    Does prescribing for opiate addiction change after national guidelines?

    Methadone, buprenorphine prescribing to opiate addicts by general practitioners, hospital doctors in England
    ABSTRACT Aim To assess changes in opiate prescribing (1995,2005) following a decade of national guidelines to address substandard opiate substitution prescribing for heroin addiction. Design A repeat national survey (1995 and 2005) using random one-in-four samples of all community pharmacies in England, achieving response rates of 75% (1847/2475) in 1995 and 95% (2349/2473) in 2005. Data were obtained on 3732 (1995 data) and 9620 (2005 data) prescriptions dispensed in the preceding month from the 936 and 1463 pharmacies who were currently dispensing. Measurements We have measured impact on practice for seven specific recommended changes. Findings Between 1995 and 2005 the number of substitute opiate prescriptions doubled (×2.03). By 2005, methadone still dominated (down from 97% to 83%), buprenorphine increased (from 1% to 16%) and other opiate medications virtually disappeared. Changes in the direction of national guidelines included: increased daily dose of methadone (from 47.3 mg to 56.3 mg), more frequent dispensing (from 38% to 60% as daily instalments), more supervised consumption (from 0% to 36%) and fewer methadone tablets (from 10.9% to 1.8%). Nevertheless, despite the increased mean daily dose, only 41.0% of prescriptions for methadone were for daily doses in the recommended 60,120 mg dose range. Only one change was not in the direction of the national guidelines,the proportion of prescriptions from GPs fell from 41% to 30%, although this still represented an approximate 50% increase in the extent of GP prescribing. Conclusion Doubling in provision of opiate substitute treatment has occurred, alongside significant improvements in the nature of this treatment. These positive changes have occurred in the direction of six out of seven of the UK national guidelines. [source]


    Estimated migration rates under scenarios of global climate change

    Aim Greenhouse-induced warming and resulting shifts in climatic zones may exceed the migration capabilities of some species. We used fourteen combinations of General Circulation Models (GCMs) and Global Vegetation Models (GVMs) to investigate possible migration rates required under CO2 -doubled climatic forcing. Location Global. Methods Migration distances were calculated between grid cells of future biome type x and nearest same-biome-type cells in the current climate. In `base-case' calculations, we assumed that 2 × CO2 climate forcing would occur in 100 years, we used ten biome types and we measured migration distances as straight-line distances ignoring water barriers and human development. In sensitivity analyses, we investigated different time periods of 2 × CO2 climate forcing, more narrowly defined biomes and barriers because of water bodies and human development. Results In the base-case calculations, average migration rates varied significantly according to the GVM used (BIOME3 vs. MAPSS), the age of the GCM (older- vs. newer-generation GCMs), and whether or not GCMs included sulphate cooling or CO2 fertilization effects. However, high migration rates (, 1000 m year,1) were relatively common in all models, consisting on average of 17% grid cells for BIOME3 and 21% for MAPSS. Migration rates were much higher in boreal and temperate biomes than in tropical biomes. Doubling of the time period of 2 × CO2 forcing reduced these areas of high migration rates to c. 12% of grid cells for both BIOME3 and MAPSS. However, to obtain migration rates in the Boreal biome that were similar in magnitude to those observed for spruce when it followed the retreating North American Glacier, a radical increase in the period of warming was required, from 100 to >1000 years. A reduction in biome area by an order of magnitude increased migration rates by one to three orders of magnitude, depending on the GVM. Large water bodies and human development had regionally important effects in increasing migration rates. Main conclusions In conclusion, evidence from coupled GCMs and GVMs suggests that global warming may require migration rates much faster than those observed during post-glacial times and hence has the potential to reduce biodiversity by selecting for highly mobile and opportunistic species. Several poorly understood factors that are expected to influence the magnitude of any such reduction are discussed, including intrinsic migrational capabilities, barriers to migration, the role of outlier populations in increasing migration rates, the role of climate in setting range limits and variation in species range sizes.     Octahedral tilt twinning and compositional modulation in NaLaMgWO6

    A combination of selected-area electron diffraction (SAED), neutron powder diffraction (NPD) and high-resolution transmission electron microscopy (HRTEM) reveals a complex superstructure in the ordered perovskite NaLaMgWO6. Through indexing of SAED patterns the unit-cell dimensions are found to be 46.8 × 7.8 × 7.9,Å, which corresponds to a 12ap× 2ap× 2ap superstructure of the simple perovskite unit cell. HRTEM images reveal the formation of an unmistakable stripe contrast that repeats with the same periodicity. Doubling of the b and c axes is brought about by a combination of layered ordering of Na and La, rock-salt ordering of Mg and W, and octahedral tilting. The a axis repeat distance results from a one-dimensional twinning of the octahedral tilts in combination with a compositional modulation. Modeling of the NPD pattern shows that the underlying tilt system is a,a,c0 with tilt angles of ,,8° about the a and b axes. The octahedral tilt-twin boundaries run perpendicular to the a axis and are separated by 6ap. Simulated HRTEM images show that octahedral tilt twinning alone cannot explain the stripes seen in the HRTEM images, rather a compositional modulation involving the A -site cations is necessary to explain the experimental images.     REFORM OF THE PERSONAL INCOME TAX SYSTEM IN AUSTRALIA

