Selected AbstractsEvaluation of gastroduodenal mucosal lesions in patients with Crohn's disease and ulcerative colitisDIGESTIVE ENDOSCOPY, Issue 3 2004Kazuhiro Maeda Background:, Patients with Crohn's disease (CD) are reported to suffer from upper gastroduodenal lesions with varying frequency, although concurrent Helicobacter pylori infection is reported to be low. Methods:, A prospective study was carried out on patients diagnosed with CD or ulcerative colitis (UC) in order to evaluate the degree of upper gastroduodenal tract involvement and the prevalence of Helicobacter pylori infection. Results:, Gastroduodenal lesions were found in 18 (78%) of 23 CD patients, the location being the stomach in 18 (78%), the duodenal bulb in 16 (70%) and the descending duodenum in 16 (70%). Bamboo joint-like lesions were found in four cases (17%) in gastric body and cardia. In contrast, gastroduodenal lesions were found in 10 (53%) of 19 UC patients, the location being the stomach in nine (47%), the duodenal bulb in six (32%), and the descending duodenum in three (16%). The H. pylori -positive rate in patients with CD and UC was 0%, and 11%, respectively. Conclusion:, Minute upper gastroduodenal lesions are much more common in CD than in UC patients, especially in the descending duodenum. Accordingly, upper gastrointestinal endoscopy would seem to be a useful means with which to obtain a definitive diagnosis in all suspected IBD cases. [source] Estimation of temporal variation in splash detachment in two Japanese cypress plantations of contrasting ageEARTH SURFACE PROCESSES AND LANDFORMS, Issue 9 2010Y. Wakiyama Abstract To elucidate splash erosion processes under natural rainfall conditions, temporal variations in splash detachment were observed using a piezoelectric saltation sensor (H11B; Sensit Co., Portland, ND, USA). Preliminary laboratory tests of Sensit suggested that they were suitable for field observations. Field observations were conducted between July and September 2006 in 21- and 36-year-old Japanese cypress (Chamaecyparis obtusa) plantations with mean stand heights of 9·2,m and 17·4,m, respectively. Splash detachment (in g m,2) was measured seven times using splash cups, and raindrop kinetic energy (in J,m,2,mm,1) in both stands was measured using laser drop-sizing (LD) gauges. Sensit was installed to record saltation counts, which were converted to temporal data of splash detachment (splash rate; in g m,2 10,min,1) using the relationship between splash detachment and saltation counts. Surface runoff was monitored using runoff plots of 0·5,m width and 2·0,m length to obtain temporal data of flow depth (in millimeters). Both total splash detachment and raindrop kinetic energy were larger in the older stand. Increased splash rates per unit throughfall were found in both stands after rainless durations longer than approximately one day in both stands. However, a lower splash rate was found in the 21-year stand after rainfall events. During extreme rainstorms, the 21-year stand showed a low runoff rate and a decline in the splash rate, while the 36-year stand showed a higher splash rate and increased flow depth. Gómez Abstract Although fish immunology has progressed in the last few years, the contribution of the normal endogenous microbiota to the overall health status has been so far underestimated. In this context, the establishment of a normal or protective microbiota constitutes a key component to maintain good health, through competitive exclusion mechanisms, and has implications for the development and maturation of the immune system. The normal microbiota influences the innate immune system, which is of vital importance for the disease resistance of fish and is divided into physical barriers, humoral and cellular components. Innate humoral parameters include antimicrobial peptides, lysozyme, complement components, transferrin, pentraxins, lectins, antiproteases and natural antibodies, whereas nonspecific cytotoxic cells and phagocytes (monocytes/macrophages and neutrophils) constitute innate cellular immune effectors. Cytokines are an integral component of the adaptive and innate immune response, particularly IL-1,, interferon, tumor necrosis factor-,, transforming growth factor-, and several chemokines regulate innate immunity. This review covers the innate immune mechanisms of protection against pathogens, in relation with the installation and composition of the normal endogenous microbiota in fish and its role on health. Knowledge of such interaction may offer novel and useful means designing