Kinds of Bite Terms modified by Bite Selected AbstractsGetting better byte by byte: a pilot randomised controlled trial of email therapy for bulimia nervosa and binge eating disorder,EUROPEAN EATING DISORDERS REVIEW, Issue 2 2008Paul Robinson Abstract One hundred and ten people in an university population responded to emailed eating disorder questionnaires. Ninty-seven fulfilling criteria for eating disorders (bulimia nervosa (BN), binge eating disorder (BED), EDNOS) were randomised to therapist administered email bulimia therapy (eBT), unsupported Self directed writing (SDW) or Waiting list control (WLC). Measures were repeated at 3 months. Diagnosis, Beck depression inventory (BDI) and Bulimia investigatory test (BITE) scores were recorded. Follow-up rate was 63% and results must be interpreted cautiously. However significantly fewer participants who had received eBT or SDW fulfilled criteria for eating disorders at follow up compared to WLC. There was no significant difference between eBT and SDW in the analysis of variance (ANOVA), although in separate analyses, eBT was significantly superior to WLC (p,<,0.02) and the difference for SDW approached significance (p,=,0.06). BDI and BITE scores showed no significant change. For eBT participants there was a significant positive correlation between words written and improvement in BITE severity score. [source] Self-injurious behaviour in people with eating disordersEUROPEAN EATING DISORDERS REVIEW, Issue 1 2005Raquel Solano Abstract Objective To determine the importance of self-injurious behaviour in people with eating disorders (ED) and to analyse the possible differences between ED subtypes. Method 109 patients with ED (51 anorexia nervosa (AN) and 58 bulimia nervosa (BN)), according to DSM-IV diagnostic criteria, who were consecutively referred to our unit, participated in this study. All cases were female. Assessment Subjects were assessed by means of a semi-structured clinical interview and self-report questionnaires (Eating Attitudes Test, EAT-40; Eating Disorders Inventory, EDI; Bulimic Investigatory Test Edinburgh, BITE; Body Shape Questionnaire, BSQ; Beck Depression Inventory, BDI; Social Anxiety Scale, SAD). Design Comparison of cases by considering the factors diagnosis and self-injurious behaviour. Results The presence of self-injurious behaviour (SIB) (32% of cases) was not associated with the diagnosis (p,=,0.28). There was no association between SIB, suicide attempts, alcohol abuse and stealing, but a positive correlation between SIB and drug abuse was found (r,=,0.284, p,<,0.003). Likewise, patients with SIB showed higher scores on severity of the disorder (EDI, p,<,0.04), depressive symptoms (BDI, p,<,0.02), social anxiety (SAD, p,<,0.02) and body image dissatisfaction (BSQ, p,<,0.03). Conclusions: Eating disorders are pathologies in which self-injurious behaviour will be commonly present. [source] Sexual orientation as a risk factor for bulimic symptomsINTERNATIONAL JOURNAL OF EATING DISORDERS, Issue 6 2006Lars Wichstrøm PhD Abstract Objective: This study was carried out to examine whether sexual orientation predicts future bulimic symptoms and whether alleged risk factors associated with non-heterosexual sexual orientation explain the increased risk. Method: A nationally representative sample of Norwegian high school students (age 14,19; N = 2,924) completed self-reports about bulimic symptoms by means of the Bulimic Investigatory Test,Edinborough (BITE), same-sex sexual experience, degree of sexual attraction to the same sex, and alleged risk factors. They were reexamined 5 years later (T2). Results: Same-sex sexual experience before T1 increased the prevalence of bulimic symptoms at T2. Males who were attracted to the same sex at T1 had higher odds for bulimic symptoms compared with heterosexual males. These associations were still significant after controlling for initial bulimic symptoms and alleged risk factors. Conclusion: Same-sex sexual experience in both genders and non-heterosexual sexual attraction among males predict future bulimic symptoms. Int J Eat Disord 2006 [source] Excessive exercise in anorexia nervosa and bulimia nervosa: Relation to eating characteristics and general psychopathologyINTERNATIONAL JOURNAL OF EATING DISORDERS, Issue 4 2002Eva Peñas-Lledó Abstract Objective Excessive exercise is a well-known phenomenon in anorexia nervosa, but less is known about its role in bulimia nervosa. In addition, there is little evidence regarding the psychopathological processes that might act as predisposing, triggering, or maintaining factors for such exercise. The present study examined the presence of excessive exercise in different women with eating disorders, and its psychopathological correlates. Methods Case notes from 63 anorexia nervosa and 61 bulimia nervosa patients were examined. Two-way multivariate analyses of variance (diagnosis × use of excessive exercise) were used to determine the impact of the two