Epstein-Barr virus (EBV) serology for predicting distant metastases in a white juvenile patient with nasopharyngeal carcinoma and no clinical response to EBV lytic induction therapy

Abstract Background. We describe a case of a 16-year-old white girl with Epstein-Barr virus (EBV)-positive nasopharyngeal carcinoma (NPC). Methods. At diagnosis, the patient had characteristic immunoglobulin (Ig)A and IgG responses to EBNA1, viral capsid antigen (VCA)-p18, and early antigens (EAs), with no detectable EBV DNA in her blood. Combined chemotherapy and radiotherapy resulted in complete remission. Eighteen months later, the patient's IgA responses to EBNA1 and p18 and both IgA and IgG anti-EA increased, without apparent recurrence. Five months later, lung metastases were found. She underwent surgical removal of the lung metastases and conventional chemotherapy, but had intraabdominal lymph node metastasis and mediastinal lesions develop. The patient was then treated with a novel treatment consisting of 5-fluorouracil plus valproic acid and subsequent valganciclovir to induce lytic EBV replication. This resulted in the first detectable EBV DNA levels in the blood but did not result in clinical response. Results. The patient's disease progressed, and the patient declined further cancer treatment and died. Conclusion. In contrast to EBV DNA load, EBV serology was useful in predicting distant NPC metastasis after initial complete remission in this patient. © 2006 Wiley Periodicals, Inc. Head Neck, 2006 [source]


Cutaneous and neurologic manifestations of biotinidase deficiency

A male newborn with no obstetric or familial antecedents, except that his parents were cousins, developed hypotonia, lethargy, and feeding problems from birth. Analysis revealed a marked metabolic acidosis and hyperammonemia. Three weeks later, he was admitted to hospital in order to receive parenteral nutrition and to undertake a study for metabolic diseases. The boy did not improve in spite of the use of parenteral nutrition and began to present with inspiratory stridor and tachypnea. One week later, he presented with an erythematous scaling eruption, which was especially intense in the lumbosacral region ( Fig. 1a,b). The scalp was only slightly affected. Figure 1. Erythematous scaling eruption, more intense in the lumbosacral region Laboratory findings were compatible with biotinidase deficiency diagnosed by demonstrating absent enzyme activity. His parents were also studied and they were found to have partial biotinidase deficiency (30% of enzyme activity). After 37 days of life, the baby was given a treatment consisting of 20 mg of biotin per day intravenously. Biochemical and neurologic alterations improved quickly. Meckel's diverticulum and a duodenal membrane were detected at the second month of life after a gastroduodenal survey, and both were operated on. The skin lesions did not improve, however, and intravenous biotin had to be increased to 40 mg/day. The eruption disappeared after 10 days. On his first birthday, he remained asymptomatic with 40 mg of oral biotin. [source]


Internal atmosphere, quality attributes and sensory evaluation of MAP packaged fresh-cut Conference pears

Summary Conference pear cubes processed at partially ripe maturity could preserve an acceptable sensory quality during 3-week storage. A processing treatment consisting of a dip in 10 g L,1 ascorbic acid and 5 g L,1 calcium chloride and a packaging atmosphere of 2.5 kPa O2 + 7% CO2 preserved the overall sensory shelf life without significant changes in relation to untreated freshly prepared samples. However, high CO2 conditions were responsible for a rapid degradation of the product during the following days. Under 0 kPa O2 atmosphere, the product underwent a progressive but slight loss of flavour; but colour and firmness variations were not detected through sensory tests. [source]


Sex-dependent response of primary moult to simulated time constraints in the rock sparrow Petronia petronia

