Recent Use of Proton Pump Inhibitor-Based Triple Therapies for the Eradication of H. pylori: A Broad Data Review

abstract Introduction. For the eradication of Helicobacter pylori a 1-week triple therapy combining proton pump inhibitors with two antibiotics has been recommended as a gold standard therapy. However, a recent broad data review on the efficacy of the different regimens is missing. Therefore, the aim of this study was to systematically review the recent literature. Methods. We undertook a broad data review of the efficacy of nine different 7-day triple therapies consisting of a proton pump inhibitor (lansoprazole, pantoprazole, omeprazole) in its standard dosage and two antibiotics. Relevant original papers on H. pylori eradication in adults, published in English or German between 1995 and 2000, were identified from MEDLINE searches. Studies were reviewed and selected according to predefined criteria. Results. Our predefined criteria were fulfilled by 79 full paper articles including 112 study arms with 8383 patients on intention-to-treat, or 6787 patients on per-protocol basis, respectively. The mean eradication rates unweighted or weighted by the number of patients in the study arm vary from 71.9% to 83.8% for intention-to-treat analysis and from 78.5% to 91.2% for per-protocol analysis. Conclusions. All nine PPI based triple therapy regimens are very effective in H. pylori eradication. The current literature review underlines that the use of either lansoprazole, omeprazole, or pantoprazole combined with two antibiotics yield similar high eradication rates. [source]


Donor-Derived Disease Transmission Events in the United States: Data Reviewed by the OPTN/UNOS Disease Transmission Advisory Committee

Donor-derived disease transmission is increasingly recognized as a source of morbidity and mortality among transplant recipients. Policy 4.7 of the Organ Procurement and Transplantation Network (OPTN) currently requires reporting of donor-derived events. All potential donor-derived transmission events (PDDTE) reported to OPTN/UNOS were reviewed by the Disease Transmission Advisory Committee (DTAC). Summary data from January 1, 2005,December 31, 2007, were prepared for presentation. Reports of PDDTE have increased from 7 in 2005, the first full year data were collected, to 60 in 2006 and to 97 in 2007. More detailed information is available for 2007; a classification system for determining likelihood of donor-derived transmission was utilized. In 2007, there were four proven and one possible donor-derived malignancy transmissions and four proven, two probable and six possible donor-derived infectious diseases transmissions. There were nine reported recipient deaths attributable to proven donor transmissions events arising from eight donors during 2007. Although recognized transmission events resulted in significant morbidity and mortality, transmission was reported in only 0.96% of deceased donor donations overall. Improved reporting, through enhanced recognition and communication, will be critical to better estimate the transmission risk of infection and malignancy through organ transplantation. [source]

,The National Stream Quality Accounting Network: a flux-based approach to monitoring the water quality of large rivers

Abstract Estimating the annual mass flux at a network of fixed stations is one approach to characterizing water quality of large rivers. The interpretive context provided by annual flux includes identifying source and sink areas for constituents and estimating the loadings to receiving waters, such as reservoirs or the ocean. Since 1995, the US Geological Survey's National Stream Quality Accounting Network (NASQAN) has employed this approach at a network of 39 stations in four of the largest river basins of the USA: the Mississippi, the Columbia, the Colorado and the Rio Grande. In this paper, the design of NASQAN is described and its effectiveness at characterizing the water quality of these rivers is evaluated using data from the first 3 years of operation. A broad range of constituents was measured by NASQAN, including trace organic and inorganic chemicals, major ions, sediment and nutrients. Where possible, a regression model relating concentration to discharge and season was used to interpolate between chemical observations for flux estimation. For water-quality network design, the most important finding from NASQAN was the importance of having a specific objective (that is, estimating annual mass flux) and, from that, an explicitly stated data analysis strategy, namely the use of regression models to interpolate between observations. The use of such models aided in the design of sampling strategy and provided a context for data review. The regression models essentially form null hypotheses for concentration variation that can be evaluated by the observed data. The feedback between network operation and data collection established by the hypothesis tests places the water-quality network on a firm scientific footing. Extraintestinal manifestations of Edwardsiella tarda infection

Summary Edwardsiella tarda, a member of the family Enterobacteriaceae, is a rare human pathogen. Gastroenteritis is the most frequently reported manifestation of E. tarda infection. In contrast, extraintestinal infection with E. tarda has rarely been reported. This study made a retrospective case and microbiological data review of patients with extraintestinal E. tarda infections to further understand this disease. This study retrospectively reviewed the charts of all isolates of E. tarda cultures from clinical specimens other than faeces at Chang Gung Memorial Hospital, Taoyuan, Taiwan from October 1998 through December 2001. Edwardsiella tarda was isolated from 22 clinical specimens from 22 hospitalised patients (13 females and nine males). The extraintestinal manifestations of E. tarda infection included biliary tract infection, bacteraemia, skin and soft tissue infection, liver abscess, peritonitis, intra-abdominal abscess, and tubo-ovarian abscess. The major underlying diseases predisposing to E. tarda extraintestinal infection were hepatobiliary diseases, malignancy and diabetes mellitus. The overall mortality rate of E. tarda extraintestinal infection in the present series was 22.7% (5/22), and four (40%) of 10 patients with bacteraemia expired. Although rare, human E. tarda extraintestinal infections can have diverse clinical manifestations and moreover may cause severe and life-threatening infections. Consequently, E. tarda should be considered a potentially important pathogen. [source]