factors upon eating characteristics (EAT-40 and BITE) and psychopathological symptoms (SCL-90-R). Results While high levels of depression were more likely among all patients who used excessive exercise, levels of anxiety and somatization were particularly high only among those anorexics who exercised excessively. Discussion Possible explanatory models are advanced to account for this pattern of findings, focusing on the possible use of exercise as an affect regulation strategy among anorexia nervosa patients. [source] EFFECTS OF TEXTURAL CHANGES IN COOKED APPLES ON THE HUMAN BITE, AND INSTRUMENTAL TESTSJOURNAL OF TEXTURE STUDIES, Issue 5-6 2003HARUKA DAN A multiple-point sheet sensor was used to measure the bite force applied to raw and cooked apple specimens during the first bite with incisors. Wedge penetration tests were compared with human bite measurements on the same samples. The shape of the force-time curves during biting of cooked apples clearly differed from that for raw apples. The first curve of cooked apple biting became jagged, and the maximum force was reduced. The second curve emerged following the first curve in most subjects as a characteristic feature of cooked apple biting, whereas it was not seen in the bite curve of a raw specimen or the wedge penetration curve of a cooked specimen. The maximum force for tissue fracture decreased for cooked apples, but the duration of biting increased. No adequate counterparts for the impulse of biting could be obtained from the load-displacement curves of the wedge penetration tests. The existence of parameters only measurable by bite tests and not by mechanical tests suggests the necessity of directly measuring the human bite. Russo Abstract The deposition of micro- and nanocrystalline bismuth telluride thin films with tailored structure and composition is of interest in view of improving the well-known material thermoelectric properties. Only a few works exist that discuss Raman scattering of Bi2Te3 crystals and films, while a Raman characterization of other phases, i.e. other lesser known compounds of the Bi-Te system, such as tsumoite (BiTe) and pilsenite (Bi4Te3), is still completely lacking. We here present a Raman investigation of Bi-Te polycrystalline thin films with controlled structure (stoichiometry and growth orientation), morphology and phase composition, produced by nanosecond pulsed laser deposition. Interpretation of Raman spectra from Bi-Te films was supported by scanning electron microscopy, energy dispersive spectroscopy (EDS) and X-Ray diffraction measurements, together with the predictions of the group theory. In this way, the first Raman characterization of Bi-rich phases (namely BiTe and Bi4Te3) has been obtained. For Bi-Te compositions characterized by a high Bi or Te content, Raman spectra reveal that segregation of elemental Bi or Te occurs. Copyright © 2008 John Wiley & Sons, Ltd. [source] Musculoskeletal underpinnings to differences in killing behavior between North American accipiters (Falconiformes: Accipitridae) and falcons (Falconidae)JOURNAL OF MORPHOLOGY, Issue 3 2008Diego Sustaita Abstract Accipiters (Accipiter spp.) and falcons (Falco spp.) both use their feet to seize prey, but falcons kill primarily with their beaks, whereas accipiters kill with their feet. This study examines the mechanistic basis to differences in their modes of dispatching prey, by focusing on the myology and biomechanics of the jaws, digits, and distal hindlimb. Bite, grip, and distal hindlimb flexion forces were estimated from measurements of physiological cross-sectional area (PCSA) and indices of mechanical advantage (MA) for the major jaw adductors, and digit and tarsometatarsal flexors. Estimated bite force, total jaw adductor PCSA, and jaw MA (averaged over adductors) tended to be relatively and absolutely greater in falcons, reflecting their emphasis on biting for dispatching their prey. Differences between genera in estimated grip force, total digit flexor PCSA, and digit MA (averaged over inter-phalangeal joints and digits) were not as clear-cut; each of these parameters scaled positively allometric in accipiters, which may reflect the scaling of both prey size, and the proportion of mammalian prey consumed by this lineage with increasing body size. Estimated tarsometatarsal force was greater in falcons than in accipiters, due to their greater MA, which may reflect selection for incurring greater forces during prey strikes. Conversely, the comparatively lower tarsometatarsal MA in accipiters reflects their capacity for greater foot speed potentially necessary for grasping elusive prey. Thus, this study elucidates how differences in jaw and hindlimb musculoskeletal morphology of accipiters and falcons are reflected in differences in their killing modes, and through differences in their force-generating capacities. Bites associated with the brown recluse spider (Loxosceles recluse) cause serious medical complications because