Comorbid conditions associated with Parkinson's disease: A population-based study

Abstract The burden of comorbidity in Parkinson's disease (PD) remains unclear. All Olmsted County, Minnesota, residents with incident PD in 1976,1995 (n = 197) plus one age- and sex-matched non-PD referent subject per case were followed for all clinical diagnoses from 5 years before through 15 years after index (i.e., year of PD onset for each case and same year for the referent subject). Both members of a case,referent pair were censored at death or emigration of either member to ensure equivalent follow-up. Cases and referent subjects were compared for summary comorbidity (Charlson index) and for the likelihood of having one or more diagnoses within each International Classification of Diseases chapter/subchapter. Before index, the groups were similar for all comparisons. After index, cases had a higher likelihood of diagnoses within the chapters "Mental Disorders" and "Diseases of the Genitourinary System," and within the subchapters "Organic Psychotic Conditions," "Other Psychoses," "Neurotic/Personality/Other Nonpsychotic Disorders," "Hereditary/Degenerative Diseases of Central Nervous System," "Symptoms," "Other Diseases of Digestive System," "Other Diseases of Urinary System," "Diseases of Veins/Lymphatics/Other Circulatory System Diseases," "Fractures of Lower Limb," "Other Diseases of Skin/Subcutaneous Tissue," "Osteopathies/Chrondropathies/Acquired Musculoskeletal Deformities," and "Pneumonia and Influenza." Digestive system and feeding mode in Cambrian naraoiid arthropods

The function of the digestive system of naraoiid arthropods is interpreted in the light of new observations on Early Cambrian specimens from China and detailed comparisons with Recent crustaceans and other arthropods. In naraoiids, paired tubular diverticulae ending as blind caeca are present along the entire midgut, and are interpreted as sites for the secretion of digestive enzymes. Naraoia bears one pair of long, ramifying, distensible diverticulae, possibly used for both food storage and digestion as suggested by Recent analogues (e.g. branchiuran and isopod crustaceans and limulids). Naraoiids were probably epibenthic scavengers/predators rather than mud-eaters. They were either opportunistic intermittent feeders (Naraoia) or more regular feeders (Misszhouia). The mud-fills of the alimentary canals are likely to be artefacts due to taphonomic and weathering processes or, less likely, to sediment ingestion by animals trapped alive in turbiditic flows. The case study of naraoiid arthropods adds to other fossil evidence supporting the idea that predation played a key role in the Early Cambrian food-webs and that organs adapted for this purpose had already reached a high level of diversity and anatomical sophistication. Role for retinoid signaling in left,right asymmetric digestive organ morphogenesis

Abstract The looping events that establish left,right asymmetries in the vertebrate gut tube are poorly understood. Retinoic acid signaling is known to impact left,right development in multiple embryonic contexts, although its role in asymmetric digestive organ morphogenesis is unknown. Here, we show that the genes for retinaldehyde dehydrogenase (RALDH2) and a retinoic acid hydroxylase (CYP26A1) are expressed in complementary patterns in the Xenopus gut during looping. A late-stage chemical genetic assessment reveals that agonists and antagonists of retinoid signaling generate abnormal gut looping topologies, digestive organ heterotaxias, and intestinal malrotations. Accessory organ deformities commonly associated with intestinal malrotation in humans, such as annular pancreas, pancreas divisum, and extrahepatic biliary tree malformations, are also induced by distinct retinoid receptor agonists. Thus, late-stage retinoic acid signaling is likely to play a critical role in asymmetric gut tube morphogenesis and may underlie the etiology of several clinically relevant defects in the digestive system. Lung cancer with metastases to the stomach and duodenum. report of three cases

Over a period of about 1.5 years between September 1999 and April 2000, three cases of lung cancer that metastasized to the stomach and duodenum were encountered. Case 1 was a 74-year-old man with lung cancer at stage IV. During chemotherapy, he passed tarry feces, which led to an endoscopic examination. Subsequently, submucosal tumorous nodules were recognized in the stomach and descending portion of the duodenum, which were diagnosed as metastases. Case 2, a 59-year-old man, underwent radiotherapy for treatment of lung cancer at stage IV. He developed obstructive jaundice 15 months later and, following percutaneous drainage to correct the icteric condition, endoscopic examination was conducted. A 5-cm submucosal tumor was found at the descending portion of the duodenum and a diagnosis of obstructive jaundice caused by a duodenal metastasis was given. Case 3, an 81-year-old male with stage IIIb lung cancer had been receiving oral Tegafur uracil. Because of hypochondriac pain that had lasted for 2 weeks, an endoscopic examination was conducted. A tumorous lesion was discovered in the horizontal part of the duodenum, which proved to be a metastasis. Metastasis of a lung cancer to the digestive system is rare: gastric metastasis is only 4.5% and metastasis to the small intestine, 5.8%. However, our experience suggests that metastases to the digestive system occur more frequently than reports would indicate. Endoscopic screening should be aggressively used, not only for those cases that develop subjective symptoms, but also for the asymptomatic cases to assess accuracy in staging, which may contribute to choosing the most appropriate therapeutic plan. Light and electron microscopic study of the anterior oesophagus of Bulla striata (Mollusca, Opisthobranchia). ,Acta Zoologica (Stockholm) 91: 125,138. The anterior oesophagus of Bulla striata was investigated with light and electron microscopy. In the most anterior region, the ridges of the oesophageal wall are covered by a ciliated columnar epithelium forming large apical blebs which are released into the lumen, an activity that is particularly intense in the oesophageal pouch. In the last two-thirds of the anterior oesophagus, the epithelium is covered with microvilli embedded in a cuticle, but apocrine secretion and cilia are absent. Subepithelial secretory cells are very abundant in the oesophageal wall, except in the roof of the pouch. They have a long neck that crosses the epithelium, whereas the cell body containing the nucleus is embedded in the connective tissue. Large electron-lucent secretory vesicles and many Golgi stacks fill most of their cytoplasm. The histochemical and cytochemical assays show that these cells secrete acid mucopolysaccharides. With the current and future studies we aim to obtain data for the establishment of relationships between morphofunctional features of the digestive system and food types in cephalaspideans. Additionally, the new data about the oesophageal pouch of B. striata may be useful for the establishment of eventual homologies with the oesophageal diverticula of other opisthobranchs. [source]


