    Real Time Myocardial Contrast Echocardiography During Supine Bicycle Stress and Continuous Infusion of Contrast Agent.

    Cutoff Values for Myocardial Contrast Replenishment Discriminating Abnormal Myocardial Perfusion
    Background: Myocardial contrast echocardiography (MCE) is a new imaging modality for diagnosing coronary artery disease (CAD). Objective: The aim of our study was to evaluate feasibility of qualitative myocardial contrast replenishment (RP) assessment during supine bicycle stress MCE and find out cutoff values for such analysis, which could allow accurate detection of CAD. Methods: Forty-four consecutive patients, scheduled for coronary angiography (CA) underwent supine bicycle stress two-dimensional echocardiography (2DE). During the same session, MCE was performed at peak stress and post stress. Ultrasound contrast agent (SonoVue) was administered in continuous mode using an infusion pump (BR-INF 100, Bracco Research). Seventeen-segment model of left ventricle was used in analysis. MCE was assessed off-line in terms of myocardial contrast opacification and RP. RP was evaluated on the basis of the number of cardiac cycles required to refill the segment with contrast after its prior destruction with high-power frames. Determination of cutoff values for RP assessment was performed by means of reference intervals and receiver operating characteristic analysis. Quantitative CA was carried out using CAAS system. Results: MCE could be assessed in 42 patients. CA revealed CAD in 25 patients. Calculated cutoff values for RP-analysis (peak-stress RP >3 cardiac cycles and difference between peak stress and post stress RP >0 cardiac cycles) provided sensitive (88%) and accurate (88%) detection of CAD. Sensitivity and accuracy of 2DE were 76% and 79%, respectively. Conclusions: Qualitative RP-analysis based on the number of cardiac cycles required to refill myocardium with contrast is feasible during supine bicycle stress MCE and enables accurate detection of CAD.     Enhanced Left Ventricular Endocardial Border Delineation with an Intravenous Injection of SonoVue, a New Echocardiography Contrast Agent:

    A European Multicenter Study
    The safety and efficacy of SonoVue (also referred to as BR1), a new contrast agent for delineating endocardial border of the left ventricle after intravenous administration, was assessed. Two hundred and eighteen patients with suspected coronary artery disease undergoing fundamental echocardiography for the assessment of left ventricle were enrolled in a prospective multicenter, single blind, cross-over study with random sequence allocation of four different doses of SonoVue. Endocardial border definition in the apical and parasternal views was scored as O = not visible, 1 = barely visible, and 2 = well visualized before and after contrast enhancement. Analysis was performed by two pairs of off-site observers. Safety of SonoVue was also assessed. Results of our study indicated that the mean improvements in the endocardial border visualization score were as follows: 3.1 ± 7.8 (95% CI, 2.5 and 3.7) for 0.5 ml, 3.4 ± 8.0 (95% CI, 2.8 and 4.0) for 1 ml, 3.4 ± 7.9 (95% CI, 2.8 and 4.0) for 2 ml, and 3.7 ± 8.0 (95% CI, 3.1 and 4.3) for 4 ml (P < 0.05 for all doses from baseline). Changes from baseline in endocardial visualization scores were also seen in the apical views (P < 0.05) and they were dose-dependent (P < 0.001). Similar enhancements of endocardial visualization scores were observed in the apical views in patients with suboptimal baseline echocardiographic images. Diagnostic confidence for assigning a score and image quality also were significantly better following contrast enhancement. No significant changes in the laboratory parameters and vital signs were noted following contrast enhancement, and the side effects were minimal. It was concluded that SonoVue is safe and effective in delineating endocardial border, including in patients with suboptimal baseline images. [source]