    This paper examines the case for reform of Australia's Personal Income Tax (PIT), argues that it is outdated, and demonstrates a growing consensus for reform. The importance of tax avoidance, particularly the use of trusts, in the Australian PIT system, and arguably its abrogation of modern-day criteria of what constitutes a ,good tax', is emphasised. Three possible ,reform' options are identified: the ,tinkering and tokenism' approach of current Government policy; moderate reform and a proposed ,significant reform option' costing around $22 billion. Essentially this comprises company and top PIT rate equalisation and a doubling of the tax-free threshold. But funding this is problematical. Two key arguments of the paper are that: (real) simplification i.e. lower compliance costs, is an important yet usually down-played objective in reform proposals; savings from reform denying PIT deductions such as work expenses are insufficient to achieve significant PIT reform. An increase in the rate of the Goods and Services Tax (GST) from 10% to 15% (with a compensation package) is therefore advocated in a revenue-neutral analysis ignoring current Government budget surpluses. The overall outcome would be a simplified, more equitable and incentive-driven PIT system that would move Australia closer to the PIT and GST policies of other OECD countries. But the political difficulties of reform mean that the Government's ,tinkering' approach is likely to continue. [source]


    Enhancing the Porosity of Mesoporous Carbon-Templated ZSM-5 by Desilication

    Abstract A tunable desilication protocol applied on a mesoporous ZSM-5 zeolite synthesized by carbon-templating is reported. The strategy enables a systematic manufacture of zeolite catalysts with moderate to very high mesoporosities. Coupling carbon-templating and desilication thus allow for more than a doubling of the original mesopore volume and mesopore surface area. The porosity effect arising from various treatment times and base amounts in the media has beenthoroughly mapped. Initially, small mesopores are created, and as desilication strength increases the average mesopore size enhances.     Induction of bilateral plasticity in sensory cortical maps by small unilateral cortical infarcts in rats

    Abstract Behavioural impairments caused by brain lesions show a considerable, though often incomplete, recovery. It is hypothesized that cortical and subcortical plasticity of sensory representations contribute to this recovery. In the hindpaw representation of somatosensory cortex of adult rats we investigated the effects of focal unilateral cortical lesions on remote areas. Cortical lesions with a diameter of ,,2 mm were induced in the parietal cortex by photothrombosis with the photosensitive dye Rose Bengal. Subsequently, animals were kept in standard cages for 7 days. On day seven, animals were anaesthetized and cutaneous receptive fields in the cortical hindpaw representations ipsi- and contralateral to the lesion were constructed from extracellular recordings of neurons in layer IV using glass microelectrodes. Receptive fields in the lesioned animals were compared to receptive fields measured in nonlesioned animals serving as controls. Quantitative analysis of receptive fields revealed a significant increase in size in the lesioned animals. This doubling in receptive field size was observed equally in the hemispheres ipsi- and contralateral to the lesion. The results indicate that the functional consequences of restricted cortical lesions are not limited to the area surrounding the lesion, but affect the cortical maps on the contralateral, nonlesioned hemisphere.     PHENOTYPIC PLASTICITY OF HERMAPHRODITE SEX ALLOCATION PROMOTES THE EVOLUTION OF SEPARATE SEXES: AN EXPERIMENTAL TEST OF THE SEX-DIFFERENTIAL PLASTICITY HYPOTHESIS USING SAGITTARIA LATIFOLIA (ALISMATACEAE)

    Separate sexes can evolve under nuclear inheritance when unisexuals have more than twice the reproductive fitness of hermaphrodites through one sex function (e.g., when females have more than twice the seed fertility of hermaphrodites). Because separate sexes are thought to evolve most commonly via a gynodioecious intermediate (i.e., populations in which females and hermaphrodites cooccur), the conditions under which females can become established in populations of hermaphrodites are of considerable interest. It has been proposed that resource-poor conditions could promote the establishment of females if hermaphrodites are plastic in their sex allocation and allocate fewer resources to seed production under these conditions. If this occurs, the seed fertility of females could exceed the doubling required for the evolution of unisexuality under low-, but not high-resource conditions (the sex-differential plasticity hypothesis). We tested this hypothesis using replicate experimental arrays of the aquatic herb Sagittaria latifolia grown under two fertilizer treatments. The results supported the sex-differential plasticity hypothesis, with females having more than twice the seed fertility of hermaphrodites under low-, but not high-fertilizer conditions. Our findings are consistent with the idea that separate sexes are more likely to evolve under unfavorable conditions. [source]


    Keeping up or Falling behind?

