Doing harm reduction better: syringe exchange in the United States

ABSTRACT Objective To trace the growth of syringe exchange programs (SEPs) in the United States since 1994,95 and assess the current state of SEPs. Methods Annual surveys of US SEPs known to North American Syringe Exchange Network (NASEN). Surveys mailed to executive directors with follow-up interviews by telephone and/or e-mail. Response rates have varied between 70% and 88% since surveys were initiated in 1996. Results The numbers of programs known to NASEN have increased from 68 in 1994,95 to 186 in 2007. Among programs participating in the survey, numbers of syringes exchanged have increased from 8.0 million per year to 29.5 million per year, total annual budgets have increased from $6.3 to $19.6 million and public funding (from state and local governments) has increased from $3.9 to $14.4 million. In 2007, 89% of programs permitted secondary exchange and 76% encouraged it. Condoms, referrals to substance abuse treatment, human immunodeficiency virus (HIV), hepatitis C virus (HCV), hepatitis B virus (HBV) counseling and testing and naloxone for overdose were among the most commonly provided services in addition to basic syringe exchange. Each of these services was provided by 40% or more of SEPs in 2007. Conclusions While syringe exchange has remained controversial in the United States, there has been very substantial growth in numbers of programs, syringes exchange and program budgets. Utilizing secondary exchange to reach large numbers of injecting drug users and utilizing SEPs as a new platform for providing health and social services beyond basic syringe exchange have been the two major organizational strategies in the growth of SEPs in the United States. Thick Prescriptions: Toward an Interpretation of Pharmaceutical Sales Practices

Anthropologists of medicine and science are increasingly studying all aspects of pharmaceutical industry practices,from research and development to the marketing of prescription drugs. This article ethnographically explores one particular stage in the life cycle of pharmaceuticals: sales and marketing. Drawing on a range of sources,investigative journalism, medical ethics, and autoethnography,the author examines the day-to-day activities of pharmaceutical salespersons, or drug reps, during the 1990s. He describes in detail the pharmaceutical gift cycle, a three-way exchange network between doctors, salespersons, and patients and how this process of exchange is currently in a state of involution. This gift economy exists to generate prescriptions (scripts) and can mask and/or perpetuate risks and side effects for patients. With implications of pharmaceutical industry practices impacting everything from the personal-psychological to the global political economy, medical anthropologists can play a lead role in the emerging scholarly discourse concerned with critical pharmaceutical studies. Impairment of Hepatic Microcirculation in Fatty Liver

ABSTRACT Fatty liver or hepatic steatosis, which is the result of the abnormal accumulation of triacylglycerol within the cytoplasm of hepatocytes, is a common histological finding in human liver biopsy specimens that is attributed to the effects of alcohol excess, obesity, diabetes, or drugs. There is a general consensus that fatty liver compromises hepatic microcirculation, the common exchange network upon which hepatic arterial and portal inflows converge, regardless of underlying etiology. A significant reduction in hepatic microcirculation has been observed in human fatty donor livers and in experimental models of hepatic steatosis. There is an inverse correlation between the degree of fat infiltration and both total hepatic blood flow and flow in microcirculation. Fatty accumulation in the cytoplasm of the hepatocytes is associated with an increase in the cell volume that reduces the size of the hepatic sinusoid space by 50% compared with a normal liver and may result in partial or complete obstruction of the hepatic sinusoid space. As a result of impaired hepatic microcirculation, the hepatocytes of the fatty liver have reduced tolerance against ischemia-reperfusion injury, which affects about 25% of the donors for liver transplantation because severe steatosis is associated with a high risk of primary nonfunction after liver transplantation. Paleogenetical study of pre-Columbian samples from Pampa Grande (Salta, Argentina)

Abstract Ancient DNA recovered from 21 individuals excavated from burial sites in the Pampa Grande (PG) region (Salta province) of North-Western Argentina (NWA) was analyzed using various genetic markers (mitochondrial DNA, autosomal STRs, and Y chromosomal STRs). The results were compared to ancient and modern DNA from various populations in the Andean and North Argentinean regions, with the aim of establishing their relationships with PG. The mitochondrial haplogroup frequencies described (11% A, 47% B, and 42% D) presented values comparable to those found for the ancient Andean populations from Peru and San Pedro de Atacama. On the other hand, mitochondrial and Y chromosomal haplotypes were specific to PG, as they did not match any other of the South American populations studied. The described genetic diversity indicates homogeneity in the genetic structure of the ancient Andean populations, which was probably facilitated by the intense exchange network in the Andean zone, in particular among Tiwanaku, San Pedro de Atacama, and NWA. The discovery of haplotypes unique to PG could be due to a loss of genetic diversity caused by recent events affecting the autochthonous populations (establishment of the Inca Empire in the region, colonization by the Europeans). ECONOMIC RESTRUCTURING AND URBAN FOOD ACCESS IN THE DOMINICAN REPUBLIC