the venom of this spider contains a powerful necrotising agent with the potential to cause severe cutaneous necrosis. Although not much is known regarding the application of negative pressure wound therapy (NPWT) to spider bites, this therapy has considerable literature support for its efficacy, cost-effectiveness and ease of use in chronic, difficult-to-heal wounds. A case study using NPWT to successfully treat a non healing upper arm wound presumed due to a venomous spider bite is presented here. The patient was successfully treated with a new, less costly NPWT product called the Versatile 1® and a new combination drain plus dressing called the Miller DermiVex® drain, both manufactured by Blue Sky Medical (Carlsbad, CA). [source] Taking Television Seriously: A Sound and Image Bite Analysis of Presidential Campaign Coverage, 1992,2004JOURNAL OF COMMUNICATION, Issue 4 2007Erik P. Bucy This study updates and builds on Hallin's landmark investigation of sound-bite news by documenting the prevalence of candidate image bites, where candidates are shown but not heard (as opposed to being shown and heard), in general election news over 4 election cycles. A visual analysis of broadcast network (ABC, CBS, and NBC) news coverage of the 1992, 1996, 2000, and 2004 U.S. presidential elections finds that image bites constituted a greater percentage of total campaign coverage than sound bites, with candidates appearing in image bites significantly more than sound bites. Even as candidate sound bites continue to shrink over time, image-bite time is increasing in duration,and candidates are being presented in image bites almost twice as much as journalists. Sound bites are also found to be largely attack and issue focused. Based on these findings, we call for greater appreciation of visual processing, nonverbal communication, and voter learning from television news in the study of media and politics. Une analyse visuelle de la couverture par les bulletins d,information des réseaux de télédiffusion (ABC, CBS et NBC) des élections présidentielles américaines de 1992, 1996, 2000 et 2004 démontre que les clips visuels ont constitué un plus grand pourcentage de la couverture électorale totale que les clips sonores, les candidats apparaissant plus fréquemment (de façon significative) dans les clips visuels que dans les clips sonores. Eine visuelle Analyse der Nachrichtenberichterstattung zu den Präsidentschaftswahlen von 1992, 1996, 2000 und 2004 auf ABC, CBS und NBC zeigte, dass Bild Bites einen größeren Anteil der Kampagnengesamtberichterstattung ausmachen als Sound Bites, und dass die Kandidaten signifikant häufiger in Bild Bites erscheinen als in Sound Bites. Boggild MSc Background Adventure travel necessarily places travelers at risk of environmental hazards. We assessed the burden of "environmental" hazards among a cohort of travelers and expatriates presenting to a large travel clinic in Nepal. Methods Data on travelers and expatriates seen at the Canadian International Water and Energy Consultants (CIWEC) clinic in Kathmandu were prospectively collected and entered into the GeoSentinel Surveillance Network database. Data on individuals receiving predefined diagnoses related to environmental hazards were extracted and analyzed. Results Of 10,499 travelers and 4,854 expatriates in the database, 2,160 were diagnosed with 2,533 environment-related illnesses. Injuries were common among both travelers and expatriates [N= 788 (6.1%) and 328 (4.9%), respectively], while altitude illness was seen almost exclusively in travelers [N= 611 (4.7%) vs N= 8 (0.1%)]. Factors independently associated with environmental diagnoses include male gender (p < 0.001), traveling for tourism (p < 0.001), and lack of pre-travel advice (p= 0.043). Three percent of travelers and 2% of expatriates presenting to CIWEC sustained a bite wound or required rabies postexposure prophylaxis. Injured travelers were less likely than others to have obtained pre-travel advice (p= 0.003), while those who sustained bite wounds were more likely to have received pre-travel advice (p < 0.001). Conclusions Environmental hazards are important causes of morbidity and potential mortality among adventure travelers and expatriates. Current pre-travel interventions are missing certain risk groups entirely and failing to have the desired educational impact in others. [source] Analysis of 154 cases of teeth with cracksDENTAL TRAUMATOLOGY, Issue 3 2006Byoung-Duck Roh Abstract,,, It is well known that cracked teeth occur most frequently in the mandibular molars with large or poor restorations, in those over 50 years of age. However, with increasing knowledge and experience with cracks of teeth, cracks appear to be found frequently in intact teeth without restorations. The aim of this study is to analyze the cases of tooth cracks in a dental hospital in a year, and to find out the characteristic features of cracks of teeth. For 1 year, each tooth that were identified as a cracked tooth was recorded and analyzed in terms of the classification of cavity and restorative material, the nature of opposing tooth, the location in the arch, the age and gender, and the clinical signs and symptoms, and treatment result. Cracked teeth were observed most frequently in the teeth with no restorations (60.4%) and with class I restorations (29.2%). The most prevalent age was in those over 40 years of age (31.2% in their 40s, 26.6% in their 50s) and the prevalence was similar in men (53.9%) and women (46.1%). Cracked teeth were found most frequently in the maxillary molars (33.8% in first molar, 23.4% in second molar) than in the mandibular molars (20.1% in first molar, 16.2% in second molar). 