Halophilic archaea in the human intestinal mucosa

Summary The human gastrointestinal tract microbiota, despite its key roles in health and disease, remains a diverse, variable and poorly understood entity. Current surveys reveal a multitude of undefined bacterial taxa and a low diversity of methanogenic archaea. In an analysis of the microbiota in colonic mucosal biopsies from patients with inflammatory bowel disease we found 16S rDNA sequences representing a phylogenetically rich diversity of halophilic archaea from the Halobacteriaceae (haloarchaea), including novel phylotypes. As the human colon is not considered a salty environment and haloarchaea are described as extreme halophiles, we evaluated and further discarded the possibility that these sequences originated from pre-colonoscopy saline lavage solutions. Furthermore, aerobic enrichment cultures prepared from a patient biopsy at low salinity (2.5% NaCl) yielded haloarchaeal sequence types. Microscopic observation after fluorescence in situ hybridization provided evidence of the presence of viable archaeal cells in these cultures. These results prove the survival of haloarchaea in the digestive system and suggest that they may be members of the mucosal microbiota, even if present in low numbers in comparison with methanogenic archaea. Investigation of a potential physiological basis of this association may lead to new insights into gastrointestinal health and disease. [source]


Histopathological alterations in the edible snail, Babylonia areolata (spotted babylon), in acute and subchronic cadmium poisoning

Abstract Histopathological alterations in 6- to 8-month-old juvenile spotted babylon, Babylonia areolata, from acute and subchronic cadmium exposure were studied by light microscopy. The 96-h LC50 value of cadmium for B. areolata was found to be 3.35 mg/L, and the maximum acceptable toxicant concentration (MATC) was 1.6 mg/L. Snails were exposed to 3.35 and 0.08 mg/L (5% of MATC) of cadmium for 96 h and 90 days, respectively. After exposure the gill, the organs of the digestive system (proboscis, esophagus, stomach, digestive gland, and rectum), and the foot were analyzed for cadmium accumulation. The results showed that most digestive organs had a high affinity for cadmium. The main target organ was the stomach, which could accumulate on average 1192.18 ,g/g dry weight of cadmium. Cadmium was shown to accumulate to a lesser extent in the digestive gland, gill, rectum, esophagus, proboscis, and foot. Histopathological alterations were observed in the gill and digestive organs (proboscis, esophagus, stomach, and rectum). The study showed that the stomach and gill were the primary target organs of both acute and subchronic exposure. Gill alterations included increased size of mucous vacuoles, reduced length of cilia, dilation and pyknosis of nuclei, thickening of basal lamina, and accumulation of hemocytes. The epithelial lining of the digestive tract showed similar alterations such as increased size of mucous vacuoles, reduced length of cilia, and dilation of nuclei. Were volatile organic compounds the inducing factors for subjective symptoms of employees working in newly constructed hospitals?

Abstract This study demonstrated possible relationships between environmental, personal, and occupational factors and changes in the subjective health symptoms of 214 employees after the relocation of a hospital in a region of Japan. Eight indoor volatile organic compounds (VOCs) were detected in at least one of the 19 rooms investigated, and total VOC (TVOC) concentrations in 8 rooms exceeded the advisable value (400 ,g/m3) established by the Ministry of Health, Labour and Welfare of Japan. Formaldehyde was detected in all the investigated rooms, but none of the results exceeded the guideline value (100 ,g/m3). Multiple logistic regression analysis was applied to select variables significantly associated with the subjective symptoms that can be induced by sick building syndrome. The results showed that subjective symptoms of deterioration in the skin, eye, ear, throat, chest, central nervous system, autonomic system, musculoskeletal system, and digestive system among employees were associated mainly with gender difference and high TVOC concentrations (>1200 ,g/m3). Long work hours (>50 h per week) in females and smoking in males were to be blamed for the deterioration of their symptoms. The present findings suggest that to protect employees from indoor environment-related adverse health effects, it is necessary to reduce the concentration of indoor chemicals in new buildings, to decrease work hours, and to forbid smoking. © 2004 Wiley Periodicals, Inc. Safety and efficacy of transdermal fentanyl in patients with cancer pain: phase IV, Turkish oncology group trial