adequate therapeutic strategies for disease prevention and treatment. [source] Sensory, chemical and microbiological assessment of farm-raised European sea bass (Dicentrarchus labrax) stored in melting iceINTERNATIONAL JOURNAL OF FOOD SCIENCE & TECHNOLOGY, Issue 3 2002Vasiliki R. Kyrana Farm raised European sea bass (Dicentrarchus labrax) were stored in melting ice for a period up to 22 days from the time of harvest, and sensory, chemical, and microbiological assessments were made at intervals. The storage life of the ungutted fish, determined by sensory evaluation of the cooked flesh, was 19 days. Of the chemical tests, only k1 value provided a useful means of monitoring early storage change. Trimethylamine, total volatile bases and pH showed practically no change during the first half of the edible storage life of the fish. Changes in free fatty acid (FFA) content and thiobarbituric acid (TBA) value could not be used to determine loss of acceptability or end of storage life. Sulphide producing bacteria constituted a very low proportion of the total aerobic flora, suggesting that the common sulphide producer Shewanella putrefaciens was not a major spoiler of sea bass in this trial. [source] The Trophic Index of Macrophytes (TIM) , a New Tool for Indicating the Trophic State of Running WatersINTERNATIONAL REVIEW OF HYDROBIOLOGY, Issue 1 2003Susanne Schneider Abstract In running waters, apart from structural degradation, nutrient input becomes increasingly important. To investigate the indicator values of as many species of submerged macrophytes as possible numerous samples of the sediment within macrophyte stands and the overlying water were taken in running waters throughout Bavaria, Germany. To develop the Trophic Index of Macrophytes (TIM), the concentrations of soluble reactive phosphorus of both the water body and the sediment pore water were used. Based on a weighted sum of the SRP-concentrations of the water body and the sediment pore water, indicator values were determined for a total of 49 species of submerged macrophytes. A detailed method is described on how and depending on which preconditions the trophic state of running waters can be determined by the TIM. An example of the TIM in the stream Rotbach is given. It shows that the TIM is a useful means to detect differences in the phosphorus loading of running waters. [source] A Comparison of Computerized and Pencil-and-Paper Tasks in Assessing Cognitive Function in Community-Dwelling Older People in the Newcastle 85+ Pilot StudyJOURNAL OF AMERICAN GERIATRICS SOCIETY, Issue 10 2007Joanna Collerton MRCP OBJECTIVES: To compare the acceptability and feasibility of computerized and pencil-and-paper tests of cognitive function in 85-year-old people. DESIGN: Group comparison of participants randomly allocated to pencil-and-paper (Wechsler Adult Intelligence and Memory Scales) or computerized (Cognitive Drug Research) tests of verbal memory and attention. SETTING: The Newcastle 85+ Pilot Study was the precursor to the Newcastle 85+ Study a United Kingdom Medical Research Council/Biotechnology and Biological Sciences Research Council cohort study of health and aging in the oldest-old age group. PARTICIPANTS: Eighty-one community-dwelling individuals aged 85. MEASUREMENTS: Participant and researcher acceptability, completion rates, time taken, validity as cognitive measures, and psychometric utility. RESULTS: Participants randomized to computerized tests were less likely to rate the cognitive function tests as difficult (odds ratio (OR)=0.16, 95% confidence interval (CI)=0.07,0.39), stressful (OR=0.18, 95% CI=0.07,0.45), or unacceptable (OR=0.18, 95% CI=0.08,0.48) than those randomized to pencil-and-paper tests. Researchers were also less likely to rate participants as being distressed in the computer test group (OR=0.19, 95% CI=0.07,0.46). Pencil-and-paper tasks took participants less time to complete (mean±standard deviation 18±4 minutes vs 26±4 minutes) but had fewer participants who could complete all tasks (91% vs 100%). Both types of task were equally good measures of cognitive function. CONCLUSION: Computerized and pencil-and-paper tests are both feasible and useful means of assessing cognitive function in the oldest-old age group. Computerized tests are more acceptable to participants and administrators. [source] Using Attributions to Understand the Effects of Explanations on Applicant Reactions: Are Reactions Consistent With the Covariation Principle?,JOURNAL OF APPLIED SOCIAL PSYCHOLOGY, Issue 2 2005Robert E. Ployhart Research has shown