Skeletal Integrity and Visceral Transplantation

Despite continuous improvement in long-term survival, there is no knowledge about risk of bone health impairment and management strategies before and after intestinal transplantation. Therefore, 147 adults were retrospectively studied via chart review; 70 long-term survivors, 53 candidates and 24 recipients with longitudinal follow-up. Evaluation process included measurement of bone mineral density (BMD) and allied biochemical markers. Both long-term survivors and candidates showed low bone mass with lower (p < 0.05) z-scores at hip, femoral neck and spine. Vitamin D deficiency and secondary hyperparathyroidism were observed in both groups. Prevalence of osteoporosis was 44% among long-term survivors and 36% in candidates with age, BMD, duration of parenteral nutrition, type of immunosuppression and rejection being significant risk factors. Fragility fractures occurred at a higher (p = 0.02) rate among long-term survivors (20%) compared to candidates (6%). The longitudinal study documented acceleration (p = 0.025) of bone loss after transplantation with a decline of 13.4% (femoral neck), 12.7% (hip) and 2.1% (spine). Alendronate reduced (p < 0.05) but did not prevent bone loss. In conclusion, intestinal transplant recipients are at risk of osteoporosis secondary to bone loss before and after transplantation. Accordingly, current management includes comprehensive preventive measures with prompt therapeutic intervention utilizing intravenous bisphosphonates or subcutaneous human PTH 1,34. [source]


Principles of risk assessment for determining the safety of chemicals: Recent assessment of residual solvents in drugs and di(2-ethylhexyl) phthalate

ABSTRACT Risk assessment of chemicals is essential for the estimation of chemical safety, and animal toxicity data are typically used in the evaluation process, which consists of hazard identification, dose,response assessment, exposure assessment, and risk characterization. Hazard identification entails the collection of all available toxicity data and assessment of toxicity endpoints based on findings for repeated dose toxicity, carcinogenicity or genotoxicity and species-specificity. Once a review is compiled, the allowable lifetime exposure level of a chemical is estimated from a dose,response assessment based on several measures. For non-carcinogens and non-genotoxic carcinogens, the no-observed-adverse-effect-level (NOAEL) is divided by uncertainty factors (e.g. with environmental pollutants) or safety factors (e.g. with food additives) to derive a tolerable daily intake (TDI) or acceptable daily intake (ADI), respectively. These factors include interspecies and individual differences, duration of exposure, quality of data, and nature of toxicity such as carcinogenicity or neurotoxicity. For genotoxic carcinogens, low dose extrapolation is accomplished with mathematical modeling (e.g. linearized multistage model) from the point of departure to obtain exposure levels that will be associated with an excess lifetime cancer risk of a certain level. Data for levels of chemicals in food, water and air, are routinely used for exposure assessment. Finally, risk characterization is performed to ensure that the established ,safe' level of exposure exceeds the estimated level of actual exposure. These principles have led to the evaluation of several existing chemicals. To establish a guideline for residual solvents in medicine, the permitted daily exposure (PDE), equivalent to TDI, of N,N-dimethylformamide was derived on the basis of developmental toxicity (malformation) and of N-methylpyrrolidone on the basis of the developmental neurotoxicity. A TDI for di(2-ethylhexyl)phthalate was derived from assessment of testicular toxicity. [source]


A European methodology for sustainable development strategy reviews

Abstract In 2005 the EU Environment Directorate initiated the production of a guidebook for peer reviews of national sustainable development strategies (NSDSs), which was published in 2006. Its objective is to support EU member states planning to evaluate their respective NSDS, supporting and stimulating all potential participants. It describes how to initiate, start, lead and conclude an evaluation process, and suggests, based on European experiences, a spectrum of methods available for this purpose. During a Commission-sponsored trial period, 2006/2007, the Netherlands was the only country to make use of this offer. However, the renewed EU Sustainable Development Strategy (EUSDS) calls for regular (peer) reviews of NSDS. Using this specific review instrument is recommended as part of a mutual learning exercise, which might stimulate a self-organized convergence of NSDSs, and better vertical integration, without establishing new competences and mechanisms on the EU level. Two new elements are suggested, a simple ,pressure,policy matrix' (PPM), supporting comprehensiveness control, and the possibility of patchwork evaluations, based on the systematique of the matrix. The Accreditation Council for Graduate Medical Education has required that training programs initiate an evaluation process to assess resident acquisition of the newly promulgated general competencies (GCs). Certain GCs (e.g., systems-based practice, problem-based learning and improvement) are somewhat more challenging to define and measure than others. Systems-based practice essentially captures the interactions of the emergency medicine resident that expand beyond isolated contact with the patient. Evaluating these various interactions is readily accomplished using a detailed ordinal evaluation form that measures commonly occurring easily identified actions. Examples of measurable items and the method by which they can be integrated into an evaluation device are presented. [source]

