Presence and Duration of Atrial Fibrillation Detected by Continuous Monitoring: Crucial Implications for the Risk of Thromboembolic Events

Introduction: Asymptomatic atrial fibrillation (AF) can expose patients to the risk of stroke. The primary objective of this study was to assess the incidence of thromboembolic events in relationship with CHADS2 (congestive heart failure, hypertension, age ,75 years, diabetes mellitus, and prior stroke, or transient ischemic attack) score and AF presence/duration. The secondary objective was to compare intermittent versus continuous monitoring strategies. Methods and Results: Data from patients with an implanted pacemaker and a history of AF were analyzed. Thromboembolic risk was quantified through CHADS2 score. Three AF groups were considered: patients with <5-minutes AF on 1 day (AF-free); patients with >5-minutes AF on 1 day but <24 hours (AF-5 minutes); patients with AF episodes >24 hours (AF-24 hours). Monitoring strategies involving 24-hour Holter, 1-week Holter, and 30-day Holter were simulated. Data from 568 patients continuously monitored for 1 year were analyzed: 171 (30%) had CHADS2 score = 0; 269 (47%) had CHADS2 score = 1; 111 (20%) had CHADS2 score = 2; and 17 (3%) had CHADS2 score , 3. During follow-up, 14 patients (2.5%) had an ischemic thromboembolic event. AF-24 hours patients numbered 223 (39.2%); AF-5 minutes, 179 (31.5%); and AF-free, 29.2%. By combining AF presence/duration with CHADS2 score, two subpopulations with markedly different risks of events (0.8% vs 5%, P = 0.035) were identified, the former corresponding to AF-free with CHADS2,2, or AF-5 minutes with CHADS2,1, or AF-24 hours with CHADS2= 0. The mean sensitivity in detecting an AF episode lasting >5 minutes was 44.4%, 50.4%, and 65.1% for 24-hour Holter, 1-week Holter, and 1-month Holter monitoring, respectively. Conclusion: In patients with recurrent AF episodes, risk stratification for thromboembolic events can be improved by combining CHADS2 score with AF presence/duration. [source]


FRET-Based Direct and Continuous Monitoring of Human Fucosyltransferases Activity: An Efficient synthesis of Versatile GDP- L -Fucose Derivatives from Abundant d- Galactose,

Abstract We have developed a facile and versatile protocol for the continuous monitoring of human fucosyltransferases activity by using fluorescence energy resonance transfer (FRET), and have explored the feasibility of its use in an inhibitor screening assay. A convenient sugar nucleotide with a fluorogenic probe, 6-deoxy-6- N -(2-naphalene-2-yl-acetamide)-,- L -galactopyranos-1-yl-guanosine 5,-diphosphate disodium salt (1), was efficiently synthesized from naturally abundant D -galactopyranose via a key intermediate, 6-azide-1,2,3,4-tetra- O -benzoyl-6-deoxy-,- L -galactopyranose (10). It was demonstrated that the combined use of the glycosyl donor 1 and a dansylated acceptor substrate, sialyl-,2,3-LacNAc derivative (2) allowed us to carry out highly sensitive, direct, and continuous in vitro monitoring of the generation of sialyl Lewis,X (SLex), which is catalyzed by human ,-1,3-fucosyltransferase,VI (FUT-VI). A kinetic analysis revealed that compound 1 was an excellent donor substrate (KM=0.94,,M and Vmax=0.14,,M,min,1) for detecting human FUT-VI activity. To the best of our knowledge, this synthetic fluorogenic probe is the most sensitive and selective donor substrate for FUT-VI among all of the known GDP-Fuc analogues, including the parent GDP-Fuc. When a dansylated asparagine-linked glycopeptide 20, which is derived from egg yolk was employed as an alternate acceptor substrate, a FRET-based assay with compound 1 could be used to directly monitor the ,1,6-fucosylation at the reducing terminal GlcNAc residue by human FUT-VIII (KM=175,,M and Vmax=0.06,,M/,min); this indicates that the present method might become a general protocol for the characterization of various mammalian fucosyltransferases in the presence of designated fluorogenic acceptor substrates. The present protocol revealed that compound 23, which was obtained by a 1,3-dipolar cycloaddition between the disodium salt 16 and 1-ethynyl-naphthalene exhibits highly potent inhibitory effects against the FUT-VI-mediated sialyl Lewis,X synthesis (IC50=5.4,,M). Continuous monitoring of interface pressure distribution in intensive care patients for pressure ulcer prevention

