Changing Patterns in Medication Use with Increasing Probability of Death for Older Medicare Beneficiaries

OBJECTIVES: To determine whether use of symptom relief drugs (e.g., antidepressants, anxiolytics, opioid analgesics, sleep aids) rises and use of two commonly prescribed classes of chronic medications (statins and osteoporosis drugs) falls with greater probability of death for older Medicare beneficiaries. DESIGN: Pooled cross-sectional study. SETTING: Noninstitutionalized older Medicare population in 2000 to 2005. PARTICIPANTS: Community-dwelling Medicare beneficiaries aged 65 and older (N=20,233). MEASUREMENTS: Use of medications measured according to dichotomous flags; intensity of use by annual medication fills. Annual probability of death modeled using logistic regression and stratified into seven groups with predicted probabilities of death that range from less than 5% to greater than 50%. Prevalence of use and intensity (mean prescription fills per month) were computed for each class of medication. RESULTS: For symptom relief medications, there is relatively constant use with increasing probability of death, along with greater intensity of use. For the two chronic medications, there was a monotonic decrease in use but at a relatively constant intensity. Decline in statin use ranged from 34.4% in the lowest mortality stratum to 17.6% for those in the highest (P<.001). Use of osteoporosis drugs fell from 10.4% to 6.6% over the same range (P<.001). CONCLUSION: Greater intensity of use of symptom relief medications with increasing probability of death is consistent with hypothesized use. The different profile for chronic medications suggests that the time to benefit is being considered regarding therapy initiation, which results in lower use. [source]


Effects of increasing fire frequency on black carbon and organic matter in Podzols of Siberian Scots pine forests

Summary Fires in boreal forests frequently convert organic matter in the organic layer to black carbon, but we know little of how changing fire frequency alters the amount, composition and distribution of black carbon and organic matter within soils, or affects podzolization. We compared black carbon and organic matter (organic carbon and nitrogen) in soils of three Siberian Scots pine forests with frequent, moderately frequent and infrequent fires. Black carbon did not significantly contribute to the storage of organic matter, most likely because it is consumed by intense fires. We found 99% of black carbon in the organic layer; maximum stocks were 72 g m,2. Less intense fires consumed only parts of the organic layer and converted some organic matter to black carbon (> 5 g m,2), whereas more intense fires consumed almost the entire organic layer. In the upper 0.25 m of the mineral soil, black carbon stocks were 0.1 g m,2 in the infrequent fire regime. After fire, organic carbon and nitrogen in the organic layer accumulated with an estimated rate of 14.4 g C m,2 year,1 or 0.241 g N m,2 year,1. Maximum stocks 140 years after fire were 2190 g organic C m,2 and 40 g N m,2, with no differences among fire regimes. With increasing fire frequency, stocks of organic carbon increased from 600 to 1100 g m,2 (0,0.25 m). Stocks of nitrogen in the mineral soil were similar among the regimes (0.04 g m,2). We found that greater intensities of fire reduce amounts of organic matter in the organic layer but that the greater frequencies may slightly increase amounts in the mineral soil. [source]


Women in the Urban Informal Sector: Perpetuation of Meagre Earnings

The argument of exploitation of women workers in the labour market notwithstanding, this article examines whether women in India are unable to participate fully in the labour market because they are required to combine their household activities with income yielding jobs. They are constrained to work in the neighbourhood of their residence (the location of the residence having been decided upon by male family members), and can access jobs only through informal contacts (which usually means they end up in jobs similar to those of the contact persons), both of which reduce their bargaining power considerably. The tendency for specialized activities to be concentrated in different geographic locations of a city further restricts the possibility of women workers being engaged in diverse jobs and thus aggravates the situation of an excess supply of labour in a particular activity. Constrained choice, limited contacts of women and physical segmentation of the labour market perpetuate forces that entrap women workers in a low-income situation with worse outcomes than those of their male counterparts. Consequently with greater intensity of work they still continue to receive low wages, while residual participation in the labour market restricts the possibilities of skill formation and upward mobility. All of these factors offer a substantive basis for policy recommendations. [source]