    D31; I38 Abstract Each year, the government decides how much to raise benefits and tax allowances. In the UK, the basis for these upratings is rarely debated, yet has major long-term consequences for the relative living standards of different groups as well as for the public finances. This paper considers the medium-term implications of present uprating policies, which vary across parameters of the tax,benefit system. Continuing these policies for 20 years, other things staying the same, would result in a near doubling of the child poverty rate alongside a substantial gain to the public finances. At the same time, pensioners are largely protected by the earnings indexation of pensioner benefits including, in time, the basic state pension. We show how difficult it will be to meet the UK child poverty targets unless the greater inequality inherent in the current regime for uprating payments and allowances is redressed. [source]


    The effect of a closed area and beach seine exclusion on coral reef fish catches

    Fish landing data from the Mombasa Marine National Park (MNP) and a marine reserve exploited by various gears were studied over a 5-yr period to determine the influence of the closed area and different gears in fisheries. The number fishing and boats per landing site was constant, but total and catch per unit effort progressively declined in all sites on an annual basis irrespective of the existence of a marine reserve, exclusion of the beach seines or use of gear. Differences between landing sites were most pronounced when analysed on a catch per area as opposed to the more standard catch per fisherman, suggesting compensation in human effort when catches decline. A marine reserve next to a closed area that excluded beach seines had the highest catch per area (5.5 kg ha,1 month,1) despite having the highest density of fishermen (0.07 ± 0.02 fishermen ha,1 month,1). The annual rate of decline in the catch was lower than the other sites at around 250 g day,1 compared with 310,400 g day,1 in the other sites. One landing site, which excluded beach seine landings for more than 20 yrs, had a high catch per area (,5.3 kg ha,1 month,1), but after experiencing a doubling in the effort of other gears (line, speargun and trap), the catch per fisherman and area were reduced. Environmental or habitat degradation and excessive effort remain the most likely explanation for the overall declines in catch from 1995 to 1999. Closed areas and beach seine exclusion have the potential to increase catch rates, but the first often reduces the total fishing area and possibly leads to a loss of total catch, at least on a time scale of less than 10 yrs. The exclusion of beach seines can lead to an increase in other gear types that can also cause reductions in catch. [source]


    Prostitution as a Male Object of Epistemological Pain

    It is not, in the first place, prostitutes' physical and psychological pain that is examined in this article, but the pain encountered in trying to come to terms with the studying of prostitution. Prostitution upsets consciousness' efforts and confuses its epistemes of representation. It reveals issues of (male) avoidance and over-rationalization that apply just as well to how business and organization are (not) studied, as to prostitution. Following Artaud, we examine how, because prostitution is both consciousness (idea, theory, representation) and body (sex, body, the physiology of the brain), it poses the problem of doubling. How can one apprehend both: (i) that one is the physical hyle (materiality) of thought and also (ii) remain aware of the contents of consciousness? Artaud claimed that only in ,cruelty' and the ,scream' could the mind and body be grasped at once. By contrast, Derrida proposes via the subjectile to glide over the space between consciousness and body, trying to acknowledge but not be stymied by the double. Finally, we turn to Irigaray who has accepted doubling and has made it epistemologically productive. [source]


    Organic Photovoltaic Cells Based On Solvent-Annealed, Textured Titanyl Phthalocyanine/C60 Heterojunctions

    Abstract Organic photovoltaic cells (OPV) with good near-IR photoactivity are created from highly textured titanyl phthalocyanine (TiOPc)/C60 heterojunctions. Vacuum deposited TiOPc thin films are converted to the near-IR absorbing "Phase II" polymorph using post-deposition solvent annealing. The Phase I,,,Phase II transition broadens the absorbance spectrum of the Pc film producing absorptivities (,,,,105,cm,1) from 600,900,nm, along with substantial texturing of the Pc layer. Atomic force microscopy and field-emission scanning electron microscopy of the solvent annealed films show that the surface roughness of the Pc layers is increased by a factor of greater than 2× as a result of the phase transformation. Current,voltage (J,V) responses for white light illumination of ITO (100,nm)/TiOPc (20,nm)/C60 (40,nm)/BCP (10,nm)/Al (100,nm) OPVs show a near doubling of the short-circuit photocurrent (JSC), with only a small decrease in open-circuit photopotential (VOC), and a concomitant increase in power conversion efficiency. Incident photon current efficiency (IPCE) plots confirmed the enhanced near-IR OPV activity, with maximum IPCE values of ca. 30% for devices using Phase II-only TiOPc films. UV-photoelectron spectroscopy (UPS) of TiOPc/C60 heterojunctions, for both Phase I and Phase II TiOPc films, suggest that the Phase II polymorph has nearly the same HOMO energy as seen in the Phase I polymorph, and similar frontier orbital energy offsets, EHOMOPc,ELUMOC60, leading to comparable open-circuit photovoltages. These studies suggest new strategies for the formation of higher efficiency OPVs using processing conditions which lead to enhance near-IR absorptivities, and extensive texturing of crystalline donor or acceptor films.     Photosensitization of TiO2 Nanostructures with CdS Quantum Dots: Particulate versus Tubular Support Architectures

    Abstract TiO2 nanotube arrays and particulate films are modified with CdS quantum dots with an aim to tune the response of the photoelectrochemical cell in the visible region. The method of successive ionic layer adsorption and reaction facilitates size control of CdS quantum dots. These CdS nanocrystals, upon excitation with visible light, inject electrons into the TiO2 nanotubes and particles and thus enable their use as photosensitive electrodes. Maximum incident photon to charge carrier efficiency (IPCE) values of 55% and 26% are observed for CdS sensitized TiO2 nanotube and nanoparticulate architectures respectively. The nearly doubling of IPCE observed with the TiO2 nanotube architecture is attributed to the increased efficiency of charge separation and transport of electrons. [source]


    The European carbon balance.