Abstract Title.,Continuous monitoring of interface pressure distribution in intensive care patients for pressure ulcer prevention. Aim., This paper is a report of a study conducted to examine whether continuous interface pressure monitoring of postoperative patients in an intensive care unit is feasible in clinical practice. Background., The interface pressure between skin and surfaces is generally evaluated for pressure ulcer prevention. However, the intensity and duration of interface pressure necessary for pressure ulcer development remains unclear because the conventional interface pressure sensors are unsuitable for continuous monitoring in clinical settings. Methods., A total of 30 postoperative patients in an intensive care unit participated in this study in 2006,2007. A sensor was built into a thermoelastic polymer mattress. The whole-body interface pressure was recorded for up to 48 hours. Pressure ulcer development was observed during the morning bed-bath. For analysis, the intensity and duration of the maximal interface pressure was evaluated. Findings., The mean age of the study group was 62·0 ± 15·4 years. Two participants developed stage I pressure ulcer and blanchable redness at the sacrum. The longest duration of pressures greater than 100 mmHg were 487·0, 273·5 and 275·7 minutes in the pressure ulcer, blanchable redness and no redness groups respectively. Conclusion., Continuous monitoring of the intensity and duration of whole-body interface pressure using the KINOTEX sensor is feasible in intensive care patients. [source]


Detection of Inadvertent Catheter Movement into a Pulmonary Vein During Radiofrequency Catheter Ablation by Real-Time Impedance Monitoring

Introduction: During radiofrequency ablation to encircle or isolate the pulmonary veins (PVs), applications of radiofrequency energy within a PV may result in stenosis. The aim of this study was to determine whether monitoring of real-time impedance facilitates detection of inadvertent catheter movement into a PV. Methods and Results: In 30 consecutive patients (mean age 53 ± 11 years) who underwent a left atrial ablation procedure, the three-dimensional geometry of the left atrium, the PVs, and their ostia were reconstructed using an electroanatomic mapping system. The PV ostia were identified based on venography, changes in electrogram morphology, and manual and fluoroscopic feedback as the catheter was withdrawn from the PV into the left atrium. Real-time impedance was measured at the ostium, inside the PV at approximately 1 and 3 cm from the ostium, in the left atrial appendage, and at the posterior left atrial wall. There was an impedance gradient from the distal PV (127 ± 30 ,) to the proximal PV (108 ± 15 ,) to the ostium (98 ± 11 ,) in each PV (P < 0.01). There was no significant impedance difference between the ostial and left atrial sites. During applications of radiofrequency energy, movement of the ablation catheter into a PV was accurately detected in 80% of the cases (20) when there was an abrupt increase of ,4 , in real-time impedance. Conclusion: There is a significant impedance gradient from the distal PV to the left atrium. Continuous monitoring of the real-time impedance facilitates detection of inadvertent catheter movement into a PV during applications of radiofrequency energy. Toward an Implantable Wireless Cardiac Monitoring Platform Integrated with an FDA-Approved Cardiovascular Stent

Continuous monitoring of blood pressure from a minimally invasive device in the pulmonary artery can serve as a diagnostic and early warning system for cardiac health. The foremost challenge in such a device is the wireless transfer of data and power from within the blood vessel to an external device while maintaining unrestricted flow through the artery. We present a miniaturized system, which is attached to the outer surface of a regular or drug-eluting FDA-approved stent. When expanded, the stent maintains a patent vessel lumen while allowing contact between the electronic sensors and the blood supply. The stent-based antenna can be used for both wireless telemetry and power transfer for the implanted electronics. Using the stent platform as both a radiating antenna and a structural support allows us to take advantage of an FDA-approved device whose safety and surgical procedure are well established. The electronics package has been reduced to an area of less than 1 mm2, with a thickness under 300 ,m. A minimally invasive implantation procedure allows the delivery of the stent-based implant in nearly any major vessel of the body. This article describes an initial prototype with two stents configured as a single dipole, a 2.4-GHz transmitter microchip, and a battery, and validates transcutaneous transmission through ex vivo and in vivo porcine studies. The results demonstrate the feasibility of a stent-based wireless platform for continuous monitoring of blood pressure. [source]


New strategies for the control of arthropod vectors of disease in dogs and cats

Abstract Arthropod-borne diseases (ABDs) in cats and dogs have a major impact on animal health and welfare and, in many cases, also on human health. Many ABDs are expected to increase in prevalence as a result of changing social habits, habitat modifications, introductions of exotic vectors and climate change. Control has, historically, focused on the use of insecticides and chemotherapy. We review alternative, emerging approaches to ABDs that currently offer promise, particularly modelling and molecular techniques and the development of novel vaccines that target molecules produced by arthropods during the bloodmeal. We argue that there is an urgent need to establish effective surveillance systems for most ABDs across various countries in order to facilitate a detailed risk analysis, which should include evaluation of potential spread to new areas and the possible introduction of new exotic species or disease agents. This will require clear and exhaustive knowledge on the distribution of ABDs in different areas, understanding of the diagnostic limitations pertaining to ABDs and standardization of techniques among reference laboratories in different countries. Continuous monitoring of insecticide resistance and the development of management strategies to minimize its onset are also essential. Ultimately, it is probable that approaches which attempt to reduce vector abundance or treat hosts with chemotherapy alone are unlikely to be effective in the long term. More suitable approaches may include greater use of a range of mutually compatible options in integrated management programmes. [source]


Rapid Vasodilation in Isolated Skeletal Muscle Arterioles: Impact of Branch Order

