EXAMINING THE "CRIMINAL CAREERS" OF PROSTITUTES WITHIN THE NEXUS OF DRUG USE, DRUG SELLING, AND OTHER ILLICIT ACTIVITIES,

This paper examines the co-occurrence of prostitution, drug use, drug selling, and involvement in non-drug crimes among women who have used serious drugs (e.g., crack, heroin). Existing perspectives on the drug use-prostitution nexus are re-examined using three dimensions of the criminal career paradigm: prevalence, lambda, and age of onset. Results show that approximately one-half of the women who reported regular drug use never prostituted, and that, except for use of crack cocaine, use of other drugs was unrelated to the prevalence, frequency, or age of onset into prostitution. The results also show that committing property crime was associated with an increased prevalence and early onset into prostitution, while selling drugs coincided with a decreased prevalence and delayed onset into prostitution. Non-adherence to antihypertensive medication and impaired cognition: which comes first?

Abstract Objective, Antihypertensive medications are important in the prevention of serious consequences of hypertension, such as stroke and heart failure. Up to one-third of elderly hypertensive patients, however, do not adhere to their medication. Adherence to medication decreases with increasing age, and with decreasing cognitive ability, thus elderly, cognitively-impaired patients have poorer control of blood pressure. Good control of blood pressure is associated with decreased prevalence of dementia and Alzheimer's disease. This study assessed the evidence that antihypertensive medications have effects on the prevalence or severity of mild cognitive impairment, dementia or Alzheimer's disease. Methods, The ISI Web of Knowledge database was searched; including replicates, the nine searches identified 14 400 publications since 1952, of which 9.9% had been published in 2009. This review considers the 18 studies meeting the set criteria published in 2009 or later. Key findings, Not all antihypertensive medications are equivalent in their positive cognitive effects, with brain-penetrating angiotensin-converting-enzyme inhibitors and possibly angiotensin receptor antagonists being the most effective. Conclusions, Based on evidence of blood-pressure control and cost, UK National Institute for Health and Clinical Excellence guidelines recommend calcium-channel blockers or thiazide-type diuretics for the treatment of hypertension in patients over 55 years. These guidelines take no account of the potential cognitive effects of the antihypertensive therapies, consideration of which might lead to a review. There may be benefit in stressing that adherence to antihypertensive medication not only decreases the risk of cardiovascular disease and death, but may also decrease the risk or severity of mild cognitive impairment, dementia and Alzheimer's disease. The Tumor Necrosis Factor Alpha ,308G>A Polymorphism Is Associated with Dementia in the Oldest Old

Objectives: To test the hypothesis that the tumor necrosis factor (TNF) ,308 G>A promoter gene polymorphism is a risk factor in age-related dementia and longevity. Design: A cross-sectional and a longitudinal study. Setting: A population-based sample of Danish centenarians. Participants: One hundred-year-old Danes (n=122) from "The Longitudinal Study of Danish Centenarians." Octogenarians (n=174) and healthy volunteers aged 18 to 30 (n=47) served as reference groups. Methods: Whether the distribution of TNF ,308 GG/GA/AA genotypes were different in centenarians than in younger age groups was investigated (Fischer exact test). Furthermore, whether the TNF ,308 G>A polymorphism was associated with the prevalence of dementia (logistic regression analysis), the plasma level of TNF-, (analysis of variance), and mortality in the following 5 years (Cox regression analysis) within the cohort of centenarians was tested. Results: The distribution of TNF ,308 genotypes was not different across the three different age groups, but the GA genotype was associated with decreased prevalence of dementia in centenarians. The few centenarians with AA carrier status had higher mortality risk and tended to show higher plasma levels of TNF-,, but the significance was questionable due to a low number of subjects with this genotype. Conclusion: It is possible that the TNF ,308 A allele is maintained during aging because subjects who are heterozygous for this polymorphism possess the optimal inflammatory response with regard to protection against age-related neurodegeneration. Lack of association of ,2-glycoprotein I polymorphisms Val247Leu and Trp316Ser with antiphospholipid antibodies in patients with thrombosis and pregnancy complications

Summary. Beta2 -glycoprotein I (,2GPI) is an important target antigen for antiphospholipid antibodies (aPL) and thus ,2GPI polymorphisms may influence aPL production and the development of antiphospholipid syndrome. We have studied the relationship between the Val247Leu and Trp316Ser ,2GPI polymorphisms and the aPL status of 230 patients referred for aPL screening. Sixty-one (26·5%) had persistent aPL [anticardiolipin antibodies (IgG and/or IgM), lupus anticoagulants and/or IgG anti-,2GPI antibodies]. A comparison of the genotypic and allelic frequencies of these two polymorphisms between the Caucasian patient population and an ethnic-matched normal control group (n = 308) showed no significant differences between aPL-positive patients, aPL-negative patients and the normal control group. This suggests that the Val or Leu allele at position 247 and the Trp or Ser allele at position 316 of ,2GPI do not play a role in the production of aPL. There was a significantly decreased prevalence of the Ser316 allele in aPL-negative women (n = 98) when compared with female normal control subjects (n = 249) {0·020 [95% confidence interval (CI) 0·00,0·04]vs 0·060 (95% CI 0·04,0·08), P = 0·0286}. Subgroup analysis showed no significant difference between female patients with thrombosis and female normal control subjects. Thus, the Ser316 allele may protect women from developing pregnancy complications by influencing an anticoagulant function of ,2GPI via a mechanism distinct from aPL production. [source]


3223: Dry eye syndrome and omega-3 fatty acids

Purpose Dry eye disease is characterized by an inflammatory component of the ocular surface. Pathways to modulate inflammation include corticoids and cyclosporine. Omega-3 fatty acids like eicosapentaenoic acid and docosahexaenoic acid represent an alternate pharmacologic way to influence the inflammatory cascade. Methods Clinical studies. Results An epidemiologic study in 32.470 healthy women showed that those with a higher intake of omega-3 fatty acids had a 68% decreased prevalence of dry eye syndrome. Hyposecretory dry eye was tested after intake of omega-3 fatty acids for 45 days. Symptoms, signs and inflammatory markers like HLA-DR improved. Hyperevaporative dry eye improved after a long-term supplementation with omega-3 fatty acids with respect to symptoms, break-up time and meibom score. Patients with refractive surgery (PRK) improved after omega-3 fatty acids intake; this was derived from the OSDI-score, Schirmer I test and tear clearance. In 102 contact-lens wearers the symptoms and signs of dry eye improved after 12 weeks therapy with omega-3 fatty acids. Conclusion Nutricionals with omega-3 fatty acids show evidence-based effects on the inflammatory component of ocular surface disease and tear film disorder. Their beneficial effect was tested for hypovolemic and hyperevaporative dry eye. Patients after refractive surgery and contact lens wearers improved after supplementation, too. In contrast to the available anti-inflammatory therapy the supplementation is apt for a long-term application. [source]