We have performed a prospective evaluation of the efficacy, safety and convenience of the transdermal therapeutic system , fentanyl (TTS-F) in Turkish cancer patients when it was newly available in Turkey. Ninety-nine patients with historically confirmed malignancy and pain entered the study; the mean age was 55.1 (16,58) years. The study duration was 28 days. Transdermal therapeutic system , fentanyl was used in opioid-naïve or pre-treated patients. Most patients reported a decrease in pain severity. Use of rescue medication decreased from day 4 to day 28. The majority of patients rated patch convenience of use as excellent. A total of 22.2% of patients experienced adverse events that were either probably related or very likely to be related to the study drug. The majority of the adverse events mentioned were related to the digestive system. Eighteen serious adverse events were reported by 13 patients. Six events were doubtfully related, and 12 events were not related to the study drug. Four patients died during the trial. None of these deaths was attributed to the study drug. In conclusion, the trial showed that TTS-F is easily managed, effective and will help to enable the appropriate opioid administration to patients who are suffering from cancer pain in Turkey. The effects of diet switching and mixing on digestion in seabirds

Abstract 1.,Animals modulate digestive function in order to optimize digestion of their current diet. Two seabird species were used to test the idea that, as a result, changing and mixing diets might adversely affect digestive performance. 2.,When switched from an energy-dense fish diet (Sprat, Sprattus sprattus (L.)) to an energy-dilute diet (Whiting, Merlangius merlangus (L.)), Lesser Black-Backed Gulls, Larus fuscus L. had worse digestive performance than birds that were acclimated to Whiting, indicating a cost of diet switching. However, when switched from Whiting to Sprat, Lesser Black-Backed Gulls had better digestive performance than birds acclimated to the Sprat diet. 3.,When switched from a Whiting to a Sprat diet some Common Guillemots, Uria aalge (Pont.), developed diarrhoea, although after acclimation birds were able to digest Sprat normally. 4.,Common Guillemots, but not Lesser Black-Backed Gulls, showed a reduction in digestive efficiency when given both diets in a mixed meal. 5.,Common Guillemots appear to have a less flexible digestive system than Lesser Black-Backed Gulls. This difference in response of the two species may be related to differences in their ecology. 6.,Subtle diet shifts may affect digestive performance of animals, and therefore digestive effects, as well as factors such as prey availability and ease of capture, might affect food choice. Barx1 and evolutionary changes in feeding

Abstract During mouse embryonic development, the Barx1 homeobox gene is expressed in the mesenchymal cells of molar teeth and stomach. During early stages of molar development, Barx1 has an instructive role, directing the as yet undetermined ectomesenchymal cells in the proximal region of the jaws to follow a multicuspid tooth developmental pathway. We review here recent results showing an absence of stomach tissue in Barx1 mutant mice. The data strongly suggest that in the presumptive stomach mesenchyme Barx1 acts to attenuate Wnt signalling allowing digestive tract endoderm to differentiate into a highly specialized stomach epithelium. In the light of these new data, we discuss the possibility that evolutionary changes in the Barx1 gene could have simultaneously altered the dentition and the digestive system, therefore positioning Barx1 as a key gene in the evolution of mammals. Bacteria,diet interactions affect longevity in the medfly ,Ceratitis capitata

Abstract Mediterranean fruit flies (Ceratitis capitata Wiedemann, Dipt.: Tephritidae) harbour a diverse community of bacteria in their digestive system. This microbiota may have important functions impacting on the fly's fitness. Recently, we described the effect of eliminating intestinal bacteria on the reproductive success of C. capitata males and females. Here, we expand the view on the nature of fly,bacteria interactions by examining the effect of bacteria on male and female longevity. Antibiotics were used to suppress the gut bacterial community and mortality rates were compared between antibiotic-treated and non-treated flies when either nutritionally stressed (maintained on sugar) or provided with a full diet. These tests revealed that eliminating the gut bacterial population prolonged longevity, but only when flies were nutritionally stressed, indicating that the effect of bacteria on lifespan was diet dependent. Considering these results in light of other known effects of bacteria on fitness components of the fly demonstrates a cost-benefit relationship between C. capitata and its gut microbiota. [source]


Fruit and fibre: the nutritional value of figs for a small tropical ruminant, the blue duiker (Cephalophus monticola)

