Global Problem: National Accountability: Framing Accountability in the Australian Context of Climate Change

Climate change has been seen as a crisis looming in the future, and has therefore not reached the top of the political agenda. This no longer holds true when looking at Australia, where climate change has become high politics. In this paper we examine the Australian electoral debate in terms of accountability framing, where the Government and Opposition were involved in a ,framing contest'. We argue that theories on accountability framing in crisis need to be modified in order to capture the complex dynamics of climate change due to its inherent scientific uncertainty and global nature. After conducting an inductive analysis of Australian Broadcasting Corporation-reporting we found three themes to be of importance for accountability framing in the ,risk society': labeling, linking and coping. Probiotics: do they have a role in oral medicine and dentistry?

This review describes current knowledge on probiotic bacteriotherapy from the oral health perspective. Recent experimental studies and results from randomized controlled trials have shown that certain gut bacteria, in particular species of Lactobacillus and Bifidobacterium, may exert beneficial effects in the oral cavity by inhibiting cariogenic streptococci and Candida sp. Probiotics have been successfully used to control gastro-intestinal diseases. They also appear to alleviate symptoms of allergy and diseases with immunological pathology. The mechanisms of probiotic action appear to link with colonization resistance and immune modulation. Lactic acid bacteria can produce different antimicrobial components such as organic acids, hydrogen peroxide, carbon peroxide, diacetyl, low molecular weight antimicrobial substances, bacteriocins, and adhesion inhibitors, which also affect oral microflora. However, data is still sparse on the probiotic action in the oral cavity. More information is needed on the colonization of probiotics in the mouth and their possible effect on and within oral biofilms. There is every reason to believe that the putative probiotic mechanisms of action are the same in the mouth as they are in other parts of the gastrointestinal tract. Because of the increasing global problem with antimicrobial drug resistance, the concept of probiotic therapy is interesting and pertinent, and merits further research in the fields of oral medicine and dentistry. Methylenedioxymethamphetamine (MDMA, ,Ecstasy'): a stressor on the immune system

Summary Drug abuse is a global problem of considerable concern to health. One such health concern stems from the fact that many drugs of abuse have immunosuppressive actions and consequently have the potential to increase susceptibility to infectious disease. This article is focused on the impact of the amphetamine derivative, methylenedioxymethamphetamine (MDMA; ,Ecstasy') on immunity. Research conducted over the last 5 years, in both laboratory animals and humans, has demonstrated that MDMA has immunosuppressive actions. Specifically, MDMA suppresses neutrophil phagocytosis, suppresses production of the pro-inflammatory cytokines tumour necrosis factor-, (TNF-,) and interleukin (IL)-1,, and increases production of the endogenous immunosuppressive cytokine (IL-10), thereby promoting an immunosuppressive cytokine phenotype. MDMA also suppresses circulating lymphocyte numbers, with CD4+ T cells being particularly affected, and alters T-cell function as indicated by reduced mitogen-stimulated T-cell proliferation, and a skewing of T-cell cytokine production in a T helper 2 (Th2) direction. For the most part, the aforementioned effects of MDMA are not the result of a direct action of the drug on immune cells, but rather caused by the release of endogenous immunomodulatory substances. Consequently, the physiological mechanisms that are thought to underlie the immunosuppressive effects of MDMA will be discussed. As many of the physiological changes elicited by MDMA closely resemble those induced by acute stress, it is suggested that exposure to MDMA could be regarded as a ,chemical stressor' on the immune system. Finally, the potential of MDMA-induced immunosuppression to translate into significant health risks for abusers of the drug will be discussed. [source]


Cloning, expression and partial characterization of a Haemaphysalis longicornis and a Rhipicephalus appendiculatus glutathione S -transferase

Abstract The ticks Haemaphysalis longicornis and Rhipicephalus appendiculatus are important parasites worldwide. The current method for control of cattle ticks involves the use of chemicals. Nevertheless, parasite resistance is an ever increasing global problem. Glutathione S -transferases (GSTs) play a central role in detoxication of xenobiotic and endogenous compounds. Several authors have noted that an increase in GST activity is associated with resistance to insecticides and acaricides. In the present study, we report the cloning and expression of GST cDNAs from H. longicornis and R. appendiculatus. In addition, we determine the effect of three acaricides (ethion, deltamethrin and diazinon) on the enzymatic activity of rGSTs. [source]


