[source] Novel therapeutic approach to eradicate tyrosine kinase inhibitor resistant chronic myeloid leukemia stem cellsCANCER SCIENCE, Issue 7 2010Kazuhito Naka Although discovery of the tyrosine kinase inhibitor (TKI) imatinib mesylate has significantly improved the prognosis of chronic myeloid leukemia (CML) patients, a rare population of CML stem cells is known to be resistant to TKI therapy, causing recurrence of CML. However, recent progress in CML stem cell biology may present a novel therapeutic avenue for CML patients. In this review, we focus on mechanisms used by CML stem cells to maintain TKI-resistance. Comprehensive approaches including mouse genetics, prospective identification of CML stem cells, and syngenic transplantation techniques have identified several key molecules or signaling pathways, including hedgehog (Hh)/Smo, promyelocytic leukemia (PML), 5-lipoxygenase (5-LO), and forkhead box class O (FOXO), that function in CML stem cell maintenance. Inhibiting some of these factors in combination with TKI administration successfully antagonized resistance of CML stem cells to TKI therapy, resulting in efficient eradication of leukemia cells in vivo. Thus, development of methods that sensitize CML stem cells to TKI therapy may lead to novel therapies to treat CML patients. Johansson ABSTRACT The design of responsive distributed database systems is a key concern for information systems managers. In high bandwidth networks latency and local processing are the most significant factors in query and update response time. Parallel processing can be used to minimize their effects, particularly if it is considered at design time. It is the judicious replication and placement of data within a network that enable parallelism to be effectively used. However, latency and parallel processing have largely been ignored in previous distributed database design approaches. We present a comprehensive approach to distributed database design that develops efficient combinations of data allocation and query processing strategies that take full advantage of parallelism. We use a genetic algorithm to enable the simultaneous optimization of data allocation and query processing strategies. We demonstrate that ignoring the effects of latency and parallelism at design time can result in the selection of unresponsive distributed database designs. [source] Attention-like processes underlying optomotor performance in a Drosophila choice mazeDEVELOPMENTAL NEUROBIOLOGY, Issue 2 2007Bruno van Swinderen Abstract The authors present a novel paradigm for studying visual responses in Drosophila. An eight-level choice maze was found to reliably segregate fly populations according to their responses to moving stripes displayed on a computer screen. Visual responsiveness was robust in wild-type flies, and performance depended on salience effects such as stimulus color and speed. Analysis of individual fly choices in the maze revealed that stereotypy, or choice persistence, contributed significantly to a strain's performance. On the basis of these observations, the authors bred wild-type flies for divergent visual phenotypes by selecting individual flies displaying extreme stereotypy. Selected flies alternated less often in the sequential choice maze than unselected flies, showing that stereotypy could evolve across generations. The authors found that selection for increased stereotypy impaired flies' responsiveness to competing stimuli in tests for attention-like behavior in the maze. Visual selective attention was further investigated by electrophysiology, and it was found that increased stereotypy also impaired responsiveness to competing stimuli at the level of brain activity. Combined results present a comprehensive approach to studying visual responses in Drosophila, and show that behavioral performance involves attention-like processes that are variable among individuals and thus sensitive to artificial selection. © 2006 Wiley Periodicals, Inc. [source] Changes and predictors of change in the physical health status of heroin users over 24 monthsADDICTION, Issue 3 2009Anna Williamson ABSTRACT Aims (i) To describe the course of physical health among the ATOS cohort over 24 months; and (ii) to examine the effects of treatment, drug use patterns and social and psychological factors on health status over 24 months. Design Longitudinal cohort. Setting Sydney, Australia. Participants A total of 615 heroin users recruited for the Australian Treatment Outcome Study (ATOS). Findings The general health of the cohort improved significantly over 24 months. Significant predictors of poor health over 24 months were: being older, being female, past month heroin, other opiate and tobacco use, past month unemployment and current major depression. Spending a greater proportion of time in residential rehabilitation (RR) was associated with better health over 24 months. No other treatment factors demonstrated a significant, independent relationship with health. Conclusions The physical health of dependent heroin users is affected by drug use and psychosocial problems. RR treatment appears to be particularly beneficial to the health of heroin users, suggesting the importance of a comprehensive approach to improving health among this group. [source] Policy implications of the widespread practice of ,pre-drinking' or ,pre-gaming' before going to public drinking establishments,are current prevention strategies backfiring?ADDICTION, Issue 1 2009Samantha Wells ABSTRACT Aim To describe the