    ONE FIGHT, ONE TEAM: THE 9/11 COMMISSION REPORT ON INTELLIGENCE, FRAGMENTATION AND INFORMATION

    In its report published in 2004, the National Commission on Terrorist Attacks Upon the United States (known as the ,9/11 Commission') analyses the functioning of the Intelligence Community (ICo). It indicates that the ICo is both over-fragmented and guilty of not sharing enough information. The Commission recommends that central control of the ICo needs to be strengthened and that more incentives for information-sharing should be designed. This article takes a critical look at these two recommendations. Sharing information carries major risks and is therefore not something that can take place as a matter of course. Moreover, information has to be subject to a selection process before it can be shared. This selection cannot be measured objectively, so mistakes in the selection are unavoidable. Strengthening central control also poses risks: it engenders more battles over territory, it does not improve understanding of the capillaries of the organization , the capillaries being where the primary processes of information gathering, validation and assessment take place , and it involves the destruction of checks and balances. Fragmentation may even be functional since it leads to redundancy, itself a safeguard against the risk of misselecting relevant information. [source]

    Psychotic phenomena in 257 young children and adolescents with bipolar I disorder: delusions and hallucinations (benign and pathological)

    Objectives:, In contrast to studies of adult bipolar I disorder (BP-I), there is a paucity of data on psychotic phenomena in child BP-I. Therefore, the aim of this work was to describe delusions and hallucinations in pediatric BP-I. Methods:, Subjects were 257 participants, aged 6,16, in either of two large, ongoing, NIMH-funded studies, ,Phenomenology and Course of Pediatric Bipolar Disorders' or ,Treatment of Early Age Mania (TEAM)'. All subjects had current DSM-IV BP-I (manic or mixed phase) with a Children's Global Assessment Scale score ,60 (definite clinical impairment), and all had cardinal mania symptoms (i.e., elation and/or grandiosity). Comprehensive assessments included the Washington University in St. Louis Kiddie Schedule for Affective Disorders and Schizophrenia (WASH-U-KSADS), which was administered to parents about their children and separately to children about themselves by experienced research clinicians. The WASH-U-KSADS contains modules for developmentally child-age-specific manifestations of numerous categories of psychotic phenomena. Results:, Psychosis was present in 76.3% (n = 196) of subjects, which included 38.9% (n = 100) with delusions, 5.1% (n = 13) with pathological hallucinations, and 32.3% (n = 83) with both. The most common delusion was grandiose (67.7%, n = 174), and the most common pathological hallucination was visual (16.0%, n = 41). Benign hallucinations occurred in 43.6% (n = 112). A median split by age yielded 6,9 year-olds (n = 139) and 10,16 year-olds (n = 118). Analyses of these two groups, and of 6, 7, 8, and 9 year-olds separately, found no significant differences in psychotic phenomena. Conclusions:, Counterintuitively, psychosis was equally prevalent in 6,9 compared to 10,16 year-olds. High prevalence of psychosis in child BP-I warrants focus in intervention strategies and is consistent with increasing evidence of the severity of child-versus adult-onset BP-I. [source]


    OUT-OF-COURT STATEMENTS BY VICTIMS OF CHILD SEXUAL ABUSE TO MULTIDISCIPLINARY TEAMS: A CONFRONTATION CLAUSE ANALYSIS

    Acknowledging the rapid growth of child sexual abuse in the United States, this Note advocates for the recognition of a limited exception to the blanket-hearsay ban on out-of-court statements made by unavailable declarants set out by the Supreme Court in Crawford v. Washington. In order to protect a criminal defendant's Sixth Amendment confrontation right, Crawford requires that hearsay evidence that is "testimonial" in nature be deemed inadmissible if the witness is unavailable and the defendant does not have a prior opportunity to cross-examine the witness against him. However, Crawford noted that, where nontestimonial hearsay is at issue, cross-examination may not be necessary. Accordingly, where a child sexual abuse victim makes statements during a structured or semi-structured forensic interview to a member of a multidisciplinary team, these statements should be deemed nontestimonial and thus admitted into evidence, without requiring cross-examination of the child. Allowing for this exception to the general hearsay ban in Crawford is not only consistent with current precedent, but it is also warranted to promote public policy and to curb the negative impact such abuse has on society.     ABUNDANCE OF IRRAWADDY DOLPHINS (ORCAELLA BREVIROSTRIS) AND GANGES RIVER DOLPHINS (PLATANISTA GANGETICA GANGETICA) ESTIMATED USING CONCURRENT COUNTS MADE BY INDEPENDENT TEAMS IN WATERWAYS OF THE SUNDARBANS MANGROVE FOREST IN BANGLADESH

