Androgens and hair growth

ABSTRACT:, Hair's importance in human communication means that abnormalities like excess hair in hirsutism or hair loss in alopecia cause psychological distress. Androgens are the main regulator of human hair follicles, changing small vellus follicles producing tiny, virtually invisible hairs into larger intermediate and terminal follicles making bigger, pigmented hairs. The response to androgens varies with the body site as it is specific to the hair follicle itself. Normally around puberty, androgens stimulate axillary and pubic hair in both sexes, plus the beard, etc. in men, while later they may also inhibit scalp hair growth causing androgenetic alopecia. Androgens act within the follicle to alter the mesenchyme,epithelial cell interactions, changing the length of time the hair is growing, the dermal papilla size and dermal papilla cell, keratinocyte and melanocyte activity. Greater understanding of the mechanisms of androgen action in follicles should improve therapies for poorly controlled hair disorders like hirsutism and alopecia. [source]


Qualitative Data Analysis for Health Services Research: Developing Taxonomy, Themes, and Theory

Objective. To provide practical strategies for conducting and evaluating analyses of qualitative data applicable for health services researchers. Data Sources and Design. We draw on extant qualitative methodological literature to describe practical approaches to qualitative data analysis. Approaches to data analysis vary by discipline and analytic tradition; however, we focus on qualitative data analysis that has as a goal the generation of taxonomy, themes, and theory germane to health services research. Principle Findings. We describe an approach to qualitative data analysis that applies the principles of inductive reasoning while also employing predetermined code types to guide data analysis and interpretation. These code types (conceptual, relationship, perspective, participant characteristics, and setting codes) define a structure that is appropriate for generation of taxonomy, themes, and theory. Conceptual codes and subcodes facilitate the development of taxonomies. Relationship and perspective codes facilitate the development of themes and theory. Intersectional analyses with data coded for participant characteristics and setting codes can facilitate comparative analyses. Conclusions. Qualitative inquiry can improve the description and explanation of complex, real-world phenomena pertinent to health services research. Greater understanding of the processes of qualitative data analysis can be helpful for health services researchers as they use these methods themselves or collaborate with qualitative researchers from a wide range of disciplines. Nurses' role in detecting deterioration in ward patients: systematic literature review

Abstract Title.,Nurses' role in detecting deterioration in ward patients: systematic literature review. Aim., This paper is a report of a review conducted to identify and critically evaluate research investigating nursing practice in detecting and managing deteriorating general ward patients. Background., Failure to recognize or act on deterioration of general ward patients has resulted in the implementation of early warning scoring systems and critical care outreach teams. The evidence of effectiveness of these systems is unclear. Possible mechanisms for low effectiveness may be inconsistent recording of patient observations by ward staff, or inconsistent application of ,calling criteria' for outreach teams, even when observations have been recorded. Methods., The literature was searched between 1990 and 2007 using four sources: electronic databases, reference lists, key reports and experts in the field. Three broad search categories were used: nursing observations, physiological deterioration and general ward patients. All research designs describing nursing observations (vital signs) on deteriorating adult patients in general hospital wards were included. Results., Fourteen studies met the inclusion and quality criteria. The findings were grouped into four main themes: recognition; recording and reviewing; reporting; and responding and rescuing. The main findings suggest that intuition plays an important part in nurses' detection of deterioration, and vital signs are used to validate intuitive feelings. The process is highly complex and influenced by many factors, including the experience and education of bedside nurses and their relationship with medical staff. Conclusion., Greater understanding of the context within which deterioration is detected and reported will facilitate the design of more effective education and support systems. [source]


