver capabilities. [source] Living donor liver transplantation in high-risk vs. low-risk patients: Optimization using statistical modelsLIVER TRANSPLANTATION, Issue 2 2006François Durand Living donors represent a recognized alternative for facilitating the access to transplantation in a period of organ shortage. However, which candidates should be preferentially considered for living-donor liver transplantation (LDLT) is debated. The aim of this study was to create statistical models to determine which strategies of selection for LDLT provide the most efficient contribution. The study included 331 patients listed for deceased-donor transplantation (DDLT) and 128 transplanted with living donors. Statistical models predicting the events following listing were created and combined in a multistate model allowing the testing of different strategies of selection for LDLT and to compare their results. Taking 3-yr survival after listing as the principal end-point, selecting the 20% patients at highest risk of death on the waiting list gave better results than selecting the 20% patients at lowest risk of death after LDLT (70% vs. 64%, respectively). These strategies resulted in waiting list mortality rates of 17% and 8%, respectively. One-year survival after LDLT was lower in high-risk patients (85%) than in low-risk patients (91%). However, the 1-yr survival benefit derived from LDLT was 75% in high-risk patients while it was nil in low-risk patients. In conclusion, LDLT is more effective for overcoming the consequences of organ shortage when performed in patients at high risk of death on the waiting list. On an individual basis, the sickest patients are those who derive the most important benefit from LDLT. This study provides incentives for considering LDLT in high-risk patients. [source] Should children be screened to undergo early treatment for otitis media with effusion?CHILD: CARE, HEALTH AND DEVELOPMENT, Issue 6 2003A systematic review of randomized trials Abstract Background Otitis media with effusion (OME) is the most common cause of acquired hearing loss in childhood and has been associated with delayed language development and behavioural problems. Some have argued that children should be screened and treated early if found to have clinically important OME. The aim of this review was to assess evidence from randomized controlled trials about the effectiveness of screening and treating children with clinically important OME in the first 4 years of their life. The primary outcome was language development. Methods We searched the Cochrane Controlled Trials Register, MEDLINE and EMBASE and reference lists of all included studies in February 2003. We also contacted the first authors of the studies included in this review. Search terms included otitis media; otitis media with effusion; glue ear; OME; screen; children; treatment; language; and behaviour. Data extraction and methodological quality assessment were performed by at least two of us for each study independently, using methods described in the Cochrane Collaboration Handbook. Results From the three included randomized controlled trials evaluating interventions among children with OME identified through screening, we found no evidence of clinically important benefit in language development. Conclusions The identified randomized trials do not show an important benefit on language development from screening the general population of asymptomatic children in the first 4 years of life to undergo early treatment for OME. Screening asymptomatic children in the first 4 years of life for OME is not recommended. [source] The benefit of geriatric intervention in surgery,increased throughput does not necessarily need more bedsCOLORECTAL DISEASE, Issue 6 2000R. B. Dunn Objective Three years ago a consultant geriatrician began regular visits to the coloproctology and general surgery wards of an acute Trust every weekday, to assist staff with the medical management, rehabilitation and discharge planning of patients aged 65 years and above. The length of stay of these patients in the first 10 months of the appointment was compared with younger concurrent controls and with historical controls in the preceding 10 months. Results The mean length of stay for the 4146 patients aged 16 years and above in a 10-month intervention period was 1.3 days shorter (95% confidence interval (CI) 0.86,1.74 days) than for the 4002 patients in the 10-month control period. This pattern was consistent across all ages and types of surgery, including major colorectal resection. Conclusion There was an important benefit to the availability of coloproctology and general surgical beds in this acute Trust. More efficient rehabilitation of elderly patients reduces ,bed blockage' post-operatively, and allows more effective use of available resources. Co-ordination of the geriatric service with the specialized stoma support service allows earlier discharge of the elderly, and their relocation to appropriate premises where long-term changes in bowel habit and problems with stoma care can be managed away from the acute surgical unit. [source] Computer-based management environment for an assembly language programming laboratoryCOMPUTER APPLICATIONS IN ENGINEERING EDUCATION, Issue 1 2007Santiago Rodríguez Abstract This article describes the environment used in the Computer Architecture Department of the Technical University of Madrid (UPM) for managing small laboratory work projects and a specific application for an Assembly Language Programming Laboratory. The approach is based on a chain of tools that a small team of teachers can use to efficiently manage a course with a large number of students (400 per year). Students use this tool chain to complete their assignments using an MC88110 CPU simulator also developed by the Department. Students use a Delivery Agent tool to send files containing their