that explanations for selection decisions may influence a variety of applicant perceptions and behavior, but an understanding of how and why this occurs remains largely unknown. This study attempts to understand the effects of explanations by adopting Kelley's (1967, 1972) covariation model of the attribution process. Specifically, explanations that vary on consensus, distinctiveness, and consistency covariation information should produce predictable effects on applicant perceptions and attributions. Results from 2 studies, the first a laboratory study and the second a field study with actual applicants, support the utility of the covariation model for understanding the influence of explanations for selection decisions on locus attributions, fairness, self-perceptions, and organizational attractiveness. These results suggest that the covariation model may be a useful means to construe the explanation-attribution-perception relationship, and thus provide a number of theoretical and practical implications. [source] Beyond bone mineral density: can existing clinical risk assessment instruments identify women at increased risk of osteoporosis?JOURNAL OF INTERNAL MEDICINE, Issue 5 2004L. E. Objective., Although osteoporosis and fragility fracture are common amongst postmenopausal women, the extent of risk varies, and measurement of bone mineral density (BMD) is the standard tool used to diagnose and assess fracture risk. Rates of diagnosis remain relatively low, and several groups have developed instruments to help identify individuals who would most benefit from BMD testing. In this paper, we review and compare the performance of these instruments to identify those most useful in the primary care setting. Design., Review of screening instruments comprised osteoporosis clinical risk factors and comparison of the sensitivity and specificity of these algorithms. Results., Validated instruments have varying complexity, but similar sensitivity and specificity for identifying individuals who are likely to have low BMD. The area under the receiver operating characteristic curve ranges from 0.75 (SOFSURF) to 0.81 (SCORE). The simplest of the instruments (OST) uses only age and weight and has an AUC of 0.79. Conclusions., The Osteoporosis Self-assessment Tool, the simplest of the instruments, performs as well as more complex tools and, because of its simplicity, may be the most useful means for the busy clinician to identify postmenopausal women who would most benefit from BMD testing. [source] Freeze-drying of tert- butanol/water cosolvent systems: A case report on formation of a friable freeze-dried powder of tobramycin sulfateJOURNAL OF PHARMACEUTICAL SCIENCES, Issue 4 2002Sakchai Wittaya-Areekul Abstract A case study is presented in which a tert -butanol (TBA)/water cosolvent system was found to be a useful means of producing freeze-dried tobramycin sulfate that readily forms a loose powder upon agitation in a specialized application in which a critical quality attribute is the ability to pour the sterile powder from the vial. Both formulation and processing variables are important in achieving acceptable physical properties of the cake as well as minimizing residual TBA levels. Liquid/liquid phase separation was observed above critical concentrations of both drug and TBA, resulting in a two-layered lyophilized cake with unacceptable appearance, physical properties, and residual TBA levels. However, the choice of tobramycin sulfate and TBA concentrations in the single-phase region of the phase diagram resulted in a lyophilized solid that can readily be poured from vials. Crystallization of TBA before drying is critical to achieving adequately low residual TBA levels, and this is reflected in the effect of thermal history of freezing on residual TBA levels, where rapid freezing results in incomplete crystallization of TBA and relatively high levels of residual solvent. Annealing at a temperature above T,g of the system after an initial freezing step significantly reduces the level of residual TBA. Roberts DDS Abstract Objective: This study evaluated relationships between caregiver responses to oral health screening questions and caries in young children. Methods: Two samples of caregivers answered identical eight-item screening questionnaires about their oral health. One sample included children enrolled in the Special Supplemental Nutrition Program for Women, Infants and Children (WIC) who were 24 to 42 months of age; the other sample included 3- to 5-year-old children attending a pediatric dental clinic. Using