Chronic sorrow in parents of children with type 1 diabetes

Abstract Title.,Chronic sorrow in parents of children with type 1 diabetes. Aim., This paper reports on a study exploring parents' longer-term experiences of having a child with type 1 diabetes. Background., Parents of children with type 1 diabetes may experience a grief reaction at diagnosis similar to that normally associated with bereavement, but little is known about their long-term emotional adaptation. Chronic sorrow, a sustained but intermittent grief reaction, is identified in adults with diabetes but has not previously been explored in relation to parents. Methodology., In-depth interviews were conducted in 2007 with a convenience sample of 17 parents of children with type 1 diabetes 7,10 years after diagnosis. Data were explored within a theoretical framework of grief, loss, adaptation, and change. Findings., Parents had adapted to the needs of diabetes management but most had not ,come to terms' with the diagnosis. They experienced a resurgence of grief at critical times during their child's development and some, particularly mothers, became upset during their interviews, even though these took place 7,10 years after their child's diagnosis. Mothers elaborated more on their emotions than fathers, but continuing feelings associated with grief, such as anger and guilt, were expressed by both fathers and mothers. Conclusion., Greater understanding of parents' long-term emotional responses and recognition that grief may never resolve in these parents may enable healthcare professionals to provide appropriate and timely support at critical times. The multidimensional nature of biodiversity and social dynamics and implications for contemporary rural livelihoods in remote Kalahari settlements, Botswana

Abstract Despite rapid socio-economic development in Kalahari drylands, contemporary research suggests that biodiversity remains important as a component of the complex portfolio of livelihood strategies, as a real and perceived safety net in times of stress, and a key factor of cultural identity. The degree to which the spatially and temporally dynamic nature of biodiversity in drylands influences livelihoods is, however, little studied, particularly in socially complex contemporary rural settlements. Greater understanding in this area is required to allow better-informed design and implementation of rural development, poverty alleviation and conservation initiatives. This is particularly true in the light of predicted increases in environmental dynamism with climate change. An interdisciplinary approach was used in two environmentally and socially distinct dryland settlements in Botswana, to investigate the extent to which the dynamic biodiverse setting influences contemporary rural livelihoods. Results illustrate that biodiversity, particularly its dynamics, is of critical contemporary importance to rural settlement livelihoods, particularly in times of inner settlement scarcity. Entitlements to biodiversity dynamics were, however, bound up by complex settlement-specific social, economic and political factors. Unless such contextual within-settlement dynamics are understood, the relative importance of biodiversity in rural development and poverty alleviation strategies in contemporary Kalahari drylands may be undermined. A systems analysis of soil and forest degradation in a mid-hill watershed of Nepal using a bio-economic model

Abstract Forest degradation, manifested through decline in forest cover, and the resulting soil erosion and organic carbon losses, is a serious problem caused by a complex coupling of bio-physical, socio-economic and technological factors in the Himalayan watersheds. Greater understanding of the linkages between these factors requires a systems approach. We have proposed such an approach using a bio-economic model to explore the system behaviour of forest degradation, soil erosion, and soil C losses in the forest areas. The outcome of the model simulation over a 20-year period indicates that soil erosion and C loss rates may increase more than four-fold by the year 2020 under the existing socio-economic and biophysical regime (the base scenario). Reductions in the population growth rate, introduction of improved agricultural technology and increase in the prices of major agricultural crops can help slow down the rates of forest decline, soil erosion and C loss or even stabilize or reverse them. The results suggest that economic incentives may be highly effective in the reduction of soil loss, as well as C release to the atmosphere. Copyright © 2005 John Wiley & Sons, Ltd. [source]


Anatomical variations of the plantaris muscle and a potential role in patellofemoral pain syndrome

Abstract The plantaris muscle has been given little attention in the reviewed literature. It is most commonly mentioned only when absent from a specimen. This study aimed to document the anatomy of the plantaris muscle and to discuss the clinical significance of the observations. Cadaveric knees (n = 46) were dissected to identify the possible variations of the plantaris muscle. The muscle conformed with standard descriptions (n = 26; 56.52%), was present but varied from previous descriptions (n = 14; 30.44%), or was absent (n = 6; 13.04%). The variations consisted of distinct interdigitations with the lateral head of the gastrocnemius muscle (n = 9; 19.57%) and a strong fibrous extension of the plantaris muscle to the patella (n = 5; 10.87%). The presence of interdigitations strengthen the argument that the plantaris muscle supplement the activity of the lateral head of the gastrocnemius muscle whereas the patellar extension suggests an involvement with patellofemoral dynamics and may play a role in the various presentations of patellofemoral pain syndrome. Greater understanding of the relationship between these and other posterior knee structures will facilitate more precise interpretation and treatment of knee injuries. Clin. Curriculum-Context Knowledge: Teacher Learning From Successive Enactments of a Standards-Based Mathematics Curriculum