Cannulated first-order (1As) and third-order arterioles (3As) of the rat gastrocnemius (G) muscle were exposed to cumulative doses of KCl, acetylcholine (Ach), or adenosine (Ado). In addition, time course and magnitude of vasodilation to localized application of these agonists were determined. 1As and 3As dilated similarly to incremental doses of the agonists. Continuous monitoring of internal diameter revealed a fast and transient dilatory response to microinjections of the agonists, with an average time delay (TD) before the onset of vasodilation of 2.8 ± 0.2 seconds (1As: 3.0 ± 0.3 seconds and 3As: 2.6 ± 0.3 seconds) and time-to-peak (TP) of 8.2 ± 0.7 seconds (1As: 10.3 ± 1 seconds and 3As:5.7 ± 0.5 seconds). No significant differences were detected for all parameters between 1As and 3As for KCl or Ado application, while 1As had a significantly longer TP and greater peak dilation than 3As to Ach. These findings demonstrate that 1As and 3As from the rat G muscle appear to have similar responsiveness to vasoactive agonists. Furthermore, the average TD before vasodilation supports a role for metabolic signals as contributors to the ROV. [source]


Solifluction processes in an area of seasonal ground freezing, Dovrefjell, Norway

Abstract Continuous monitoring of soil temperatures, frost heave, thaw consolidation, pore water pressures and downslope soil movements are reported from a turf-banked solifluction lobe at Steinhøi, Dovrefjell, Norway from August 2002 to August 2006. Mean annual air temperatures over the monitored period were slightly below 0°C, but mean annual ground surface temperatures were around 2°C warmer, due to the insulating effects of snow cover. Seasonal frost penetration was highly dependent on snow thickness, and at the monitoring location varied from 30,38,cm over the four years. The shallow annual frost penetration suggests that the site may be close to the limit of active solifluction in this area. Surface solifluction rates over the period 2002,06 ranged from 0.5,cm yr,1 at the rear of the lobe tread to 1.6,cm yr,1 just behind the lobe front, with corresponding soil transport rates of 6,cm3,cm,1 yr,1 and 46,cm3,cm,1 yr,1. Pore water pressure measurements indicated seepage of snowmelt beneath seasonally frozen soil in spring with artesian pressures beneath the confining frozen layer. Soil thawing was associated with surface settlement and downslope soil displacements, but following clearance of the frozen ground, later soil surface settlement was accompanied by retrograde movement. Summer rainfall events caused brief increases in pore pressure, but no further soil movement. Surface displacements exceeded maximum potential frost creep values and it is concluded that gelifluction was an important component of slow near-surface mass movements at this site. Temporal and spatial variations in solifluction rates across the area are likely to be considerable and strongly influenced by snow distribution. Copyright © 2008 John Wiley & Sons, Ltd. [source]

NERVE STIMULATION IN THYROID SURGERY: IS IT REALLY USEFUL?

Background: Monitoring of the recurrent laryngeal nerve (RLN) has been claimed in some studies to reduce rates of nerve injury during thyroid surgery compared with anatomical dissection and visual identification of the RLN alone, whereas other studies have found no benefit. Continuous monitoring with endotracheal electrodes is expensive whereas discontinuous monitoring by laryngeal palpation with nerve stimulation is a simple and inexpensive technique. This study aimed to assess the value of nerve stimulation with laryngeal palpation as a means of identifying and assessing the function of the RLN and external branch of the superior laryngeal nerve (EBSLN) during thyroid surgery. Methods: This was a prospective case series comprising 50 consecutive patients undergoing total thyroidectomy providing 100 RLN and 100 EBSLN for examination. All patients underwent preoperative and postoperative vocal cord and voice assessment by an independent ear, nose and throat surgeon, laryngeal examination at extubation and all were asked to complete a postoperative dysphagia score sheet. Dysphagia scores in the study group were compared with a control group (n = 20) undergoing total thyroidectomy without nerve stimulation. Results: One hundred of 100 (100%) RLN were located without the use of the nerve stimulator. A negative twitch response occurred in seven (7%) RLN stimulated (two bilateral, three unilateral). Postoperative testing, however, only showed one true unilateral RLN palsy postoperatively (1%), which recovered in 7 weeks giving six false-positive and one true-positive results. Eighty-six of 100 (86%) EBSLN were located without the nerve stimulator. Thirteen of 100 (13%) EBSLN could not be identified and 1 of 100 (1%) was located with the use of the nerve stimulator. Fourteen per cent of EBSLN showed no cricothyroid twitch on EBSLN stimulation. Postoperative vocal function in these patients was normal. There were no instances of equipment malfunction. Dysphagia scores did not differ significantly between the study and control groups. Conclusion: Use of a nerve stimulator did not aid in anatomical dissection of the RLN and was useful in identifying only one EBSLN. Discontinuous nerve monitoring by stimulation during total thyroidectomy confers no obvious benefit for the experienced surgeon in nerve identification, functional testing or injury prevention. Using the rate of respiration to monitor events in the infection of Escherichia coli cultures by bacteriophage T4