    Abstract We estimated the long-term carbon balance [net biome production (NBP)] of European (EU-25) croplands and its component fluxes, over the last two decades. Net primary production (NPP) estimates, from different data sources ranged between 490 and 846 gC m,2 yr,1, and mostly reflect uncertainties in allocation, and in cropland area when using yield statistics. Inventories of soil C change over arable lands may be the most reliable source of information on NBP, but inventories lack full and harmonized coverage of EU-25. From a compilation of inventories we infer a mean loss of soil C amounting to 17 g m,2 yr,1. In addition, three process-based models, driven by historical climate and evolving agricultural technology, estimate a small sink of 15 g C m,2 yr,1 or a small source of 7.6 g C m,2 yr,1. Neither the soil C inventory data, nor the process model results support the previous European-scale NBP estimate by Janssens and colleagues of a large soil C loss of 90 ± 50 gC m,2 yr,1. Discrepancy between measured and modeled NBP is caused by erosion which is not inventoried, and the burning of harvest residues which is not modeled. When correcting the inventory NBP for the erosion flux, and the modeled NBP for agricultural fire losses, the discrepancy is reduced, and cropland NBP ranges between ,8.3 ± 13 and ,13 ± 33 g C m,2 yr,1 from the mean of the models and inventories, respectively. The mean nitrous oxide (N2O) flux estimates ranges between 32 and 37 g C Eq m,2 yr,1, which nearly doubles the CO2 losses. European croplands act as small CH4 sink of 3.3 g C Eq m,2 yr,1. Considering ecosystem CO2, N2O and CH4 fluxes provides for the net greenhouse gas balance a net source of 42,47 g C Eq m,2 yr,1. Intensifying agriculture in Eastern Europe to the same level Western Europe amounts is expected to result in a near doubling of the N2O emissions in Eastern Europe. N2O emissions will then become the main source of concern for the impact of European agriculture on climate. [source]


    Impacts of climate change on fire activity and fire management in the circumboreal forest

    Abstract Forest fires are a significant and natural element of the circumboreal forest. Fire activity is strongly linked to weather, and increased fire activity due to climate change is anticipated or arguably has already occurred. Recent studies suggest a doubling of area burned along with a 50% increase in fire occurrence in parts of the circumboreal by the end of this century. Fire management agencies' ability to cope with these increases in fire activity is limited, as these organizations operate with a narrow margin between success and failure; a disproportionate number of fires may escape initial attack under a warmer climate, resulting in an increase in area burned that will be much greater than the corresponding increase in fire weather severity. There may be only a decade or two before increased fire activity means fire management agencies cannot maintain their current levels of effectiveness. [source]


    Assessing forest growth across southwestern Oregon under a range of current and future global change scenarios using a process model, 3-PG

    Summary With improvements in mapping regional distributions of vegetation using satellite-derived information, there is an increasing interest in the assessment of current limitations on forest growth and in making projections of how productivity may be altered in response to changing climatic conditions and management policies. We utilised a simplified physiologically based process model (3-PG) across a 54 000 km2 mountainous region of southwestern Oregon, USA, to evaluate the degree to which maximum periodic mean annual increment (PAI) of forests could be predicted at a set of 448 forest inventory plots. The survey data were pooled into six broad forest types (coastal rain forest, interior coast range forest, mixed conifer, dry-site Douglas-fir, subalpine forest, and pine forest) and compared to the 3-PG predictions at a spatial resolution of 1 km2. We found good agreement (r2 = 0.84) between mean PAI values of forest productivity for the six forest types with those obtained from field surveys. With confidence at this broader level of integration, we then ran model simulations to evaluate the constraints imposed by (i) soil fertility under current climatic conditions, (ii) the effect of doubling monthly precipitation across the region, and (iii) a widely used climatic change scenario that involves modifications in monthly mean temperatures and precipitation, as well as a doubling in atmospheric CO2 concentrations. These analyses showed that optimum soil fertility would more than double growth, with the greatest response in the subalpine type and the least increase in the coastal rain forests. Doubling the precipitation increased productivity in the pine type (> 50%) with reduced responses elsewhere. The climate change scenario with doubled atmospheric CO2 increased growth by 50% on average across all forest types, primarily as a result of a projected 33% increase in photosynthetic capacity. This modelling exercise indicates that, at a regional scale, a general relationship exists between simulated maximum leaf area index and maximum aboveground growth, supporting the contention that satellite-derived estimates of leaf area index may be good measures of the potential productivity of temperate evergreen forests.     Salvage laryngectomy and pharyngocutaneous fistulae after primary radiotherapy for head and neck cancer: A national survey from DAHANCA

