Selected AbstractsISSUES AFFECTING AUSTRALIA'S RURAL OCCUPATIONAL THERAPY WORKFORCEAUSTRALIAN JOURNAL OF RURAL HEALTH, Issue 2 2000Jeannine Millsteed ABSTRACT The unequal distribution of health workers across Australia in favour of urban areas affects the provision of effective health services to rural and remote communities. Additional pressures on the current and future supply of occupational therapists may arise from a restructuring of the health labour force and demographic changes in the age structure of the population. Projections made on the basis of these data indicate that employment growth for occupational therapists will create a demand for 9600 therapists in 2005, or 79.9% more than the number of occupational therapists employed in 1994. Factors such as reductions in the level of immigration and the number of people of working age, and a diminishing population of school leavers to fill student places in universities will make it difficult to meet the projected demand for occupational therapists. Occupational therapy labour force planning suffers from a lack of detailed data on under-serviced areas. Such data are critical for clarifying the magnitude of the projected discrepancy between future demand and supply needs for occupational therapists in rural and urban areas in Australia. Sheriff Abstract Aim:, Mechanical factors such as the muscle pump have been proposed to augment flow by several mechanisms. The potential for limb angular motion to augment local perfusion pressure (pressure = ½,r2,2, where , is the fluid density, r the radius and , the angular velocity) has been overlooked. We sought to test the hypothesis that limb angular motion augments limb arterial pressure. Methods:, Nine human subjects performed horizontal shoulder flexion (,±90° at 0.75 Hz for 30 s). We measured finger arterial pressure (photoplethysmography) in the moving (Trial 1) and non-moving arm (Trial 2) in separate trials along with the pressure (strain gauge) generated at the fingers within a length of water-filled tubing mounted on the moving arm in both trials. Results:, Arm swinging raised (P < 0.05) the mean pressure measured in the tubing by 11 ± 2 and 14 ± 2 mmHg (Trials 1 and 2 respectively). In response to exercise, the rise in mean finger arterial pressure in the swinging limb (18 ± 3 mmHg, Trial 1) exceeded (P < 0.05) the rise in the resting limb (8 ± 2 mmHg, Trial 2) by an amount similar to the 11 mmHg rise in pressure generated in the tubing in Trial 1. Conclusions:, We conclude that the swinging of a limb creates centrifugal force (a biomechanical centrifuge) which imparts additional pressure to the arteries, but not the veins owing to the venous valves, which further widens the arterial,venous pressure difference. [source] Urban and industrial water use in the Krishna Basin, India,IRRIGATION AND DRAINAGE, Issue 4 2009Daniel J. Van Rooijen Bassin Krishna; utilisation urbaine de l'eau; utilisation industrielle de l'eau; modélisation Abstract Regional urbanization and industrial development require water that may put additional pressure on available water resources and threaten water quality in developing countries. In this study we use a combination of census statistics, case studies, and a simple model of demand growth to assess current and future urban and industrial water demand in the Krishna Basin in southern India. Water use in this "closed" basin is dominated by irrigation (61.9 billion cubic metres (BCM) yr,1) compared to a modest domestic and industrial water use (1.6 and 3.2 BCM yr,1). Total water diversion for non-irrigation purposes is estimated at 7,8% of available surface water in the basin in an average year. Thermal power plants use the majority of water used by industries (86% or 2.7 BCM yr,1), though only 6.8% of this is consumed via evaporation. Simple modelling of urban and industrial growth suggests that non-agricultural water demand will range from 10 to 20 BCM by 2030. This is 14,28% of basin water available surface water for an average year and 17,34% for a year with 75% dependable flow. Although water use in the Krishna Basin will continue to be dominated by agriculture, water stress, and the fraction of water supplies at risk of becoming polluted by urban and industrial activity, will become more severe in urbanized regions in dry years. L'urbanisation régionale et le développement industriel demandent de l'eau, ce qui peut augmenter la pression sur les ressources en eau disponibles dans les pays en développement. Dans cette étude nous utilisons une combinaison de données de recensement, des études de cas et un modèle simple de croissance de la demande pour évaluer la demande en eau urbaine et industrielle actuelle et future dans le bassin Krishna en Inde du sud. Les usages de l'eau dans ce bassin « fermé » sont dominés par l'irrigation (61.9 milliards de m3/an) alors que les usages domestiques et industriels sont modestes (1.6 et 3.2 milliards de m3/an). L'eau utilisée en dehors de l'irrigation est estimée à 7 ,8% de l'eau de surface disponible dans le bassin en année moyenne. Turris MSN RN Aim., The aim of this paper is to present a feminist critique of the concept of patient satisfaction. Background., Fiscal restraint, health care restructuring, shifting demographics, biomedical technological advances, and a significant shortage of health care professionals are stretching health care systems across North America to the breaking point. A simultaneous focus on consumerism and health service accountability is placing additional pressure on the system. The concept of patient satisfaction, with roots in the consumer movement of the 1960s, has both practical and political relevance in the current health care system and is commonly used to guide research related to consumer experiences of health care. Because the quality of health care encounters may lead to treatment-seeking delays, patient satisfaction research may be an effective vehicle for addressing this public health issue. However, there is wide agreement