Selected AbstractsIs there a SSRI dose response in treating major depression?DEPRESSION AND ANXIETY, Issue 1 2003The case for re-analysis of current data, for enhancing future study design Abstract It has been widely stated that the available research data has not demonstrated a SSRI dose response for major depression. We re-evaluated the methods used to analyze the SSRI data by clarifying two key alternative definitions of dose response and their implications for enhancing analysis of currently available data as well as future study design. We differentiated "potential" dose response, which focuses exclusively on response excluding tolerability effects and asks whether differences in dose can result in significant differences in response, from "expressed" dose response, which incorporates all tolerability effects currently associated with dose (including those caused by study protocol or treatment practice) and asks whether differences in dose do result in significant differences in response. To analyze potential dose response for all studies, one should use a "dose-tolerant" sample, i.e., an ITT sample from which dropouts due to adverse events have been removed. To analyze an expressed dose response, an ITT sample is the optimum sample if the study conforms to several design specifications. In the absence of conformance to these specifications, an ITT sample may be an approximation of the appropriate sample. Given design limitations of currently available studies, a dose-tolerant sample may provide a more informative approximation of an optimal sample to be used in evaluating the expressed dose response that could be expected in the best clinical practice. Future studies of dose-response relations could be enhanced by taking into account the principles noted above, and currently available data should be reanalyzed based on these principles. [source] Genetic Probes of Three Theories of Maternal Adjustment: I. Recent Evidence and a Model,FAMILY PROCESS, Issue 3 2001David Reiss M.D. Studies focusing on genetic and social influences on maternal adjustment will illumine mother's marriage, parenting, and the development of psychopathology in her children. Recent behavioral genetic research suggests mechanisms by which genetic and social influences determine psychological development and adjustment. First, heritable, personal attributes may influence individuals' relationships with their family members. These genetically influenced family patterns may amplify the effects of adverse, heritable personal attributes on adjustment. Second, influences unique to siblings may be the most important environmental determinants of adjustment. We derive three hypotheses on maternal adjustment from integrating these findings from genetic studies with other contemporary research on maternal adjustment. First, mother's marriage mediates the influence of her heritable, personal attributes on her adjustment. Second, mother's recall of how she was parented is partially genetically influenced, and both her relationships with her spouse and her child mediate the impact of these genetically influenced representations on her current adjustment. Third, characteristics of mother's spouse are important influences on difference between her adjustment and that of her sister's These sibling-specific influences are unrelated to mother's heritable attributes. The current article develops this model, and the companion article describes the Twin Mom Study that was designed to test it as well, as its first findings. Data from this study can illumine the role of family process in the expression of genetic influence and lead to specific family interventions designed to offset adverse genetic influences. [source] I. Veterans Affairs Cooperative Study of Polyenylphosphatidylcholine in Alcoholic Liver Disease: Effects on Drinking Behavior by Nurse/Physician TeamsALCOHOLISM, Issue 11 2003Charles S. Lieber Background: This multicenter prospective, randomized, double-blind placebo-controlled trial was designed to evaluate the effectiveness of polyenylphosphatidylcholine against the progression of liver fibrosis toward cirrhosis in alcoholics. Seven hundred eighty-nine alcoholics with an average intake of 16 drinks per day were enrolled. To control excessive drinking, patients were referred to a standard 12-step,based alcoholism treatment program, but most patients refused to attend. Accordingly, study follow-up procedures incorporated the essential features of the brief-intervention approach. An overall substantial and sustained reduction in drinking was observed. Hepatic histological and other findings are described in a companion article. Methods: Patients were randomized to receive daily three tablets of either polyenylphosphatidylcholine or placebo. Monthly follow-up visits included an extensive session with a medical nurse along with brief visits with a study physician (hepatologist or gastroenterologist). A detailed physical examination occurred every 6 months. In addition, telephone consultations with the nurse were readily available. All patients had a liver biopsy before entry; a repeat biopsy was scheduled at 24 and 48 months. Results: There was a striking decrease in average daily alcohol intake to approximately 2.5 drinks per day. This was sustained over the course of the trial, lasting from 2 to 6 years. The effect was similar both in early dropouts and long-term patients, i.e., those with a 24-month biopsy or beyond. Conclusions: In a treatment trial of alcoholic liver fibrosis, a striking reduction in alcohol consumption from 16 to 2.5 daily drinks was achieved with a brief-intervention approach, which consisted of a relative economy of therapeutic efforts that relied mainly on treatment sessions with a medical nurse accompanied by shorter