Abstract Objectives: To evaluate whether different implants placed immediately upon tooth extraction may affect the dimension and composition of the peri-implant soft tissues. Material and Methods: Eight beagle dogs received implants randomly installed into the distal socket of 3P3 and 4P4. Four commercially available implant systems were evaluated: 3i Osseotite Certain straight; Astra MicroThreadÔ -OsseoSpeedÔ; Thommen SPI Element®; and Straumann ITI standard. Each animal provided four test implant sites. All animals were sacrificed 6 weeks after implant placement, providing specimens for the evaluation of the soft tissue dimensions by histometric analysis. Results: The biological width at 6 weeks after implant placement consisted of a junctional epithelium measuring between 2 and 2.7 mm and a connective tissue component between 1 and 1.8 mm with no statistical differences among the four implant systems. Conclusion: This study failed to demonstrate differences in the soft tissue healing outcome when placing four different implant systems into fresh extraction sockets. Nevertheless, the length of the epithelium achieved with the four implant systems is longer than what has been reported when placing implants in healed-ridge experimental models. Each animal provided four test implant sites. All animals were sacrificed at 6 weeks after implant placement, providing specimens for histo-morphometric analysis of bone to implant contact (BIC), bone area, new bone formation, as well as histometric measurements of the ridge alterations. Results: No statistically significant difference was observed among the four-implant systems. The mean BIC % ranged between 58.5% and 72.1%. Bone modelling of the buccal plate was marked and amounted approximately to 2.5 mm, independently of the system used. Conclusion: This study failed to demonstrate differences in the healing pattern after 6 weeks when placing four different implant systems in fresh extraction sockets. In spite of achieving predictable osteointegration with the four implants studied, the occurrence of buccal bone resorption may limit the use of this surgical approach. Future developments should aim at improving the benefits to patients by decreasing treatment time, minimizing surgical stages, and maximizing esthetic outcomes. Using knowledge from studies of immediate implant placement, one-stage, immediate loading protocols, the authors developed the immediate provisional. Purpose: The purpose of this study was to develop a protocol to provide an immediate solution for restoring a single missing tooth in the esthetic zone. The protocol should be simple, predictable, cost effective, and allow the use of other techniques to improve esthetic outcome. Materials and Method: This prospective clinical study included 24 patients treated from August 1999 to October 2000. Single-tooth implant replacement was done according to immediate provisional protocol. Thirteen of the 24 patients had immediate implant placement after tooth extraction. All implants were placed in the esthetic zone. During surgery, emphasis was placed on obtaining primary stability by achieving bicortical anchorage and maximum insertion torque of at least 40 Ncm. CeraOne (Nobel Biocare) abutments were used, and provisional crowns were fabricated immediately before wound closure. The occlusion was protected by adjacent teeth. Results: Within the follow-up period of between 1 month and 15 months, all fixtures in the 24 patients were stable. Crestal bone loss greater than one thread-width was not detected. The esthetic result was considered satisfactory by all patients. Conclusions: The implant placement and restoration protocol used in this study showed promising initial results for both the immediate implant and healed extraction site groups. The desirable goals of patient satisfaction, excellent esthetic outcomes, and no increase in treatment cost were achieved in this protocol. Further studies to elucidate the potential of the immediate provisional protocol are justified. [source] Early cortical bone healing around loaded titanium implants: a histological study in the rabbitCLINICAL ORAL IMPLANTS RESEARCH, Issue 2 2009Elke Slaets Abstract Objectives: To identify the role of immediate implant loading on the early phases of the bone healing responses. Material and methods: Implants were placed in rabbit tibial diaphyses and left to heal for 3, 7, 14, 28 or 42 days. Half of the animals received an immediate loading protocol of 2.2 N at 3 Hz for 1800 cycles and 5 days/week, whereas the others served as unloaded controls. Histological assessment was combined with histomorphometrical measurements. Results: At early time-points, an endosteal and periosteal new bone formation was found, while the cortex itself contained damaged osteocytes. At later time-points, new bone formation was also found at the cortical level itself. Differences between groups were found mainly in this new bone formation process, with larger reactions for the endosteal and periosteal bone in the loaded group after 28 and 42 days, respectively. At the end-point of the experiment, bone formation at the cortical level was reduced in the loaded group compared with the control group. Conclusions: These results show