Abstract: Evaluation of complex systems is generally complicated and time consuming. Evaluation is needed for nearly all engineering tasks and the obstacles related to evaluation increase in proportion to complexity. New techniques can be used to automate manual evaluation and to overcome the obstacles related to evaluation that cannot be solved (or can only be solved with great difficulty) with conventional computing. In this study, a methodology was developed to handle the heuristic knowledge of experts for evaluation purposes. In this method, knowledge was represented as a reference model of evaluation objectives, production rules, measures, methods and parameters. A ,common evaluation process' and ,common evaluation model', which simplify and speed up the evaluation process and decrease evaluation cost, were proposed and developed. A hybrid expert,fuzzy system, called ,intelligent evaluation system' (INES), which can be used for evaluation of complex systems was developed. To define a process and develop a system that simplifies and speeds up evaluation can save time, decrease cost and provide reusability. As the evaluation of complex systems includes uncertainty in some aspects, fuzzy logic was incorporated with an expert system for reasoning. INES was implemented successfully for the evaluation of an air defence system, which is a complex system used to protect a region from all air threats. [source]


A CRITICAL REVIEW OF CHILD CUSTODY EVALUATION REPORTS

This study examined 52 child custody reports drafted by doctoral-level psychologists from across the United States to determine (a) the nature, scope, and quality of the evaluation process as reflected in report content; (b) the degree to which practice as documented in reports is congruent with practice as described by survey data; and (c) the manner in which evaluation results are communicated to the court. In general, the findings suggest that evaluation procedures identified in reports are consistent with those described in past survey research and with custody guidelines. Evaluations tend to be court ordered, comprehensive, and well written. Ways in which reports can be improved were identified. [source]


Assessment of Fuel-Cell-Based Passenger Cars

Abstract Highly efficient energy conversion systems with fuel cells for vehicles, as well as for stationary and portable applications, are currently being discussed all over the world. Fuel cell technology is expected to help reduce primary energy demand and emissions of limited and climate-relevant pollutants. The high flexibility of fuel cell systems with respect to energy carriers opens up possibilities of modifying the energy sector in the long term. Introducing new fuels based on low-carbon, or in the long term carbon-free, energy carriers can contribute to reducing greenhouse gas emissions as well as locally and regionally active atmospheric pollutants. The use of hydrogen as feed gas for fuel cells on the basis of it being a non-fossil, renewable energy, leads to special benefits with respect to conserving resources and climate protection, but at present still represents a medium- to long-term prospect. A major milestone on the road to market success for all energy conversion systems with fuel cells is the reduction of costs. The definition of the ,appropriate" fuel represents a serious obstacle to the market introduction of fuel-cell-powered vehicles. Presenting data from a well-to-wheel analysis of various vehicle fuel systems at FZJ this article aims to discuss the potential benefits of future vehicle concepts with fuel cells in terms of primary energy use and greenhouse gas emissions. Results from a comparison of international studies on this subject will be used to identify relevant assumptions that lead to different answers in the evaluation process. [source]


Calibration accuracy of a judgmental process that predicts the commercial success of new product ideas