Advanced fluorescence in situ hybridization to localize and quantify gene expression in Japanese medaka (Oryzias latipes) exposed to endocrine-disrupting compounds

Abstract In an earlier study, we described the development of fluorescence in situ hybridization (FISH) using confocal microscopy to localize and quantify gene expression in fish. Here, we report the results of FISH application to investigate effects of model endocrine-disrupting chemicals (EDCs), 17,-ethinylestradiol (EE2) and 17,-trenbolone (TB), on expressions of EDC-responsive genes in Japanese medaka (Oryzias latipes) at the cellular/tissue level paired with histological observation. Gene expressions of vitellogenin-II (Vit-II), androgen receptor (AR), and cytochrome P450 gonadal aromatase (CYP19a) were determined after exposure to 5, 50, or 500 ng/L of EE2 or 50, 500, or 5,000 ng/L of TB for 7 d. Exposure to the greatest concentration of EE2 or TB significantly reduced fecundity and caused histological alterations in gonads. 17,-Ethinylestradiol induced Vit-II expression in both male gonads and liver relative to controls and resulted in greater intensity of hematoxylin staining in hepatocytes, which was significantly correlated with Vit-II induction in liver. When exposed to EE2 at less than 50 ng/L, CYP19a expression associated with early stage oocytes was greater than that in controls. However, at 500 ng/L, this trend was reversed. The greater Vit-II expression in testis from all EE2 groups, and the lesser expression of CYP19a in ovaries from the 500 ng/L group, likely is related to changes in the number of cells in which these genes are predominantly expressed rather than to an increase in expression per cell. 17,-Trenbolone significantly induced AR expression in ovaries but did not alter AR expression in female liver. It was concluded that FISH combined with histology enables advanced elucidation of molecular effects of chemicals by associating changes in gene expression with certain tissues and/or cell types and allows these changes to be related to histological effects. SONG VARIATION IN AN AVIAN RING SPECIES

Abstract., Divergence of mating signals can occur rapidly and be of prime importance in causing reproductive isolation and speciation. A ring species, in which two reproductively isolated taxa are connected by a chain of intergrading populations, provides a rare opportunity to use spatial variation to reconstruct the history of divergence. I use geographic variation in the song of a likely ring species, the greenish warbler (Phylloscopus trochiloides) to reconstruct the microevolutionary steps that occurred during divergence of a trait that is often important in speciation in birds. Populations of a western Siberian (P. t. viridanus) and an eastern Siberian (P. t. plumbeitarsus) form of the greenish warbler meet, but do not interbreed in central Siberia; these forms are connected by a chain of interbreeding populations extending in a ring to the south around the treeless Tibetan Plateau. I show that: (1) song structure differs greatly between the two Siberian forms, which share the same habitat; (2) song structure changes gradually around the ring; (3) singing behavior is relatively simple in the Himalayas, but becomes increasingly complex to the north, both to the west and east of the Tibetan Plateau; and (4) song varies along independent axes of complexity in the western and eastern south-north clines. By comparing geographic variation in singing behavior and ecological variables, I distinguish among possible causes of song divergence, including selection based on the acoustic environment, stochastic effects of sexual selection, and selection for species recognition. I suggest that parallel south-to-north ecological gradients have caused a greater intensity of sexual selection on song in northern populations and that the stochastic effects of sexual selection have led to divergence in song structure. Functional magnetic resonance imaging response to increased verbal working memory demands among patients with multiple sclerosis