Mechanics of land subsidence due to groundwater pumping

Abstract This paper presents the formulation of the basic mechanics governing the changes in stress states from groundwater pumping and comparisons among predicted land subsidence from this mechanics with existing analyses and field data. Land subsidence is a growing, global problem caused by petroleum and groundwater withdrawal, mining operations, natural settlement, hydro-compaction, settlement of collapsible soils, settlement of organic soils and sinkholes. This paper is concerned with the land subsidence due to groundwater level decline by groundwater pumping. It is shown that the stress state consists of asymmetric stresses that are best simulated by a Cosserat rather than a Cauchy continuum. Land subsidence from groundwater level decline consists of vertical compression (consolidation), shear displacement and macro-rotation. The latter occurs when conditions are favorable (e.g. at a vertical interface) for the micro-rotation imposed by asymmetric stresses to become macro-rotation. When the length of the cone of depression is beyond ,2 times the thickness of the aquifer, simple shear on vertical planes with rotation is the predominant deformation mode. Otherwise, simple shear on horizontal planes is present. The predicted subsidence using the mechanics developed in this paper compares well with data from satellite-borne interferometric synthetic aperture radar. Copyright © 2009 John Wiley & Sons, Ltd. Level-Sets fields, placement and velocity based formulations of contact-impact problems

Abstract By introducing unknown Sign -like fields of Level-Sets type, the Signorini-Moreau dynamic contact conditions are set merely as boundary equations. From this setting, a continuous hybrid weak,strong formulation for dynamic contact between deformable solids is derived and a new Lagrangian formulation (we call stabilized) generalizing both the classical and augmented ones is obtained. Friction phenomena are treated similarly. In the global problem, the irregular Sign -like fields stand for the intrinsic contact unknown ones. This problem is discretized by means of time, space and collocation schemes. Some numerical experimentations are carried out, showing the potential of our developments. Community-acquired methicillin-resistant Staphylococcus aureus skin infections: a review of epidemiology, clinical features, management, and prevention

Community-acquired methicillin-resistant Staphylococcus aureus (CAMRSA) infection is a global problem of epidemic proportions. Many of the patients who develop CAMRSA skin lesions do not have infection-associated risk factors. Abscess, abscess with accompanying cellulitis, and cellulitis are the most common presentations of cutaneous CAMRSA infection; occasionally, these CARMSA-related lesions are misinterpreted as spider or insect bites. Other manifestations of cutaneous CAMRSA infection include impetigo, folliculitis, and acute paronychia. The management of CAMRSA skin infection includes incision and drainage, systemic antimicrobial therapy, and adjuvant topical antibacterial treatment. In addition, at the initial visit, bacterial culture of the lesion should be considered. Direct skin-to-skin contact, damage to the skin surface, sharing of personal items, and a humid environment are potential mechanisms for the acquisition and transmission of cutaneous CAMRSA infection. Measures that strive to eliminate these causes are useful for preventing the spread of CAMRSA skin infection. [source]


Awareness and determinants of family planning practice in Jimma, Ethiopia

Background:, The continuing growth of the world population has become an urgent global problem. Ethiopia, like most countries in sub-Saharan Africa, is experiencing rapid population growth. Currently, the country's population is growing at a rate of 3%, one of the highest rates in the world and if it continues unabated, the population will have doubled in 23 years, preventing any gain in the national development effort. Aim:, To determine the level and determinants of family planning awareness and practice in one Ethiopian town. Methodology:, A quantitative study using a descriptive survey design was conducted in Jimma University Hospital. Discussion:, The findings revealed that the knowledge and practice of modern contraception methods was low. Most women's contraceptive knowledge and practice was influenced by socio-cultural norms such as male/husband dominance and opposition to contraception, and low social status of women. A lack of formal education for women was identified as a key factor in preventing change in the patterns of contraceptive knowledge and use by women in this part of Ethiopia. Conclusion:, The support and encouragement for women and men to enter and complete formal education is essential in bringing about a cultural and social change in attitude towards the economic and social value of family planning. This study and others suggest that education can address the imbalance in decision making about contraception and the role of women in society generally. [source]

Classification and design of teledermatology practice: What dermatoses?