ABSTRACT This study characterizes the teacher learning that stems from successive enactments of innovative curriculum materials. This study conceptualizes and documents the formation of curriculum-context knowledge (CCK) in three experienced users of a Standards-based mathematics curriculum. I define CCK as the knowledge of how a particular set of curriculum materials functions to engage students in a particular context. The notion of CCK provides insight into the development of curricular knowledge and how it relates to other forms of knowledge that are relevant to the practice of teaching, such as content knowledge and pedagogical content knowledge. I used a combination of video-stimulated and semistructured interviews to examine the ways the teachers adapted the task representations in the units over time and what these adaptations signaled in terms of teacher learning. Each teacher made noticeable adaptations over the course of three or four enactments that demonstrated learning. Each of the teachers developed a greater understanding of the resources in the respective units as a result of repeated enactments, although there was some important variation between the teachers. The learning evidenced by the teachers in relation to the units demonstrated their intricate knowledge of the curriculum and the way it engaged their students. Furthermore, this learning informed their instructional practices and was intertwined with their discussion of content and how best to teach it. The results point to the larger need to account for the knowledge necessary to use Standards-based curricula and to relate the development and existence of well-elaborated knowledge components to evaluations of curricula. Enhancing Knowledge Transfer in Classroom Versus Online Settings: The Interplay Among Instructor, Student, Content, and Context

ABSTRACT This article integrates management education and organizational learning theories to identify the factors that drive the differences in student outcomes between the online and classroom settings. We draw upon theory on knowledge transfer barriers in organizations to understand the interlinking relationships among presage conditions, deep learning process, and product in the 3P model of student learning. We test our model in the context of undergraduate education and find that confidence in the instructor's expertise, perceived content relevance, and the social richness of the classroom learning environment enhance student enjoyment of the course. Confidence in instructor's expertise and perceived content relevance also contribute to greater understanding of causal relationships among course concepts. Enjoyment is positively associated with learning performance in the classroom, but not online, and student ability is positively associated with learning performance in the online context, but not in the classroom. Our results have implications for course designs in the traditional classroom context and the more innovative online environment. [source]


The role of fruit traits of bird-dispersed plants in invasiveness and weed risk assessment

Abstract Aim, Birds play a major role in the dispersal of seeds of many fleshy-fruited invasive plants. The fruits that birds choose to consume are influenced by fruit traits. However, little is known of how the traits of invasive plant fruits contribute to invasiveness or to their use by frugivores. We aim to gain a greater understanding of these relationships to improve invasive plant management. Location, South-east Queensland, Australia. Methods, We measure a variety of fruit morphology, pulp nutrient and phenology traits of a suite of bird-dispersed alien plants. Frugivore richness of these aliens was derived from the literature. Using regressions and multivariate methods, we investigate relationships between fruit traits, frugivore richness and invasiveness. Results, Plant invasiveness was negatively correlated to fruit size, and all highly invasive species had quite similar fruit morphology [smaller fruits, seeds of intermediate size and few (< 10) seeds per fruit]. Lower pulp water was the only pulp nutrient trait associated with invasiveness. There were strong positive relationships between the diversity of bird frugivores and plant invasiveness, and in the diversity of bird frugivores in the study region and another part of the plants' alien range. Main conclusions, Our results suggest that weed risk assessments (WRA) and predictions of invasive success for bird-dispersed plants can be improved. Scoring criteria for WRA regarding fruit size would need to be system-specific, depending on the fruit-processing capabilities of local frugivores. Frugivore richness could be quantified in the plant's natural range, its invasive range elsewhere, or predictions made based on functionally similar fruits. [source]