96.1% of the cracked teeth responded to the bite test, and 81.1% of the cracked teeth were observed in the mesiodistal direction. The prevalence of cracked tooth was highest in the intact teeth with no restoration, in maxillary molars, and in those over 40 years of age. When examining a intact maxillary posterior tooth that is sensitive to a bite and thermal change, crack in the mesiodistal direction need to be considered one of the causes. Shulman Abstract,,, To explore the association between incisal trauma and occlusal characteristics using oral examination and health interview data from the Third National Health and Nutrition Examination Survey 1988,1994 (NHANES III). Incisal trauma examinations were performed on 15 364 individuals 6,50 years of age using an ordinal scale developed by the National Institute of Dental and Craniofacial Research. Occlusal examinations were performed on 13 057 individuals 8,50 years of age. We fitted separate multivariate logistic regression models for maxillary and mandibular incisor trauma adjusting for socio-demographic variables (age, gender, race-ethnicity) and occlusal characteristics (overbite, overjet, open bite). 23.45% of all individuals evidenced trauma on at least one incisor, with trauma more than four times more prevalent on maxillary (22.59%) than on mandibular incisors (4.78%). Males (OR = 1.67) had greater odds of trauma than females; Whites (OR = 1.37) and non-Hispanic Blacks (OR = 1.37) had greater odds of trauma than Mexican,Americans. The odds of trauma increased with age, peaked from age 21 to 30 (OR = 2.92), and declined. As overjet increased, so did the odds of trauma. Compared to individuals with ,0-mm overjet, odds of trauma increased from 1,3 mm (OR = 1.42) to 4,6 mm (OR = 2.42) to 7,8 mm (OR = 3.24) to >8 mm (OR = 12.47). Trauma to incisors is prevalent but mostly limited to enamel. Trauma to maxillary incisors is associated with overjet, gender, race-ethnicity, and age, while trauma to mandibular incisors is associated with gender, age, and overbite. [source] Do ,Quiet' Places Make Animals Placid?ETHOLOGY, Issue 6 2005Island vs. Most animals that possess potent venom display a wide variety of warning messages to discourage predators. Tiger snakes are large and highly venomous elapids that exhibit these anti-predator behaviours. We compared the anti-predator behaviours of two neighbouring and genetically indistinguishable populations in Western Australia (Herdsman Lake, HL and Carnac Island, CI). CI is free from human, native and feral predation. All of these factors represent a continual threat on HL situated on the mainland. Neither body size, nor sex influenced defensive behaviours. However, we observed a marked inter-population difference among adults in the degree to which anti-predator behaviours were displayed when snakes were continually aggravated: HL snakes exhibited a typical warning signal (flat-neck) and bite, while CI snakes remained very docile. In stark contrast, neonates of both populations exhibited marked anti-predator behaviours and both populations were indistinguishable in terms of the intensity of display. Neonates reared in captivity, hence regularly confronted by human predators, became more defensive in comparison with neonates exposed to natural conditions on CI; similarly several adult CI snakes kept in captivity became more defensive. Our results highlight the extreme behavioural plasticity of snakes. We also hypothesize that CI snakes may become more placid over time as they grow up in an environment free from predation. Todryk Abstract Protective immunity generated following malaria infection may be comprised of Ab or T cells against malaria Ag of different stages; however, the short-lived immunity that is observed suggests deficiency in immune memory or regulatory activity. In this study, cellular immune responses were investigated in individuals receiving Plasmodium falciparum sporozoite challenge by the natural (mosquito bite) route as part of a malaria vaccine efficacy trial. Parasitemia, monitored by blood film microscopy and PCR, was subsequently cleared with drugs. All individuals demonstrated stable IFN-,, IL-2 and IL-4 ex vivo ELISPOT effector responses against P. falciparum -infected RBC (iRBC) Ag, 28 and 90,days after challenge. However, infected RBC-specific central memory responses, as measured by IFN-, cultured ELISPOT, were low and unstable over time, despite CD4+ T cells being highly proliferative by CFSE dilution, and showed an inverse relationship to parasite density. In support of the observation of poor memory, co-culture experiments showed reduced responses to common recall Ag, indicating malaria-specific regulatory activity. This activity could not be accounted for by the expression of IL-10, TGF-,, FOXP3 or CTLA-4, but proliferating T cells expressed high levels of CD95, indicating a pro-apoptotic phenotype. Lastly, there was an inverse relationship between FOXP3 expression, when measured 10 days after challenge, and ex vivo IFN-, measured more than 100 days later. This study shows that malaria infection elicits specific Th1 and Th2 effector cells, but concomitant weak central memory and regulatory activity, which may help to explain the short-lived immunity observed. Another aim was to determine the role of different functional conditions of this muscle in the adaptation process. Ninety-six young male rats were split into two groups: one was fed a hard diet and the other a soft diet to develop different functional capacities in the masticatory muscles. After 2 wk, half of the rats in both groups were fitted with an appliance that raised the bite by 2 mm. The measurements on the muscles were performed in situ. The insertion of the appliance stretched the anterior masseter muscle. After 4 wk, the vertical dentoskeletal dimension, the muscle belly, and the sarcomeres showed no difference in length among the groups. However, the aponeurosis was longer in the rats wearing the appliance compared to the controls, and among the bite block groups it was longer in the rats fed a hard diet. Length adaptation occurred in the aponeurosis. Clinically this may imply a need for reactivation of functional appliances to increase their efficiency, at a rate possibly depending on masseter muscles functional condition. Barrett Abstract The effects of genotypic variation in ryegrasses on sward structure, bite dimensions and intake rate by dairy cows were investigated. Two experiments were conducted. In Experiment 1, swards were in a vegetative state whereas, in Experiment 2, they were partly reproductive and were taller with higher herbage mass but lower leaf proportion than in Experiment 1. Applicability of relationships between sward structure and bite characteristics, previously established from artificial or hand-constructed swards, to field conditions were tested. Additional short-term intake rates and/or sward structural characteristics were considered as indicators of potential intake for use in protocols for the evaluation of grass varieties. Four cultivars were studied: AberElan, Twins (diploid and tetraploid perennial ryegrasses respectively), Polly, a hybrid ryegrass (perennial × Italian ryegrass) and Multimo (Italian ryegrass), each established in 200-m2 plots in four replicated blocks. Herbage intake rate was determined by short-term liveweight change (taking account of insensible weight loss) using 16 dairy cows allocated to four balanced groups with each plot grazed by one group for a 1-h assessment period. One block was grazed per day, over a 4-d experimental period, with each group grazing each variety in a complete crossover design. Sward characteristics and bite rate were also measured in both experiments. Bite dimensions were subsequently estimated, with bite depth being determined as a function of extended tiller height (ETH) in both experiments. Within both experiments, bite mass and intake rate did not differ significantly between swards of different cultivars despite swards containing Multimo generally having a higher ETH and water-soluble carbohydrate concentration and lower green leaf mass, sward bulk density and neutral-detergent fibre concentration than the other swards. However, bite depth was significantly higher (P < 0·01) in swards containing Multimo swards than in the others and, in Experiment 1, bite depth, as a proportion of ETH, was higher in swards containing Multimo and lower in those containing Twins than in the other two cultivars, whereas there was no difference in Experiment 2. Taking both experiments together, the mean bite depth was 0·5 of ETH with sward bulk density accounting for almost half the variance in the relationship between bite depth and ETH. The bulk density of the bite (bite mass per unit bite volume), measured in Experiment 2, followed a similar pattern to sward bulk density, increasing in the order Multimo, Polly, AberElan and Twins. It is concluded that the relationships between sward characteristics and bite dimensions, derived from artificial swards, are applicable to field swards, although the range in natural ryegrass sward characteristics is usually not as wide as in experiments using artificial swards. Lack of precision in