    Feeding Practices of Dog and Cat Owners Reflect Attitudes Toward Pet Foods

    The PET DINER study was a telephone survey conducted to better understand why pet owners make certain nutritional decisions for their pets. Both dog and cat owners were included, which allowed us to differentiate the way people feed pet cats vs. pet dogs and how that might reflect owners' attitudes about pet foods. Prospective survey respondents were selected from local telephone books using a randomized process. A total of 18,194 calls were made from the five study sites between May and August, 2004. 1074 people (6%) representing 619 dogs and 455 cats completed the survey. Detailed information about feeding practices was collected. Pet owners' perceptions were assessed based on agreement/disagreement with statements regarding pet foods and the pet food industry. The Mann,Whitney Rank Sum test was used to detect differences between dog and cat owners and the Kruskal,Wallis one-way analysis of variance on ranks was used to evaluate differences among pet owners based on both type of pet and feeding practices (,75% commercial vs. ,50% home-prepared diets). More cats than dogs ate commercial pet food as ,75% of their main diet (96% vs. 87%, respectively; p < 0.001), and more dogs than cats ate a home-prepared food as ,50% of their main diet (6% vs. 3%, respectively; p < 0.009). For five of the statements, a difference in the attitudes of cat vs. dog owners was detected (p < 0.05). In general, this reflected more positive attitudes regarding commercial foods and less positive attitudes regarding home-prepared foods on the part of cat vs. dog owners. Analysis based on both type of pet and feeding practices identified significant differences among groups for 18/26 statements. However, when the Dunn's Method for pairwise multiple comparisons was applied, significant differences were due to feeding practices rather than type of pet for 17/18 statements. Only differences in attitudes about the statement ,raw bones can be safely fed to pets' remained significant for cat vs. dog owners feeding commercial foods. In conclusion, more cat owners than dog owners feed ,75% commercial foods to their pets and this is reflected in different attitudes about the nutritional soundness of commercial pet foods.     Carotenemia in Infancy and its Association with Prevalent Feeding Practices

    It is characterized by an abnormal yellowish orange pigmentation of the skin, most prominently seen in the palms, soles, and naso-labial folds. Infant feeding patterns have shown an increasing trend toward the usage of homogenized and pureed vegetables as well as meat-based commercial preparations. Whether this is reflected in an increased incidence of carotenemia in this age group still remains unclear. We report a series of infants identified by a retrospective review of records, observed over a 3-year period (1999,2002) in a tertiary children's hospital, who developed the condition that resolved spontaneously without intervention, as they grew older, on a changing diet.     Risk factors for faecal sand excretion in Icelandic horses

    Summary Reasons for performing study: Sandy soil is often mentioned as a risk factor in the development of sand-related gastrointestinal disease (SGID) in the horse. There are other variables, but few studies confirm any of these. Objective: To investigate soil type, pasture quality, feeding practice in the paddock, age, sex and body condition score as risk factors for sand intake in the horse. Methods: Faeces were collected from 211 Icelandic horses on 19 different studs in Denmark together with soil samples and other potential risk factors. Sand content in faeces determined by a sand sedimentation test was interpreted as evidence of sand intake. Soil types were identified by soil analysis and significance of the data was tested using logistic analysis. Results: Of horses included in the study, 56.4% showed sand in the faeces and 5.7% had more than 5 mm sand as quantified by the rectal sleeve sedimentation test. Soil type had no significant effect when tested as main effect, but there was interaction between soil type and pasture quality. Significant interactions were also found between paddock feeding practice and pasture quality. Conclusion: To evaluate the risk of sand intake it is important to consider 3 variables: soil type, pasture quality and feeding practice. Pasture quality was identified as a risk factor of both short and long grass in combination with sandy soil, while clay soil had the lowest risk in these combinations. Feeding practice in the paddock revealed feeding directly on the ground to be a risk factor when there was short (1,5 cm) or no grass. Also, no feeding outdoors increased the risk on pastures with short grass, while this had no effect in paddocks with no grass. More than 50% of all horses investigated in this study had sand in the faeces. Potential relevance: The identification of risk factors is an important step towards prevention of SGID. Further research is necessary to determine why some horses exhibit more than 5 mm sand in the sedimentation test and whether this is correlated with geophagic behaviour. [source]


    Feeding practices of HIV-1-infected mothers: The role of counsellors

    Abstract In this issue of Acta Paediatrica, Chopra et al. report that voluntary counselling is central to preparing mothers for making a proper informed choice about adequate feeding practices to prevent their infants from acquiring HIV infection. The recommendations given and the way in which counselling is performed are the most important determinants of a mother's decision about how to feed her infant. In this article, we summarize the main arguments for and against breastfeeding by HIV-infected mothers. Conclusions : Further studies are needed to determine the alternatives to breastfeeding in countries where there is no access to safe formula feeding or to antiretroviral drugs. HIV-positive mothers should be made aware of the available feeding alternatives through adequate counselling from properly trained persons. [source]