The spatial distribution of badgers, setts and latrines: the risk for intra-specific and badger-livestock disease transmission

The spatial distribution of wildlife hosts and the associated environmental distribution of their excretory products are important factors associated with the risk of disease transmission between wildlife and livestock. At a landscape scale, heterogeneous distribution of a wildlife host will create regional hot spots for disease risk, while at the farm level, distributional patterns of wildlife excretory products as well as habitat use are of primary importance to the assessment of disease risk to livestock. In the UK, badgers have been implicated in the transmission of bovine tuberculosis to cattle. In this study, we focus on the spatial and social organization and habitat use of badgers as well as the distributions of their excretions at latrine and sett sites to assess intra- and inter-species (badger,cattle) disease risk. Across the study site, badger latrines and setts were found in prominent clusters, at distances of up to 250 and 200 m respectively. This was partly due to small-scale clustering of latrines around sett sites, so that disease risk may be higher within the vicinity of setts. The clustered distribution suggests that sites of high risk for TB transmission may be localised within farms. Exclusion of cattle from the few sett and latrine sites within their grazing pasture is therefore likely to provide an effective way of reducing the risk of disease transmission. We also found evidence of social sub-division within badger social groups based on differences in the use of main and outlier setts. This may contribute to localised clusters of infection within the badger population, resulting in heterogeneous patterns of environmental disease risk to the wider host community. A greater understanding of variation in host behaviour and its implications for patterns of disease will allow the development of more targeted and effective management strategies for wildlife disease in group-living hosts. Unplanned attempts to quit smoking: a qualitative exploration

ABSTRACT Aims To gain a greater understanding of the process of unplanned attempts to quit smoking and the use of support in such attempts. Design Qualitative study using semi-structured interviews with 20 smokers and ex-smokers. Setting Twenty-four general practices in Nottinghamshire, UK. Participants Smokers and ex-smokers who reported that their most recent attempt to quit smoking was unplanned. Measurements Descriptions of the unplanned quit attempts and reported use of support within these. Findings Smokers who report making ,unplanned' quit attempts exhibit substantial variation in what they mean by this; many quit attempts reported as ,unplanned' were actually delayed and involved some planning and use of cessation support. Conclusions Reported ,unplanned' quit attempts often involve elements of planning and delay for quitters to access to cessation support. It is important, therefore, that smoking cessation services offer flexible and adaptable support which can be used readily by potential quitters. [source]


Why is the choice of future climate scenarios for species distribution modelling important?

Abstract Species distribution models (SDMs) are common tools for assessing the potential impact of climate change on species ranges. Uncertainty in SDM output occurs due to differences among alternate models, species characteristics and scenarios of future climate. While considerable effort is being devoted to identifying and quantifying the first two sources of variation, a greater understanding of climate scenarios and how they affect SDM output is also needed. Climate models are complex tools: variability occurs among alternate simulations, and no single ,best' model exists. The selection of climate scenarios for impacts assessments should not be undertaken arbitrarily - strengths and weakness of different climate models should be considered. In this paper, we provide bioclimatic modellers with an overview of emissions scenarios and climate models, discuss uncertainty surrounding projections of future climate and suggest steps that can be taken to reduce and communicate climate scenario-related uncertainty in assessments of future species responses to climate change. Linking habitat selection, emigration and population dynamics of freshwater fishes: a synthesis of ideas and approaches