research, policy and prevention implications of pre-drinking or pre-gaming; that is, planned heavy drinking prior to going to a public drinking establishment. Methods The authors describe the phenomenon of pre-drinking, motivations for pre-drinking and its associated risks using available research literature, media and popular internet vehicles. Results Heavy drinking prior to going out has emerged as a common and celebrated practice among young adults around the world. Apparent motivations are: (i) to avoid paying for high priced drinks at commercial drinking establishments; (ii) to achieve drunkenness and enhance and extend the night out; and (iii) to socialize with friends, reduce social anxiety or enhance male group bonding before going out. Limited existing research on pre-drinking suggests that it is associated with heavy drinking and harmful consequences. We argue that policies focused upon reducing drinking in licensed premises may have the unintended consequence of displacing drinking to pre-drinking environments, possibly resulting in greater harms. Conclusions Effective policy and prevention for drinking in licensed premises requires a comprehensive approach that takes into account the entire drinking occasion (not just drinking that occurs in the licensed environment), as well as the ,determined drunkenness' goal of some young people. Verwey Abstract Circadian rhythms in mammalian behaviour and physiology rely on daily oscillations in the expression of canonical clock genes. Circadian rhythms in clock gene expression are observed in the master circadian clock, the suprachiasmatic nucleus but are also observed in many other brain regions that have diverse roles, including influences on motivational and emotional state, learning, hormone release and feeding. Increasingly, important links between circadian rhythms and metabolism are being uncovered. In particular, restricted feeding (RF) schedules which limit food availability to a single meal each day lead to the induction and entrainment of circadian rhythms in food-anticipatory activities in rodents. Food-anticipatory activities include increases in core body temperature, activity and hormone release in the hours leading up to the predictable mealtime. Crucially, RF schedules and the accompanying food-anticipatory activities are also associated with shifts in the daily oscillation of clock gene expression in diverse brain areas involved in feeding, energy balance, learning and memory, and motivation. Moreover, lesions of specific brain nuclei can affect the way rats will respond to RF, but have generally failed to eliminate all food-anticipatory activities. As a consequence, it is likely that a distributed neural system underlies the generation and regulation of food-anticipatory activities under RF. Thus, in the future, we would suggest that a more comprehensive approach should be taken, one that investigates the interactions between multiple circadian oscillators in the brain and body, and starts to report on potential neural systems rather than individual and discrete brain areas. [source] Clinical methods for the evaluation of endothelial function , a focus on resistance arteriesFUNDAMENTAL & CLINICAL PHARMACOLOGY, Issue 3 2006Robinson Joannides Abstract Endothelial dysfunction is a key event in the pathophysiology of cardiovascular diseases and appears as a strong independent predictor of cardiovascular events. In this context, biological evaluation of endothelial circulating markers can be helpful. However, functional tests using pharmacological stimuli appear more specific for the study of resistance arteries. These methods consist in the evaluation of the endothelium-dependent changes in regional vascular flow in response to local infusion of substances that act through endothelial receptors without modification of systemic arterial pressure and in comparison with a non endothelium-dependent relaxation. Flow is measured by Doppler and intravascular ultrasound in coronary circulation, laser Doppler in skin and by venous occlusion plethysmography in peripheral muscular arteries. Similar studies can be performed ex vivo using isolated resistance arteries obtained from fat subcutaneous biopsies. In addition, other information can be obtained from reactive hyperemia and the study of the flow-mediated dilatation of conduit arteries to enable a selective and comprehensive approach of the heterogeneity of endothelial function in pathophysiology. Stone Abstract Haplotype-based association studies have been proposed as a powerful comprehensive approach to identify causal genetic variation underlying complex diseases. Data comparisons within families offer the additional advantage of dealing naturally with complex sources of noise, confounding and population stratification. Two problems encountered when investigating associations between haplotypes and a continuous trait using data from sibships are (i) the need to define within-sibship comparisons for sibships of size greater than two and (ii) the difficulty of resolving the joint distribution of haplotype pairs within sibships in the absence of parental genotypes. We therefore propose first a method of orthogonal transformation of both outcomes and exposures that allow the decomposition of between- and within-sibship regression effects when sibship size is greater than two. We conducted a simulation study, which confirmed analysis using all members of a sibship is statistically more powerful than methods based on cross-sectional analysis or