Abstract The growing interest in applications of bacteriophages creates a need for improvements in the production processes. Continuous monitoring of the phage production is an essential aspect of any control strategy and, at present, there is no completely satisfactory option. The approach presented here uses IR-spectrometry to continuously measure the rate of respiration (CO2 released) of Escherichia coli infected by phage T4 at various multiplicities of infection (MOI). Within the trends in these data, or in other aspects of the rate of respiration, it was possible to reliably and reproducibly identify five features that reflected specific events in the infection process. These included two events in the host cell apparent growth rate and events in the magnitude of the host cell density, in the measurement of OD600 or in the specific rate of respiration. All of these correlations were within 95% confidence showing that they are suitable for the monitoring and control of E. coli populations infected by phage T4. Continuous Subcutaneous Glucose Monitoring System in diabetic mothers during labour and postnatal glucose adaptation of their infants

Abstract Aims To assess a new technique for continuous monitoring of glucose concentration during labour in diabetic mothers. A second objective was to study maternal glucose levels in relation to postnatal glucose adaptation and the need for intravenous (IV) glucose treatment in the newborn infant. Methods Fifteen pregnant women with insulin-treated diabetes mellitus participated in this prospective pilot study. To measure their glucose control during labour we used the Continuous Subcutaneous Glucose Monitoring System (CGMS; Medtronic, Minneapolis, MN, USA) to calculate the mean glucose concentration and the area under the curve (AUC) in the last 120 min before delivery. All infants of these women were transferred to the neonatal care unit for early oral feeding and blood glucose measurements up to 14 h after delivery. Infants received IV glucose if blood glucose values were repeatedly < 2.2 mmol/l. Results All women coped well with the CGMS monitoring. AUC 0,120 min before delivery, mean glucose concentration 0,120 min before delivery and cord plasma insulin level were all significantly associated with the need for IV glucose in the newborn children. Conclusions In this study we found an association between maternal glucose concentrations during labour and postnatal glucose adaptation and need for IV glucose treatment in the infants. Online monitoring of glucose levels during delivery might help us to achieve maternal normoglycaemia and further reduce the risk of postnatal hypoglycaemia in the offspring. [source]


Temporal and spatial variations in periglacial soil movements on alpine crest slopes

Abstract This paper describes up to ten years of continuous monitoring of frost heave, creep and associated parameters on high mountain crest slopes in the Japanese and Swiss Alps, aiming to evaluate spatial and interannual variations in the rates and controls of soil movement. Shallow frost creep re,ecting diurnal frost heave activity dominates the crest slopes that lack a vegetation mat and have a thin debris mantle with good drainage. Seasonal frost heave activity can induce slightly deeper movement where ,ne soil exists below the depth reached by diurnal freeze,thaw penetration, although the shallow bedrock impedes movements below 20 cm depth. As a result, downslope velocity pro,les display strong concavity with surface velocities of 2,50 cm a,1. The frost creep rates vary spatially, depending on the soil texture, slope gradient, frequency of temperature cycling across 0 °C and moisture availability during freeze,thaw periods. Soil movements recur in every freeze,thaw period, although with some interannual variations affected by the length of seasonal snow cover and the occurrence of precipitation during freeze,thaw periods. "Hands-Free" Continuous Transthoracic Monitoring of Pericardiocentesis Using a Novel Ultrasound Transducer

Background: Pericardiocentesis can be monitored with a hand-held transducer. The purpose of this study was to assess the feasibility of monitoring pericardiocentesis using a novel ultrasound transducer, which can be attached to the chest wall, developed in our laboratory (CONTISON). Methods: We studied nine patients with large pericardial effusions. The 2.5-MHz transducer is spherical in its distal part and mounted in an external housing to permit steering in 360 degrees. The external housing is attached to the chest wall using an adhesive patch. The CONTISON transducer was placed at the cardiac apex and an apical four-chamber view obtained. Pericardiocentesis was performed from the subcostal position. The pericardial effusion was continuously imaged. Mitral inflow velocity signals were recorded before and after pericardiocentesis. When fluid was first obtained, 50 mL of fluid were discarded after which 5 mL of agitated saline was injected through the needle. Results: In the first patient the pericardiocentesis needle was seen in the left ventricular cavity. Saline injection produced a contrast effect in the left ventricle. The needle was gradually withdrawn until contrast was seen in the pericardial sac. A total of 1100 mL was removed without further complications. The second patient had clear fluid followed by blood stained aspirate. The echocardiogram revealed gradual appearance of granular echoes within the pericardial sac, suggestive of intrapericardial clot that was subsequently surgically evacuated. In the remaining seven patients, agitated saline produced a contrast effect in the pericardial sac indicative of proper needle position. Mitral flow velocity paradoxus was noted in five patients, and it resolved after pericardiocentesis in four patients. No adjustment of the transducer was required. Conclusion: The CONTISON transducer permitted continuous monitoring of pericardiocentesis. This technique could potentially facilitate pericardiocentesis. Current-Free Deposition of Prussian Blue with Organic Polymers: Towards Improved Stability and Mass Production of the Advanced Hydrogen Peroxide Transducer

