Selected AbstractsEpidemiology of gallstone disease in Chandigarh: A community-based studyJOURNAL OF GASTROENTEROLOGY AND HEPATOLOGY, Issue 5 2001Virendra Singh Abstract Background: Cholelithiasis is frequent in our country (India), and is a common cause for abdominal surgery. We studied the prevalence of gallstone disease and its association with various risk factors in the city of Chandigarh. Methods: A house-to-house survey of residents aged 15 years or above in a subsector of Chandigarh was conducted as a part of a pilot survey. All individuals with a history of gallstone disease, and equal or more number of asymptomatic individuals were asked to attend the outpatient department of the Postgraduate Medical Institute. On the basis of the pilot survey, the sample size to be studied was 2648 and we screened 2649 persons. Results: Two hundred and fifty-four individuals attended the outpatient services of the Institute, and 248 underwent an ultrasound. There were 37 symptomatic and 211 asymptomatic individuals (male : female, 93:155). Gallstone disease was seen in 24 out of 37 (64.9%) in the symptomatic, and seven out of 211 (3.3%) in the asymptomatic group. Out of these, 27 females and four males had gallstone disease (mean age: 48.30 ± 16.03 years). Approximately 67% of patients were between 20 and 60 years of age. Gallstone disease was more frequently seen in those from high socioeconomic status as compared to middle socioeconomic status (,2 = 8.9, d.f. = 1, P < 0.01), and in multiparous as compared to nulliparous females (,2 = 4.8, d.f. = 1, P < 0.05). Body mass index, smoking, alcohol consumption, and a vegetarian/non-vegetarian diet did not influence the prevalence of gallstone disease. Conclusions: Gallstone disease is frequent in northern India. Gallstones were frequent in those belonging to high socioeconomic status and multiparous females. However, body mass index, smoking, alcohol or other dietary habits did not make a difference. Nascimento-Carvalho MD Abstract Two different socioeconomic groups of children with pneumonia were studied, and their clinical and demographic aspects were evaluated. The diagnosis of pneumonia was based on findings of cough and tachypnea, or on crackles on auscultation or on radiologically confirmed infiltrate. This was a prospective cross-sectional study conducted at the Professor Hosannah de Oliveira Pediatric Center, which cares for children of lower socioeconomic status (PHOPC), and at one private hospital which cares for children from middle to high socioeconomic status (Aliança Hospital, AH). Demographics and clinical differences were assessed by the Pearson chi-square test or Fisher's exact test as appropriate; means of continuous variables were compared by Mann-Whitney U-test. In a 26-month period, 3,431 cases were recruited. The 2,476 cases identified at the PHOPC were younger than the 955 identified at AH (2.2,±,2.3 vs. 4.5,±,3.1 years, P,<,0.0001) and had higher scores for severity (3.5,±,1.5 vs. 2.7,±,1.7, P,<,0.0001), duration of hospitalization (days) (10.9,±,12.1 vs. 6.2,±,7, P,<,0.0001), frequency of tobacco smoker in the household (48% vs. 31%, P,<,0.0001), cardiopathy (15.3% vs. 5.9%, P,=,0.003), fever (44.4% vs. 36.3%, P,=,0.0001), tachypnea (67.6% vs. 32.3%, P,<,0.0001), crackles (69.5% vs. 64.9%, P,=,0.02), somnolence (19.9% vs. 10.4%, P,<,0.0001), malnutrition (13.7% vs. 5%, P,<,0.0001), hospitalization rate (27.4% vs. 22.5%, P,=,0.003), and death (0.9% vs. 0.1%, P,=,0.009). However, other features were more frequent among AH cases: parent's university level of education (38.2% vs. 1.0%, P,<,0.0001), underlying chronic illness (40.6% vs. 28.5%, P,<,0.0001), asthma (62.7% vs. 50.8%, P,=,0.01), rhinitis (9.2% vs. 0.4%, P,<,0.0001), previous use of antibiotics (34.3% vs. 27.1%, P,=,0.001), and wheezing (53.1% vs. 42.2%, P,<,0.0001). Children of lower socioeconomic status have more serious lower respiratory tract disease, whereas children with pneumonia of middle to high socioeconomic status have more allergic diseases (rhinitis, asthma) and wheezing. Van Hattem Summary Background, Development of both basal cell carcinoma (BCC) and cutaneous malignant melanoma (MM) is associated with acute and intermittent sun exposure. In contrast to MM, the association between socioeconomic status (SES) and BCC is not well documented. Objectives, To investigate the incidence of BCC according to SES, stratifying by age and tumour localization in a large population-based cohort. To assess changes over time in the distribution of the patients with BCC across the SES categories. Methods, All patients with a histologically confirmed first primary BCC (n = 27,027) diagnosed between 1988 and 2005 in the Southeast of the Netherlands were stratified by sex, age (25,44, 45,64 and , 65 years), period of diagnosis, SES category (based on income and value of housing) and localization of the BCC. Age-standardized BCC incidence rates were calculated for the year 2004 by SES category and localization. Ordinal regression was used to assess changes over time in the proportion of patients with BCC by sex, age and SES. Results For men in all age groups higher BCC incidence in the highest SES category was observed, which remained significant after stratification for tumour localization. For women a consistent relationship was found only in younger women (< 65 years) for truncal BCCs, which occurred more frequently in high SES groups. Between 1990 and 2004, the proportion of BCC patients with high SES increased (+6%) and the proportion with low SES decreased (,7%). Conclusions, High SES is associated with increased incidence of BCC among men. Our data suggest that BCC is changing from a disease of the poor to a disease of the rich. Franco Abstract , Objective: To obtain information on the level of total fluoride intake from food, beverages and toothpaste by children at the age of 22,25 months of low and high socioeconomic status (SES) in major Colombian cities. Methods:, Daily fluoride intake was assessed by the duplicate plate method and by recovered toothpaste solution during a 3-day period and afterwards analysed by the microdiffusion method. Results:, Mean daily fluoride intake was 0.11 (±0.10), 0.14 (±0.12), 0.10 (±0.07) and 0.07 (±0.06) mg/kg body weight (bw)/day in Bogotá, Medellín, Manizales and Cartagena, respectively. The total fluoride intake was higher in low-SES subjects in the cities of Medellín and Bogotá. In the high-SES children of the four cities, the average intakes ranged from 0.06 to 0.09 mg F/kg bw, whereas, the low-SES children in three cities had intakes between 0.11 and 0.21 mg F/kg bw (Cartagena, 0.07). Toothpaste (containing 1000,1500 ppm F, with 1500 ppm F being more common) accounted for approximately 70% of total fluoride intake, followed by food (24%) and beverages (<6%). More than half the children had their teeth brushed by an adult, on average twice a day, using 0.22,0.65 g of toothpaste. Conclusion:, Children from three Colombian cities have a mean total daily fluoride intake above the ,optimal range'. Health authorities should promote an appropriate use of fluoridated dentifrices discouraging the use of dentifrices containing 1500 ppm F in children younger than 6 years of age and promoting a campaign of education of parents and oral health professionals on adequate toothbrushing practices.
