Selected AbstractsRandomized controlled trial of dexamphetamine maintenance for the treatment of methamphetamine dependenceADDICTION, Issue 1 2010Marie Longo ABSTRACT Aim To investigate the safety and efficacy of once-daily supervised oral administration of sustained-release dexamphetamine in people dependent on methamphetamine. Design Randomized, double-blind, placebo-controlled trial. Participants Forty-nine methamphetamine-dependent drug users from Drug and Alcohol Services South Australia (DASSA) clinics. Intervention Participants were assigned randomly to receive up to 110 mg/day sustained-release dexamphetamine (n = 23) or placebo (n = 26) for a maximum of 12 weeks, with gradual reduction of the study medication over an additional 4 weeks. Medication was taken daily under pharmacist supervision. Measurements Primary outcome measures included treatment retention, measures of methamphetamine consumption (self-report and hair analysis), degree of methamphetamine dependence and severity of methamphetamine withdrawal. Hair samples were analysed for methamphetamine using liquid chromatography-mass spectrometry. Findings Treatment retention was significantly different between groups, with those who received dexamphetamine remaining in treatment for an average of 86.3 days compared with 48.6 days for those receiving placebo (P = 0.014). There were significant reductions in self-reported methamphetamine use between baseline and follow-up within each group (P < 0.0001), with a trend to a greater reduction among the dexamphetamine group (P = 0.086). Based on hair analysis, there was a significant decrease in methamphetamine concentration for both groups (P < 0.0001). At follow-up, degree of methamphetamine dependence was significantly lower in the dexamphetamine group (P = 0.042). Dexamphetamine maintenance was not associated with serious adverse events. Conclusions The results of this preliminary study have demonstrated that a maintenance pharmacotherapy programme of daily sustained-release amphetamine dispensing under pharmacist supervision is both feasible and safe. The increased retention in the dexamphetamine group, together with the general decreases in methamphetamine use, degree of dependence and withdrawal symptom severity, provide preliminary evidence that this may be an efficacious treatment option for methamphetamine dependence. Linaldeddu Summary The aim of this research was to study the changes in net photosynthesis and stomatal conductance values in 3-year-old cork oak and holm oak seedlings growing in natural conditions and inoculated with Apiognomonia quercina, Biscogniauxia mediterranea, Botryosphaeria corticola and Pleurophoma cava. Throughout the 4-month experimental period, the evolution of visual external symptoms and the values of physiological variables were periodically recorded. All pathogens caused stem lesions around the infection point; however, the lesions caused by B. corticola were longer in both oak species. On cork oak seedlings, all pathogens induced a significant and gradual reduction in net photosynthesis and stomatal conductance values, whereas other physiological disturbances were induced only by B. corticola infections on holm oak seedlings. [source] Analysis of undrained cavity expansion in elasto-plastic soils with non-linear elasticityINTERNATIONAL JOURNAL FOR NUMERICAL AND ANALYTICAL METHODS IN GEOMECHANICS, Issue 1 2002Lai Fa Cao Abstract A large strain analysis of undrained expansion of a spherical/cylindrical cavity in a soil modelled as non-linear elastic modified Cam clay material is presented. The stress,strain response of the soil is assumed to obey non-linear elasticity until yielding. A power-law characteristic or a hyperbolic stress,strain curve is used to describe the gradual reduction of soil stiffness with shear strain. It is assumed that, after yielding, the elasto-plastic behaviour of the soil can be described by the modified Cam clay model. Based on a closed-form stress,strain response in undrained condition, a numerical solution is obtained with the aid of simple numerical integration technique. The results show that the stresses and the pore pressure in the soil around an expanded cavity are significantly affected by the non-linear elasticity, especially if the soil is overconsolidated. The difference between large strain and small strain solutions in the elastic zone is not significant. The stresses and the pore pressure at the cavity wall can be expressed as an approximate closed-form solution. Copyright © 2001 John Wiley & Sons, Ltd. [source] Atrial Fibrillation Burden During the Post-Implant