using subsets of sib-pairs. Second, we propose a simple permutation approach to avoid errors of inference due to the within-sibship correlation of any errors in haplotype assignment. These methods were applied to investigate the association between mammographic density (MD), a continuously distributed and heritable risk factor for breast cancer, and single nucleotide polymorphisms (SNPs) and haplotypes from the VDR gene using data from a study of 430 twins and sisters. We found evidence of association between MD and a 4-SNP VDR haplotype. In conclusion, our proposed method retains the benefits of the between- and within-pair analysis for pairs of siblings and can be implemented in standard software. Genet. [source] Classifying MLH1 and MSH2 variants using bioinformatic prediction, splicing assays, segregation, and tumor characteristicsHUMAN MUTATION, Issue 5 2009Sven Arnold Abstract Reliable methods for predicting functional consequences of variants in disease genes would be beneficial in the clinical setting. This study was undertaken to predict, and confirm in vitro, splicing aberrations associated with mismatch repair (MMR) variants identified in familial colon cancer patients. Six programs were used to predict the effect of 13 MLH1 and 6 MSH2 gene variants on pre-mRNA splicing. mRNA from cycloheximide-treated lymphoblastoid cell lines of variant carriers was screened for splicing aberrations. Tumors of variant carriers were tested for microsatellite instability and MMR protein expression. Variant segregation in families was assessed using Bayes factor causality analysis. Amino acid alterations were examined for evolutionary conservation and physicochemical properties. Splicing aberrations were detected for 10 variants, including a frameshift as a minor cDNA product, and altered ratio of known alternate splice products. Loss of splice sites was well predicted by splice-site prediction programs SpliceSiteFinder (90%) and NNSPLICE (90%), but consequence of splice site loss was less accurately predicted. No aberrations correlated with ESE predictions for the nine exonic variants studied. Seven of eight missense variants had normal splicing (88%), but only one was a substitution considered neutral from evolutionary/physicochemical analysis. Combined with information from tumor and segregation analysis, and literature review, 16 of 19 variants were considered clinically relevant. Bioinformatic tools for prediction of splicing aberrations need improvement before use without supporting studies to assess variant pathogenicity. Classification of mismatch repair gene variants is assisted by a comprehensive approach that includes in vitro, tumor pathology, clinical, and evolutionary conservation data. [source] Detection of pathogenic gene copy number variations in patients with mental retardation by genomewide oligonucleotide array comparative genomic hybridization,,HUMAN MUTATION, Issue 11 2007Yao-Shan Fan Abstract Genomic imbalance is a major cause of developmental disorders. Microarray-based comparative genomic hybridization (aCGH) has revealed frequent imbalances associated with clinical syndromes, but also a large number of copy number variations (CNVs), which have complicated the interpretation of results. We studied 100 consecutive patients with unexplained mental retardation and a normal karyotype using several platforms of CGH arrays. A genomewide array with 44,290 oligonucleotide probes (OaCGH44K) detected imbalances in 15% of cases studied with sizes ranged from 459,kb to 19,Mb while revealing a small number of CNVs (0.72/individual). Another platform with ,240,000 oligonucleotide probes (OaCGH244K) revealed a large number of CNVs (20/individual) in selected cases and their normal parents. We used a comprehensive approach for interpreting the results of aCGH, including consideration of the size, inheritance and gene content of CNVs, and consultation with an online Database of Genomic Variants (DGV) and Online Mendelian Inheritance in Men (OMIM) for information on the genes involved. Our study suggests that genomewide oligonucleotide arrays such as the OaCGH44K platform can be used as a powerful diagnostic tool for detection of genomic imbalances associated with unexplained mental retardation or syndromic autism spectrum disorders. It is interesting to note that a small number of common variants were revealed by OaCGH244K in some study subjects but not in their parents and that some inherited CNVs had altered breakpoints. [source] A comprehensive approach to characterization of the nonlinearity of runoff in the headwaters of the Tarim River, western ChinaHYDROLOGICAL PROCESSES, Issue 2 2010Jianhua Xu Abstract Nonlinear characteristics of the runoff processes in the headwaters of the Tarim River were identified and evaluated using several selected methods, including wavelet analysis, correlation dimension, and R/S analysis. Time-series of annual data describing runoff, average temperature, and precipitation from 1957 to 2005 were used to construct and test empirical models. The primary findings of this study were as follows: (1) The annual runoff of the headwaters are complex and nonlinear in nature, and they each presented periodic, nonlinear trends at the chosen time scales, chaotic dynamics, and long-memory characteristics. (2) These nonlinear trends appeared to have resulted from the regional climatic changes that occurred during the study period. The periodicity of changes in runoff occurred on an approximately 25-year cycle, which appeared