How do parents perceive their adolescent's diabetes: a qualitative study

Abstract Background/aims The developmental tasks of adolescence, combined with physical changes, can interfere with self-management behaviour. Yet little is known about how parents view these challenges as they attempt to help their children cope with diabetes. Our objective was to understand how living with an adolescent with diabetes influences parents' perceptions of their child's well-being, their relationship with their child, and how they perceive the influence of peers and school on their child's diabetes. Methods Twenty-eight parents of adolescents with Type 1 diabetes, aged 13,18 years, participated in focus groups. Transcripts were analysed using qualitative methods to determine dominant themes and incidence density. Results Themes included how diabetes negatively influences their adolescent's lifestyle, how diabetes makes it difficult for parents to understand developmental challenges experienced by their child, concerns regarding the potential to develop long-term complications, perceptions on how diabetes impacts on their relationship with their child and relationships with peers and how their children's school impacts on their diabetes self-management Conclusions This qualitative focus group study provides insight into parental perceptions of adolescents living with Type 1 diabetes, specifically as it relates to lifestyle implications, relationships with parents, peers and physicians, and school experiences. Interactions of Temperament and Culture: The Organization of Diversity in Samoan Infancy

Although most are minor adaptations, some culturally patterned adjustments can have profound organizational effects on the niche and the child's developmental trajectory. Research conducted in Samoa suggests at least two distinct adaptations of the modal developmental niche for infants and toddlers keyed to different temperamental profiles: interpersonally assertive and behaviorally restrained. I argue that these two different variants of the modal niche emerge from dynamic interplay of different temperamental profiles, ethnotheories of child development, and child-rearing practices. These different niches can be developmentally significant in that they channel the individual's development in contrastive ways and introduce different future developmental challenges and opportunities. My larger point is that these different manifestations of the developmental niche represent one way in which social, cultural, and ecological factors on the one hand, and individual diversity on the other hand, interact to organize and constrain individual diversity. Toward a Biopsychosocial Model for 21st -Century Genetics

Advances in genomic research are increasingly identifying genetic components in major health and mental health disorders. This article presents a Family System Genetic Illness model to address the psychosocial challenges of genomic conditions for patients and their families, and to help organize this complex biopsychosocial landscape for clinical practice and research. This model clusters genomic disorders based on key characteristics that define types of disorders with similar patterns of psychosocial demands over time. Key disease variables include the likelihood of developing a disorder based on specific genetic mutations, overall clinical severity, timing of clinical onset in the life cycle, and whether effective treatment interventions exist to alter disease onset and/or progression. For disorders in which carrier, predictive, or presymptomatic testing is available, core nonsymptomatic time phases with salient developmental challenges are described pre- and post-testing, including a long-term adaptation phase. The FSGI model builds on Rolland's Family System Illness model, which identifies psychosocial types and phases of chronic disorders after clinical onset. The FSGI model is designed to be flexible and responsive to future discoveries in genomic research. Its utility is discussed for research, preventive screening, family assessment, treatment planning, and service delivery in a wide range of healthcare settings. [source]


Socialization Ambiguity in Samoan Adolescents: A Model for Human Development and Stress in the Context of Culture Change

Rapid globalization is forcing youth around the world to confront new developmental challenges, and conceptual models are needed that can capture this experience and its developmental implications. Exposure to nontraditional lifestyles opens up new socialization opportunities and raises the possibility of stress-inducing dissonance between participating socializing agents and the messages they deliver. Socialization ambiguity is introduced as a model for culture change and adolescent stress, and it is applied to a sample of 10- to 20-year-olds (N=295) in the islands of Samoa. A physiological marker of stress (antibodies against the Epstein-Barr virus) is employed to overcome some of the difficulties associated with measuring stress outcomes. Socialization ambiguity is found to be a significant source of stress on the remote island of Savai'i and the transitional area of rural Upolu, although the direction of the association is different, possibly reflecting divergent socialization goals in these two regions. Emerging Adulthood: What Is It, and What Is It Good For?

ABSTRACT,This article asserts that the theory of emerging adulthood is a useful way of conceptualizing the lives of people from their late teens to their mid- to late 20s in industrialized societies. The place of emerging adulthood within the adult life course is discussed. The weaknesses of previous terms for this age period are examined, and emerging adulthood is argued to be preferable as a new term for a new phenomenon. With respect to the question of whether emerging adulthood is experienced positively or negatively by most people, it is argued that it is positive for most people but entails developmental challenges that may be difficult and there is great heterogeneity, with some emerging adults experiencing serious problems. With respect to the question of whether or not emerging adulthood is good for society, it is argued that claims of the dangers of emerging adulthood are overblown, but emerging adulthood is probably a mixed blessing for society.