    Abstract Independent observer teams made concurrent counts of Irrawaddy dolphins Orcaella brevirostris and Ganges River dolphins Platanista gangetica gangetica in mangrove channels of the Sundarbans Delta in Bangladesh. These counts were corrected for missed groups using mark-recapture models. For Irrawaddy dolphins, a stratified Lincoln-Petersen model, which incorporated group size and sighting conditions as covariates, and a Huggins conditional likelihood model, which averaged models that individually incorporated group size, sighting conditions, and channel width as covariates, generated abundance estimates of 397 individuals (CV = 10.2%) and 451 individuals (CV = 9.6%), respectively. For Ganges River dolphins, a stratified Lincoln-Petersen model, which incorporated group size as a covariate, and a Huggins conditional likelihood model, which averaged the same models described above, generated abundance estimates of 196 individuals (CV = 12.7%) and 225 individuals (CV = 12.6%), respectively. Although the estimates for both models were relatively close, the analytical advantages of the Huggins models probably outweigh those of the Lincoln-Petersen models. However, the latter should be considered appropriate when simplicity is a priority. This study found that waterways of the Sundarbans support significant numbers of Irrawaddy and Ganges River dolphins, especially compared to other areas where the species have been surveyed.     Abstract Accelerator mass spectrometry (AMS) has developed in the last 30 years many notable applications to the spectrometry of radioisotopes, particularly in radiocarbon dating. The instrumentation science of trace element AMS (TEAMS) that analyzes stable isotopes, also called Accelerator SIMS or MegaSIMS, while unique in many features, has also shared in many of these significant advances and has pushed TEAMS sensitivity to concentration levels surpassing many competing mass spectroscopic technologies.     AN EVALUATION OF GENERIC TEAMWORK SKILLS TRAINING WITH ACTION TEAMS: EFFECTS ON COGNITIVE AND SKILL-BASED OUTCOMES

    This study evaluated the utility of generic teamwork skills training for enhancing the effectiveness of action teams. Results from 65 4-person action teams working on an interdependent command and control simulator revealed that generic teamwork skills training had a significant and positive impact on both cognitive and skill-based outcomes. Trained team members evidenced higher levels of declarative knowledge regarding teamwork competencies and demonstrated greater proficiency in the areas of planning and task coordination, collaborative problem-solving, and communication. Furthermore, results indicated that cognitive and skill based outcomes were interrelated. Team members' declarative knowledge regarding teamwork competencies positively affected planning and task coordination, collaborative problem solving, and communication skills. However, we found that the effects of declarative knowledge differed across team members depending on their roles and responsibilities. The team benefited the most from the knowledge held by the team member who occupied the most critical position in the workflow. Implications of these findings for future research and practice are discussed. [source]


    JUSTICE IN TEAMS: ANTECEDENTS AND CONSEQUENCES OF PROCEDURAL JUSTICE CLIMATE

    This study examined antecedents and consequences of procedural justice climate (Mossholder, Bennett, & Martin, 1998; Naumann & Bennett, 2000) in a sample of manufacturing teams. The results showed that climate level (i.e., the average procedural justice perception within the team) was significantly related to both team performance and team absenteeism. Moreover, the effects of climate level were moderated by climate strength, such that the relationships were more beneficial in stronger climates. In addition, team size and team collectivism were significant antecedents of climate level, and team size and team demographic diversity predicted climate strength. [source]