Period After CRT Using Device-Based DiagnosticsJOURNAL OF CARDIOVASCULAR ELECTROPHYSIOLOGY, Issue 8 2006BURKHARD HÜGL M.D. Aims: Cardiac resynchronization therapy (CRT) is increasingly used in congestive heart failure (CHF) patients (with cardiac dyssynchrony). In addition to delivering therapy, CRT devices offer a variety of diagnostic tools for continuous long-term monitoring of clinically relevant information (i.e., occurrence and duration of arrhythmia episodes). Methods and Results: Eighty-four patients with drug-refractory CHF in NYHA-class II,IV received a CRT device. The response to CRT was assessed by determining NYHA class at baseline and at 3 months follow-up. Atrial fibrillation (AF) burden (defined as time of AF per day) was continuously measured by the device. A significant gradual reduction of AF burden (from 9.88 ± 12.61 to 4.20 ± 9.24 [hours/day]) and number of patients experiencing AF episodes (from 26 to 13) were observed during CRT. Conclusions: (1) Diagnostic features for long-term monitoring of physiological variables provide useful information on the state and course of AF and may improve disease management. (2) AF burden reduces over time during the first 3 months after CRT implantation. [source] Reproduction Phase-Related Expression of ,-Endorphin-Like Immunoreactivity in the Nucleus Lateralis Tuberis of the Female Indian Major Carp Cirrhinus mrigala: Correlation with the Luteinising Hormone Cells-Ovary AxisJOURNAL OF NEUROENDOCRINOLOGY, Issue 5 2006A. Sakharkar Abstract The present study aimed to determine whether ,-endorphin immunoreactivity (bEP-ir) in the neurones of the nucleus lateralis tuberis (NLT) is linked to the seasonal cycle and shows correlation with the number of luteinising hormone (LH) cells in the pituitary gland and ovaries in the teleost, Cirrhinus mrigala. Although LH cells were moderately immunostained during the resting phase (December to January), the morphological profile suggested increased synthetic and secretory activity during the preparatory (February to April) and prespawning (May to June) phases. However, LH immunoreactivity was greatly reduced (P < 0.001) in the spawning (July to August) phase, suggesting massive discharge of the hormone; this pool was partly replenished in the postspawning (September to November) phase. The ovaries grew rapidly in the preparatory and prespawning phases; maximal size was attained during spawning, when ovulation occurred. Thereafter, the ovaries regressed. The NLT of C. mrigala is divisible into the pars lateralis (NLTl) and medialis (NLTm). During the postspawning and resting phases, bEP-ir was readily detectable in the NLTm as well as NLTl neurones. However, a steady reduction in the immunoreactivity was observed in the NLTm neurones during the preparatory through spawning phases (P < 0.001), suggesting a negative correlation with the LH cells-ovary axis. Thus, the inhibitory influence of ,-endorphin on the gonadotrophin-releasing hormone (GnRH)-LH axis appears to be attenuated during the preparatory through spawning phases. This may be necessary for the rapid stimulation of the axis culminating in spawning. Neurones of the NLTl also showed a gradual reduction in bEP-ir during the preparatory and prespawning phases (P < 0.01) and may therefore play a similar role. However, significant augmentation of the immunoreactivity was noticed in these neurones during the spawning phase (P < 0.001), the physiological significance of which is unknown. Although the present study demonstrated a temporal correlation between the ,-endorphin in the NLT, LH cells and the ovary, we suggest that the peptide in the NLTl and NLTm may show functional duality during the spawning phase. Araujo Objective:, The objective of this study was to examine the histomorphometric features and evaluate the expression of epidermal growth factor (EGF) and transmembranic receptor (EGFr) and the proliferative potential of epithelial cells from normal and hereditary gingival fibromatosis (HGF) gingival tissues. Background:, EGF is a multifunctional cytokine with a variety of biological effects including stimulation of cell proliferation by binding to its specific EGFr. Methods:, Immunohistochemistry was performed to measure EGF and EGFr expression and the epithelial cell proliferation was determined by measuring proliferating cell nuclear antigen (PCNA). Results:, Histomorphometric evaluation indicated that in HGF the mean height of the epithelial papillae was higher compared to the normal gingiva (NG), whereas mean epithelial area and number of epithelial papillae were quite similar in both