chi-square and relative risk, questionnaire findings were related to children's caries history based on clinical caries exams. Results: Questions significantly (P < 0.05) related to children's caries in the older sample included caregivers' poorer rating of their oral health, less frequent dental visits, current or recent caries, and history of tooth loss due to caries. However, only questions pertaining to tooth loss were related to caries in the younger sample. Conclusion: Caregivers' reported loss of teeth due to caries was significantly associated with caries development in their children in both samples, and may be a useful means for early identification of children at high risk. [source] The Shadows of the Law: Contemporary Approaches to Regulation and the Problem of Regulatory ConflictLAW & POLICY, Issue 4 2003Fiona Haines Regulatory analyses often assume that compliance is desirable, with literature focusing on strategies to encourage "excellence" in adherence to regulatory goals. Yet, it is not unusual for disparate regulatory goals to exist that are based on competing values of what constitutes the "good society." It is this conflict that forms the substance of this paper. In cases of competing regulatory goals, techniques that encourage exemplary compliance in one area can create incentives to breach regulatory provisions of a competing regime. In such cases, generic regulatory techniques are unable to provide a useful means for resolving regulatory conflict but do allow a political delegation of conflict resolution to the "scientific" strategies of the regulator. In turn, the regulator places responsibility on companies for resolving competing regulatory demands. Successive delegation leads to juridification as well as regulators vying to retain primacy for their regime. This problem is examined through analysis of responsibilities for subcontractor safety under Australian health and safety law and sections of the Australian Trade Practices Act 1974 aimed at protecting competition. [source] Evaluation of PCR primers from putative transcriptional regulator genes for identification of Staphylococcus aureusLETTERS IN APPLIED MICROBIOLOGY, Issue 1 2005D. Liu Abstract Aims:, To examine if PCR primers derived from putative transcriptional regulator genes can be useful for Staphylococcus aureus identification. Methods and Results:,Staphylococcus aureus gene sequences that encode transcriptional regulators were retrieved from GenBank and compared with other DNA sequences via BLAST searches. Two uniquely present, putative transcriptional regulator genes (i.e. Sa0836 and Sa0856) were selected as a consequence and PCR primers (Sa0836F/R and Sa0856F/R) were then designed from these genes for evaluation. A total of 84 bacterial strains/isolates including 23 Staph. aureus, 18 nonaureus Staphylococcus and 43 other common bacterial isolates were examined. The results indicated that PCR primers from Sa0836 and Sa0856 recognized genomic DNA from Staph. aureus only, but not from other non-aureus Staphylococcus or common bacteria. Conclusions:, PCR detection of the putative transcriptional regulator genes Sa0836 and Sa0856 represents a useful means of identifying Staph. aureus from other bacteria. Significance and Impact of the Study:, The existence of species,species transcriptional regulator genes may be a common phenomenon in bacteria. Besides their value as novel diagnostic markers, further investigation on the putative transcriptional regulator genes Sa0836 and Sa0856 and their related products may shed light on the molecular mechanisms of Staph. aureus adaptation and virulence. Simple sequence repeats, or microsatellite markers, are the most suitable marker for genetic fingerprinting because they are multi-allelic and co-dominant. [source] Formation of microporous poly(hydroxybutyric acid) membranes for culture of osteoblast and fibroblastPOLYMERS FOR ADVANCED TECHNOLOGIES, Issue 12 2009Han-Shiang Huag Abstract Microporous membranes of a biodegradable polymer, poly(hydroxybutyric acid) (PHB), were prepared by a phase-inversion process and their cell compatibility was evaluated in vitro. A ternary system, ethanol/chloroform/PHB, was employed to prepare the membranes, wherein ethanol and chloroform were served as the nonsolvent and solvent for PHB, respectively. In the phase-inversion process, the polymer dissolution temperature was varied from 80 to 120°C to yield membranes with specific morphologies, such as globular particles, porous channels, etc. Moreover, cell viability was examined on the