    Feeding practices of infants through the first year of life in Italy

    Aim: To investigate infant feeding practices through the first year of life in Italy, and to identify factors associated with the duration of breastfeeding and early introduction of solid foods. Methods: Structured phone interviews on feeding practices were conducted with 2450 Italian-speaking mothers randomly selected among women who delivered a healthy-term singleton infant in November 1999 in Italy. Interviews were performed 30 d after delivery and when the infants were aged 3, 6, 9 and 12 mo. Type of breastfeeding was classified according to the WHO criteria. Results: Breastfeeding started in 91.1% of infants. At the age of 6 and 12mo, respectively, 46.8% and 11.8% of the infants was still breastfed, 68.4% and 27.7% received formula, and 18.3% and 65.2% were given cow's milk. Solids were introduced at the mean age of 4.3 mo (range 1.6,6.5 mo). Introduction of solids occurred before age 3 and 4 mo in 5.6% and 34.2% of infants, respectively. The first solids introduced were fruit (73.1%) and cereals (63.9%). The main factors (negatively) associated with the duration of breastfeeding were pacifier use (p > 0.0001), early introduction of formula (p > 0.0001), lower mother's age (p > 0.01) and early introduction of solids (p= 0.05). Factors (negatively) associated with the introduction of solids foods before the age of 3 mo were mother not having breastfed (p > 0.01), early introduction of formula (p > 0.01), lower infant bodyweight at the age of 1 mo (p= 0.05) and mother smoking (p= 0.05). Conclusion: The duration of breastfeeding in Italy is still inadequate, as well as compliance with international recommendations for timing of introduction of complementary foods. National guidelines, public messages and educational campaigns should be promoted in Italy. [source]

    The course of some bone remodelling plasma metabolites in healthy horses and in horses offered a calcium-deficient diet

    Summary An inquiry was carried out to assess the concentrations of plasma metabolites related to bone remodelling in 21 saddle horses of Warmblood breed aged 4,26 years, five draught horses of Ardennes breed aged 4,10 years, and 10 Ardennes foals aged 9,11 months. They were fed according to normal feeding practice in Belgium. The changes in some bone remodelling plasma metabolite concentrations were studied when an unbalanced diet was offered and later corrected for four Warmblood horses. Bone formation was evaluated by bone alkaline phosphatase (BALP), total alkaline phosphatase (TALP) and osteocalcin (bone gla-protein, OC). Bone resorption was assessed by hydroxyproline (HYP). Total calcium, ionized calcium, phosphorus (P) and 25-hydroxyvitamin D3 [25-(OH)D] concentrations were more or less constant. The comparison of four bone remodelling factors between the Ardennes and Warmblood horses showed higher concentrations in the Ardennes breed. Bone marker concentrations decreased according to age. The correction of the unbalanced Ca : P diet induced inconsistent effects at plasma level. The interpretation of the different bone parameters appeared to be difficult if not associated with other parameters such as a complete anamnesis and clinical examination of the animal in addition to dietary evaluation. Zusammenfassung Verlauf verschiedener Knochenmarker bei gesunden Pferden und bei Pferden, welche mit einer in Bezug auf Kalzium unausgewogenen Ration gefüttert wurden Eine Studie zur Erfassung der Konzentrationen von Knochenmarkern wurde bei 21 Warmblütern im Alter von 4 bis 26 Jahren, fünf Ardenner Kaltblütern im Alter von 4 bis 10 Jahren und 10 Ardenner Kaltblutfohlen im Alter von 9 bis 11 Monaten durchgeführt. Die Pferde wurden gemäss der normalen Fütterungpraxis in Belgien gefüttert. Der Verlauf der Knochenmarkerkonzentrationen wurde auch bei vier Pferden gemessen, die zunächst mit einer unausgewogenen Ration in Bezug auf Kalzium und dann mit einer korrigierenden Ration gefüttert wurden. Der Knochenaufbau wurde anhand der Aktivität der knochenspezifischen alkalischen Phosphatase (BALP), der totalen alkalischen Phosphatasen (TALP) und anhand des Osteocalcin (bone gla-proteine, OC) gemessen. Der Knochenabbau wurde anhand des Hydroxyprolins (HYP) gemessen. Die Konzentrationen des totalen Kalziums, ionisierten Kalziums, Phosphors (P), und 25-Hydroxyvitamin D3 [25(OH)D] waren unverändert. Beim Vergleich der vier gemessenen Knochenmakerkonzentrationen bei den Ardenner Kaltblütern mit den Warmblutpferden konnte gezeigt werden, dass die Kaltblüter deutlich höhere Konzentrationen hatten als die Warmblüter. Die Konzentrationen der Marker nahmen mit steigendem Alter der Pferde ab. Die Korrektur der unausgewogenen Ca:P Ration ergab nicht eindeutige Veränderungen der Plasmakonzentrationen der verschiedenen Marker. Die Interpretation der verschiedenen Knochenmarker erscheint schwierig, wenn nicht andere Parameter, wie eine komplette Anamnese und eine klinische Untersuchung, sowie eine Auswertung der Ration hinzugezogen werden.     Supplementary winter feeding of wild red deer Cervus elaphus in Europe and North America: justifications, feeding practice and effectiveness