groups. The EGF and EGFr positive cells were observed in the basal, spinous and granular cell layers of both normal and HGF tissues, with a gradual reduction from the basal layer. Although the expressions of EGF and EGFr in the control group were significantly higher than those from HGF, in HGF the epithelial papilla tips showed increased number of proliferating cells and elevated expression of EGF and EGFr. There was a correlation between the proliferative potential of epithelial cells and the expression of EGF or EGFr only in the epithelial papilla tips of HGF gingiva. Conclusion:, Our data suggest that EGF and EGFr in the oral epithelium of HGF gingiva may stimulate epithelial cell proliferation, with the resultant apical migration of the oral epithelium and formation of the slender deep epithelial papillae; however, without hyperplastic alterations. [source] THE EFFICACY OF SHORT DAILY DIALYSIS,A SINGLE-CENTRE EXPERIENCEJOURNAL OF RENAL CARE, Issue 3 2010Glenda Rayment M Nursing (Renal) SUMMARY Studies have shown that patients converted to short daily haemodialysis (SDHD) have reported many clinical benefits, decreased complications during dialysis and a better quality of life. A six-month prospective cohort study was conducted to examine the efficacy of SDHD to patients previously receiving three times per week haemodialysis therapy. Following informed consent, participants received haemodialysis daily, Monday,Saturday, between 2 and 2.5 hours for each treatment and followed-up for a six-month period. The participants continued to experience hypotension, cramping and headache and were noncompliant with fluid intake. There was a gradual reduction in blood pressure, cessation of antihypertensives and reduction of erythropoietin therapy (ERT). There were no hospital admissions or reports of access complications. The nursing staff reported an increase in activity levels and nursing interventions with the participants following conversion to SDHD. However, the participants reported a better quality of life. When the ice retreated, mountain hares colonized deglaciated land, and spread over northern Europe. 2.,Since the Weichsel, the mountain hare's distribution in Europe has been gradually reduced and at present comprises Ireland and the Scottish Highlands, high altitudes in the Alps, isolated forests in eastern Poland, most of Fennoscandia and from the Baltic countries eastwards through Russia. Declines during the last century have been observed in Sweden and Russia. 3.,This review defines and evaluates causes for this gradual reduction and fragmentation of the mountain hare's distribution, with special focus on interactions with brown hares Lepus europaeus. The relative importance of diseases, predation, cultivation and interactions with other herbivores than brown hares are discussed. 4.,A plausible cause of the possible permanent disappearance of mountain hares in Europe appears to be exclusion by interspecific competition and hybridization with, and/or epidemic diseases mediated by, the congeneric brown hare. WALTON Summary The transmission of scabies occurs with the burrowing of Sarcoptes scabiei var. hominis mites into the skin. Infestation invariably leads to the development of localized cutaneous inflammation, pruritis and skin lesions. Classical transmission studies document an initial increase in S. scabiei numbers subsequent to primary infestation with a gradual reduction as host immunity develops. However, certain individuals fail to control infection and develop severe crusting of the skin, accompanied with extremely high mite burdens, elevated antibody levels and eosinophilia. These individuals have the nonhealing form of the human disease known as crusted scabies. The genetic predisposition for susceptibility or resistance to S. scabiei infection in humans is hypothesized to correlate with the dominance of an IgE-driven Th2 response in severe disease or an interferon-,-dominated Th1 response that promotes parasite control. However, recent data reveals complexities in cytokine regulation in the skin and the mechanisms of acquired resistance and immune escape. In this review, we consider the recent immunological and biomolecular advances in understanding the human host immune response to S. scabiei infestations in the context of earlier studies and attempt to reconcile apparent differences and emphasize those aspects of the Th1/Th2 model that are supported or refined. [source] Ultrastructure of the differentiating zygotospores of Porphyra leucosticta (Rhodophyta)PHYCOLOGICAL RESEARCH, Issue 4 2002Ioannes Tsekos SUMMARY The