    Unusual Left Atrial Appendage Mass: Atypical Presentation of Papillary Fibroelastoma

    Papillary fibroelastomas are small, histologically benign neoplasms that are typically found on the valvular endocardium. We report a patient with a papillary fibroelastoma in an unusual location: the left atrial appendage. Although the mass was visualized both by computed tomography (CT) and transesophageal echocardiography, the diagnosis was histologically confirmed after surgical excision. [source]


    Adenylyl cyclase Rv0386 from Mycobacterium tuberculosis H37Rv uses a novel mode for substrate selection

    Class III adenylyl cyclases usually possess six highly conserved catalytic residues. Deviations in these canonical amino acids are observed in several putative adenylyl cyclase genes as apparent in several bacterial genomes. This suggests that a variety of catalytic mechanisms may actually exist. The gene Rv0386 from Mycobacterium tuberculosis codes for an adenylyl cyclase catalytic domain fused to an AAA-ATPase and a helix-turn-helix DNA-binding domain. In Rv0386, the standard substrate, adenine-defining lysine-aspartate couple is replaced by glutamine-asparagine. The recombinant adenylyl cyclase domain was active with a Vmax of 8 nmol cAMP·mg,1·min,1. Unusual for adenylyl cyclases, Rv0386 displayed 20% guanylyl cyclase side-activity with GTP as a substrate. Mutation of the glutamine-asparagine pair either to alanine residues or to the canonical lysine-aspartate consensus abolished activity. This argues for a novel mechanism of substrate selection which depends on two noncanonical residues. Data from individual and coordinated point mutations suggest a model for purine definition based on an amide switch related to that previously identified in cyclic nucleotide phosphodiesterases.     An Unusual Zip Gun Suicide,Medicolegal and Ballistic Examination

    Abstract:, Home-made guns are imitations of typical firearms and usually have handgun characteristics. This article presents an unusual case of a suicide carried out by means of a fatal gunshot wound to the head using a home-made zip gun. A 49-year-old male, with a history of paranoid psychosis was found dead in the dwelling place of a family house. The investigation at the crime scene did not lead to suspicion of a gunshot wound because of the unusual nature of the firearm used. A medical examiner diagnosed an opened head injury as the primary cause of the victim's death. The autopsy findings provided immediate grounds for further inspection of the crime scene. Subsequently, a simple zip gun, which had been overlooked during the scene investigation, was discovered. An undeformed projectile recovered from the victim's head was consistent with the use of the home-made firearm. Following the completion of the investigations and autopsy, the death was classified as a suicide. [source]


    Detection of unusual rotavirus genotypes G8P[8] and G12P[6] in South Korea

    Abstract Five hundred four fecal specimens, collected between 2004 and 2006 from young children with acute diarrhea, were screened for rotavirus by ELISA with VP6-specific antibody. Of these samples, 394 (78.2%) were confirmed as group A rotavirus and they underwent G- and P typing using a combination of ELISA, RT-PCR, and sequence analysis methods. The dominant circulating G serotype was G1 (35.6%) followed by G3 (26.4%), G4 (14.7%), and G2 (11.9%). There was a low prevalence of G9 (1.0%) and of unusual G type rotavirus, in particular, G12 (0.5%) and G8 (0.3%). Of the P genotype rotavirus in circulation, P[8] (53.0%) was most common followed by P[6] (15.5%), P[4] (15.2%), and P[9] (2.3%). Determination of G- and P type combinations revealed that G1P[8] strains were most prevalent (25.4%), amid G3P[8] (16.8%), G2P[4] (6.3%), and G4P[6] (6.1%) strains. Unusual or rare combinations such as G2P[6], G2P[8], G3P[4], G2P[9], G1P[9], G3P[9], G12P[6], G1P[4], G3P[6], and G8P[8] were also found. Owing to the recent emergence of G8 and G12 rotavirus, the findings from this study are important since they provide new information concerning the local and global spread of rotavirus genotypes. J.     Is Post-Communist Health Spending Unusual?