implementations. These files are stored in one of the Department servers. Every student laboratory assignment is tested by an Automatic Project Evaluator that executes a set of previously designed and configured tests. These tools are used by teachers to manage mass courses thereby avoiding restrictions on students working on the same assignment. This procedure may encourage students to copy others' laboratory work and we have therefore developed a complementary tool to help teachers find "replicated" laboratory assignment implementations. This tool is a plagiarism detection assistant that completes the tool-chain functionality. Jointly, these tools have demonstrated over the last decade that important benefits can be gained from the exploitation of a global laboratory work management system. Hawthorne Abstract The prevalence of Type 2 diabetes in women of childbearing age continues to grow as the incidence of Type 2 diabetes increases. Recent evidence shows that treatment of gestational diabetes ensures the best possible outcome for pregnancy complicated by gestational diabetes. Metformin is a logical treatment in these circumstances but there has always been concern about its safety for the fetus, particularly as it crosses the placenta and it may increase the risk of teratogenesis. Although evidence is accumulating that metformin is useful and has a role in polycystic ovary syndrome, a condition of insulin resistance, it is not yet accepted as treatment for Type 2 diabetes in pregnancy and gestational diabetes. Observational data supports the use of metformin in Type 2 diabetes in pregnancy and its role in gestational diabetes is currently under investigation. Metformin may become an important treatment for women with either gestational or Type 2 diabetes in pregnancy and indeed may have additional important benefits for women, including reducing insulin resistance, body weight and long-term risk of diabetes. There is a need for a randomized controlled trial in women with Type 2 diabetes in pregnancy with long-term follow-up of both mothers and children. Until then the best advice remains that optimized glycaemic control prior to conception and during pregnancy is the most important intervention for best possible pregnancy outcome. Specifically, its main purpose is to present some historical perspective on the behaviour of the monetary and fiscal policies pursued in Greece during the period from the early 1830s until the introduction of the euro. For Greece, the lessons of historical experience are very important. Since the formation of the modern Greek state, government officials have striven , sometimes making hard efforts , to keep abreast of international monetary developments. This was because they understood that the participation of a peripheral, poor and inflation-prone country with a weak currency and an underdeveloped money market, like Greece of the time, in a monetary club of powerful economies could improve her international credit standing and imply important benefits in terms of exchange rate and monetary stability, and long-term foreign borrowing. UNDERHILL The role of private market agents in global monetary and financial governance has increased as globalization has proceeded. This shift in both markets and patterns of governance has often been encouraged by states themselves in pursuit of liberalization policies. Much of the literature views these developments in a positive light, yet there are other aspects of these developments that also merit attention. This article supports its central propositions with two cases of emerging global financial governance processes: the Basel II capital adequacy standards for international banking supervision and the International Organization of Securities Commissions-based transnational regulatory processes underpinning the functioning of cross-border securities markets. Based on the case findings, the article argues first that private sector self-regulation and/or public-private partnership in governance processes can leave public authorities vulnerable to dependence on the information and expertise provided by private agents in a fast-moving market environment. Policy in the vital domain of financial regulation has been increasingly aligned to private sector preferences to a degree that should raise fears of bureaucratic capture. Second, the article contends that the overall outcome in terms of global financial system efficiency and stability has been mixed, bringing a range of important benefits but also instability and crisis for many societies to a degree that has led to challenges to global governance itself. The case material indicates that the input, output and accountability phases of legitimacy in global monetary and financial governance are highly problematic, and much of the problem relates to the way in which private market agents are integrated into the decision-making process. Third, the article posits that a better consideration of these three ,phases' of legitimacy and their interrelationships is likely to enhance the political underpinnings and legitimacy of global financial and monetary order. [source] Why ,Health' is not a Central Category for Public Health PolicyJOURNAL OF APPLIED PHILOSOPHY, Issue 2 2009STEPHEN JOHN abstract We normally think that public health policy is an important political activity. In turn, we normally understand the value of public health policy in terms of the promotion of health or some health-related good (such as opportunity for health), on the basis of the assumption that health is an important constituent or determinant of wellbeing. In this paper, I argue that the assumption that the value of public health policy should be understood in terms of health leads us to overlook important benefits generated by such policy. To capture these benefits we need to understand the ends of public health policy in terms of the