formed membranes. Two cell lines, osteoblast hFOB1.19 and fibroblast L929, were cultured in vitro. It was found that these two types of cells exhibited different responses on different membranes: the hFOB1.19 cells showed significant increase in cell proliferation with increase in surface roughness, whereas the L929 cells demonstrated an opposite trend, preferring to attach and grow on a flat surface. PHB membranes with different morphologies exhibit different cell compatibilities, which may be useful means for the architectural design of materials for tissue engineering. [source] Sequence-specific gene silencing in murine muscle induced by electroporation-mediated transfer of short interfering RNATHE JOURNAL OF GENE MEDICINE, Issue 1 2004Tsunao Kishida Abstract Background Post-genomic biomedical research requires efficient techniques for functional analyses of poorly characterized genes in living organisms. Sequence-specific gene silencing in mammalian organs may provide valuable information on the physiological and pathological roles of predicted genes in mammalian systems. Here, we attempted targeted gene knockdown in vivo in murine skeletal muscle through the electroporation-mediated transfer of short interfering RNA (siRNA). Methods siRNA duplexes corresponding to the firefly luciferase (Luc), green fluorescent protein (GFP), or glyceraldehyde-3-phosphate dehydrogenase (GAPD) genes were delivered by electroporation into the tibial muscle of normal or enhanced GFP (EGFP) transgenic mice. Plasmid vectors carrying the Luc, hRluc or ,-galactosidase (,-gal) reporter genes were also delivered. The Luc and hRluc activities in the muscle lysates were assayed. The EGFP and GAPD expression was detected by fluorescence microscopic observation and RT-PCR, respectively. Results When Luc-specific siRNA was co-delivered with the Luc expression vector into the tibial muscle, the reporter gene expression was markedly suppressed (less than 1% of the control level) for 5 days. As little as 0.05 µg of siRNA almost completely blocked the reporter gene expression from 10 µg of the plasmid. To examine whether siRNA can also suppress expression of an endogenous gene, transgenic mice carrying the EGFP gene received intramuscular transfection of a mixture of ,-gal plasmid and GFP-specific siRNA. ,-Gal-positive cells failed to express detectable levels of EGFP, while EGFP expression was not inhibited in control mice that received nonspecific siRNA. Expression of GAPD was also suppressed by the specific siRNA. Conclusions The present system may provide a useful means of phenotypic analysis of genetic information in mammalian organs for basic research as well as therapeutic molecular targeting in the post-genomic era. [source] Effects of reverse Trendelenburg position and inguinal compression on femoral vein cross-sectional area in infants and young childrenANAESTHESIA, Issue 4 2009E. H. Suk Summary This study evaluated the effects of the reverse Trendelenburg position and additional inguinal compression on the cross-sectional area of the femoral vein in paediatric patients. Seventy subjects were allocated to two groups: the infants group and the children group. Cross-sectional area of the femoral vein was measured just below the inguinal ligament using ultrasound. Three measurements were obtained for each patient: (i) supine, (ii) reverse Trendelenburg position and (iii) reverse Trendelenburg position with inguinal compression. In the infants group, femoral vein cross-sectional area increased by a mean (SD) of 21.1 (15.2) % in the reverse Trendelenburg position and by 60.7 (30.8) % in the reverse Trendelenburg position with inguinal compression; whereas in the children group, femoral vein cross-sectional area increased by 24.7 (15.8) % in the reverse Trendelenburg position and by 100.3 (50.7) % in the reverse Trendelenburg position with inguinal compression. Inguinal compression in the reverse Trendelenburg position offers a useful means of increasing femoral vein cross-sectional area in paediatric patients. [source] Retention of airborne latex particles by a bacterial and viral filter used in anaesthesiaANAESTHESIA, Issue 3 2001apparatus We have developed a series of laboratory tests to evaluate the efficiency of a heat and moisture exchanger filter (PallÔ BB25) in retaining latex particles in order to protect allergic patients during anaesthesia. Latex particles were nebulised with cornstarch as a support and collected for assay in a flask, with or without the filter integrated into the experimental circuit. With the