    What factors determine a country's spending on health? And what factors determine the share of spending financed by the public sector? Taking these factors into account, is post-communist health spending unusual? For the OECD economies, we find that per capita health spending is strongly related to per capita income, with an elasticity of about 1.5. The elasticity for developing economies is close to one. Spending is also positively related to the elderly dependency rate, but the relationship is weaker than a static comparison of spending by the elderly and non-elderly would suggest. Even though health spending as a share of GDP in the post-communist countries of eastern and central Europe is below the OECD average, there is evidence of above normal health spending in most countries when we control for income and demographics. For Hungary, the ,excess' spending reached over three percentage points of GDP in 1994. For the OECD sample, four development indicators account for half the variation in the public sector share of total health spending. Political variables help explain the remainder. If the post-communist countries converge to the market economy pattern, the share of public financing will fall, yet still remain well above half. [source]

    Suspected involvement of the X chromosome in placental mesenchymal dysplasia

    ABSTRACT, So far, 46 cases of placental mesenchymal dysplasia have been reported worldwide. We encountered 15 cases of placental mesenchymal dysplasia (PMD) including 7 cases delivered in our hospital. The incidence of PMD in our hospital was therefore, 7/30, 758 (0.02%). The PMD had a peculiar appearance. In the gross findings, large placenta with intestine-like dilatation of the vessels on the fetal side was reported. Microscopically, cistern-like dilatation of the stem villi, fetal artery thrombosis, and villous hemorrhage were reported. However, we believe most of these findings are secondary rather than the primary of mesenchymal dysplasia. Therefore, we investigated 15 other cases of mesenchymal dysplasia, and found including vascular abnormality of the stem, intermediate and terminal villi in all case of PMD. The abnormality was observed in the vessels of the periphery of the stem villi and their vessel walls were thin and appeared weak. The intermediate villous vessels were unusual, tangled. The terminal villous abnormalities showed chorangiosis and stromal hyperplasia. These findings are mesenchymal dysplasia origin. Moreover, PMD showed female-predominant. 14/15 was female among our cases, We discuss the relationship between mesenchymal dysplasia and the X chromosome in this paper. [source]


    An Ecological and Economic Assessment of the Nontimber Forest Product Gaharu Wood in Gunung Palung National Park, West Kalimantan, Indonesia

    We studied the demographic effect and economic returns of harvesting aromatic gaharu wood from fungus-infected trees of Aquilaria malaccensis Lam. at Gunung Palung National Park, Indonesia, to evaluate the management potential of gaharu wood. Aquilaria malaccensis trees openface> 20 cm in diameter occurred at low preharvest densities (0.16,0.32 ha) but were distributed across five of six forest types surveyed. During a recent harvest, 75% of trees were felled, with harvest intensities ranging from 50% to 100% among forest types. Overall, 50% of trees contained gaharu wood, but trees at higher elevations contained gaharu wood more frequently ( 73%) than trees at lower elevation (27%). The mean density of regeneration ( juveniles> 15 cm in height) near adult trees (3,7 m away) was 0.2/m2, 200 times greater than at random in the forest (10/ha), but long-term data on growth and survivorship are needed to determine whether regeneration is sufficient for population recovery. Gaharu wood extraction from Gunung Palung was very profitable for collectors, generating an estimated gross financial return per day of US $8.80, triple the mean village wage. Yet, the estimated sustainable harvest of gaharu wood at natural tree densities generates a mean net present value of only $10.83/ha, much lower than that of commercial timber harvesting, the dominant forest use in Kalimantan. Returns per unit area could be improved substantially, however, by implementing known silvicultural methods to increase tree densities, increase the proportion of trees that produce gaharu wood, and shorten the time interval between successive harvests. The economic potential of gaharu wood is unusual among nontimber forest products and justifies experimental trials to develop small-scale cultivation methods. Resumen: Datos ecológicos y económicos son esenciales para la identificación de productos forestales no maderables tropicales con potencial para la extracción sostenible y rentable en un sistema bajo manejo. Estudiamos el efecto demográfico y los beneficios económicos de la cosecha de la madera aromática gaharu de árboles de Aquilaria malaccenis Lam infectados por hongos en el Parque Nacional Gunung Palung Indonesia para evaluar el potencial de manejo de la madera. Arboles de Aquilaria malaccenis> 20 cm de diámetro ocurrieron en bajas densidades precosecha (0.16,0.32 ha,1) pero se distribuyeron en cinco de los seis tipos de bosque muestreados. Durante una cosecha reciente, 75% de los árboles fueron cortados, con intensidades de cosecha entre 50 y 100% en los tipos de bosque. En conjunto, 50% de los árboles contenían madera gaharu, pero árboles de elevaciones mayores contenían madera gaharu más frecuentemente ( 73%) que árboles de elevaciones menores (27%).     Violent recidivism among mentally disordered offenders in Japan