Abstract Multiple sclerosis (MS) patients frequently experience impaired verbal working memory (VWM). Functional magnetic resonance imaging (fMRI) may help identify neural mechanisms underlying these deficits. Neuroimaging studies of healthy adults have characterized responses associated with increased VWM demands during the n-Back task, suggesting that this experimental paradigm could help identify neural correlates of VWM deficits among MS patients. Fifteen MS patients and 15 matched control participants completed the n-Back during whole-brain fMRI. Mean signal during adjacent 0-Back blocks was subtracted, on a voxel-wise basis, from mean signal observed during n-Back blocks. Resulting difference scores for 1-, 2-, and 3-Back were compared across groups and difficulty levels. Signal intensity was positively related to difficulty level in anterior regions, including premotor, supplementary motor, and dorsolateral prefrontal cortices. MS patients exhibited significantly greater intensity in these areas compared to controls during the 1-Back, while portions of the left superior frontal gyrus, cingulate, and parahippocampal gyri were relatively less intense at more difficult levels. MS group responses were slower during the 1-Back and tended to be slower during the 3-Back; however, accuracy did not differ at any level. Lesion load was positively related to only 1-Back activity and unrelated to any performance measure. Results suggest that compensatory activity occurs among MS patients matched on performance accuracy. Furthermore, compensatory activity occurs predominantly in regions associated with VWM, and this may decline relative to controls as task demands increase. The Effect of Seed Vernalization and Irradiation on Growth and Photosynthesis of Field Bean Plants (Vicia faba L. minor) and on Nitrogenase Activity of Root Nodules

The effect of seed vernalization (2,3°C; 36 days) and irradiation intensity during vegetation of field bean upon growth and maximum leaf photosynthesis (Pmax) as well as specific (SNA) and total nitrogenase activity (TNA) of root nodules was investigated. The measurements were performed at the initial stage of pod development of varieties with undetermined (cv. Nadwi,a,ski) and determined (cv. Tibo) growth habit. In comparison with plants in shading, those which grew in full irradiation produced larger leaf area and more dry weight, achieving higher Pmax values. Stimulation of leaf area enlargement and dry weight increase, especially in respect to the dry weight of underground parts had a positive and significant effect upon TNA value. Accumulation of dry weight was higher in vernalized than non-vernalized plants, but only in the case of cv. Nadwi,la,ski, in full irradiation conditions. In the case of plants grown in poorer irradiation conditions TNA values were about 68 % (cv. Nadwi,la,ski) and 54 % (cv. Tibo) lower, due to significant limitation of root nodules. TNA value of vernalized cv. Nadwi,la,ski plants was 66 % higher in comparison with non-vernalized plants. However, the effect was visible only in full irradiation conditions, due to simultaneous greater intensity of root nodules production and higher SNA value. A positive effect of vernalization could also be observed in the decrease of a negative correlation between dry weight of root nodules and SNA in both varieties. Zusammenfassung Der Einfluss der Samenvernalisation (3,3°C; 36 Tage) und der Strahlungsintensität während der Vegetation von Fababohnen hinsichtlich des Wachstums und der maximalen Blattfotosynthese (Pmax) sowie der spezifischen (SNA) und Gesamtnitrogenaseaktivität (TNA) der Wurzelknöllchen wurde untersucht. Die Messungen wurden im ersten Stadium der Hülsenentwicklung an Sorten mit undeterminierten (cv. Nadwi,la,ski) und determinierten (cv. Tibo) Wachstumsstrukturen durchgeführt. Ein Vergleich von Pflanzen unter Beschattung mit Pflanzen unter voller Strahlung zeigte, dass beschattete Pflanzen eine gröiere Blattfläche und höheres Trockengewicht produzierten und damit höhere Pmax -Werte erreichten. Stimulation der Blattflächenvergröierung und der Trockenge wichtszunahme, insbesondere im Hinblick auf die Trockengewichte der unterirdischen Pflanzenteile hatte eine positive und signifikante Auswirkung auf den DNA-Wert. Die Akkumulation von Trockenmasse war in vernalisierten Pflanzen höher als in nicht vernalisierten; dies gilt nur für Sorte Nadwi,la,ski unter dem Einfluss voller Strahlung. Im Fall der Pflanzen, die unter geringer Strahlung wuchsen, waren die TNA-Werte ca. 68 % (cv.Nadwi,la,ski) und 54 % (cv. Tibo) geringer als Folge einer signifikanten Begrenzung des Wurzelknöllchenwachstums. TNA-Werte vernalisierter cv. Nadwi,la,ski-Pflanzen waren um 66 % höher im Vergleich mit nicht vernalisierten Pflanzen. Allerdings zeigte sich diese Wirkung nur unter voller Strahlung als Folge einer vergleichsweise gröieren Intensität der Wurzelknöllchenproduktion und höherer SNA-Werte. Predictors of grief following the death of one's child: the contribution of finding meaning,