that the immediate loading protocol caused no differences in the sequential events leading to osseointegration in cortical bone. However, the processes of new bone formation originating from the endosteum and the periosteum lasted longer compared with the unloaded controls. Bianchi Abstract Objectives: The aim of the present study was to evaluate the long-lasting efficacy of a combined surgical protocol, using immediate implant and subepithelial connective tissue graft for single-tooth replacement. The advantages of this single-center, longitudinal, randomized, blind examiner research were the following: preservation of both keratinized mucosa amount and bone tissue, optimal peri-implant marginal sealing, satisfactory aesthetic results, reduction in treatment time. Materials and methods: In the time period from 1990 to 1998, 116 patients were consecutively admitted for treatment with a total of 116 solid screw ITI-implants supporting single crowns. Ninety-six patients underwent the proposed combined treatment (test group), while 20 received only single immediate implants (control group). The observation time extended from 1 up to 9 years. Results: The 9-year cumulative survival rate was 100% for both test and control groups. Comparative statistical analysis of soft and hard tissue peri-implant parameters demonstrated better results in the test group than in the control during every single 3-year analysis and especially in the last observation interval. The test group also showed very good results in terms of aesthetic parameters, which estimated the keratinized mucosa width, the alignment of crown emergence profile and the patient's satisfaction. Conclusion: Single-tooth replacement by immediate solid screw ITI implants in association with connective tissue autograft was demonstrated to be a predictable procedure. Moreover, this treatment can be considered as a sure system to reach an excellent functional and harmonious aesthetic restoration. Résumé Le but de l'étude présente a été d'évaluer l'efficacitéà long terme d'un protocole chirurgical combiné utilisant l'implant placé immédiatement et un greffon tissulaire de tissu conjonctif sous-épithélial pour le remplacement d'une dent unique. Les avantages de cette recherche randomisée longitudinale dans un seul centre et en aveugle étaient les suivants : préservation de la quantité de muqueuse kératinisée et du tissu osseux, une fermeture marginale paroïmplantaire optimale, des résultats esthétiques satisfaisants et une réduction du temps de traitement. Durant les années 1990 à 1998, 116 patients ont été admis pour le traitement de 116 implants ITI en vis pleines portant des couronnes uniques. Nonante-six patients ont recu le traitement proposé (groupe test) tandis que 20 autres n'ont reçu que des implants placés immédiatement en une étape (groupe contrôle). Le temps d'observation s'échelonnait de un à neuf ans. Le taux de survie cumulatif à neuf années était de 100% dans les deux groupes. L'analyse statistique comparative des tissus mous et durs paroïmplantaires a constaté des meilleurs résultats dans le groupe test que dans le groupe contrôle durant chaque analyse de trois ans et spécialement dans la dernière période d'observation. Le groupe test montrait également de très bons résultats esthétiques concernant la largeur de la muqueuse kératinisée, l'alignement du profil de l'émergence de la couronne et la satisfaction du patient. Die Vorteile dieser an einem Zentrum durchgeführten randomisierten, longitudinalen Blindstudie waren die folgenden: Erhaltung sowohl der gesamten keratinisierten Mukosa wie auch des Knochengewebes, der optimale perimplantäre marginale Abschluss, die befriedigenden ästhetischen Resultate und die Verkürzung der Behandlungszeit. Material und Methoden: In der Zeit von 1990 bis 1998 bekamen in der Folge 116 Patienten insgesamt 116 ITI-Vollschraubenimplantate, die mit Einzelkronen versorgt wurden. 96 Patienten wurden nach der vorgestellten kombinierten Methode behandelt (Testgruppe), währenddem die übrigen 20 Patienten einzig das Sofortimplantat erhielten (Kontrollgruppe). Zusammenfassung: Der Einzelzahnersatz mittels Sofortimplantation einer ITI-Vollschraube in Verbindung mit einem freien Bindegewebetransplantat erwies sich als gut beherrschbare Methode. Man kann diese Behandlung sogar als sicher für den Erhalt einer hervorragenden funktionellen, harmonischen und ästhetischen Rekonstruktion empfehlen. Las ventajas de esta investigación unicentro, longitudinal, aleatoria, examinador ciego fueron las siguientes: preservación de tanto de la cantidad de mucosa queratinizada como del tejido óseo, sellado marginal periimplantario óptimo, resultados estéticos satisfactorios, reducción del tiempo del tratamiento. Material y métodos: En el periodo de tiempo desde 1990 a 1998, se admitieron 116 pacientes consecutivos para tratamiento con un total de 116 implantes macizos roscados ITI soportando coronas unitarias. Noventa y seis pacientes se sometieron al tratamiento propuesto (grupo de prueba), mientras 20 recibieron solo implantes inmediatos unitarios (grupo de control). El tiempo de observación se extendió de 1 9 años. Resultados: El índice acumulado de supervivencia a los 9 años fue del 100% tanto para el grupo