ultrastructure of zygotosporogenesis is described for the red alga Porphyra leucosticta Thuret. Packets of eight zygotosporangia, each packet derived from a single carpogonium are interspersed among vegetative cells. Zygotospore differentiation in Porphyra can be separated into three developmental stages. (i) Young zygotospores exhibit a nucleus and a large centrally located, lobed plastid with pyrenoid. Mucilage is produced within concentric membrane structures during their dilation, thus resulting in the formation of mucilage sacs. Subsequently, these sacs release their contents, initiating the zygotospore wall formation. Straight-profiled dictyosomes produce vesicles that also provide wall material. During the later stages of young zygotospores, starch polymerization commences, (ii) Medium-aged zygotospores are characterized by the presence of fibrous vacuoles. These are formed from the ,fibrous vacuole associated organelles'. The fibrous vacuoles finally discharge their contents. (iii) Mature zygotospores are recognized by the presence of numerous cored vesicles produced by dictyosomes. Cored vesicles either discharge their contents or are incorporated into the fibrous vacuoles. There is a gradual reduction of starch granules during zygotospore differentiation. Mature zygotospores are surrounded by a fibrous wall, have a large chloroplast with pyrenoid and well-depicted phycobilisomes but are devoid of starch granules. [source] Denial in cancer patients, an explorative reviewPSYCHO-ONCOLOGY, Issue 1 2007M. S. Vos Abstract Denial is a clinically relevant concept in cancer patients. It has been investigated and discussed extensively. Its definition, however, has been subject to different theoretical trends over time. From a psychoanalytical viewpoint, denial is a pathological, ineffective defence mechanism. On the other hand, according to the stress and coping model denial can be seen as an adaptive strategy to protect against overwhelming events and feelings. In this explorative review the different concepts and the prevalence of denial in cancer patients are described. The relationship between denial and background characteristics and the influence of denial on quality of life are reviewed also. The prevalence of denial of diagnosis in cancer patients ranged from 4 to 47%, denial of impact occurred 8,70% and denial of affect in 18,42% of patients. Elderly cancer patients were more likely to deny. Cultural background seemed to play a role in the prevalence of denial. Neither type of cancer nor gender seemed to be related to denial. At the most, men might be more likely to deny during the terminal phase. In a limited number of longitudinal studies, a gradual reduction in denial was found over the course of the illness. The effect of denial on physical and social functioning remained unclear while the effect on psychological functioning seemed to depend on the concept of denial used. Distractive strategies were found to reduce distress, whereas passive escape mechanisms turned out to decrease psychological well-being. Future research on the prevalence and the (mal)adaptive properties of denial in cancer patients has to be based on a clear concept, longitudinal designs and careful recording of background variables. [source] Production and characterization of poly-3-hydroxybutyrate from biodiesel-glycerol by Burkholderia cepacia ATCC 17759BIOTECHNOLOGY PROGRESS, Issue 2 2010Chengjun Zhu Abstract Glycerol, a byproduct of the biodiesel industry, can be used by bacteria as an inexpensive carbon source for the production of value-added biodegradable polyhydroxyalkanoates (PHAs). Burkholderia cepacia ATCC 17759 synthesized poly-3-hydroxybutyrate (PHB) from glycerol concentrations ranging from 3% to 9% (v/v). Increasing the glycerol concentration results in a gradual reduction of biomass, PHA yield, and molecular mass (Mn and Mw) of PHB. The molecular mass of PHB produced utilizing xylose as a carbon source is also decreased by the addition of glycerol as a secondary carbon source dependent on the time and concentration of the addition. 