the measurement of short-term intake and in sward-based measurements is likely to preclude their use in the evaluation of grass varieties. [source] Acoustic measurement of intake and grazing behaviour of cattleGRASS & FORAGE SCIENCE, Issue 2 2000Acoustic analysis of grazing behaviour was found to allow accurate identification of chewing and biting, and estimation of intake by cattle. Four steers grazed six types of turves, three were short (14, 16 and 20 cm tall), leafy turves from mowed Setaria lutescens with high application of N fertilizer, and three were tall (36, 49 and 62 cm tall), mature turves from uncut areas with low application of N fertilizer. Each animal grazed ten bites from each turf type while behaviour was videotaped and grazing sounds were recorded with a wireless microphone taped to the animal's forehead. The intake was measured by the difference in pre- and post-grazing turf weight, corrected for water loss. Chews and bites differed in acoustic characteristics, with bites producing more output in the 3,8 kHz frequency band. Discriminant analysis of the acoustic characteristics correctly classified chews and bites with 94% accuracy. Intake was predicted with a high degree of accuracy by total energy flux density (fJ m,2) in chewing sounds (EFDC), EFDC per chew and average intensity of chewing sound in the 1,2 and 2,3 kHz bands (R2=0·90, CV=3·6%). Although bite mass ranged from 1·37 to 4·50 g, EFDC per g of intake (CV=0·015) was not affected by treatments or animals. Chewing sounds carried more information about intake than biting sounds and chews per bite. Analysis of grazing sounds has the potential to overcome many of the problems associated with the measurement of grazing intake. The purpose of this study is to evaluate the efficacy of intraglandular injection of botulinum toxin type A (BTA) in patients with first bite syndrome. Methods Five patients with first bite syndrome developed after head and neck surgery were treated by injection of BTA into parotid gland. All patients completed a 4-item quality-of-life survey with a 10-point response scale designed to measure outcome of intraglandular injection of BTA. Results The first bite syndrome without or with sialogogue and degree of interference with daily activity with or without eating or drinking improved significantly at 1 and 3 month after injection (p < .05). Isbister Abstract Aim: To describe the clinical features, investigation, diagnosis and treatment of ulcers attributed to white-tail (WT) spider bites or necrotic arachnidism. Methods: The study was a prospective case series of patients referred to the Hunter Area Toxicology Service (a tertiary referral toxicology unit servicing a population of 500 000) with an ulcer or skin lesion that had been attributed to either a suspected WT spider bite or necrotic arachnidism. Eleven patients with skin lesions or necrotic ulcers were referred between January 2000 and June 2002. Results: In two patients that were inpatients in other hospitals, investigation and follow up was not possible. In both cases there was no history of spider bite and Staphylococcus aureus was cultured. In nine patients, a diagnosis other than spider bite was made following appropriate investigation and follow up, including: (i) two cases of dermatophytoses, (ii) three staphylococcal infections, (iii) one case of pyoderma gangrenosum, (iv) one case of cutaneous polyarteritis nodosa, (v) one case of Nocardia braziliensis and (vi) one infected diabetic ulcer. There was only one case where the person recalled seeing a spider bite them, but the patient did not collect the spider for identification. The median time to diagnosis was 3 weeks (interquartile range: 3,9 weeks) and 3.5 years in one case. Appropriate treatment was initiated once the correct diagnosis was made and all cases resolved. Conclusions:, In this series, all cases initially referred as WT spider bites or necrotic arachnidism were found to have alternative diagnoses with appropriate investigations. This demonstrates that spider bites are an unlikely cause of necrotic ulcers and that all ulcers should be properly investigated with bacterial, fungal and mycobacterial cultures and skin biopsy for histopathology. Lung MD A previously healthy 7-year-old white boy presented to St. Louis Children's Hospital with a 1-day history of headache, malaise, temperature of 38.7 °C, and a progressively erythematous, tender calf with central dusky purpura. On the morning of admission, his mother noticed a 2-mm crust on the patient's right calf with a 3-cm × 3-cm area of surrounding erythema. No history of recent trauma or bite was obtained. He had suffered two episodes of nonbloody, nonbilious emesis during the last day. In addition, over the previous 12 h, he presented brown urine without dysuria. His mother and brother had suffered from gastroenteritis over the previous week without bloody diarrhea. On initial physical examination, there was a 6-cm × 11-cm