    ABSTRACT 1.,Supplementary winter feeding of game animals, and particularly deer, is a common practice throughout northern (continental) Europe and parts of North America. Feeding is normally associated with maintaining high densities of animals for hunting, in terms of: (i) maintaining or increasing body weights and condition overwinter; (ii) improving reproductive performance and fertility; (iii) increasing overwinter survival; and (iv) reducing levels of damage caused to agriculture and forestry or the natural heritage. We consider the balance of evidence on the effectiveness of winter feeding of red deer Cervus elaphus in achieving these objectives. Where that evidence is equivocal, we attempt to reconcile apparent contradictions to evaluate the circumstances under which winter feeding may or may not be effective. 2.,In general, feeding of red deer on open range appears to have relatively little effect on body weights or fecundity. Effects on increasing antler size and quality are variable and seem to depend on the degree to which animals may be mineral limited on native range. Effects on survival are similarly ambiguous. It is apparent, however, that to be effective in reducing mortality, any supplementation is required early in the season and not simply when heavy mortalities are already being experienced. If provision of supplementary foods is delayed until animals are perceived already to be in poor condition, such feeding may have little effect. 3.,One of the primary goals of winter feeding in both Europe and the USA has become the prevention of environmental damage, particularly damage to commercial and native forests, while maintaining deer populations at densities suitable for hunting. Again, empirical evidence for effectiveness in this regard is inconclusive, with some studies showing a decrease in damage caused, some showing no effect and others showing a significant increase in local impact. 4.,There are equally a number of problems associated with the provision of supplementary feeds overwinter. Those animals which come to the feeding stations may develop a reliance on the food supplement provided, reducing intake of natural forages to near zero; where feed provided is less than 100% of daily requirement, such animals may regularly lose, rather than gain condition. Feed provision is also extremely uneven at such feeding stations; dominant stags displace younger stags and hinds from the feed provided until they have themselves finished feeding. Concentrations of high densities of animals around small feed-areas may also increase the risk of infection and lead to development of high parasite burdens. 5.,In an attempt to assess the current status and distribution of supplementary winter feeding in Scotland, a questionnaire was circulated to a number of individual across the country. Results of this survey are summarized and conclusions presented on the likely effectiveness of current feeding practices in achieving their aims. [source]


    Exclusive Breast-feeding: Does It Have the Potential to Reduce Breast-feeding Transmission of HIV-1?

    Exclusive breast-feeding is unambiguously the optimal infant feeding practice and is universally promoted in the absence of human immunodeficiency virus (HIV-1). It is associated with reduced morbidity and mortality from diarrheal and respiratory diseases. Recent findings suggest that exclusive breast-feeding may pose less risk of HIV-1 transmission than the more common practice of mixed feeding (i.e., breast-feeding concurreptwith the feeding of water, other fluids, and foods), which has important infant feeding policy implications for low-resource settings. This paper reviews the biologic mechanisms associated with exclusive breast-feeding that provide protection against gastrointestinal, respiratory, and atopic diseases, and evaluates the relevance of these mechanisms for HIV-1 transmission. Potential mechanisms include reduction in dietary antigens and enteric pathogens that may maintain integrity of the intestinal mucosal barrier and limit inflammatory responses of the gut mucosa; promotion of beneficial intestinal microflora that may increase resistance to infection and modulate the infant's immune response; alteration in specific antiviral or anti-inflammatory factors in human milk that may modulate maternal hormonal or immunologic status; and maintenance of mammary epithelial integrity that may reduce viral load in breast milk.     Effects of protein hydrolysate in weaning diets for Atlantic cod (Gadus morhua L.) and Atlantic halibut (Hippoglossus hippoglossus L.)

    Abstract The study aims to test whether predigested dietary protein enhances the utilization of formulated diets at weaning, and also whether it stimulates intestinal maturation. In this study, Atlantic cod [Gadus morhua L.; 41 days posthatch (dph)] and Atlantic halibut [Hippoglossus hippoglossus L.; 63 days postfirst feeding (dpff)] were weaned onto diets with graded levels of protein hydrolysate. By increasing the inclusion of dietary protein as hydrolysate from 0 to 400 g kg,1, cod increased the rate of survival from 7 ± 1% to 18 ± 2% (82 dph; regression, P = 4*10,7). In halibut, the survival rate decreased from 57 ± 9% to 22 ± 7% as the inclusion of protein in the form of hydrolysate increased from 0 to 450 g kg,1 (119 dpff; regression, P = 8*10,5). Growth was not affected in any of the species. Results in specific activities of the intestinal enzymes leucine aminopeptidase (LAP) and alkaline phospatase (AP) supported the results in survival in halibut and partly also in cod, by showing increased activities in groups with increased survival (anova, P < 0.05). The lower optimal level of hydrolysed protein in halibut than in cod is suggested mainly because of a slower feeding practice in halibut, which allows more extensive nutrient leaching before ingestion.     Minimal enteral feeding reduces the risk of sepsis in feed-intolerant very low birth weight newborns

    Abstract Aims: To evaluate the efficacy and safety of minimal enteral feeding (MEF) nutritional practice in feed-intolerant very low birth weight (VLBW) infants. Methods: A retrospective design using data reported in the clinical charts of VLBW newborns consecutively observed in neonatal intensive care units (NICU) that presents feed intolerance. During the study period, two feeding strategies were adopted: total parenteral nutrition (PN) (group 1) or PN plus MEF (group 2), for at least 24 h. Primary outcome was the time to reach full enteral feeding; secondary outcomes were the occurrence of sepsis, the time to regain birth weight, the length of hospitalization, the occurrence of necrotizing enterocolitis (NEC) Bell stage >II and death. Results: In total, 102 newborns were evaluated: 51 in group 1, and 51 in group 2. Neonates in group 2 achieved full enteral nutrition earlier (8 days, interquartile range [IQR] 5) compared with subjects receiving total PN (11 days, IQR 5, p < 0.001). A reduction of sepsis episodes was observed in group 2 (15.7%) compared with group 1 (33.3%, p = 0.038). Additionally, subjects in group 2 regained their birth weight and were discharged earlier. The occurrence of NEC and death were similar in the two groups. Conclusion: Minimal enteral feeding in very low birth weight infants presenting feed intolerance reduces the time to reach full enteral feeding and the risk of sepsis. This feeding practice does not increase the risk of necrotizing enterocolitis and death. [source]