Abstract Tropical forests throughout the world are home to a guild of small ruminants that consume fruit as a substantial portion of their diet. Because the rumen is relatively inefficient at digesting nonstructural carbohydrates and only slowly digests cellulose, the feeding adaptations of frugivorous ruminants are enigmatic. We examined the nutritional value of wild figs to blue duikers, one of the smallest and most frugivorous ruminants, through chemical analyses and a series of digestion trials with six species of wild African figs. These figs were high in fat, protein, cell wall, lignin and Ca : P ratios, low in sugar and starch, and high in unextractable, fibre-bound tannins when compared with many other fruits. The fibre-bound tannins and protein caused protein digestibility and nitrogen balance to be consistently low or negative. The high fibre content of the figs allowed duikers to only digest 30,60% of energy contained in the figs. However, duikers were able to consume enough digestible energy to maintain body mass during 4-day trials. Therefore, a ruminant digestive system is beneficial to mammals eating high fibre, high-tannin tropical fruit like figs, especially if the mammal is small enough to harvest a sufficient amount to meet its daily energy requirements and has adaptations for reducing the effects of tannins on protein digestibility. Résumé Les forêts tropicales du monde entier abritent toute une faune de petits ruminants dont les fruits constituent une part non négligeable de l'alimentation. Comme le rumen est relativement peu efficace pour digérer les hydrates de carbone non structuraux et ne digère que lentement la cellulose, les adaptations alimentaires des ruminants frugivores sont encore énigmatiques. Nous avons examiné la valeur nutritionnelle des figues sauvages pour le céphalophe bleu, un des ruminants les plus petits et parmi les plus frugivores, au moyen d'analyses chimiques et d'une série d'essais de digestion avec six espèces de figues sauvages africaines. Ces figues étaient riches en graisses, en protéines, en parois cellulaires, en lignine, et leur rapport Ca/P était élevé; elles avaient un contenu faible en sucre et en amidon, et beaucoup de tanins impossibles à extraire, liés aux fibres, comparés à de nombreux autres fruits. Les tanins liés aux fibres et les protéines faisaient que la digestibilité des protéines et l'équilibre azotéétaient en permanence faibles ou négatifs. Le contenu en fibres élevé des figues ne permettait aux céphalophes de digérer que 30 à 60% de l'énergie contenue dans les figues. Cependant, pendant les quatre jours du test, les céphalophes ont pu consommer suffisamment d'énergie digestible pour conserver leur masse corporelle. C'est pourquoi le système digestif des ruminants est bénéfique pour les mammifères qui consomment des fruits tropicaux riches en fibres et en tanins, comme les figues, spécialement si le mammifère est assez petit pour pouvoir en trouver une quantité suffisante pour répondre à ses besoins quotidiens en énergie, et qu'il possède des adaptations qui lui permettent de réduire les effets des tanins sur la digestibilité des protéines. Nutritional condition of Anguilla anguilla starved at various salinities during the elver phase

The effects of food deprivation and environmental salinity (<1, 10 and 20) on survival, fish morphology, organization of the digestive system and body lipid reserves in European eel Anguilla anguilla during the transition from glass eel to elver, were evaluated. Fasted elvers kept in fresh water were able to withstand starvation for >60 days, while those in brackish environments (salinity 10 and 20) reached the level of irreversible starvation at 37 and 35 days, respectively. The high level of lipid reserves contained in liver inclusions and the abdominal cavity (perivisceral deposits) in elvers might explain their long resistance to starvation and differences in fasting tolerance under different salinities. Fasting resulted in a significant reduction of the elvers' condition factor and body depth. There were severe histopathological changes in the digestive system and musculature, such as the alteration of the liver organization, and hepatic glycogen and lipid content, shrinkage of enterocytes and reduction of their height, pancreas degeneration, autolysis of the oesophageal and intestinal mucosa and disarrangement of myofibrils and degeneration of trunk musculature. Degeneration of the oesophageal and intestinal mucosa as a consequence of fasting might have impaired digestive and osmoregulatory functions in feed-deprived fish, directly affecting the tolerance to starvation and survival. Length of food deprivation was associated with a significant increase in mortality, coefficient of variation, cannibalism and point of no return at high salinities. Mortality was dependent on food deprivation and salinity concentrations. Environmental salinity directly influenced the ability of elvers to withstand starvation; once glass eels metamorphosed into elvers, they tolerated starvation better in fresh water than in brackish environments. The ontogeny of the alimentary tract of larval pandora, Pagellus erythrinus L.

The ontogenesis of the alimentary tract and its associated structures (liver, pancreas, gall bladder) was studied in common pandora Pagellus eythrinus L., a promising species for diversification in Mediterranean aquaculture. Mass production of pandora has been limited so far by high larval and juvenile mortalities, which appear to be related to nutritional deficiencies. The development of the larval digestive system was studied histologically from hatching (0 DAH) until day 50 (50 DAH) in reared specimens, obtained by natural spawning from a broodstock adapted to captivity. At first feeding (3,4 DAH) both the mouth and anus had opened and the digestive tract was differentiated in four portions: buccopharynx, oesophagus, incipient stomach and intestine. The pancreas, liver and gall bladder were also differentiated at this stage. Soon after the commencement of exogenous feeding (5,6 DAH), the anterior intestinal epithelium showed large vacuoles indicating the capacity for absorption of lipids, whereas acidophilic supranuclear inclusions indicating protein absorption were observed in the posterior intestinal epithelium. Both the bile and main pancreatic ducts had opened in the anterior intestine, just after the pyloric sphincter, at this stage. Intestinal coiling was apparent since 4 DAH, while mucosal folding began at 10 DAH. Scattered mucous cells occurred in the oral cavity and the intestine, while they were largely diffused in the oesophagus. Gastric glands and pyloric caeca were firstly observed at 28 DAH and appeared well developed by 41 DAH, indicating the transition from larval to juvenile stage and the acquisition of an adult mode of digestion. Description and ontogeny of young Stolephorus baganensis and Thryssa kammalensis, two Engraulididae from Peninsular Malaysia

