Selected AbstractsTreatment of Uncorrected Refractive Error Improves Vision-Specific Quality of LifeJOURNAL OF AMERICAN GERIATRICS SOCIETY, Issue 6 2006Anne L. Coleman MD OBJECTIVES: To evaluate the benefit of eyeglasses and magnifiers in elderly patients with uncorrected refractive errors. DESIGN: A single-center, randomized, prospective, controlled trial (September 2001 to August 2003). SETTING: Los Angeles County, California. PARTICIPANTS: One hundred thirty-one community-dwelling persons aged 65 and older who had habitual distance visual acuity of 20/32 or worse and whose distant visual acuity, near visual acuity, or both could be improved with eyeglasses, a magnifier, or both by two lines of acuity or more. INTERVENTION: Sixty-six were randomized to receive a prescription and voucher for free eyeglasses, a magnifier, or both immediately, and 65 were randomized to receive a prescription and voucher after the 3-month follow-up visit (the control group). MEASUREMENTS: Primary outcome was vision-specific functioning as measured using the 25-item National Eye Institute,Visual Functioning Questionnaire (NEI-VFQ). Secondary outcomes were distance and near visual acuity and overall functioning as measured using the Rosow-Breslau function questionnaire. RESULTS: In the intention-to-treat analysis of 3-month follow-up data, participants who received the eyeglasses prescription and voucher immediately had greater improvement in NEI-VFQ composite scores than the control group (P<.01). They also had greater improvement in perceptions of their general vision (P<.01), distance visual acuity (P=.03), near visual acuity (P=.04), and mental health (P=.02). CONCLUSION: Correction of uncorrected refractive error, one of the leading causes of visual impairment in older people, improved the vision-specific quality of life of community-dwelling older persons. [source] Evaluation of Melbourne Edge Test contrast sensitivity measures in the visually impairedOPHTHALMIC AND PHYSIOLOGICAL OPTICS, Issue 4 2005James S. Wolffsohn Abstract Aim:, Contrast sensitivity (CS) provides important information on visual function. This study aimed to assess differences in clinical expediency of the CS increment-matched new back-lit and original paper versions of the Melbourne Edge Test (MET) to determine the CS of the visually impaired. Methods:, The back-lit and paper MET were administered to 75 visually impaired subjects (28,97 years). Two versions of the back-lit MET acetates were used to match the CS increments with the paper-based MET. Measures of CS were repeated after 30 min and again in the presence of a focal light source directed onto the MET. Visual acuity was measured with a Bailey,Lovie chart and subjects rated how much difficulty they had with face and vehicle recognition. Results:, The back-lit MET gave a significantly higher CS than the paper-based version (14.2 ± 4.1 dB vs 11.3 ± 4.3 dB, p < 0.001). A significantly higher reading resulted with repetition of the paper-based MET (by 1.0 ± 1.7 dB, p < 0.001), but this was not evident with the back-lit MET (by 0.1 ± 1.4 dB, p = 0.53). The MET readings were increased by a focal light source, in both the back-lit (by 0.3 ± 0.81, p < 0.01) and paper-based (1.2 ± 1.7, p < 0.001) versions. CS as measured by the back-lit and paper-based versions of the MET was significantly correlated to patients' perceived ability to recognise faces (r = 0.71, r = 0.85 respectively; p < 0.001) and vehicles (r = 0.67, r = 0.82 respectively; p < 0.001), and with distance visual acuity (both r = ,0.64; p < 0.001). Conclusions:, The CS increment-matched back-lit MET gives higher CS values than the old paper-based test by approximately 3 dB and is more repeatable and less affected by external light sources. Clinically, the MET score provides information on patient difficulties with visual tasks, such as recognising faces. [source] The effects of wearing corrective lenses for presbyopia on distance visionOPHTHALMIC AND PHYSIOLOGICAL OPTICS, Issue 1 2003Mona B. McGarry Abstract The possibility of patient behaviour concerning the wearing of reading spectacles influencing the magnitude and rate of change in distance vision has been hypothesised but not fully examined. This study examined the changes in distance vision over time of 674 presbyopic patients between the ages of 44 and 69 who came to their eye professional needing reading spectacles. Prior to this study, these patients had never worn spectacles. The majority of patients opted for full-eye (complete) reading spectacles and 150 patients selected bifocals. It was found that patients wearing complete reading spectacles eventually adapted to wearing their reading spectacles for intermediate or distance work. Through X2, it was found that there were significant differences in the reduction of distance visual acuity between groups based on the amount of time spent wearing reading spectacles for intermediate or distance work. The greater the number of hours spent wearing reading spectacles and viewing beyond their focal point, the greater the change in distance visual acuity. It also appeared to affect the degree of change in their prescription. Age was not significantly related. [source] Time-course and characteristic morphology of retinal changes following combination of verteporfin therapy and intravitreal triamcinolone in neovascular age-related macular degenerationACTA OPHTHALMOLOGICA, Issue 2 2010Shilla Lie Abstract. Purpose:, To identify characteristic morphological changes of the retina over time and the association with visual function after combined