Abstract We examine the accuracy of forecasts of the commercial potential of new product ideas by experts at an Inventor's Assistance Program (IAP). Each idea is evaluated in terms of 37 attributes or cues, which are subjectively rated and intuitively combined by an IAP expert to arrive at a forecast of the idea's commercialization prospects. Data regarding actual commercialization outcomes for 559 new product ideas were collected to examine the accuracy of the IAP forecasts. The intensive evaluation of each idea conducted by the IAP produces forecasts that accurately rank order the ideas in terms of their probability of commercialization. The focus of the evaluation process on case-specific evidence that distinguishes one idea from another, however, and the corresponding neglect of aggregate considerations such as the base rate (BR) and predictability of commercialization for new product ideas in general, yields forecasts that are systematically miscalibrated in terms of their correspondence to the actual probability of commercialization. Entrepreneurs and venture capitalists enter into contracts that influence their behaviour and mitigate the agency costs. In particular, they select an appropriate kind and structure of financing and specify the rights as well as the duties of both parties. The typical features of venture capital investments are an intensive screening and evaluation process, active involvement of venture capitalists in their portfolio companies, staging of capital infusions, use of special financing instruments such as convertible debt or convertible preferred stock, syndication among venture capitalists or limited investment horizon. Applying a ,stages of change' model to enhance a traditional evaluation of a research transfer course

Abstract The aim of this study was to utilize an evaluation tool based on Prochaska's model of change in order to assess behaviour change as part of an evaluation process for a research transfer training programme (RTTP). The RTTP was a training programme offered to scientists in a psychiatry department and research institute to gain skills in research transfer. In addition to a traditional course evaluation framework evaluating overall satisfaction with the course and whether or not learning objectives were met, an additional ,stages of change' evaluation tool designed to assess change along a continuum was utilized. This instrument measured change in participants' attitudes, intentions and actions with respect to research transfer practice and consisted of a 12-question survey completed by participants prior to taking the course and 3 months post-course. In two out of the three categories, attitudes and intention to practice, there was positive change from pre- to post-course (P < 0.05). Although there was a trend of increased RT-related action, this was less robust and did not reach significance. For the RTTP transfer course, a ,stages of change' model of evaluation provided an enhanced understanding by showing changes in participants that would otherwise have been overlooked if only changes in RT behaviour were measured. Additionally, evaluating along a change continuum specifically identifies areas for improvement in future courses. The instrument developed for this study could also be used as a pre-course, participant needs assessment to tailor a course to the change needs of participants. Finally, this ,stages of change' approach provides insight into where barriers to change may exist for research transfer action. [source]


Analysis of the possibility of the relationship between various implant-related measures: an 18-month follow-up study

Summary, Better clarification of the long-term relationship among the various implant-related measures could improve the evaluation process for dental implants. Thus, the aim of the present study was to determine the potential correlations among the volumetric features and nitric oxide content of peri-implant sulcus fluid (PISF) and measures of implant stability, and the marginal bone loss. Completely edentulous patients (n = 15) treated with dental implants and ball attachment mandibular over dentures were included. Resonance frequency analysis (RFA), marginal bone level measurements, PISF volume and spectrophotometrically determined nitrite levels were recorded for all dental implant sites. Measures for early (n = 16) and delayed (n = 14) loaded dental implants were comparatively analysed for a period of 18 months. Some random correlations between PISF volume and marginal bone level, PISF nitrite level and marginal bone level and PISF volume and PISF nitrite content, and RFA and PISF volume were observed. However, the only constant correlation was noticed between implant stability (RFA scores) and marginal bone level. This correlation was negative and significant for all dental implants and for delayed loaded implants (P < 0·05). The pattern of loading seemed to affect the extent, but not the pattern of this relationship. While some of the implant-related measures may be strongly associated (e.g. dental implant stability and marginal bone level), not all measures from a single implant site are likely to be related. Such associations may be under the influence of a variety of factors including the loading protocol of dental implants. [source]


Judgmental Discounting and Environmental Risk Perception: Dimensional Similarities, Domain Differences, and Implications for Sustainability

Environmental risks constitute a special category of risks because they often involve consequences that are highly uncertain, strongly delayed, occurring at distant places, and,therefore,mostly borne by others. Economic, decision,theoretic, and psychological research about the way people deal with such consequences is reviewed. Two major findings are presented: first, there is evidence that discounting mechanisms are stable across different preference dimensions (uncertainty, temporal, spatial, and social distance). Second, discount rates tend to vary across different problem domains (e.g., environmental vs. health vs. financial risks). In particular, it appears that temporal discounting is less pronounced for environmental risks than in other domains. Several factors are identified that affect the nature of the risk evaluation process, and it is argued that environmental risks differ from other risks on such factors. These environmental-risk characteristics may have important implications for policy strategies to promote environmental sustainability. Contrary to other domains, appealing to the public's long-term preferences may be successful. Also in policy making, insights from standard economic decision theory to environmental decision making should be applied with caution. [source]


