    A Simple, One-Step Approach to Durable and Robust Superhydrophobic Textiles,

    Abstract Superhydrophobic textile fabrics are prepared by a simple, one-step gas phase coating procedure by which a layer of polymethylsilsesquioxane nanofilaments is grown onto the individual textile fibers. A total of 11 textile fabrics made from natural and man made fibers are successfully coated and their superhydrophobic properties evaluated by the water shedding angle technique. A thorough investigation of the commercially relevant poly(ethylene terephthalate) fabric reveals an unparalleled long-term water resistance and stability of the superhydrophobic effect. Because of the special surface geometry generated by the nanoscopic, fibrous coating on the microscopic, fibrous textiles, the coated fabric remains completely dry even after two months of full immersion in water and stays superhydrophobic even after continuous rubbing with a skin simulating friction partner under significant load. Furthermore, important textile parameters such as tensile strength, color, and haptics are unaffected by the silicone nanofilament coating. For the first time, an in-depth characterization of the wetting properties, beyond simple contact angle measurements, as well as a thorough evaluation of the most important textile parameters is performed on a superhydrophobic fabric, which reveals a true potential for application.     Direct atomic layer deposition (ALD) on composite electrodes leads to ultrathin conformal protective coatings without disrupting inter-particle electronic pathways. Al2O3 -coated natural graphite (NG) electrodes obtained by direct ALD on the as-formed electrode show exceptionally durable capacity retention even at an elevated temperature of 50,°C. In sharp contrast, ALD on powder results in poorer cycle retention than bare NG. [source]

    A Review of the Biologic Effects, Clinical Efficacy, and Safety of Silicone Elastomer Sheeting for Hypertrophic and Keloid Scar Treatment and Management

    Silicone elastomer sheeting is a medical device used to prevent the development of and improve the appearance and feel of hypertrophic and keloid scars. The precise mechanism of action of silicone elastomer sheeting has not been defined, but clinical trials report that this device is safe and effective for the treatment and prevention of hypertrophic and keloid scars if worn over the scar for 12 to 24 hours per day for at least 2 to 3 months. Some of the silicone elastomer sheeting products currently on the market are durable and adhere well to the skin. These products are an attractive treatment option because of their ease of use and low risk of adverse effects compared to other treatments, such as surgical excision, intralesional corticosteroid injections, pressure therapy, radiation, laser treatment, and cryotherapy. Additional controlled clinical trials with large patient populations may provide further evidence for the efficacy of silicone elastomer sheeting in the treatment and prevention of hypertrophic and keloid scars. The purpose of this article is to review the literature on silicone elastomer sheeting products and to discuss their clinical application in the treatment and prevention of hypertrophic and keloid scars.     Outcomes of surgical treatment of intrathoracic stomach

    SUMMARY The purpose of this study is to assess the long-term outcomes after surgical repair of intrathoracic stomach. Prospectively collected data was retrospectively reviewed. Patients underwent a phone questionnaire 1 year postoperatively to assess gastroesophageal reflux disease-related symptoms and surgical satisfaction. In addition, objective evaluation for integrity of hiatal hernia repair was undertaken either by esophagram or endoscopy. Any recurrence was considered a failure. Forty-one patients underwent surgical repair of a large paraesophageal hernia with intrathoracic stomach during the study period. Thirty-four patients underwent a laparoscopic repair, and seven patients underwent a transthoracic repair. An antireflux procedure was performed on 28 patients, and 13 patients had only hernia reduction and hiatal closure. In the laparoscopic group, two patients required conversion to open laparotomy, as one was unable to tolerate the pneumoperitoneum, and the other had mediastinal bleeding. Thirty-eight (93%) were available for 1-year follow-up. There were three (7.8%) recurrences, one requiring emergency transabdominal repair, and the other two being asymptomatic 1-cm recurrences. All patients report a high degree of satisfaction with surgery. There is a high incidence of short esophagus in patients with intrathoracic stomach. The surgical repair is safe and durable, with high patient satisfaction at 1-year follow-up. [source]


    Novel therapeutics with enhanced biological activity generated by the strategic introduction of silicon isosteres into known drug scaffolds

    Abstract The strategic replacement of carbon with silicon within biologically prevalidated drug scaffolds can generate focused libraries of pharmaceutically relevant agents with novel, durable and marketable intellectual property. This approach can be cost-effective and of lower developmental risk because known drugs have recognized pharmacology and toxicity profiles, proven safety in humans, and established manufacturing and formulation methods. The change in shape, charge, and lipophilicity that can result from the addition of silicon can favorably alter the biological activity and toxicology of the parent drug. Silicon-containing derivatives of indomethacin are COX-2 selective, suggesting they will not be associated with the classical toxicities associated with nonselective inhibition of the cyclooxygenases. The silicon-indomethacin derivatives also demonstrated superior anti-cancer activity at clinically achievable concentrations when tested in vitro against a human pancreatic cancer cell line, MiaPaCa-2, and a panel of 14 human multiple myeloma cell lines. Bioorganosilicon chemistry represents an attractive approach for emerging biopharmaceutical organizations seeking to rapidly develop a portfolio of novel pharmacological agents that have the potential for enhanced therapeutic and pharmacological benefit.     Tales from Two Deltas: Catfish Fillets, High-Value Foods, and Globalization