Which technology to apply?
Abstract Dermatologists are mostly confined to urban regions and rural population is deprived of specialist care. Teledermatology Practice (TDP) is a solution to overcome this global problem. Tools for TDP includes video conference, store and forward, hybrid, mobile, satellite communication, integration model, nurse-led teledermatology, teledermatology focusing on difficult-to-manage cases, teledermoscopy, and teledermatopathology with combined applications. This article reviews the feasibility studies focusing teledermatology tools and analyses the possible options in designing TDP. Categorizing dermatoses for TDP depends on the purpose and types of technology. The dermatoses presenting from a remote geographic regions requires any of the following approaches (i) only TDP, (b) initial TDP followed by face-to-face, (iii) initial face-to-face followed by TDP and (iv) only face-to-face examination. The technology should suit the dermatoses, meet the purpose, be cost-effective and provide better management with follow-up care. We recommend store and forward as a basic TDP model as most dermatoses are diagnosed and follow-up care is delivered. Leprosy, pigmented skin lesions, leg ulcers, HIV and endemic dermatoses require screening and triage services using mobile teledermatology. Counselling and education require videoconference. Rural dermatology's camps require satellite communication mounted on a vehicle. Objective assessment (vitiligo and leg ulcer) after treatment requires integration model at a tertiary centre. Difficult-to-manage cases require second opinion using hybrid/store and forward TDP. Lower rural centre are provided with mobile/ store and forward teledermatology services. Selected or major community centre should be equipped with hybrid teledermatology and linked to a tertiary centre. This process helps healthcare administration to plan a TDP to cover all dermatoses, utilizing the available health care professional (HCP) and technology with minimum budget investment. Land degradation control and its global environmental benefits

Abstract Acknowledged by world leaders as a global problem, land degradation has been taken seriously in three ways: its extent and the proportion of the global population affected; international environmental policy responses; and its inter-relation with other global environmental issues such as biodiversity. Messages about land degradation have, however, suffered from abuses, which have rendered appropriate policy responses ineffective. For control to be effective, the paper argues that the synergies between land degradation and the two other main global environmental change components (biodiversity and climate change) should be more fully exploited. A focus on the interlinkages, of which there are six possible permutations, is fully supported by empirical findings that suggest that land degradation control would not only technically be better served by addressing aspects of biodiversity and climate change but also that international financing mechanisms and the major donors would find this more acceptable. The DPSIR (Driving Force, Pressure, State, Impacts, Response) conceptual framework model is used to illustrate how land degradation control could be more effective, tackling not only the drivers of change but also major developmental issues such as poverty and food insecurity. Common characteristics of upper and lower airways in rhinitis and asthma: ARIA update, in collaboration with GA2LEN

This update aimed to review the new evidence available to support or refute prior Allergic Rhinitis and its Impact on Asthma (ARIA) statements. A Medline search of publications between 2000 and 2005 was conducted, with articles selected by experts. New evidence supports previous ARIA statements, such as: (i) allergic rhinitis (AR) is a risk factor for asthma; (ii) patients with persistent rhinitis should be evaluated for asthma; (iii) most patients with asthma have rhinitis; (iv) a combined strategy should be used to treat the airways and (v) in low- to middle-income countries, a different strategy may be needed. The increased risk of asthma has also been found among sufferers from non-AR. Recent reports show AR is a global problem. Many studies demonstrated parallel increasing prevalence of asthma and rhinitis, but in regions of highest prevalence, it may be reaching a plateau. Factors associated with a reduced risk of asthma and AR have been identified, confirming previous findings of protection related to exposure to infections. Treatment of rhinitis with intranasal glucocorticosteroids, antihistamines, leukotriene antagonists or immunotherapy may reduce morbidity because of asthma. To take advantage of the paradigm of unified airways, there is a need to rationalize diagnosis and treatment to optimize management. [source]


Is losing sleep making us obese?