Abstract This study examined the relative contribution of objective risk factors and meaning-making to grief severity among 157 parents who had lost a child to death. Participants completed the Core Bereavement Items (CBI; Burnett, Middleton, Raphael, & Martinek, 1997), Inventory of Complicated Grief (ICG; Prigerson et al., 1995), questions assessing the process and degree of sense-making and benefit-finding, and the circumstances surrounding their losses. Results showed that the violence of the death, age of the child at death, and length of bereavement accounted for significant differences in normative grief symptoms (assessed by the CBI). Other results indicated that the cause of death was the only objective risk factor that significantly predicted the intensity of complicated grief (assessed by the ICG). Of the factors examined in this study, sense-making emerged as the most salient predictor of grief severity, with parents who reported having made little to no sense of their child's death being more likely to report greater intensity of grief. PARTNER AGGRESSION SEVERITY AS A RISK MARKER FOR MALE AND FEMALE VIOLENCE RECIDIVISM

Pretreatment aggression severity was examined as a risk marker for recidivism in the treatment of partner aggression. Intact married couples experiencing husband-to-wife partner aggression were recruited from the community and participated in either conjoint group treatment or gender-specific group treatment. Elevated levels of husband and wife physical aggression and wife psychological aggression before treatment predicted the continuation and severity of physical aggression by both spouses during treatment and in the following year, with no significant differences across treatment formats. These results indicate that high levels of psychological and physical aggression signify a poor prognosis for both conjoint and gender-specific group treatment programs, suggesting the need for interventions of greater intensity, duration, and/or focus for individuals highest in psychological and physical aggression. ASSOCIATION OF STRAWBERRY YOGURT SENSORY PROPERTIES WITH PRODUCT COMPOSITION BY PROCRUSTES ANALYSIS

ABSTRACT The flavor of eight samples of commercial strawberry yogurt was studied by Free-Choice Profile analysis (FCP). Generalized Procrustes Analysis (GPA) applied to FCP allowed differentiation between samples and highlighted flavor attributes responsible for the observed differences. The relation between sensory and physicochemical datasets was studied by means of GPA. Those samples with higher carbohydrate content were perceived as sweeter, having stronger strawberry flavor, and with more dairy and yogurt flavors. Samples with higher titratable acidity, ash and protein content were perceived as more acidic and higher in intensity of "faulty" or "defective" flavors. Higher moisture content was associated with lower intensity of "dairy" flavors (creamy, dairy, and yogurt) and greater intensity of rancid flavor. It is concluded that, though not often used to this end, GPA is a suitable method to study the relationship of sensory and instrumental measurements. [source]


Sexual selection for male dominance reduces opportunities for female mate choice in the European bitterling (Rhodeus sericeus)

Abstract Sexual selection involves two main mechanisms: intrasexual competition for mates and intersexual mate choice. We experimentally separated intrasexual (male,male interference competition) and intersexual (female choice) components of sexual selection in a freshwater fish, the European bitterling (Rhodeus sericeus). We compared the roles of multiple morphological and behavioural traits in male success in both components of sexual competition, and their relation to male reproductive success, measured as paternity of offspring. Body size was important for both female choice and male,male competition, though females also preferred males that courted more vigorously. However, dominant males often monopolized females regardless of female preference. Subordinate males were not excluded from reproduction and sired some offspring, possibly through sneaked ejaculations. Male dominance and a greater intensity of carotenoid-based red colouration in their iris were the best predictors of male reproductive success. The extent of red iris colouration and parasite load did not have significant effects on female choice, male dominance or male reproductive success. No effect of parasite load on the expression of red eye colouration was detected, though this may have been due to low parasite prevalence in males overall. In conclusion, we showed that even though larger body size was favoured in both intersexual and intrasexual selection, male,male interference competition reduced opportunities for female choice. Females, despite being choosy, had limited control over the paternity of their offspring. Our study highlights the need for reliable measures of male reproductive success in studies of sexual selection. [source]