    Design of an Ultrasound Contrast Agent for Myocardial Perfusion

    Myocardial contrast echography (MCE) has been a major research objective in cardiovascular ultrasound for almost two decades. The design of a contrast agent fulfilling the needs of MCE requires taking into consideration a number of points: a basic decision has to be made whether a deposit agent or a free-flowing agent would be more appropriate and whether an agent active at low/medium mechanical index (MI) is preferable to an agent active only at high MI; only a small percentage of the cardiac output enters the coronary microcirculation, which means that highly sensitive bubble detection methods, such as harmonic imaging or pulse inversion, are needed; the low velocity of blood in the microcirculation that leads to extensive bubble destruction during imaging means that intermittent imaging and/or an agent active at low MI is (are) required; the duration of the contrast effect must be sufficient to allow a complete examination and is affected by the rate of contrast administration; the performance of the contrast agent should not be equipment-dependent. The ultimate goal in MCE is to be able to quantify blood flow in the various segments to determine if adequate oxygenation is achieved. Ultrasound-mediated bubble destruction followed by the measurement of bubble replenishment kinetics opens new perspectives for quantification. SonoVue is a free-flowing ultrasound contrast agent made of sulphur hexafluoride microbubbles stabilized by a highly elastic phospholipid monolayer. SonoVue is able to produce myocardial opacification at a wide range of acoustic pressures and in particular at Mis as low as 0.1. Its performance is not equipment-dependent. Good results for myocardial opacification have been observed in all animal species tested (dogs, minipigs, rabbits), using continuous as well as intermittent imaging. Trials are in progress to demonstrate the clinical utility of SonoVue for rest and stress perfusion studies, in particular for the diagnosis of CAD, the detection of myocardial infarction, the assessment of the success of interventions and myocardial viability, and the detection of hibernating myocardium.     Superparamagnetic Hyperbranched Polyglycerol-Grafted Fe3O4 Nanoparticles as a Novel Magnetic Resonance Imaging Contrast Agent: An In Vitro Assessment

    Abstract Hyperbranched polyglycerol-grafted, magnetic Fe3O4 nanoparticles (HPG-grafted MNPs) are successfully synthesized by surface-initiated ring-opening multibranching polymerization of glycidol. Reactive hydroxyl groups are immobilized on the surface of 6,9,nm Fe3O4 nanoparticles via effective ligand exchange of oleic acid with 6-hydroxy caproic acid. The surface hydroxyl groups are treated with aluminum isopropoxide to form the nanosized macroinitiators. The successful grafting of HPG onto the nanoparticles is confirmed by infrared and X-ray photoelectron spectroscopy. The HPG-grafted MNPs have a uniform hydrodynamic diameter of (24.0,±,3.0) nm, and are very stable in aqueous solution, as well as in cell culture medium, for months. These nanoparticles have great potential for application as a new magnetic resonance imaging contrast agent, as evidenced by their lack of cytotoxicity towards mammalian cells, low uptake by macrophages, excellent stability in aqueous medium and magnetic fields, and favorable magnetic properties. Furthermore, the possibility of functionalizing the hydroxyl end-groups of the HPG with cell-specific targeting ligands will expand the range of applications of these MNPs. [source]


    Nonblinking and Nonbleaching Upconverting Nanoparticles as an Optical Imaging Nanoprobe and T1 Magnetic Resonance Imaging Contrast Agent

    Core/shell upconverting nanoparticles (UCNPs) of NaGdF4:Er3+,Yb3+/NaGdF4 (see figure) are shown to serve as a multimodal imaging probe that works for both background-free optical imaging and magnetic resonance imaging (MRI). The nonblinking and nonbleaching properties of UCNPs can contribute to minimization of possible artifacts in long-term imaging experiments. Owing to Gd3+ ions in the host matrix, contrast is enhanced in T1 -weighted MRI. [source]

    Synthesis and Evaluation of Water-Soluble Fluorinated Dendritic Block-Copolymer Nanoparticles as a 19F-MRI Contrast Agent

    Abstract Well-defined water-soluble fluorinated polymer nanoparticles (PNPs) with a high fluorine content and biocompatibility were successfully prepared by living radical polymerization (LRP) of 2,2,3,3-tetrafluoropropyl methacrylate (TFPMA) from the polyamidoamine dendrimer macroinitiator (PAMAM-Br), and successive block polymerization of carboxybetaine monomer (CMB). The obtained core,shell type PNPs (PAMAM- g -PTFPMA- b -PCMB) showed high solubility in water and a sphere-like structure with a diameter in the range of 15,80,nm in water. The short 19F-NMR spin,lattice relaxation time (T1) (<250,ms) of PAMAM- g -PTFPMA- b -PCMB allowed the use of fast repetition time. The spin,spin relaxation time (T2) was evaluated to be as low as 10 ms. 19F-MRI in vitro signals can be detectable even at concentrations lower than 1 µM (particle concentration). These results demonstrate that a new type of 19F-MRI contrast agent can be developed by the molecular design using the dendrimer-initiated LRP method. [source]