    PATTERNS OF INTERDEPENDENCE IN WORK TEAMS: A TWO-LEVEL INVESTIGATION OF THE RELATIONS WITH JOB AND TEAM SATISFACTION

    A questionnaire study in 17 school and 24 engineering teams examined affective reactions to task and goal interdependence at the group and individual level of analysis. Group-level task interdependence was positively related to group members' job and team satisfaction. Within-group differences in the degree of task interdependence were unrelated to affective responses. Interactions revealed that within-group task interdependence is positively related to both job and team satisfaction only if the degree of goal interdependence in the work team is high. [source]


    GS14P ROUTINE USE OF MEDICAL EMERGENCY TEAMS IN MANAGING SURGICAL EMERGENCIES

    Introduction Trauma teams and cardiac arrest teams provide an urgent and expert multi-disciplinary response to time critical emergencies. The present study documents the contribution of a medical emergency team (MET) to managing non-trauma surgical emergencies. Materials and Methods Data was prospectively collected over a two year period concerning the contribution of medical emergency teams to the resuscitation of all patients with non-trauma surgical emergencies and altered vital signs in hospital wards. Results Over the study period, the details of 19 patients with surgical emergencies were recorded. 63% of emergencies occurred outside of normal working hours. In 53% of cases, the surgical registrar was off-site or physically unavailable to attend the emergency immediately. In 11% of cases, the medical emergency team was activated prior to the arrival of the surgical registrar. In 26% of cases, the patient was left unattended whilst awaiting arrival of the surgical registrar. The medical emergency team provided resuscitation procedures and arranged urgent investigations in all patients, physically transported the patient to the operating theatre in 16% of patients and prepared for general anaesthetic in the operating theatre in 11% of cases. The surgical registrar complemented the medical emergency team response by liaising with consultant surgeons, anaesthetists and operating theatre staff in all cases. All patients received definitive treatment within 30 minutes of MET response. Conclusion Routine use of medical emergency teams in the initial resuscitation of patients with surgical emergencies expedites definitive management. [source]


    Environmental management in large-scale building projects,learning from Hammarby Sjöstad

    In an old industrial and harbour area of Stockholm, a new city for 30 000 people will be built in the next ten years. The Hammarby Sjöstad project is unique in its size and municipal organization as well as in its ambitious environmental objectives. In a case study based on interviews and document analysis, the environmental management process of this project is researched. The City of Stockholm will follow up the compliance with set goals; our study is a qualitative one focusing on the management process. We develop concepts and models as an aid for municipal management of future construction projects. Many factors outside the formal ones are considered important. Data is structured chronologically as events and from a stakeholder perspective, including the City, the developers and the contractors. The main focus is, however, on the City's Project Management Team. Collected data is furthermore analysed with the aid of key concepts, derived from organization theory, planning and construction practice and as suggested by the data. Preliminary results indicate that the continued study of informal means of control is just as important as that of the formal ones, and that identifying key situations and tools for environmental management should be in focus for the rest of the research study.     Working in partnership , developing a young people's service in a rural environment

    Specific objective, The Project reviewed , The Provision of Services for Early Psychosis in Cornwall. It looked at staff feedback, user and carers' views, base line assessment and National Guidelines of Best Practice, and proposed significant changes to meet the needs of young people. Method, The Review was carried out under the auspices of the National Clinical Governance Team and used as its framework the RAID model of change. Results, This resulted in a series of recommendations, which informed new developments. Current initiatives include a Health Promotion Programme, Training Strategies in Primary Care and effective partnerships with young people's services. Conclusions, It proposes significant changes to the correct system of care by working outside the traditional healthcare settings and across the service boundaries of health, education and social care. The rurality of Cornwall provides a considerable challenge for service delivery. This review ensured that ownership rested with users, careers, staff and others who contributed and shaped its future direction. [source]