Pall BB25 filter in the circuit, no natural latex proteins were detected by measurement of either total protein or antigenic latex proteins. The Pall BB25 filter may represent a useful means of preventing inhalation of latex particles during anaesthesia in susceptible patients. [source] Cities and the Geographies of "Actually Existing Neoliberalism"ANTIPODE, Issue 3 2002Neil Brenner This essay elaborates a critical geographical perspective on neoliberalism that emphasizes (a) the path,dependent character of neoliberal reform projects and (b) the strategic role of cities in the contemporary remaking of political,economic space. We begin by presenting the methodological foundations for an approach to the geographies of what we term "actually existing neoliberalism." In contrast to neoliberal ideology, in which market forces are assumed to operate according to immutable laws no matter where they are "unleashed," we emphasize the contextual embeddedness of neoliberal restructuring projects insofar as they have been produced within national, regional, and local contexts defined by the legacies of inherited institutional frameworks, policy regimes, regulatory practices, and political struggles. An adequate understanding of actually existing neoliberalism must therefore explore the path,dependent, contextually specific interactions between inherited regulatory landscapes and emergent neoliberal, market,oriented restructuring projects at a broad range of geographical scales. These considerations lead to a conceptualization of contemporary neoliberalization processes as catalysts and expressions of an ongoing creative destruction of political,economic space at multiple geographical scales. While the neoliberal restructuring projects of the last two decades have not established a coherent basis for sustainable capitalist growth, it can be argued that they have nonetheless profoundly reworked the institutional infrastructures upon which Fordist,Keynesian capitalism was grounded. The concept of creative destruction is presented as a useful means for describing the geographically uneven, socially regressive, and politically volatile trajectories of institutional/spatial change that have been crystallizing under these conditions. The essay concludes by discussing the role of urban spaces within the contradictory and chronically unstable geographies of actually existing neoliberalism. Throughout the advanced capitalist world, we suggest, cities have become strategically crucial geographical arenas in which a variety of neoliberal initiatives,along with closely intertwined strategies of crisis displacement and crisis management,have been articulated. [source] Triggering of proteinase-activated receptor 4 leads to joint pain and inflammation in miceARTHRITIS & RHEUMATISM, Issue 3 2009Jason J. McDougall Objective To investigate the role of proteinase-activated receptor 4 (PAR-4) in mediating joint inflammation and pain in mice. Methods Knee joint blood flow, edema, and pain sensitivity (as induced by thermal and mechanical stimuli) were assessed in C57BL/6 mice following intraarticular injection of either the selective PAR-4 agonist AYPGKF-NH2 or the inactive control peptide YAPGKF-NH2. The mechanism of action of AYPGKF-NH2 was examined by pretreatment of each mouse with either the PAR-4 antagonist pepducin P4pal-10 or the bradykinin antagonist HOE 140. Finally, the role of PAR-4 in mediating joint inflammation was tested by pretreating mice with acutely inflamed knees with pepducin P4pal-10. Results PAR-4 activation caused a long-lasting increase in joint blood flow and edema formation, which was not seen following injection of the control peptide. The PAR-4,activating peptide was also found to be pronociceptive in the joint, where it enhanced sensitivity to a noxious thermal stimulus and caused mechanical allodynia and hyperalgesia. The proinflammatory and pronociceptive effects of AYPGKF-NH2 could be inhibited by pepducin P4pal-10 and HOE 140. Finally, pepducin P4pal-10 ameliorated the clinical and physiologic signs of acute joint inflammation. Conclusion This study demonstrates that local activation of PAR-4 leads to proinflammatory changes in the knee joint that are dependent on the kallikrein,kinin system. We also show for the first time that PARs are involved in the modulation of joint pain, with PAR-4 being pronociceptive in this tissue. Thus, blockade of articular PAR-4 may be a useful means of controlling joint inflammation and pain. HAMILTON Archaeological and molecular data have revealed that the present