Estrogen and progesterone hormone receptor status in breast carcinoma: Comparison of immunocytochemistry and immunohistochemistry

Abstract We evaluated the correlation between histologic and cytologic specimens in the determination of estrogen receptor (ER) and progesterone receptor (PR) status in breast carcinoma and investigated the causes of clinically significant discrepancies. We analyzed 70 immunoassays for ER and 60 for PR from 71 patients with breast carcinoma. Concordance between cytology and histology was 89% for ER and 63% for PR using scores from pathology reports. Concordance between cytology and histology was 98% for ER and 91% for PR using consensus scores (obtained after reevaluation by the team of pathologists). Thirty of 130 (23%) tests had clinically relevant discrepancies, 53% of which were caused by wrong interpretation of cytologic findings, 10% by wrong interpretation of histologic findings, 17% by sampling error and 20% were not available for reevaluation. Wrong interpretation of the results for ER and PR status in cytology was a far more frequent cause of clinically relevant discrepancies than sampling errors. Present and future therapeutic strategies for idiopathic oligozoospermia

The effectiveness of medical treatment for idiopathic oligozoospermia (IO) has been at best doubtful until now and a logical consequence of this unsatisfactory situation has been the partial displacement of this approach by assisted reproduction techniques. This state of affairs has resulted from insufficient investigation, inappropriately designed clinical trials and consistent disregard for the principles of evidence-based medicine. Protocol-related shortcomings and wrong interpretation of the data available have also been some of the all too frequent problems encountered in this therapeutic approach. In this rather misty situation, it appears that, of the therapeutic agents used so far, follicle stimulating hormone (FSH) (mainly FSH-secretagogues) may exert some beneficial effects on a number of biological endpoints related to spermatogenesis and sperm maturation. The short and medium term prospects of medical treatment for IO rest mainly with improvement of investigative procedures to a higher degree of sophistication, with emphasis placed on identifying the causes rather than the results of dysfunction so that a better selection of candidates can be made. Moreover, the introduction of prognostic indices for evaluation of the beneficial effects of a therapeutic agent may be of paramount importance. Finally, a better assessment of the preparations available and, possibly, the introduction of new more specific agents may also be an important step forward in this field. This type of large-scale effort should not be left to individual investigators or special centres working independently, but it may come under the auspices of a central regulating agency so that undisputed results from large, multicentre and uniform studies might be obtained, if medical treatment is to remain a good option. In this context, it may also be emphasized that andrology's main task should always be to treat the male with the problem rather than his healthy female partner, whenever this is possible. [source]


Scaling and correlation analysis of galactic images

Different scaling and autocorrelation characteristics and their application to astronomical images are discussed: the structure function, the autocorrelation function, Fourier spectra and wavelet spectra. The choice of the mathematical tool is of great importance for the scaling analysis of images. The structure function, for example, cannot resolve scales that are close to the dominating large-scale structures, and can lead to the wrong interpretation that a continuous range of scales with a power law exists. The traditional Fourier technique, applied to real data, gives very spiky spectra, in which the separation of real maxima and high harmonics can be difficult. We recommend as the optimal tool the wavelet spectrum with a suitable choice of the analysing wavelet. We introduce the wavelet cross-correlation function, which enables us to study the correlation between images as a function of scale. The cross-correlation coefficient strongly depends on the scale. The classical cross-correlation coefficient can be misleading if a bright, extended central region or an extended disc exists in the galactic images. An analysis of the scaling and cross-correlation characteristics of nine optical and radio maps of the nearby spiral galaxy NGC 6946 is presented. The wavelet spectra allow us to separate structures on different scales like spiral arms and diffuse extended emission. Only the images of thermal radio emission and H, emission give indications of three-dimensional Kolmogorov-type turbulence on the smallest resolved scales . The cross-correlations between the images of NGC 6946 show strong similarities between the images of total radio emission, red light and mid-infrared dust emission on all scales. The best correlation is found between total radio emission and dust emission. Thermal radio continuum and H, emission are best correlated on a scale of about , the typical width of a spiral arm. On a similar scale, the images of polarized radio and H, emission are anticorrelated, a fact that remains undetected with classical cross-correlation analysis. [source]