to be correlated with temperature and precipitation cycles. In addition, the annual runoff exhibited a significant, positive correlation with the temperature and precipitation factors at the 4-, 8-, 16-, and 32-year temporal scales. (3) The correlation dimensions of the attractor derived from the runoff time series for the Hotan, Yarkand, and Aksu rivers were all greater than 3·0 and non-integral, implying that all three rivers are dynamic chaotic systems that are sensitive to initial conditions, and that the dynamic modelling of their annual runoff requires at least four independent variables. (4) The computed Hurst exponents indicate that a long-term memory characteristic exists in the annual runoff processes. However, there were some differences observed, with the Aksu and Yarkand rivers demonstrating a persistent trait, and the Hotan River exhibiting an anti-persistent feature. Bodar Abstract A risk assessment on zinc and zinc compounds was carried out within the framework of Council Regulation 793/93/EEC on Existing Chemicals. This risk assessment basically followed the European Union (EU) technical guidance documents (TGDs). These TGDs are built on the current knowledge on quantitative risk assessments, mainly for organic chemicals. This article describes the tailor-made approach for the zinc risk assessment. This work lasted almost a decade and involved the contributions of all EU member states and industry, who discussed the risk assessment during technical meetings. The risk assessment is initially based on scientific findings but is interrelated with pragmatic considerations. It follows a comprehensive approach, covering both environmental and human health. In the environmental part, new methodologies were developed to deal with the natural background of zinc, essentiality, speciation, and the use of species sensitivity distributions. The major results and the process of drawing conclusions of the risk assessment are outlined: potential environmental risks of zinc and zinc compounds may occur at local and regional scales in surfacewater, sediment, and soil. No potential health risks were identified for consumers and man indirectly exposed via the environment. For workers, potential health risks were identified only for zinc oxide and zinc chloride. Kenen Reform of the international financial architecture has made progress but has not dealt decisively with the need to involve private sector creditors in resolving debt-related crises. It has relied unduly on voluntary approaches combined with large-scale official financing. A comprehensive approach requires the use of temporary standstills to protect debtors against litigation. These can help to resolve ,liquidity' crises as well as ,solvency' crises. Proposals by Krueger (2001, 2002) provide a way to resolve the problem of a sovereign debtor with an unsustainable debt burden but offer no solution to problems involving private sector debt or to liquidity crises. They would also require an amendment to the Articles of Agreement of the International Monetary Fund, which could prove difficult. This paper proposes a less radical approach , adding rollover clauses and collective-action clauses to sovereign and private debt contracts, backed by strict limits on IMF financing. It resembles, but goes further than, the contractual approach favoured by the US Treasury. Thielecke Summary The therapeutic management of the irritable bowel syndrome (IBS) is ineffective and not satisfying either patients or practitioners. Research in functions of the enteric nervous system and its interaction with the central nervous system is the basis for the development of emerging pharmaceuticals in therapy of the IBS. These pharmaceuticals include agents such as opioid agonists, psychotropic agents and particularly serotonin receptor modulators. These novel pharmaceuticals aim to provide a more comprehensive approach in the therapy of the IBS and will serve both patients and practitioners. So far, the US Food and Drug Administration has approved two agents specifically for the treatment of the IBS, both belonging to the group of serotonin receptor modulators. However, questions remain whether a single therapy is sufficient in the management of IBS because this disease is influenced by biological and psychological as well as cultural and social factors. In particular, our contribution is twofold: (i) we provide a formalism to deal with both qualitative and quantitative "periodicity-dependent" constraints between repeated events, considering user-defined periodicities as well; and (ii) we propose an intensional approach to temporal reasoning, which is based on the operations of intersection and composition. [source] A critical review of aspirin in the secondary prevention of noncardioembolic ischaemic strokeINTERNATIONAL JOURNAL OF STROKE, Issue 4 2010Domenico Inzitari Both secondary prevention (such as lifestyle modifications, pharmacotherapy or surgery) and an understanding of the influence of risk factors (including the different aetiologic mechanisms of cerebral ischaemia) play a pivotal role in reducing the burden of recurrent stroke. Regarding the types of preventative treatments available, variations exist across all clinical studies, including differences in target populations (including the type of cerebral ischaemia), risk factors, length of follow-up, drop-out rates and outcomes, which makes translating the results of clinical trials to individual patients difficult. However, with such limitations in mind, this