reinforcing visits with a physician. This approach deserves generalization to address the heavy drinking problems commonly encountered in primary care and medical specialty practices. BASKURT ABSTRACT Although the shear-dependent and reversible phenomenon of red blood cell (RBC) aggregation has been studied for decades, its role as a determinant of in vivo blood flow in both health and disease has not yet been fully documented. In this brief review, we present compelling arguments, supported by literature evidence, that in vivo flow dynamics are more affected by RBC aggregation than by RBC adhesion to endothelial cells (ECs). A companion article (i.e., a "counter-point") published in this issue of the journal argues that in disease states, RBC-EC adhesion is the more important determinant. [source] Comparative analysis of neonicotinoid binding to insect membranes: II.PEST MANAGEMENT SCIENCE (FORMERLY: PESTICIDE SCIENCE), Issue 10 2004An unusual high affinity site for [3H]thiamethoxam in Myzus persicae, Aphis craccivora Abstract Neonicotinoids represent a class of insect-selective ligands of nicotinic acetylcholine receptors. Imidacloprid, the first commercially used neonicotinoid insecticide, has been studied on neuronal preparations from many insects to date. Here we report first intrinsic binding data of thiamethoxam, using membranes from Myzus persicae Sulzer and Aphis craccivora Koch. In both aphids, specific binding of [3H]thiamethoxam was sensitive to temperature, while the absolute level of non-specific binding was not affected. In M persicae, binding capacity (Bmax) for [3H]thiamethoxam was ca 450 fmol mg,1 of protein at 22 °C and ca 700 fmol mg,1 of protein at 2 °C. The negative effect of increased temperature was reversible and hence not due to some destructive process. The affinity for [3H]thiamethoxam was less affected by temperature: Kd was ca 11 nM at 2 °C and ca 15 nM at 22 °C. The membranes also lost binding sites for [3H]thiamethoxam during prolonged storage at room temperature, and upon freezing and thawing. In A craccivora, [3H]thiamethoxam was bound with a capacity of ca 1000 fmol mg,1 protein and an affinity of ca 90 nM, as measured at 2 °C. Overall, the in vitro temperature sensitivity of [3H]thiamethoxam binding was in obvious contrast to the behaviour of [3H]imidacloprid studied in parallel. Moreover, the binding of [3H]thiamethoxam was inhibited by imidacloprid in a non-competitive mode, as shown with M persicae. In our view, these differences demonstrate that thiamethoxam and imidacloprid, which represent different structural sub-classes of neonicotinoids, do not share the same binding site or mode. Copyright © 2004 Society of Chemical Industry [source] A review of forecast error covariance statistics in atmospheric variational data assimilation.THE QUARTERLY JOURNAL OF THE ROYAL METEOROLOGICAL SOCIETY, Issue 637 2008I: Characteristics, measurements of forecast error covariances Abstract This article reviews the characteristics of forecast error statistics in meteorological data assimilation from the substantial literature on this subject. It is shown how forecast error statistics appear in the data assimilation problem through the background error covariance matrix, B. The mathematical and physical properties of the covariances are surveyed in relation to a number of leading systems that are in use for operational weather forecasting. Different studies emphasize different aspects of B, and the known ways that B can impact the assimilation are brought together. Treating B practically in data assimilation is problematic. One such problem is in the numerical measurement of B, and five calibration methods are reviewed, including analysis of innovations, analysis of forecast differences and ensemble methods. Another problem is the prohibitive size of B. This needs special treatment in data assimilation, and is covered in a companion article (Part II). Examples are drawn from the literature that show the univariate and multivariate structure of the B -matrix, in terms of variances and correlations, which are interpreted in terms of the properties of the atmosphere. The need for an accurate quantification of forecast error statistics is emphasized. I. Fisher Abstract The purpose of this article and a companion article is to explore a number of topics in Statistics in Business and Industry. This article sketches the history of Quality Management, from its emergence in the late 19th and early 20th centuries through to the present day. Particular emphasis is placed on activities in Japan immediately following the end of the Second World War, and subsequent developments elsewhere in the world. We draw a careful distinction between Quality Management and various methodologies that aid in its implementation, such as Six Sigma. In the words of one management practitioner, Norbert Vogel, ,TQM in its broadest sense examines all aspects of management and the alternative methodologies being promoted are merely sub-sets of what should be an integrated management system.' Scheithauer MD Abstract Controversy surrounds the recent 2007 WHO Classification of Tumours of the Nervous System. A number of nosologic issues remain to be resolved, some a reflection of conceptual disagreement, others the result of inadequate data to permit their definitive resolution. Among these and discussed herein are (i) the nosologic place of highly anaplastic oligoastrocytic tumors, (ii) the forms and significance of microvascular changes in high-grade gliomas, (iii) the makeup of the glioneuronal tumors category, (iv) the subclassification of pineal parenchymal tumors of intermediate type, and (v) the classification of principle forms of mesenchymal neoplasms, specifically hemangiopericytoma and solitary fibrous tumor.