    Fluorinated Polymer Nanoparticles as a Novel 19F MRI Contrast Agent Prepared by Dendrimer-Initiated Living Radical Polymerization

    Abstract Novel fluorinated polymer nanoparticles (PNPs) for 19F MRI were prepared by living radical polymerization initiated by a dendrimer. The dendritic macroinitiator with Br substituents was synthesized from hydroxy-group-terminated G2 polyamidoamine dendrimers. The arborescent fluorinated polymers of 2,2,3,3-tetrafluoropropyl methacrylate and 2,2,2-trifluoroethyl methacrylate were characterized in molecular weight, the number of arms, the degree of polymerization per arm, and the diameter as a whole. The PNP diameter was precisely controlled by the molecular weight in the range of 3,25,nm. In addition, the fluorinated PNP gave a narrow resonance by 19F NMR spectroscopy. These results indicate that the fluorinated PNP can be used as a new type of 19F MRI agent. [source]

    Potential MRI Contrast Agents Based on Micellar Incorporation of Amphiphilic Bis(alkylamide) Derivatives of [(Gd,DTPA)(H2O)]2,

    Abstract DTPA-bisamide derivatives with alkyl chains containing 14, 16 and 18 carbon atoms were synthesized and complexes of various trivalent lanthanide ions (Ln = Gd, La, Pr, Eu) were formed. Variable temperature proton NMR spectroscopy of paramagnetic praseodymium(III) and europium(III) complexes revealed that long aliphatic substituents considerably increase the energy barrier for the intramolecular rearrangement around the lanthanide ion. The gadolinium(III) complexes were incorporated into mixed micelles, and photon correlation spectroscopy showed that the mean sizes of all the micelles were within the same range. The NMRD curves of all three DTPA-bisamide-gadolinium complexes incorporated in mixed micelles display higher relaxivity values than the commercially available Gd,DTPA contrast agent. The higher relaxivity obtained for the micellar DTPA-bisamide-gadolinium complexes with C14 and C16 chains relative to the micellar DTPA-bisamide-GdIII C18 chain complex could be attributable to the fact that the alkyl chain containing 18 carbon atoms is longer than the alkyl chain of the major component of the micelles, DPPC, in which it is inserted.     Porous Polymersomes with Encapsulated Gd-Labeled Dendrimers as Highly Efficient MRI Contrast Agents

    Abstract The use of nanovesicles with encapsulated Gd as magnetic resonance (MR) contrast agents has largely been ignored due to the detrimental effects of the slow water exchange rate through the vesicle bilayer on the relaxivity of encapsulated Gd. Here, the facile synthesis of a composite MR contrast platform is described; it consists of dendrimer conjugates encapsulated in porous polymersomes. These nanoparticles exhibit improved permeability to water flux and a large capacity to store chelated Gd within the aqueous lumen, resulting in enhanced longitudinal relaxivity. The porous polymersomes, ,130,nm in diameter, are produced through the aqueous assembly of the polymers, polyethylene oxide- b -polybutadiene (PBdEO), and polyethylene oxide- b -polycaprolactone (PEOCL). Subsequent hydrolysis of the caprolactone (CL) block resulted in a highly permeable outer membrane. To prevent the leakage of small Gd-chelate through the pores, Gd was conjugated to polyamidoamine (PAMAM) dendrimers via diethylenetriaminepentaacetic acid dianhydride (DTPA dianhydride) prior to encapsulation. As a result of the slower rotational correlation time of Gd-labeled dendrimers, the porous outer membrane of the nanovesicle, and the high Gd payload, these functional nanoparticles are found to exhibit a relaxivity (R1) of 292 109,mM,1,s,1 per particle. The polymersomes are also found to exhibit unique pharmacokinetics with a circulation half-life of >3.5,h and predominantly renal clearance. [source]