day faunas of many island groups in Melanesia, Polynesia, and Micronesia are not representative of the biodiversity generated within this region on an evolutionary timescale. Erroneous inferences regarding the mechanisms of speciation and the significance of long distance dispersal in shaping the present diversity of these island systems have resulted from this incomplete diversity and distributional data. The lizard fauna east of Samoa has been suggested to derive entirely from human-mediated introductions, a distribution congruent with biogeographic patterns for other Pacific species. Distinguishing between introduced populations and those that result from natural colonization events is difficult, although molecular data provide a useful means for elucidating population history and identifying the likely sources of introductions. We use molecular data (1726 bp of mitochondrial DNA and 286 bp of nuclear DNA) to evaluate a population of arboreal lizards from the Cook Islands and to determine whether this arboreal skink population is the sole endemic component of the lizard fauna east of Samoa or the result of human-mediated introduction. © 2010 The Linnean Society of London, Biological Journal of the Linnean Society, 2010, 100, 318,328. [source] Analysis of chemical and metabolic components in traditional Chinese medicinal combined prescription containing Radix Salvia miltiorrhiza and Radix Panax notoginseng by LC-ESI-MS methodsBIOMEDICAL CHROMATOGRAPHY, Issue 8 2007Ying-Jie Wei Abstract High-performance liquid chromatography,electrospray ionization-mass spectrometry (LC-ESI-MS) methods were developed for the analysis of chemical and metabolic components in traditional Chinese medicinal combined prescription containing Radix Salvia miltiorrhiza and Radix Panax notoginseng (commonly known as Fufang Danshen prescription, FDP). The HPLC experiments used a reversed-phase Zorbax C18 column with the column temperature at 30°C and a binary mobile phase system consisting of aqueous formic acid (0.1%, v/v) and acetonitrile using a gradient elution at the flow rate of 1.0 mL/min. The ESI-MS was operated with a single-quadrupole mass spectrometer in both negative and positive ion modes. 36 major chromatographic peaks of FDP, including 14 saponins, 13 phenolic acids and nine diterpenoid quinones were characterized by their MS spectra and in comparison with some of the reference standards. In addition, after oral administration of extraction of FDP, the rat's plasma, urine and feces were also analyzed; 53 metabolic components including 30 original components and 23 transformative components of FDP were detected, and possible metabolic pathways of some components in FDP were given. The analysis of chemical and metabolic components in FDP by HPLC-MS methods could be a useful means of identifying the multi-components of FDP and to hint at their possible metabolic mechanism of action in the body. Sheen Background: This paper reports a 7-year experience of cryoablation for colorectal and non-colorectal liver metastases. Methods: A retrospective review was undertaken of patients treated in two adjacent UK centres in the north-west of England. Results: Over a 7-year period (1993,2000), 57 patients underwent cryotherapy for malignant hepatic tumours (41 colorectal, 16 non-colorectal). In the patients with colorectal metastases, preoperative carcinoembryonic antigen (CEA) levels fell significantly, from a mean of 444·1 to 6·22 µg/l (P = 0·002). One patient died, two developed cryoshock and six had cardiorespiratory complications. All patients with colorectal metastases subsequently received 5-fluorouracil-based chemotherapy. The remaining 16 patients with non-colorectal tumours (seven neuroendocrine metastases, five hepatocellular carcinomas, three sarcomas, one cholangiocarcinoma) all received cryotherapy alone, with no major complications. The median survival for patients with non-colorectal metastases was 37 months, compared with 22 months for those with colorectal metastases (P = 0·005). Conclusion: Hepatic cryotherapy is effective and safe, as demonstrated by the significant reduction in postoperative CEA concentration and the low risk of complications. However, this initial short-term success was not reflected in 5-year survival rates. Yamaya Carbon monoxide (CO) can be detected in exhaled air and is increased in asthmatic patients not treated with corticosteroids. However, it is uncertain whether exhaled CO is related to severity of asthma. To study whether exhaled CO is related to severity of asthma in clinical