Summary Obesity has become pandemic. In America, as obesity has increased, the amount of sleep Americans get per night has decreased, and studies are now showing an association. Epidemiological studies on short sleep duration (SSD) and obesity have been conducted in children and adults, and show an overall positive association. Leptin and ghrelin, two hormones that control appetite, have been studied as a mechanism for SSD causing obesity. Low leptin and high ghrelin levels have been seen in sleep deprivation, the effect of which is an increase in appetite that could be linked to obesity. Decreasing media use, namely television and computers, could be one way to increase nightly sleep and potentially help people lose weight. Paediatric studies have shown an association with bedroom media use and shorter sleep duration. Adult studies are lacking in this area. Limitations in the literature include self-report in a majority of sleep studies and only a suggested causal link between SSD and obesity among all of the epidemiological studies. In conclusion, obesity is a global problem with great complexity. Encouraging people to get more sleep could be one part of the solution to help them lose weight and gain health. [source]


Aorta protein networks in marginal and acute zinc deficiency

Abstract Human zinc deficiency is a global problem and may influence the development of cardiovascular disease. Our objective was to determine Zn deficiency affected pathways and protein interactions in rat aorta and their likely influence on stress-induced atherogenesis. In two separate studies, rats were given diets acutely (<1,mg Zn/kg) or marginally (6,mg Zn/kg) deficient in Zn. Both studies included Zn adequate controls (35,mg Zn/kg) and the acute deficiency study included a pair-fed group. After 6,wk, proteins from thoracic aorta were separated by 2-DE. Proteins affected by zinc deficiency were identified by principal component analysis. Multiple correlations of identified proteins indicated protein networks of related function. Proteins clusters decreased in zinc deficiency were related to fatty acid and carbohydrate metabolism. Structurally related proteins, including zyxin and over nine transgelin 1 proteins, were either increased or decreased by acute and marginal deficiencies. PKC, was significantly decreased in Zn deficiency suggesting that Zn may regulate the phosphorylation of target proteins. Zn deficiency-related changes in structural, carbohydrate and fatty acid-related proteins may be disadvantageous for maintaining vascular health and are consistent with a protective role for zinc in the development of atherosclerosis. Outcome Analysis of Patients with Squamous Cell Carcinoma of the Head and Neck and Hepatitis C Virus,

Abstract Objective/Hypothesis: Infection with the hepatitis C virus (HCV) is a global problem with over 170 million people infected. Recently, we have noticed that a large number of patients diagnosed with squamous cell carcinoma of the head and neck (SCCHN) have also been diagnosed with HCV. A review of the literature reveals little information concerning this patient population. The objective of this study was to compare the outcome of SCCHN patients who have been exposed to HCV with naïve SCCHN patients. Study Design: Retrospective chart review. Methods: A retrospective chart review from June 1991 through December 2002 was performed to identify patients diagnosed with SCCHN who were screened for HCV. Patients were stratified into two groups (HCV positive and HCV negative). Data were recorded on patients for status of disease at last clinic visit, pretreatment serum albumin and hematocrit levels, and RNA quantities of HCV. Statistical analysis was performed using paired t test to compare serum albumin and hematocrit levels. Kaplan-Meier survival curves were used to compare outcomes. The log-rank test was used to determine significance. Cox regression was used to examine the association of prognostic predictor variables with overall survival and disease-free survival. Results: There was no difference noted in 5 year survival between hepatitis C positive and hepatitis C negative groups in overall outcomes (66.7% vs. 67.9%, P = 1.000) or 5 year disease-free survival (90.5% vs. 80.8%, P = .514). The two groups, HCV positive versus HCV negative, also had similar serum albumin levels (3.62 g/dL vs. 3.72 g/dL, P = .37) as well as serum hematocrit levels (42.9% vs. 41.0%, P = .12). Serum levels of hepatitis C RNA were obtained in seven patients, with only one being undetectable. The only prognostic predictor variable that was significantly associated with overall survival was age. None of the predictor variables were significantly associated with disease-free survival. Conclusion: Co-infection with HCV, although prevalent in the Veterans Administration Hospital population, did not affect patient outcome as defined by disease-free survival. Patients who were seropositive for HCV had comparable serum albumin levels as well as serum hematocrit when compared with HCV negative patients. [source]