The article describes how economic restructuring in the Dominican Republic during the 1980s and 1990s established the basis for urban food access challenges during the 2000s. Primarily based on research in Santiago, the second largest Dominican city, the article provides insights into how export-oriented development strategies, expanding trade liberalization, domestic political struggles, and patriarchal relations influenced access to food for low-income residents. During the early 2000s, many Santiago residents were engaged in an elaborate, androcentric exchange network that linked gendered income-generating strategies to credit-bearing food merchants who were, in turn, conjoined to a sequence of brokers all of whom were eventually linked to domestic and international producers by credit relations. Analysis of these findings illustrates how and why this exchange network existed, the importance of credit relations to its maintenance, and the ways in which government and U.S. food policies influenced urban provisioning patterns among the most economically and socially vulnerable population of Santiago. I argue that the rapidly changing social and spatial configurations of Latin American and Caribbean cities calls for innovative applied anthropological research into the processes that structure access to food resources by food insecure groups. By focusing on household food procurement in conjunction with exchange relations for a key staple, the article highlights practices and policies that enable and constrain food access for such groups. The article provides empirical data relevant to scholars and practitioners concerned with understanding the structural origins of the present-day food crisis in developing countries. [source]


An overview of bitumen trade in the Near East from the Neolithic (c.8000 BC) to the early Islamic period

The aim of the paper is to summarise the present state of knowledge concerning bitumen trade in the Near East from the Palaeolithic (70,000 BP) to the Early Islamic period. During the Palaeolithic and Early Neolithic period, bitumen utilisation was mostly concentrated in settlements close to oil seeps. From the Ubaid 3 period, bitumen from the Mosul area became more important and was traded as far as the southern Persian Gulf. The Uruk period is a turning point for Mesopotamian history as settlements evolved into city-states. These cities had a great need for raw materials, and this marks the beginning of large-scale exploitation of Hit bitumen. This bitumen was traded at settlements along the Euphrates, where a large trade network was established. Hit bitumen entered the Persian Gulf at the turn of the second millennium (Dilmun period). Bitumen from Iraq (Mosul and Hit) became predominantly used in most settlements along the southern coast of the Gulf. During this period Iranian bitumen was also exported and this supply tended to increase, especially during the Partho-Sasanian period. Dead Sea bitumen had its own exchange network, which was concentrated across present-day Israel and Egypt where it was extensively used for mummification. [source]


Preservation of Muscular and Elastic Artery Distensibility After an Intercontinental Cryoconserved Exchange: Theoretical Advances in Arterial Homograft Generation and Utilization

Abstract While the situation of tissue donation and transplantation differs between Latin American and European countries, a common problem is tissue deficiency. Hence, at present, there is a pressing need to generate alternatives so as to increase the possibilities of obtaining the requested materials. Consequently, it would be of significant interest to establish an intercontinental network for tissue exchange, to improve international cooperation, and to help patients that need tissue transplantation, and to evaluate the feasibility of using an intercontinental network for the exchange of cryopreserved arteries (cryografts), preserving the arterial distensibility and ensuring a reduced native artery,cryograft biomechanical mismatch. Distensibility was studied in ovine arteries divided into three groups: intact (in vivo tests, conscious animals), fresh control (in vitro tests immediately after the artery excision, Uruguay), and cryografts (in vitro tests of cryopreserved-transported-defrosted arteries, Spain). Histological studies were performed so as to analyze changes in the endothelial layer and elastic components. The comparison between fresh control and cryografts showed that neither the cryopreservation nor the exchange network impaired the distensibility, despite the expected histological changes found in the cryografts. The comparison between intact and cryografts showed that the cryografts would be capable of ensuring a reduced biomechanical mismatch. The cryopreservation and the intercontinental network designed for artery exchange preserved the arterial distensibility. It could be possible to transfer cryografts between Latin America and Europe to be used in cardiovascular surgeries and/or for tissue banking reprocessing, with basic biomechanical properties similar to those of the fresh and/or native arteries. Tracking neolithic interactions in southeast China: Evidence from stone adze geochemistry