promotion of ,physical safety'. I then go on to argue that the idea that ,health' is an important category for evaluating or estimating individuals' wellbeing in the normative context of social policy is confused. I then clarify the relationship between my arguments and QALY-based accounts of health assessment. In the final section of the paper, I defend this surprising conclusion against various attacks. [source] CAT4 (Cable Actuated Truss,4 Degrees of Freedom): A Novel 4 DOF Cable Actuated Parallel ManipulatorJOURNAL OF FIELD ROBOTICS (FORMERLY JOURNAL OF ROBOTIC SYSTEMS), Issue 12 2002Craig Kossowski The CAT4 (Cable Actuated Truss,4 degrees of freedom) robot is a novel, passively jointed, parallel robot utilizing six control cables for actuation. The architecture has been under development at the Queen's University Robotics Laboratory. The robot utilizes a passive jointed linkage with 18 revolute joints to constrain the end effector motion and provide the desired structural stability, restricting the end effector to 3 translational degrees of freedom (DOF) and 1 DOF for end effector pitch. This central mechanism together with winched cable actuation gives a number of important benefits for applications where the advantages of a parallel robot are required in conjunction with light weight. Six electric motor driven winches control the length of the cable actuators that extend from the top frame to points on the end effector raft and jointed linkage to create a stiff, but lightweight, actuated robot. Simulation work on the robot is presented giving the kinematics, including a computational estimate of the workspace for a specific configuration. [source] PERCEPTION OF IRRADIATED FOODS AMONG STUDENTS (SECONDARY, UNIVERSITY [FOOD SCIENCE AND NONFOOD SCIENCE]) AND ADULTS IN ARGENTINAJOURNAL OF FOOD PROCESSING AND PRESERVATION, Issue 3 2008ALEJANDRA FLORES ABSTRACT A survey on beliefs and purchase intent of irradiated foods was conducted among 300 Argentine students, covering secondary/high school, food science and nonfood science university, and 100 nonstudent adults. Other factors considered were the type of information provided (introductory, process description and benefits) and the city of residence (small agricultural city and big city). The most important benefits were considered to be microorganism reduction and improved sanitation. On the other hand, insecurity and doubts were the main reasons why respondents would not buy these foods. Only 14% said they would definitely buy irradiated foods. Considering their doubts and insecurities, it is probable that if these respondents see a food labeled as "irradiated," they would not choose to buy it. Within developing countries such as Argentina, the promotion of this technology by government agencies would be very costly, and at present, these costs would not seem to be justified. PRACTICAL APPLICATIONS The negative perceptions of consumers would hinder the successful implementation of food irradiation. The doubts or insecurities that consumers of developing countries have are similar to those that consumers of developed countries have. Most respondents answered that irradiated foods should be labeled as such. If food irradiation was to be pursued further, the place to start would be in food science curricula as this is the basis of future professionals in charge of researching its use and/or implementing the process. [source] States' Senior Residential Property Tax Abatements: Uncontroversial Benefit or Looming but Unrecognized Problem?POLITICS & POLICY, Issue 4 2010CHARLES LOCKHART All the U.S. states create residential property tax abatement policies that provide important benefits for a rapidly growing older population. These policies vary sharply, and we seek to explain variation in their generosity. We employ regression in conjunction with a cross-sectional data set of the 50 states focused early in the current decade. Several variables prominent in explaining cross-state variation in other public policies exert only modest effects on these programs. Rather, the primary influence is a positive one for a cultural orientation toward government helping a broad swath of the citizenry. We attribute this unusual pattern of explanatory factors to (1) abatement programs' older target populations; (2) their tax expenditure status; and (3) their tendency to be categorized as economic development rather than social programs. This explanatory pattern holds theoretical importance inasmuch as it provides few constraints on the growth of at least some programs serving older citizens. Todos los estados de la Unión Americana establecen políticas de reducción de impuestos para las propiedades residenciales que dan beneficios importantes para la población, en rápido crecimiento, de adultos mayores. Estas políticas varían bruscamente y buscamos explicar la variación en su generosidad. Empleamos una regresión conjuntamente con datos transversales de 50 estados y enfocados en el principio de la década actual. Diversas variables que explican la variación entre estados en otras políticas públicas sólo ejercen efectos modestos en estos programas de reducción de impuestos. En cambio, la influencia más importante es una positiva para la orientación cultural relacionada con la ayuda del gobierno para una amplia porción de la ciudadanía. Atribuimos este diseño inusual de factores explicativos a: (1) la población objetivo de adultos mayores de estos programas de reducciones; (2) al estatus de los adultos mayores en los programas de reducciones y (3) a la tendencia de que la reducción de impuestos para las propiedades residenciales sea categorizada como desarrollo económico en lugar de programas sociales. [source] GOAL ATTAINMENT SCALING: AN EFFECTIVE OUTCOME MEASURE FOR RURAL AND REMOTE HEALTH SERVICESAUSTRALIAN JOURNAL OF RURAL HEALTH, Issue 5 2002Ruth Cox ABSTRACT: The aim of this paper