    Abstract: This article examines two places of catfish production, the Mekong River Delta of Vietnam and the Mississippi River Delta of the United States, and uses the concept of globalization to illustrate how these distant places have been brought into competition and how this competition is mediated. Bringing these deltas together is a similar commitment to an economic development strategy that is based on catfish production, a desire to gain access to wealthy consumers who are willing to purchase this high-value food item, and processing and transportation technologies that allow this perishable product to be made more "durable" and to be shipped great distances. Mediating this relationship are consumers' preferences, product labeling, and the U.S. state. This case study illustrates the heterogeneous outcomes of globalization as these deltas are brought into a relationship that, in some ways, is closer than their absolute distance may indicate. The "backlash" forces, such as nontariff trade barriers, nationalism, and a still-powerful state (as both a regulator and consumer), characterize these globalizations. [source]

    Summary A Competitive, Sustainable and Diverse Agriculture: A View of the CAP Beyond 2013 New challenges make clear that past achievements of the CAP cannot be taken for granted. A strong European agricultural policy remains necessary after 2013, at the service of Europe's citizens and agricultural sector. However, in order to realise the vision embedded in the European agricultural model, the CAP will have to evolve. Beyond dealing with the negative consequences of the economic crisis, more attention should go to competitiveness and entrepreneurship. The functioning of the supply chain should be improved leading to a fairer distribution of costs and benefits. Producer organisations should be expanded and strengthened. Work is needed to put into practice the concept of green growth and to explore the synergy between the demand for public goods and the need for higher farm income. An improved system of direct support remains justified, to compensate for extra costs and to stabilise income. The CAP post-2013 should offer a strong EU framework, able to meet shared challenges, with clear objectives and sufficient funding. Within that framework, diversity is a fact, and regions should be able to deploy CAP policies and funds in a more flexible way to accommodate local needs and problems and to be able to react to changing circumstances. De par l'apparition de nouveaux défis, il est clair que les réalisations antérieures de la PAC ne peuvent être considérées comme acquises.Une politique agricole européenne forte, au service des citoyens et du secteur agricole de l'Europe, reste indispensable après 2013. Cependant, pour concrétiser la vision comprise dans le modèle agricole européen, la PAC devra évoluer. Au-delà du traitement des conséquences négatives de la crise économique, l'attention doit se porter davantage sur la compétitivité et l'esprit d'entreprise. Il faudrait améliorer le fonctionnement de la filière de l'offre pour obtenir une répartition des coûts et des avantages plus équitable. Les organisations de producteurs devraient se développer et se renforcer. Des travaux sont nécessaires pour mettre en pratique le concept de croissance verte et pour étudier les synergies entre la demande de biens d'intérêt public et le renforcement nécessaire des revenus agricoles. Un système de soutien direct amélioré reste justifié, pour compenser les coûts supplémentaires et stabiliser les revenus. La PAC d'après 2013 devrait fournir un cadre européen solide, capable d'atteindre les défis partagés, avec des objectifs clairs et des financements suffisants. Au sein de ce cadre, la diversité est un fait et les régions devraient pouvoir déployer les politiques et les fonds de la PAC de manière plus flexible pour répondre aux besoins et problèmes locaux et pour permettre de réagir aux changements. Die GAP wird sich jedoch weiterentwickeln müssen, um der Vision aus dem europäischen Agrarmodell entsprechen zu können. Das Augenmerk sollte hierbei nicht nur auf den Umgang mit den negativen Auswirkungen der Wirtschaftskrise, sondern ebenfalls auf Wettbewerbsfähigkeit und Unternehmertum gerichtet werden. Die Funktionsfähigkeit der Wertschöpfungskette sollte erhöht werden und zu einer gerechteren Verteilung von Kosten und Nutzen führen. Erzeugerorganisationen sollten ausgeweitet und gestärkt werden. Es muss viel getan werden, um das Konzept des Grünen Wachstums umzusetzen und die Synergien zwischen der Nachfrage nach öffentlichen Gütern und der Notwendigkeit für höhere Einkommen in der Landwirtschaft zu untersuchen. Ein verbessertes System für die Direktzahlungen ist nach wie vor gerechtfertigt, um zusätzliche Kosten auszugleichen und die Einkommen zu stabilisieren. Nach 2013 sollte die GAP einen stabilen EU-Rahmen bieten, um den gemeinsamen Herausforderungen mit klaren Zielen und ausreichender Finanzierung begegnen zu können.     Summary Agricultural and Rural Development in China: Achievements and Challenges China has made great advances in its agricultural and rural development since the reforms and opening-up that began in 1978. It has not only fed its population of 1.3 billion, but has also contributed to international agricultural development and food security. Agricultural production registered great development, providing sufficient food and clothes for 21 per cent of the world's population with 9 per cent of the arable land. In the process farmers' living standards improved remarkably and rural public utilities and services were greatly enhanced. China is now in a key transition period of accelerating the transformation and modernisation of traditional agriculture and rural society. It is facing significant challenges. Agriculture is still one of the weakest industries in China and it is proving difficult to sustain increases in grain output and farmers' incomes. The problems of uneven development in rural areas have become increasingly prominent and the gap between urban and rural development is tending to widen. The Chinese government will respond strategically to these challenges and will firmly pursue the construction of a new efficient and sustainable socialist countryside, along the path of modernisation with Chinese characteristics. It will also make greater contributions to world agriculture and rural development. Les progrès de la Chine en termes de développement agricole et rural depuis le début des réformes et l'ouverture en 1978 ont été considérables. Non seulement le pays a nourrit une population de 1.3 millions d'habitants mais il a contribué au développement et à la sécurité alimentaire au niveau international. La production agricole a fortement augmenté et a fournit suffisamment de nourriture et de vêtements à 21 pour cent de la population du monde avec 9 pour cent des terres cultivables. Ce processus s'est accompagné d'une hausse considérable du niveau de vie des agriculteurs et d'une grande amélioration des services publics dans les zones rurales. La Chine est maintenant à un moment clé de sa période de transition, caractérisé par une accélération de la transformation et de la modernisation de l'agriculture et de la société rurale traditionnelles. Des défis importants se présentent à elle. L'agriculture reste une des industries chinoises les plus fragiles et il se révèle difficile de continuer à augmenter la production céréalière et les revenus des agriculteurs. Die Agrarproduktion wurde erheblich ausgeweitet und deckt nun 21 Prozent des weltweiten Bedarfs an Lebensmitteln und Kleidung bei gerade einmal 9 Prozent der Weltackerfläche. Dabei haben sich die Lebensbedingungen für die Landwirte sowie das Angebot an öffentlichen Einrichtungen und Dienstleistungen im ländlichen Raum deutlich verbessert. China durchläuft gerade eine wichtige Übergangsphase, in der sich der Wandel und die Modernisierung der traditionellen Landwirtschaft und der Landbevölkerung noch schneller vollziehen, und steht großen Herausforderungen gegenüber. Die Landwirtschaft ist nach wie vor einer der schwächsten Sektoren in China, und es erweist sich als schwierig, die Steigerungsraten bei der Getreideerzeugung und den Einkommen in der Landwirtschaft aufrecht zu erhalten. Die Probleme der ungleichmäßigen Entwicklung in ländlichen Gebieten werden immer offensichtlicher, und die Kluft zwischen städtischer und ländlicher Entwicklung droht sich auszuweiten. Die chinesische Regierung wird diesen Herausforderungen strategisch begegnen und , ganz im Sinne einer Modernisierung mit chinesischen Merkmalen , daran festhalten, einen neuen sozialistischen ländlichen Raum effizient und nachhaltig zu gestalten.     The Contribution of Bioenergy to a New Energy Paradigm