The authors conducted a sourcing study of stone adzes recently excavated from the Damaoshan site on Dongshan Island, Fujian province, China. They used XRF, ICP-MS, and petrographic thin-section techniques for geochemical and petrographic analysis of six stone adzes from the site and three geological samples from Dongshan Island. The chemical data of these samples compared with other lavas in Fujian, Zhejiang, Penghu, and Taiwan suggest that the Damaoshan people imported stone raw materials either from the Penghu Archipelago in the Taiwan Strait or from the Niutoushan area on the Fujian coast. This indicates that exchange networks along the southeast coast of China, and possibly spanning the Taiwan Strait, had developed as early as 4300,5000 yr B. P. © 2005 Wiley Periodicals, Inc. Process integration technology review: background and applications in the chemical process industry

Abstract Process integration is a holistic approach to process design and operation which emphasizes the unity of the process. Process integration design tools have been developed over the past two decades to achieve process improvement, productivity enhancement, conservation in mass and energy resources, and reductions in the operating and capital costs of chemical processes. The primary applications of these integrated tools have focused on resource conservation, pollution prevention and energy management. Specifically, the past two decades have seen the development and/or application of process integration design tools for heat exchange networks (HENs), wastewater reduction and water conservation networks, mass exchange networks (MENs), heat- and energy-induced separation networks (HISENs and EISENs), waste interception networks (WINs) and heat- and energy-induced waste minimization networks (HIWAMINs and EIWAMINs), to name a few. This paper provides an overview of some of these developments and outlines major driving forces and hurdles. The fundamental aspects of this approach along with their incorporation in an overall design methodology will be discussed. Cultural Identification and Second Language Pronunciation of Americans in Norway

Schumann's Acculturation Theory as presented in The Pidginization Process: A Model for Second Language Acquisition (1978) predicts that the degree of a learner's success in second language (L2) acquisition depends upon the learner's degree of acculturation. Attempts to test this theory have not been particularly fruitful due to the lack of an adequate measure of acculturation and the particular linguistic markers selected to measure success in L2 acquisition. This study proposes to measure sojourners' acculturation in terms of their social exchange networks (Milroy & Wei, 1995). It measures L2 success in terms of pronunciation, which in the view of many scholars (Guiora, Beit,Hallahmi, Brannon, Dull, & Scovel, 1972; Labov, 1972; Scovel, 1988) is the strongest linguistic marker of a speaker's cultural identification. Using this framework, the current study provides strong evidence in support of Schumann's Acculturation Theory. The acculturation experiences and L2 pronunciation of 9 American women residing in Norway are described and the relationship examined. It is concluded that learners who developed positive network connections with native speakers of Norwegian evidenced more native,like pronunciation than those who had greater difficulty establishing such relationships. CONTRASTING SUBSISTENCE STRATEGIES IN THE EARLY IRON AGE?

Summary. What can students of the past do to establish the predominant land-use and settlement practices of populations who leave little or no artefactual discard as a testament to their lifeways? The traditional answer, especially in Eastern Europe, is to invoke often exogenous nomadic pastoralists whose dwelling in perpetuo mobile was based on yurts, minimal local ceramic production and high curation levels of wooden and metal containers. Such a lacuna of understanding settlement structure and environmental impacts typifies Early Iron Age (henceforth ,EIA') settlements in both Bulgaria and eastern Hungary , a period when the inception of the use of iron in Central and South-East Europe has a profound effect on the flourishing regional bronze industries of the Late Bronze Age (henceforth ,LBA'). The methodological proposal in this paper is the high value of palynological research for subsistence strategies and human impacts in any area with a poor settlement record. This proposal is illustrated by two new lowland pollen diagrams , Ezero, south-east Bulgaria, and Sarló-hát, north-east Hungary , which provide new insights into this research question. In the Thracian valley, there is a disjunction between an area of high arable potential, the small size and short-lived nature of most LBA and EIA settlements and the strong human impact from the LBA and EIA periods in the Ezero diagram. In the Hungarian Plain, the pollen record suggests that, during the LBA,EIA, extensive grazing meadows were established in the alluvial plain, with the inception of woodland clearance on a massive scale from c.800 cal BC, that contradicts the apparent decline in human population in this area. An attempted explanation of these results comprises the exploration of three general positions , the indigenist thesis, the exogenous thesis and the interactionist thesis. Neither of these results fits well with the traditional view of EIA populations as incoming steppe nomadic pastoralists. Instead, this study seeks to explore the tensions between local productivity and the wider exchange networks in which they are entangled. [source]