macular tender purpuric plaque with a central punctum on the right inner calf, which was warm and tender to the touch, with erythematous streaking towards the popliteal fossa ( Fig. 1). The inguinal area was also erythematous with tender lymphadenopathy and induration, but without fluctuance. Laboratory studies included an elevated white blood cell count of 20,800/,L with 6% bands, 86% segs, and 7% lymphocytes, hemoglobin of 12.5 g/dL, hematocrit of 35.1%, and platelets of 282,000/,L. The prothrombin time/activated partial tissue thromboplastin was 10.4/28.0 s (normal PT, 9.3,12.3 s; normal PTT, 21.3,33.7 s) and fibrinogen was 558 mg/dL (normal, 192,379 mg/dL). Urinalysis showed 1+ protein, 8,10 white blood cells, too numerous to count red blood cells, and no hemoglobinuria. His electrolytes, blood urea nitrogen (BUN), and creatine were normal. The urine culture was negative. Blood culture after 24 h showed one out of two bottles of coagulase negative Staphylococcus epidermidis. Figure 1. Over the next 2 days, the surrounding rim of erythema expanded. The skin within the plaque cleared and peeled at the periphery. The coagulase negative staphylococci in the blood culture were considered to be a contaminant. Cefotaxime and oxacillin were given intravenously. His leg was elevated and cooled with ice packs. The patient's fever resolved within 24 h. The lesion became less erythematous and nontender with decreased warmth and lymphadenopathy. The child was discharged on Duricef for 10 days. Because the patient experienced hematuria rather than hemoglobinuria, nephritis was suggested. In this case, poststreptococcal glomerulonephritis was the most likely cause. His anti-streptolysin-O titer was elevated at 400 U (normal, <200 U) and C3 was 21.4 mg/dL (normal, 83,177 mg/dL). His urine lightened to yellow,brown in color. His blood pressure was normal. Results. Over 50 articles were found and relevant information and data were reviewed by the authors. It was found that the multifactorial nature of anterior open bite makes its management difficult and various treatment modalities are being used. Clinicians must be able to diagnose the problem and choose the best treatment. Conclusion. Successful treatment of anterior open bite greatly relies on both diagnosis and therapeutics. Although there are many different treatment modalities available, stability after treatment is still a critical issue as evidence on long term stability of various treatment options is lacking. Maxillary hypoplasia, deep palatal vault, anterior open bite, crowding of the dental arch, severely delayed tooth eruption, and dental malocclusion are the main oral manifestations of this syndrome. In this report, a case of Apert syndrome with glucose-6-phosphate dehydrogenase (G6PD) deficiency is presented. The patient, a 4-year-old male and the fourth child of healthy parents, was admitted to our department because of delayed tooth eruption. He had all the cardinal symptoms of the Apert syndrome. Clinical examination revealed that primary centrals, canines and first molars erupted; however, primary second molars and laterals had not erupted. The patient had no dental caries. Preventive treatments were applied, and subsequently, the patient was taken to long-term follow up. Animal bite wounds and their subsequent infection are relatively common. Incidence rates for dog bites are significantly higher among children aged 0,9 years, especially among boys. Although bite wounds may initially look innocuous, they frequently lead to serious infection with a potential for life-threatening complications. The microbiology of dog bite wounds is usually polymicrobial, typically including anaerobes, Staphylococcus aureus and Pasteurella species. A case is described of a 22-month-old boy who, subsequent to a dog bite over the left maxilla, suffered infection of the dental follicle of the primary maxillary canine with Pasteurella multocida. The infection proved difficult to treat, requiring several attempts at incision and drainage of the abscess together with systemic antibiotics, and resulted in the eventual loss of the tooth. Bites associated with the brown recluse spider (Loxosceles recluse) cause serious medical complications because the venom of this spider contains a powerful necrotising agent with the potential to cause severe cutaneous necrosis. Although not much is known regarding the application of negative pressure wound therapy (NPWT) to spider bites, this therapy has considerable literature support for its efficacy, cost-effectiveness and ease of use in chronic, difficult-to-heal wounds. A case study using NPWT to successfully treat a non healing upper arm wound presumed due to a venomous spider bite is presented here. The patient was successfully treated with a new, less costly NPWT product called the Versatile 1® and a new combination drain plus dressing called the Miller DermiVex® drain, both manufactured by Blue Sky Medical (Carlsbad, CA).