Abstract We report on a novel approach for open-circuit (current-free) deposition of Prussian blue. Synthesis of Prussian blue is carried out by reduction of ferric ferricyanide with organic molecules, which are known to form polymers upon oxidation. The proposed interfacial deposition is a result of the synthesis in the presence of electrode support due to preconcentration of precursors at the interface. The resulting modified electrodes displayed the dramatically improved operational stability in hydrogen peroxide continuous monitoring with no loss of either electrochemical or analytical performance characteristics compared to electrodes with electrochemically deposited Prussian blue. The shown possibility to deposit stable films with regular structure in the absence of any external voltage could open new horizons for sensor science. [source]


Development of a Novel Automatic Potentiometric System for Determination of Selenium and Its Application in Pharmaceutical Formulations and Anodic Slime

Abstract Poly(vinyl chloride) polymeric membrane sensors containing Sn(IV) phthalocyanine dichloride (SnPC) and Co(II) phthalocyanine (CoPC) as novel electroactive materials dispersed in o -nitrophenyl octylether (o -NPOE) as a plasticizer are examined potentiometrically with respect to their response toward selenite (SeO32,) ions. Fast Nernstian response for SeO32, ions over the concentration ranges 7.0×10,6,1.0×10,3 and 8.0×10,6,1.0×10,3 mol L,l at pH,3.5,8.5 with lower detection limit of 5.0×10,6 and 8.0×10,6 mol L,1 and calibration slopes of ,25.4 and ,29.7,mV decade,1 are obtained with SnPC and CoPC based membrane sensors, respectively. The proposed sensors reveals by the modified separate solution method (MSSM) a good selectivity over different anions which differ significantly from the classical Hofmeister series. A segmented sandwich membrane method is used to determine complex formation constants of the ionophores in situe in the solvent polymeric sensing membranes. Membrane incorporating CoPC in a tubular flow detector is used in a two channels flow injection set up for continuous monitoring of selenite at a frequency of ca. 50 samples h,1. Direct determination of selenium in pharmaceutical formulations and anodic slime gives results in good agreement with data obtained using standard ICP method. [source]

Integration of continuous-flow sampling with microchip electrophoresis using poly(dimethylsiloxane)-based valves in a reversibly sealed device

Abstract Here we describe a reversibly sealed microchip device that incorporates poly(dimethylsiloxane) (PDMS)-based valves for the rapid injection of analytes from a continuously flowing stream into a channel network for analysis with microchip electrophoresis. The microchip was reversibly sealed to a PDMS-coated glass substrate and microbore tubing was used for the introduction of gas and fluids to the microchip device. Two pneumatic valves were incorporated into the design and actuated on the order of hundreds of milliseconds, allowing analyte from a continuously flowing sampling stream to be injected into an electrophoresis separation channel. The device was characterized in terms of the valve actuation time and pushback voltage. It was also found that the addition of sodium dodecyl sulfate (SDS) to the buffer system greatly increased the reproducibility of the injection scheme and enabled the analysis of amino acids derivatized with naphthalene-2,3-dicarboxaldehyde/cyanide. Results from continuous injections of a 0.39,nL fluorescein plug into the optimized system showed that the injection process was reproducible (RSD of 0.7%, n,=,10). Studies also showed that the device was capable of monitoring off-chip changes in concentration with a device lag time of 90,s. Finally, the ability of the device to rapidly monitor on-chip concentration changes was demonstrated by continually sampling from an analyte plug that was derivatized upstream from the electrophoresis/continuous flow interface. A reversibly sealed device of this type will be useful for the continuous monitoring and analysis of processes that occur either off-chip (such as microdialysis sampling) or on-chip from other integrated functions. Microautosamplers for discrete sample injection and dispensation

Abstract Microfluidic systems show considerable potential for use in the continuous reaction and analysis of biosamples for various applications, such as drug screening and chemical synthesis. Typically, microfluidic chips are externally connected with large-scale autosamplers to inject specific volumes of discrete samples in the continuous monitoring and analysis of multiple samples. This paper presents a novel microelectromechanical system (MEMS)-based autosampler capable of performing the discrete injection and dispensation of variable-volume samples. This microdevice can be integrated with other microfluidic devices to facilitate the continuous monitoring and analysis of multiple biosamples. By means of electroosmotic focusing and switching controlled by the direct application of electric sources on specific fluid reservoirs, a precise sample volume can be injected into the specified outlet port. Fluorescence dye images verify the performance of the developed device. An injection-and-washing scheme is developed to prevent cross-contamination during the continuous injection of different samples. This approach renders feasible the injection of several discrete samples using a single microchip. Compared to its large-scale counterparts, the developed microautosampler is compact in size, has low fabrication costs, is straightforward to control, and most importantly, is readily integrated with other microfluidic devices (e.g., microcapillary electrophoresis chips) to form a microfluidic system capable of the continuous monitoring and analysis of bioreactions. The proposed microautosampler could be promising towards realizing the micrototal analysis system (,-TAS) concept. [source]


Organochlorine contaminants in sea turtles: Correlations between whole blood and fat

