Nutritional factors affecting serum phenylalanine concentration during pregnancy for identical twin mothers with phenylketonuria

The effect of energy, protein, fat, and phenylalanine on serum phenylalanine concentrations during pregnancy for a set of identical twins with phenylketonuria (PKU) was examined. Blood samples were collected one to two times per week. The subjects completed a 3-d food record prior to each blood collection. The effect of the factors on serum phenylalanine levels was evaluated statistically using time-series analysis. Dietary intakes of the nutrients evaluated were similar for the subjects. For one subject, there were highly significant effects of energy, protein, and fat on serum phenylalanine levels. In contrast, these nutrients had no significant effect on serum phenylalanine for the other subject. Dietary phenylalanine had no significant effect on serum phenylalanine for either twin. Conclusions: There was no effect of phenylalanine intake and no consistent effect of energy, protein, or fat on serum phenylalanine. Other dietary or environmental factors or a combination of factors may impact serum phenylalanine levels of pregnant women with PKU. [source]


Monetary costs associated with bulimia

Abstract Objective: The direct monetary costs for food and laxatives, diet pills, and diuretics used by individuals with bulimia nervosa (BN) have not been studied. Method: Ten participants with a presenting clinical diagnosis of BN completed a 7-day food record at the outset of treatment in order to provide estimates of weekly and yearly monetary costs for food and associated symptoms. Results: Participants reported means of 3.6 purge episodes, 2.5 objective binge episodes, and 2.4 subjective binge eating episodes per week. Mean total food costs were $106.98/week (SD = $53.88) or $5,581.79/year (SD = $2,811.58). Costs associated with binge eating and purging were 32.7% of all food costs (average: $1,599.45/year). Conclusion: Financial costs of bulimia symptoms are significant, and appear to constitute a substantial minority of all food spending by individuals with BN. © 2008 by Wiley Periodicals, Inc. Growth and Bone Mineral Accretion During Puberty in Chinese Girls: A Five-Year Longitudinal Study,,

Abstract There are few longitudinal data on bone development during puberty in children with low calcium intake. This 5-yr longitudinal study showed that, in Chinese girls, the mean apparent calcium retention efficiency during puberty was 40.9%, PHV occurred at 3,0 yr before menarche, and peak bone mineral accretion occurred 1 yr later than PHV. Chinese girls have high calcium retention efficiency during puberty. Introduction: There are few longitudinal data on bone development during puberty in children with low dietary calcium intake. The aim of this study was to examine the rate of growth and bone mineral accretion and study the predictors of total body BMC during puberty in a 5-yr longitudinal study with Chinese girls. Materials and Methods: Ninety-two girls, 9.5,10.5 yr of age at baseline, from the unsupplemented control group of a school milk intervention trial were included in this analysis. Data on anthropometric measurements, total body BMC as assessed by DXA, and calcium intake as assessed by a 3-day food record were obtained at baseline and 1, 2, 4, and 5 yr. Results: The mean age of menarche was 12.1 ± 1.0 yr. The mean annual rate of bone mineral accretion was 197.4 g/yr during the follow-up period, representing a calcium accretion rate of 162.3 mg/d. This calcium retention rate and the average dietary calcium intake of 444.1 mg/d gave an apparent calcium retention efficiency of 40.9%. Peak height velocity (PHV) occurred at 3,0 yr before menarche. Peak bone mineral accretion occurred 1 yr later than PHV. There was a decrease in size-corrected BMD in the year before menarche. In the linear mixed-effects model analysis containing body size and lifestyle factors, we found that height, body weight, and calcium intake were significant independent predictors of total body BMC. Conclusions: Chinese girls with low habitual dietary calcium intake have high calcium retention efficiency during puberty. Because calcium intake is a significant predictor of total body BMC, increasing dietary calcium intake may have beneficial effects on bone mineral accretion in these girls. [source]