The morphology of the digestive system was useful to distinguish the larvae of Stolephorus baganensis and Thryssa kammalensis before the full development of their dorsal and anal fins. The relative positions of these fins, the length of the anal fin, and body depth, were good criteria for identifying individuals >10·0 mm LS. For both species, the relative growth of the ten morphometric characters studied was best explained by linear piecewise regressions indicating inflection in allometry at specific standard lengths. Most of these sudden changes in growth rate occurred between 6·9 and 10·0 mm LS for S. baganensis and between 5·8 and 8·9 mm LS for T. kammalensis. Double-centred principal component analysis (PCA) confirmed important changes in the external morphology of both species during this transition period. Prior to this period, the main parameters characterizing the global morphological changes of both species were the length and position of dorsal and anal fins whereas they were body depth and eye diameter (only in S. baganensis) for larger specimens. Complete development of scales did not appear as a suitable criterion to define the end of the larval period, which, instead, was set at the size at which larvae presented an adult-like pigmentation (respectively 35·0 mm LS and 55·0 mm LS in S. baganensis and T. kammalensis). Two pathogens of GreenshellÔ mussel larvae, Perna canaliculus: Vibrio splendidus and a V. coralliilyticus/neptunius -like isolate

Abstract Bacterial pathogens of GreenshellÔ mussel (GSM) larvae can cause batch losses during hatchery production. Twenty-two isolates were screened using a larval bioassay. Two strains were identified as potential pathogens. Phenotypic identification of these strains revealed two non-reactive Gram-negative, oxidase positive rods. Sequencing of the 16S rRNA gene identified Vibrio splendidus and a V. coralliilyticus/neptunius -like isolate as pathogens of GSM larvae, with an ability to cause 83% and 75% larval mortality in vitro, respectively, at a concentration of 102 CFU mL,1. Histopathology indicated that the route of infection was via the digestive system. Using healthy larvae as target hosts, Koch's postulates were confirmed for the two isolates. This is the first report on pathogens of GSM larvae. [source]

Abstract Background The study of premature deaths in people with intellectual disability (ID) has become the focus of recent policy initiatives in England. This is the first UK population-based study to explore cause-specific mortality in adults with ID compared with the general population. Methods Cause-specific standardised mortality ratios (SMRs) and exact 95% confidence intervals were calculated by age and sex for adults with moderate to profound ID living in the unitary authorities of Leicester, Leicestershire and Rutland, UK, between 1993 and 2006. Causes of death were also studied to determine how often ID and associated conditions, such as Down syndrome, were mentioned. Results A total of 503 (17% of population) adults with ID died during the 14-year study period (30 144 person-years). Relatively high cause-specific mortality was seen for deaths caused by congenital abnormalities (SMR = 8560), diseases of the nervous system and sense organs (SMR = 1630), mental disorders (other than dementia) (SMR = 1141) and bronchopneumonia (SMR = 647). Excess deaths were also seen for diseases of the genitourinary system or digestive system, cerebrovascular disease, other respiratory infections, dementia (in men only), other circulatory system diseases (in women only) and accidental deaths (in women only). Two-fifths (n = 204; 41%) of deaths recorded in adults with ID mentioned ID or an associated condition as a contributing cause of death. Conclusions Strategies to reduce inequalities in people with ID need to focus on decreasing mortality from potentially preventable causes, such as respiratory infections, circulatory system diseases and accidental deaths. The lack of mention of ID on death certificates highlights the importance of effective record linkage and ID reporting in health and social care settings to facilitate the government's confidential inquiry into causes of death in this population. Recently identified a novel neuropeptide manserin colocalize with the TUNEL-positive cells in the top villi of the rat duodenum

Abstract We recently isolated a novel 40 amino acid neuropeptide designated manserin from the rat brain. Manserin is derived from secretogranin II, a member of granin acidic secretory protein family by proteolytic processing, as previously reported secretoneurin and EM66. Manserin peptide are localized in the endocrine cells of the pituitary. In this study, we further investigated the manserin localization in the digestive system by immunohistochemical analysis using antimanserin antibody. In the duodenum, manserin immunostaining was exclusively observed in the nuclei of top villi instead of cytosol as observed in neurons in our previous study. Interestingly, manserin-positive cells in the duodenum are colocalized with terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling (TUNEL) positive cells, the cells whose DNA was damaged. Since the top villi of duodenum epithelial cells are known to undergo spontaneous apoptosis during epithelial cell turn over, and since other peptides such as secretoneurin and EM66 derived from SgII have been reported to be cancer-related, these results indicated that manserin peptide may have a role in apoptosis and/or cancer pathogenesis in the digestive organ. Lactational State Modifies Alcohol Pharmacokinetics in Women