Influence of the Non-Perfect Step Input Concentration at the Feed Side of the Membrane Surface on the Diffusion Coefficient Evaluation

Abstract Summary: Vapor diffusion coefficients in polymeric membranes were evaluated from dynamic permeation experiments. A membrane separated the diffusion cell into two parts , upstream and downstream. At the start of the experiment the concentration change in the upstream part (feed side) was made by substituting the input stream of pure nitrogen by the stream of permeant vapors. The solution of the Fick's second law with the step input concentration function is used for the evaluation of diffusion coefficients. The realization of the step input function can be difficult and its imperfection can negatively influence the evaluation process. This contribution deals with the description of the experimentally obtained input function and the study of its influence on evaluated values of diffusion coefficients. The mathematical model, which includes the non-perfect step input concentration function and the transport through a polymer was developed. The results of this study enable the estimation of diffusion coefficient evaluation errors as dependence on the experimental arrangements and on the membrane transport properties. Investigating the use of sampling for maximising the efficiency of student-generated faculty teaching evaluations

Purpose, Surveys of medical students are widely used to evaluate course content and faculty teaching within the medical school. Gathering information that accurately reflects student perceptions requires that students buy into the evaluation process and be willing to provide thoughtful responses to the teaching evaluation. To maintain student commitment, it is important that medical students are not overburdened with poorly planned evaluations. Sampling might decrease the number of evaluations required of students and might also reduce the proportion of non-responses and other forms of inattentive response biases. Methods, A sampling technique employed within a large medical lecture is described and evaluated. A generalisability study of the teacher evaluations is conducted. Results, A high response rate and high levels of reliability were obtained by sampling a small proportion of the total class. The largest source of error was related to rater and utilising sufficient numbers of student-raters is critical to achieving reliable results. Conclusion, Sampling can reduce evaluation demands placed on students, and preserve reliability and increase the validity of mean evaluation scores. With computer presentation, efficient sampling techniques become practical and should be part of software packages used to present teacher evaluations. [source]


The ,Collaborative Care' curriculum: an educational model addressing key ACGME core competencies in primary care residency training

Aim, The ,Collaborative Care' curriculum is a 12-month senior resident class project in which one evidence-based clinical guideline is designed, implemented and evaluated in our residency practice. This curriculum specifically addresses three of the six Accreditation Council for Graduate Medical Education (ACGME) core competencies: Practice-Based Learning and Improvement, Interpersonal and Communication Skills and System-Based Practices. Additionally, the project enhances the quality of patient care within the model family practice centre in a family practice residency. Methods, During the project, the third-year residency class selects the disease, develops the clinical guideline, leads its implementation and guides the evaluation process. Select faculty members serve as mentors and coach the resident class through each phase of the project. Specific educational objectives are developed for each content area: evidence-based medicine, clinical guideline development, continuous quality improvement and team leadership. A series of seminars are presented during the project year to provide ,just-in-time' learning for the key content and skills required for each step in the project. By working together to develop the practice guideline, then working with nurses and allied health staff to implement the guideline and review its effectiveness, the resident team gains competence in the areas of practice-based learning and improvement, interpersonal and communication skills and system-based practices. Results, The self-reported level of resident confidence in skill acquisition for each content area was measured for each resident at the time of graduation from the residency programme. Results from the first 2 years of this curriculum are reported (resident n = 12), and demonstrate a high level of physician confidence in the skills addressed and their utility for future practice. Conclusions, The senior resident seminar and team project model reported here creates learning experiences that appear to address at least three of the ACGME general competency expectations: practice-based learning and improvement, interpersonal communication skills, and systems-based practice. From the initial resident feedback, this educational model seems to establish a high level of physician confidence in the skills addressed and their utility for future practice. [source]