1H-NMR revealed that molecular masses decreased due to the esterification of glycerol with PHB resulting in chain termination (end-capping). However, melting temperature and glass transition temperature of the end-capped polymers showed no significant difference when compared to the xylose-based PHB. The fermentation was successfully scaled up to 200 L for PHB production and the yield of dry biomass and PHB were 23.6 g/L and 7.4 g/L, respectively. © 2009 American Institute of Chemical Engineers Biotechnol. Prog., 2010 [source] Diversity of Tropical Tree Seedling Responses to DroughtBIOTROPICA, Issue 6 2007Martijn Slot ABSTRACT Drought is an important seedling mortality agent in dry and moist tropical forests, and more severe and frequent droughts are predicted in the future. The effect of drought on leaf gas exchange and seedling survival was tested in a dry-down experiment with four tree species from dry and moist forests in Bolivia. Seedlings were droughted and wilting stage and gas exchange were monitored. Drought led to a gradual reduction of photosynthesis and stomatal conductance over time, whereas respiration and photosynthetic water-use efficiency initially increased with drought and then declined. Seedlings gradually went through the different wilting stages, until they eventually died, but the trajectory differed for the four species. The strong relationship between wilting stage and photosynthesis means that simple field observations can provide valuable information on plant physiological performance. Three different drought strategies were identified. Dry forest species Ceiba samauma shed its leaves and survived. The moist forest species Cariniana ianeirensis postponed drought stress by having low rates of transpiration and high water-use efficiency. Dry forest Astronium urundeuva and moist forest Triplaris americana followed an opportunistic strategy; they are early successional species that can quickly grow to maturity but periodic drought can be lethal. Strikingly, dry and moist forest species did not differ clearly in their drought tolerance strategies. RESÚMEN La sequía es una causa importante de mortalidad de plántulas en bosques tropicales secos y húmedos, y para el futuro se predicen sequías más severas y frecuentes. El efecto de la sequía en el intercambio de gases de las hojas y en la sobrevivencia de plántulas fue comprobado en un experimento de sequía con cuatro especies arbóreas de bosques secos y bosques húmedos en Bolivia. Las plántulas fueron sometidos a un período de sequía y se monitoreo la etapa de marchitamiento y el intercambio de gases. A lo largo del tiempo la sequía causo una reducción gradual en la fotosíntesis y en la conductividad estomática, mientras que la respiración y la eficiencia fotosintética del uso del agua inicialmente aumentaron con la sequía y luego disminuyeron. Las plántulas pasaron gradualmente a través de las diferentes etapas de marchitamiento, hasta que eventualmente murieron, pero la trayectoria varió entre las cuatro especies. La fuerte relación entre la etapa de marchitamiento y fotosíntesis significa que simples observaciones de campo pueden proveer información importante sobre el desempeño fisiológico de la planta. Se identificaron tres tipos de respuestas a la sequía. La especie del bosque seco Ceiba samauma perdió sus hojas y sobrevivió. La especie de bosque húmedo Cariniana ianeirensis postergó el estrés hidrico teniendo una baja tasa de transpiración y una alta eficiencia del uso del agua. Las especies Astronium urundeuva del bosque seco y Triplaris americana del bosque húmedo siguieron una estrategia oportunística; son especies de estados sucesionales tempranos que rápidamente pueden alcanzar la madurez pero eventos de sequía pueden ser letales. Jwo Background: Treatment of short bowel syndrome is problematical. Small bowel tissue engineering has achieved modest results in animal studies. The aim of this study was to investigate intestinal regeneration in a novel surgical model. Methods: Roux-en-Y bypass procedures were performed on 40 Wistar rats weighing 250,350 g. Animals were killed at 1, 2, 3, 4, 8, 12 and 24 weeks after implantation with a 3-cm silicone tube. The spatio temporal relationship of intestinal regeneration was analysed using three-dimensional multislice computed tomography, and examination of sequential morphological changes on gross or histological findings and measurement of missing intestinal tissue (growth defects). Results: Progressive intestinal regeneration on a silicone tube was identifiable in 35 animals. Most adhesions were initially localized on the tube but spread to a distal site 4 weeks after implantation. Growth defects decreased with time, with a marked reduction in the first 4 weeks and a gradual reduction to week 24 after implantation. Luminal patency shown radiologically as well as sequential histological findings, such as mucosal lining, matrix remodelling and muscular regeneration, suggested that regeneration of intestinal tissue took place, not merely scar contraction. Conclusion: Non-invasive as well as histomorphological assessment followed intestinal regeneration