    Prediabetes and the big baby,

    Abstract The concept of prediabetes has come to the fore again with the worldwide epidemic of Type 2 diabetes. The careful observations of W. P. U. Jackson and his colleagues in Cape Town, South Africa 50 years ago still deserve attention. Maternal hyperglycaemia cannot be the only cause of fetal macrosomia, and the pathophysiological reason for the unexplained stillbirth in late diabetic pregnancy still eludes us. The biochemical concepts of ,facilitated anabolism' and ,accelerated starvation' were developed by Freinkel as explanations of the protective mechanisms for the baby during the stresses of pregnancy. Some of these nutritional stresses may also occur in the particular form of early childhood malnutrition known in Africa as kwashiorkor, where subcutaneous fat deposition, carbohydrate intolerance, islet hyperplasia and sudden death may follow a period of excess carbohydrate and deficient protein intake. Different feeding practices in different parts of the world make comparisons uncertain, but there is evidence for insulin resistance in both the macrosomic fetus of the hyperglycaemic mother and in the child with established kwashiorkor. These adaptive changes in early development may play both a physiological and a pathological role. Worldwide studies of hyperglycaemia in pregnancy are gradually establishing acceptable diagnostic criteria, appropriate screening procedures and an evidence base for treatment. Nevertheless the challenge of prediabetes and the big baby is still with us,in Jackson's words,,diabetes mellitus is a fascinating condition,the more we know about it the less we understand it'.     Bed-sharing practices of initially breastfed infants in the first 6 months of life

    Abstract This paper explores the manner in which bed-sharing is practised by breastfeeding infants in the UK, and examines how alternate definitions and interpretations of breastfeeding and bed-sharing can lead to confusion in understanding what bed-sharing entails. Longitudinal studies on parent,infant bed-sharing practices are scarce, but are vital to our understanding of normative bed-sharing behaviour. We present data from a longitudinal study of sleeping and feeding practices in England involving 97 initially breastfed infants from birth to 6 months of age whose behaviour was monitored weekly for a 6-month period. Results demonstrate that bed-sharing practices covary with breastfeeding practices, and that a single model of bed-sharing behaviour does not adequately reflect the experience of all infants. Our findings have ramifications for the way in which case,control studies attempt to ,measure' bed-sharing, and our understanding and interpretation of bed-sharing risk factors.     Young child feeding practices and child nutritional status in rural Ghana

    Abstract A study was conducted in the Manya Krobo district of Ghana with the objective of studying young child feeding practices and child nutrition situation in the area. The study was a cross-sectional survey involving 400 mothers with young children between 0 and 18 months. A combination of methods, including structured interviews using questionnaire, dietary assessment and anthropometry, was used to collect data for the study. The data obtained were analysed using spss version 10 in Windows. Means and standard deviations were generated for continuous variables and frequency distribution for categorical variables. The results revealed that although breastfeeding rates were high (97%), complementary feeding practices were less than ideal with as many as 14% of the children being introduced to complementary foods below the age of 3 months. The nutritional quality of complementary foods were poor and the prevalence of stunting among the children was high (20%). For adequate complementary feeding and improved child nutrition in this population, nutrition education intervention programmes aimed at improving nutrient intake among young children, through improved diet diversity and increased use of local foods rich in iron and other nutrients, need to be undertaken.     Food consumption patterns, dietary quality and health status of expectant mothers: case studies in suburban and rural communities in Ghana

    Abstract The study was conducted to determine the food consumption patterns, dietary quality and health status of expectant women and also to assess their level of awareness of nutritional requirements during pregnancy. Food beliefs, taboos and superstitions held by the women were also investigated. Using the purposive sampling technique, 30 pregnant women attending the Maternal and Child Health Clinic at Dodowa (rural) and University Hospital, Legon (suburban), were selected for the study. A structured questionnaire was used to collect information on the respondents' nutritional knowledge, beliefs, taboos and superstitions and health status. A food frequency questionnaire and the 24-h dietary recall method were used to obtain information on food consumption patterns and dietary quality. The study revealed that, although the majority of the respondents (83.3%) had some knowledge of nutrition, not all were applying it in their feeding practices because of financial constraints. With regard to consumption patterns, most of the women (83.3%) were having three meals a day, while the rest either ate twice a day or anytime they were hungry. Seventy-three per cent of the respondents also increased their food intake during pregnancy. Foods eaten were based mainly on starchy roots and tubers, cereals and vegetables. Legumes, oilseeds and fruits were often lacking in the main meals of respondents. Although animal products were consumed daily, the quantities taken were very small to provide for adequate protein, especially in the rural area. Various types of food were avoided by some of the expectant mothers for reasons such as nausea, loss of appetite, taboos and superstition. With regard to dietary quality, respondents from the rural area had diets of poorer quality in terms of nutrient intake compared with those from the suburban area. In both communities, iron contents of diets were quite low. Forty-seven per cent of the women interviewed were anaemic, with a higher prevalence of anaemia being observed in the rural community. Common ailments reported by the women were dizziness, headache, waist pains and malaria. Based on the results of the study, it was recommended that nutrition education for pregnant women should be stepped up at antenatal clinics especially in rural areas. [source]