    Objective. In 1998, the Danish Society for Head and Neck Oncology decided to conduct a nationwide survey at the five head and neck oncology centers with the aim of evaluating the surgical outcome of salvage laryngectomy after radiotherapy with special emphasis on identifying factors that could contribute to the development of pharyngocutaneous fistulae. Patients. A total of 472 consecutive patients undergoing postirradiation salvage laryngectomy in the period July 1, 1987,June 30, 1997 were recorded at the five head and neck oncology centers in Denmark. Age ranged from 36 to 84 years, median 63 years, 405 men and 67 women. Primary tumor site was glottic larynx (n = 242), supraglottic larynx (n = 149), other larynx (n = 45), pharynx (n = 27), and other (n = 9). All patients had received prior radiotherapy. Results. Median time between radiotherapy and laryngectomy was 10 months (range, 1,348 months). A total of 89 fistulae lasting at least 2 weeks were observed, corresponding to an overall average fistulae risk of 19%. The number of performed laryngectomies per year decreased linearly (from 58 to 37), whereas the annual number of fistulae increased slightly (from 7 to 11), which meant that the corresponding estimated fistulae risk increased significantly from 12% in 1987 to 30% in 1997. Other significant risk factors for fistulae in univariate analysis included younger patient age, primary advanced T and N stage, nonglottic primary site, resection of hyoid bone, high total radiation dose, and large radiation fields. Multiple logistic regression analysis of these parameters suggested that nonglottic tumor site, late laryngectomy period (1987,1992 vs 1993,1997), and advanced initial T stage were independent prognostic factors for fistulae risk. Surgical parameters like resection of thyroid/tongue base/trachea or radiotherapy parameters like overall treatment time or fractions per week did not influence fistulae risk. Conclusions. The risk of fistulae is especially high in patients initially treated with radiotherapy for nonglottic advanced stage tumors. A significant decrease in the number of performed salvage laryngectomies over the 10 years was seen. Over the same time period, the annual number of fistulae remained almost constant.     A robust algorithm for configurational-force-driven brittle crack propagation with R-adaptive mesh alignment

    Abstract The paper considers a variational formulation of brittle fracture in elastic solids and proposes a numerical implementation by a finite element method. On the theoretical side, we outline a consistent thermodynamic framework for crack propagation in an elastic solid. It is shown that both the elastic equilibrium response as well as the local crack evolution follow in a natural format by exploitation of a global Clausius,Planck inequality in the sense of Coleman's method. Here, the canonical direction of the crack propagation associated with the classical Griffith criterion is the direction of the material configurational force which maximizes the local dissipation at the crack tip and minimizes the incremental energy release. On the numerical side, we exploit this variational structure in terms of crack-driving configurational forces. First, a standard finite element discretization in space yields a discrete formulation of the global dissipation in terms configurational nodal forces. As a consequence, the constitutive setting of crack propagation in the space-discretized finite element context is naturally related to discrete nodes of a typical finite element mesh. Next, consistent with the node-based setting, the discretization of the evolving crack discontinuity is performed by the doubling of critical nodes and interface segments of the mesh. Critical for the success of this procedure is its embedding into an r-adaptive crack-segment reorientation procedure with configurational-force-based directional indicator. Here, successive crack releases appear in discrete steps associated with the given space discretization. These are performed by a staggered loading,release algorithm of energy minimization at frozen crack state followed by the successive crack releases at frozen deformation. This constitutes a sequence of positive-definite discrete subproblems with successively decreasing overall stiffness, providing an extremely robust algorithmic setting in the postcritical range.     Simulated changes in active/break spells during the Indian summer monsoon due to enhanced CO2 concentrations: assessment from selected coupled atmosphere,ocean global climate models

    Abstract The simulations by ten coupled GCMs under the Intergovernmental Panel on Climate Change Assessment Report-4 are used to study the implication of possible global climate change on active/break spells of the Indian summer monsoon (ISM). The validation of the mean daily cycle of the summer monsoon precipitation over the Indian core region and the spatial pattern of the ISM precipitation climatology with observation suggest that six models simulate fairly well, whereas four models differ from observation. Thus, the identification of active/break spells is confined to six models. The sensitivity to climate change has been assessed from two experiments, namely, 1% per year CO2 increase to doubling and 1% per year CO2 increase to quadrupling. The changes in the daily mean cycle and the standard deviation of precipitation, frequency, and duration of active/break spells in future climate change are uncertain among the models and at times among two experiments.     Enhanced resolution modelling study on anthropogenic climate change: changes in extremes of the hydrological cycle

    Abstract Changes in variability and extremes of the hydrological cycle are studied in two 30 year simulations using a general circulation model at high horizontal resolution. The simulations represent the present-day climate and a period in which the radiative forcing corresponds to a doubling of the present-day concentrations of atmospheric greenhouse gases. In most regions and seasons the probability density function of daily precipitation experiences a stretching associated with a higher probability of heavy precipitation events in the warmer climate. Whereas extremely long wet spells show only moderate changes, the extremely long dry spells are extended at middle latitudes over most land areas. At high latitudes the changes in annual maximum river runoff are mainly controlled by changes in snow budget. Eight out of 14 selected major rivers show a statistically significant change in 10 year return values of the annual maximum discharge.     Improving cellular function through modulation of energy metabolism