A quantitative analysis of energy intake reported by young men

Abstract Aim:, To quantitatively analyse energy intake reported by young men and the accuracy of the Goldberg cut-off method for identifying misreporters. Methods:, This was a cross-sectional study in which: food intake was assessed by a four-day food record; resting metabolic rate was assessed by indirect calorimetry; percentage body fat was measured by dual-energy X-ray absorptiometry; and energy expenditure was assessed by physical activity record completed simultaneously with food intake measurements. Energy intake was analysed by direct comparison of energy intake and energy expenditure and by the Goldberg cut-off. Subjects: 34 healthy men aged 18,25 years. Setting: Queensland University of Technology, Queensland, Australia. Main outcome measures: percentage of misreporters in a group of young men using different methods. Statistical analyses: data are presented as means and standard deviations. The analyses were conducted using Statistic for Windows 5.5 software. Results:, Seven underreporters were identified by direct comparison of energy intake and energy expenditure. The Goldberg cut-off found six out of the seven underreporters identified by direct comparison of energy intake and energy expenditure, but wrongly identified two acceptable reporters as underreporters. The sensitivity and specificity of the Goldberg cut-off method were 0.86 and 0.93, respectively. Conclusions:, Seven out of 34 participants underreported their energy intake. In the absence of physical activity measurements, the Goldberg cut-off method identified underreporters in this group of young men with assessed resting metabolic rate. [source]


Comparison of nutrient intake between different dietary assessment methods in elderly male volunteers

Abstract Aim:, The objective of the study was to compare nutritional intake results obtained from the 4 × 4-day weighed records with those obtained from a food frequency questionnaire, repeated 24-hour recalls, a seven-day food record and a seven-day checklist in elderly men. Methods:, Seventy-five healthy men aged 55,88 years and living at home in Cambridge, UK, took part in the study. Energy and nutrients had means and standard deviations calculated. Misreporting was based on an evaluation of food intake level, calculated as reported energy intake divided by predicted basal metabolic rate. This was in relation to a plausible physical activity level, calculated as the ratio of energy expenditure divided by predicted basal metabolic rate. Statistical significance was assessed via one-way anova. Results:, The nutrient density was higher for protein and potassium for the group with low food intake level values in all dietary assessment methods. For some methods, this was also true for calcium, carotene, iron and vitamin C. All methods yielded similar results. Conclusions:, The present study indicates that selective underreporting exists. Surprisingly, the simplest method, the simplified 24-hour recall, performed as well as more complicated methods. [source]


Nutritional variables related to gingival health in adolescent girls

Abstract , In order to study the nutritional variables associated with gingival health, a case-control study was designed to control strong variables whose effect on gingival status may obscure the potential effect of weaker ones, such as nutrition. Two groups of 27 gingivitis-affected and -unaffected female adolescents were selected. All were aged 17,19 years, with mean age of the two groups statistically not different. All were non-smokers, all reported daily toothbrushing frequency of twice/day or more, and none had clinical signs of hyponutrition. Mean DMFT of the two groups was statistically not different. The effect of nutritional variables, obtained by a three-day food record and by assessing the nutritional status of the girls, on presence/absence of gingivitis was evaluated by a variety of stepwise logistic regression analyses. Age (positive correlation), riboflavin, calcium and frequency of fibre intake (negative correlations) significantly explained the risk for gingivitis. Strong intercorrelation between riboflavin and calcium was also found, due to the high quantity of milk consumed by the girls, since this food provided the main source of riboflavin and calcium. The data suggest that some dietary measures may be useful for the maintenance of healthy gingival status. [source]