    Biomass is a widely available resource that is receiving increased consideration as a renewable substitute for fossil fuels. Developed sustainably and used efficiently, it can induce growth in developing countries, reduce oil demand, and address environmental problems. The potential benefits include: reduction of greenhouse gases, recuperation of soil productivity and degraded land, economic benefits from adding value to agricultural activities and improving access to and quality of energy services. The production of bioenergy involves a range of technologies, including solid combustion, gasification, and fermentation. These technologies produce energy from a diverse set of biological resources - traditional crops, crop residues, energy-dedicated crops, dung, and the organic component of urban waste. The results are bioenergy products that provide multiple energy services: cooking fuel, heat, electricity and transportation fuels. It is this very diversity that holds the potential of a win-win-win for the environment, social and economic development. Bioenergy has to be viewed not as a replacement for oil, but as an element of a portfolio of renewable sources of energy. Coherent and mutually supportive environmental and economic policies may be needed to encourage the emergence of a globally dispersed bioenergy industry that will pursue a path of sustainable development. La biomasse est une resource largement répandue, qui commence à retenir l'attention comme substitut renouvelable aux énergies fossiles. En l'utilisant de façon efficace et durable, on peut accélérer la croissance des pays en voie de développement, réduire la demandepour le pétrole et résoudre certains problèmes d'environnement. Au nombre des bénéfices potentiels il faut mettre : la réduction des émissions de gaz à effet de serre, la reconstitution de la fertilité dessols et des terres dégradées, les avantages économiques liés à l'accroissement de la production agricole et à l'amélioration des services énergétiques, tant en qualité qu'en accessibilité. La production de bioénergie met en oeuvre un large éventail de techniques parmi lesquelles la combustionde produits solides, la gazéification et la fermentation. Elles produisent de l'énergie à partir d'une grande variété de sources biologiques : cultures traditionnelles, résidus de cultures, cultures spécialisées, fumiers et déchets organiques urbains. Les produits bio-énergétiques qui en résultent couvrent une grande variété d'usages : énergie de cuisson, chauffage, électricité et transports. C'est précisément sur cette diversité que repose l'espoir de gains dans toutes les directions, sociales, environnementales et économiques. Il ne faut pas voir la bioénergie comme un simple substitut au pétrole, mais comme un portefeuille de ressources renouvelables. Pour encourager l'émergence d'une industrie bioénergétique largement répandue et susceptible de contribuer au développement durable, il faudra sans doute élaborer des politiques économiques et environnementales cohérentes, capables de se soutenir mutuellement. Diese Technologien erzeugen Energie mittels unterschiedlicher biologischer Ressourcen , traditionelle Feldfrüchte und deren Rückstände, spezielle Energiepflanzen, Mist sowie der organische Anteil städtischer Abfälle. Die daraus erzeugte Bioenergie kann zum Kochen, zum Heizen, als Elektrizität oder als Treibstoff genutzt werden. Gerade in dieser Vielfalt liegt der potenzielle Gewinn für die Umwelt und die soziale sowie die wirtschaftliche Entwicklung. Bioenergie sollte nicht als ein Ersatz für Öl, sondern als Bestandteil des Portfolios erneuerbarer Energiequellen angesehen werden.     Delivering on the Potential of the New CAP