There is growing evidence that moult speed affects plumage quality. In many bird species, males and females differ in terms of breeding effort, survival expectation and the relationship between fitness and plumage quality. Consequently, differences in moult strategies between the sexes can be expected. The aim of this study was to assess whether, under simulated time constraints and with no parental investment in the previous breeding season, males and females differed in: a) timing and duration of primary moult, b) growth rates of individual primary feathers, and c) number of concurrently growing feathers. We investigated the effect of time constraints generated by a treatment consisting of two decreasing photoperiods (slow changing photoperiod, SCP=2,min,day,1 and fast changing photoperiod, FCP=8,min,day,1) on the primary post-nuptial moult of captive rock sparrows Petronia petronia. Females started to moult on average 14 and 15,days later than males in both experimental groups. Primary moult duration was 10 (FCP) and 24 (SCP) days longer in males than in females, and, within sex, 34 (females) and 48 (males) days longer in SCP birds than in FCP ones. Females renewed a larger number of primaries simultaneously (5.7% in FCP and 12.8% in SCP) and had a higher total daily feather mass grown (9.9% in FCP and 22.4% in SCP), even though daily growth rates of individual primaries did not differ between sexes. As a result, males and females completed their primary moult at the same time within treatment. The observed differences in timing, duration and energy allocation for primary moult between the sexes probably have a genetic basis, as birds did not engage in reproduction during the preceding breeding season. Family-focused treatment for bipolar disorder in adults and youth

Levels of familial expressed emotion during an acute episode are consistently associated with rates of recurrence among bipolar patients. This article briefly reviews the evidence for expressed emotion (EE) as a prognostic indicator and then illustrates family-focused treatment (FFT) with adults and adolescents suffering from bipolar disorder. FFT is a time-limited, modularized treatment consisting of psychoeducation, communication enhancement training, and problem-solving skills. Controlled trials indicate that FFT is an efficacious adjunct to pharmacotherapy for patients with bipolar disorder. Conditioning of stress in Nile tilapia

A Pavlovian conditioning paradigm was used to induce a connection between a conditioned stimulus, light (CS), associated with an unconditioned stimulus, confinement (US) in Nile tilapia Oreochromis niloticus, which resulted in a conditioned endocrine response (CR) to the CS alone manifested as an increase in plasma cortisol. Individual isolated Nile tilapia were submitted for 10 days to the conditioning treatment consisting of turning on a light (CS) for 1 min with subsequent 30 min confinement (US). On the 10th day of the experiment, plasma cortisol was not increased when fish were subjected to no handling at all, or only light, or even a daily stressor for the 9 days. On the other hand, at the 10th day cortisol was significantly increased only when light was presented either with or without pairing with the stressor. These results confirmed that the cue, light (CS), was not stressful in itself, but when given as the CS in the absence of the US post conditioning the hypothalamus,pituitary,interrenal axis was activated. Therefore, it was concluded that memory of a previous experience with a stressor can be recalled by a conditioned stimulus and induce stress, which is the first demonstration of a memory-induced stress in fishes. Effect of different methods for the induction of spermiation on semen quality in European eel

Abstract Five hormonal treatments with human chorionic gonadotropin (hCG) were tested for the induction of maturation and spermiation in male farmed eels. The main aim was to optimize previously used hormonal treatments to achieve shorter induction treatments, longer spermiation periods and/or higher sperm quality. Fish treated for just 3 weeks (treatment E) or until the onset of spermiation (treatment C) showed the worst results, while the treatment consisting of weekly administration of 1.5 IU hCG g,1 fish (treatment A) induced the highest percentage of spermiating males, the highest number of sperm samples and sperm volumes and densities similar to the rest of the treatments (B: half hormone dosage, or D: biweekly administration). Evaluation of the sperm quality was performed by computer-assisted sperm analysis (CASA), considering the percentage of total motile spermatozoa, the percentage of fast and medium-velocity spermatozoa, as well as different motility parameters. Sperm samples from A-D groups showed between 44% and 54% motile spermatozoa, and between 10% and 15% fast spermatozoa, while samples from E-treated males showed 0% motile cells. No significant differences were found in the spermatozoa straight line velocity (VSL), curvilinear velocity (VCL) or the angular velocity (VAP), neither spermatozoa beating cross frequency (BCF) between A,D groups. Oral cavity squamous cell carcinomas in young patients in a selected Malaysian centre

