Cost-effectiveness analysis of two strategies for mass screening for colorectal cancer in France

Abstract The implementation of colorectal cancer mass screening is a high public health priority in France, as in most other industrialised countries. Despite evidences that screening using guaiac fecal occult blood test may reduce colorectal cancer mortality, no European country has organised widespread mass screening with this test. The low sensitivity of this test constitutes its main limitation. Immunological tests, which provide higher sensitivity than the guaiac test, may constitute a satisfactory alternative. This study was carried out to compare the costs and the effectiveness of 20 years of biennial colorectal cancer (CRC) screening with an automated reading immunological test (Magstream) with those obtained with a guaiac stool test (Haemoccult). The model used to estimate the costs and effectiveness of successive biennial CRC screening campaigns was a transitional probabilistic model. The parameters used in this model concerning costs and CRC epidemiological data were calculated from results obtained in the screening program run in Calvados or from published results of foreign studies because of the lack of French studies. The use of Magstream for 20 years of biennial screening costs 59 euros more than Haemoccult per target individual, and should lead to a mean increase in individual life expectancy of 0.0198 years (i.e. about one week), which corresponds to an incremental cost-effectiveness ratio of 2980 euros per years of life saved. Our results suggest that using an immunological test could increase the effectiveness of CRC screening at a reasonable cost for society. Copyright © 2003 John Wiley & Sons, Ltd. [source]


Shedding the Pounds but not the Stigma: Negative Attributions as a Function of a Target's Method of Weight Loss

Overweight individuals are perceived as possessing several negative attributes, which often leads them to attempt to lose weight. The current research examined if method of weight loss influences participants' attributions and perceptions of a formerly overweight target individual. Participants were randomly assigned to one of three conditions: weight loss via diet/exercise, weight loss via surgery, or control (no description of weight loss). Results indicated that, in general, the surgery targets were perceived more negatively than the diet/exercise targets. Moreover, diet/exercise targets were perceived as being less healthy, and, for female participants, more responsible for their weight than control targets. These data suggest that individuals who lose weight are still prone to the negative attributions associated with the overweight. [source]


Visibility and the Stigmatization of Cancer: Context Matters,

Few experimental studies have been conducted to investigate the effect of disease characteristics, such as visibility, on the stigmatization of cancer. The current study tested the reactions of undergraduates to vignettes that manipulated the visibility of cancer and the context in which help was needed by an individual with cancer. Results showed that participants were less willing to help the target individual when the cancer was visible and the context allowed stigmatization to be subtle. Further research should consider the relationships among disease attributes, the specific context where help is needed, and the stigmatization of individuals with cancer. [source]


Knowledge and Attitudes Toward Infertility and Childless Couples

This study of 456 female and 205 male college students investigated knowledge and attitudes toward infertility and reactions to couples with varied fertility status. The gender and career status of the target individual also were varied. The most negative affect and stories were indicated for those described as childless by choice. The male target character also was rated more negatively than the female target character. The greatest responsibility and control were assigned to childless-by-choice and childless-no-explanation groups. The most positive affect and stories were indicated for those described as childless with no explanation given. Infertile couples were attributed less control and responsibility and elicited some degree of anger and hostility from others. [source]


Do weapons automatically capture attention?

Two experiments explored whether weapons automatically capture attention or whether eyewitnesses can overcome the weapon focus effect if so instructed. Witnesses heard a lecture that either instructed them to attend to the target individual and avoid fixating on the weapon or presented unrelated information. Subsequently, they observed the target carrying either a weapon or a book and attempted to remember his appearance. Control witnesses reported fewer correct and more incorrect details when he carried a weapon rather than the book. However, the reports of educated witnesses did not differ between object conditions. Additionally, witnesses' ability to avoid weapon focus was unaffected by weapon unusualness and elevated arousal levels, and control witnesses provided better descriptions of the weapon than the book. Prevention of Type 2 diabetes mellitus.

A review of the evidence, its application in a UK setting
Abstract Type 2 Diabetes mellitus (T2DM) is a complex metabolic, multifactorial disease, which affects the quality, quantity and style of life. People with T2DM have a life expectancy that can be shortened by as much as 15 years, with up to 75% dying of macrovascular complications. To reduce the impact of T2DM in the 21st century, we need an approach that not only optimally treats the person with established diabetes but also prevents diabetes from occurring in the first place. The best evidence for prevention of diabetes is for interventions that target individuals at highest risk. Targeting patients who have impaired glucose tolerance with lifestyle changes including physical activity and dietary factors has been shown to be effective in the Chinese, North American and Finnish populations. In order for such lifestyle interventions to be successful in other populations, they need to be culturally sensitive, individualized and sustained. Some pharmacological agents including metformin and acarbose have also been shown to be effective, although the profile of those who respond is different. There continues to be a need to develop and evaluate interventions that target communities and populations at risk in a UK setting. Diabet. Med. (2004) [source]


E-commerce, Transportation, and Economic Geography

ABSTRACT This paper explores possible ways in which growth in Internet retailing (e-retailing) may affect the spatial distribution of economic activities. After a brief overview of e-retailing, a categorization of possible spatial impacts is introduced. These include impacts on the retail industry, such as substitution of e-retail for brick-and-mortar retail, impacts on transportation, such as substitution of freight transportation for personal transportation in goods delivery, and pervasive impacts that affect the whole economy. The latter category includes uniform delivered pricing, spatial leveling of accessibility, and marketing strategies that target individuals rather than regions. The question of whether e-retailing and brick-and-mortar retailing are truly substitutes is taken up in the next section, along with potential implications of multi-channel retailing. The final section of the paper defines some critical research directions. [source]