    Background,A new forensic mental health law was enacted in Japan in 2003, enabling development of specialist services. Before their establishment, it is important to determine the nature, frequency and correlates of the problems they are designed to ameliorate. Aims,To establish rates of violent recidivism among mentally disordered offenders before the new legislation, and examine associated risk factors. Method,Data were extracted from one complete annual (1980) national cohort of people judged by the Court or prosecutor to be without responsibility for a criminal offence, or of sufficiently diminished responsibility for it to be diverted for psychiatric treatment. The outcome measure was violent recidivism after community discharge and before 1991. Results,Fifty-two (10%) of 489 in the cohort were arrested or convicted of further violent offences. Violent recidivism was most strongly associated with a substance-related disorder, but histories of violence, homelessness and short index admissions were independently related. Conclusions,Violent recidivism was so unusual that, on this outcome, it could take many years to show any effect of the new service. Desistance from substance use, compliance with treatment and maintenance of stable housing may be better indicators of success, and their achievement a good preventive strategy.     Name change among offender patients: an English high security hospital sample

    Background There is scant literature on change of name among psychiatric patients but a more substantial amount on the use of aliases among offenders. No one to our knowledge has explored name changes among offender patients. Aim This study was undertaken to establish the prevalence of name change among high security hospital patients and to compare those who changed name with those who did not. Hypothesis It was hypothesized that name change would be associated with male gender, repeated offending and a diagnosis of personality disorder. Method The clinical records of all patients resident in one high security hospital on 31 January 2000 were searched. As all such patients are compulsorily detained, all name changes must be recorded. Any change prior to that date was also noted, together with basic demographic and diagnostic information; supplementary data were obtained from the special hospitals' case register. Results Seventy-one patients (17%) of the resident population changed names (exclusive of a woman changing her surname on marriage). Name changing was associated with disrupted upbringing. Patients with personality disorder were more likely to change names than those with psychosis, regardless of sex, age or ethnic group. Those with psychosis were more likely to select unusual or symbolic names. Conclusions On the evidence of previous literature, people from this serious offender patient population were more likely to change names than other psychiatric patients but less likely than non-mentally-disordered offenders. Changes by people with psychosis seemed related to their illness, whereas changes by those with personality disorder might reflect childhood disruptions in rearing patterns.     Subungual Metastasis from a Rectal Primary: Case Report and Review of the Literature

    BACKGROUND Subungual metastases from colorectal cancer are unusual and have mainly been reported in patients with lung, genitourinary, and breast cancer. OBJECTIVE We present the case of a 72-year-old man with rectal adenocarcinoma and a subungual metastasis to the left thumb 5 years later. METHODS A case report and a brief review of the literature of subungual metastases are given. RESULTS The thumb was amputated and the patient died 6 months later with extensive metastatic disease. CONCLUSION Metastatic carcinoma should be considered in the differential diagnosis of persistent subungual masses, particularly in patients with a history of cancer. The prognosis with such lesions is generally poor. [source]


    Sebaceous Carcinoma, Basal Cell Carcinoma, Trichoadenoma, Trichoblastoma, and Syringocystadenoma Papilliferum Arising within a Nevus Sebaceus

    Background. Nevus sebaceus has a well-documented potential to develop a wide variety of neoplasms of both epidermal and adnexal origins. It is highly unusual for more than three tumors to arise simultaneously within a single nevus sebaceus. Sebaceous carcinoma arising within a nevus sebaceus is a rare occurrence. Objective. The objective was to report the case of a patient with a nevus sebaceus that simultaneously developed five distinct neoplasms of epidermal and various adnexal origins and to report the fourth case of sebaceous carcinoma arising within a nevus sebaceus. Methods. A 45-year-old woman presented with a nevus sebaceusthat contained five separate neoplasms, including sebaceous carcinoma, basal cell carcinoma, trichoadenoma, trichoblastoma, and syringocystadenoma papilliferum. Results. Complete excision of the nevus sebaceus and the five tumors was performed. Systemic work-up showed no evidence of metastatic disease or association with Muir-Torre syndrome. Conclusion. This case report highlights the diverse neoplastic potential of nevus sebaceus and demonstrates the capacity of this hamartoma to develop aggressive tumors, such as sebaceous carcinoma. Prophylactic excision or at least close clinical surveillance for sudden development of new growths is warranted in all cases of nevus sebaceus. [source]