courses, exhaled CO concentrations were measured on a CO monitor by vital capacity manoeuvre in 20 mild asthmatics treated with inhaled ,2-agonists alone, 20 moderate asthmatics treated with inhaled corticosteroids, and 15 stable asthmatics treated with high dose inhaled corticosteroids and oral corticosteroids once a month over 1 years. Exhaled CO concentrations were also measured in 16 unstable severe asthmatics who visited the hospital every 7 or 14 days for treatment with high dose inhaled corticosteroids and oral corticosteroids. The mean values of exhaled CO in severe asthma over 1 year were 6.7 ± 9.5 p.p.m. (n = 31, mean ± SD) and significantly higher than those of non-smoking control subjects (1.2 ± 0.9 p.p.m., n = 20, P < 0.01). Exhaled CO concentrations in unstable severe asthmatics were significantly higher than those in stable severe asthmatics. However, exhaled CO concentrations in mild and moderate asthmatics did not differ significantly from those in non-smoking control subjects (P > 0.20). There was a significant relationship between the exhaled CO concentrations and forced expiratory volume in one second in all asthmatic patients. These findings suggest that exhaled CO concentrations may relate to the severity of asthma and measurements of exhaled CO concentrations may be a useful means of monitoring airway inflammation in asthma. Gorelick MD Abstract Objectives: To develop and validate a multivariable model, using information available at the time of patient triage, to predict the level of care provided to pediatric emergency patients for use as a severity of illness measure. Methods: This was a retrospective cohort study of 5,521 children 18 years of age or younger treated at four emergency departments (EDs) over a 12-month period. Data were obtained from abstraction of patient records. Logistic regression was used to develop (75% of sample) and validate (25% of sample) models to predict any nonroutine diagnostic or therapeutic intervention in the ED and admission to the hospital. Data on ED length of stay and hospital costs were also obtained. Results: Eight predictor variables were included in the final models: presenting complaint, age, triage acuity category, arrival by emergency medical services, current use of prescription medications, and three triage vital signs (heart rate, respiratory rate, and temperature). The resulting models had adequate goodness of fit in both derivation and validation samples. The area under the receiver operating characteristic curve was 0.73 for the ED intervention model and 0.85 for the admission model. The Revised Pediatric Emergency Assessment Tool (RePEAT) score was then calculated as the sum of the predicted probability of receiving intervention and twice the predicted probability of admission. The RePEAT score had a significant univariate association with ED costs (r= 0.44) and with ED length of stay (r= 0.27) and contributed significantly to the fit of multivariable models comparing these outcomes across sites. Conclusions: The RePEAT score accurately predicts level of care provided for pediatric emergency patients and may provide a useful means of risk adjustment when benchmarking outcomes. [source] Comparison of two different PCR amplification products (the 18-kDa protein gene vs.CLINICAL & EXPERIMENTAL DERMATOLOGY, Issue 4 2003RLEP repetitive sequence) in the diagnosis of Mycobacterium leprae Summary To determine the best molecular method for diagnosing leprosy, two sets of Mycobacterium leprae -specific primers were compared. Fresh biopsies and slit skin smear samples were obtained from 67 leprosy patients and examined by touchdown (TD) PCR using primers amplifying either a 129-bp fragment of the RLEP repetitive sequence or a 360-bp fragment of the 18-kDa protein gene of M. leprae. Seventeen of 30 (56.7%) biopsy specimens and four of 37 (10.8%) slit skin smear specimens were positive using the primer for the 18-kDa protein gene, whereas 24 of 30 (80%) biopsy and 27 of 37 (73%) slit skin smear samples showed detectable PCR products in the RLEP repetitive sequence. Twenty-one of 31 cases (67.7%) with a bacterial index of zero were PCR positive for the primer RLEP repetitive sequence. These results demonstrate that detection of M. leprae using PCR with primers to a RLEP sequence is more sensitive and specific than PCR with the 18-kDa protein gene primers and also slit smears with acid fast staining. PCR of RLEP repetitive sequences is therefore a useful means of detecting M. leprae DNA even when it is present at very low levels. [source] |