that patient satisfaction is an under-theorized concept. Using current conceptualizations of patient satisfaction, we end up all too often producing a checklist approach to ,achieving' patient satisfaction, rather than developing an understanding of the larger issues underlying individual experiences of health care. We focus on the symptoms rather than the problems. Discussion., Without further theoretical refinement, the results of research into patient satisfaction are of limited use. To push forward theoretical development we might apply a variety of theoretical lenses to the analysis of both the concept and the results of patient satisfaction research. Feminism, in particular, offers a perspective that may provoke further refinement of patient satisfaction as a concept. Conclusions., Without a deeper understanding of the values and beliefs (or the worldview) that informs our approaches to researching patient satisfaction, researchers will be reacting to the most obvious indicators and failing to address the underlying issues related to individual experiences of health care. [source] Micellar electrokinetic capillary chromatography determination of zinc bacitracin and nystatin in animal feedJOURNAL OF SEPARATION SCIENCE, JSS, Issue 9 2006Rade Injac Abstract An MEKC procedure was developed for the separation of zinc bacitracin (Zn-BC) and nystatin (NYS) in mixtures and in animal feedstuff. The running buffer was 15 mM borate/19 mM phosphate, pH 8.2, containing 20 mM SDS and 10% v/v methanol. Samples were run at 25°C, the applied voltage was 25 kV, and an additional pressure of 5 mbar was applied. Both analytes were detected by UV simultaneously at 215 nm, Zn-BC alone at 192 and 254 nm, and NYS alone at 305 nm. The method was shown to be specific, accurate (recoveries were 100.0 ± 0.6% and 100.1 ± 0.6% for Zn-BC and NYS, respectively), linear over the tested range (correlation coefficients 0.9991 and 0.9994), and precise (RSD below 1.3% for both analytes). The method was applied to determine Zn-BC and NYS as additives in animal feed. [source] Nature conservation and urban development control in the Portuguese planning system: a new impetus against old praxis?ENVIRONMENTAL POLICY AND GOVERNANCE, Issue 5 2008Teresa Fidelis Abstract Natura 2000 areas bring a new incentive to assess the performance of land-use planning in protecting environmental values from the impacts of development pressures. In the last decades, urban growth and consequent environmental impacts on natural areas have been a major concern for the Portuguese land-use planning system. Sprawl around sensitive areas has been revealed to be a persistent phenomenon in spite of the increasing challenges underlying land-use plans. This article critically analyses the content of three main documents recently adopted by the Portuguese government , the ,National Strategy for Sustainable Development', the ,National Policy Programme for Spatial Planning' and the ,Sector Plan for Natura 2000' , seeking prospects to innovate future plans at lower levels in order to prevent additional pressures on natural areas. First, the article reviews the recent theoretical debate on planning for the protection of natural areas. Results evidenced by recent EU evaluation reports are used to propose a set of guidelines to evaluate planning guidance at national level. Second, it critically analyses the three planning documents, bearing in mind the main features of the planning system and the proposed guidelines. The article is concluded with a discussion of their potential, exploring whether they bring a new impetus to the role of land-use planning against an outdated and persistent praxis, or whether, on the contrary, further efforts to strengthen planning guidance remain to be formulated. [source] Virus infection and grazing exert counteracting influences on survivorship of native bunchgrass seedlings competing with invasive exoticsJOURNAL OF ECOLOGY, Issue 2 2006C. M. MALMSTROM Summary 1,Invasive annual grasses introduced by European settlers have largely displaced native grassland vegetation in California and now form dense stands that constrain the establishment of native perennial bunchgrass seedlings. Bunchgrass seedlings face additional pressures from both livestock grazing and barley and cereal yellow dwarf viruses (B/CYDVs), which infect both young and established grasses throughout the state. 2,Previous work suggested that B/CYDVs could mediate apparent competition between invasive exotic grasses and native bunchgrasses in California. 3,To investigate the potential significance of virus-mediated mortality for early survivorship of bunchgrass seedlings, we compared the separate and combined effects of virus infection, competition and simulated grazing in a field experiment. We infected two species of young bunchgrasses that show different sensitivity to B/CYDV infection, subjected them to competition with three different densities of exotic annuals crossed with two clipping treatments, and monitored their growth and first-year survivorship. 4,Although virus infection alone did not reduce first-year survivorship, it halved the survivorship of bunchgrasses competing with exotics. Within an environment in which competition strongly reduces seedling survivorship (as in natural grasslands), virus infection therefore has the power to cause additional seedling mortality and alter patterns of establishment. 5,Surprisingly, clipping did not reduce bunchgrass survivorship further, but rather doubled it and disproportionately increased survivorship of infected bunchgrasses. 6,Together with previous work, these findings show that B/CYDVs can be potentially powerful elements influencing species interactions in natural grasslands. 7,More generally, our findings demonstrate the potential significance of multitrophic interactions in virus ecology. Although sometimes treated collectively as plant ,predators', viruses and herbivores may exert influences that are distinctly different, even counteracting. [source] |