    Pigmented Bowen's Disease (Squamous Cell Carcinoma in situ): A Mimic of Malignant Melanoma

    Background. Darkly pigmented individuals may manifest unusual or uncharacteristic presentations of various skin conditions, including heavy pigmentation of cutaneous tumors. Objective. To increase the awareness of an unusual presentation of Bowen's disease in a darkly pigmented individual. Methods. We report the case of a 52 year old black woman that presented with a lesion clinically consistent with malignant melanoma. However, histopathologic examination revealed pigmented Bowen's disease. Results. A biopsy is almost always indicated to confirm the diagnosis of lesions in darkly pigmented individuals. Conclusion. This case is presented to reinforce the idea that pigmented Bowen's disease should be considered in the differential diagnosis of malignant melanoma. [source]


    Postembryonic development of the cranial lateral line canals and neuromasts in zebrafish

    Abstract The development of the cranial lateral line canals and neuromast organs are described in postembryonic zebrafish (0,80 days postfertilization). Cranial canal development commences several weeks after hatch, is initiated in the vicinity of individual neuromasts, and occurs in four discrete stages that are described histologically. Neuromasts remain in open canal grooves for several weeks during which they dramatically change shape and increase in size by adding hair cells at a rate one-tenth that in the zebrafish inner ear. Scanning electron microscopy demonstrates that neuromasts elongate perpendicular to the canal axis and the axis of hair cell polarization and that they lack a prominent nonsensory cell population surrounding the hair cells,features that make zebrafish neuromasts unusual among fishes. These results demand a reassessment of neuromast and lateral line canal diversity among fishes and highlight the utility of the lateral line system of postembryonic zebrafish for experimental and genetic studies of the development and growth of hair cell epithelia.     Depressive pseudodementia in a child with autism

    Depression is rare in early childhood and unusual in autism in this age group. We describe a female child aged 6 years with autism who presented with regression of developmental skills previously gained. Her sleep and appetite were poor, her affect was sad, and she had morbid speech content. She responded to treatment with antidepressant medication. When this clinical picture occurs in adults it is called depressive pseudodementia; paediatric neurologists and neuropsychiatrists need to be aware of it in children. [source]


    Pulmonary non-Hodgkin's lymphoma (NHL) of diffuse large B-cell type with simultaneous humeral involvement in a young lady: An uncommon presentation with cytologic implications

    Abstract A bronchogenic carcinoma, almost invariably, presents as a lung mass. Primary pulmonary lymphomas are rare. We report an unusual case of a pulmonary non-Hodgkin's lymphoma (NHL) with simultaneous involvement of the right humerus in a 37 year old lady. Bronchial lavage smears showed atypical cells with irregular nuclear membranes raising a suspicion of a hematolymphoid tumor, over a small cell carcinoma that was the closest differential diagnosis. Biopsy from the lung mass and from the lesion in the humerus showed an identical malignant round cell tumor with prominent apoptosis. On immunohistochemistry (IHC), tumor cells were diffusely positive for leukocyte common antigen (LCA), CD20 and MIB1 (70%), while negative for cytokeratin (CK), epithelial membrane antigen (EMA) synaptophysin, chromogranin, neuron specific enolase (NSE), CD3, and CD10. Diagnosis of a pulmonary NHL of diffuse large B-cell type with involvement of the humerus was formed. The case is presented to create an index of suspicion for the possibility of a NHL on respiratory samples, while dealing with small round cells with irregular nuclear membranes. IHC is necessary to confirm he diagnosis. A simultaneous association in the humerus in our case makes it unusual. Diagn. Cytopathol. 2010. © 2009 Wiley-Liss, Inc. [source]


    Aspiration cytology of sarcomatoid carcinoma of the breast: Report of a case with cystic change