    The PET DINER study was a telephone survey conducted to better understand why pet owners make certain nutritional decisions for their pets. Both dog and cat owners were included, which allowed us to differentiate the way people feed pet cats vs. pet dogs and how that might reflect owners' attitudes about pet foods. Prospective survey respondents were selected from local telephone books using a randomized process. A total of 18,194 calls were made from the five study sites between May and August, 2004. 1074 people (6%) representing 619 dogs and 455 cats completed the survey. Detailed information about feeding practices was collected. Pet owners' perceptions were assessed based on agreement/disagreement with statements regarding pet foods and the pet food industry. The Mann,Whitney Rank Sum test was used to detect differences between dog and cat owners and the Kruskal,Wallis one-way analysis of variance on ranks was used to evaluate differences among pet owners based on both type of pet and feeding practices (,75% commercial vs. ,50% home-prepared diets). More cats than dogs ate commercial pet food as ,75% of their main diet (96% vs. 87%, respectively; p < 0.001), and more dogs than cats ate a home-prepared food as ,50% of their main diet (6% vs. 3%, respectively; p < 0.009). For five of the statements, a difference in the attitudes of cat vs. dog owners was detected (p < 0.05). In general, this reflected more positive attitudes regarding commercial foods and less positive attitudes regarding home-prepared foods on the part of cat vs. dog owners. Analysis based on both type of pet and feeding practices identified significant differences among groups for 18/26 statements. However, when the Dunn's Method for pairwise multiple comparisons was applied, significant differences were due to feeding practices rather than type of pet for 17/18 statements. Only differences in attitudes about the statement ,raw bones can be safely fed to pets' remained significant for cat vs. dog owners feeding commercial foods. In conclusion, more cat owners than dog owners feed ,75% commercial foods to their pets and this is reflected in different attitudes about the nutritional soundness of commercial pet foods.     Managing infant feeding practices: the competing needs of bulimic mothers and their children

    Aim., This paper seeks to explain how bulimic mothers accommodated infant feeding demands in conjunction with managing their disordered eating practices. Background., Eating disorders are chronic and disabling illnesses primarily affecting women. There are few qualitative studies describing bulimia in the context of motherhood. Design., The study employed an inductive qualitative approach. Methods., A purposive sample of childbearing women (n = 16), who self-defined as living with an eating disorder, were recruited. Data were generated from one-to-one interviews; a thematic analysis identified key issues. Results., Participants were primarily responsible for ensuring child/ren's socialisation processes, including modelling appropriate dietary behaviours and these demands often conflicted with their personal needs for food restraint. Pressures to participate in social activities with children were widely experienced as stressful especially when these events focused on food. Participants viewed early and repeated exposure to ,healthy' eating as protective against their children acquiring an eating disorder and in this respect commercial child-care facilities provided alternative environments for children to explore food-related activities. Conclusions., Participants employed a variety of strategies to ensure children's exposure to normalising influences and socialising processes. Concerns about personal competencies with respect to food preparation and storage were articulated by all participants. Relevance to clinical practice., Professionals involved with providing care to mothers and their infants are well placed to support bulimic clients and to foster confidence in their mothering skills. Early and appropriate intervention is key to effecting positive changes in bulimic patterns, with potential benefits to women's future health and well-being and that of their children. [source]


    Nurses' knowledge of current guidelines for infant feeding and weaning

    Abstract Background Poor infant feeding practices are common causes of nonorganic failure to thrive and may exacerbate the effects of many chronic conditions. It is important therefore that parents receive correct and consistent feeding advice from health professionals. The aim of this study was to determine whether hospital paediatric nursing staff are familiar with the recommendations of the Department of Health (DOH) Weaning Report (DOH, 1994). Methods A total of 127 members of nursing staff (79%) at Derbyshire Children's Hospital were interviewed. Knowledge of recommended practices for the introduction and use of cow's milk, gluten, milk products, mashed foods, vitamin supplements and drinks was assessed. Results Overall, 42 people (33%) answered all questions correctly. In each clinical area, six outpatient (64%), 19 special care baby unit (61%) and 17 ward (20%) staff provided correct answers to all questions. No significant difference was found between staff at each grade in the number of questions answered correctly. Conclusions Knowledge of national infant feeding and weaning guidelines was limited suggesting that DOH recommendations are not widely understood or recognized. Further nutrition education and local dissemination of information is required if nurses are to continue to advise parents on aspects of infant feeding. [source]


    Infant feeding practices of Pakistani mothers in England and Pakistan

    Abstract Objectives To investigate infant feeding practices followed by Pakistani mothers in Pakistan and in England. To establish if practices conform to current guidelines and to investigate reasons for adherence and nonadherence. Methods Ninety mothers of weaning age children were interviewed; 45 were in England and 45 in Pakistan. A questionnaire available in English and Urdu sought to find out about the methods of milk feeding and weaning used and the advice received, together with general beliefs about weaning. Results Characteristics of the infants in terms of current age, gender distribution, birth order of baby and age of weaning showed no significant differences between the two groups. Thus, differences between the two groups could be attributed to cultural differences rather than any of these factors. Chi-square analysis showed that the initial method of feeding chosen was significantly different (P < 0.001, d.f. = 2) with 73% of mothers in Pakistan breast-feeding compared with 24% in England. Similar proportions of mothers in both groups commenced weaning between 3 and 4 months. Common weaning foods included rice, cereals and eggs with progression to fruit and vegetables and family food in Pakistan, and fruit, vegetables, meat and convenience foods (especially sweet options) in England. Both groups of mothers wanted more information about infant feeding practices. Conclusion Mothers in Pakistan demonstrated more confidence in weaning practices than in England because of experiences with other siblings and advice from relatives. More advice from health professionals was requested and is needed by all mothers in order to improve weaning practices of the infants. [source]