    Surface Modification of Exfoliated Layered Gadolinium Hydroxide for the Development of Multimodal Contrast Agents for MRI and Fluorescence Imaging

    Abstract A novel method for modifying the surface of magnetic-resonance-contrasting layered gadolinium hydroxide (LGdH) is developed providing them with water- and bio-compatibility and acid-resistance, all of which are essential for medical applications. A stable colloid of exfoliated layers is synthesized by exchanging interlayer anions of LGdH with oleate ions. The delaminated layers are successively coated with phospholipids with poly(ethylene glycol) tail groups, and their effectiveness as a contrast agent for magnetic resonance imaging (MRI) is demonstrated. The adaptability of this surface modification approach for incorporating functional molecules and fabricating a fluorescent colloid of LGdH, which has the potential utility as a multimodal probe, is also demonstrated. This result provides a novel approach for expanding the applications of layered inorganic materials and developing a new class of MRI contrast agents. [source]


    Supercritical-Fluid-Assisted One-Pot Synthesis of Biocompatible Core(, -Fe2O3)/Shell(SiO2) Nanoparticles as High Relaxivity T2 -Contrast Agents for Magnetic Resonance Imaging

    Abstract Monodisperse iron oxide/microporous silica core/shell composite nanoparticles, core(, -Fe2O3)/shell(SiO2), with a diameter of approximately 100,nm and a high magnetization are synthesized by combining sol,gel chemistry and supercritical fluid technology. This one-step processing method, which is easily scalable, allows quick fabrication of materials with controlled properties and in high yield. The particles have a specific magnetic moment (per kg of iron) comparable to that of the bulk maghemite and show superparamagnetic behavior at room temperature. The nanocomposites are proven to be useful as T2 MRI imaging agent. They also have potential to be used in NMR proximity sensing, theranostic drug delivery, and bioseparation. [source]


    Inorganic Nanoparticles for MRI Contrast Agents

    Abstract Various inorganic nanoparticles have been used as magnetic resonance imaging (MRI) contrast agents due to their unique properties, such as large surface area and efficient contrasting effect. Since the first use of superparamagnetic iron oxide (SPIO) as a liver contrast agent, nanoparticulate MRI contrast agents have attracted a lot of attention. Magnetic iron oxide nanoparticles have been extensively used as MRI contrast agents due to their ability to shorten T2* relaxation times in the liver, spleen, and bone marrow. More recently, uniform ferrite nanoparticles with high crystallinity have been successfully employed as new T2 MRI contrast agents with improved relaxation properties. Iron oxide nanoparticles functionalized with targeting agents have been used for targeted imaging via the site-specific accumulation of nanoparticles at the targets of interest. Recently, extensive research has been conducted to develop nanoparticle-based T1 contrast agents to overcome the drawbacks of iron oxide nanoparticle-based negative T2 contrast agents. In this report, we summarize the recent progress in inorganic nanoparticle-based MRI contrast agents. [source]


    Ultrasound Contrast Agents for Brain Perfusion Imaging and Ischemic Stroke Therapy

    ABSTRACT Stroke is one of the major causes of death and disabilities in industrialized countries. Ultrasound imaging is a largely wide spread bedside technique that is easily accessible and valuable in case of emergency but suffers from the fact that the ultrasound wave has to cross the skull for brain imaging. However, ultrasound contrast agents and new contrast-specific imaging modalities have helped to improve the diagnostic quality of transcranial ultrasonography. This review article surveys and discusses the current state of microbubbles technology and the use of contrast-enhanced transcranial ultrasound for the assessment of brain perfusion. Future aspects and expecta tions in contrast agent functionality, such as targeting and drug or gene delivery, acceleration of thrombolysis, and imaging technology, are also discussed. [source]

Molar relaxivities of HPMA copolymer-linked nitroxides increased linearly in proportion to the number of nitroxides attached per gram of copolymer. HPMA copolymer-linked nitroxides with 15, 20 and 30 mol-% nitroxide exhibited higher relaxivities than gadolinium diethylenetriaminepentaacetic acid (Gd-DTPA).