Objective, This instalment in the series on professional assessment summarises how peers are used in the evaluation process and whether their judgements are reliable and valid. Method, The nature of the judgements peers can make, the aspects of competence they can assess and the factors limiting the quality of the results are described with reference to the literature. The steps in implementation are also provided. Results, Peers are asked to make judgements about structured tasks or to provide their global impressions of colleagues. Judgements are gathered on whether certain actions were performed, the quality of those actions and/or their suitability for a particular purpose. Peers are used to assess virtually all aspects of professional competence, including technical and non-technical aspects of proficiency. Factors influencing the quality of those assessments are reliability, relationships, stakes and equivalence. Conclusion, Given the broad range of ways peer evaluators can be used and the sizeable number of competencies they can be asked to judge, generalisations are difficult to derive and this form of assessment can be good or bad depending on how it is carried out. Integrating evaluation units into the political environment of government: The role of evaluation policy

Most discussions of evaluation policy focus on the substance and process of doing evaluations. However, doing evaluations in government requires careful consideration not only of evaluation but also of the larger political structure into which it is expected to fit. I argue in this chapter that success for evaluation in government depends as much on the political context within which evaluation operates as it does on the merits of the evaluation process itself. For convenience, I divide the contextual governmental pressures on evaluation into three kinds: those stemming from the overarching structure of our democracy, those stemming from the bureaucratic climate of a particular agency, and those stemming from the dominant professional culture within that agency. I then examine how those three kinds of pressures have, in my experience, affected the independence, credibility, and ethical position of the evaluation units and evaluators concerned. Finally, I offer some suggestions for evaluation policy in the hope of avoiding a repetition of past evaluative failures that resulted either from unawareness of political relationships in government or from the inability of small evaluation units to protect their work in the face of much more powerful political forces. © Wiley Periodicals, Inc. Evaluation of the Acceptability and Usability of a Decision Support System to Encourage Safe and Effective Use of Opioid Therapy for Chronic, Noncancer Pain by Primary Care Providers

Abstract Objective., To develop and evaluate a clinical decision support system (CDSS) named Assessment and Treatment in Healthcare: Evidenced-Based Automation (ATHENA)-Opioid Therapy, which encourages safe and effective use of opioid therapy for chronic, noncancer pain. Design., CDSS development and iterative evaluation using the analysis, design, development, implementation, and evaluation process including simulation-based and in-clinic assessments of usability for providers followed by targeted system revisions. Results., Volunteers provided detailed feedback to guide improvements in the graphical user interface, and content and design changes to increase clinical usefulness, understandability, clinical workflow fit, and ease of completing guideline recommended practices. Revisions based on feedback increased CDSS usability ratings over time. Practice concerns outside the scope of the CDSS were also identified. Conclusions., Usability testing optimized the CDSS to better address barriers such as lack of provider education, confusion in dosing calculations and titration schedules, access to relevant patient information, provider discontinuity, documentation, and access to validated assessment tools. It also highlighted barriers to good clinical practice that are difficult to address with CDSS technology in its current conceptualization. For example, clinicians indicated that constraints on time and competing priorities in primary care, discomfort in patient-provider communications, and lack of evidence to guide opioid prescribing decisions impeded their ability to provide effective, guideline-adherent pain management. Iterative testing was essential for designing a highly usable and acceptable CDSS; however, identified barriers may limit the impact of the ATHENA-Opioid Therapy system and other CDSS on clinical practices and outcomes unless CDSS are paired with parallel initiatives to address these issues. A screening tool for vulnerability assessment of pesticide leaching to groundwater for the islands of Hawaii, USA

Abstract This paper describes an updated version of a screening tool for groundwater vulnerability assessment to evaluate pesticide leaching to groundwater, based on a revised version of the attenuation factor. The tool has been implemented in a geographical information system (GIS) covering the major islands of the state of Hawaii, USA. The Hawaii Department of Agriculture currently uses the tool in their pesticide evaluation process as a first-tier screening tool. The basic soil properties and pesticide properties necessary to compute the index, and estimates of their uncertainty, are included in the GIS. Uncertainties in soil and pesticide properties are accounted for using first-order uncertainty analysis. Classifications of pesticides as ,likely', ,uncertain' or ,unlikely' to leach are made on the basis of the uncertainty and a comparison of the revised attenuation factor with values and uncertainties of two reference chemicals. The reference chemicals represent what are considered to be a ,leachable' and a ,non-leachable' pesticide under Hawaii conditions. It is concluded that the tool is suitable for screening new and already used pesticides for the islands of Hawaii. PHYSICA STATUS SOLIDI (C) - CURRENT TOPICS IN SOLID STATE PHYSICS, Issue 10 2004