Abstract Aim: This study aims to evaluate the demographics, to analyze the clinical and pathological findings, treatment and the outcome of oral cavity squamous cell carcinomas (OSCC) in patients aged 40-years old and below in our centre. Methods: Records of patients who were diagnosed with OSCC in the Universiti Kebangsaan Malaysia Medical Centre, Kuala Lumpur, from 1998 to 2003 were analyzed. Their socioeconomic data such as their age, gender and race, risk factors, family history and genetic predisposition were assessed. Other data examined included the sites and stage of the tumor, histopathology results, treatment modality and outcome of OSCC. Results: There were 19 patients with no gender predilection. Malays made up most patients diagnosed with OSCC, followed by Chinese and Indians. There was no significant high-risk habit in this group. Most of the sites involved were the tongue (73.7%), buccal mucosa (15.8%) and alveolus (10.5%). Most of the patients (57.9%) were diagnosed with stage IV disease. Eighteen patients underwent treatment consisting of surgery or radiotherapy or in combination. A 5-year follow up revealed that nine patients (47.3%) survived and remain well, four patients (21.1%) had a recurrence, two patients (10.5%) died of the disease. Conclusion: The majority of patients presented with stage IV disease without any significant high-risk habit. Surgery remains the mainstay of treatment, however, 5-year follow-up showed a less than 50% survival rate. [source]


Functional analysis and treatment of bizarre vocalizations exhibited by an adult with schizophrenia: a replication and extension

A functional analysis suggested that the bizarre vocalizations exhibited by a woman with a diagnosis of schizophrenia were maintained by attention. A treatment consisting of awareness training, competing response training, differential reinforcement of the competing response, and attention extinction was effective in reducing bizarre vocalizations. Psychotherapy as monotherapy for the treatment of bipolar II depression: a proof of concept study

Objectives:, We conducted a proof of concept study to determine the feasibility of using an individual psychotherapy, Interpersonal and Social Rhythm Therapy (IPSRT), as monotherapy for the acute treatment of bipolar II depression. Methods:, Unmedicated individuals (n = 17) meeting DSM-IV criteria for bipolar II disorder and currently depressed received weekly psychotherapy (IPSRT) for 12 weeks. After 12 weeks of acute treatment, individuals received an additional 8 weeks of follow-up treatment consisting of continued weekly IPSRT with supplementary lamotrogine for IPSRT non-responders. Results:, By week 12, 41% (n = 7) of the sample responded to IPSRT monotherapy (defined as ,50% reduction in depression scores without an increase in mania scores), 41% (n = 7) dropped out of or were removed from the study, and 18% (n = 3) did not respond to treatment. By week 20, 53% (n = 9) had achieved a response and 29% (n = 5) achieved a full remission of symptoms. Conclusions:, Interpersonal and Social Rhythm Therapy appears to be a promising intervention for a subset of individuals with bipolar II depression. A randomized controlled trial is needed to systematically evaluate the efficacy of IPSRT as an acute monotherapy for bipolar II depression. [source]


Topoisomerase 2, plays a pivotal role in the tumor biology of stage IV thymic neoplasia

Abstract BACKGROUND. Microsatellite studies in histologic types B3 and C thymic neoplasia detected gains on chromosome 17q, which contains the Her-2/neu and its juxtaposed topoisomerase 2, (T2,) genes. The study aimed to evaluate their impact on tumor biology and survival of advanced thymic neoplasia patients. METHODS. From 1991 to 2005, 36 consecutive stage IV thymic carcinoma patients were treated, 18 men and 18 women, aged 11 to 84 years. There were 22 thymic carcinoma, 13 type B3, and 1 type B2 thymoma. Patients received treatment consisting of surgical resection, combination chemotherapy with the CAP (cyclophosphamide, Adriamycin, cisplatin) regimen, or radiation therapy potentiated by high-dose weekly 5-fluorouracil infusion. Permutations of these 3 treatment modalities were prescribed as necessary. RESULTS. T2, gene amplification was detected in 4 of 14 thymic carcinoma and 1 of 15 type B3 thymoma. Three thymic carcinoma patients had Her-2/neu coamplification and these 3 patients had rapidly growing tumor and extensive disease at initial diagnosis. CAP was prescribed in 28 patients and 20 patients responded (response rate, 71.4%, 95% confidence interval [CI]: 52.8% to 85%); all responders overexpressed (,10% nuclei positive) the T2, protein, whereas 4 nonresponders had very low expression. T2, overexpression predicts CAP response, and its absence predicts resistance (P = .001). Overall survival was significantly prolonged if the tumor was resectable (P = .001), of type B3 histology (P = .0039), and had no Her-2 gene amplification (P = .0081). CONCLUSION. T2, and Her-2/neu genes play a pivotal role in the tumor biology, CAP response, and survival of advanced thymic neoplasia patients. Cancer 2007;109:502,509. © 2006 American Cancer Society. [source]