    Assessing prolonged recovery in first-episode psychosis

    The Early Psychosis Prevention and Intervention Centre (EPPIC) is a comprehensive, specialized treatment service for individuals residing in the western metropolitan region of Melbourne who are experiencing their first psychotic episode. A subprogramme of EPPIC, the Treatment Resistance Early Assessment Team (TREAT), has been developing a framework for the management of individuals experiencing ,prolonged recovery' in early psychosis. TREAT is a consultation team that provides technical assistance to clinicians within EPPIC, comprising senior clinicians with expertise in the biopsychosocial treatment of early psychosis and persisting positive and negative symptoms. A system has recently been set up within the TREAT framework to routinely assess clinical and functional outcomes of these clients using standardized instruments. Case managers are trained to conduct assessments at multiple timepoints over the duration of their clients' treatment at EPPIC. A summary will be presented on the current sample (n=15) and examples of clinician and client feedback reports will be illustrated. Discussion is also provided on the development of training, procedures and materials to enhance integration of clinician and client outcome measures into routine clinical practice. [source]

    Foreign Sales and Small Firm Growth: The Moderating Role of the Management Team

    The premise of this article is that the management team of a small firm plays a key role in internationalization outcomes. Specifically, it is hypothesized that a greater degree of behavioral integration within a small firm's management team enables it to manage the complexity of foreign sales growth more effectively, leading to greater overall firm growth. Findings, based on data collected from two different industries (software products, food processing), support the hypothesis and indicate that the behavioral integration of the management team moderates the relationship between foreign sales growth and overall firm growth.     An Approach to Interdisciplinary Training in Postgraduate Education

    Aims, A primary goal for clinical graduate training is to provide the student with the expertise required for specialist treatment in the actual discipline. At the same time there is an increasing need for a broader perspective on specialist care and the students should be aware of the limitations inherent in own specialty. In order to plan treatment in the best interest of the patient, and to be prepared to take part in treatments involving other specialties, the students should be exposed to interdisciplinary cooperation throughout the training. An approach to joint academic and clinical training with the purpose of providing graduate students with a broader perspective on specialist care is described and discussed. Material and methods, During their first year graduate students in the 7 different disciplines complete a joint Core Curriculum consisting of 8 different courses to stimulate a scientific approach to their profession and understanding of basic biologic mechanisms. To create a learning environment focusing on the development of interdisciplinary competence, a joint clinic has been established. Teams of students from different disciplines have been organized in order to establish ,partnership' for the treatment of patients with complex problems. The students also take part in the sessions held by a faculty Team of experts for assessment and treatment planning of referred patients with complex problems. Furthermore, faculty members conduct courses and seminars for students from other disciplines and students also participate in selected parts of the regular program in other disciplines. Results, Formal evaluation has so far been conducted for the Core Curriculum. Most students respond that they are satisfied with the courses, and the curriculum has also been adjusted based on the comments. The students report that treatment of patients in need of interdisciplinary treatment has been facilitated by having ,partners' in other disciplines. Participation in the Team of expert's sessions has been appreciated, and the attendance at interdisciplinary courses and seminars has been good. Conclusions, Based on the experience over the last 5 years, the interdisciplinary aspects of graduate training should be expanded to stimulate a holistic approach also to specialist treatment.     Once More With Feeling: Ethnographic Reflections on the Mediation of Tension in a Small Team of Call Centre Workers

    This article explores the labour process of a team of call-centre workers based in a multi-client call centre in the West Midlands. Founded on the basis of a 13-month ethnographic study into workplace resistance in call-centre environments, this article provides insights into control in call centres, focusing on sexuality, internal team dynamics and discipline. It is argued that control is exerted through management and information technology but it is crucially exerted laterally in the team and sexuality is an important medium of such control. This article focuses specifically on how worker sexuality is deployed to regulate the tension between contradictory imperatives faced by workers. The article then considers the emotional content of the call-centre labour process, arguing that the apparent resolution of potentially contradictory logic, in fact, depends upon the development by call-centre workers, encouraged by more senior employees, of informal, pseudo-sexualized client relations at the point of production. Crucially however the fieldwork reveals that the demands placed upon customer service representatives are subtly gendered. [source]

