Shapiro Summary., Prophylactic infusion therapy, both primary and secondary, has proven of great benefit to patients with haemophilia, specifically those with severe disease or bleeding episodes and patterns that have lead to development of arthropathy. At this time, optimal outcome in patients with severe haemophilia has been proven achievable with primary prophylaxis initiated at an early age in a regimen of three times weekly or every other day for patients with factor VIII deficiency, and twice weekly for those with factor IX deficiency. Despite the demonstrated benefit of primary prophylaxis, this treatment regimen has not been uniformly adopted into clinical practice even in developed countries. In developing countries, where issues of allocation of precious health care resources are of paramount importance, access to adequate treatment for persons with haemophilia on a programme of on-demand therapy is not commonly available; the cost of primary prophylaxis, even with intermediate purity plasma-derived factor concentrates or plasma fractions such as cryoprecipitate, renders this treatment the exception rather than the rule. [source] Role for Postoperative Radiation Therapy in Adenoid Cystic Carcinoma of the Head and Neck,THE LARYNGOSCOPE, Issue 7 2004Damon A. Silverman MD Objective: Clarify the role for postoperative radiation for adenoid cystic carcinoma (ACC) of the head and neck as it relates to tumor site, T-stage, and surgical margin status. Study Design: Retrospective cohort study at an academic tertiary care hospital. Methods: A review of 129 patients with biopsy-proven ACC was performed. Previous treatment failures and nonoperative candidates were excluded, with 75 patients considered eligible for further study. Patients were grouped according to treatment modality and Kaplan-Meier estimates of overall survival, locoregional control, and distant control were compared using log-rank tests. Patients were also stratified according to tumor site, T-stage, and surgical margin status, and pair-wise comparisons of treatment outcome within each group were performed using Wald tests from Cox proportional hazards models. Results: Twenty-five patients were treated with surgery alone, and 50 were treated with surgery and postoperative radiation. There was no significant difference in outcome between treatment groups when correlated with tumor site (P = .89). However, postoperative radiation was associated with improved overall survival for advanced T-stage (T4) tumors (P = .019) and greater locoregional control for patients with microscopically positive margins (P = .018). There was no demonstrated benefit of postoperative radiation for patients with microscopically negative margins (P = .93). Conclusions: The findings of this study suggest that advanced T-stage and positive microscopic margins are important factors in determining the necessity for postoperative radiation therapy for ACC of the head and neck and that radiation therapy may not be necessary for patients with early T-stage tumors and negative surgical margins. [source] Beta-blocker Utilization and Outcomes in Patients Receiving Cardiac Resynchronization TherapyCLINICAL CARDIOLOGY, Issue 7 2010Andrew Voigt MD Introduction Optimal pharmacologic therapy (OPT) is considered a prerequisite to consideration for cardiac resynchronization therapy (CRT). Hypothesis Medications such as beta-blockers (BB) with demonstrated benefit in heart failure (HF) are being under utilized in patients receiving CRT. Methods Consecutive patients receiving a CRT-capable defibrillator in 2004 at a tertiary care center for standard indications were studied. Clinical data and medications upon hospital discharge were recorded. Patients were followed for endpoints of death or transplantation. Results Of 177 patients receiving a CRT device, 129 (73%) received BB therapy (group 1). Of the 48 patients not on BBs (group 2), relative contraindications were documented in 21 (allergy in 3, hypotension or inotrope-dependent HF in 4, chronic obstructive pulmonary disease [COPD] in 6, and amiodarone therapy in 8). The remaining 27 patients (group 3) did not receive BB therapy despite absence of documented justification. Compared to group 1, group 3 patients were similar in terms of clinical characteristics and angiotensin-converting enzyme inhibitor (ACEI) use, but were less likely to be on statin therapy. Patients were followed for a mean of 19.9 ± 9.2 mo. After adjusting for age, QRS duration, creatinine, left ventricular ejection fraction (LVEF), statin use, and presence of ischemic HF etiology, patients not receiving BB therapy in the absence of contraindication had increased risk of death or transplantation (odds ratio [OR]: 3.1, p = 0.043). Conclusions Absence of BB therapy appears to be independently associated with poor outcome in CRT recipients. Brown The present paper examines one set of potential reasons for the paradox as to why so few firms have adopted activity-based costing (ABC) despite the demonstrated benefits of this costing system. A cross-sectional survey of Australian firms is used to examine the influence of seven technological and organizational factors on firms' initial interest in ABC and their decision to adopt it or not. The organizational factors of top management support, the support of an internal champion, and organizational size were shown to be associated with initial interest in ABC. The decision to adopt or reject ABC had one organizational factor associated with it, the support of an internal champion. [source] Asthma prevention: Breast is best?JOURNAL OF PAEDIATRICS AND CHILD HEALTH, Issue 7 2004A Kemp Abstract: Whilst breastfeeding has been considered to exert a preventative effect on the development of allergic disease, several recent publications have challenged this view, particularly with respect to the long-term outcomes for asthma. There are many other beneficial effects of breastfeeding apart from the possibility of allergy prevention. The suggestion that breastfeeding may increase the development of allergic disease raises concerns about the appropriate steps to take for primary prevention of allergy. It is concluded that breastfeeding can still be recommended for the beneficial effects in reducing atopic disease in childhood in addition to the other demonstrated benefits, and that there are unresolved questions concerning the few studies that suggest the possibility of increased allergic disease in later life.
