Selected AbstractsAnalysis of bone mineral density in urolithiasis patientsINTERNATIONAL JOURNAL OF UROLOGY, Issue 4 2005HIDENORI TSUJI Abstract Background:, The association between hypercalciuria and bone mineral density (BMD) has been already recognized. The aim of the present study is to relate BMD to age and sex and to evaluate the calcium metabolism and hypercalciuria-defined dietary or non-dietary category in patients with urolithiasis. Methods:, The BMI of the L2,L4 lumbar vertebrae was measured in 310 renal stone patients (191 men and 119 women). Percent age matched score (%AMS), which is the percent ratio of measured BMD to the mean BMD of age-matched control subjects, was utilized for the appraisal of BMD. Low BMD groups were defined by lower than 90% of %AMS. Results:, Low BMD was observed in 27.7% of urinary stone patients, which was not a significant difference to that of control subjects (23.5%) who were measured in the health examination. In male patients with urolithiasis, the frequency of patients in whom BMD had been apt to decrease since youth was high, but there was not a proven significant difference among the three age groups (20,39 years old, 40,59 years old and 60 years old or older). In contrast, for female stone patients, the frequency of low BMD markedly increased in patients aged 40 years or older, when menopause occurs. Furthermore, in female stone patients with hypercalciuria, the frequency of reduced BMD reached more than 40%. When the cause was non-dietary hypercalciuria (classified mainly on the daily amount of urinary calcium excretion after ingestion of calculus test diet), the frequency of reduced BMD reached 65% (P < 0.01). Conclusions:, In case female stone patients with non-dietary hypercalciuria become menopausal, not only the risk of recurrent lithiasis increases, but the possibility of developing osteopenia in the future also increases. Appropriate treatments for prophylactic effects on urolithiasis or osteopenia should be considered, as judged from BMD, diet, sex, urinary calcium excretion and other factors synthetically. [source] A Pilot Study of Banxia Houpu Tang, a Traditional Chinese Medicine, for Reducing Pneumonia Risk in Older Adults with DementiaJOURNAL OF AMERICAN GERIATRICS SOCIETY, Issue 12 2007Koh Iwasaki MD OBJECTIVES: To evaluate whether the traditional Chinese herbal medicine Banxia Houpu Tang (BHT, formula magnolia et pinelliae) prevents aspiration pneumonia and pneumonia-related mortality in elderly people. DESIGN: A prospective, observer-blinded, randomized, controlled trial. SETTING: Two long-term care hospitals for handicapped elderly patients in Japan from March 2005 to February 2006. PARTICIPANTS: One hundred four elderly patients (31 men and 73 women; mean age±standard deviation 83.5±7.8) with dementia and cerebrovascular disease, Alzheimer's disease, or Parkinson's disease. INTERVENTION: Ninety-five participants (mean age 84.0, M:F=28:67) were randomly assigned to the BHT treatment group (n=47) or the control group (n=48) and took BHT or placebo for 12 months. MEASUREMENTS: The occurrence of pneumonia, mortality due to pneumonia, and the daily amount of self-feeding. RESULTS: Complete data were available for analysis on 92 of the 95 subjects randomized. Four patients in the BHT group developed pneumonia, and one of them died as a result. Fourteen patients in the control group developed pneumonia, and six of them died as a result. There was a significant difference between the two groups in pneumonia onset (P=.008), and a tendency toward significance in pneumonia-related mortality (P=.05). The relative risk of pneumonia in the BHT group compared with the control group was 0.51 (95% confidence interval (CI)=0.27,0.84, P=.008) and that of death from pneumonia was 0.41 (95% CI=0.10,1.03, P=.06) according to the Cox proportional hazards model. No adverse events were observed from treatment with BHT. The BHT group was able to maintain self-feeding better than the control group (P=.006). CONCLUSION: Treatment with BHT reduced the risk of pneumonia and pneumonia-related mortality in elderly patients with dementia. [source] Changes in physical activity in pre-schoolers and first-grade children: longitudinal study in the Czech RepublicCHILD: CARE, HEALTH AND DEVELOPMENT, Issue 3 2009E. Sigmund Abstract Background The transition from kindergarten to first year at school is associated with a variety of psychosocial changes in children. The aim of this longitudinal study was to identify the changes in children's physical activity (PA) upon entry to first year at school; and to identify the days of the school week that exhibit low PA. Methods We monitored the PA levels of 176 children twice: initially in kindergarten and again in first-year classes at school. The age (mean ± standard deviation) of children at kindergarten was 5.7 ± 0.5 years and 6.7 ± 0.5 years at the first year of elementary school. We evaluated PA employing the activity energy expenditure (AEE , kcal/kg/day) from Caltrac accelerometer and daily amount of steps (STEPS) from Yamax pedometer. Participants were monitored over 7 days to include a weekend. Results The mean AEE was 11.5 in pre-school girls and 12.9 kcal/kg/day in boys; and STEPS were 9923 steps/day in girls and 11 864 in boys on weekdays. At weekends, it was 11.5 kcal/kg/day in girls and 12.7 kcal/kg/day in boys and 10 606 steps/day in girls and 11 182 steps/day in boys. The mean AEE and STEPS in first-grade girls and boys was 9.9 and 9.8 kcal/kg/day respectively, and 7911 and 8252 steps/day respectively on weekdays, and 8.8 and 9.0 kcal/kg/day and 6872 and 7194 steps/day respectively at weekends. First-grade school children had significantly lower PA than pre-school children on weekdays (P < 0.0001) and at weekends (P < 0.0001). Decline in PA on weekdays was during time spent at school (P < 0.0001) and not during after-school children's