Abstract,,, The consequences of individual behaviour to dynamics of populations has been a critical question in fish ecology, but linking the two has proven difficult. A modification of Sale's habitat selection model provides a conceptual linkage for relating resource availability and individual habitat selection to exploratory behaviour, emigration and population-level responses. Whole-population experiments with pupfish Cyprinodon macularius that linked all factors along this resource to population continuum lend support to this conceptual model, and illustrate that emigration may be much more common in fish populations than considered in most individual- or population-based models. Accommodating emigration can enhance the ecological appropriateness of behavioural experiments and increase confidence in extrapolation of experimental observations to population-level effects. New experimental designs and advancing technologies offer avenues for assessing population consequences of habitat selection and emigration by individual fish. Emigration often is the key linkage between individual behaviour and population responses, and greater understanding of the underlying factors affecting this often-overlooked demographic parameter could offer new approaches for management and conservation of fishes. Based on a literature review and the articles in this special issue we make three recommendations that we believe will allow theory to advance and be applicable in practice. First, that discourse between key stakeholders is encouraged in order to achieve greater understanding. Second, that focus is placed on "growth as a process," rather than as a "change in amount." A compendium of human mitochondrial gene expression machinery with links to disease

Abstract Mammalian mitochondrial DNA encodes 37 essential genes required for ATP production via oxidative phosphorylation, instability or misregulation of which is associated with human diseases and aging. Other than the mtDNA-encoded RNA species (13 mRNAs, 12S and 16S rRNAs, and 22 tRNAs), the remaining factors needed for mitochondrial gene expression (i.e., transcription, RNA processing/modification, and translation), including a dedicated set of mitochondrial ribosomal proteins, are products of nuclear genes that are imported into the mitochondrial matrix. Herein, we inventory the human mitochondrial gene expression machinery, and, while doing so, we highlight specific associations of these regulatory factors with human disease. Major new breakthroughs have been made recently in this burgeoning area that set the stage for exciting future studies on the key outstanding issue of how mitochondrial gene expression is regulated differentially in vivo. This should promote a greater understanding of why mtDNA mutations and dysfunction cause the complex and tissue-specific pathology characteristic of mitochondrial disease states and how mitochondrial dysfunction contributes to more common human pathology and aging. The molecular determinants of sunburn cell formation

Abstract: Sunburn cell (SBC) formation in the epidermis is a characteristic consequence of ultraviolet radiation (UVR) exposure at doses around or above the minimum erythema dose. SBC have been identified morphologically and biologically as keratinocytes undergoing apoptosis. There is evidence that SBC formation is a protective mechanism to eliminate cells at risk of malignant transformation. The level of DNA photodamage is a major determinant of SBC induction by a process controlled by the tumor suppressor gene p53. However, extra-nuclear events also contribute to SBC formation, such as the activation of death receptors including CD95/Fas. UVR triggers death receptors either by direct activation of these surface molecules or by inducing the release of their ligands such as CD95 ligand or tumor necrosis factor. Oxidative stress also appears to be involved, probably via mitochondrial pathways, resulting in the release of cytochrome C. Pathways which modify SBC formation are now extensively studied given the importance of apoptosis in eliminating irreparably damaged cells. A greater understanding of the mechanisms that induce and prevent UVR-induced apoptosis will contribute to our understanding of mechanisms relevant in genomic integrity. [source]


Anomalous Oxidation States in Multilayers for Fuel Cell Applications

Abstract Significant recent interest has been directed towards the relationship between interfaces and reports of enhanced ionic conductivity. To gain a greater understanding of the effects of hetero-interfaces on ionic conductivity, advanced analytical techniques including electron microscopy (TEM/STEM), electron energy loss spectroscopy (EELS), and secondary ion mass spectrometry (SIMS) are used to characterize CeO2/Ce0.85Sm0.15O2 multilayer thin films grown by pulsed laser deposition. High quality growth is observed, but ionic conductivity measured by impedance spectroscopy and 18O tracer experiments is consistent with bulk materials. EELS analysis reveals the unusual situation of layers containing only Ce(IV) adjacent to layers containing both Ce(III) and Ce(IV). Post oxygen annealing induced oxygen diffusion and mixed oxidation states in both layers, but only in the vicinity of low angle grain boundaries perpendicular to the layers. The implications of the anomalous behavior of the Ce oxidation states on the design of novel electrolytes for solid oxide fuel cells is discussed. [source]


Summer drought decreases soil fungal diversity and associated phenol oxidase activity in upland Calluna heathland soil