The Fifth International Symposium on Blue Laser and Light Emitting Diodes (ISBLLED-2004) was held in Gyeongju, Korea from 15,19 March 2004. Gyeongju, the ancient capital of the thousand-year Silla kingdom (57 B.C. to 935 A.D.) provided additional pleasure to the participants as an exceptional open-air museum with antique treasures scattered all around the city. During the last decade we have witnessed remarkable developments in wide-gap semiconductors and light emitting devices in the spectral range from the visible to deep UV. The purpose of the Symposium was to provide a forum for intensive discussion on the issues and main progress especially in optoelectronic devices, material growth and characterization, and quantum structures of wide bandgap semiconductors. A total of 243 papers including 220 contributed and 23 invited ones were presented and discussed by 487 participants from 17 countries world-wide. Among them, 154 manuscripts were submitted and reviewed by the usual evaluation process of physica status solidi. Some were rejected or withdrawn, and finally 139 papers are published in the special issues of physica status solidi (a), (b), and (c). We gratefully acknowledge the referees for their careful review. The papers are grouped into 7 categories. The subheadings and the number of papers in each are as follows: Optoelectronic devices, 43; Growth and characterization, 45; Nano and quantum structures, 21; Contacts, 8; Zinc oxide, 9; Indium nitride and indium rich InGaN, 6; Others, 7. The special session of the Symposium, "The LED Highlight", designed partially to meet the challenging targets of the technology, i.e., energy savings and clean environment preservation, drew much attention and is edited as a special coloured section in this issue. The next symposium is scheduled for Montpellier, France, in 2006. We wish the organizers of that symposium the best of luck and hope to see all of the ISBLLED-2004 participants again at ISBLLED-2006. Process improvement evaluation approach using flow diagrams

Abstract Maintaining company processes will afford optimum operation and enhance the company's quality management system. A vital part of maintaining processes is their periodic evaluation. An important part of the monitoring and evaluation process is the flow diagram. A detailed flow diagram is an important tool that affords ease in visualizing the total process and is an aid in locating problem areas. The evaluation of a process is broken down into 15 steps. These steps cover gathering pertinent information, such as problem symptoms from knowledgeable sources and carrying these through their route to potential problem areas to the problems, potential causes, and the root cause of the problem. Knowing the problems brings about the need to assess their impact on the process operation as well as what changes to the process are needed and what impact these changes will have on the product , its improved quality and cost , and other vital information, such as, production rate increase, competition status, and company image. With this information, the appropriate changes are made according to a process change action plan. Sustainable soil remediation by refrigerated condensation at sites with "high-concentration" recalcitrant compounds and NAPL: Two case studies

Remediation of recalcitrant compounds at sites with high concentrations of volatile organic compounds (VOCs) or nonaqueous-phase liquids (NAPLs) can present significant technical and financial (long-term) risk for stakeholders. Until recently, however, sustainability has not been included as a significant factor to be considered in the feasibility and risk evaluation for remediation technologies. The authors present a framework for which sustainability can be incorporated into the remediation selection criteria focusing specifically on off-gas treatment selection for soil vapor extraction (SVE) remediation technology. SVE is generally considered an old and standard approach to in situ remediation of soils at a contaminated site. The focus on off-gas treatment technology selection in this article allows for more in-depth analysis of the feasibility evaluation process and how sustainable practices might influence the process. SVE is more commonly employed for recovery of VOCs from soils than other technologies and generally employs granular activated carbon (GAC), catalytic, or thermal oxidation, or an emerging alternative technology known as cryogenic-compression and condensation combined with regenerative adsorption (C3,Technology). Of particular challenge to the off-gas treatment selection process is the potential variety of chemical constituents and concentrations changing over time. Guidance is available regarding selection of off-gas treatment technology (Air Force Center for Environmental Excellence, 1996; U.S. Environmental Protection Agency, 2006). However, there are common shortcomings of off-gas treatment technology guidance and applications; practitioners have rarely considered sustainability and environmental impact of off-gas treatment technology selection. A prospective evaluation of hemoptysis cases in a tertiary referral hospital