critical albeit nonsystematic review, which compared aspirin with other antiplatelets and in combination with other drugs, showed that the benefit from aspirin treatment is consistently shown in ischaemic stroke, while harms are limited. Furthermore, no definite superiority is apparent across different antiplatelet therapies. Dual antiplatelet regimens may expose to a slight but measurable higher risk of haemorrhagic complications, perhaps in selective groups of patients (i.e. those with severe small-vessel disease or in selective racial groups). Based on our analysis, the indication of aspirin as the first-line choice, also recommended by several acknowledged international or national guidelines, may be confirmed. However, the complex nature of patients at risk of recurrent ischaemic stroke necessitates a comprehensive approach, which should be driven by the primary care physician, whose role is central to successful actions for secondary stroke prevention. The discussion was conducted in relation to sustainable development. Through the examination of the current situation and the challenges to sustainable development, and taking into account the imbalance in the distribution of the benefits of science and new technologies, the authors advocate a comprehensive approach to promote cooperation and capacity-building in this area. They argue that linkages should be adopted between micro-levels and macro-levels of analysis by elevating rights and related issues from individuals to the national level in the field of the right to science and technology, and from the national to the international level in the field of sustainable development in order to institutionalise and ensure individual and national rights to science, technology and sustainable development. Traditionally, understanding of maternal distress has been based on the medical model and dysfunction. The concept of maternal distress needs development so that it describes responses ranging from normal stress responses to those indicating mental health problem/s. Data sources., The SCOPUS, CINAHL and Medline databases were searched for the period from 1995 to 2009 using the keywords ,psychological distress', ,emotional distress' and ,maternal distress'. Review methods., Steps from Rodgers' evolutionary concept analysis guided the conduct of this concept analysis. Results., Four attributes of maternal distress were identified as responses to the transition to motherhood, with the level of each response occurring along a continuum: stress, adapting, functioning and control, and connecting. Antecedents to maternal distress include becoming a mother, role changes, body changes and functioning, increased demands and challenges, losses and gains, birth experiences, and changes to relationships and social context. The consequences of maternal distress are compromised mental health status, maternal role development, quality of life, ability to function, quality of relationships and social engagement. The extent of the impact depends on the level of maternal distress. Conclusion., Clearer interpretation of maternal distress offers a comprehensive approach to understanding maternal emotional health during the transition to motherhood. Acknowledging women's experiences and providing more appropriate support could alleviate some of the struggles and hardships experienced by mothers. HULME Summary 1Government departments, environmental managers and conservationists are all facing escalating pressure to address and resolve a diversity of invasive alien species (IAS) problems. Yet much research to date is primarily concerned with quantifying the scale of the problem rather than delivering robust solutions and has not adequately addressed all stages of the invasion process, and only a few studies embrace the ecosystem approach. 2Three successive steps, prevention, eradication and control, form the cornerstones of recommended best practices aimed at managing IAS. The goal of such actions is the restoration of ecosystems to preserve or re-establish native biodiversity and functions. 3Prevention is widely promoted as being a more environmentally desirable strategy than actions undertaken after IAS establishment, yet is hindered by the difficulty in separating invasive from non-invasive alien species. Furthermore, the high number of candidate IAS, the investment required in taxonomic support and inspection capacity, and the expense of individual risk assessments may act against the net benefits of prevention. More rewarding avenues may be found by pursuing neural networks to predict the potential composition of pest assemblages in different regions and/or model introduction pathways to identify likely invasion hubs. 4Rapid response should be consequent on early detection but, when IAS are rare, detection rates are compromised by low occurrence and limited power to discern significant changes in abundance. Power could be increased by developing composite indicators that track trends in a suite of IAS with similar life histories, shared pathways and/or habitat preferences. 