    Audit of feeding practices in babies <1200 g or 30 weeks gestation during the first month of life

    Aim: In 2002, the composition of the breast milk fortifier used in our hospital changed, giving increased protein and energy. We therefore decided to prospectively audit nutritional management in our unit and to compare nutritional intake and growth in our babies with published data. Methods: Data were prospectively collected over a 3-month period on infants <1200 g or 30 weeks gestation. Prescribed and delivered volumes of all parenteral and enteral fluids were recorded. Babies were weighed as per unit protocol. Results: Thirty-four infants met the audit criteria. Data are median (range). After the first week of life, energy and protein intakes were 147 (78,174) kcal/kg/day and 3.9 (2.1,4.8) g/kg/day respectively. Daily weight gain was 17 (,3.2,35.4) g/kg and was significantly associated with both energy and protein intakes (P < 0.001). However, standard deviation scores for weight fell from 0.15 (,1.9,2.0) at birth to ,1.0 (,2.9,0.8) by 36 weeks corrected age. Time to commencing enteral feeds was 1 (1,3) day and to full enteral feeds was 8 (5,28) days. One infant was diagnosed with necrotising enterocolitis and eight with chronic lung disease. Mean protein intake was significantly lower in babies with chronic lung disease (P = 0.005). Conclusion: Overall, nutritional intakes and weight gain in this cohort of babies lie within the recommended ranges, although protein intakes in the smallest babies are at the lower end of the range. Enteral feeds are introduced early and advanced rapidly, but we have a low incidence of necrotising enterocolitis. However, babies still fell across weight centiles, suggesting that actual intakes for these tiny babies may be inadequate. [source]


    Breastfeeding failure in a longitudinal post-partum maternal nutrition study in Hong Kong

    Objective: To describe factors associated with breastfeeding failure during the first 6 months post-partum in a sample of Hong Kong Chinese women participating in a longitudinal study of maternal nutrition. Methodology: Forty-four Hong Kong Chinese lactating mothers who intended to breastfeed exclusively for at least 3 months were recruited and followed for 6 months post-partum. Demographic data were compared with 20 mothers who intended to use formula feeding. Mothers were followed up at 2 and 6 weeks and 3 and 6 months and details of infant feeding practices were obtained. Information was sought on breastfeeding management in hospital, reasons for discontinuation of breastfeeding or for providing supplements to babies and intention to seek, and sources of, lactation support. Results: Thirty-nine mothers who planned to breastfeed completed the follow up. Compared with mothers in the formula-feeding group, breastfeeding mothers were more likely to be professionals or housewives. Continuation of any breastfeeding (total and partial) was noted in 30 (77%), 22 (57%), 16 (41%) and 12 (31%) mothers at 2 and 6 weeks and 3 and 6 months post-partum, respectively. The majority (97%) of mothers stated that they were given information on the benefits and management of breastfeeding. However, late initiation of breastfeeding and providing supplements to babies were common. Perceptions of insufficient milk supply (44%), breast problems (31%) and being too tired (28%) were the main reasons stated for stopping breastfeeding or for providing supplements to babies. Midwives from the postnatal wards and hotlines were the main sources of lactation support. Conclusions: These results highlight difficulties in sustaining breastfeeding, either exclusive or partial, in Hong Kong Chinese women. Despite being recruited on the basis of intending to exclusively breastfeed for 3 months, less than half these mothers were still breastfeeding and only approximately one-third were exclusively or predominantly breastfeeding at 3 months. More needs to be done within the hospital environment to initiate breastfeeding immediately after birth and to avoid giving unnecessary supplements and more effort is needed to foster a mother's confidence, commitment and knowledge of breastfeeding.     ABSTRACT Background : Although the predominant beneficial effect of fluoride occurs locally in the mouth, the adverse effect, dental fluorosis, occurs by the systemic route. The caries attack rate in industrialized countries, including the United States and Canada, has decreased dramatically over the past 40 years. However, the prevalence of dental fluorosis in the United States has increased during the last 30 years both in communities with fluoridated water and in communities with nonfluoridated water. Dental fluorosis is closely associated with fluoride intake during the period of tooth development. Methods: We reviewed the major changes in infant feeding practices that have occurred since 1930 and the changes in fluoride intakes by infants and young children associated with changes in feeding practices. Results and Conclusions: Based on this review, we conclude that fluoride intakes of infants and children have shown a rather steady increase since 1930, are likely to continue to increase, and will be associated with further increase in the prevalence of enamel fluorosis unless intervention measures are instituted. Recommendations: We believe the most important measures that should be undertaken are (1) use, when feasible, of water low in fluoride for dilution of infant formulas; (2) adult supervision of toothbrushing by children younger than 5 years of age; and (3) changes in recommendations for administration of fluoride supplements so that such supplements are not given to infants and more stringent criteria are applied for administration to children.     Supplementary winter feeding of wild red deer Cervus elaphus in Europe and North America: justifications, feeding practice and effectiveness