Abstract Background and Aims:, Hemoptysis is symptomatic of a potentially serious and life-threatening thoracic disease. The purpose of this study was to evaluate the relative frequency of the different causes of hemoptysis, the change of the frequency of diseases, the value of the evaluation process and the outcome in a tertiary referral hospital. Methods:, A prospective study was carried out on consecutive patients presented with hemoptysis. Results:, A total of 178 patients (136 male, 42 female) were included to the study. Lung cancer (51), pulmonary embolism (23) and bronchiectasis (23) constituted most of the diagnosis. The most frequent cause of hemoptysis in males was by far lung carcinoma (50). Twelve cases of bronchiectasis and 11 cases of pulmonary embolism were observed in females. While lung cancer and pulmonary embolism were associated with mild to moderate amounts of bleeding (84% and 100%, respectively), patients with active tuberculosis and pulmonary vasculitis had severe to massive hemoptysis (50% and 44%, respectively). Transthoracic and other organ biopsies, spiral computed tomography (CT) angiography (X pres/GX model TSX-002a, Toshiba, Tochigi Ken, Japan) and aortography yielded high diagnostic results in our group (100%, 67%, 59% and 100%, respectively). The most frequent final diagnosis in patients with normal chest radiograph was pulmonary embolism (seven cases). Conclusions:, Lung cancer, pulmonary embolism and bronchiectasis were the main causes of hemoptysis in this prospective cohort; however, this is the first report showing pulmonary embolism as a leading cause of hemoptysis. CT angiography with high-resolution CT should be the primary diagnostic modality if the initial investigation is inconclusive in hemoptysis cases. Predictors of Candidate Maturation Among Potential Living Donors

The shortage of deceased donor allografts and improved outcomes in partial organ transplantation have led to widespread application of adult-to-adult living donor liver transplantation. Donor selection limits overall utilization of this technique and predictors of candidate maturation have been inadequately studied to date. We therefore collected data on 237 consecutive potential donors including their age, sex, ethnicity, relationship to the recipient, education, employment and religious beliefs and practices. Of these 237 candidates, 91 (38%) were excluded for medical and psychosocial reasons, 53 (22%) withdrew from the process predonation and 93 (39%) underwent partial liver donation. In multivariate analyses, the relationship between the donor and the recipient was highly predictive of successful donation. For pediatric recipients, no parents voluntarily withdrew from the evaluation process. For adult recipients, spouses are the most likely to donate, followed by parents, children and siblings. Additional predictors for donation included self-description as religious but not regularly practicing, part-time employment and higher education. Race, ethnicity, gender and age did not predict donation in multivariate analysis. Further understanding of the complex decision to donate may improve donation rates as well as permit more efficient and cost-effective donor evaluation strategies. [source]


A feed is only as good as its ingredients , a review of ingredient evaluation strategies for aquaculture feeds

Abstract The evaluation of feed ingredients is crucial to nutritional research and feed development for aquaculture species. In evaluating ingredients for use in aquaculture feeds, there are several important knowledge components that should be understood to enable the judicious use of a particular ingredient in feed formulation. This includes information on (1) ingredient digestibilities, (2) ingredient palatability and (3) nutrient utilization and interference. Diet design, feeding strategy, faecal collection method and method of calculation all have important implications on the determination of the digestible value of nutrients from any ingredient. There are several ways in which palatability of ingredients can be assessed, usually based on variable inclusion levels of the ingredient in question in a reference diet and feeding of those diets under an apparent satietal or self-regulating feeding regimes. However, the design of the diets, the parameters of assessment and the feeding regime can all be subject to variation depending on subtleties of the experimental design. Clearly, issues relating to feed intake are the key performance criteria in palatability assessments, and it is important that such experiments maintain sufficient stringency to allow some self-discrimination of the test feeds by the fish. The ability of fish to use nutrients from the test ingredient, or defining factors that interfere with that process, is perhaps the most complex and variable part of the ingredient evaluation process. It is crucial to discriminate effects on feed intake from effects on utilization of nutrients from ingredients (for growth and other metabolic processes). To allow an increased focus on nutrient utilization by the animals, there are several experimental strategies that can be adopted, which are based on variations in diet design and feeding regime used. Other issues such as ingredient functionality, influence on immune status and effects on organoleptic qualities are also important consideration in determining the value of ingredients in aquaculture feed formulations. A key aspect to note is the need to design all experiments with sufficient experimental capacity to detect significant effects. [source]