Familial aggregation in the night eating syndrome

Abstract Objective: This study examined the extent to which the night eating syndrome (NES) affects first-degree relatives of NES and control probands. Method: NES participants and controls were assessed with the Night Eating Questionnaire (NEQ), the Night Eating Syndrome History and Inventory (NESHI), 10 day sleep and food records, the Eating Disorder Examination (EDE), the Structured Clinical Interview for DSM IV Axis I Disorders (SCID I), and a Family History Questionnaire (FHQ) to assess the presence of NES among first-degree relatives. A proband predictive model, using logistic regression analyses and the generalized estimating equation to control for correlation among observations within families was used to assess familial aggregation. Results: The odds of an NES proband having an affected first-degree relative were significantly greater than that of a control proband (odds ratio = 4.9, p < .001). A number of covariates were included in the model: proband body mass index (BMI) (kg/m2), proband gender, proband age, proband ethnicity, first-degree relative gender, relationship to proband (i.e., mother, father, or sibling), and the interaction between relationship to proband and proband status (night eater or control); none was statistically significant (p > .05). Estimating food intakes in Australia: validation of the Commonwealth Scientific and Industrial Research Organisation (CSIRO) food frequency questionnaire against weighed dietary intakes

Abstract Background:, There is a dearth of knowledge about the foods that Australian adults eat and a need for a flexible, easy-to-use tool that can estimate usual dietary intakes. The present study was to validate a commonly used Australian Commonwealth Scientific and Industrial Research Organisation (CSIRO) food-frequency questionnaire (C-FFQ) against two 4-day weighed food records (WFR), as the reference method. Methods:, The C-FFQ, as the test item, was administrated before the WFR. Two 4-day WFR were administrated 4 weeks apart. Under-reporting was established using specific cut-off limits and estimated basal metabolic rate. Seventy-four women, aged 31,60 years, were enrolled from a free-living community setting. Results:, After exclusion for under-reporting, the final sample comprised 62 individuals. Correlations between protein intake from the WFR and urinary urea were significant. Overall agreement between FFQ and WFR was shown by ,levels of agreement' (LOA) and least products regressions. There was presence of fixed and proportional bias for almost half the nutrients, including energy, protein, fat and carbohydrates. For most of the nutrients that did not present bias, the LOA were 50,200%. Agreement was demonstrated for percentage dietary energy protein and fat; carbohydrate; and absolute amounts of thiamine, riboflavin, magnesium and iron. However, relative intake agreement was fair to moderate, with approximately 70% of (selected) nutrients exact or within ±1 quintile difference. Conclusion:, The C-FFQ is reasonable at measuring percentage energy from macronutrients and some micronutrients, and comprises a valuable tool for ranking intakes by quintiles; however, it is poor at measuring many absolute nutrient intakes relative to WFR. Quantifying calcium intake in school age children: Development and validation of the Calcium Counts!© food frequency questionnaire,

Quantifying dietary behavior is difficult and can be intrusive. Calcium, an essential mineral for skeletal development during childhood, is difficult to assess. Few studies have examined the use of food frequency questionnaires (FFQs) for assessing calcium intake in school-age children. This study evaluated the validity and reliability of the Calcium Counts!© FFQ (CCFFQ) for estimating calcium intake in school children in the US. Healthy children, aged 7,10 years (n = 139) completed the CCFFQ and 7-day weighed food records. A subset of subjects completed a second CCFFQ within 3.6 months. Concurrent validity was determined using Pearson correlations between the CCFFQ and food record estimates of calcium intake, and the relationship between quintiles for the two measures. Predictive validity was determined using generalized linear regression models to explore the effects of age, race, and gender. Inter- and intra-individual variability in calcium intake was high (>300 mg/day). Calcium intake was ,300 mg/day higher by CCFFQ compared to food records. Concurrent validity was moderate (r = 0.61) for the entire cohort and higher for selected subgroups. Predictive validity estimates yielded significant relationships between CCFFQ and food record estimates of calcium intake alone and in the presence of such potential effect modifiers as age group, race, and gender. Test,retest reliability was high (r = 0.74). Although calcium intake estimated by the CCFFQ was greater than that measured by food records, the CCFFQ provides valid and reliable estimates of calcium intake in children. Weighted Normality-Based Estimator in Correcting Correlation Coefficient Estimation Between Incomplete Nutrient Measurements