    ABSTRACT 1.,Supplementary winter feeding of game animals, and particularly deer, is a common practice throughout northern (continental) Europe and parts of North America. Feeding is normally associated with maintaining high densities of animals for hunting, in terms of: (i) maintaining or increasing body weights and condition overwinter; (ii) improving reproductive performance and fertility; (iii) increasing overwinter survival; and (iv) reducing levels of damage caused to agriculture and forestry or the natural heritage. We consider the balance of evidence on the effectiveness of winter feeding of red deer Cervus elaphus in achieving these objectives. Where that evidence is equivocal, we attempt to reconcile apparent contradictions to evaluate the circumstances under which winter feeding may or may not be effective. 2.,In general, feeding of red deer on open range appears to have relatively little effect on body weights or fecundity. Effects on increasing antler size and quality are variable and seem to depend on the degree to which animals may be mineral limited on native range. Effects on survival are similarly ambiguous. It is apparent, however, that to be effective in reducing mortality, any supplementation is required early in the season and not simply when heavy mortalities are already being experienced. If provision of supplementary foods is delayed until animals are perceived already to be in poor condition, such feeding may have little effect. 3.,One of the primary goals of winter feeding in both Europe and the USA has become the prevention of environmental damage, particularly damage to commercial and native forests, while maintaining deer populations at densities suitable for hunting. Again, empirical evidence for effectiveness in this regard is inconclusive, with some studies showing a decrease in damage caused, some showing no effect and others showing a significant increase in local impact. 4.,There are equally a number of problems associated with the provision of supplementary feeds overwinter. Those animals which come to the feeding stations may develop a reliance on the food supplement provided, reducing intake of natural forages to near zero; where feed provided is less than 100% of daily requirement, such animals may regularly lose, rather than gain condition. Feed provision is also extremely uneven at such feeding stations; dominant stags displace younger stags and hinds from the feed provided until they have themselves finished feeding. Concentrations of high densities of animals around small feed-areas may also increase the risk of infection and lead to development of high parasite burdens. 5.,In an attempt to assess the current status and distribution of supplementary winter feeding in Scotland, a questionnaire was circulated to a number of individual across the country. Results of this survey are summarized and conclusions presented on the likely effectiveness of current feeding practices in achieving their aims. [source]


    Sociocultural influences on infant feeding decisions among HIV-infected women in rural Kwa-Zulu Natal, South Africa

    Abstract The promotion of exclusive breastfeeding for 6 months, followed by rapid transition to alternative food sources may be an important public health approach to the reduction of mother-to-child transmission of HIV through breastmilk. The basic ethical principle of ,informed choice' requires that HIV positive women are provided with adequate information about their options. However, information is only one factor that affects their decisions. The objective of this ethnographic study was to identify sociocultural influences on infant feeding decisions in the context of a large cohort study designed to assess the impact of a breastfeeding counselling and support strategy to promote exclusive breastfeeding on postnatal transmission of HIV in African women. Following an initial period of exploratory interviewing, ethnographic techniques were used to interview 22 HIV positive women about their views on infant feeding and health. Interviews were tape-recorded, transcribed and analysed with a text analysis program. Five themes of influences on feeding decisions emerged: (1) social stigma of HIV infection; (2) maternal age and family influences on feeding practices; (3) economic circumstances; (4) beliefs about HIV transmission through breastmilk; and (5) beliefs about the quality of breastmilk compared to formula. The study highlights the role of cultural, social, economic and psychological factors that affect HIV positive women's infant feeding decisions and behaviour. [source]


    Review of ICU nutrition support practices: implementing the nurse-led enteral feeding algorithm

    Abstract Many intensive care units (ICUs) have standard feeding protocols which promote safe early initiation of enteral feeding. The use of these protocols has been shown to increase the incidence of enteral feeding and achieve greater adequacy of nutrition support. A multidisciplinary working party developed and implemented a nurse-led enteral feeding algorithm which enabled senior nursing staff to set safe and nutritionally adequate target feed volumes based upon patient body weight. The algorithm incorporated best practice-based referral criteria so that patients at nutritional risk were referred for tailored dietetic assessment. The aims were to determine compliance with the ICU nurse-led enteral feeding algorithm and to ascertain its safety and efficacy. A 3-month prospective audit was conducted by specialist ICU dietitians. Data were obtained from electronic patient records and through observing feeding practices. Data collected included prescribed feed type and infusion rate versus volume received, frequency of gastric aspiration and prokinetic usage. In all, 90% (n = 43) of referrals received by the dietitian met the referral criteria. Absolute compliance with patients receiving correct type and volumes of feed, with a correct feed prescription and an accurate documented weight was just 2% (n = 1). Despite this finding, 60% of patients were actually receiving the correct feed regimen. If the nurse-led enteral feeding algorithm is wholly adhered to, the ICU dietitian need not formally assess every ICU patient. Nursing staff require further support in assessing patient body weight alongside an ongoing intensive educational programme for the multidisciplinary team and regular reaudit. [source]


    The effect of current grain feeding practices on hindgut starch fermentation and acidosis in the Australian racing Thoroughbred

    A survey of 72 thoroughbred trainers in several regions of NSW was conducted to better define current grain feeding practices, and the risks involved with grain feeding in the Australian thoroughbred racing industry. Results from the survey indicate that horses in the industry are currently being fed an average of 7.3 ± 0.23 kg of grain concentrate per day, with oats, commercial pre-mixed diets and corn most commonly used. The feeding of extruded, micronised and steam flaked grains was uncommon. The results of grain and faecal sample analyses conducted during the survey indicate that incomplete starch digestion in the equine small intestine and subsequent hindgut starch fermentation and low hindgut pH are common. These observations quantify the adverse impact of current grain feeding practices on faecal measures of gut health and demonstrate that when selecting grains for use in the thoroughbred racehorse, greater emphasis should be placed on the starch digestibility characteristics of grains.     Protein feeds coproduction in biomass conversion to fuels and chemicals

