The principal components model: a model for advancing spirituality and spiritual care within nursing and health care practice

Aim., The aim of this study was to generate a deeper understanding of the factors and forces that may inhibit or advance the concepts of spirituality and spiritual care within both nursing and health care. Background., This manuscript presents a model that emerged from a qualitative study using grounded theory. Implementation and use of this model may assist all health care practitioners and organizations to advance the concepts of spirituality and spiritual care within their own sphere of practice. The model has been termed the principal components model because participants identified six components as being crucial to the advancement of spiritual health care. Design., Grounded theory was used meaning that there was concurrent data collection and analysis. Theoretical sampling was used to develop the emerging theory. These processes, along with data analysis, open, axial and theoretical coding led to the identification of a core category and the construction of the principal components model. Methods., Fifty-three participants (24 men and 29 women) were recruited and all consented to be interviewed. The sample included nurses (n = 24), chaplains (n = 7), a social worker (n = 1), an occupational therapist (n = 1), physiotherapists (n = 2), patients (n = 14) and the public (n = 4). The investigation was conducted in three phases to substantiate the emerging theory and the development of the model. Results., The principal components model contained six components: individuality, inclusivity, integrated, inter/intra-disciplinary, innate and institution. Conclusion., A great deal has been written on the concepts of spirituality and spiritual care. However, rhetoric alone will not remove some of the intrinsic and extrinsic barriers that are inhibiting the advancement of the spiritual dimension in terms of theory and practice. Relevance to clinical practice., An awareness of and adherence to the principal components model may assist nurses and health care professionals to engage with and overcome some of the structural, organizational, political and social variables that are impacting upon spiritual care. Toward a Theory of Self-Reconciliation Following Mistakes in Nursing Practice

Purpose: To explore nurses' responses to making mistakes in hospital-based practice in the US. Methods: A grounded theory approach was used to explore the process that occurs after nurses perceive that they have made mistakes in practice. Theoretical sampling was used and data were collected until saturation occurred. Ten participants, who were registered nurses, described 17 personal mistakes. The mistakes they described occurred in hospitals. All participants were practicing nursing either in hospitals or in other work settings. Findings: A process of "Self-Reconciliation After Making Mistakes in Hospital Practice" was identified, with four distinct categories: reality hitting, weighing in, acting, and reconciling. The core category was reconciliation of the self, personally and professionally. Conclusions: This research was a first step toward the development of a theory of mistake making in nursing practice. This response to making mistakes is consistent with previous research and is related to cognitive dissonance theory. The responses to mistakes varied from less healthy responses of blaming and silence to healthier responses that included disclosure, apologizing, and making amends. Further research to develop the theory and to determine helpful interventions is suggested. [source]


Using a synthesised technique for grounded theory in nursing research

Aims., To introduce a synthesised technique for using grounded theory in nursing research. Background., Nursing increasingly uses grounded theory for a broadened perspective on nursing practice and research. Nurse researchers have choices in how to choose and use grounded theory as a research method. These choices come from a deep understanding of the different versions of grounded theory, including Glaser's classic grounded theory and Strauss and Corbin's later approach. Design., Grounded theory related literature review was conducted. Methods., This is a methodological review paper. Results., Nursing researchers intent on using a grounded theory methodology should pay attention to the theoretical discussions including theoretical sampling, theoretical sensitivity, constant comparative methods and asking questions, keeping memoranda diagramming, identification of a core category and a resultant explanatory theory. A synthesised approach is developed for use, based on Strauss and Corbin's style of sampling and memoranda writing, but selecting theoretical coding families, that differ from the paradigm model of Strauss and Corbin, from the wide range suggested by Glaser. This led to the development of a multi-step synthesised approach to grounded theory data analysis based on the works of Glaser, Charmaz and Strauss and Corbin. Conclusions., The use of this synthesised approach provides a true reflection of Glaser's idea of ,emergence of theory from the data' and Strauss and Corbin's style of sampling and memoranda writing is employed. This multi-step synthesised method of data analysis maintains the philosophical perspective of grounded theory. Relevance to clinical practice., This method indicates how grounded theory has developed, where it might go next in nursing research and how it may continue to evolve. [source]


The role of community mental health nurses caring for people with schizophrenia in Taiwan: a substantive grounded theory

Aim and objectives., The aim was to develop a practice theory that can be used to guide the direction of community nursing practice to help clients with schizophrenia and those who care for them. Design., Substantive grounded theory was developed through use of grounded theory method of Strauss and Corbin. Methods., Two groups of participants in Taiwan were selected using theoretical sampling: one group consisted of community mental health nurses and the other group was clients with schizophrenia and those who cared for them. The number of participants in each group was determined by theoretical saturation. Semi-structured one-to-one in-depth interviews and unstructured non-participant observation were utilized for data collection. Data analysis involved three stages: open, axial and selective coding. During the process of coding and analysis, both inductive and deductive thinking were utilized and the constant comparative analysis process continued until data saturation occurred. To establish trustworthiness, the four criteria of credibility, transferability, dependability and confirmability were followed along with field trial, audit trial, member check and peer debriefing for reliability and validity. Results., A substantive grounded theory, the role of community mental health nurses caring for people with schizophrenia in Taiwan, was developed through utilization of grounded theory method of Strauss and Corbin. Conclusion., In this paper, results and discussion focus on causal conditions, context, intervening conditions, consequences and phenomenon. Relevance to clinical practice., The theory is the first to contribute knowledge about the field of mental health home visiting services in Taiwan to provide guidance for the delivery of quality care to assist people in the community with schizophrenia and their carers. Research Article: Participation of patients with chronic illness in nursing care: An Iranian perspective

Abstract The increasing number of chronically ill people has served as an impetus for the promotion of patient participation in nursing care. However, little is known about patient participation in Iran. The aim of this study was to identify the factors that are relevant to patient participation and the nature of that participation, as experienced by chronically ill patients and registered nurses in Iran. Grounded theory was used as the method. Twenty-two participants were recruited by using purposeful and theoretical sampling. The data were generated by semistructured interviews and participant observations. Constant comparison was used for the data analysis. This study indicated that participation is an interactive process between nurses, patients, and family members in the caregiving context. Participation occurred when the caring agents worked together. The core category of "convergence of the caring agents" emerged. The subcategories emerged as the levels of participation and included "adhering", "involving", "sharing", and "true participation". The factors related to the caring agents and caregiving context could be considered as predictors of the level of participation when caring for these patients. [source]