    The ambivalent consequences of mitochondrial stimulation on cellular activity have been well established. Mitochondria supply the cell with energy through a process of oxidative phosphorylation but thereby generate free radicals, resulting in the accumulation of hydrogen peroxide in the cytoplasm. We have investigated the impact of cellular senescence as well as UV irradiation, on the balance between these two activities. The adenosine triphosphate (ATP) level, DNA and protein synthesis in fibroblasts obtained from donors between 30 and 90 years of age appeared to be significantly influenced by the aging process. Both DNA and protein synthesis could be stimulated by increasing intracellular ATP levels. In-vitro senescent fibroblasts showed a reduction in the level of ATP as well as a shift in mitochondrial membrane potential. At the same time, there was an increase in intracellular hydrogen peroxide with increasing population doubling, indicating a clear dysfunction of the metabolic machinery in the mitochondria of senescent cells. To counteract this degradation of the energy pool, we treated cells with creatine, which is known to restore the pool of phosphocreatine in the mitochondria. Creatine treatment significantly increased cell survival after UV exposure, stimulated the repair of UVB-induced DNA damage in keratinocytes and caused a significant reduction in the number of sunburn cells in a UVB-exposed reconstituted skin model. These results clearly indicate that restoration of the energy pool in mitochondria increased cellular self-defense mechanism. These data show the important role played by the mitochondrial energy metabolism on the aging process, and indicate a possible therapy that can be used to counteract this negative effect. Treatment with creatine seems to provide the necessary boost to the cellular metabolism, which leads to an induction of a significant amount of protection and repair to human skin cells. [source]


    Abundance and clonal replication in the tropical corallimorpharian Rhodactis rhodostoma

    Abstract. The corallimorpharian Rhodactis rhodostoma appears to be an opportunistic species capable of rapidly monopolizing patches of unoccupied shallow substrate on tropical reefs. On a fringing coral reef at Eilat, Israel, northern Red Sea, we examined patterns of abundance and clonal replication in R. rhodostoma in order to understand the modes and rates of spread of polyps across the reef flat. Polyps were abundant on the inner reef flat (maximum 1510 polyps m,2 and 69% cover), rare on the outer reef flat, and completely absent on the outer reef slope at >3 m depth. Individuals cloned throughout the year via 3 distinct modes: longitudinal fission, inverse budding, and marginal budding. Marginal budding is a replicative mode not previously described. Cloning mode varied significantly with polyp size. Approximately 9% of polyps cloned each month, leading to a clonal doubling time of about 1 year. The rate of cloning varied seasonally and depended on day length and seawater temperature, except for a brief reduction in cloning during midsummer when polyps spawned gametes. Polyps of R. rhodostoma appear to have replicated extensively following a catastrophic low-tide disturbance in 1970, and have become an alternate dominant to stony corals on parts of the reef flat.     Climate change and vector-borne viral diseases potentially transmitted by transfusion

    Vector-borne diseases occur when infectious agents (virus, protozoa, bacteria, or helminthes) are transmitted to their hosts by a carrier organism. Climate conditions and their changes play a role in the inter-relationship between these agents, the vectors and the host (or hosts). This review is focused on arthropod-borne viruses (Arboviruses). These viruses are transmitted between susceptible vertebrate hosts by blood-feeding arthropods, and may be transmitted by blood transfusion, tissue and organ transplantation and breast feeding. The lifecycle of arboviruses is influenced by changes in temperature, rainfall, humidity, length of day, average daily solar radiation and/or storm patterns, as well as changes in the frequency of rare events such as floods or droughts. A plethora of studies have suggested that climate changes, particularly temperature changes, are likely to be induced by increase in the amount of greenhouse gases, such as methane, carbon dioxide (CO2) and chlorofluorocarbons, which deplete ozone in the atmosphere leading to an increase in ultraviolet radiation. Current models predict that ambient temperature will increase by 3,5°C on average with a doubling in CO2 concentration in the atmosphere. Vectors, pathogens and hosts each survive and reproduce within a range of optimal climatic conditions: temperature and precipitation being most important, while sea level elevation, wind and daylight duration are also important. Climate changes may affect important determinants of vector-borne disease transmission including (i) vector survival and reproduction, (ii) the vector's biting rate, and (iii) the pathogen's incubation rate within the vector organism. Droughts can increase the dissemination of arboviral diseases in urban areas by allowing a boost in the population of mosquitoes in foul water concentrated in catch basins where they breed. Furthermore, eggs can be vertically infected with arboviruses and heat waves speed up the maturation of the mosquitoes and of the viruses within mosquitoes. Droughts also cause a decline in mosquito predators like frogs, darning needles and dragonflies. In addition, birds congregate around shrinking water sites, enhancing circulation of viruses among birds and mosquitoes. In conclusion, the seriousness of some of the recent epidemics like West Nile virus and Dengue appear to has been influenced by climate change. As most of the arboviral infections are asymptomatic in humans, there is an increased opportunity for blood, organ and tissue donations by infected individuals during the viraemic period, resulting in an increased risk of transmission of arboviruses.     Hospital Charges Attributable to a Primary Diagnosis of Infectious Diseases in Older Adults in the United States, 1998 to 2004