Despite a multipronged internal communications program, enterprise-level efforts did not sustain the employee enthusiasm that accompanied the July 2008 launch of the Exelon 2020 low-carbon roadmap, and the company went back to the drawing board. The Exelon 2020 Engagement Team,this time rechartered around action rather than conversation about employee environmental initiatives,has driven a second round of efforts.     Organisation and features of hospital, intermediate care and social services in English sites with low rates of delayed discharge

    Abstract In recent years, there has been significant concern, and policy activity, in relation to the problem of delayed discharges from hospital. Key elements of policy to tackle delays include new investment, the establishment of the Health and Social Care Change Agent Team, and the implementation of the Community Care (Delayed Discharge) Act 2003. Whilst the problem of delays has been widespread, some authorities have managed to tackle delays successfully. The aim of the qualitative study reported here was to investigate discharge practice and the organisation of services at sites with consistently low rates of delay, in order to identify factors supporting such good performance. Six ,high performing' English sites (each including a hospital trust, a local authority, and a primary care trust) were identified using a statistical model, and 42 interviews were undertaken with health and social services staff involved in discharge arrangements. Additionally, the authors set out to investigate the experiences of patients in the sites to examine whether there was a cost to patient care and outcomes of discharge arrangements in these sites, but unfortunately, it was not possible to secure sufficient patient participation. Whilst acknowledging the lack of patient experience and outcome data, a range of service elements was identified at the sites that contribute to the avoidance of delays, either through supporting efficiency within individual agencies or enabling more efficient joint working. Sites still struggling with delays should benefit from knowledge of this range. The government's reimbursement scheme appears to have been largely helpful in the study sites, prompting efficiency-driven changes to the organisation of services and discharge systems, but further focused research is required to provide clear evidence of its impact nationally, and in particular, how it impacts on staff, and patients and their families.     The influences of data precision on the calculation of temperature percentile indices

    Abstract Percentile-based temperature indices are part of the suite of indices developed by the WMO CCl/CLIVAR/JCOMM Expert Team on Climate Change Detection and Indices. They have been used to analyse changes in temperature extremes for various parts of the world. We identify a bias in percentile-based indices which consist of annual counts of threshold exceedance. This bias occurs when there is insufficient precision in temperature data, and affects the estimation of the means and trends of percentile-based indices. Such imprecision occurs when temperature observations are truncated or rounded prior to being recorded and archived. The impacts on the indices depend upon the type of relation (i.e. temperature greater than or greater than or equal to) used to determine the exceedance rate. This problem can be solved when the loss of precision is not overly severe by adding a small random number to artificially restore data precision. While these adjustments do not improve the accuracy of individual observations, the exceedance rates that are computed from data adjusted in this way have properties, such as long-term mean and trend, which are similar to those directly estimated from data that are originally of the same precision as the adjusted data.     Abstract Homelessness and substance misuse have risen dramatically over the past 30 years in the UK. The role of the primary care nurse has been signalled as important in working with people who have drug and alcohol problems, and for improving the general health of homeless people. This article focuses on the role of the primary care nurse in a Supervised Methadone and Resettlement Team (SMART). The team works in central Bristol, in southwest England, with people who are homeless and using illegal opiates. The aim of this report is to provide descriptive information that demonstrates the value of the primary care nurse, working in a multiagency partnership, in dealing with the problems of this homeless population, many of whom have problems associated with illicit drug use. Client outcomes from a small sample of homeless persons are also described. [source]


    Incidence of In-Hospital Falls in Geriatric Patients Before and After the Introduction of an Interdisciplinary Team,Based Fall-Prevention Intervention