    Abstract Agriculture has changed greatly in the past in response to changing human needs. Now agriculture is being called on to provide raw materials for very large-scale fuel and chemical production. Agriculture will change again in response to this demand and all producers and users of agricultural feedstocks will be affected by this change. For example, livestock feeding practices have already changed in response to the availability of distillers' grains from corn ethanol production. A fuels industry based on herbaceous biomass energy crops will be many-fold larger than the existing corn ethanol industry and will produce its own set of impacts on livestock feeding. We explore here one of these impacts: the availability of large new sources of feed protein from biomass energy crops. In addition to structural carbohydrates, such as cellulose and hemicellulose, herbaceous biomass energy crops can easily be produced with approximately 10% protein, called ,leaf protein'. This leaf protein, as exemplified by alfalfa leaf protein, is superior to soybean meal (SBM) protein in its biological value. Leaf protein recovery and processing fit well into many process flow diagrams for biomass fuels. When leaf protein is properly processed to concentrate it and remove antinutritional factors, as we have learned over the years to do with soybean meal protein, protein in leaf protein concentrate (LPC) will probably be at least as valuable in livestock diets as SBM protein. If LPC is used to meet 20% of total animal protein requirements (i.e., market penetration of 20%) then the potential utilization of leaf protein concentrate could reach as much as 24 million metric tons annually. This leaf protein will replace protein from SBM and other sources. This much leaf protein will reduce by approximately 16 million hectares the amount of land required to provide protein for livestock. Likewise the amount of land required to meet fuel needs will effectively be reduced by 8 million hectares because this land will effectively do ,double duty' by producing needed animal protein as well as feedstocks for fuel production. © 2009 Society of Chemical Industry and John Wiley & Sons, Ltd [source]


    Factors associated with successful establishment of breastfeeding in very preterm infants

    Abstract Aim:, To describe feeding practices at hospital discharge in relation to characteristics of the very preterm infants (VPI) and their mothers. Methods:,Design. Prospective hospital-based registration of very preterm infants born with a gestational age ,32 weeks in Denmark during 2004,2008. Subjects. Healthy mothers and VPI without diseases causing eating disabilities at discharge. Results:, A total of 478 VPI were registered. At discharge, 60% were exclusively breastfed, 35% were exclusively bottlefed, and 5% were both breast- and bottle-fed. Mothers of high social class (p = 0.000) and ,not smoking' (p = 0.003) were significantly more often breastfeeding their preterm infant(s) at discharge. Single births infants tended more often to be breastfed (p = 0.09). Infant age at discharge and duration of hospitalization did not influence breastfeeding at discharge. Increase in weight z-score from birth to discharge was largest in the bottlefeeding-group compared with the breastfeeding-group (p = 0.000) probably as a result of feeding practice the last week(s) of hospitalization. Conclusion:, Breastfeeding can successfully be established in very preterm infants. Mothers of low social classes, smokers, multiple birth and very preterm infants with low weight for age may need extra attention in breastfeeding establishing policies. [source]


    Cholinergic and oxidative stress mechanisms in sudden infant death syndrome

    Abstract Aim:, To determine whether biochemical parameters of cholinergic and oxidative stress function including red cell acetylcholinesterase (AChE), serum/plasma thyroglobulin, selenium, iron, ferritin, vitamins C, E, and A affect risk in apparent life-threatening event (ALTE), sudden infant death syndrome (SIDS), and sudden unexpected death in infancy (SUDI). To assess these biochemical parameters as a function of age; and for influence of pharmacology and epidemiology, including infant health, care, and feeding practices. Methods:, A multicentre, case,control study with blood samples from 34 ALTE and 67 non-ALTE (control) infants matched for age, and 30 SIDS/SUDI and four non-SIDS/non-SUDI (post-mortem control) infants. Results:, Levels/activity of the biochemical parameters were not significantly different in ALTE vs. control infants, with the exception of higher vitamin C levels in the ALTE group (p = 0.009). In ALTE and control groups, AChE and thyroglobulin levels increased and decreased respectively from birth to attain normal adult levels from 6 months. Levels of iron and ferritin were higher in the first 6 month period for all infant groups studied, intersecting with vitamin C levels peaking around 4 months of age. Conclusion:, Lower AChE levels and higher combined levels of iron and vitamin C in the first 6 months of life may augment cholinergic and oxidative stress effect, particularly at the age when SIDS is most prevalent. This may contribute to risk of ALTE and SIDS/SUDI events during infancy. [source]


    Feeding practices of HIV-1-infected mothers: The role of counsellors

    Abstract In this issue of Acta Paediatrica, Chopra et al. report that voluntary counselling is central to preparing mothers for making a proper informed choice about adequate feeding practices to prevent their infants from acquiring HIV infection. The recommendations given and the way in which counselling is performed are the most important determinants of a mother's decision about how to feed her infant. In this article, we summarize the main arguments for and against breastfeeding by HIV-infected mothers. Conclusions : Further studies are needed to determine the alternatives to breastfeeding in countries where there is no access to safe formula feeding or to antiretroviral drugs. HIV-positive mothers should be made aware of the available feeding alternatives through adequate counselling from properly trained persons. [source]