Background: Given the physiological adaptations of the digestive system during lactation, the present study tested the hypothesis that lactation alters alcohol pharmacokinetics. Methods: Lactating women who were exclusively breastfeeding a 2- to 5-month-old infant and 2 control groups of nonlactating women were studied. The first control group consisted of women who were exclusively formula-feeding similarly aged infants, whereas the other consisted of women who had never given birth. A within-subjects design study was conducted such that women drank a 0.4 g/kg dose of alcohol following a 12-hour overnight fast during one test session (fasted condition) or 60 minutes after consuming a standard breakfast during the other (fed condition). Blood alcohol concentration (BAC) levels and mood states were obtained at fixed intervals before and after alcohol consumption. Results: Under both conditions, the resultant BAC levels at each time point were significantly lower and the area under the blood alcohol time curve were significantly smaller in lactating women when compared with the 2 groups of nonlactating women. That such changes were due to lactation per se and not due to recent parturient events was suggested by the finding that alcohol pharmacokinetics of nonlactating mothers, who were tested at a similar time postpartum, were no different from women who had never given birth. Despite lower BAC levels in lactating mothers, there were no significant differences among the 3 groups of women in the stimulant effects of alcohol. However, lactating women did differ in the sedative effects of alcohol when compared with nulliparous but not formula-feeding mothers. That is, both groups of parous women felt sedated for shorter periods of time when compared with nulliparous women. Conclusions: The systemic availability of alcohol was diminished during lactation. However, the reduced availability of alcohol in lactating women did not result in corresponding changes in the subjective effects of alcohol. [source]


Digestive system and feeding mode in Cambrian naraoiid arthropods

The function of the digestive system of naraoiid arthropods is interpreted in the light of new observations on Early Cambrian specimens from China and detailed comparisons with Recent crustaceans and other arthropods. In naraoiids, paired tubular diverticulae ending as blind caeca are present along the entire midgut, and are interpreted as sites for the secretion of digestive enzymes. Naraoia bears one pair of long, ramifying, distensible diverticulae, possibly used for both food storage and digestion as suggested by Recent analogues (e.g. branchiuran and isopod crustaceans and limulids). Naraoiids were probably epibenthic scavengers/predators rather than mud-eaters. They were either opportunistic intermittent feeders (Naraoia) or more regular feeders (Misszhouia). The mud-fills of the alimentary canals are likely to be artefacts due to taphonomic and weathering processes or, less likely, to sediment ingestion by animals trapped alive in turbiditic flows. The case study of naraoiid arthropods adds to other fossil evidence supporting the idea that predation played a key role in the Early Cambrian food-webs and that organs adapted for this purpose had already reached a high level of diversity and anatomical sophistication. Quality assessment in general practice trainers

Introduction General practice trainers hold a key position in general practice training, especially through their provision of a role model. Their own competence in general practice care is important in this regard. The purpose of the study was to evaluate whether a quality assessment programme could identify the strengths and weaknesses of GP trainers in four main domains of general practice care. Methods The quality assessment programme comprised validated tests on four domains of general practice: general medical knowledge, knowledge of medical-technical skills, consultation skills and practice management. The criterion for the identification of relative strengths and weaknesses of GP trainers was a variation in the scores of trainers indicating higher and lower scores (strengths and weaknesses) within each domain. Results GP trainers (n=105) were invited to participate in the study and 90% (n=94) did so. The variation in scores allowed the indication of strengths and weaknesses. Main strengths were: general medical knowledge of the digestive system; knowledge of medical skills relating to the skin; consultation skills concerning empathy; practice management with regard to accessibility. Main weaknesses were: general medical knowledge of the neurological system; knowledge of the medical/technical skills relating to the endocrine metabolic and nutritional system; consultation skills regarding shared decision making; practice management involving cooperation with staff and other care providers. Discussion This first systematic evaluation of GP trainers identified their strengths and weaknesses. The weaknesses identified will be used in the improvement process as topics for collective improvement in the GP trainers' general curriculum and in individual learning plans. [source]


Estrogen and its receptors in cancer

Abstract The involvement of estrogen and its receptors in the development of cancer has been known for years. However, the exact mechanism responsible is far from clear. The estrogen-mediated carcinogenic process is complicated by recent findings, which reveal that estrogens have multiple functions in cells, which can be either adverse or beneficial, and that the effects of estrogen may be cell-type or organ dependent. The estrogenic effect may be also greatly influenced by the state of two estrogen receptors, ER, and ER,. This review will discuss the role and function of estrogens and its receptors in cancers of three categories: (1) Breast cancer and gynecologic cancers, (2) Cancers of endocrine organs, (3) Lung cancer and cancers of digestive system. Disturbance of circadian gene expression in hepatocellular carcinoma