Abstract Natural moisture limitation during summer drought can constitute a stress for microbial communities in soil. Given globally predicted increases in drought frequency, there is an urgent need for a greater understanding of the effects of drought events on soil microbial processes. Using a long-term field-scale drought manipulation experiment at Clocaenog, Wales, UK, we analysed fungal community dynamics, using internal transcribed spacer-denaturing gradient gel electrophoresis (DGGE), over a 1-year period in the 6th year of drought manipulation. Ambient seasonality was found to be the dominant factor driving variation in fungal community dynamics. The summer drought manipulation resulted in a significant decline in the abundance of dominant fungal species, both independently of, and in interaction with, this seasonal variation. Furthermore, soil moisture was significantly correlated with the changes in fungal diversity over the drought manipulation period. While the relationship between species diversity and functional diversity remains equivocal, phenol oxidase activity was decreased by the summer drought conditions and there was a significant correlation with the decline of DGGE band richness among the most dominant fungal species during the drought season. Climatically driven events such as droughts may have significant implications for fungal community diversity and therefore, have the potential to interfere with crucial ecosystem processes, such as organic matter decomposition. Movement and behaviour of large southern bluefin tuna (Thunnus maccoyii) in the Australian region determined using pop-up satellite archival tags

Abstract Pop-up satellite archival tags (PSATs) were deployed on 52 large (156,200 cm length to caudal fork) southern bluefin tuna (Thunnus maccoyii) in the western Tasman Sea during the austral winters of 2001,2005. Southern bluefin tuna (SBT) were resident in the Tasman Sea for up to 6 months with movements away from the tagging area occurring at highly variable rates. The data indicated a general tendency for SBT to move south from the tagging area in the Western Tasman Sea. Four individuals migrated west along the southern continental margin of Australia and into the Indian Ocean. Three individuals moved east into the central Tasman Sea, with one individual reaching New Zealand. We also describe the first observed migration of an SBT from the Tasman Sea to the Indian Ocean spawning grounds south of Indonesia. Individuals spent most of their time relatively close to the Australian coast, with an estimated 84% of time spent in the Australian Fishing Zone. SBT favored temperatures between 19 and 21°C, adjusting their depth to the vertical temperature distribution. Distinct diurnal diving patterns were observed and adjustment of depth to maintain constant ambient light levels over a 24-h period. The findings of this study are a significant advance toward greater understanding of the spatial dynamics of large SBT and understanding the connectivity between distant regions of their distribution. A Study of the Views of Women in the ICT Industry in New Zealand
Using data from in-depth interviews with female information and communication technology (ICT) professionals from New Zealand's four main cities, this article uses a social constructivist framework to investigate the women's perceptions of their ICT work place. The results show that there are regional differences in organization type, job category and salaries, as well as in the perceptions of the women towards their environment. Most women did not actively seek to be employed in ICT , rather their entry was serendipitous. While they enjoyed working in the environment, there was an obvious gendering of the workforce with most technical positions being held by men and women working mainly in the softer side of ICT. The women perceived their gendered roles to be a highly important and an integral part of ICT and believed their salaries were equitable with their male colleagues. Having a greater understanding of how women view their ICT work place will contribute to attracting and retaining them in an industry where a shortage of skills is envisaged in the near future. Advancing Gender Equality: The Role of Women-Only Trade Union Education

The need to improve the level of membership and the number of women activists has been a central feature of overall ,renewal' strategies of many British trade unions. Within this, equality education has been a key part of policy-making. This research draws on detailed case studies of two trade unions and focuses on their women-only education courses. We suggest that a greater understanding of the contribution of different types of trade union education to the advance of equality is a key factor in the ability of unions to maintain a central role at workplace level, within the context of an increasingly diverse labour market. Distinct Nanostructures from a Molecular Shuttle: Effects of Shuttling Movement on Nanostructural Morphologies