5The assessment of management options will benefit from an ecosystem perspective that considers the manipulation of native competitors, consumers and mutualists, and reviews existing management practices as well as mitigates other environmental pressures. The ease with which an IAS can be targeted should not only address the direct management effects on population dynamics but also indirect effects on community diversity and structure. Where the goal is to safeguard native biodiversity, such activities should take into account the need to re-establish native species and/or restore ecosystem function in the previously affected area. 6Synthesis and applications. A comprehensive approach to IAS management should include consideration of the: (i) expected impacts; (ii) technical options available; (iii) ease with which the species can be targeted; (iv) risks associated with management; (v) likelihood of success; and (vi) extent of public concern and stakeholder interest. For each of these issues, in addition to targeting an individual species, the management of biological invasions must also incorporate an appreciation of other environmental pressures, the importance of landscape structure, and the role of existing management activities and restoration efforts. MICLESCU Challenges have emerged following the revival of nitric oxide (NO) from ,something old', a simple gas derived from nitrogen and oxygen with a role in the early stages of evolution, into ,something new', an endogenously formed biological mediator regulating a wide variety of physiological functions. Although pain is a common sensation, it encompasses multiple neurobiologic components, of which NO is only one. In pain research, the study of NO is complicated by convoluted problems related mostly to the effects of NO, which are pro- or anti-nociceptive depending on the circumstances. This dual function reflects the multi-faceted roles of the NO molecule described in physiology. This review covers current information about NO and its implications in pain mechanisms. In addition, it follows the pain pathways, demonstrating the role of NO in peripheral nociceptive transmission as well in central sensitization. This knowledge may provide the scientific basis for developing new drugs that are indicated for different types of pain, drugs that may be related to the chemical links of NO. A comprehensive approach to understanding the effects of NO will help clinicians identify novel agents that combine the pharmacological profile of native drugs with a controllable manner of NO release. Inhibitors of NO synthesis may have analgesic effects and would be of interest for treating inflammatory and neuropathic pain. Marques Abstract Advances in high-throughput process development and optimization involve the rational use of miniaturized stirred bioreactors, instrumented shaken flasks and microtiter plates. As expected, each one provides different levels of control and monitoring, requiring a compromise between data quantity and quality. Despite recent advances, traditional shaken flasks with nominal volumes below 250 mL and microtiter plates are still widely used to assemble wide arrays of biotransformation/bioconversion data, because of their simplicity and low cost. These tools are key assets for faster process development and optimization, provided data are representative. Nonetheless, the design, development and implementation of bioprocesses can present variations depending on intrinsic characteristics of the overall process. For each particular process, an adequate and comprehensive approach has to be established, which includes pinpointing key issues required to ensure proper scale-up. Recently, focus has been given to engineering characterization of systems in terms of mass transfer and hydrodynamics (through gaining insight into parameters such as kLa and P/V at shaken and microreactor scale), due to the widespread use of small-scale reactors in the early developmental stages of bioconversion/biotransfomation processes. Within this review, engineering parameters used as criteria for scaling-up fermentation/bioconversion processes are discussed. We first review the literature on the pervasiveness of trauma among women and the ways in which current service systems fail to address their needs. We then describe the four core principles of the model grantees developed to test in the project. Working through a project Steering Committee, grantees mandated that services be (a) integrated, (b) trauma-informed, (c) consumer-involved, and (d) comprehensive. Johnson Abstract While high throughput and combinatorial techniques have played an instrumental role in materials development and implementation, numerous problems in materials science and engineering are too complex and necessitate a prohibitive number of experiments, even when considering high throughput and combinatorial approaches, for a comprehensive approach to materials design. Here, we propose a unique combination of high throughput experiments focused on binary formulations that, in combination with advanced modeling, has the potential to facilitate true materials design and optimization in ternary and more complex systems for which experiments are never required. Extensive research on the development of photopolymerizable monomer formulations has produced a vast array of potential monomer/comonomer, initiator and additive combinations. This array dramatically expands the range of material properties that are achievable; however, the vast number of potential formulations has eliminated any possibility of comprehensive materials design or optimization. This limitation is addressed by maximizing the benefits and unique capabilities of high throughput experimentation coupled with predictive models for material behavior and properties. The high throughput experimentation-model combination is useful to collect a limited amount of data from as few as 11 experiments on binary combinations