over time in this model, which provides scope for further studies. [source] Axonal loss occurs early in dominant optic atrophyACTA OPHTHALMOLOGICA, Issue 3 2010Dan Milea Abstract. Purpose:, This study set out to investigate retinal nerve fibre layer (RNFL) thickness and best corrected visual acuity (BCVA) in relation to age in healthy subjects and patients with OPA1 autosomal dominant optic atrophy (DOA). Methods:, We carried out a cross-sectional investigation of RNFL thickness and ganglion cell layer density in 30 healthy subjects and 10 patients with OPA1 DOA using optical coherence tomography (OCT). We then performed a regression analysis of RNFL thickness and BCVA versus age. Results:, Both healthy subjects and DOA patients demonstrated a gradual reduction in RNFL thickness with age; the relationship was best described statistically by a model that assumed a constant offset between the two groups. Best corrected VA decreased significantly with age in DOA patients, in whom BCVA was correlated with peripapillary RNFL thickness in the inferior and superior peripapillary quadrants and with total macular thickness at eccentricities of 500,3000 ,m. The observations were best described by a constant offset of 41.9 ,m separating the two groups and an annual decrease in RNFL thickness of 0.48 ,m (p < 0.0001). In patients with DOA, increasing age was associated with decreasing BCVA (p = 0.046). Conclusions:, This cross-sectional study found evidence of comparable age-related decreases in RNFL thickness in healthy subjects and in DOA patients, where the deficit in DOA patients is best described using a model that assumes the deficit between the groups does not vary with age. The gradual reduction of BCVA with age may be a consequence of a relative deficit in RNFL thickness that is established before the second decade of life. [source] Acute atrial arrhythmogenesis in murine hearts following enhanced extracellular Ca2+ entry depends on intracellular Ca2+ storesACTA PHYSIOLOGICA, Issue 2 2010Y. Zhang Abstract Aim:, To investigate the effect of increases in extracellular Ca2+ entry produced by the L-type Ca2+ channel agonist FPL-64176 (FPL) upon acute atrial arrhythmogenesis in intact Langendorff-perfused mouse hearts and its dependence upon diastolic Ca2+ release from sarcoplasmic reticular Ca2+ stores. Methods:, Confocal microscope studies of Fluo-3 fluorescence in isolated atrial myocytes were performed in parallel with electrophysiological examination of Langendorff-perfused mouse hearts. Results:, Atrial myocytes stimulated at 1 Hz and exposed to FPL (0.1 ,m) initially showed (<10 min) frequent, often multiple, diastolic peaks following the evoked Ca2+ transients whose amplitudes remained close to control values. With continued pacing (>10 min) this reverted to a regular pattern of evoked transients with increased amplitudes but in which diastolic peaks were absent. Higher FPL concentrations (1.0 ,m) produced sustained and irregular patterns of cytosolic Ca2+ activity, independent of pacing. Nifedipine (0.5 ,m), and caffeine (1.0 mm) and cyclopiazonic acid (CPA) (0.15 ,m) pre-treatments respectively produced immediate and gradual reductions in the F/F0 peaks. Such nifedipine and caffeine, or CPA pre-treatments, abolished, or reduced, the effects of 0.1 and 1.0 ,m FPL on cytosolic Ca2+ signals. FPL (1.0 ,m) increased the incidence of atrial tachycardia and fibrillation in intact Langendorff-perfused hearts without altering atrial effective refractory periods. These effects were inhibited by nifedipine and caffeine, and reduced by CPA. Conclusion:, Enhanced extracellular Ca2+ entry exerts acute atrial arrhythmogenic effects that is nevertheless dependent upon diastolic Ca2+ release. These findings complement reports that associate established, chronic, atrial arrhythmogenesis with decreased overall inward Ca2+ current. The efficacy of lofexidine/naloxone was compared with lofexidine/placebo in a double-blind, randomized, placebo-controlled trial in 89 opiate-dependent patients. There were no significant differences between the two groups in the proportion of patients completing detoxification or in the length of stay. Patients in the active naloxone group demonstrated gradual reductions in levels of withdrawal and craving over the detoxification period. At completion of detoxification, patients who received naloxone maintained a level of withdrawal consistently lower than that in the placebo group; however, naloxone did not substantially accelerate the resolution of the withdrawal syndrome. Implications for future research are discussed. [source] |