Abstract A solvent driven molecular shuttle that contains a TCBD chromophore and in which the macrocycle can be positioned close to or far from the TCBD unit with a change of solvent is prepared. Several distinct nanostructures are obtained by control of the shuttling movement of the macrocycle: i) in a mixed solvent of CHCl3/n -C6H14 (1/1, v/v), the macrocycle locates at the peptide station, and interlaced nanofibers form as a result of the extended intermolecular dipole,dipole interactions of the TCBD units; ii) in a solvent of dimethyl sulfoxide, the macrocycle moves along the long alkyl-chain, and worm-like nanoparticles form because the macrocycle obstructs the intermolecular dipole,dipole interactions of the TCBD units. This system confirms that the molecular aggregation behaviors can be controlled by the shuttling movement of the macrocycle. Exploitation of the molecular shuttle to control the molecular aggregation behaviors will provide greater understanding in the field of molecular shuttle applications. [source]


Ovarian Hormones and Migraine Headache: Understanding Mechanisms and Pathogenesis,Part 2

Migraine headache is strongly influenced by reproductive events that occur throughout the lifespan of women. Each of these reproductive events has a different "hormonal milieu," which might modulate the clinical course of migraine headache. Estrogen and progesterone can be preventative or provocative for migraine headache under different circumstances depending on their absolute serum levels, constancy of exposure, and types of estrogen/progesterone derivatives. Attacks of migraine with and without aura respond differently to changes in ovarian hormones. Clearly a greater knowledge of ovarian hormones and their effect on migraine is essential to a greater understanding of the mechanisms and pathogenesis of migraine headache. [source]


Decision-making in community-based paediatric physiotherapy: a qualitative study of children, parents and practitioners

Abstract Approaches to practice based on partnership and shared decision-making with patients are now widely recommended in health and social care settings, but less attention has been given to these recommendations in children's services, and to the decision-making experiences of non-medical practitioners and their patients or clients. This study explored children's, parents' and practitioners' accounts of shared decision-making in the context of community-based physiotherapy services for children with cerebral palsy. Semi-structured interviews were conducted with 11 children with cerebral palsy living in an inner city area of northern England, and with 12 of their parents. Two focus groups were conducted with 10 physiotherapy practitioners. Data were analysed using the constant comparative method. When asked explicitly about decision-making, parents, children and practitioners reported little or no involvement, and each party saw the other as having responsibility for decisions. However, when talking in more concrete terms about their experiences, each party did report some involvement in decision-making. Practitioners' accounts focused on their responsibility for making decisions about resource allocation, and thereby, about the usefulness and intensity of interventions. Parents indicated that these practitioner-led decisions were sometimes in conflict with their aspirations for their child. Parents and children appeared to have most involvement in decisions about the acceptability and implementation of interventions. Children's involvement was more limited than parents'. While parents could legitimately curtail unacceptable interventions, children were mostly restricted to negotiating about how interventions were implemented. In these accounts the involvement of each party varied with the type of issue being decided and decision-making appeared more unilateral than shared. In advocating shared decision-making, greater understanding of its weaknesses as well as its strengths, and greater clarity about the domains that are suitable for a shared decision-making approach and the roles of different parties, would seem a helpful step. Testing the convergent validity of the contingent valuation and travel cost methods in valuing the benefits of health care

Abstract In this study, the convergent validity of the contingent valuation method (CVM) and travel cost method (TCM) is tested by comparing estimates of the willingness to pay (WTP) for improving access to mammographic screening in rural areas of Australia. It is based on a telephone survey of 458 women in 19 towns, in which they were asked about their recent screening behaviour and their WTP to have a mobile screening unit visit their nearest town. After eliminating missing data and other non-usable responses the contingent valuation experiment and travel cost model were based on information from 372 and 319 women, respectively. Estimates of the maximum WTP for the use of mobile screening units were derived using both methods and compared. The highest mean WTP estimated using the TCM was $83.10 (95% C.I. $99.06,$68.53), which is significantly less than the estimate of $148.09 ($131.13,$166.60) using the CVM. This could be due to the CVM estimates also reflecting non-use values such as altruism, or a range of potential biases that are known to affect both methods. Further tests of validity are required in order to gain a greater understanding of the relationship between these two methods of estimating WTP. Copyright © 2001 John Wiley & Sons, Ltd. [source]