    Recent discussions on the CAP have focused on the budget. However, in the public debate the policy itself is often still a caricature of the old CAP that existed until the early 1990s. The CAP has changed fundamentally over the past decade. The recent direction of the CAP , markets and rural development , was set by the European Council in Göteborg and Lisbon. Strong economic performance must go hand in hand with the sustainable use of natural resources. The key elements of the new CAP are a market policy where intervention is a safety net, income stabilisation is delivered through decoupled aids subject to cross-compliance, and a reinforced rural development policy that focuses on jobs, growth and sustainability. We must use the new CAP to unlock the potential for growth, jobs and innovation and put good ideas into practice. We need to work in partnership with farmers, foresters, the agrifood business, NGOs, the population of rural areas, the research community and of course public authorities. But to achieve all of this we need a stable budgetary environment, in which farmers and businesses can plan. In short, we need the resources to deliver on the potential of the new CAP. Les discussions sur la PAC, récemment, se sont focalisées sur le budget. En même temps, le contenu politique de la réforme, tel qu'il est vu dans le débat public, n'est rien d'autre que la caricature de l'ancienne PAC telle qu'elle existait avant les années 90. Mais pour réaliser tout cela, il faut encore un environnement budgétaire stable, permettant aux agriculteurs et aux industriels de planifi er leurs actions. En d'autres termes, il faut des ressources pour que la nouvelle PAC tienne ses promesses. Die jüngsten Diskussionen über die GAP konzentrierten sich auf den Haushalt. Eine hohe wirtschaftliche Leistungsfähigkeit muss mit der nachhaltigen Nutzung der natürlichen Ressourcen Hand in Hand gehen. Die Schlüsselelemente der neuen GAP sind eine Marktpolitik, in der die Intervention als Sicherheitsnetz dient und eine Einkommensstabilisierung mittels entkoppelter Beihilfen erfolgt, für die Cross Compliance gilt; sowie eine gestärkte Politik zur Entwicklung des ländlichen Raums, welche sich auf Arbeitsplätze, Wachstum sowie Nachhaltigkeit konzentriert. Wir müssen die neue GAP dazu verwenden, das Potenzial für Wachstum, Arbeitsplätze und Innovationen frei zu setzen, und gute Ideen in die Tat umsetzen.     Summary EU Enlargement to Turkey? The Challenges Ahead for Agriculture and Rural Areas In December 2004 the European Council agreed that accession negotiations with Turkey could begin in October 2005. Although the Copenhagen political criteria have been decisive so far, the criteria relating to the functioning of markets and ability to implement EU legislation will weigh heavily in the negotiations. Despite recent policy reforms, Turkey's agriculture still enjoys levels of protection similar or higher than those of the EU, but its gross value added per worker is relatively very low. One third of Turkey's employment is in agriculture, where most farms are small. The animal health situation is far below the standard achieved in the EU. Food supply chains are fragmented and inefficient. To survive and compete within the single market of the EU, the agricultural and food sectors must undergo deep structural change, which will have far-reaching economic and social repercussions. Projections according to current policy trends show that Turkey could take over one-third of the total cohesion budget allocation. However, the scalingdown or delaying of budget transfers would be short-sighted. If the EU accepts Turkey as a member, it would be in the interest of the EU as a whole that Turkey's economy - and especially its backward rural economy - should embark on a sustainable growth path as soon as possible.     Summary Agriculture Under the Public Eye: Who Cares for What? The new CAP is becoming geared towards a more sustainable agriculture that takes account of the needs of future generations. Achieving sustainability, however, means meeting three challenges-, (a) profit - strengthening the viability and competitiveness of the agricultural sector; (b) planet - the ecological challenge of promoting good environmental practices; and (c) people-the social challenge to improve the living conditions and economic opportunities in rural areas. In the food chain we see responses to consumer concerns about food safety, quality and welfare issues through the development and implementation of mandatory and voluntary quality control and assurance schemes. And the consolidation and internationalisation of the food retailing and the manufacturing industry is expected to continue. An important duality has emerged. On the one side, we find a state system of regulation, on the other a system of self-regulation, largely driven by the major forces in supply chain management, the food retailers in particular. A simple and effective regulatory environment for the agri-food complex is essential if we are to achieve our competitiveness goals. One of the priorities for discussion by politicians, therefore, should be whether current and expected policy and industry developments should lead to a review of the balance between markets and government, ,who cares for what?' l'agriculture aux yeux des politiques publiques qui doit faire quoi?     Adhesion Characteristics of PDMS Surfaces During Repeated Pull-Off Force Measurements,