Abstract Monitoring toxic organochlorine(OC)compounds is an important aspect in wildlife studies, especially in protected species such as sea turtles. The goal of this study was to determine whether blood OC concentrations can predict those in adipose tissue of sea turtles. Blood offers many benefits for monitoring OCs. It can be collected nondestructively from live turtles and can be sampled repeatedly for continuous monitoring. Organochlorine concentrations in blood may better represent the exposure levels of target tissues, but blood concentrations may fluctuate more than those in fatty tissues following recent dietary exposure or lipid mobilization. Paired fat and blood samples were collected from 44 live, juvenile loggerhead sea turtles and 10 juvenile Kemp's ridley sea turtle carcasses. Organochlorines were analyzed using gas chromatography with electron capture detection and mass spectrometry. Lipid-normalized OC concentrations measured in the blood significantly correlated to levels found in the fat samples of both species. This result suggests that sea turtle blood is a suitable alternative to fatty tissues for measuring OCs because blood concentrations reasonably represent those observed in the paired fat samples. However, blood OC concentrations calculated on a wet-mass basis were significantly and inversely correlated to lipid content in the fat samples. Therefore, caution should be used when monitoring spatial or temporal trends, as OC levels may increase in the blood following mobilization of fat stores, such as during long migrations, breeding, or disease events. Oxidative stress biomarkers in bivalves transplanted to the Guadalquivir estuary after Aznalcóllar spill

Abstract Biological effects of metals were studied in clams (Scrobicularia plana) transplanted to Guadalquivir estuary (Spain) at several times after the spill of toxic metals from Aznalcóllar pyrite mine (southwest Spain) (April 1998) using biochemical biomarkers responsive to reactive oxygen species. Significant As, Cd, Fe, Mn, Ni, and Pb uptake was found in clams living for seven months at the estuary (from July 1999). Increased activity of antioxidant (catalase, glucose-6-phosphate, and 6-phosphogluconate dehydrogenase) and glutathione-related (glutathione reductase and glyoxalase I and II) enzymes was also found after short exposures; the levels of malondialdehyde and metallothionein increased also, particularly with long exposures. Clams living four weeks at the estuary (from March 2000) but not at a reference site also accumulated metals. The higher malondialdehyde and lower reduced-glutathione levels and the more oxidized glutathione status confirmed the oxidative stress of clams living at the estuary, while no marked increase of antioxidant activities was found this time. Lower metal availability along the second transplant could explain the limited responses in this shorter experiment. Although the status of Guadalquivir estuary has recovered since Aznalcóllar spill, continuous monitoring is needed to confirm its progress and to be alert to possible deterioration after heavy rains. Estimation of net nitrogen flux between the atmosphere and a semi-natural grassland ecosystem in Hungary

The aim of this work is to estimate the net N balance (deposition , emission) between the atmosphere and a semi-arid, semi-natural grassland (Bugac station, Central Hungary, CarboEurope IP, NitroEurope IP level 3 site). Dry deposition of N compounds has been determined by the inferential method, based on continuous monitoring of NO2 gas and daily 24-hour concentration measurements of HNO3 vapour, NH3 gas, and NH4+ and NO3, particles, using dry deposition velocities from the literature, measured above surfaces with the same characteristics as Bugac station. The bi-directional flux of NH3 within the atmosphere and the canopy (excluding soil emission) has also been estimated by the inferential method. Wet deposition of nitrate and ammonium ions was calculated on the basis of daily precipitation sampling and concentration measurements of nitrate and ammonium ions. To estimate the soil-atmosphere exchange of different gaseous N forms (N2, NO, N2O, NH3), the DNDC model was used as validated by the chamber measurements of NO and N2O soil emission fluxes. Soil emissions of NO and N2O have been determined by dynamic and static soil chamber methods, respectively. The measurement and modelling activity covers a complete year. Using the measured and modelled data, the calculated N balance at Bugac station between August 2006 and July 2007 is estimated at ,8.8 kg N ha,1 year,1 (deposition) as a sum of the deposition and emission terms (,10.4 and 1.6 kg N ha,1 year,1, respectively). Due to the warm and dry weather during the examined period, wet fluxes were substantially lower than usual, which may also have altered the regular yearly course of dry deposition and emission. Geochemistry of the Bagnères-de-Bigorre thermal waters from the North Pyrenean Zone (France)

Abstract Thermal springs are poorly known in the sedimentary sites of the Pyrenees. In this paper, we describe the ,Bagnères-de-Bigorre' springs which occur in a remarkably active seismotectonic context. A chemical and isotopic study of 15 spring waters (both cold and thermal, ranging in temperature from 7.0 to 49.9°C), and continuous monitoring of a single spring allow us to characterise water,rock interactions, fluids paths and mixing processes. Three groups of waters are distinguished: (I) SO42+,Ca2+,Cl, thermal waters (II) SO42+,Cl,,Ca2+ thermal waters and (III) HCO3,,Ca2+ cold shallow waters. Their characteristics suggest interactions with Mesozoic evaporite and carbonate formations. O and D isotopes from thermal waters indicate a local meteoric origin of Atlantic signature and a recharge elevation of 800 to 1000 m, which corresponds to a single feeding area. Their ,13C values (,2.8 to ,9.6,) are consistent with carbonate dissolution, slight fractionation and a surficial organic input leading to ,13C depletion. Sr isotopes (0.70751 to 0.70777), Na+/Cl, and (Ca2+ + Mg2+)/SO42, ratios as well as thermodynamic calculations show that the dissolution of anhydrite and halite-bearing Triassic layers control the chemical composition of group-I and -II waters. The contrasting trends of cation/Cl, ratios and TDS of waters from groups I and II suggest the existence of two different circulation paths at depth as well as dilution with surficial waters similar to group III. Calculated mixing proportions show that three waters from group I are diluted from 17 to 66%, whereas all waters from group II are mixed. The aquifer temperature is estimated to be in the range 55,64°C using the retrograde and prograde solubilities of anhydrite and chalcedony, respectively. Accordingly, the mean depth of the reservoir is around 1.7 km, which allows us to constrain the depth of the Triassic layer. [source]