Abstract Circadian rhythm plays an important role in the regulation of digestive system. The human circadian rhythm is controlled by at least nine circadian genes. The aims of this study are to understand the expression of the circadian genes between hepatocellular carcinoma tissues and nontumor tissues, and to explore the possible mechanism(s) contributing to the difference. We analyzed differential expression of the 9 circadian genes in 46 hepatocellular carcinoma and paired noncancerous tissues by real-time quantitative RT-PCR and immunohistochemical detection. We also tested the possible regulatory mechanism(s) by direct sequencing and methylation PCR analysis. Our results showed that decreased expression levels of PER1, PER2, PER3, CRY2, and TIM in hepatocellular carcinomas were observed. Decreased-expression of these genes was not caused by genetic mutations, but by several factors, such as promoter methylation, overexpression of EZH2 or other factors. The downexpression of more circadian genes may result in disturbance of cell cycle, and it is correlated with the tumor size. Downregulation of circadian genes results in disturbance of circadian rhythm in hepatocellular carcinoma which may disrupt the control of the central pacemaker and benefit selective survival of cancerous cells and promote carcinogenesis. © 2008 Wiley-Liss, Inc. ABSTRACT Brain,gut axis represents a complex reflex circuit that integrates the communication between cortex and the digestive system. Disturbances of the neuromodulatory processes in the brain,gut axis generate functional digestive disorders mainly centered on the pain symptoms and motility disorders. This article reviews structural and pathophysiologic aspects of the brain,gut axis and explains how the neuromodulatory interventions currently used in order to treat gastrointestinal conditions related to the brain,gut axis disturbances. The neuromodulation can be realized by pharmacologic targeting mainly receptors in the periphery or using electrical stimulation applied at different levels of the nervous system or directly in the muscular layers of the bowels resulting in modulation of the digestive system activity. The efficacy of the methods using electrostimulation is dependent on the parameters of the physical system used: amplitude, frequency, burst time of the electrical current, and also the positioning of the electrodes. While pharmacologic interventions are largely used at the moment, neuromodulatory interventions involving electrical stimulation showed clinical efficacy in research trials and have promise. Social class inequalities in childhood mortality and morbidity in an English population

Summary The objective of this study was to examine the association between social class of the head of household at the time of birth and mortality and morbidity during the first 10 years of life in a cohort of all 117 212 children born to women who both lived, and delivered in hospital, in Oxfordshire or West Berkshire during the period 1 January 1979 to 31 December 1988. Logistic regression was used to estimate social class gradients, with odds ratios (OR), for mortality during the early neonatal period, late neonatal period, post-neonatal period, post-infancy period and throughout the first 10 years of life. Logistic regression was also used to estimate social class gradients, with ORs, for hospital admission rates for 16 broad groups of diseases during years 0,3, 4,6, 7,10 and throughout the first 10 years of life. Poisson regression was used to estimate social class gradients, with effect sizes, for overall hospital admission rates during years 0,3, 4,6, 7,10 and throughout the first 10 years of life. The study revealed a significant social class gradient in mortality during the first 10 years of life (adjusted OR for each decrement in social class category 1.08; [95% confidence interval 1.03, 1.14]). The study also revealed a significant adjusted social class gradient in hospital admission rates for 14 of the 16 groups of diseases during the first 10 years of life. For the majority of these, the social class gradients had attenuated somewhat by the later childhood years. However, the social class gradient persisted throughout the first 10 years of life for diseases of the respiratory system (1.07 [1.05, 1.08]), diseases of the digestive system (1.06 [1.04, 1.09]), and injury and poisoning (1.07 [1.06, 1.09]). In addition, a significant adjusted social class gradient was found in overall hospital admission rates for each age group studied. This study suggests that there are significant social class inequalities in a wide range of adverse child health outcomes. [source]

Cellular functions within tissues are strictly regulated by the tissue microenvironment which comprises extracellular matrix and extracellular matrix-deposited factors such as growth factors, cytokines and chemokines. These molecules are metabolized by matrix metalloproteinases (MMP), a disintegrin and metalloproteinases (ADAM) and ADAM with thrombospondin motifs (ADAMTS), which are members of the metzincin superfamily. They function in various pathological conditions of both neoplastic and non-neoplastic diseases by digesting different substrates under the control of tissue inhibitors of metalloproteinases (TIMP) and reversion-inducing, cysteine-rich protein with Kazal motifs (RECK). In neoplastic diseases MMP play a central role in cancer cell invasion and metastases, and ADAM are also important to cancer cell proliferation and progression through the metabolism of growth factors and their receptors. Numerous papers have described the involvement of these metalloproteinases in non-neoplastic diseases in nearly every organ. In contrast to the numerous review articles on their roles in cancer cell proliferation and progression, there are very few articles discussing non-neoplastic diseases. This review therefore will focus on the properties of MMP, ADAM and ADAMTS and their implications for non-neoplastic diseases of the cardiovascular system, respiratory system, central nervous system, digestive system, renal system, wound healing and infection, and joints and muscular system. Effect of a dysmenorrhea Chinese medicinal prescription on uterus contractility in vitro