of 10 analyzed monomers, and then use this limited data set to predict and optimize formulation properties in ternary resins that would have necessitated at least 1000 high throughput experiments and several orders of magnitude greater numbers of traditional experiments. Most approaches reported in the literature have been ad-hoc and specific to certain risks; a general and comprehensive approach is lacking. To address this, we present a structured methodology for risk identification. Supply chain networks are in many ways similar to chemical plants, therefore well-established methods and concepts from chemical process risk management can be adapted to supply chains. Drawing from this analogy, we propose to represent supply chain structure and operations using flow and work-flow diagrams, equivalent to process flow diagrams (PFDs) and operating procedures. Following the HAZard and OPerability (HAZOP) analysis method common in process safety, risk identification can be performed by systematically generating deviations in different supply chain parameters and identifying their possible causes, consequences, safeguards, and mitigating actions. Stevens RN Nursing interventions to help women reduce their risk of contracting HIV must be designed from an in-depth understanding of the complex sociocultural patterns of sexuality in particular communities and among specific subgroups. Objective: In this data collection phase of a community-based HIV prevention project, the objective was to understand HIV risk-taking and HIV risk-reduction activities of lesbians and bisexual women. Design: Qualitative field study. Setting: Data were collected in women's bars and dance clubs and at selected lesbian/bisexual community events in San Francisco. Participants: Interviews were conducted with 1,189 racially diverse, socially and sexually active lesbians and bisexual women. Results: Inductive content analysis produced two themes: realities of sexual behavior and sexual expressions and their meanings. Realities of sexual behavior included an assumption that women who have sex with other women cannot get HIV, a lack of familiarity with HIV prevention strategies, inconsistent practice of safer sex with men and/or women, and the negative effect of alcohol or drug use on safer sex efforts. Sexual expressions and their meaning included trust in monogamy, a sense that safer sex practices detracted from intimacy and eroticism, the difficulty of negotiating sexual behaviors with men or women, and dealing with partner resistance to safer sex practices. Conclusions: Specific recommendations for practice are the need for nurses to understand the range and diversity of women's sexual behaviors, to develop skills in conducting inclusive sexual histories, and to develop a comprehensive approach to sexual health. [source] Effect of a Comprehensive Lifestyle Modification Program on the Bone Density of Male Heavy DrinkersALCOHOLISM, Issue 5 2010Toshifumi Matsui Background:, Heavy alcohol drinking is implicated in osteoporosis. Although abstinence is rapidly followed by a restoration of osteoblastic activity, little is known about the contributions of alcohol-related factors or the effectiveness of a lifestyle modification program (LMP) on bone density. Methods:, We conducted a study of 138 male alcoholic patients to investigate whether drinking history and concurrent factors were associated with the bone density of the calcaneus. A 2.5-months LMP in an institutionalized setting was completed by 20 of them, and its effect on bone density, serum parathyroid hormone (PTH), and 1.25-(OH)2 vitamin D levels were assessed. Results:, The patients had a high prevalence of daytime drinking (93.5%), continuous drinking (84.1%), and current smoking (82.0%) with mean duration of alcohol abuse of 30.0 ± 12.8 years. The patients had lower bone density than a reference control group (Z-scores: ,0.45 ± 1.02). Multiple stepwise regression analysis identified age, poor activities of daily living (ADL), continuous drinking, absence of liver cirrhosis, depression, and dementia as determinants of low bone density. The bone density of the 20 participants in the LMP improved 2.3% (p = 0.0003) with a more ameliorating effect on bone density than a conventional abstinence therapy (p = 0.014 for interventional effect). The upper normal range of PTH levels at baseline were significantly decreased, and 1.25-(OH)2 vitamin D levels also had a trend toward decrease during the abstinence. Conclusions:, Alcoholic patients may have many complications such as poor ADL and dementia, which are independently associated with decreased bone density. The results of this study support the idea that comprehensive approach to lifestyle factors to minimize risk of osteoporosis is the best way to improve bone density. A static rational general equilibrium framework is developed in which heterogeneous agents allocate land and time endowments between alternative uses. This modeling has important advantages. First, Pareto-optimal and voluntary-contribution equilibrium allocations are obtained in a unified set-up. Second, the suboptimality result of the decentralized equilibrium, the free-rider problem on the provision of this nonexcludable public good, and different mechanisms to return the economy to its first-best are analyzed. Finally, a methodological critique is made of some empirical literature, and it is suggested that our theoretical microeconomic-based structure seems to be a suitable starting point for empirical research.