    Abstract To mimic the adhesive effects of gecko toes, artificial surfaces have been manufactured recently using polydimethylsiloxanes (PDMS). However, the effects of repeated contacts on the adhesive properties remain largely unexplored. In this paper we report on the effect of repeated pull-off force measurements on the adhesion behavior of PDMS (polymer kit Sylgard 184, Dow Corning) tested with a borosilicate glass probe. A decrease in pull-off force with increase in number of test cycles is found until a plateau is reached. The initial value and the rate of change in pull-off force strongly depend on the sample preparation procedure, including curing time and cross-linking. It is proposed that the behavior is due to steady coverage of the probe with free oligomers. The results are crucial for developing reusable, durable, and residue-free bioinspired adhesives.     Influence of contamination on resin bond strength to nano-structured alumina-coated zirconia ceramic

Zirconia ceramic disks were coated with nano-structured alumina, utilizing the hydrolysis of aluminum nitride powder. After immersion in saliva or the use of a silicone disclosing agent, specimens were cleaned with phosphoric acid etching or with tap water rinsing only. Uncontaminated specimens served as controls. Plexiglas tubes filled with composite resin were bonded with a phosphate monomer [10-methacryloxydecyl-dihydrogenphosphate (MDP)]-containing resin (Panavia 21). Subgroups of eight specimens each were stored in distilled water at 37°C, either for 3 d without thermal cycling (TC) or for 150 d with 37,500 thermal cycles from 5 to 55°C. The tensile bond strength (TBS) was determined using a universal testing machine at a crosshead speed of 2 mm min,1. The topography of the debonded surface was scrutinized for fractographic features, utilizing both optical and scanning electron microscopy. The TBS to uncontaminated nano-structured alumina-coated zirconia ceramic was durable, while contamination significantly reduced the TBS. Phosphoric acid cleaning was effective in removal of saliva contamination from the coated bonding surface but was not effective in removal of the silicone disclosing agent. Nano-structured alumina coating improves resin bonding to zirconia ceramic and eliminates the need for air-abrasion before bonding. [source]

    Flame-retardant finishing of cotton fleece fabric: part I. The use of a hydroxy-functional organophosphorus oligomer and dimethyloldihydroxylethyleneurea

    Abstract Cotton fleece has become a popular fashion in recent years. However, most of the 100% cotton fleece fabric is not able to meet the federal flammability standard (,16 CFR Part 1610: Standard for the Flammability of Clothing Textiles') without chemical treatment. In this research, we investigated the use of the combination of a hydroxy-functional organophosphorus oligomer (HFPO) as the flame-retarding agent and dimethyloldihydroxylethyleneurea (DMDHEU) as the binder to reduce the flammability of cotton fleece. We found that HFPO is effective in reducing the flammability of the cotton fleece whereas DMDHEU enhances the effectiveness of HFPO due to phosphorus,nitrogen synergism. The flammability as well as other properties of the treated cotton fleece is affected by both the concentration of HFPO and that of DMDHEU. The cotton fleece treated with HFPO/DMDHEU passes the federal flammability standard and shows high strength retention with little change in fabric whiteness and hand. We also found that the flame-retardant finishing system is durable to multiple home launderings. The combination of HFPO and DMDHEU has the potential to become a practical flame-retardant finishing system to reduce the flammability of cotton fleeces. Copyright © 2006 John Wiley & Sons, Ltd.     Molecular-Level Dispersion of Graphene into Poly(vinyl alcohol) and Effective Reinforcement of their Nanocomposites