photodynamic therapy (PDT) and intravitreal triamcinolone (IVTA). Methods:, In this retrospective study, 40 patients (40 eyes) were treated with PDT and same-day IVTA. Optical coherence tomography (OCT), fluorescein angiography (FA) and evaluation of distance visual acuity (VA) were performed. The anatomical changes within intra- and subretinal compartments and their detailed analysis and grading were the main outcome measures. Results:, Intraretinal fluid (IRF) and subretinal fluid (SRF) by OCT decreased until 3 months (p < 0.01). At month 3, intraretinal cystoid spaces (ICS) had resolved or decreased in 84% of eyes, SRF in 58% and pigment epithelial detachment (PED) in 50%. Mean best-corrected VA (BCVA) improved significantly at month 1 (p < 0.01). Mean central retinal thickness (CRT) increased from 334 ,m at baseline to 439 ,m at day 1 (p = 0.03) before decreasing to 286 ,m at day 7 (p = 0.06), 233 ,m at month 1 (p = 0.001) and 255 ,m at month 3 (p = 0.001). Conclusion:, Combined verteporfin/IVTA therapy induces distinct time-related effects on the retina within the different intra- and subretinal compartments. [source] Improvement of reading speed after providing of low vision aids in patients with age-related macular degenerationACTA OPHTHALMOLOGICA, Issue 8 2009Nhung Xuan Nguyen Abstract. Purpose:, Age-related macular degeneration (AMD) is the most common cause of severe visual impairment, including loss of reading ability, among elderly persons in developed countries. The aim of the present study was to evaluate reading ability before and after providing of appropriate low vision aids. Methods:, Five hundred and thirty patients with different stages of AMD (age 82 ± 8 years) were included in this retrospective study. All patients underwent a standardized ophthalmological examination including evaluation of magnification requirement and careful providing of low vision aids. Before and after the provision of low vision aids, reading speed [words per minute (wpm)] was evaluated using standardized reading texts. Results:, For the whole group, the average best-corrected distance visual acuity of the better eye was 0.18 ± 0.15, with 69% of patients having visual acuity of 0.1 (20/200) or better. The mean magnification requirement was 7.4 ± 6.3-fold (range 2,25). Visual rehabilitation was achieved with optical visual aids in 58% of patients, whereas 42% of patients needed electronically closed-circuit TV systems. Mean reading speed was 20 ± 33 wpm before and increased significantly to 72 ± 35 (p < 0.0001) after the provision of low vision aids for the whole group. Between patients with visual acuity < 0.1 and patients with visual acuity of 0.1 or better, there are highly significant differences in reading speed before (0.4 ± 3.8 versus 20 ± 28 wpm, p , 0.0001) and after providing of visual aids (40 ± 13 versus 84 ± 30 wpm, p , 0.0001). Patients with severe visual impairment (visual acuity , 0.1) showed significantly lower improvement of reading speed compared to patients with visual acuity of 0.1 or better following rehabilitation (p , 0.0001). Before providing of low vision aids, only 16% of patients were able to read; in contrast, reading ability was achieved in 94% of patients after the provision of low vision aids for the whole group. Conclusion:, Our results indicate the great value of low vision rehabilitation through adequate providing of vision aids for the improvement of reading ability, with a highly significant increase of reading speed without training of eccentric viewing in patients with retained central fixation. The prompt implementation of low vision aids in patients with macular degeneration will help them to maintain and regain their reading ability, which can lead to an increase in independence, communication, mental agility and quality of life. [source] Efficacy of intravitreal bevacizumab (Avastin®) therapy for early and advanced neovascular age-related macular degenerationACTA OPHTHALMOLOGICA, Issue 6 2009Ilse Krebs Abstract. Purpose:, To evaluate the safety and efficacy of intravitreal bevacizumab therapy for early and advanced neovascular age-related macular degeneration (ARMD). Methods:, A consecutive series of eyes with neovascular ARMD treated with monthly intravitreal injections of bevacizumab (1.25 mg/0.05 ml) as long as there was evidence of activity on fluorescein angiography (FA) and optical coherence tomography (OCT) was included and observed for 6 months. For further analysis they were assigned to either an early (untreated/newly diagnosed) or an advanced (predominantly fibrotic/pre-treated) ARMD group. We examined distance visual acuity (VA) with Early Treatment Diabetic Retinopathy Study (ETDRS) charts and central retinal thickness with OCT, as well as lesion size and safety aspects. Results:, Forty-four patients (44 eyes) were enrolled (21 early lesions, 23 advanced lesions). Mean VA changed from 0.74 logMAR at baseline to 0.68 logMAR at month 6 (P = 0.01). Improvement in VA was statistically significant only in eyes with early lesions (n = 21) from month 1 (P = 0.015) up to month 6 (P = 0.03). The changes in central retinal thickness (CRT) (P < 0.001) and total lesion size (P < 0.001) were significant in both groups (early and advanced) at all time-points during follow-up. No significant ocular or systemic adverse effects were observed. Conclusion:, Intravitreal bevacizumab was tolerated well by our patients and we did not identify any apparent short-term safety concerns. We observed stabilization in VA overall, with significant improvement in the early lesion group. [source] |