Summary. Consider the problem of estimating the correlation between two nutrient measurements, such as the percent energy from fat obtained from a food frequency questionnaire (FFQ) and that from repeated food records or 24-hour recalls. Under a classical additive model for repeated food records, it is known that there is an attenuation effect on the correlation estimation if the sample average of repeated food records for each subject is used to estimate the underlying long-term average. This paper considers the case in which the selection probability of a subject for participation in the calibration study, in which repeated food records are measured, depends on the corresponding FFQ value, and the repeated longitudinal measurement errors have an autoregressive structure. This paper investigates a normality-based estimator and compares it with a simple method of moments. Both methods are consistent if the first two moments of nutrient measurements exist. Furthermore, joint estimating equations are applied to estimate the correlation coefficient and related nuisance parameters simultaneously. This approach provides a simple sandwich formula for the covariance estimation of the estimator. Finite sample performance is examined via a simulation study, and the proposed weighted normality-based estimator performs well under various distributional assumptions. The methods are applied to real data from a dietary assessment study. [source]


Carbohydrate intake, serum lipids and apolipoprotein E phenotype show association in children

Abstract Aim:, To study the association between carbohydrate intake and serum lipids in children, and influence of apolipoprotein E phenotype (apoE) on the association. Subjects/methods:, A total of 644 children from a prospective, randomized atherosclerosis prevention trial (STRIP) participated in this longitudinal study at age 5 (n = 644), 7 (n = 585) and 9 (n = 550) years. ApoE phenotype, fasting triglyceride, total, low-density lipoprotein (LDL) and high-density lipoprotein (HDL) cholesterol concentrations and 4-day food records were analysed. Results:, An increase in the total carbohydrate intake by 1 E% (percentage of total daily energy intake) associated with a decrease in HDL cholesterol by 0.006 mmol/L (p < 0.001) when adjusted for saturated, monounsaturated and polyunsaturated fatty acid, age, gender, body mass index and STRIP study group. The inverse association between total carbohydrate intake and HDL cholesterol was evident in children with apoE3 (p < 0.001) or apoE4 (p < 0.001), but not in those with apoE2 (p = 0.78). An increase in total carbohydrate intake by 1 E% increased triglycerides by 0.02 mmol/L (p < 0.001) independently of apoE phenotype, while 1 E% increase in sucrose intake increased triglycerides by 0.01 mmol/L (p < 0.001). Conclusion:, Carbohydrate intake has a relatively small effect on serum lipids in children. Children with the apoE3 or E4 but not with E2 phenotype show reduction in HDL cholesterol with increasing carbohydrate intake indicating that genetic and environmental factors interact with children's lipoprotein metabolism. Maternal consumption of dairy products during pregnancy and lactation, and the development of cow's milk antibodies in the offspring

Abstract Objective: To assess whether the maternal consumption of milk and milk products affects development of cow's milk (CM) antibodies in infants. Design: A randomized pilot trial using food frequency questionnaires (mothers) and food records (infants). Setting: Families with a newborn infant with increased HLA-DQB1-conferred risk of type 1 diabetes and at least one first-degree relative affected by type 1 diabetes from 16 hospitals in Finland between April 1995 and November 1997. Subjects and intervention: Infants randomized to receive a hydrolysed formula when breast milk was not available during their first 6,8 mo (n=112). Of these, 13 dropped out by the age of 3 mo and two were excluded due to incomplete CM antibody data. Results: Maternal milk protein intake from cheese during pregnancy was inversely related to IgA-class antibody titres to beta-lactoglobulin (BLG) and casein (CAS) at 3 mo, and to IgA antibody titres to BLG at 6 mo. Maternal consumption of raw milk products during lactation was positively related to the development of IgA antibody titres to CAS at 6 mo, and inversely correlated to IgG antibody titres to bovine serum albumin (BSA) and IgA antibody titres to CAS at 2 y. Maternal cheese consumption was inversely related to the IgG antibody titres to CM formula and CAS and to the IgA antibody titres to CAS in early infancy. Conclusions: Few associations were established between maternal CM protein intake and CM protein antibody levels in the infants. The milk and milk products taken by the mother differed in their impact on the emerging CM antibody response in the offspring. [source]