    Abstract Despite great recent progress with carbon nanotubes and other nanoscale fillers, the development of strong, durable, and cost-efficient multifunctional nanocomposite materials has yet to be achieved. The challenges are to achieve molecule-level dispersion and maximum interfacial interaction between the nanofiller and the matrix at low loading. Here, the preparation of poly(vinyl alcohol) (PVA) nanocomposites with graphene oxide (GO) using a simple water solution processing method is reported. Efficient load transfer is found between the nanofiller graphene and matrix PVA and the mechanical properties of the graphene-based nanocomposite with molecule-level dispersion are significantly improved. A 76% increase in tensile strength and a 62% improvement of Young's modulus are achieved by addition of only 0.7,wt% of GO. The experimentally determined Young's modulus is in excellent agreement with theoretical simulation. [source]


    Intratumoral cisplatin/epinephrine gel in advanced head and neck cancer: A multicenter, randomized, double-blind, phase III study in North America,

    Abstract Background. The objective was to evaluate the efficacy and safety of a novel intratumoral cisplatin/epinephrine injectable gel (CDDP/epi gel) for local control and palliation of tumor-related symptoms in patients with recurrent or metastatic head and neck squamous cell carcinoma (HNSCC). Patients and Methods. Eighty-seven patients were randomly assigned to either CDDP/epi or placebo gel in this phase III, double-blind study. Tumors were ,20 cm3; most recurrences (88%) were in a previously irradiated field. The most symptomatic or threatening tumor was designated as the target tumor. Dose: 0.25 mL CDDP/epi gel/cm3 tumor volume. Treatments: ,6 weekly intratumoral injections in an 8-week period. Primary outcomes: target tumor response and symptom relief. Results. During the blinded phase, 34% (21 of 62) of patients achieved an objective response (CR or PR) in the target tumor treated with CDDP/epi gel vs 0% (0 of 24) treated with placebo gel (p < .001). Responses occurred within a median of four treatments (range, 2,6) and were durable (median, 95 days; range, 34,168+ days). More patients treated with CDDP/epi gel achieved palliative benefit than did those treated with placebo gel (37% vs 12%, p = .036). Most frequent side effects were local pain and local cutaneous reactions, which resolved over 3,12 weeks. Renal and hematologic toxicities were rare. Conclusions. This phase III trial showed that CDDP/epi gel significantly reduces tumor burden, palliates tumor-related symptoms, and is an effective local treatment for recurrent tumors. © 2003 Wiley Periodicals, Inc.     Benefits and risks of interferon therapy for hepatitis B,

    Alpha interferon is the only licensed drug for hepatitis B with immunomodulatory as well as viral inhibitory properties. Potential advantages of interferon compared to nucleoside analogs include a lack of drug resistance, a finite and defined treatment course, and a higher likelihood for hepatitis B surface antigen (HBsAg) clearance. Approximately 30% of hepatitis B e antigen (HBeAg)-positive and 40% of HBeAg-negative cases have a sustained virological response (when defined as HBeAg seroconversion and/or hepatitis B virus (HBV) DNA levels below 20,000 copies/mL, respectively) 6 months after completion of a 48-week course of peginterferon alfa-2a These responses remain durable in 80% and 50% of cases, respectively, when evaluated several years later. Recent studies have shown that changes in HBsAg and HBeAg concentration during treatment predict sustained virological response and serial monitoring of HBsAg is helpful in predicting HBsAg clearance. HBeAg-positive patients with genotype A have higher rates of HBeAg and HBsAg clearance, whereas HBeAg-negative patients with genotype D have the lowest rate of response to interferon therapy. Long-term follow-up of virological responders to either standard alpha interferon or peginterferon has demonstrated a progressive increase in the rate of HBsAg clearance, particularly in patients who were initially HBeAg-positive. Future studies need to address if specific virological benchmarks during therapy can be used to tailor treatment duration. Conclusion: Peginterferon alfa has a place as first-line therapy of hepatitis B in patients who are carefully selected on the basis of pretreatment serum HBV DNA and aminotransferase levels, safety considerations, and viral genotype.     Hepatitis B e antigen seroconversion after lamivudine therapy is not durable in patients with chronic hepatitis B in Korea

    It has been suggested that hepatitis B e antigen (HBeAg) seroconversion after lamivudine therapy is durable in Caucasians with chronic hepatitis B (CHB). However, little is known whether it is also durable in endemic areas of hepatitis B virus (HBV) infection. We evaluated the posttreatment durability of lamivudine-induced HBeAg seroconversion and the predictive factors for relapse in Korean patients with CHB. We retrospectively analyzed 98 HBeAg-positive patients with CHB who were treated with lamivudine between August 1996 and December 1997. Lamivudine was given at a dose of 150 mg per day. After HBeAg seroconversion, lamivudine was continued for an additional 2 to 4 months, and posttreatment monitoring continued for up to 24 months. HBeAg seroconversion was achieved in 34 of the 98 patients (34.7%). The mean duration of treatment in these seroconverters was 9.3 ± 3.0 months. During the follow-up period, the cumulative relapse rates at 1 year and 2 years posttreatment were 37.5% and 49.2%, respectively. Most relapses were accompanied by elevation of serum alanine transaminase (94%) and reappearance of HBeAg (81%). Pretreatment serum HBV DNA levels and the duration of additional lamivudine therapy after HBeAg seroconversion were 2 independent predictive factors for posttreatment relapse. In conclusion, lamivudine-induced HBeAg seroconversion was not durable in this endemic area. And the duration of additional lamivudine therapy after HBeAg seroconversion significantly affected the posttreatment relapse. Further studies are needed to determine the duration of lamivudine and to elucidate the cause of high relapse after HBeAg seroconversion in endemic areas of HBV. [source]


