Applying circular posterior-hinged craniotomy to malignant cerebral edemas

Abstract Malignant brain edemas are often fatal, regardless of whether they are treated conservatively with sedation, blood pressure management, mannitol-therapy, hyperventilation and hypothermia, or non-conservatively with routine trepanation. Unfortunately, temporal trepanation may result in significant brain damage through herniation of the cerebrum at the edges of the trepanation openings. In one case of a 26-year-old male with severe head injury, a circular posterior-hinged craniotomy (CPHC) was performed after an ineffective unitemporal trepanation for evacuation of an acute subdural hematoma. This ultimately successful operation prompted experimental and morphologic investigations on a new surgical procedure for lowering intracranial pressure (ICP). In 12 of 15 human cadavers, an experimentally ICP was lowered by a CPHC with between 9,21 mm of frontal elevation of the calvaria. Using computer simulation, the frontal elevations of the calvaria were "virtually" performed on 3D reconstructions from CT scans of skulls, and the intracranial volume gained was measured with a computer software program. The volume increase of the cranial cavity showed a relatively constant relation to the cranial capacity and was increased by 6.0% (±0.4%) or 78 cm3 with a 10 mm elevation and by 12.4% (±0.7%) or 160 cm3 with a 20 mm elevation. There were no significant differences with skulls of different ages or ethnic origin; however, a significant effect of gender (F = 7.074; P , 0.013) on the gained volume in percent of the cranial capacity for the 20 mm elevation was observed. Speech adaptation after treatment of full edentulism through immediate-loaded implant protocols

Abstract Objectives: The objective of the present study was to evaluate the influence of immediate loading of implants on speech adaptation. Material and methods: Ten patients (mean age 54, 6 females) were examined before surgery and 1, 3, 6 and 12 months afterwards. Articulation analysis was done using objective DAT-recoded data evaluated by two groups of speech and language therapists and a computer software program. Besides, patient VAS-scores, myofunctional problems and hearing impairment were recorded and analysed. Results: In the present study only one patient suffered from deteriorated speech after immediate loading. Other patients showed unaffected or improved articulation 3 to 6 months after surgery with a strident and interdental pronunciation mostly becoming addental. Furthermore, myofunctional problems occurred in one patient, other patients adapted to the new situation after three months. Hearing impairment did not influence speech pathology in this study. Conclusion: Immediate loading of oral implants does not seem to compromise the normal 3,6 months speech adaptation period. Whether such procedure presents advantages to the conventional 2-stage rehabilitation remains to be investigated. [source]


Computer-supported cognitive behavioral treatment of anxiety disorders

This article reviews the empirical literature pertaining to the use of computer-supported cognitive-behavioral treatment of anxiety disorders, including palmtop computers, virtual reality exposure therapy, and personal computer software programs. The advantages and disadvantages unique to each type of technology are described. The review concludes with a discussion of ethical issues, barriers to the use of technology by clinicians, and suggestions for a process by which scientists and practitioners can conceptualize how technology can advance our understanding of anxiety and our dissemination of effective treatments. © 2004 Wiley Periodicals, Inc. Using a Motion-Capture System to Record Dynamic Articulation for Application in CAD/CAM Software

Abstract Purpose: One of the current limitations of computer software programs for the virtual articulation of the opposing teeth is the static nature of the intercuspal position. Currently, software programs cannot identify eccentric occlusal contacts during masticatory cyclic movements of the mandible. Materials and Methods: Chewing trajectories with six degrees of freedom (DOF) were recorded and imposed on a computer model of one subject's maxillary and mandibular teeth. The computer model was generated from a set of high-resolution ,-CT images. To obtain natural chewing trajectories with six DOF, an optoelectronic motion-capturing system (VICON MX) was used. For this purpose, a special mandibular motion-tracking appliance was developed for this subject. Results: Mandibular movements while chewing elastic and plastic food samples were recorded and reproduced with the computer model. Examples of mandibular movements at intraoral points are presented for elastic and plastic food samples. The potential of such a kinematic computer model to analyze the dynamic nature of an occlusion was demonstrated by investigating the interaction of the second molars and the direction of the biting force during a chewing cycle. Conclusions: The article described a methodology that measured mandibular movements during mastication for one subject. This produced kinematic input to 3D computer modeling for the production of a virtual dynamic articulation that is suitable for incorporation into dental CAD/CAM software. [source]