Abstract Extraordinarily effective heavy metal ion scavenger: We show here that the material K2xMnxSn3,xS6 (x=0.5,0.95) (KMS-1) overcomes the limitations of the known heavy metal ion sorbents, showing the capability to rapidly reduce the concentrations of Cd, Hg, and Pb ions well-below the legally acceptable levels for drinking water. KMS-1 is inexpensive, easily prepared in large quantities, and may play a role in addressing the global problem of water contamination with heavy metal ions. The paradigm of heavy metal ion absorption of KMS-1 is the ability to form very strong MS bonds. [source]


Combating resistance in a challenging, changing environment

Abstract The prevalence of antimicrobial resistance for both Gram-positive and Gram-negative pathogens is escalating worldwide. Outbreaks of community- and hospital-acquired methicillin-resistant Staphylococcus aureus (MRSA) are being reported more frequently. Although antimicrobial resistance is well recognised as a global problem, decisions about appropriate intervention and treatment should be made at the level of the local hospital or healthcare system. Thus, local surveillance to identify prevalent pathogens, detect bacterial resistance and identify particular strains is necessary for selecting optimal treatment regimens. In addition, bactericidal antimicrobial agents with novel mechanisms of action and activity against multidrug-resistant bacteria, together with improved infection control measures, are needed to address this growing medical problem more effectively. The American national interest and global public goods

Since the end of the Cold War, Americans have been divided over how to be involved with the rest of the world. In the wake of the 11 September terrorist attacks, the debate between those who favour a unilateral foreign policy and those who advocate a multilateral approach has been brought to the fore in American politics and the media. In this article, Joseph Nye proposes a conception of the American national interest grounded in multilateralism. He argues that, although the United States remains the world's leading power, it cannot act alone to solve global problems such as transnational terrorism, the proliferation of weapons of mass destruction and global warming. Although the United States is the only country in a position to take the lead in protecting ,global public goods', such as an open international economic system and international stability, it will maintain its current predominance only if it works to establish international consensus on issues of global importance. 1.,Globalization and Violence: The Challenge to Ethics

Despite its many benefits, globalization has proven to harbor a good deal of violence. This is not only a matter of the proliferation of weapons of mass destruction inaugurated by the atomic bombing of Hiroshima, but includes many forms of indirect or "structural violence" resulting from the routine of economic and political institutions on the global scale. In this essay, the multifaceted phenomena of violence are approached from the standpoint of ethics. The prevailing political thinking associated with "realism" fails to address the problems of militarism and of hegemonic unilateralism. In contrast, many philosophers are critically rethinking the problem of global violence from different ethical perspectives. Despite sharing similar concerns, philosophers nevertheless differ over the role of philosophical reflection and the potentials of reason. These differences appear in two contrasting approaches associated with postmodern philosophy and discourse ethics. In the analysis of discourse ethics, attention is paid to Karl-Otto Apel's attempt of philosophically grounding a macroethics of planetary co-responsibility. At the heart of the essay is the analysis of the problem of violence, including terrorism, by Jürgen Habermas, who explains the phenomenon of violence in terms of the theory of communicative action as the breakdown of communication. Jacques Derrida's deconstruction of the notion of "terrorism" also is analyzed. According to the principle of discourse ethics, all conflicts between human beings ought to be settled in a way free of violence, through discourses and negotiations. These philosophers conclude that the reliance on force does not solve social and global problems, including those that are the source of violence. The only viable alternative is the "dialogical" multilateral relations of peaceful coexistence and cooperation among the nations for solving social and global problems. They emphasize the necessity of strengthening the international rule of law and institutions, such as a reformed United Nations. [source]