    Large particle hyaluronic acid for the treatment of facial lipoatrophy in HIV-positive patients: 3-year follow-up study

    Objectives Facial lipoatrophy can be a stigmatizing side effect of antiretroviral (AVR) treatment for HIV-infected patients. We sought to evaluate the long-term efficacy and safety of a new formulation of hyaluronic acid that can be injected in larger amounts and into deeper skin layers during 3 years of follow-up. Methods Twenty patients received injections of Restylane SubQÔ. Refill treatment was offered at 12 and 24 months. Treatment effects were evaluated using ultrasound, the Global Aesthetic Improvement Scale, visual analogue scale (VAS) and the Rosenberg self-esteem scale. Results Seventeen patients remained at 36 months. Mean (± standard deviation) total cutaneous thickness increased from 6 ± 1 mm at baseline to 12 ± 1 mm (P<0.001) at 36 months. Response rate (total cutaneous thickness >10 mm) was 70%. Fifteen patients classified their facial appearance as very much or moderately improved. VAS increased from 39 ± 25 to 70 ± 20 (P<0.05) and higher self-esteem scores were reported. Local swelling and tenderness after treatment was common. Persistent papules found in several patients after treatment were removed effectively with hyaluronidase injections. Three patients, treated only at baseline, still had higher total cutaneous thickness scores at 36 months. Conclusions Our results indicate that a large particle hyaluronic acid formulation is a durable and well-tolerated dermal filler for treating HIV-positive patients with facial lipoatrophy. [source]


    The Elusive Path to Cartilage Regeneration

    Abstract Numerous attempts have been made to develop an efficacious strategy for the repair of articular cartilage. These endeavors have been undaunted, if not spurred, by the challenge of the task and by the largely disappointing outcomes in animal models. Of the strategies that have been lately applied in a clinical setting, the autologous-chondrocyte-transplantation technique is the most notorious example. This methodology, which was prematurely launched on the clinical scene, was greeted with enthusiasm and has been widely adopted. However, a recent prospective and randomized clinical trial has revealed the approach to confer no advantage over conventional microfracturing. Why is the repair of articular cartilage such a seemingly intractable problem? The root of the evil undoubtedly lies in the tissue's poor intrinsic healing capacity. But the failure of investigators to tackle the biological stumbling blocks systematically rather than empirically is hardly a less inauspicious circumstance. Moreover, it is a common misbelief that the formation of hyaline cartilage per se suffices, whereas to be durable and functionally competent, the tissue must be fully mature. An appreciation of this necessity, coupled with a thorough understanding of the postnatal development of articular cartilage, would help to steer investigators clear of biological cul-de-sacs.     Generation, persistence and plasticity of CD4 T-cell memories

    Summary The development of immune memory mediated by T lymphocytes is central to durable, long-lasting protective immunity. A key issue in the field is how to direct the generation and persistence of memory T cells to elicit the appropriate secondary response to provide protection to a specific pathogen. Two prevailing views have emerged; that cellular and molecular regulators control the lineage fate and functional capacities of memory T cells early after priming, or alternatively, that populations of memory T cells are inherently plastic and subject to alterations in function and/or survival at many stages during their long-term maintenance. Here, we will review current findings in CD4 T-cell memory that suggest inherent plasticity in populations of memory CD4 T cells at all stages of their development , originating with their generation from multiple types of primed CD4 T cells, during their persistence and homeostatic turnover in response to T-cell receptor signals, and also following secondary challenge. These multiple aspects of memory CD4 T-cell flexibility contrast the more defined lineages and functions ascribed to memory CD8 T cells, suggesting a dynamic nature to memory CD4 T-cell populations and responses. The flexible attributes of CD4 T-cell memory suggest opportunities and mechanisms for therapeutic manipulation at all phases of immune memory development, maintenance and recall. [source]


    Fabrication of a Multilayered Low-Temperature Cofired Ceramic Micro-Plasma-Generating Device

    Plasma technology is currently being used in innumerable industrial applications. Some of the common uses of this technology include surface cleaning and treatment, sputtering and etching of semiconductor devices, excitation source for chemical analyses, cutting, environmental cleanup, sterilization, and phototherapy. The harsh conditions that these devices must endure require robust refractory materials systems for their fabrication and reliability. Low-temperature cofired ceramic (LTCC) material systems provide a durable and cost-effective platform for the manufacture of such devices, and allow for possible integration into meso-scale microsystems. Our designs are based on RF microstriplines that capacitively couple and ionize small gas discharge sites over the top electrode. In this paper, we have built several iterations of this micro-plasma generating device using LTCC material systems. The impact of electrode ink selection and processing, lamination methods, dielectric layer thickness, and electrode design has been investigated. Several micro-plasma-generating devices were then evaluated for power requirements, output stability, and long-term reliability. [source]