Getting ready for user involvement in a systematic review

Abstract Objective, This paper aims to support the critical development of user involvement in systematic reviews by explaining some of the theoretical, ethical and practical issues entailed in ,getting ready' for user involvement. Background, Relatively few health or social care systematic reviews have actively involved service users. Evidence from other research contexts shows that user involvement can have benefits in terms of improved quality and outcomes, hence there is a need to test out different approaches in order to realize the benefits of user involvement and gain a greater understanding of any negative outcomes. Design, Setting up a service-user reference group for a review of user involvement in nursing, midwifery and health visiting research involved conceptualizing user involvement, developing a representation framework, identifying and targeting service users and creating a sense of mutuality and reciprocity. Setting and participants, Recruitment was undertaken across England by two researchers. Members from 24 national consumer organizations were selected to participate in the review. Main variables studied, Learning was gained about finding ways of navigating consumer networks and organizations, how best to communicate our goals and intentions and how to manage selection and ,rejection' in circumstances where we had stimulated enthusiasm. Results and conclusions, Involving service users helped us to access information, locate the findings in issues that are important to service users and to disseminate findings. User involvement is about relationships in social contexts: decisions made at the early conceptual level of research design affect service users and researchers in complex and personal ways. Functional polymorphisms in dopamine and serotonin pathway genes,

Abstract There is mounting evidence on the functional significance of single nucleotide and simple repeat sequence polymorphisms in both the coding and regulatory regions of genes in the monoamine neurotransmitter pathways. Many of these gene variants have been associated with human behavioral disorders and traits, and thus have important clinical relevance. This review summarizes the literature on the published functional studies from a molecular, cellular, and neurobiological perspective, and notes their possible behavioral consequences. Functional studies have adopted a variety of strategies. Pharmacological studies have focused on the effects of gene variation at the protein level in terms of binding to ligands or drugs. Other key investigations have determined effects on gene expression at the level of transcription in mammalian cell cultures, lymphoblasts, and/or human postmortem brain tissue. This has enabled the comparison of in vitro and in vivo data, and furthermore provides an improved perceptive of their respective advantages. Additionally, molecular biological approaches have identified transcription factors (DNA-binding proteins) that interact with the motifs within the polymorphisms themselves. Various neuroimaging studies have further determined the relationship of genotype with protein availability in the brain, and thus have contributed to our understanding of the in vivo functional significance of gene variants. Finally, there is growing evidence from both human and animal studies on the interaction of functional polymorphisms with the environment in determining a behavioral outcome. Taken together, these findings have contributed to a greater understanding of the plausible molecular mechanisms that underpin the functional significance of polymorphisms in monoamine neurotransmitter pathway genes, and how they may influence behavioral phenotypes. Young infants' triangular communication with their parents in the context of maternal postpartum psychosis: Four case studies,

With increasing data on the dynamics of normative couples as they transition to parenthood and become a triad, the need for greater understanding of the impact of parental psychopathology on this transition has become clear. The goal of the current article is to begin exploring this area that has received little attention to date, by describing case examples from a study of clinical families as they transitioned to parenthood. Four representative cases were selected from a pool of 13 mother,father,baby triads, for whom the mother had been hospitalized conjointly with her infant due to a psychotic episode during the postpartum period. The families were observed as part of a clinical consultation that included a semistructured play paradigm known as the Lausanne Trilogue Play (LTP; E. Fivaz-Depeursinge, & A. Corboz-Warnery, 1999). Interactions were scored using standardized measures as well as clinical impressions. All families from the clinical sample were noted to struggle and frequently failed to achieve the goals of play. The impact on the infants in terms of their developing sense of self as well as their defensive strategies in this context are discussed, with clinical implications explored. [source]