Abstract Dysmenorrhea is a common gynecologic complaint. After their ,rst menstrual period, 30%,60% of American women suffer from some level of discomfort. It is estimated that 6 billion work hours are lost in this manner every year in the United States which equals an economic loss of nearly US$200 million. Dysmenorrhea is not only a problem for women but also one which affects quality of life and even reduces productivity in general. Dysmenorrhea is directly related to elevated levels of PGF2, (prostaglandins F2,) and is treated using nonsteroid anti-in,ammatory drugs in Western medicine. Though ef,cacy of the latter is rapid, there are many side effects to the liver, kidney, and digestive system. The anti-in,ammatory effect is temporary, and such drugs are unable to provide a long-term cure. Because of this, Chinese medicinal therapy is being considered as a feasible alternative medicine. In this study, Wen-Jing Tang (one of the dysmenorrhea Chinese medicinal prescriptions) was selected. A 50% alcoholic solution was used to extract active ingredients and create a freeze-dried product. At ,rst, Wen-Jing Tang was used to suppress spontaneous contractions and prostaglandins F2, -induced contractions of rat uterine smooth muscle in vitro. Then, an assessment was performed to determine the mechanism of the prescription. Acetylcholine, ergonovine, propranolol, oxytocin, and KCl were used to analyze the physiological mechanisms of WJT. The results show that antagonism of both PGF2, and ACh are the major mechanisms for treating dysmenorrhea by Wen-Jing Tang. Furthermore, the antagonistic effect of KCl-depolarization contractions may be an auxiliary mechanism of the curative effect. Copyright © 2003 John Wiley & Sons, Ltd. [source]


Onset of Apoptosis in the Cystic Duct During Metamorphosis of a Japanese Lamprey, Lethenteron reissneri

Abstract A nonparasitic lamprey in Japan, Lethenteron reissneri, stops feeding prior to the commencement of metamorphosis. Resumption of feeding cannot take place due to major alterations in the digestive system, including loss of the gall bladder (GB) and biliary tree in the liver. This degeneration of bile ducts is considered to depend on programmed cell death or apoptosis, but molecular evidence of apoptosis remains lacking. Using terminal deoxynucleotidyl transferase dUTP nick-end labeling (TUNEL) staining and immunohistochemistry with an antibody against active caspase-3, we showed that epithelial cells of the cystic duct (CD) and GB became TUNEL-positive by the early metamorphosing stage. Immunohistochemical staining of active caspase-3, a key mediator in the apoptotic cascade, showed that the apoptotic signal was initiated in the region around the CD in the late larval phase. In later stages, active caspase-3-positive epithelial cells were also observed in the large intrahepatic bile duct (IHBD) and peripheral small IHBDs. At the early metamorphosing stage, bile canaliculi between hepatocytes were dilated and displayed features resembling canaliculi in cholestasis. Onset of apoptosis around the CD, which is the pathway for the storage of bile juice, and progression of apoptosis towards the large IHBD, which is the pathway for the secretion of bile juice, may lead to temporary intrahepatic cholestasis. The present study represents the first precise spatial and temporal analysis of apoptosis in epithelial cells of the biliary tract system during metamorphosis of any lamprey species. Identification and partial characterization of selected proteolytic enzymes in the digestive system of giant freshwater Prawn Macrobrachium rosenbergii (De Man) Postlarvae

Abstract Biochemical assays and substrate SDS-PAGE were conducted to partially characterize and identify various types of proteases present in the digestive tract of PL15 giant freshwater prawn (Macrobrachium rosenbergii). Casein hydrolytic assay of the enzyme extracts showed major proteolytic activities at pH 3.0, 6.0 and 9.0, while assay of preincubated enzyme extracts with phenylmethylsulphonyl fluoride (PMSF), a serine protease inhibitor produced a 33.17% reduction in alkaline protease activity. When specific inhibitors tosyl-lysine chloromethyl ketone and tosyl-phenylalanine chloromethyl ketone were used, they resulted in a reduction in activity of proteases in the enzyme extracts by 82.41% and 55.03%, respectively, confirming the presence of trypsin and chymotrypsin, while ethylenediamine tetraacetic acid produced protease activity reduction in 33.92% showing the presence of metalloproteases in the digestive tract of the prawn. Further characterization of the alkaline proteases using SDS-PAGE technique, after incubating the extract in the presence or absence of specific inhibitors, produced six bands corresponding to molecular masses of between 13.48 and 136.1 kDa; two trypsin bands of 13.48 and 36.4 kDa, three chymotrypsin bands in the range of 23.0,73.4 kDa and one for metalloprotease of 136.1 kDa, all of which were identified from a zymogram. This study suggests that protein digestion in M. rosenbergii is initiated by an acid protease followed by a combination of action of alkaline proteases: trypsin, chymotrypsin and metalloproteases.