Multiyear to daily radon variability from continuous monitoring at the Amram tunnel, southern Israel

SUMMARY Radon is a naturally occurring radioactive noble gas generated within mineral grains of uranium bearing rocks by alpha decay from radium. The Amram tunnel (A. Bloch Geophysical Observatory) is a particularly suitable location for the investigation of radon variability. Located in the arid environment of the Arava desert, near Elat, the 170 m tunnel that constitutes the observatory enables radon monitoring in a desert environment and under fairly stable environmental conditions. The analysis of the temporal variability of continuous measurements of radon and environmental parameters at the Amram tunnel over a period of several years shows a complex temporal pattern characterized by non-stationary and multiscale features. Radon concentrations exhibit multiyear variability in the form of a increasing trend of ,1000 Bq m,3 yr,1 in the mean and much larger trends up to ,2500 Bq m,3 yr,1 in the maximum radon levels. Radon concentrations also display strong seasonal patterns, with maxima in summer and minima in winter, ranging from 2.5 kBq m,3 in winter to 35 kBq m,3 in summer. Intraseasonal variability is characterized by very large radon anomalies, with sharp increases of more than 20 kBq m,3 relative to the base level, that occur in spring and summer and last for several days. Daily periodic variability with maxima around midnight appears also in spring and summer, being absent in the cold months. Radon variability at seasonal, intraseasonal and daily timescales is associated with the air temperature outside the tunnel, specifically the temperature gradient between the external environment and the more stable environment inside the tunnel where the measurements are performed. Electrophysiologic characteristics and radiofrequency ablation of focal atrial tachycardia arising from para-Hisian region

Summary This study describes the electrophysiologic characteristics and radiofrequency ablation of focal atrial tachycardia (AT) arising from para-Hisian region in 14 (6.0%) patients of a consecutive series of 224 patients patients. Inverted or biphasic P wave in V1 and uncharacteristic P wave in inferior leads were observed during tachycardia, suggesting that there isn't a characteristic P-wave morphology for para-Hisian AT. During electrophysiological study, tachycardia could be induced with programmed atrial extrastimuli in 11 patients while a spontaneous onset and offset with ,warm-up and cool-down' phenomenon were seen in other three patients. Moreover, the tachycardias were sensitive to intravenous administration of adenosine triphosphate in all patients. On the basis of these findings, the mechanism is suggestive of triggered activity or micro-reentry, but automaticity cannot be conclusively excluded. Radiofrequency energy was delivered to the earliest site of atrial activation during AT. Ablating energy was carefully titrated, starting at 5 W and increasing gradually upto a maximum of 40 W, to achieve the ceasing of tachycardia. The long-term outcome was a 100% success rate in these 14 patients and there were no irreversible complications associated with ablation. Thus, the mapping and ablation of focal AT arising from para-Hisian region is safe and effective, delivery of radiofrequency energy in a titrated manner and continuous monitoring of atrioventricular (AV) conduction advocated to minimise the risk of damage to the anterograde AV conduction. Detection of potentially enterotoxigenic food-related Bacillus cereus by PCR analysis

Summary This study provides data on the prevalence of potentially pathogenic Bacillus cereus in foods from catering kitchens by evaluating the occurrence of B. cereus and the presence of virulence-associated genes. B. cereus was detected in 72/250 (28.8%) food samples. Specifically, B. cereus was highlighted in 34/74 (45.9%) pastries, 16/40 (40%) rice samples, 4/38 (10.5%) potato meals, 6/54 (11.1%) mozzarella samples and 12/44 (27.3%) meat meals. PCRs aimed at the hbl (C, D, A, B), nhe (A, B, C), bceT and cytK genes demonstrated a widespread distribution of the toxin-encoding genes among B. cereus isolates. The results highlight the frequent failure of control measures in catering kitchens and the need for intensive and continuous monitoring in order to assess the human health risk, as proposed by Regulation (EC) no. 1441/2007 on microbiological criteria for foodstuffs. Abstract Title.,Continuous monitoring of interface pressure distribution in intensive care patients for pressure ulcer prevention. Aim., This paper is a report of a study conducted to examine whether continuous interface pressure monitoring of postoperative patients in an intensive care unit is feasible in clinical practice. Background., The interface pressure between skin and surfaces is generally evaluated for pressure ulcer prevention. However, the intensity and duration of interface pressure necessary for pressure ulcer development remains unclear because the conventional interface pressure sensors are unsuitable for continuous monitoring in clinical settings. Methods., A total of 30 postoperative patients in an intensive care unit participated in this study in 2006,2007. A sensor was built into a thermoelastic polymer mattress. The whole-body interface pressure was recorded for up to 48 hours. Pressure ulcer development was observed during the morning bed-bath. For analysis, the intensity and duration of the maximal interface pressure was evaluated. Findings., The mean age of the study group was 62·0 ± 15·4 years. Two participants developed stage I pressure ulcer and blanchable redness at the sacrum. The longest duration of pressures greater than 100 mmHg were 487·0, 273·5 and 275·7 minutes in the pressure ulcer, blanchable redness and no redness groups respectively. Conclusion., Continuous monitoring of the intensity and duration of whole-body interface pressure using the KINOTEX sensor is feasible in intensive care patients. [source]