    Abstract Sarcomatoid/metaplastic carcinomas of the breast are rare breast malignancies that show a myriad of cytohistologic patterns in aspirates. These poorly differentiated invasive carcinomas contain both ductal and mesenchymal elements with transitional forms displaying either spindle, squamous, chondroid, or osseous differentiation. We describe such a neoplasm in a 68-yr-old woman, the diagnosis of which was missed at the initial fine-needle aspiration (FNA) due to cystic change.     The Role of Research Institutions in Seed,related Disaster Relief: Seeds of Hope Experiences in Rwanda

    The article describes the efforts of a coalition of agricultural research centres, Seeds of Hope (SOH) in the rebuilding of Rwanda, after the genocide and war of 1994. Research involvement in emergency relief and rehabilitation was unusual at the time and SOH had to forge its unique complementary role. Focusing on crop and variety development and conservation it: provided technical advice to relief agencies on seed procurement; used its baseline ken to assess the effects of war on seed diversity and seed security; made preparations to restore specific germplasm (which, fortunately, proved unnecessary) and spent substantial effort on rebuilding human resource capacity in research as well as basic scientific facilities. The involvement of SOH highlighted the critical, yet very different, roles for research during emergency versus rehabilitation periods and demonstrated the cost effectiveness of building in a diagnostic component , before massive seed or germplasm distributions are programmed.     Effects of hind limb denervation on the development of appendicular ossicles in the Dwarf African Clawed Frog, Hymenochirus boettgeri (Anura: Pipidae)

    Abstract Sesamoids and other appendicular ossicles are common in other classes of vertebrates but comparatively rare in amphibians. The pipid frog Hymenochirus boettgeri (Boulenger, G. A. 1899. On Hymenochirus, a new type of aglossal batrachians. , Annals of the Magazine of Natural History Series 7: 122,125) is unusual among anurans in having seven (or more) appendicular ossicles in each hind limb. Sesamoids are often associated with muscles and tendons, and their development is usually regarded as mediated by or correlated with function. This study investigated the effects of paralysis (loss of function) on development of ossicles in the hind limb of Hymenochirus. Complete denervation of the right sciatic nerve was performed at developmental stages 63 and 66, and the animals maintained for a further 6,7 or 12,13 weeks. Specimens were cleared and double stained for cartilage and bone. There were no gross morphological differences between control and sham operated groups. The lunulae were not affected by paralysis, whereas the fabella arose later and/or regressed in some specimens. The distal os sesamoides tarsalia (OST) was shorter in paralysed individuals, and both the distal OST and cartilagines plantares showed delayed maturation. Denervation of the hind limb thus affected the timing of appearance, maintenance and rate of maturation of some sesamoid bones in Hymenochirus, but had no effect on others. [source]


    Histology, histochemistry and morphometry of the ovary of the adult plains viscacha (Lagostomus maximus) in different reproductive stages

    Abstract Lagostomus maximus is a South American Hystricognathi rodent whose reproductive behaviour shows characteristics unusual for mammals, such as polyovulation (200,800 oocytes) and a high rate of embryo mortality. Thirty-six mature females captured in the province of Buenos Aires showed different physiological reproductive stages. Most of them presented a postpartum oestrus in August,September. This characteristic is different from that reported in other geographical areas. The stages considered were: anoestrus, follicular phase, early pregnancy and late pregnancy. The ovaries were light-pink and smooth and presented a tortuous cord-like aspect. Many primordial follicles were found in all the females studied. Follicles in different states of maturation and primary and accessory corpora lutea were observed in the cortex. These structures were smaller than those present in other related species. Follicles did not project into the surface of the organ. Calcified follicles of diverse size were found in all the ovaries. Atretic follicles were found in all the stages analysed. Interstitial tissue surrounding the follicles and the corpora lutea was also observed. The number and proportion of different cortical structures varied in the physiological stages analysed. The ovaries of the viscacha have differential characteristics in comparison to other Hystricognathi, some of them related to polyovulation. [source]


