Paraneoplastic pemphigus/paraneoplastic autoimmune multiorgan syndrome

Paraneoplastic pemphigus is the term used for an exclusive subset of pemphigus. The clinical lesions may resemble pemphigus, pemphigoid, erythema multiforme, graft-vs.-host disease, or lichen planus. A common denominator in all patients is the concomitant occurrence of either occult or confirmed systemic neoplasm. It is imperative to confirm the diagnosis through microscopy, where intraepidermal suprabasal cleavage, epidermal acantholysis, dyskeratotic keratinocytes and vacuolar changes in the basal epidermis, interfacial dermatitis, and epidermal exocytosis can be seen. Furthermore, the deposition of immunoglobulin G (IgG) and complement in the epidermal intercellular spaces, detected by direct and/or indirect immunofluorescence, is equally crucial for confirming the diagnosis. Experiences of loss and chronic sorrow in persons with severe chronic illness

Aims and objectives., The aims of the present study were to describe losses narrated by persons afflicted with severe chronic physical illness and to identify the concomitant occurrence of chronic sorrow. Background., Reactions connected with repeated losses are referred to in the literature as chronic sorrow, which has recently been described in conjunction with chronic illness. Design., A qualitative study with an abductive approach of analysis, including both inductive and deductive interpretations. Method., The study is based on 30 persons of working age with average disease duration of 18 years. The average age was 51 years. All of the persons had personal assistance for at least three months because of considerable need for help in daily life due to physical disability. Each person was interviewed twice. There was also an independent assessment of the deductive results concerning chronic sorrow. Results., The inductive findings show that all persons had experienced repeated physical, emotional and social losses. Most common were ,Loss of bodily function', ,Loss of relationship', ,Loss of autonomous life' and ,Loss of the life imagined'. ,Loss of identity' included the loss of human worth, dignity and a changed self-image. In addition, the deductive findings suggest that chronic sorrow exists in the study population. Sixteen of 30 participating subjects were assessed by both assessors to be in a state of chronic sorrow and there was an especially high agreement with respect to one criterion of chronic sorrow ,Loss experience, ongoing or single event' (28 of 30 subjects). Conclusions., This study shows that persons with severe chronic illness often experience recurring losses. These experiences are consistent with the phenomenon of chronic sorrow. Relevance to clinical practice., Knowledge of the existence of chronic sorrow in persons with chronic illness will enable nurses to support these persons in a more sensitive and appropriate way. Curcuma Aromatica Inhibits Diabetic Nephropathy in the Rat

ABSTRACT:, To test the possible involvement of reactive oxygen species (ROS) in the etiology of diabetic complications and therapeutic potential of antioxidant biofactors, we studied the effects of Curcuma aromatica (C. aromatica) on the pathologic events in streptozotocin-treated diabetic rats. Administration of streptozotocin (100 mg/kg, i.p.) increased plasma levels of glucose, triglyceride, cholesterol, urea nitrogen (BUN), creatinine, and lipid peroxidation products but decreased plasma albumin levels and suppressed the growth of animals. Histological examination revealed a marked injury in renal glomeruli and proximal tubules with concomitant occurrence of 8-hydroxy-2,-deoxyguanosine (8-OHdG) and mitochondrial 4-hydroxy-2-nonenal (4-HNE). Urinary excretion of 8-OHdG was also increased in streptozotocin-treated animals. Administration of streptozotocin decreased the mitochondrial localization of both Cu/Zn-superoxide dismutase (SOD) and cytochrome C in the kidney without affecting the localization of Mn-SOD. When animals were given 1.5%C. aromatica- containing diet for 1 wk before and 8 wk after administration of streptozotocin, all the events induced by streptozotocin except for hyperglycemia decreased markedly. Thus, C. aromatica may have therapeutic potential for the prevention of hyperglycemia-associated diabetic complications. [source]


Detection of human papilloma virus subtypes 16 and P16ink4a in invasive squamous cell carcinoma of the fallopian tube and concomitant squamous cell carcinoma in situ of the cervix

Abstract Squamous cell carcinoma (SCC) of the fallopian tube is rare and often diagnosed postoperatively. Cervical cancer is considered as a long-term sequaele, resulting from sexual transmitted infection with certain common high-risk human papilloma virus (HPV) types. The role of human papilloma virus in the development of the tubal SCC is unknown. We report an unusual case of SCC of the fallopian tube, synchronously occurring with cervical SCC in situ in a 49-year-old patient. Histological examination of the entire endometrium revealed no involvement. Both tubal and cervical lesions showed the presence of high risk HPV 16 by PCR and increased expression of p16INK4a protein. Both SCC of the fallopian tube and cervical SCC in situ were positive for p63, while the non-involved tubal epithelium was positive for WT-1, but negative for p63. In conclusion, the concomitant occurrence of fallopian tube and cervical SCC can be explained by: (i) the ,field effect' of HPV infection resulting in the concomitant development of primary SCC in various sites of the female genital tract; (ii) the primary fallopian tube SSC metastasizing to the uterine cervix; or (iii) primary cervical SCC metastasizing to the fallopian tube. The detection of HPV 16 and p16INK4a in both the fallopian tube and cervical SCCs strengthens the hypothesis of the ,field effect' of HPV infection. [source]


Sequential Pancreaticoduodenectomy after Living Donor Liver Transplantation for Cholagiocacinoma

Liver transplantation (LT) for patients with primary sclerosing cholangitis (PSC) is often contraindicated due to concomitant occurrence of cholangiocarcinoma (CC). Cases of simultaneous pancreaticoduodenectomy (PD) with LT have been sporadically reported; however, the applicability of such an invasive procedure to patients with CC has not been validated. We report here a case of sequential PD performed 44 days after a successful living donor liver transplantation (LDLT) using a left lobe graft. Although a clear pancreatic juice leakage through the drain persisted for days after surgery, the patient recovered from the complication and was discharged 32 days after the procedure. Currently, 1 year after LDLT, the patient is doing well with no evidence of recurrence. In conclusion, a sequential PD following LDLT is a safe and feasible option to treat CC complicating PSC. Long-term follow-up and accumulation of cases are necessary to evaluate the effectiveness of this procedure for this complicated disease. [source]