    OBJECTIVES: To describe total and average hospital charges associated with infectious disease (ID) hospitalizations and specific ID categories and to estimate ID hospitalization rates in adults aged 65 and older in the United States from 1998 through 2004. DESIGN: Retrospective analysis of hospital discharge data obtained from the Nationwide Inpatient Sample for 1998 through 2004. SETTING: United States. PATIENTS: Older adults hospitalized in the United States from 1998 through 2004. MEASUREMENTS: Hospital charges and hospitalization rates for IDs described according to year, age group, sex, U.S. Census region, and ID category. Charges for non-ID hospitalizations were also described. Hospital charges were adjusted for inflation. RESULTS: From 1998 through 2004, total charges for ID hospitalizations exceeded $261 billion and accounted for 13% of all hospital charges for older adults. Total charges for ID hospitalizations increased from $31.4 billion in 1998 to $45.7 billion in 2004. The average annual ID hospital charge was lower than the average annual non-ID hospital charge during the study period ($21,342 vs $22,787, P<.001). The average annual rate for ID hospitalizations was 503 per 10,000 older adults, which remained stable during the study period. CONCLUSION: The total charges for ID hospitalizations and for all hospitalizations in older adults in the United States increased 45% and nearly 40%, respectively, during the 7-year study period, whereas the population of older adults grew by only 5%. Sustained increases of such magnitude will have major implications for the U.S. healthcare system as it prepares for the more than doubling of the older U.S. adult population during the first 30 years of this century.     Neuroimaging of cortical development and brain connectivity in human newborns and animal models

    Abstract Significant human brain growth occurs during the third trimester, with a doubling of whole brain volume and a fourfold increase of cortical gray matter volume. This is also the time period during which cortical folding and gyrification take place. Conditions such as intrauterine growth restriction, prematurity and cerebral white matter injury have been shown to affect brain growth including specific structures such as the hippocampus, with subsequent potentially permanent functional consequences. The use of 3D magnetic resonance imaging (MRI) and dedicated postprocessing tools to measure brain tissue volumes (cerebral cortical gray matter, white matter), surface and sulcation index can elucidate phenotypes associated with early behavior development. The use of diffusion tensor imaging can further help in assessing microstructural changes within the cerebral white matter and the establishment of brain connectivity. Finally, the use of functional MRI and resting-state functional MRI connectivity allows exploration of the impact of adverse conditions on functional brain connectivity in vivo. Results from studies using these methods have for the first time illustrated the structural impact of antenatal conditions and neonatal intensive care on the functional brain deficits observed after premature birth. In order to study the pathophysiology of these adverse conditions, MRI has also been used in conjunction with histology in animal models of injury in the immature brain. Understanding the histological substrate of brain injury seen on MRI provides new insights into the immature brain, mechanisms of injury and their imaging phenotype. [source]


    Alterations in renal cilium length during transient complete ureteral obstruction in the mouse

    Abstract The renal cilium is a non-motile sensory organelle that has been implicated in the control of epithelial phenotype in the kidney. The contribution of renal cilium defects to cystic kidney disease has been the subject of intense study. However, very little is known of the behaviour of this organelle during renal injury and repair. Here we investigate the distribution and dimensions of renal cilia in a mouse model of unilateral ureteral obstruction and reversal of ureteral obstruction. An approximate doubling in the length of renal cilia was observed throughout the nephron and collecting duct of the kidney after 10 days of unilateral ureteral obstruction. A normalization of cilium length was observed during the resolution of renal injury that occurs following the release of ureteral obstruction. Thus variations in the length of the renal cilium appear to be a previously unappreciated indicator of the status of renal injury and repair. Furthermore, increased cilium length following renal injury has implications for the specification of epithelial phenotype during repair of the renal tubule and duct. [source]


    Testing liana cutting and controlled burning as silvicultural treatments for a logged forest in the eastern Amazon

    Summary 1In the eastern Brazilian Amazon, logged forests frequently include patches where liana density is particularly high. In these so-called liana tangles, competition from lianas is predicted to reduce tree growth significantly, thus effectively impeding future timber production. To begin to develop a silvicultural strategy for these patches, the impact of liana cutting and controlled burning on liana density, tree growth and tree regeneration in liana-dominated patches was investigated in a logged forest in the eastern Brazilian Amazon. 2The two treatments (liana cutting and controlled burning) and a control were installed in 40 × 40-m plots in a randomized complete block design of six blocks. Treatments were conducted during October 1997, and tree diameter growth and mortality, canopy cover, regeneration and liana density were monitored over 2 years. 3Mean mortality following burning was significantly higher for lianas (79%) than for trees (48%), as was the mean coppicing rate of top-killed stems (42% for lianas vs. 20% for trees). Coppicing combined with some recruitment from seed resulted in liana densities in the burned plots returning to 70% of the values in the control plots only 2 years post-treatment. 4Canopy light transmittance, estimated from hemispherical canopy photographs taken at 1 m above the ground, increased significantly from c. 4% in controls to 8% in cut and 12% in burned treatments, and these differences persisted over the 2-year study period. 5In the absence of silvicultural intervention, mean tree diameter increments were low (1·3 mm year,1), suggesting that the successional transition to higher stature forest was occurring very slowly. Each of the treatments resulted in a more than doubling of mean annual tree growth (3 and 2·8 mm year,1 for liana-cut and burned treatments, respectively). The treatments also significantly reduced the occurrence of trees that showed no growth over the study period, from 56% in controls to 30% in cut and 32% in burned treatments. 6The results of this study suggest that although burning resulted in increased tree growth, rapid recolonization of surviving trees by lianas and the high vulnerability of burned stands to unwanted repeat burns are likely to cancel out any of the possible benefits of controlled burning as a silvicultural treatment for liana-dominated patches. Liana cutting, on the other hand, showed promise and its adoption as part of a larger strategy for the recuperation of the timber production potential of logged tropical forests seems warranted.     Treatment of Idiopathic Hyperphosphatasia With Intensive Bisphosphonate Therapy