M&As AND THE GLOBAL STRATEGIES OF TNCs

Most of the motivations for M&As that feature in the global strategies of transnational corporations (TNCs) are a means of reshaping competitive advantages within their respective industries. They have some effect on the TNCs of all or most industries and so to that extent they are not necessarily sector-specific. However, it may be that some of the motives which we outline affect some industries more than others, and in that sense they can be expected to be associated with a greater intensity of M&As in certain sectors than others. We identify the likelihood of M&As across industries, and discuss how the general factors that have promoted the recent M&A wave have had a bigger impact on the global strategies of TNCs in the industries in which the propensity to engage in M&As has been the highest. The regional dimension is also considered. [source]


Dynamics of affective experience and behavior in depressed adolescents

Background:, Depression is often characterized as a disorder of affect regulation. However, research focused on delineating the key dimensions of affective experience (other than valence) that are abnormal in depressive disorder has been scarce, especially in child and adolescent samples. As definitions of affect regulation center around processes involved in initiating, maintaining, and modulating the occurrence, intensity, and duration of affective experiences, it is important to examine the extent to which affective experiences of depressed youth differ on these dimensions from those of healthy youth. Methods:, The affective behavior and experience of adolescents with major depressive disorder (MDD; n = 75) were compared to a demographically matched cohort of healthy adolescents (n = 77). Both samples were recruited from community high schools. A multi-source (parents and adolescent), multi-method (interviews, behavioral observations, questionnaires) assessment strategy was used to examine positive and negative affects. Results:, Depressed youth had significantly longer durations, higher frequency, and greater intensity when experiencing angry and dysphoric affects and shorter durations and less frequency of happy affect when compared to healthy youth. The most consistent, cross-method results were evident for duration of affect. Conclusions:, Clinically depressed adolescents experienced disturbances in affective functioning that were evident in the occurrence, intensity, and duration of affect. Notably, the disturbances were apparent in both positive and negative affects. [source]


Intrinsic brain connectivity in fibromyalgia is associated with chronic pain intensity,

Objective Fibromyalgia (FM) is considered to be the prototypical central chronic pain syndrome and is associated with widespread pain that fluctuates spontaneously. Multiple studies have demonstrated altered brain activity in these patients. The objective of this study was to investigate the degree of connectivity between multiple brain networks in patients with FM, as well as how activity in these networks correlates with the level of spontaneous pain. Methods Resting-state functional magnetic resonance imaging (FMRI) data from 18 patients with FM and 18 age-matched healthy control subjects were analyzed using dual-regression independent components analysis, which is a data-driven approach for the identification of independent brain networks. Intrinsic, or resting-state, connectivity was evaluated in multiple brain networks: the default mode network (DMN), the executive attention network (EAN), and the medial visual network (MVN), with the MVN serving as a negative control. Spontaneous pain levels were also analyzed for covariance with intrinsic connectivity. Results Patients with FM had greater connectivity within the DMN and right EAN (corrected P [Pcorr] < 0.05 versus controls), and greater connectivity between the DMN and the insular cortex, which is a brain region known to process evoked pain. Furthermore, greater intensity of spontaneous pain at the time of the FMRI scan correlated with greater intrinsic connectivity between the insula and both the DMN and right EAN (Pcorr < 0.05). Conclusion These findings indicate that resting brain activity within multiple networks is associated with spontaneous clinical pain in patients with FM. These findings may also have broader implications for how subjective experiences such as pain arise from a complex interplay among multiple brain networks. [source]