leisure time. Conclusion The parts of the week when first-grade school children show low PA are the times spent in lessons and in after-school nursery and at weekends. PA needs to be promoted using intervention programmes mainly during the after-school nursery programmes and at weekends. [source] Concentration of copper, iron, manganese and zinc in muscle, fat and bone tissue of lambs of the breed German Merino Landsheep in the course of the growing period and different feeding intensitiesJOURNAL OF ANIMAL PHYSIOLOGY AND NUTRITION, Issue 3-4 2007G. Bellof Summary A growth experiment with 108 lambs (breed German Merino Landsheep) was carried out in order to examine how gender, body weight and feeding intensity affect trace element concentrations in tissues and carcass. The lambs (50% male and 50% female) were fattened at three levels of feeding intensity (,low', ,medium' and ,high' by varying daily amounts of concentrate and hay) and slaughtered at different final body weights (30, 45 or 55 kg). Six male and six female animals were sacrificed at 18 kg live weight at the beginning of the comparative slaughter experiment. The left half carcass of each animal was divided into muscle tissue, fat tissue as well as bones and sinews and analysed for the trace elements copper (Cu), iron (Fe), manganese (Mn) as well as zinc (Zn). The body weight level influenced the Zn concentrations significantly in all tissues. In addition, the Fe concentration in the fat tissue was influenced by the body weight as well as the Cu content in the bone tissue. An influence due to gender could be seen for the Zn concentration in the muscle and fat tissue and for the Fe content in the fat and bone tissue as well as for the Cu concentration in the bones. The feeding intensity affected the Cu content in the muscle and bone tissue and also the Zn content in the muscle tissue. In the present study with lambs at body weight range from 18 to 55 kg on an average, 127 mg Fe, 87 mg Zn, 1.5 mg Cu as well as 1.1 mg Mn per kilogram dry matter were found in the bone tissue. In lamb muscle tissue combined from all parts (body weight range from 18 to 45 kg, both genders) the highest concentrations were for Zn and Fe [3.42 and 1.31 mg/100 g meat (wet weight basis)], while Cu remained far below these levels (0.08 mg/100 g meat and Mn was even below the detection limit of 0.025 mg/kg). Lamb muscle is a valuable source for highly available haem-Fe as well as for Zn and Cu in human nutrition. The nutrient levels of seaweeds are also shown in comparison to currently applicable reference nutrient intakes or guideline daily amounts of nutrients and are contrasted with terrestrial foodstuffs with respect to selected nutrients. For the purpose of comparison, a sample serving size of 8 g dry weight of seaweed is used to illustrate the potential contribution of seaweeds to the diet. [source] Estimation of the relative antiinflammatory efficacies of six commercial preparations of Harpagophytum procumbens (Devil's Claw)PHYTOTHERAPY RESEARCH, Issue 3 2010Nassima Abdelouahab Ouitas Abstract The current work compared the relative efficacies of six commercial formulations of H. procumbens. Each formulation was assayed for the content of harpagoside (1), harpagide (2), verbascoside (3) and 8- O-p -coumaroyl harpagide (4) and, based on the recommended dosages, the total daily amounts were determined and used to establish anti-/proinflammatory (A/P) factors. The formulations were compared using ex vivo porcine skin for their activities towards COX-2 by Western blotting. The results showed great variation in the amounts of compounds 1,4 within the six formulations examined. The relative proportions of 1,4 also varied widely between the products and this inconsistency was reflected in the A/P factors, which correlated with the COX-2 expression (R2 = 0.9496). Although the data support the beneficial antiinflammatory effects from the use of some of the brands tested, others would appear potentially to exacerbate inflammation. To conclude, a ratio based upon the amount and relative proportions of anti- and proinflammatory compounds can be used to predict relative antiinflammatory properties. Also, with access to a diversity of ostensibly similar commercial products, the patient may experience varying therapeutic responses. Finally, current pharmacopoeia monographs, which are generally concerned with a minimum harpagoside content, are inadequate for ensuring the quality of products based on H. procumbens. [source] Inhibition of bacterial translocation by chronic ethanol consumption in the rat,APMIS, Issue 12 2001VALERIA BENDER BRAULIO Chronic ethanol ingestion has been associated with small intestine morphological changes, disrupted host mucosal defenses and bacterial overgrowth. Since bacterial translocation (BT) may result from such alterations, we have investigated the potential effect of chronic ethanol consumption on BT. For this purpose, male Wistar rats were fed a liquid diet containing 5% v/v ethanol for 4 weeks (EG, n=16), and a pair-fed group received equal daily amounts of calories in a similar diet without ethanol (PFG, n=16). On experimental day 29, distal ileum ligature and small intestine inoculation of a tetracycline-resistant E. coli strain (Tc®E. coli R6) followed by duodenal ligature was performed. After 1 or 5 h post inoculation, mesenteric lymph nodes, liver, spleen and kidney were excised. Unexpectedly, rats of the EG presented markedly less BT to the mesenteric lymph nodes (p<0.001) and to the other organs examined compared to rats of the PFG. This BT inhibition was observed at 1 and 5 h after bacterial inoculation, and may be attributed exclusively to chronic ethanol ingestion. Since alcoholism is well known to decrease host immunity, these results suggest that other factors, independent of the immune function, may be involved in the BT inhibition observed in this study.