    Abstract In a family with IH, a rare high turnover bone disease, two older siblings were wheelchair-bound with severe skeletal deformity by age 15. Their youngest affected sibling was treated intensively with intravenous bisphosphonates for 3 years. The treatment was well tolerated and prevented the development of deformity and disability. Introduction: Idiopathic hyperphosphatasia (IH, also known as juvenile Paget's disease) is a rare genetic bone disease characterized by very high bone turnover and progressive bony deformity. Inhibitors of bone resorption have been used to suppress bone turnover in the short term, but there is no published data on long-term efficacy. Materials and Methods: An 11-year-old girl with IH, who had two severely affected older siblings, presented with progressive deformity and deafness and long bone fractures. Conventional pediatric doses of pamidronate had failed to prevent clinical deterioration or suppress bone turnover completely. Intensive bisphosphonate therapy (frequent 5-mg ibandronate infusions) was given to try and arrest progression of the skeletal disease. Growth and development, pure tone audiometry, biochemistry, radiology, densitometry (DXA), and bone histology were monitored. Results: A total of 45 mg ibandronate was given over 3 years until skeletal maturity was reached (20, 15, and 10 mg for years 1,3, respectively). Ibandronate treatment was well tolerated, and biochemical markers of bone turnover suppressed to within the age-appropriate normal range There was some progression of her thoracic kyphosis, but she had no further fractures and remained mobile and active at an age when her siblings had become wheelchair-bound. A significant recovery of hearing (p < 0.01) was documented, particularly at low frequencies. Radiographs showed improvement in spinal osteoporosis and cortical bone dimensions and arrest of progressive acetabular protrusion. Areal bone density increased substantially (lumbar spine z-score from ,2.2 to + 1.8). Tetracycline-labeled bone biopsy specimens were taken before and after 18 months of intensive treatment. The second biopsy showed suppression of bone turnover and a doubling of trabecular thickness, with no mineralization defect, and no osteopetrosis. Conclusions: Intensive bisphosphonate treatment prevented the development of deformity and disability and improved hearing in this child with IH. The dose of bisphosphonate, which is substantially greater than is usually used in pediatric bone disease, had no adverse effects, in particular on bone mineralization. [source]


    A Cost-Effectiveness Analysis of Angiotensin-Converting Enzyme Inhibitors and Angiotensin Receptor Blockers in Diabetic Nephropathy

    The aim of this study was to estimate the cost-effectiveness of renin-angiotensin-aldosterone system blockers in patients with diabetic nephropathy. A cost-effectiveness analysis was performed based on a meta-analysis of studies investigating the effect of angiotensin-converting enzyme (ACE) inhibitors and angiotensin receptor blockers (ARBs) as part of a treatment regimen on the incidence of end-stage renal disease (ESRD) in patients with diabetic nephropathy. The primary outcome was the cost to prevent 1 patient from developing ESRD. Cost analysis was performed from a third-party payer perspective in 2006 US dollars. As part of a treatment regimen, ARBs significantly reduced the incidence of ESRD and doubling of serum creatinine concentration (P<.05) but not total mortality. The cost to prevent 1 patient from developing ESRD was $31,729 (95% confidence interval, $19,443,$85,442; P<.01), $189,190 (P=.13) and $51,585 (P=.068) for patients receiving ARBs, ACE inhibitors, or either of them, respectively. This study demonstrates that blocking the RAAS, which delays the progression to ESRD, appears to be cost-effective. The current analysis favors ARBs in terms of cost-effectiveness. [source]


    ACE Inhibitors and Protection Against Kidney Disease Progression in Patients With Type 2 Diabetes: What's the Evidence?

    Although angiotensin-converting enzyme inhibitors are frequently used as antihypertensive agents to lower blood pressure and slow progression of nephropathy in patients with type 2 diabetes, evidence of their efficacy has been drawn primarily from small trials with surrogate end points. No adequately powered, long-term trials have tested their effects to reduce the incidence of hard end points, such as progression to end-stage renal disease or even doubling of serum creatinine in the population of patients with nephropathy from type 2 diabetes. While the results of angiotensin-converting enzyme inhibitor trials from nondiabetic causes and even type 1 diabetes may be extrapolated to the patient with nephropathy associated with type 2 diabetes, the hard evidence is not available. This review critically evaluates the limited evidence in support of angiotensin-converting enzyme inhibitors as renal-protective agents in people with type 2 diabetes. [source]


    Conferencing in schools: punishment, restorative justice, and the productive importance of the process of conversation,

    Abstract During the decade to 1999 in New Zealand, schools experienced more than doubling of the rates of suspension and exclusion. The higher suspension rates of Maori, males and in low decile schools were of particular concern. The Ministry of Education enacted a variety of responses to this situation, including encouraging the use of restorative conferencing in schools. This article builds on learning from the Restorative Conferencing in Schools Trial and shows how the process of a conversation can be productive of more and less peaceable relationships. It is argued that school communities are uniquely placed in civic life, and that it is worthwhile considering restorative conferencing and restorative practices more generally in relation to their behaviour management and disciplinary functions. Restorative practices are advanced from a social constructionist position, which is a very different philosophy from the philosophy of punishment and judgement that dominates in many schools.