    Falls are among the most common unwanted events in older hospital inpatients, but evidence of effective prevention is still limited compared with that in the community and in long-term care facilities. This article describes a prevention program and its effects on the incidence of falls in geriatric hospital wards. It was a prospective cohort study with historical control including all 4,272 patients (mean age 80, 69% female) before and 2,982 (mean age 81, 69% female) after introduction of the intervention. The intervention included fall-risk assessment on admission and reassessment after a fall; risk alert; additional supervision and assistance with the patients' transfer and use of the toilet; provision of an information leaflet; individual patient and caregiver counseling; encouragement of appropriate use of eyeglasses, hearing aids, footwear, and mobility devices; and staff education. Measurements included standardized fall-incidence reporting, activity of daily living and mobility status, number of falls and injurious falls, and number of patients who fell. Before the intervention was introduced, 893 falls were recorded. After the intervention was implemented, only 468 falls were recorded (incidence rate ratio (IRR)=0.82, 95% confidence interval (CI)=0.73,0.92), 240 versus 129 total injurious falls (IRR=0.84, 95% CI=0.67,1.04), 10 versus nine falls with fracture (IRR=1.40, 95% CI=0.51,3.85) and 611 versus 330 fallers. The relative risk of falling was significantly reduced (0.77, 95% CI=0.68,0.88). A structured multifactorial intervention reduced the incidence of falls, but not injurious falls, in a hospital ward setting with existing geriatric multidisciplinary care. Improvement of functional competence and mobility may be relevant to fall prevention in older hospital inpatients. [source]

    A literature review: factors that impact on nurses' effective use of the Medical Emergency Team (MET)

    Aims and objectives., The aim of this literature review is to identify factors, both positive and negative, that impact on nurses' effective use of the Medical Emergency Team (MET) in acute care settings. Background., Outcomes for patients are often dependent on nurses' ability to identify and respond to signs of increasing illness and initiate medical intervention. In an attempt to improve patient outcomes, many acute hospitals have implemented a rapid response system known as the Medical Emergency Team (MET) which has improved management of critically ill ward patients. Subsequent research has indicated that the MET system continues to be underused by nurses. Design., A comprehensive thematic literature review. Methods., The review was undertaken using key words and the electronic databases of Cumulative Index to Nursing and Allied Health Literature (CINAHL), OVID/MEDLINE, Blackwell Synergy, Science Direct and Informit. Fifteen primary research reports were relevant and included in the review. Results., Five major themes emerged from the analysis of the literature as the major factors effecting nurses' use of the MET system. They were: education on the MET, expertise, support by medical and nursing staff, nurses' familiarity with and advocacy for the patient and nurses' workload. Conclusions., Ongoing education on all aspects of the MET system is recommended for nursing, medical and MET staff. Bringing MET education into undergraduate programs to prepare new graduates entering the workforce to care for acutely ill patients is also strongly recommended. Further research is also needed to determine other influences on MET activation. Relevance to clinical practice., Strategies that will assist nurses to use the MET system more effectively include recruitment and retention of adequate numbers of permanent skilled staff thereby increasing familiarity with and advocacy for the patient. Junior doctors and nurses should be encouraged to attend ward MET calls to gain skills in management of acutely ill patients. [source]


    Incident Command Skills in the Management of an Oil Industry Drilling Incident: a Case Study

    The successful management of a complex, hazardous event in many domains demands a high level of incident command skills. In the oil and gas exploration and production industry, these skills were required by members of an Incident Management Team (IMT) established to respond the failure of a drilling riser in the Gulf of Mexico. When an incident occurs, members of an industrial IMT form an interdisciplinary, interdependent, but ad-hoc team. As actual experience of dealing with major incidents of this nature is relatively rare, IMT members have to rely on emergency exercises in training, along with existing domain-specific knowledge. Following a serious incident on an offshore drilling rig, semi-structured interviews with the on-shore strategic and tactical level IMT members (n=7) were conducted. These interviews have resulted in the identification and definition of incident command skills for members of an industrial IMT, namely decision making, situation awareness, communication, leadership, and teamwork, all of which can be affected by stress, as well as organisational factors that influenced the outcome of the incident. Limitations in current incident management training were identified, namely the need for specific incident command skills training. A framework is suggested around which specific incident command skills training can be structured. Key learnings from this case study are also presented which can provide guidance for the training and preparation of industrial incident management teams. [source]