Role of proteinuria in defining pre-eclampsia: Clinical outcomes for women and babies

Summary 1.,The presence of proteinuria is not essential to the diagnosis of pre-eclampsia under many diagnostic consensus statements. The aim of the present study was to assess maternal and perinatal outcomes after proteinuric pre-eclampsia compared with other non-proteinuric disease presentations. 2.,An individual patient data review (n = 670) was undertaken for 2003,2006 at a tertiary referral centre in Sydney (NSW, Australia). Women were diagnosed in accordance with the Australasian Society for the Study of Hypertension in Pregnancy Consensus Statement. Data were analysed with the Chi-squared test, t -tests and non-parametric tests. Statistical significance was set at P < 0.05. 3.,The proteinuric cohort had higher systolic and diastolic blood pressure recordings than the non-proteinuric cohort (160/102 and 149/94 mmHg, respectively; P < 0.001), and were also administered magnesium sulphate more frequently (44 vs 22%, respectively; P < 0.001), delivered at earlier gestation (37 vs 38 weeks, respectively; P < 0.001), required operative delivery more frequently (63 vs 48%, respectively; P < 0.001) and received more antihypertensive medications during the antenatal period (72 vs 57%, respectively; P < 0.001). Acute renal failure and acute pulmonary oedema were rare. Four cases of eclampsia all occurred in non-proteinuric women. The perinatal mortality rate was lower for the offspring of women with proteinuric pre-eclampsia compared with offspring of non-proteinuric women (13/1000 and 31/1000, respectively; P = 0.006). 4.,The results of the present study indicate that the presence of proteinuria denotes a group of women who have higher antenatal blood pressure, who deliver at earlier gestation and require operative delivery more commonly, although it is not an indicator of other markers of maternal morbidity or perinatal mortality. Detecting Tropical Forests' Responses to Global Climatic and Atmospheric Change: Current Challenges and a Way Forward

ABSTRACT Because of tropical forests' disproportionate importance for world biodiversity and for the global carbon cycle, we urgently need to understand any effects on these ecosystems from the ongoing changes in climate and atmosphere. This review, intended to complement existing data reviews on this topic, focuses on three major classes of challenges that we currently face when trying to detect and interpret directional changes in tropical forests. One is the very limited existing information on the historical context of study sites. Lasting effects from past climate, natural disturbances, and/or human activities could be significantly affecting current-day processes in tropical forests and need to be investigated for all active field sites. Second, while progress has been made in recent years on standardizing and refining research approaches, a number of methods- and data-limitations continue to affect efforts both to detect within-forest changes and to relate them to ongoing environmental change. Important outstanding needs are improved sampling designs, longer time-series of observations, filling key data gaps, and data access. Finally, forest responses to ongoing environmental change are complex. The effects of many simultaneously changing environmental factors are integrated by the plants, and their responses can involve significant lags, carryovers, and non-linearities. Specifying effects of individual environmental changes, however, is required for accurate ecosystem-process models and thus for projecting future impacts on these forests. After discussing these several types of challenges and ways to address them, I conclude with a priority agenda for this critical area of research. RESUMEN Debido a la importancia desproporcionada de los bosques tropicales para la biodiversidad mundial y para el ciclo global del carbono, es urgente identificar los impactos sobre estos ecosistemas provocados por los cambios actuales en el clima y en la atmósfera. Este artículo de revisión, escrito con el propósito de complementar otras revisiones recientes, se enfoca en tres principales clases de retos que enfrentamos actualmente en la detección e interpretación de cambios direccionales en los bosques tropicales. Primero es la gran escasez de información histórica acerca de los sitios de estudio. Los procesos actuales en los bosques tropicales pueden reflejar los efectos prolongados del pasado climático, las perturbaciones naturales y/o las actividades humanas, por lo que deben de ser investigados en todos los sitios actuales de estudio. Segundo, a pesar de avances recientes en la estandarización y el refinamiento de los métodos de investigación, nuestra habilidad para detectar cambios en los bosques y ligarlos a los grandes cambios ambientales sigue siendo limitada. Para garantizar avances en el área se requiere mejorar los diseños de muestreo, extender las series de observación en el tiempo a plazos mayores, llenar ciertos vacíos claves en el conocimiento, y facilitar el acceso a los datos existentes. Por último, se requiere de enfoques que tomen en cuenta la complejidad de las respuestas de los bosques a los cambios ambientales. Las plantas integran los efectos de cambios simultáneos en múltiples factores ambientales, y sus respuestas pueden ser no lineales e incluir efectos de retraso y acarreo. No obstante, es importante también especificar los efectos individuales de los diferentes cambios ambientales para afinar los modelos de procesos a nivel del ecosistema, y así poder proyectar los impactos futuros sobre estos bosques. Después de discutir dichos retos y estrategias para enfrentarlos, concluyo con una agenda de prioridades para esta área crítica de investigación. [source]