    Anatomy of a Pennine peat slide, Northern England

    Abstract This paper describes and analyses the structure and deposits of a large UK peat slide, located at Hart Hope in the North Pennines, northern England. This particular failure is unusual in that it occurred in the winter (February, 1995) and shows excellent preservation of the sedimentary structures and morphology, both at the failure scar and downstream. The slide was triggered by heavy rain and rapid snowmelt along the line of an active peatland stream flush. Detailed mapping of the slide area and downstream deposits demonstrate that the slide was initiated as a blocky mass that degenerated into a debris flow. The slide pattern was complex, with areas of extending and compressive movement. A wave-like motion may have been set up in the failure. Within the slide site there was relatively little variability in block size (b axis); however, downstream the block sizes decrease rapidly. Stability analysis suggests the area at the head of the scar is most susceptible to failure. A ,secondary' slide area is thought to have only been initiated once the main failure had occurred. Estimates of the velocity of the flowing peat mass as it entered the main stream channel indicate a flow velocity of approximately 10 m s,1, which rapidly decreases downstream. A sediment budget for the peat slide estimates the failed peat mass to be 30 800 t. However, sediment delivery to the stream channel was relatively low. About 37% of the failed mass entered the stream channel and, despite moving initially as debris flow, the amount of deposition along the stream course and on the downstream fan is small (only about 1%).     Estimation of dispersal distances of the obligately plant-associated ant Crematogaster decamera

    1. In obligate symbioses with horizontal transmission, the population dynamics of the partner organisms are highly interdependent. Host population size limits symbiont number, and distribution of partners is restricted by the presence and thus dispersal abilities of their respective partner. The Crematogaster decamera,Macaranga hypoleuca ant,plant symbiosis is obligate for both partners. Host survival depends on colonisation by its ant partner while foundress queens require hosts for colony establishment. 2. An experimental approach and population genetic analyses were combined to estimate dispersal distances of foundresses in their natural habitat in a Bornean primary rainforest. 3. Colonisation frequency was significantly negatively correlated with distance to potential reproductive colonies. Results were similar for seedlings at natural densities as well as for seedlings brought out in the area experimentally. Population genetic analysis revealed significant population differentiation with an FST of 0.041 among foundresses (n = 157) located at maximum 2280 m apart. In genetic spatial autocorrelation, genotypes of foundresses were significantly more similar than expected at random below 550 m and less similar above 620 m. Direct estimation of dispersal distances by pedigree analysis yielded an average dispersal distance of 468 m (maximum 1103 m). 4. For ants that disperse on the wing, genetic differentiation at such small spatial scales is unusual. The specific nesting requirements of the queens and the necessity for queens to find a host quickly could lead to colonisation of the first suitable seedling encountered, promoting short dispersal distances. Nonetheless, dispersal distances of C. decamera queens may vary with habitat or host spatial distribution. [source]


    The ecological and evolutionary significance of frost in the context of climate change

    The effects that below-freezing temperature (frost) can have at times of year when it is unusual are an interesting ecological phenomenon that has received little attention. The physiological consequence of formation of ice crystals in plant tissue is often death of the plants, or at least of sensitive parts that can include flower buds, ovaries, and leaves. The loss of potential for sexual reproduction can have long-lasting effects on the demography of annuals and long-lived perennials, because the short-term negative effects of frosts can result in longer-term benefits through lowered populations of seed predators. The loss of host plants can have dramatic consequences for herbivores, even causing local extinctions, and the loss of just flowers can also affect populations of seed predators and their parasitoids. Frosts can cause local extinctions and influence the geographical distribution of some species. The potential for global climate change to influence the frequency and distribution of frost events is uncertain, but it seems likely that they may become more frequent in some areas and less frequent in others.     Diagnosis at dusk: Malignant hypertension and phaeochromocytoma in a 6-year-old girl

    Abstract Phaeochromocytoma is a rare catecholamine-secreting tumour that may arise at any age, but is particularly unusual in childhood. The case of a 6-year-old girl who presented with a prolonged history of general malaise, headaches and abdominal pain is reported. On examination, she was noted to have malignant hypertension. Subsequent imaging of the abdomen demonstrated a left adrenal mass, with the diagnosis of phaeochromocytoma being confirmed by serial raised urinary metanephrines. Sympathetic blockade was established prior to definitive surgical treatment, resulting in complete resolution of the patient's symptoms and hypertension. Genetic screening of the family has since identified a previously undocumented missense mutation in the patient's VHL gene. The case raises the importance of routine measurement of blood pressure in all paediatric patients regardless of age, presentation or other factors. [source]
