Informal care: the views of people receiving care

Abstract Informal care is perceived to be the best option for people who require assistance to look after themselves. National and international studies of informal care have focused on the carer, not on the care provided, or the needs and experiences of the care recipients. In the present study, 55 people receiving informal care (21 males and 24 females, mean age = 67.6) were surveyed to determine the type of assistance that they receive, perceptions of the quality of their care, feelings about being looked after by a carer and their perceptions of the services which would be useful. A random sample of 531 households were selected as part of a larger study into informal care in the west of Ireland. A total of 98 carers were identified and 55 of the people they looked after were well enough to participate in the study. Over two-thirds of carers assisted with household chores (e.g. cleaning, preparing meals and shopping). Other activities which carers assisted with included keeping the person safe from household accidents (62%), personal care (42%), and dressing and undressing (31%). Whilst most were very satisfied with the quality of care, a minority reported dissatisfaction, and stated that their carer showed signs of anger and frustration. Common concerns related to the health of the carer, their safety when the carer is not available and the cost of being cared for. Financial support for the person receiving care and the carer were the main priorities for these individuals. The present study points to a need for greater involvement of care recipients in planning services relating to informal care, and support and access to health professionals for people receiving care. People receiving care are also concerned about the level of financial support for themselves and their carers. [source]


Despite the Northern Territories: Hokkaido's Courting of the Russian Far East

Recent studies of Japan's postwar policy toward Russia have persuasively argued that the intrinsic (symbolic and psychological) value rather than the instrumental (economic or strategic) value that the Northern Territories (Southern Kuriles) represents to Japan is the most important obstacle to the normalization of relations between the two countries. Theoretically, there are three ways in which the intrinsic value of the disputed islands might be substantially depreciated and the instrumental value of closer bilateral ties appreciated: (1) major concessions from Russia, which are highly unlikely, (2) the emergence of a security or strategic of common concern to Tokyo and Moscow prompting the two sides to offer mutual concessions on the territorial issue or indefinitely postpone its resolution, and (3) a substantial expansion of economic, cultural, and social ties between the Japanese and the Russians, dramatically improving Japanese attitudes toward Russia. This study explores the third possibility, with a particular focus on developments at the subnational level, the level that has been largely ignored by students of Russian-Japanese relations. Namely, the study examines relations between Hokkaido and the Russian Far East since the 1960s and asks: Do the same logic and dynamic that operate at the national level apply at the regional level? Does the intrinsic importance of the territorial dispute prevail over considerations of economic and other tangible values at the subnational level as it does at the national level? The paper concludes that while Hokkaido has not deviated from the Japanese government,s official position on the territorial issue,that all the islands belong to Japan, the dispute has not prevented the provincial administration, municipal governments, or nongovernmental organizations in Hokkaido from launching and sustaining initiatives to cement closer ties with their northern neighbors, with growing economic and human ties playing important roles in the process. Nurses' experiences with telephone triage and advice: a meta-ethnography

Abstract Aims., This study is a meta-ethnography of nurses' experiences with telephone triage and advice and factors that facilitate or impede their decision-making process. Background., Telephone triage and advice services are a rapidly expanding development in health care. Unlike traditional forms of nursing practice, telenurses offer triage recommendations and advice to the general public without visual cues. Data sources., Published qualitative research on telephone triage and advice were sought from interdisciplinary research databases (1980,2008) and bibliographical reviews of retrieved studies. Review methods., Our systematic search identified 16 relevant studies. Two researchers independently reviewed, critically appraised, and extracted key themes and concepts from each study. We followed techniques of meta-ethnography to synthesize the findings, using both reciprocal and refutational translation to compare similar or contradictory findings, and a line-of-arguments synthesis. Results., We identified five major themes that highlight common issues and concerns experienced by telenurses: gaining and maintaining skills, autonomy, new work environment, holistic assessment, and stress and pressure. A line-of-arguments synthesis produced a three-stage model that describes the decision-making process used by telenurses and highlights how assessments largely depend on the ability to ,build a picture' of the patient and the presenting health issue. Conclusion., Telenurses experience a range of common concerns and issues which either impede or facilitate the decision-making process. Although ,building a picture' of the patient is key to making assessments over the telephone, final triage decisions are influenced by balancing the conflicting demands of being both carer and gatekeeper to limited healthcare services. Adolescents with Type 1 Diabetes: transition between diabetes services

Aim., The research aimed to develop a sustainable and coordinated approach to facilitating the transition between diabetes services for adolescents. The objectives were to: (1) involve key diabetes health delivery stakeholders in expressing their concerns and issues about current service delivery and ways to improve same, and (2) reveal from the perspective of the adolescents living with Type 1 Diabetes their experiences surrounding the process of transition. Background., This paper presents research that sought to identify the major concerns and issues that stakeholders had about transition and to reveal the experience of transition for the adolescent with Type 1 Diabetes. Key representatives from seven public diabetes services in Adelaide, South Australia worked collaboratively to answer the objectives of this inquiry. Approach., Interview data were generated and analysed using a response focus framework provided by fourth generation evaluation research. In this study, the focus was on common concerns, claims and issues raised by health care professionals (n = 21) and adolescents (n = 10) aged between 15 and 18 years about transferring from children's to adult diabetes services. Findings., Data revealed education and dietetic advice was reactive rather than proactive and that the paediatric model of care is philosophically and practically different to the adult model of diabetes care. Three phases of transition were identified: preparation, formal transition and evaluation. Our findings indicated that these stages of transition were not being fully implemented in health units. Conclusion., The project findings have set the scene to establish a multidisciplinary working party to work collaboratively across agencies to develop effective transition pathways. Relevance to clinical practice., The role of diabetes nurse educators and dietitians in South Australia is under-used throughout the transition process. Diabetes nurse educators are in an ideal position to prepare, coordinate and evaluate transitional processes. [source]

The Ties between Citizens as Colleagues
Some analogies are better than others for understanding the ties and responsibilities between citizens of a state. Citizens are better understood as particular kinds of colleagues than as either strangers or members of close-knit communities such as family or friends. Colleagues are diverse, separate and relatively distant individuals whose involuntary interdependence as equals in a practice or institution creates common concerns; this entails special responsibilities of communication, consideration and trust, which are capable of extension beyond the immediate group. Citizens likewise are involuntarily interdependent in political practices, and have comparable concerns and obligations that are more substantial than liberal advocates of constitutional patriotism recommend. But these are distinct from and potentially more extensible than those between co-nationals sharing a common culture, which are proposed by nationalists and some communitarians. The relationship of citizens is a more valid ground for associative obligations than others apart from family and friends. [source]
