NEDERGAARD The aim of this study was to present a method to obtain basic biological data on the in situ cellular immune response towards cancer. Using stereology, we estimated the density and frequency of immune cells of 10 different phenotypes in cone biopsies from 20 patients with FIGO stage I cervical squamous cell carcinoma. The anatomic distribution of immune cells with respect to intraepithelial, periepithelial or stromal compartments was recorded in normal epithelium, dysplastic epithelium and carcinoma. We estimated the number of immune cells per cancer cell, and the 3D total number of immune cells, inside cancer tissue. The tumor volume was estimated in 3D and corrected for shrinkage occurring during tissue processing. We found more immune cells in cancer compared to dysplasia and normal epithelia. A median total number of 278 ? 103 CD3+, 69.1 ? 103 CD4+ and 113 ? 103 CD8+ cells were present in the cancers. A median number of 63 CD3+, 11 CD4+ and 29 CD8+ cells were present per cancer cell. The average volume of tumors in stage IA was significantly smaller than that of stage IB. This method was found to be usable and of potential value in clinical pathology research, and for development and evaluation of immunotherapy. Prendiville The treatment of squamous cervical intraepithelial neoplasia is to remove or destroy the transformation zone (TZ). It is likely that no method of treatment is superior to another if it is performed properly and the limited available evidence supports this view. The significant advantages of excision (simplicity, cost, outpatient procedure, histological examination of the entire TZ) mean that treatment thresholds may have lowered over the last decade. Long-term pregnancy-related morbidity associated with excision has been reported recently. The evidence would suggest that this increase equates to a genuine increase in serious adverse outcome for cone biopsy but not large loop excision of the transformation zone (LLETZ). The available data also point to an increase in both incomplete excision and premature labour associated with the excision of large endocervical TZs. The clinical implications arising from this are firstly that women with large type 2 and 3 TZs need appropriate counselling before treatment and that the threshold for treating young women with mild abnormalities needs review. [source] Lethal endometrial recurrence after cone biopsy for microinvasive cervical adenocarcinomaJOURNAL OF OBSTETRICS AND GYNAECOLOGY RESEARCH (ELECTRONIC), Issue 3 2008Piksi Singh Abstract The follow up of microinvasive cervical adenocarcinoma treated conservatively is difficult because recurrences are often out of range and Pap smears difficult to interpret. A 30-year-old woman with a microinvasive adenocarcinoma with clear, but narrow, margins on cone biopsy was treated conservatively. After 2 years of close follow up in which no recurrence was detected, dilatation and curettage performed for infertility revealed adenocarcinoma invading secretory endometrium. The carcinoma resembled her cervical adenocarcinoma histologically, immunohistochemically and was HPV DNA 16 positive. A radical hysterectomy showed carcinoma involving the entire endometrium and the uppermost 3 mm of the endocervical canal, but sparing the remainder of the cervix. The patient died of disseminated carcinoma at the age of 34 years. The location of the recurrence was the reason it escaped detection for so long despite close follow up. [source] |