de prueba como el de control. El análisis estadístico comparativo de los parámetros de los tejidos blandos y duros periimplantarios demostró mejores resultados en el grupo de prueba que en el de control, durante cada análisis de 3 años y especialmente en el último intervalo de observación. El grupo de prueba también mostró muy buenos resultados en términos de parámetros estéticos, que estimaron la anchura de la mucosa queratinizada, la alineación del perfil de emergencia de la corona y la satisfacción del paciente. Conclusión: La sustitución de un diente unitario por implantes macizos roscados inmediatos ITI en asociación con autoinjertos de tejido conectivo demostró ser un procedimiento predecible. Más aun, este tratamiento puede ser considerado como un sistema seguro para alcanzar una excelente restauración estética funcional y armónica. [source] Collagen membranes at immediate implants: a histomorphometric study in dogsCLINICAL ORAL IMPLANTS RESEARCH, Issue 9 2010Marco Caneva Abstract Aim: To evaluate the influence of resorbable membranes on hard tissue alterations and osseointegration at implants placed into extraction sockets in a dog model. Material and methods: In the mandibular premolar region, implants were installed immediately into the extraction sockets of six Labrador dogs. Collagen-resorbable membranes were placed at the test sites, while the control sites were left uncovered. Implants were intended to heal in a submerged mode. After 4 months of healing, the animals were sacrificed, and ground sections were obtained for histomorphometric evaluation. Results: After 4 months of healing, a control implant was not integrated (n=5). Both at the test and at the control sites, bone resorption occurred. While the most coronal bone-to-implant contact was similar between the test and the control sites, the alveolar bone crest outline was maintained to a higher degree at the buccal aspect of the test sites (loss: 1.7 mm) compared with the control sites (loss: 2.2 mm). Conclusions: The use of collagen-resorbable membranes at implants immediately placed into extraction sockets contributed to a partial (23%) preservation of the buccal outline of the alveolar process. Chen Abstract Objectives: To evaluate healing of marginal defects in immediate transmucosal implants grafted with anorganic bovine bone, and to assess mucosal and radiographic outcomes 3,4 years following restoration. Material and methods: Thirty immediate transmucosal implants in maxillary anterior extraction sites of 30 patients randomly received BioOssÔ (N=10; BG), BioOssÔ and resorbable collagen membrane (N=10; BG+M) or no graft (N=10; control). Results: Vertical defect height (VDH) reductions of 81.2±5%, 70.5±17.4% and 68.2±16.6%, and horizontal defect depth (HDD) reductions of 71.7±34.3%, 81.7±33.7% and 55±28.4% were observed for BG, BG+M and control groups, respectively, with no significant inter-group differences. Horizontal resorption was significantly greater in control group (48.3±9.5%) when compared with BG (15.8±16.9%) and BG+M (20±21.9%) groups (P=0.000). Ten sites (33.3%) exhibited recession of the mucosa after 6 months; eight (26.7%) had an unsatisfactory esthetic result post-restoration due to recession. Mucosal recession was significantly associated (P=0.032) with buccally positioned implants (HDD 1.1±0.3 mm) when compared with lingually positioned implants (HDD 2.3±0.6 mm). In 19 patients followed for a mean of 4.0±0.7 years, marginal mucosa and bone levels remained stable following restoration. Conclusion: BioOssÔ significantly reduced horizontal resorption of buccal bone. There is a risk of mucosal recession and adverse soft tissue esthetics with immediate implant placement. However, this risk may be reduced by avoiding a buccal position of the implant in the extraction socket. [source] Implants in fresh extraction sockets: a prospective 5-year follow-up clinical studyCLINICAL ORAL IMPLANTS RESEARCH, Issue 12 2008Daniele Botticelli Abstract Objective: The aim of this prospective study was to evaluate the 5-year clinical outcome of the ,immediate implants.' Material and methods: One week after the cementation of the prosthesis, a clinical baseline examination was carried out. Clinical measurements were performed of the following: plaque, mucositis, probing pocket depth, and soft tissue position. The height of the keratinized mucosa was measured at the buccal/lingual aspects. Standardized intra-oral radiographs were taken. The marginal level of bone to implant contact [radiographic (Rx) bone level] was measured, and Rx bone level change over time was evaluated. The clinical/radiographic measurements were repeated on a yearly basis. The subjects were enrolled in a carefully supervised oral hygiene program. Results and conclusion: It was demonstrated that ,immediate implants' that were loaded after 5,7 months had a high success rate. During the 5-year interval, no implant was lost, and the mean Rx bone level at the implants was maintained or even improved. The plaque and mucositis scores were low (<20%) at baseline and at all re-examinations. Implant sites located adjacent to the teeth showed bone gain during the initial period while sites that were facing edentulous zones lost some bone. [source] |