is to demonstrate the utility of Goal Attainment Scaling (GAS) as an effective, multidisciplinary measure of client outcomes for rural and remote health services. Goal Attainment Scaling was adopted by the Spinal Outreach Team (SPOT) as a client-focussed evaluation tool, as it is sensitive to the individual nature of clients' presenting issues and the multidisciplinary focus of the team. It enables individualised goals to be set on a five-point scale. Goal Attainment Scaling was introduced to the SPOT service after a pilot trial established guidelines for its effective implementation. An ongoing review process ensures that goal scaling remains realistic and relevant. Service outcomes can be effectively summarised using a frequency distribution of GAS scores. One of the important benefits of GAS is its facilitation of collaborative goal setting between clinician and client. Goal Attainment Scaling is recommended to rural and remote multidisciplinary health services because of its ability to summarise outcomes from heterogeneous service activities. [source] A language perspective to environmental management and corporate responsibilityBUSINESS STRATEGY AND THE ENVIRONMENT, Issue 4 2009Maria Joutsenvirta Abstract Few environmental management scholars have applied a research approach that focuses on analysing the language use through which managers and other societal actors come to describe, explain or otherwise account for environmental and social problems. This article discusses some of the important benefits that treating linguistic materials as ,sites of language use' offers for studying corporate responsibilities in various societal challenges such as climate change, biodiversity loss and poverty. Findings from a longitudinal discourse analysis of the debate between a leading global forest industry company (Stora Enso) and a global environmental organization (Greenpeace) demonstrate the utility of a research approach that focuses on the discussants' language use. The article shows how the application of a language perspective opens up new avenues for understanding how certain ways of talking about corporate responsibilities may hinder or facilitate our efforts to steer corporate actions into a more balanced relationship with nature and society. [source] Tumor blood flow interruption after radiotherapy strongly inhibits tumor regrowthCANCER SCIENCE, Issue 7 2008Katsuyoshi Hori To clarify the therapeutic significance of interrupting tumor blood flow after irradiation, we investigated X-irradiation-induced changes in hemodynamic parameters (blood flow, extravasation and washout of fluorescein isothiocyanate-dextran, and interstitial fluid pressure) in a variant of Yoshida sarcoma, LY80. Tumors in anesthetized male Donryu rats received local irradiation (10 Gy). At 48 h after irradiation, tumor blood flow increased significantly; at 72,96 h after irradiation, a 2,2.5-fold increase was observed. All parameters then consistently showed improved tumor microcirculation, which probably contributed to regrowth of cancer because certain cells survived irradiation. Rats received an intravenous dose (10 mg/kg) of a combretastatin derivative, AC7700 (AVE8062), which interrupts tumor blood flow and disrupts tumor vessels. At all times evaluated after irradiation, AC7700 completely stopped tumor blood flow. Radiotherapy efficacy was significantly enhanced when combined with AC7700: AC7700 given 48 h after irradiation, when tumor blood flow increased significantly, remarkably suppressed tumor regrowth compared with AC7700 given 48 h before irradiation. Also, postirradiation AC7700 completely inhibited not only primary tumor regrowth but also regional lymph node metastases in half of tumor-bearing rats and led to a significant improvement in survival. These results strongly suggest that the combination effect was enhanced via interruption of increased tumor blood flow after irradiation. This therapeutic combination and timing may have important benefits, even in tumors with low sensitivity to either treatment alone, because the effect was considerably greater than additive. Our data thus show that destruction of tumor microcirculation after irradiation is quite effective for preventing cancer recurrence. T. Holgate Summary Omalizumab is a humanized monoclonal anti-IgE antibody developed for the treatment of allergic disease, with established efficacy in patients with moderate-to-severe allergic asthma and in patients with intermittent (seasonal) and persistent (perennial) allergic rhinitis (AR). Omalizumab is known to result in a marked reduction in serum levels of free IgE and down-regulation of IgE receptors on circulating basophils. Recent work has shed further light on its mechanism of action, showing significant and profound reductions in tissue (nasal and bronchial) eosinophils and in bronchial IgE+ cells (mast cells), as well as T cells and B cells. Omalizumab treatment was also shown to be associated with down-regulation of IgE receptors on circulating (precursor) dendritic cells, suggesting that blocking IgE may inhibit more chronic aspects of allergic inflammation involving T cell activation. Further work with omalizumab demonstrated it to have important benefits in patients with poorly controlled asthma despite high-dose inhaled corticosteroid therapy, and analysis of clinical data suggests that the patients who are the best ,responders' to anti-IgE treatment are those with asthma at the more severe end of the spectrum. Notably, systemic anti-IgE therapy with omalizumab has been shown to improve symptoms, quality of life and disease control (asthma exacerbations) in patients with concomitant asthma and persistent AR. These impressive clinical data and the studies elucidating the anti-inflammatory profile of omalizumab also serve to emphasize the fundamental importance of IgE in the pathogenesis of allergic diseases.