    Otological manifestations of acute leukaemia: report of two cases and review of literature

    Otological manifestations exceptionally reveal acute leukaemia, whereas leukaemic infiltration of any tissue is frequent at postmortem examination. We present two cases of acute myeloblastic leukaemia revealed by a loss of hearing due to a middle-ear leukaemic infiltration. The characteristics of such a clinical and radiological finding are emphasized. It is suggested that middle and inner ear can be a ,sanctuary' localization, which might sometimes require radiation therapy to achieve durable and complete remission. [source]


    The suitability of computer-based training for workers with limited formal education: a case study from the US agricultural sector

    The suitability of computer-based instruction (CBI) for workers with limited education was evaluated in an Hispanic orchard workforce that reported little computer experience and 5.6 mean years of formal education. Ladder safety training was completed by employees who rated the training highly (effect size [d_gain] = 5.68), and their knowledge of ladder safety improved (d_gain = 1.45). There was a significant increase (p < 0.01) in safe work practices immediately after training (d_gain = 0.70), at 40 days post training (d_gain = 0.87) and at 60 days (d_gain = 1.40), indicating durability. As in mainstream populations, reaction or affective ratings correlated well with utility ratings, but not with behavior change. This demonstrates that an agricultural workforce with limited formal education can learn job safety from CBI and translate the knowledge to work practice changes, and those changes are durable.     Carceral Chicago: Making the Ex-offender Employability Crisis

    Abstract This article explores the urban labor market consequences of large-scale incarceration, a policy with massively detrimental implications for communities of color. Case study evidence from Chicago suggests that the prison system has come to assume the role of a significant (urban) labor market institution, the regulatory outcomes of which are revealed in the social production of systemic unemployability across a criminalized class of African,American males, the hypertrophied economic and social decline of those ,receiving communities' to which thousands of ex-convicts return, and the remorseless rise of recidivism rates. Notwithstanding the significant social costs, the churning of the prison population through the lower reaches of the labor market is associated with the further degradation of contingent and informal-economy jobs, the hardening of patterns of radical segregation, and the long-term erosion of employment prospects within the growing ex-offender population, for whom social stigma, institutional marginalization and economic disenfranchisement assume the status of an extended form of incarceration. Résumé La politique publique d'incarcération massive, aux implications largement préjudiciables aux communautés de couleur, affecte également le marché du travail des villes. Une étude de cas sur Chicago indique que le système pénitentiaire a fini par devenir une institution importante du marché du travail (urbain) dont les réglementations se traduisent à la fois par la production sociale d'une inemployabilité systémique pour une classe criminalisée de males afro-américains, par le déclin économique et social hypertrophié des ,communautés d'accueil' vers lesquelles retournent des milliers d'ex-prisonniers, et par l'accroissement impitoyable des taux de récidive. Malgré de forts coûts sociaux, le brassage de la population carcérale dans les niveaux inférieurs du marché du travail se combine à la dégradation accrue des postes occasionnels et offerts par l'économie parallèle, mais aussi au durcissement des types de ségrégation radicale et à une érosion durable des perspectives d'emploi au sein de la population grandissante des ex-délinquants pour lesquels stigmatisation sociale, marginalisation institutionnelle et non-reconnaissance économique revêtent une forme d'incarcération prolongée.     High-dose chemotherapy for male germ cell tumor

    Abstract, Today, 20,30% of male patients with advanced germ cell tumor (GCT) do not have durable, complete remission in spite of cis -platinum (CDDP)-based chemotherapy. High-dose chemotherapy (HDCT) has been tried in CDDP refractory GCT patients. Initially HDCT was performed with autologous bone marrow transplantation in heavily treated patients. However, the clinical outcome was not good and the treatment-related death rate was not ignorable. Therefore, earlier introduction of HDCT with peripheral blood stem cell transplantation was preferable as it renders HDCT more effective and less toxic, and multicycle HDCT is feasible. The durable free rate of recent HDCT for refractory GCT patients is 32,65%. HDCT is also performed as first line chemotherapy for poor prognosis GCT patients. Induction chemotherapy followed by multicycles of HDCT was tried. The durable free rate of recent HDCT as first line chemotherapy is 43,73%. Although previous reports suggest the superiority of HDCT, one recent randomized controlled trial (RCT) failed to show an improvement with one cycle of HDCT followed by three cycles of standard-dose chemotherapy (SDCT) compared with four cycles of SDCT. Ongoing RCT comparing multicycles of HDCT with SDCT for poor prognostic GCT patients will clarify the role of HDCT. Recently, new regimens of HDCT containing paclitaxel have been devised. In this review, the history, current status and future of HDCT for advanced or refractory GCT will be discussed. [source]