Safety of Deferring the Reimplantation of Pacing Systems After Their Removal for Infectious Complications in Selected Patients: A 1-Year Follow-Up Study

Introduction: Recent expert consensus guidelines mention that one of the principles for infected device replacement following removal is to "reevaluate carefully if there is a continued need for a new cardiac device replacement." This is a Class I recommendation, which nevertheless suffers from a very low level of evidence (level of evidence C), since no study has revisited the systematic practice of reimplanting the same device based on a meticulous clinical reassessment. In the present paper, we examined the safety of withholding the implantation of pacing systems in selected patients. Methods and Results: Between January 2005 and December 2007, 188 consecutive patients underwent extractions of infected pacing systems at 2 medical centers. "Low-risk" patients were identified by (1) a spontaneous heart rate >45 bpm, (2) no symptomatic asystole during monitoring, (3) QRS duration <120 ms when history of AV block was noted, (4) no high-degree AV block during continuous monitoring. They remained device-free, unless an adverse clinical event occurred mandating the reimplantation. The primary study endpoint was rate of sudden death and syncope after a 12-month follow-up. Among the 74 (39.4%) "low-risk" patients, a single patient suffered a bradycardia-related syncopal event corresponding to a 1.3% (95% CI, 0.0,3.9) rate of primary endpoint. Pacing systems were also reimplanted in 24 patients (32.4%) for syncope (n = 1), nonsevere bradycardia-reated symptoms (n = 17), cardiac resynchronization (n = 2), and for reassurance in 4 asymptomatic patients. Conclusion: After removal of infected pacing systems, these preliminary data demonstrated that a strategy of nonsystematic device reimplantation associated with close surveillance was safe in "low-risk" patients, allowing the administration of antimicrobials in a device-free state. Exclusive use of acid citrate dextrose for anticoagulation during extracorporeal photopheresis in patients with contraindications to heparin: An effective protocol,

Abstract Extracorporeal photopheresis (ECP) routinely uses heparin for anticoagulation. For patients with contraindications to heparin, alternative anticoagulation using acid citrate dextrose (ACD-A) has been reported to be safe and effective. However, detailed ECP protocols that exclusively use ACD-A anticoagulation and minimize citrate toxicity have not yet been published. We report a protocol that completely replaces heparin with ACD-A for ECP, which was developed at the University of California, Los Angeles (UCLA), and our experience since its implementation. ECP was performed with the UVAR XTS photopheresis system using ACD-A and control of the rate of citrate infusion. Calcium gluconate solution was administered prophylactically and as needed for symptoms of citrate toxicity. The medical records of patients who underwent ECP using the ACD-A protocol between January 2003 and July 2006 were reviewed. The incidence and severity of citrate toxicity and the technical data for all procedures were analyzed. During this period, 94 ECP procedures were performed with ACD-A anticoagulation on five patients. All patients tolerated the procedures well without significant complications. Only minimal symptoms of citrate toxicity (grade 1) were observed in 24.5% of all procedures; symptoms resolved promptly following administration of additional calcium gluconate. In conclusion, an effective protocol for ECP using ACD-A anticoagulation exclusively in patients with contraindications to heparin employs continuous monitoring of flow rates and prophylactic administration of calcium gluconate to minimize citrate toxicity. A portable system for continuous monitoring of bird nests using digital video recorders

ABSTRACT A variety of photographic methods have been described for monitoring nest predation. All have limitations for studying active nests in remote situations, such as size, expense, volume of data recorded, and types of trigger mechanisms. We developed a digital video surveillance system using infrared cameras to monitor predation at bird nests. The main advantage of this system over other video recorders is the small size of the recorder that can run continuously at 29 frames/s for more than 3 days. The recorder's built-in monitor makes it more transportable and allows for easy setup. Digital data is compact, can be reviewed quickly, and requires less physical storage space than videotapes. We recorded nest predation by mammals, birds, and snakes as well as egg and nestling losses not caused by predation. System failure rates were low and the total cost was comparable to ($700 US) video cassette recorders that are often used to monitor nests. Todos los métodos tienen limitaciones para estudiar nidos activos a distancia, como su tamaño, costo, volumen de los datos grabados y mecanismos para activar el equipo.
